1. Influence of egg density on larval development and adult body size of cat fleas (Ctenocephalides felis).

Abstract

Fleas (Siphonaptera) are holometabolous insects with larval and adult stages that exhibit vastly different ecologies from each other. Adult fleas are parasitic and feed exclusively on the blood of a vertebrate host, whereas flea larvae do not live on hosts and consume dried faecal blood from adult fleas. Because flea larvae rely on adult flea faeces for food, excrement and eggs must fall in the same location; thus, larval density is likely high in these restricted habitats. However, the influence of larval density on the subsequent adult stage has not been examined. In the present study, we utilized egg density to investigate density‐dependent effects on larval development and adult body size in the cat flea (Ctenocephalides felis Bouché) (Siphonaptera: Pulicidae). Specifically, eggs were collected to create three different larval densities (n = 50, 100 and 150 per 56.7 cm2), and hatched larvae from all groups were fed an excess amount of adult faecal pellets. Larval development was measured by recording the proportion of eggs that developed to the pupal stage and the proportion of eggs that reached adulthood (eclosion). The body size of eclosed adults was quantified for both sexes using head length and length of the total body. We found that the number of eggs had no effect on the proportion of larvae that pupated or the proportion of larvae that eclosed; however, higher egg densities resulted in larger body sizes for both sexes. Overall, these data yield significant insight into how the ecology of larval fleas impacts the biology of the resultant adults. [ABSTRACT FROM AUTHOR]

6. Prediction and Classification of Flood Susceptibility Based on Historic Record in a Large, Diverse, and Data Sparse Country

Abstract

The emergence of Machine learning (ML) algorithms has shown competency in a variety of fields and are growing in popularity in their application to geospatial science issues. Most recently, and notably, ML algorithms have been applied to flood susceptibility (FS) mapping. Leveraging high-power computing systems and existing ML algorithms with national datasets of Canada, this project has explored methods to create a national FS layer across a geographically large and diverse country with limited training data. First, approaches were considered on how to generate a map of FS for Canada at two different levels, (i) national, which combined all training data into one model, and (ii) regional, where multiple models were created, based on regional similarities, and the results were mosaicked to generate a FS map. The second experiment explored the predictive capability of several ML algorithms across the geographically large and diverse landscape. Results indicate that the national approach provides a better prediction of FS, with 95.7% of the test points, 91.5% of the pixels in the training sites, and 89.6% of the pixels across the country correctly predicted as flooded, compared to 65.5%, 80.6% and 75.6%, respectively, in the regional approach. ML models applied across the country found that support vector machine (svmRadial) and Neural Network (nnet) performed poorly in areas away from the training sites, while random forest (parRF) and Multivariate Adaptive Regression Spline (earth) performed better. A national ensemble model was ultimately selected as this blend of models compensated for the biases found in the individual models in geographic areas far removed from training sites.

7. Accessing the Impact of Meteorological Variables on Machine Learning Flood Susceptibility Mapping

Abstract

Machine learning (ML) algorithms have emerged as competent tools for identifying areas that are susceptible to flooding. The primary variables considered in most of these works include terrain models, lithology, river networks and land use. While several recent studies include average annual rainfall and/or temperature, other meteorological information such as snow accumulation and short-term intense rain events that may influence the hydrology of the area under investigation have not been considered. Notably, in Canada, most inland flooding occurs during the freshet, due to the melting of an accumulated snowpack coupled with heavy rainfall. Therefore, in this study the impact of several climate variables along with various hydro-geomorphological (HG) variables were tested to determine the impact of their inclusion. Three tests were run: only HG variables, the addition of annual average temperature and precipitation (HG-PT), and the inclusion of six other meteorological datasets (HG-8M) on five study areas across Canada. In HG-PT, both precipitation and temperature were selected as important in every study area, while in HG-8M a minimum of three meteorological datasets were considered important in each study area. Notably, as the meteorological variables were added, many of the initial HG variables were dropped from the selection set. The accuracy, F1, true skill and Area Under the Curve (AUC) were marginally improved when the meteorological data was added to the a parallel random forest algorithm (parRF). When the model is applied to new data, the estimated accuracy of the prediction is higher in HG-8M, indicating that inclusion of relevant, local meteorological datasets improves the result.

Publisher Copyright: © 2022 IAEA, Vienna. DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limits and regulate β N in the ITER baseline scenario, based on plasma response to probing 3D fields, was demonstrated. Finally, innovative tokamak operation using a negative triangularity shape showed many attractive features for future pilot plant operation.

Abstract

‘Disability’ is inherently intersectional. A displaced person will always have a socially constructed gender, the implications of which will be different in each geographic and cultural location and never fixed anywhere. The displaced person may be internally displaced and still retain formal citizenship, or may have crossed an international border to a refugee camp or an urban location. He may be in the hands of a smuggler attempting a clandestine border crossing. She may be incarcerated in a detention centre. They may be displaced also from their family and social networks, and from the attendant support and enablement. In this chapter we highlight factors necessary to grapple with as a starting point for mapping the ways in which gender, disability and forced migration interact. We set out some key facts and definitions, discuss tensions engaged in these intersections and, illustrate these by drawing on others’ and our own empirical work. The task here is to make the intersectional identities, experiences and lives of refugees with impairments visible in a more holistic manner through considering their social embodiment in particular places and at specific times. In the conclusion we discuss the necessity for process-focused, rather than programmatic, thinking and practice.

Abstract

DIII-D research is addressing critical challenges in preparation for ITER and the next generation of fusion devices through focusing on plasma physics fundamentals that underpin key fusion goals, understanding the interaction of disparate core and boundary plasma physics, and developing integrated scenarios for achieving high performance fusion regimes. Fundamental investigations into fusion energy science find that anomalous dissipation of runaway electrons (RE) that arise following a disruption is likely due to interactions with RE-driven kinetic instabilities, some of which have been directly observed, opening a new avenue for RE energy dissipation using naturally excited waves. Dimensionless parameter scaling of intrinsic rotation and gyrokinetic simulations give a predicted ITER rotation profile with significant turbulence stabilization. Coherence imaging spectroscopy confirms near sonic flow throughout the divertor towards the target, which may account for the convection-dominated parallel heat flux. Core-boundary integration studies show that the small angle slot divertor achieves detachment at lower density and extends plasma cooling across the divertor target plate, which is essential for controlling heat flux and erosion. The Super H-mode regime has been extended to high plasma current (2.0 MA) and density to achieve very high pedestal pressures (~30 kPa) and stored energy (3.2 MJ) with H 98y2 ≈ 1.6–2.4. In scenario work, the ITER baseline Q = 10 scenario with zero injected torque is found to have a fusion gain metric independent of current between q 95 = 2.8–3.7, and a lower limit of pedestal rotation for RMP ELM suppression has been found. This paper explores the "sporadic hyper-precarity" encountered by migrant domestic workers in Hong Kong when the city was hit by the Omicron outbreaks in early 2022. Migrant workers have long been suffering from job insecurity and structural vulnerability due to the contractization and flexibilization of work. The paper discusses how this structural vulnerability came to intersect with the health risks induced by the COVID pandemic. Adding to the debates of the structural precarity characterizing migrant work, we will further interrogate how workers are also susceptible to "sporadic hyper-precarity" - the kind of sporadic risks, uncertainty, vulnerabilities and stigmatization at times of crisis. Abstract

Background: Complex movement and balance impairments in people with Parkinson disease (PD) contribute to high fall risk. Comprehensive balance assessment is warranted to identify intrinsic fall risk factors and direct interventions., Objective: The purpose of this study was to examine the psychometric properties of 3 balance measures of a force platform (FP) system in people with PD., Methods: Forty-two community-dwelling individuals with idiopathic PD completed the testing protocol. Test-retest reliability was assessed for the Limits of Stability Test (LOS), Motor Control Test (MCT), and Sensory Organization Test (SOT). Intraclass correlation coefficients (ICC [2,1]) were calculated to determine test-retest reliability and minimal detectable change. Validity was assessed by comparing the FP measures with criterion gait and balance measures using Pearson product moment correlations. Multiple regression analyses examined the contribution of PD characteristics to FP measures., Results: All primary FP variables demonstrated excellent test-retest reliability (ICC=.78-.92). The SOT and LOS demonstrated fair to good correlations with criterion measures, whereas the MCT had fair correlations to balance measures only. Both SOT composite equilibrium and MCT average latency were moderately associated with disease severity., Limitations: This study's sample had a relatively small number of participants with a positive fall history, which may limit the generalizability of the findings., Conclusions: This study's findings provide support that FP measures are reliable and valid tests of balance impairment in people with PD. Disease severity was significantly associated with SOT and MCT measures, perhaps reflecting that these tests are meaningful indicators of decline in postural control with disease progression. The Defence Medical Services has made a significant investment to address the culture and process by which risk to patient safety is managed within its organisation. This paper describes the decision process and technical considerations in the design of an automated reporting system together with the implementation procedure aimed to maximise compliance. Abstract

Healthcare provided by the Defence Medical Services (DMS) is acknowledged to be of a high standard but patients' experiences of it has not been measured and collated in a consistent and meaningful way, which has limited strategic quality improvement initiatives. Responsibility for implementing and delivering a programme of healthcare governance and assurance for the DMS rests with the Inspector General (IG). An important aspect of this role is to nurture a culture of continuous improvement in the DMS and under this leadership the IG team has prioritised a number of projects to address this. The project to improve patient experience data capture was prioritised in the work schedule as it incorporated initiatives that would lead to improved quality in DMS healthcare, information exploitation and ultimately patient safety. This is the first in a series of articles that will document this important work and describe the methodological considerations associated with the initial questionnaire design, collaboration with NHS partners, the pilot study and progress towards the introduction of the definitive DMS tool later this year., (Published by the BMJ Publishing Group Limited. The Immediate Life Support (ILS) course is a 1 day programme designed by the Resuscitation Council (UK) to bridge the gap between Basic Life Support (BLS) and Advanced Life Support (ALS). During the build up to Operation TELIC, the aim was to provide ILS training to all staff who may be involved in the acute care of patients within PCRF ARGUS, and also to members of staff from other units who may be involved in similar situations. In total, 101 staff received ILS training during Op TELIC, all of whom were successful in completing the course.

Abstract

Objectives: To determine whether a 1-cm margin is necessary for cancer control during nephron-sparing surgery (NSS) for renal cell carcinoma (RCC)., Methods: A retrospective review of 67 patients who underwent NSS for RCC between 1990 and 2000 was conducted. The data collected included patient demographics, tumor size and location, histologic type and grade, margin status (positive or negative), and the shortest distance of normal parenchyma (in millimeters) around the tumor in the final pathologic specimen. Recurrence was determined from the clinical follow-up, which included physical examination, ultrasonography or computed tomography, and various laboratory tests., Results: Fifty-five cases were performed open and 12 laparoscopically. The mean follow-up was 60 months (range 5 to 124). The mean tumor size was 3.0 cm (range 0.9 to 11.0). Seven patients were found to have a positive margin; 1 died of metastatic RCC, 1 was alive with systemic recurrence, and 5 had no evidence of disease. Of 11 patients with a negative margin distance of less than 1 mm, 9 were recurrence free, 1 had simultaneous local and pulmonary relapse, and the other had pulmonary recurrence only. The remainder of the study patients (n = 49) had negative margins greater than 1 mm, and all were alive without evidence of disease at the last follow-up., Conclusions: This review questions the necessity of a 1-cm margin to prevent recurrence after NSS for RCC. Additional studies to determine the optimal margin distance should be conducted.

25. Ureteral stenting after ureteroscopy for distal ureteral calculi: a multi-institutional prospective randomized controlled study assessing pain, outcomes and complications.

Abstract

Purpose: We compare postoperative pain, stone-free rates and complications after ureteroscopic treatment of distal ureteral calculi with or without the use of ureteral stents., Materials and Methods: A total of 113 patients with distal ureteral calculi amenable to ureteroscopic treatment were prospectively randomized into stented (53) and unstented (60) groups. Stones were managed with semirigid ureteroscopes with or without distal ureteral dilation and/or intracorporeal lithotripsy. Preoperative and postoperative pain questionnaires were obtained from each patient. Patients with stents had them removed 3 to 10 days postoperatively. Radiographic followup was performed postoperatively to assess stone-free rates and evidence of obstruction., Results: Six patients randomized to the unstented group were withdrawn from the study after significant intraoperative ureteral trauma was recognized, including 3 ureteral perforations, that required ureteral stent placement, leaving 53 with stents and 54 without for analysis. Patients with stents had statistically significantly more postoperative flank pain (p = 0.005), bladder pain (p <0.001), urinary symptoms (p = 0.002), overall pain (p <0.001) and total narcotic use (p <0.001) compared to the unstented group. Intraoperative ureteral dilation or intracorporeal lithotripsy did not statistically significantly affect postoperative pain or narcotic use in either group (p >0.05 in all cases). Overall mean stone size in our study was 6.6 mm. There were 4 (7.4%) patients without stents who required postoperative readmission to the hospital secondary to flank pain. All patients (85%) who underwent imaging postoperatively were without evidence of obstruction or ureteral stricture on followup imaging (mean followup plus or minus standard deviation 1.8 +/- 1.5 months), and the stone-free rate was 99.1%., Conclusions: Uncomplicated ureteroscopy for distal ureteral calculi with or without intraoperative ureteral dilation can safely be performed without placement of a ureteral stent. Patients without stents had significantly less pain, fewer urinary symptoms and decreased narcotic use postoperatively.

Consonant recognition in quiet using the Nonsense Syllable Test (NST) [Resnick et al., J. Acoust. Soc. Am. Suppl. 1 58, S114 (1975)] was investigated in 62 normal hearing subjects 20 to 65 years of age at their most comfortable listening levels (MCLs) and at 8 dB above and below MCL. Although overall consonant recognition performance was high (as expected for normal listeners), the effects of age decade, relative presentation level, and NST subsets were all significant, as was the interaction of age X level. The interactions of age X NST subset, and age X subset X level were nonsignificant. These findings suggest that consonant recognition decreases with normal aging, particularly below MCL. However, the relative perceptual difficulty of the seven subtests is the same across age groups. Confusion matrices were similar across levels and age groups. Percent information transmitted for several consonant features was calculated from the confusion matrices. Older subjects showed decrements in performance primarily for the features recognized relatively less accurately by the younger subjects. The results suggest that normal hearing older individuals listening in quiet have decreased consonant recognition ability, but that their confusions are similar to those of younger persons.

Abstract

Consonant recognition in quiet and in noise was investigated as a function of age for essentially normal hearing listeners 21-68 years old, using the nonsense syllable test (NST) [Resnick et al., J. Acoust. Soc. Am. Suppl. 1 58, S114 (1975)]. The subjects audited the materials in quiet and at S/N ratios of +10 and +5 dB at their most comfortable listening levels (MCLs). The MCLs approximated conversational speech levels and were not significantly different between the age groups. The effects of age group, S/N condition (quiet, S/N +10, S/N +5) and NST subsets, and the S/N condition X subset interaction were all significant. Interactions involving the age factor were nonsignificant. Confusion matrices were similar across age groups, including the directions of errors between the most frequently confused phonemes. Also, the older subjects experienced performance decrements on the same features that were least accurately recognized by the younger subjects. The findings suggest that essentially normal older persons listening in quiet and in noise experience decreased consonant recognition ability, but that the nature of their phoneme confusions is similar to that of younger individuals. Even though the older subjects met the same selection criteria as did younger ones, there was an expected shift upward in auditory thresholds with age within these limits. Sensitivity at 8000 Hz was correlated with NST scores in noise when controlling for age, but the correlation between performance in noise and age was nonsignificant when controlling for the 8000-Hz threshold. These associations seem to implicate the phenomena underlying the increased 8000-Hz thresholds in the speech recognition problems of the elderly, and appear to support the concept of peripheral auditory deterioration with aging even among those with essentially normal hearing.

This study replicated Silman's [J. Acoust. Soc. Am. 66 (1979a)] experiment comparing acoustic reflex thresholds in normal hearing young and elderly subjects. The original study was modified so that the criterion for normal hearing was extended through 8000 Hz, and the range of auditory thresholds among the younger group was selected to approximate those of the elderly subjects as closely as possible. The data confirmed those reported by Silman, showing an elevation of the ART for broadband noise, but nor for tonal activators in the elderly group.

Acoustic reflex thresholds (ARTs) at 500, 1000, and 2000 Hz from 1833 ears were subjected to multiple linear regression analyses using a stepwise inclusion model to determine the effects of hearing losses in the 500- to 4000-Hz range for each of the activators. Although statistically significant regression equations were developed, these accounted for only 1 to 47% of the variance of ART levels. In light of the wide variance of ARTs unexplainable directly from audiometric data, it was deemed more appropriate to consider the range of ARTs from the standpoint of the sampling distribution of ARTs as a function of hearing level. Deciles were calculated in the 10th through 90th percentile ranges. These data corroborate and expand upon previously reported material on the distributions of ARTs among ears with normal hearing and cochlear impairment.

In this retrospective study of 19 patients with hemorrhagic cholecystitis, 14 (74%) patients demonstrated one of the following unusual sonographic features: focal gallbladder wall irregularity; intraluminal membranes; coarse, nonshadowing, nonmobile intraluminal echoes. The clinical presentation of these cases (abdominal pain, 100%; leukocytosis, 74%; fever, 63%) is identical to the classic presentation of acute cholecystitis. Overt gastrointestinal bleeding did not occur.

Multiple linear regression equations were derived to define the expected levels of acoustic reflex thresholds (ARTs) at 500, 1000, and 2000 Hz predictable from hearing levels at 500-4000 Hz in 646 ears. When the hearing level at the activator frequency is less than or equal to 50 dB HL, ARTs tend to be best defined by a constant value of 86-90 dB HL with small adjustments due to the loss at the activator or higher frequencies. When the hearing level at the activator frequency is 55-90 dB HL, the ART is best predicted by a constant plus the degree of loss at that frequency plus the loss at 4000 Hz. These data present the clinician with empirically derived bases for determining the effects of other frequencies on a given ART. Application of these findings permits the clinician to apply known relationships in determining whether a particular ART is representative of those expected for normal and/or cochlear impaired ears, elevated or depressed, without having to rely on vague intuitions of such relationships. Clinical applications are discussed.

Abstract

Dichotic CV recognition at interaural temporal onset asynchronies (lag times) of 0, 30, 60, and 90 ms was investigated in normal hearing young and elderly subjects. The results for the young group were consistent with those reported in previous studies (Studdert-Kennedy et al., 1970; Berlin et al., 1973a). The older group demonstrated a mean right ear advantage (REA) at simultaneity of 12.7%, which was not significantly different from that of the young group (12.4%). This suggests that the processes subserving the REA are not affected by age. However, there was a significant reduction in the total (right ear plus left ear) dichotic scores of the elderly group compared to the young; suggesting an age-associated reduction in the channel capacity of the aging auditory system. Further, there were striking aberrations of the dictotic lag effect in the elderly group compared to the young subjects, consistent with age-related changes in auditory temporal processing. The findings are discussed with respect to the aging auditory system and the nature of dichotic speech processing.
