Background : : To evaluate and compare the efficacy of tissue ablation in electrocardiogram (ECG)-synchronized and non-synchronized irreversible electroporation (IRE) ablation using radiological and histological analyses. Methods : : Five Yorkshire swine underwent 2,250 or 3,000 volt IRE ablation of multiple liver zones with high-rate, non-synchronized irreversible electroporation (nsIRE) pulses delivered at 240 pulses per minute (PPM) (n = 12) and ECG-synchronized irreversible electroporation (esIRE) pulses at either medium rate delivered at 50 PPM (n = 12) or low rate at 20 PPM (n = 6). We evaluated and compared the volume of ablation zones and IREinduced cell death between esIRE and nsIRE groups using radiological and histological analyses. Results : : In ultrasound examination, no significant difference was observed between the size of esIRE and nsIRE treated areas in all three dimensions (P = 0.93, 240 PPM vs. 50 PPM and P = 0.89, 240 PPM vs. 20 PPM). The ablation areas were also well-correlated in gross pathological analysis, with no significant difference between esIRE and nsIRE groups (P = 0.55, 240 PPM vs. 50 PPM and P = 0.56, 240 PPM vs. 20 PPM). Gross and microscopic examinations demonstrated complete cell death in both esIRE and nsIRE cohorts with preservation of large blood vessels and bile ducts in both groups. No cardiac dysrhythmia was noted in esIRE group. Conclusion : : In our study, no histological or radiological differences were observed between esIRE and nsIRE ablated areas. The esIRE ablation maintained its ability to create complete focal cell death.

A 64-year old man had developed a giant mediastinal lymphocele after undergoing esophagectomy for the treatment of esophageal squamous cell carcinoma. The thoracic duct was embolized with six micro-coils, followed by embolization using a 1:3 mixture of N-butyl-2-cyanoacrylate (Histoacryl; B. Braun, Melsungen, Germany) and ethiodized oil. Resolution of the lymphocele was achieved within 5 days after embolization. To the best of our knowledge, ours is the first reported case of thoracic duct embolization for the treatment of mediastinal lymphocele.

Budd-Chiari syndrome (BCS) is a heterogeneous group of disorders characterized by hepatic venous outflow obstruction. Abernethy malformation is a congenital vascular malformation defined by diversion of portal blood away from the liver. Both conditions are rare vascular diseases. We report here the first case of a patient with combined type II Abernethy malformation and BCS from China. The inferior vena cava obstruction was treated with percutaneous balloon angioplasty; close follow-up was elected for the Abernethy malformation.

Background: The purpose of this study was to evaluate and compare the clinical effectiveness and costs of using the Caterpillar Arterial Embolization Device (Caterpillar) and fibered coils in arterial embolization cases. Methods: In this multicenter retrospective study, demographic, laboratory, and procedural data were collected on a total of 48 patients between February 2020 and September 2020. Data were collected on 16 Caterpillar placements and matched with 32 controls who underwent coil embolization of the same vessel with a similar size. Clinical and procedural outcomes including type and size of vessels, time to vessel occlusion, fluoroscopy time, total procedure time, and costs were analyzed and compared. Results: Relative time to occlusion was significantly decreased in the Caterpillar group compared to the controls (57 ± 34 s vs. 11 min 44 s ± 8 min 13 s, p = 0.00001611). Fluoroscopy time (6.9 ± 15 min vs. 19.2 ± 14, p = 0.017) and total procedure time (81.0 ± 36 min vs. 111.5 ± 49 min, p = 0.015) were significantly reduced compared to the coil group. Lastly, overall cost of embolic materials was 1050 ± 0 USD for the Caterpillar group compared to 2312.75 ± 1382.84 USD in the coil group (p = 0.000532). Conclusion: The Caterpillar embolic devices appear safe and effective in arterial occlusion. Compared to fibered coils, the Caterpillar device results in decreased time to vessel occlusion, decreased fluoroscopy and procedural time, and decreased costs, making the Caterpillar an appealing choice for arterial embolization.

Background: The use of a transjugular intrahepatic portosystemic shunt (TIPS) has been established as an effective treatment for portal hypertension. Despite the rapid development of this use, serious peri-procedural complications have been reported in over 10% of cases. This has largely been attributed to the access to the portal vein, also referred to as a “blind puncture”, which often requires multiple attempts. The aim of this study was to demonstrate the safety, reproducibility and accuracy of the use of real-time 3D fusion image-guided (3DFIG) single puncture TIPS to minimize the complications that are related to the “blind puncture” of TIPS procedures. Methods: A 3DFIG TIPS approach was utilized on 22 pigs by combining pre-procedural cross-sectional imaging (CT, MR or CBCT) with intra-procedural cone beam CT or angiogram imaging, which allowed for the improved 3D visual spatial orientation of the portal vein and real-time tracking of the needle in 3D. Results: Thirty-five portosystemic shunts were successfully deployed in all 22 subjects without any peri-procedural complications. Overall, 91% (32/35) of the procedures were carried out using a single puncture. In addition, the mean fluoroscopy time in our study was more than 12 times lower than the proposed reference level that has previously been proposed for TIPS procedures. Conclusion: Multi-modality real-time 3DFIG TIPS can be performed safely using a single puncture, without complications, and can potentially be used in both emergency and non-emergency clinical situations.

Misplacement of an inferior vena cava (IVC) filter in a gonadal vein is a rare complication of IVC filter placement. We report a case of a filter misplaced in the ovarian vein of a pregnant woman with agenesis of the infrarenal IVC and bilateral lower extremity deep venous thrombosis. The filter was removed by a loop-snare technique through an internal jugular vein. IVC agenesis and dilated right gonadal vein should be kept in mind when an IVC filter is being inserted in the infrarenal location through the jugular approach.

Background: The purpose of this study is to describe a single institution’s experience using Oncozene (OZ) microspheres for transarterial chemoembolization (OZ-TACE) of hepatocellular carcinoma (HCC), and to compare tolerability, safety, short-term radiographic tumor response, progression-free survival (PFS), and overall survival (OS) of these procedures to TACE (LC-TACE) performed with LC beads (LC). Methods: A retrospective, matched cohort study of patients undergoing DEB-TACE (drug-eluting bead transarterial chemoembolization) with OZ or LC was performed. The cohort comprised 23 patients undergoing 29 TACE with 75 or 100 μm OZ and 24 patients undergoing 29 TACE with 100–300 μm LC. Outcome measures were changes in liver function tests, complications, treatment tolerability, short-term radiographic tumor response according to modified RECIST criteria for HCC, PFS, and 1-year OS. The Mann–Whitney U test, Fisher exact test, and log rank test were used to compare the groups. Results: The BCLC or Child–Pugh scores were similar between the OZ and LC group. However, the two groups differed with respect to the etiology of background cirrhosis (p = 0.02). All other initial demographic and tumor characteristics were similar between the two groups. OZ-TACE used less doxorubicin per treatment compared to LC-TACE (median 50 vs. 75 mg; p = 0.0005). Rates of pain, nausea, and postembolization syndrome were similar, irrespective of the embolic agent used. OZ-TACE resulted in an overall complication rate comparable to LC-TACE (20.7% vs. 10.3%; p = 0.47). LC-TACE resulted in a higher percent increase in total bilirubin on post-procedure day 1 (median 18.8 vs. 0%; p = 0.05), but this difference resolved at 1 month. Both OZ-TACE and LC-TACE resulted in similar complete (31% vs. 24%) and objective (66% vs. 79%) target lesion response rates on 1-month post-TACE imaging. Both OZ-TACE and LC-TACE had similar median progression-free survival (283 vs. 209 days; p = 0.14) and 1-year overall survival rates (85% vs. 76%; p = 0.30). Conclusion: With a significantly reduced dose of doxorubicin, TACE performed with Oncozene microspheres in a heterogeneous patient population is well-tolerated, safe, and produces a similar radiological response and survival rate when compared to LC Bead TACE.

Aims: To evaluate the feasibility of using the Willis covered stent in the management of patients with traumatic carotid-cavernous fistulas (CCFs). Materials and Methods: Twelve consecutive patients with 14 post-traumatic CCFs referred for treatment with Willis covered stents were enrolled in this prospective study. Data on technical success, initial and final angiographic results, mortality, morbidity and final clinical outcome were collected. Follow-u p was at one, three, six, and 12 months, and yearly thereafter. Results: Deployment of covered stents was technically successful in all the patients without complications. One covered stent was placed in eight patients and two covered stents in four. Angiographic results following stent placement showed a complete occlusion in nine patients with 11 CCFs, and an incomplete occlusion in three patients. Angiographic follow-up (mean, 14.00 ± 6.93 months; range, 6-24 months) revealed complete occlusion and no obvious in-stent stenosis in all the patients. Clinical follow-up (mean, 17.75 ± 7.05 months; range, 7-28 months) demonstrated full recovery in 11 patients, and improvement in one. Conclusion: This initial experience indicates that the use of the Willis covered stent is a feasible procedure and may be an alternative treatment option for CCFs. [ABSTRACT FROM AUTHOR]
