14. Self-isolation of an Italian long-term care facility during COVID-19 pandemic: A comparison study on care-related infectious episodes

Abstract

The aim of this retrospective cohort study is to understand if and how much the preventive self-isolation approach might have been a valid model to avoid care-related infection, not only from COVID-19 but also from other non-viral infectious diseases. From March to May 2020, the healthcare and management staff of the Villa Santa Maria long-term care facilities, located in the village of Montenero di Bisaccia (Campobasso, Molise, Italy), decided to carry out a preventive self-isolation plan to safeguard the residents from SARS-CoV-2. The impact on other infectious diseases was evaluated by analyzing the antibiotic therapies prescription trend among the inpatients. Our data showed that although self-isolation protected residents and caregivers from SARS-CoV-2, it can also be associated with mobility reduction, leading to an increase in bedridden pathologies, namely, pressure ulcers and pressure sores. The simultaneous isolation of residents and caregivers in the same location significantly reduced any outside influence as a cause of possible infections.

Abstract

Background: The introduction of whole new foods in a population may lead to sensitization and food allergy. This constitutes a potential public health problem and a challenge to risk assessors and managers as the existing understanding of the pathophysiological processes and the currently available biological tools for prediction of the risk for food allergy development and the severity of the reaction are not sufficient. There is a substantial body of in vivo and in vitro data describing molecular and cellular events potentially involved in food sensitization. However, these events have not been organized in a sequence of related events that is plausible to result in sensitization, and useful to challenge current hypotheses. The aim of this manuscript was to collect and structure the current mechanistic understanding of sensitization induction to food proteins by applying the concept of adverse outcome pathway (AOP). Main body: The proposed AOP for food sensitization is based on information on molecular and cellular mechanisms and pathways evidenced to be involved in sensitization by food and food proteins and uses the AOPs for chemical skin sensitization and respiratory sensitization induction as templates. Available mechanistic data on protein respiratory sensitization were included to fill out gaps in the understanding of how proteins may affect cells, cell-cell interactions and tissue homeostasis. Analysis revealed several key events (KE) and biomarkers that may have potential use in testing and assessment of proteins for their sensitizing potential. Conclusion: The application of the AOP concept to structure mechanistic in vivo and in vitro knowledge has made it possible to identify a number of methods, each addressing a specific KE, that provide information about the food allergenic potential of new proteins. When applied in the context of an integrated strategy these methods may reduce, if not replace, current animal testing approaches. The proposed AOP will be shared at the www.aopwiki.org platform to expand the mechanistic data, improve the confidence in each of the proposed KE and key event relations (KERs), and allow for the identification of new, or refinement of established KE and KERs. © 2017 The Author(s).

Abstract

Abstract Wheat is one of the most important crops in the world in terms of human nutrition. With regards to health, some individuals exhibit wheat-related disorders such as food allergy to wheat (FAW). In this disorder, gluten is involved, particularly the gliadins which are among the main proteins responsible for FAW. Food processing, as well as digestibility and intestinal transport are key factors to consider since they may affect the allergenic potential of food allergens. Wheat is always consumed after heat processing and this step may impact epitope accessibility by inducing aggregation and may irreversibly destroy conformational epitopes. Our aim was to investigate the effects of heating and digestion on the structure of well-known allergens (total gliadins and α-gliadins) and their capacity to maintain their allergenic potential after crossing an intestinal barrier. The sizes of the processed (heated and heated/digested) proteins were characterized by laser light scattering and chromatographic reverse phase. The IgE-binding capacities of native and processed proteins were checked using a dot blot with sera from wheat allergenic patients. Furthermore, the abilities of these samples to cross the intestinal barrier and to induce mast cell degranulation were investigated by combining two in vitro cellular models, Caco-2 and RBL-SX38. The heat treatment of total gliadins and α-gliadins induced the production of large aggregates that were hardly recognized by IgE of patients in dot-blot. However, after limited pepsin hydrolysis, the epitopes were unmasked, and they were able to bind IgE again. Native proteins (gliadins and α-type) and processed forms were able to cross the Caco-2 cells in small amount. Permeability studies revealed the capacity of α-gliadins to increase paracellular permeability. In the RBL assay, the total native gliadins were able to trigger cell degranulation, but none of their processed forms. However after crossing the CaCo-2 monolayer, processed gliadins recovered their degranulation capacity to a certain extent. Total native gliadins remained the best allergenic form compared to α-type. Résumé La prévalence des maladies allergiques en particulier des allergies alimentaires ne cesse d'augmenter. Les allergies alimentaires touchent environ de 4 % des adultes et 6–8 % des enfants. Les légumineuses sont des allergènes alimentaires importants et sont responsables d'allergies IgE médiées en particulier d'anaphylaxies. La directive européenne (UE/1169) a rendu obligatoire la déclaration de trois d'entre elles : arachide, soja et lupin. Les réactions allergiques au pois sont classiques dans la petite enfance et surviennent classiquement avec le petit pois et le pois cassé. La dénomination binomiale du pois est Pisum sativum. Deux allergènes majeurs de la famille des globulines 7S ont été décrits : une viciline Pis s 1 et une conviciline Pis s 2. Récemment, le pois est utilisé comme un ingrédient dans de nombreux aliments industriels. Les protéines de pois commerciales, extraites du pois jaune sec dépelliculé et contenant plus de 80 % de protéines, sont utilisées par les industriels pour leurs propriétés émulsifiantes et gélifiantes dans les viandes reconstituées, boissons, pâtisseries et d'autres applications comme substituts du gluten et des protéines animales. Abstract In recent decades, the prevalence of allergic diseases has increased dramatically, and food allergy plays a key part in this increase. The prevalence of food allergy is about 4% in adults and 6–8% in children. Legumes are among the key classes of food responsible for IgE-mediated allergic reactions, especially severe food anaphylaxis. The Labeling Directive (UE 1169/2011) requires manufacturers to declare three legumes: peanut, soy and lupin. Allergic reactions provoked by peas are common in early childhood in legume allergy, especially with green peas and split peas in our countries. The binomial denomination of pea is Pisum sativum. Two major allergens from the 7S-gobulin family have been identified in pea: a vicilin-like protein, Pis s 1, and a convicilin, Pis s 2. Recently, pea has been used as an ingredient in various processed foods. Commercial pea proteins, extracted from yellow dry dehulled pea and containing more than 80% of proteins, are used by manufacturers for their emulsifying and gelling properties in minced meat products, drinks, baked goods and other applications as substitutes for gluten and for animal protein. Abstract

We developed a procedure for growing normal epidermal keratinocyte stem cells isolated from a single punch biopsy of adult human skin in long-term culture. Primary skin epithelial cells were maintained in collagen-coated plates with irradiated human neonatal foreskin fibroblasts (line HPI.1) as a feeder for more than 120 days, approximately 115 population doublings, without signs of replicative senescence. Clonal analysis revealed the presence of holoclones, meroclones, and paraclones. Only emerging colonies with high proliferative potentials and extensive capacities for division (holoclones and meroclones) were subcultured, favoring the expansion of stem cells and progenitors capable of prolonged self-maintenance when subcloned, thus accounting for the prevailing long-term proliferation of the original culture. We found that meroclones included bipotent progenitors capable of generating both keratinocytes and mucin-producing cells. The numbers of these cells were greater after confluence, suggesting that commitment for their differentiation occurred late in the life of a single clone. On a three-dimensional gelatin matrix and on a collagen layer containing the fibroblast feeder, cells isolated from the expansion of holoclones and meroclones formed stratified cohesive layers of keratinocytes that were able to further differentiate, as in normal skin. These results indicate that our procedure will serve as a valuable tool to study expansion of epidermal stem cells as well as the growth mechanisms and cell products associated with their growth and differentiation.

ABSTRACT Cigarette smoking plays an important role as a cause of morbidity and mortality in humans, involving respiratory, cardiovascular, digestive and reproductive systems. Tobacco smoke contains a large number of molecules, some of which are proven carcinogens. Although not fully understood, polymorphonuclear leukocytes seem to play a crucial role in the mechanisms by which tobacco smoke compounds are implicated in smoke-related diseases. In this paper the effects of an aqueous cigarette smoke extract on the expression of adhesion molecules of polymorphonuclear leukocytes together with the changes in the cell morphology have been related to the chemiluminescence activity. The results obtained show that polymorphonuclear leukocytes treated with aqueous cigarette smoke extract are significantly impaired, as suggested by the changes of chemiluminescence activity, of membrane receptors (CD18, CD62), myeloperoxidase expression and of cell morphology. Altogether the present data indicate that treated polymorphonuclear leukocytes are ineffectively activated and therefore unable to phagocytize zymosan particles. Abstract

Background: ω-5 gliadin is a major allergen in exercise-induced wheat allergy (EIWA), but it is also implicated in immediate-type reactions to wheat. An ImmunoCAP assay to measure ω-5 gliadin-specific IgE has become available. This study aimed to evaluate this new biological test in wheat allergy diagnosis and to also determine if it was able to discriminate EIWA from other types of wheat allergy. Methods: Sixty-one patients with wheat allergy were divided into 3 groups as a function of their symptoms (EIWA, immediate-type reactions and atopic dermatitis). These patients underwent skin prick tests with purified ω gliadins and ImmunoCAP to wheat flour, gluten and recombinant ω-5 gliadin. Results: The experimental data showed that 78% of EIWA patients had a positive skin prick test to natural ω-5 gliadin and the same proportion had detectable specific IgE to recombinant ω-5 gliadin, indicating that ω-5 gliadin is the main allergen, but not the only one, in our population. Additionally, we showed that this detection was not EIWA specific since ω-5 gliadin-specific IgE was detected in 30% of other patients who had a wheat allergy. These results lead to a positive predictive value of 37.5% and to a negative predictive value of 91%. Conclusions: Although not specific to EIWA, the new ImmunoCAP ω-5 gliadin is an important biological test because of its negative predictive value. In case of food-dependent exercise-induced allergy, the absence of ω-5 gliadin-specific IgE will almost completely exclude the implication of wheat. Copyright © 2008 S. Background: Antigenic profiles obtained by ELISA with IgE from patients with wheat food allergy (WFA) established that major allergens are albumins/globulins (AG) for children suffering from atopic eczema/dermatitis syndrome (AEDS), ω5-gliadins for adults suffering from wheat-dependent exercise-induced anaphylaxis (WDEIA), anaphylaxis or urticaria and low-molecular-weight (LMW) glutenin subunits for patients with anaphylaxis. We aimed to characterize a new mast cell transfectant for its ability to degranulate with wheat proteins and patient sera and compare these results to those obtained by ELISA. Methods: Thirty sera from patients with WFA were tested: 14 with AEDS (group 1) and 16 with WDEIA, anaphylaxis or urticaria (group 2). An IgE Fc receptor (FcεRI) humanized rat RBL-2H3 line was established by transfection with cDNAs encoding α-, β- and γ-subunits for the human IgE receptor. Results: A humanized RBL clone was selected for its capacity to express mRNA α-, β- and γ-subunits of FcεRI, to bind allergen-specific human IgE and to degranulate. In group 1, sera induced enhanced degranulation with AG extract, but rarely reacted with gliadins and glutenins. In group 2, half of the sera showed degranulation with LMW glutenins whereas the AG fraction and lipid transfer proteins were rarely positive. ω5-Gliadins did not appear as a major allergen in degranulation assays, although functional allergen-specific IgE was measurable in appreciable amounts. Conclusion: Our data demonstrate that in wheat food allergen evaluation, correlation exists between mast cell degranulation and IgE measurements, depending on the type of allergen. 39. Robust Multiuser Interference Cancellation for OFDM Systems With Frequency Offset.

Abstract

In orthogonal frequency division multiple access systems clusters of subcarriers are assigned to different users for parallel data transmissions. The subcarriers are overlapped, but orthogonal to each other such that there is no intercarrier interference (ICI). However, synchronization errors among users cause the loss of the orthogonality and introduce ICI resulting in multiple-access interference. Synchronization between users is particularly difficult in the uplink channel where the user signals are potentially asynchronous and affected by different frequency offsets due to misalignment in carrier frequencies and Doppler shifts. This paper proposes a method to lower the effects of different frequency offsets among user signals in an OFDMA uplink system. The multiple access interference due to the user frequency misalignments is reduced by reconstructing and removing the interfering signals in the frequency domain. An approach based on the selective cancellation method, is proposed and its performance is analyzed by means of theoretical analysis and computer simulations. The effectiveness of the proposed system has been evaluated in the case of ideal and no-ideal frequency offset estimation and has been compared with that of the classical successive and parallel cancellation schemes. Simulation results show that the proposed approach allows performance close to the ideal case, i.e., with ideal frequency synchronization among users, with a low increase of the implementation complexity. This paper presents the characterisation of polyclonal antibodies directed against two different N-terminal sequences of omega -gliadins. Both antisera recognised specifically the corresponding omega-gliadin fractions but showed different reactivities against omega-gliadins extracted from bread and durum wheats. Antibodies directed against the 'SRLL' type (omega 5 type) recognised several omega-gliadin components in bread and durum wheat extracts whereas antibodies directed against the 'AREL' type (omega 2 type) reacted specifically with an omega -gliadin component from bread wheat. The narrow specificity of this antiserum made it potentially interesting for the detection of bread wheat additions in durum wheat pasta. Abstract

This work investigated the effect of repeated applications on enhanced degradation of metalaxyl in two different agricultural soils used for cultivation of orange and lemon from Casa Branca and Itapetininga districts of São Paulo State, Brazil. Soil samples were collected from areas repeatedly treated with commercial ridomil 50GR for six successive years, and from other areas never exposed to this fungicide. At the laboratory, soil samples received a 14C-metalaxyl solution and its degradation was studied through radiometric techniques to measure biomineralization and recovery of extractable- and soil-bound products. Enhanced degradation was verified only in one soil, although partial degradation and mineralization of the fungicide were detected in both soils. The different rates and patterns of metalaxyl degradation in the soils were probably due to their different physical, chemical, and biological characteristics.

Abstract: Breeding has enabled the development of wheat genotypes with different quality characteristics and end uses. Despite its numerous positive properties that make it one of the most cultivated crops, wheat is known to induce allergic reactions in predisposed consumers. Genetic modification (GM) technology for crop improvement has recently emerged and its impact on allergenicity must be evaluated, as recommended by the Codex Alimentarius. Our aim was to determine whether the variation in the amount of allergenic polypeptides in five GM wheats, along with their untransformed genotypes, was equivalent to the variation observed among twenty commercial cultivars, either durum or bread wheats. Since the most important factor involved is the amount of allergenic polypeptides, we performed Enzyme-Linked Immunoabsorbent Assays with IgE from twenty-two patients suffering from food or respiratory allergy to wheat on two well characterised soluble protein fractions. Statistical analyses showed a significant effect of the genotypes and sera, both by considering GM lines and untransformed genotypes. This study leads us to conclude that a wide variation exists in the amount of allergenic polypeptides among durum and bread wheat cultivars, and that the differences observed between GM wheats and their parents are within the range of cultivated wheats. Abstract: Twenty-eight wheat cultivars representative of the three main European wheat producing countries, France, UK and Germany, were selected as a source for the preparation of a reference gliadin. One kilogram of kernels from each cultivar were mixed and milled. The resulting white flour was defatted and vacuum dried. Albumins and globulins were eliminated by extraction using 0.4M NaCl solution and gliadins were extracted with 60% ethanol. The gliadin extracts were concentrated, desalted by ultrafiltration, freeze-dried, and homogenised. After tests had shown good solubility and homogeneity, aliquots of the reference gliadin were sent to 16 different laboratories for further investigations: The material was analysed by various methods including RP-HPLC, SE-HPLC, RP-HPLC-ESI-MS, MALDI-TOF, capillary electrophoresis, acid-PAGE, 2D-PAGE, SDS-PAGE and immunoblotting and ELISA-tests with different monoclonal and polyclonal antibodies. The results showed that the gliadin composition of the source flour and the reference gliadin matched perfectly, demonstrating that no major gliadin components had been lost during the isolation procedure. The reference gliadin showed good immunochemical sensitivity with different gliadin antibodies in enzyme immunoassays. Because of its high protein and gliadin content, good solubility, homogeneity, stability and representative character, the product is regarded as a suitable universal reference material. Background: An improved understanding of pathways to alcohol use disorder (AUD) among service members may inform efforts to reduce the substantial impact of AUD on this population. This study examined whether the relationship between a service-related risk factor (combat exposure) and later AUD varied based on individual differences in genetic liability to AUD., Methods: The sample consisted of 1203 US Army soldiers of genetically determined European ancestry who provided survey and genomic data in the Army STARRS Pre/Post Deployment Study (PPDS; 2012-2014) and follow-up survey data in wave 1 of the STARRS Longitudinal Study (2016-2018). Logistic regression was used to estimate the conditional effect of combat exposure level (self-reported in PPDS) on odds of probable AUD diagnosis at follow-up, as a function of a soldier's polygenic risk score (PRS) for AUD., Results: The direct effect of combat exposure on AUD risk was non-significant (AOR=1.12, 95 % CI=1.00-1.26, p=.051); however, a significant combat exposure x PRS interaction was observed (AOR=1.60, 95 % CI=1.03-2.46, p=.033). Higher combat exposure was more strongly associated with elevated AUD risk among soldiers with heightened genetic liability to AUD., Conclusions: The effect of combat exposure on AUD risk appeared to vary based on a service member's level of genetic risk for AUD. Continued investigation is warranted to determine whether PRS can help stratify AUD risk within stress-exposed groups such as combat-deployed soldiers.
