Abstract

Background NRG Oncology NSABP B-39/RTOG 0413 compared whole-breast irradiation (WBI) to accelerated partial-breast irradiation (APBI). APBI was not equivalent to WBI in local tumor control. Secondary outcome was quality of life (QOL). Methods The QOL sub-study used validated self-report questionnaires, including the Breast Cancer Treatment Outcome Scale (BCTOS) and the 36-Item Short Form Health Survey (SF-36) vitality scale. Assessments occurred before random assignment, at treatment completion (chemotherapy or radiotherapy), 4 weeks later, at 6, 12, 24, and 36 months. Primary aims: cosmesis change equivalency (baseline to 3 years; a priori margin of equivalence 0.4 standard deviations) and fatigue change superiority (baseline to end of treatment [EOT]) for APBI vs WBI, by patient groups treated with or without chemotherapy when appropriate. Results From March 21, 2005 to May 25, 2009, 975 patients enrolled in this sub-study; 950 had follow-up data. APBI had 3-year cosmesis equivalent to WBI (95% CI = -0.0001 to -0.16; equivalence margin -0.22 to -0.22) in all patients. The APBI group without chemotherapy had less EOT fatigue (P  = .011; mean score APBI 63 vs WBI 59); the APBI group receiving chemotherapy had worse EOT fatigue (P  = .011; APBI 43 vs WBI 49). The APBI group reported less pain (BCTOS) at EOT (WBI 2.29 vs APBI 1.97) but worse pain at 3 years (WBI 1.62 vs APBI 1.71). APBI patients reported greater convenience of care than with WBI and reported less symptom severity at EOT and 4 weeks later. Conclusion Cosmetic outcomes were similar for the APBI and WBI groups, with small statistically significant differences in other outcomes that varied over time. Abstract

Introduction Breast cancer (BC) is the most commonly diagnosed invasive cancer in women in the United States and the second leading cause of cancer-related deaths (1). Radiation therapy (RT) remains [...], Increased rates of locoregional recurrence are observed in patients with basal-like breast cancer (BC) despite the use of radiation therapy (RT); therefore, approaches that result in radiosensitization of basal-like BC are critically needed. Using patients' tumor gene expression data from 4 independent data sets, we correlated gene expression with recurrence to find genes significantly correlated with early recurrence after RT. The highest-ranked gene, TTK, was most highly expressed in basal-like BC across multiple data sets. Inhibition of TTK by both genetic and pharmacologic methods enhanced radiosensitivity in multiple basal-like cell lines. Radiosensitivity was mediated, at least in part, through persistent DNA damage after treatment with TTK inhibition and RT. Inhibition of TTK impaired homologous recombination (HR) and repair efficiency, but not nonhomologous end-joining, and decreased the formation of Rad51 foci. Reintroduction of wild-type TTK rescued both radioresistance and HR repair efficiency after TTK knockdown; however, reintroduction of kinase-dead TTK did not. In vivo, TTK inhibition combined with RT led to a significant decrease in tumor growth in both heterotopic and orthotopic, including patient-derived xenograft, BC models. These data support the rationale for clinical development of TTK inhibition as a radiosensitizing strategy for patients with basal-like BC, and efforts toward this end are currently underway.

Abstract

NCCN guidelines indicate that cancer clinical trials (CCTs) are the best management for patients with cancer. However, only 5% of patients enroll in them. We examined oncologists' perceived barriers and facilitators to discussing CCTs. This qualitative study was part of the ASCO-ACCC Initiative to Increase Racial and Ethnic Diversity in Clinical Trials. Barriers and facilitators at the system, trial, provider, and patient levels were examined. To achieve triangulation, patient encounters were reviewed using chart-stimulated recall (CSR) methods, thereby obtaining a valid assessment of physician performance. Ten oncology providers participated in this study. Nine were oncologists, and one was a clinical research coordinator; five were female; four were White; three were Asian; and three were Black. Barriers to offering CCTs were a lack of trial availability; ineligibility; a lack of knowledge; assumptions about patient interest, benefits, or harms; patient's disease factors; and negative attitudes. Facilitators of offering CCTs were a physical space to discuss trials; greater trial availability; a systematic approach to offering trials; patient factors; patients seeking trials; a lack of comorbidities; patients being younger in age; patients being aware of, asking about, or hearing of trials from their surgeon; and higher levels of altruism. 35. Focused Instruction in Spanish Syllabification

Abstract

It is well established that learners transfer sounds from their first to their second language when speaking (see Major, 2008, and references therein). Recent work also indicates that learners may transfer their perception of sounds from the first to the second language. In some cases, errors in production and perception are explicitly corrected due to problems with miscommunication. On the other hand, when such errors do not lead to direct misunderstandings, the errors may be left uncorrected, which may lead to fossilization. This article examines a salient characteristic of English-accented Spanish: putting two adjacent vowels in hiatus rather than producing them as a diphthong. Data show that a simple treatment based on direct negative evidence of syllabification can significantly improve learners' knowledge of these Spanish sound sequences.

Abstract

This article describes the pilot of a school-based creative group-counseling program for children called ArtBreak, a choice-based studio art experience based on the restorative possibilities of art making delineated in the expressive therapies continuum (ETC; Kagin & Lusebrink, 1978). The ETC features a developmental hierarchy in relation to how art is created and the therapeutic properties of associated media. Learning from the pilot suggests the ETC is a valuable framework for supporting groups of children in kinesthetic, affective, and cognitive ways. 40. Repeated Readings in Poetry versus Prose: Fluency and Enjoyment for Second-Graders

Abstract

This study examined the effectiveness of repeated readings in four second-grade classrooms in two urban elementary schools in Northwest Ohio. As determined by the Ohio Department of Education, 75% of the students in each building must score at or above the proficient level. For the 2008-2009 school year Building A, a school within one of Ohio's urban settings, had a passage rate of 65.7% and Building-B, in the same district, had a lower passage rate of 58.1%. To evaluate the effectiveness of the 10-week repeated reading session of poetry and prose the following research questions were addressed: (a) What is the effect of repeated readings of poetry versus repeated reading of narrative passages on fluency (Words Correct Per Minute) over a 10-week intervention? (b) What is the effect of repeated readings of poetry versus repeated reading of narrative passages on fluency (prosody) over a 10-week intervention? (c) What is the effect of repeated readings of poetry versus repeated reading of narrative passages on reading comprehension over a 10-week intervention? (d) What is the effect of repeated reading of poetry on students' attitudes toward reading as compared to the repeated readings of narrative passages? Mixed methods design was selected for this study due to both quantitative and qualitative methods of data collection. The quantitative data represents the scores on the pre- and post-tests of Words Correct Per Minute, retelling rubric, Elementary Reading Attitude Survey, and prosody rubric. The qualitative data within the study consisted of student questionnaires, student interviews, and teacher journals following the 10-week repeated readings project. Abstract

PURPOSE Cancer trial participants do not reflect the racial and ethnic diversity in the population of people with cancer in the United States. As a result of multiple system-, patient-, and provider-level factors, including implicit bias, cancer clinical trials are not consistently offered to all potentially eligible patients. MATERIALS AND METHODS ASCO and ACCC evaluated the utility (pre- and post-test knowledge changes) and feasibility (completion rates, curriculum satisfaction metrics, survey questions, and interviews) of a customized online training program combined with facilitated peer-to-peer discussion designed to help research teams identify their own implicit biases and develop strategies to mitigate them. Discussion focused on (1) specific elements of the training modules; (2) how to apply lessons learned; and (3) key considerations for developing a facilitation guide to support peer-to-peer discussions in cancer clinical research settings. We evaluated discussion via a qualitative assessment. RESULTS Participant completion rate was high: 49 of 50 participating cancer programs completed training; 126 of 129 participating individuals completed the training (98% response rate); and 119 completed the training and evaluations (92% response rate). Training increased the mean percentage change in knowledge scores by 19%-45% across key concepts (eg, causes of health disparities) and increased the mean percentage change in knowledge scores by 10%-31% about strategies/actions to address implicit bias and diversity concerns in cancer clinical trials. Knowledge increases were sustained at 6 weeks. Qualitative evaluation validated the utility and feasibility of facilitated peer-to-peer discussion. CONCLUSION The pilot implementation of the training program demonstrated excellent utility and feasibility. Our evaluation affirms that an online training designed to raise awareness about implicit bias and develop strategies to mitigate biases among cancer research teams is feasible and can be readily implemented in cancer research settings.
