Background and Aim: We have developed a new rat model that mimics the natural course of diabetic nephropathy seen in type 2 diabetes., Methods: Nine days after intravenous injection of streptozotocin (STZ) (40 mg/kg) or vehicle to 8-week-old male Sprague-Dawley rats, the animals' right kidneys were surgically removed. Two weeks after surgery, the STZ-injected rats were fed on either a high-fat (ST+HF) or a normal (ST) diet, while the vehicle-injected rats were fed on the high-fat diet (HF)., Results: Baseline biochemical markers did not differ between the three groups. Only the ST+HF group showed a significant increase in plasma glucose levels after 15 weeks, and simultaneously plasma insulin levels started to decrease, followed by an increase in plasma total cholesterol and triglyceride levels at 25 weeks and slightly later by an increase in blood pressure. In the ST+HF group, significant microalbuminuria was detected at 15 weeks followed by overt proteinuria, both of which were absent in the other two groups. Also in ST+HF, the creatinine clearance rate increased until week 15, and then gradually decreased. Histologically, ST+HF rats showed mesangial expansion at week 25, and diffuse glomerular sclerosis at the end of the experiments., Conclusion: The chronological changes in biochemical, physiological and histological markers in ST+HF rats are reminiscent of human type 2 diabetes and nephropathy. Our new model of type 2 diabetic nephropathy should help us to understand the pathophysiology of the disease and serve to explore measures to prevent and treat diabetic nephropathy.

Objectives: We studied the difference between effects of therapeutic dose and sub-therapeutic dose of gliclazide on the glucose-induced insulin secretion., Methods: The normal rat pancreas was isolated and perfused with Krebs-Ringer buffer containing 1-14 mmol/l glucose. Influcences of 0.25 and 2.5 microg/ml gliclazide on the glucose concentration-insulin secretion curve was examined., Results: Gliclazide at 0.25 microg/ml significantly potentiated 5-8 mmol/l glucose-induced insulin secretion (2.5 +/- 0.5 vs 1.0 +/- 0.3 mU for 15 min at 6.5 mmol/l glucose, P<0.01), but did not give influence on either 1-3 or 10-14 mmol/l glucose-induced insulin secretion. The glucose concentration, at which half-maximal insulin secretion was observed, was lower with gliclazide (5.9 mmol/l) than in the control (7.5 mmol/l). Gliclazide at 2.5 microg/ml markedly increased the maximally glucose-stimulated insulin secretion from 3.9 +/- 0.5 mU for 15 min in the control to 6.6 +/- 0.7 mU for 15 min (P<0.01). The half-maximal insulin secretion was observed at a lower glucose concentration (5.0 mmol/l) than in the absence of gliclazide., Conclusion: Gliclazide in sub-therapeutically low dose has different effects on insulin secretion from in therapeutic dose, namely sharpens the insulin secretion sensitivity to glucose with no influence on the maximal insulin secretion. It is possible that low doses of gliclazide might be of interest in some type 2 diabetics whose main pathophysiology is the blunting of insulin secretion response to hyperglycemia.

Aims: Although it is commonly recognized that ethanol suppresses gluconeogenesis, the influence of alcohol intake on blood glucose levels remains controversial. Ethanol may act on both glucose production and glucose consumption in the liver. Thus, we studied each effect of ethanol on glucose oxidation, gluconeogenesis, glycogenesis and glycogenolysis in the liver., Methods: The rat liver was isolated and cyclically perfused with a medium containing 50 mmol/l ethanol., Results: Ethanol enhanced 14C-glucose oxidation in the liver from 1.09 +/- 0.11 to 1.41 +/- 0.14 micromol for 20 min (p < 0.05). Gluconeogenesis from 14C-lactate was markedly reduced by ethanol from 8.0 +/- 1.3 to 1.5 +/- 0.6 micromol for 12 min (p < 0.01). Ethanol increased glycogenolysis (net hepatic glucose output, 0.47 +/- 0.10 vs. 0.22 +/- 0.04 mmol/30 min, p < 0.01), and then decreased hepatic glycogen content (179 +/- 38 vs. 273 +/- 39 mg in the presence of 1 mU/ml insulin after 30 min of perfusion, p < 0.05). Ethanol decreased the direct glycogenesis from 14C-glucose from 0.55 +/- 0.08 to 0.33 +/- 0.05 micromol per 100 mg glycogen for 30 min (p < 0.01). Ethanol inhibited the indirect glycogenesis from 14C-lactate from 0.21 +/- 0.04 to 0.09 +/- 0.01 micromol per 100 mg glycogen for 30 min (p < 0.01)., Discussion: The influence of ethanol on the blood glucose regulation by the liver seems to be different between fasted and fed states. Namely, ethanol has both the hypoglycemic effects through decreased gluconeogenesis and increased glucose oxidation and the hyperglycemic effects through decreased glycogenesis and increased glycogenolysis., (Copyright (c) 2004 S. To evaluate the direct effects of a barbiturate on cerebral functions without its influence on brain perfusion pressure, circulatory hormones and metabolites, the electroencephalogram (EEG) was studied in the isolated rat head. Male Wistar rats were anesthetized, and EEG electrodes were inserted into the cranium. A Krebs-Ringer bicarbonate buffer solution containing heparinized rat whole blood, 20 mmol/l glucose, 200 mmol/l mannitol and 0.1 mg/ml dexamethasone was used for the perfusate. The bilateral common carotid arteries were cannulated, pumped at a rate of 6 ml/min and the head was isolated. The venous effluent was reoxygenated and recirculated into the brain. Twenty-five min after isolation of the heads pentobarbital was added to the perfusate at concentrations of 0.1, 0.5 and 2.5 mg/ml. EEG was recorded before and during perfusion. EEG activity could be recorded for more than 25 min after the beginning of perfusion. EEG activity gradually declined from 42+/-5 microV before perfusion (in vivo) to 4+/-1 microV at 25 min after the beginning of perfusion. Then, 3 min after the addition of pentobarbital, the EEG activity became significantly higher in the high dose groups; 12+/-3 microV in the 0.5 mg/ml group (p<0.05) and 12+/-1 microV in 2.5 mg/ml group (p<0.05) compared with the group without pentobarbital (2+/-2 microV). The present study suggests that a barbiturate has mitigating effects on the brain damage induced by the in vitro brain perfusion.

To study the role of B-cells in the regulation of glucagon secretion by glucose, the rat pancreas was perfused with 0.4 mmol/l diazoxide. Perfusate glucose was 5 mmol/l of a basal concentration, and then was decreased to 1 mmol/l, or was increased to 15 mmol/l. Insulin secretion was suppressed by diazoxide below the detectable level at each glucose concentration. Glucagon secretion was increased two-fold during the glucopenic perfusion without diazoxide, but was not changed at a low glucose concentration in the presence of diazoxide. During the glucose-excessive perfusion for 15 min, glucagon secretion was lowered from 0.69 +/- 0.17 pmol at 5 mmol/l glucose to 0.36 +/- 0.10 pmol at 15 mmol/l glucose (p < 0.05) without diazoxide, whereas that was inversely increased from 0.55 +/- 0.14 at 5 mmol/l glucose to 0.85 +/- 0.13 pmol at 15 mmol/l glucose (p < 0.05) in the presence of diazoxide. These results suggest that appropriate insulin secretion is necessary for the normal responses of glucagon secretion to hypoglycemia and hyperglycemia in the non-diabetic rat pancreas.

Objectives: To study effects of diabetes mellitus on parathyroid hormone (PTH) secretion, the rat parathyroid glands were perfused via the bilateral carotic arteries., Material and Methods: Diabetic rats showed plasma glucose levels above 30 mmol/l, but no change in plasma PTH concentration, 14 days after an administration of 70 mg/kg of streptozotocin. Krebs-Ringer bicarbonate buffer containing 0.1% bovine serum albumin was used for perfusate. Perfusate calcium was lowered from 2.5 mmol/l to 0.5 mmol/l., Results: In the normal rat parathyroid glands, PTH secretion was promptly evoked by calcipenia (from below 0.25 ng for 2.5 min in the perfusion with normal calcium to 0.73 +/- 0.17 ng during calcipenia time 0-2.5 min), and slightly decreased (0.51 +/- 0.12 ng during calcipenia time 2.5-5 min), and then increased (0.75 +/- 0.16 ng during calcipenia time 7.5-10 min). Diabetes abolished the early phase of PTH secretion (below 0.25 ng during calcipenia time 0-2.5 min), but did not affect the late phase of PTH secretion (0.64 +/- 0.14 ng during calcipenia time 7.5-10 min). Insulin treatment with 25 U/kg daily for 14 days completely normalized the early phase of PTH secretion., Conclusion: It is suggested that an insulin-deficient diabetes, not quantitatively but qualitatively, impaires the PTH secretion.

The brain requires oxygen and glucose for energy metabolism. Electroencephalogram (EEG) was recorded to determine the effect of glucose concentration on the cerebral function in hypoxic episode. Rats were divided into 3 groups: a streptozotocin-induced diabetic hyperglycaemic group, a normoglycaemic group, and an insulin-induced hypoglycaemic group. Hypoxia was induced by ventilating with 100% N2 for 3 min. EEG amplitude both 5 and 10 min after anoxia loading was higher in the diabetic hyperglycaemic than in the normoglycaemic group, though not significantly. Time for decreasing the EEG amplitude during anoxia loading was significantly longer in the hyperglycaemic than in the normoglycaemic group. Time for recovering the EEG amplitude after anoxia loading was significantly shorter in the hyperglycaemic group and was longer, though not significantly, in the hypoglycaemic group than in the normoglycaemic one. These results suggest the brain is more tolerant to hypoxia during diabetic hyperglycaemia than during normoglycaemia.

To study effects of acidosis on glucagon secretion and gluconeogenetic action of glucagon, rat pancreas and liver were perfused with media of pH 6.4, 6.9, 7.4 and 7.9. Glucagon secretion from the pancreas during glucopenic perfusion (1 mmol/l) was blunted at alkalotic pH, and was augmented at acidotic pH; 0.28+/-0.18 at pH 7.9 (P<0.01), 3.57+/-0.94 at pH 6.9 (P<0.01) and 1.72+/-0.36 at pH 6.4 (P<0.01), vs. 0.66+/-0.25 pmol for 15 min at pH 7.4. Incorporation rate of 14C of lactate-U-14C into glucose carbon one was decreased at pH 7.9 (1.2+/-0.2% for 15 min, P<0.05) and was increased at pH 6.9 (2.8+/-0.5%, P<0.05) compared to that at pH 7.4 (1.9+/-0.3%). Percent increasing rate of lactate gluconeogenesis by 1 nmol/l glucagon was not different within a range of pH 6.4-7.9. Thus, glucagon-stimulated gluconeogenesis from lactate was smaller at pH 7.9 (2.2+/-0.6%) and was significantly greater at pH 6.9 (4.9+/-0.9%, P<0.05) than that at pH 7.4 (3.2+/-0.6%). These results suggest that the pancreatic glucagon secretion and the glucagon-stimulated hepatic gluconeogenesis play more important roles in the maintainance of blood glucose level in the stress states associated with acidosis than without acidosis.

To study the effects of diabetes on the renal actions of parathyroid hormone (PTH), we observed urinary excretion of cyclic adenosine monophosphate (cAMP) and phosphorus in isolated perfused rat kidney. Diabetic rats were kept for 7 days after an intraperitoneal injection of 70 mg/kg streptozotocin (STZ). STZ-induced diabetic rats were treated with a daily injection of 20 U/kg lente-type insulin for 7 days. Plasma albumin, calcium, phosphorus, and PTH levels were not different among normal control, diabetic and insulin-treated diabetic groups. In the control rat kidney, the addition of PTH increased urinary cAMP excretion from 8 +/- 3 to 190 +/- 49 pmol/5 min and urinary phosphorus excretion from 11.3 +/- 4.4 to 33.6 +/- 10.8 microg/5 min. In the STZ-diabetic rat kidney, basal urinary cAMP was impaired, and PTH altered neither urinary cAMP nor phosphorus excretion (from below 0.7 to below 0.7 pmol/5 min, and from 15.5 +/-4.5 to 13.6 +/- 8.1 microg/5 min, respectively). Insulin treatment completely recovered the PTH actions. These results show that insulinopenic diabetes induces PTH resistance in the kidney.

The pathogenesis of glucocorticoid-induced insulin resistance in the peripheral tissue was studied by perfusion experiments of the isolated rat hindquarter and insulin tolerance tests in the rat with the cutting off operation of blood supplies to the liver, kidneys, intestines and pancreas. The rat was injected with 0.5 mg/kg dexamethasone for 7 days. Just after the multiple functional organectomies, insulin was loaded in doses of 0.1 and 0.5 U/kg. Plasma insulin levels during 0-10 min were approximately 100-20 and 500-100 microU/ml, respectively. The decreasing rates in plasma glucose level after injections of saline and 0.1 U/kg insulin were much smaller in dexamethasone-treated than control rats (0.0+/-7.2 vs 13.3+/-3.4%/10 min, p<0.05, and 8.0+/-5.8 vs 39.4+/-3.7%/10 min, p<0.01 respectively). The decreasing rate after 0.5 U/kg insulin loads was similar between both groups (40.2+/-10.0 in dexamethasone-treated and 52.3+/-7.3%/10 min in control rats). Plasma non-esterified fatty acids were raised in dexamethasone-treated compared to control rats (1.23+/-0.23 vs 0.73+/-0.08 mM, p<0.01). In the hindquarter perfusion study, glucose uptake in the dexamethasone-treated rat leg was no less than in the normal rat leg under a palmitate-free condition, and was decreased by the addition of 1.0 mM palmitate without and together with 100 and 500 microU/ml insulin. These results suggest that the glucocorticoid-induced peripheral insulin resistance is characterised by the decreased sensitivity and the preserved responsiveness to insulin, and is caused mainly by an elevated non-esterified fatty acid level.

To study the effects of supraphysiologically high sodium concentrations on insulin secretion, the rat pancreas was perfused with normal Krebs-Ringer bicarbonate buffer (KRBB) solution and with KRBB solutions containing an additional 25 and 50 mmol/l NaCl. The first phase of insulin secretion in response to 18 mmol/l glucose was enhanced in a dose-dependent manner, from 20.8+/-4.0 pmol for 4 min in normal KRBB to 36.2+/-10.3 pmol (P<0.05) and 60.3+/-14.6 pmol (P<0.01) with addition of 25 and 50 mmol/l NaCl, respectively. The second phase secretion was significantly increased by the addition of 50 mmol/l NaCl (259.4+/-38.6 vs 170.9+/-47.4 pmol for 16 min P<0.05). The addition of 50 mmol/l isethionate Na to normal KRBB solution also increased the first and second phases of insulin secretion in response to glucose. The increase in osmolarity by the addition of mannitol to normal KRBB solution did not affect the glucose-induced insulin secretion. A high sodium concentration affected neither tolbutamide-induced nor arginine-induced insulin secretion. It is suggested that sodium concentrations have an important role in insulin secretion in response to glucose, but not to other secretagogues.

In order to study the effects of troglitazone on insulin resistance associated with elevated plasma free fatty acid (FFA), the hindquarters of rats treated with troglitazone for 14 days were perfused with a medium containing 15 mmol/l glucose, 0-1,000 microU/ml insulin, and 0 or 1.0 mmol/l palmitate. In the absence of palmitate, net glucose uptake was similar between control and troglitazone-treated rat hindquarters at each insulin concentration: 47 +/- 10 and 45 +/- 9 mumol for 30 min at 0 microU/ml insulin; and 94 +/- 14 and 99 +/- 13 mumol for 30 min at 1,000 microU/ml insulin respectively. Addition of palmitate abolished insulin stimulation of net glucose uptake in control rat hindquarter but did not decrease it in troglitazone-treated rat hindquarter (60 +/- 11 vs 99 +/- 12 mumol for 30 min at 1,000 microU/ml insulin, p < 0.01). It is suggested that the "FFA-tolerant effect" is one mechanism by which troglitazone reduces insulin resistance.

To study the effect of a therapeutic dose (3 microM) of AY-4166 on the glucose-stimulated insulin secretion, the rat pancreas was perfused with 5 to 15 mM glucose. AY-4166 did not affect the basal insulin secretion at 5 mM glucose, and increased the first and second phases of insulin secretion stimulated by 7.5 to 15 mM glucose. The dose-response curve of the insulin secretion to glucose was shifted to the left side by AY-4166. These results suggest that AY-4166 ist not a stimulator releasing insulin independently of glucose concentrations, but a potentiator of insulin secretion.

To clearly understand the hyperglycemic action of glucocorticoids, we studied the action of glucagon on lactate gluconeogenesis in the liver of rats 7 days after adrenalectomy and after treatment with 1 mg/kg dexamethasone for 7 days. The liver was isolated and cyclically perfused at 20 ml/min with 25 ml of perfusion medium containing 5 mM lactate, [U-14C]lactate, and 0-100 ng/ml glucagon. In the absence of glucagon, incorporation of [14C]lactate into glucose carbon 1 did not change significantly in the adrenalectomized rat liver (1.66 +/- 0.12% of total radioactivity for 5 min) and increased in the dexamethasone-treated rat liver (3. 61 +/- 0.54%, P < 0.01) compared to the normal rat liver (1.99 +/- 0. 28%). The response of lactate gluconeogenesis to glucagon was extremely blunted in the adrenalectomized rat liver and was much larger in the dexamethasone-treated rat than in the normal rat liver (at a glucagon concentration of 100 ng/ml, 2.13 +/- 0.33, 8.55 +/- 1. 06, and 4.61 +/- 0.53% for 5 min, respectively). Glucagon binding to liver plasma membrane was not changed by adrenalectomy and was decreased by dexamethasone treatment. These results suggest that glucocorticoids induce hyperglycemia by increasing the response to glucagon, together with the high basal activity of hepatic gluconeogenesis. To clearly understand the autoregulation of glucocorticoid secretion by the glucocorticoid level, we studied the effects of cortisol and dexamethasone on the cortisol secretion in the perfused guinea pig adrenal glands. The basal cortisol secretion was suppressed by cortisol infusion at 0.1 microgram/ml, but not affected by dexamethasone infusion at 1 microgram/ml. The cortisol response to ACTH after pre-perfusion for 35 min was much quicker and larger in the presence of 0.1 microgram/ml cortisol, and was quicker in the presence of 1 microgram/ml dexamethasone than in the absence of glucocorticoid. It is considered that glucocorticoids play a permissive role in the action of ACTH on the adrenal gland.

The bihormonal control by insulin and glucagon of blood ketone body level was studied. Mixed solutions with various molar ratios of glucagon and insulin (G/I) were subcutaneously infused continuously for five days by use of the osmotic minipump in the normal rats. The concentrations of insulin and glucagon solution were set at the high G/I molar ratio, the moderate G/I molar ratio and the low G/I molar ratio. In addition, the moderate G/I molar ratio group was divided into three sub-groups: low glucagon and low insulin, moderate glucagon and moderate insulin, and high glucagon and high insulin. After five days, the rats were decapitated to measure plasma ketone body, free fatty acid (FFA), glucose, insulin and glucagon. The FFA level was not significantly different among three groups. The glucose level was not different between the high and moderate G/I molar ratio groups, and decreased in the low G/I molar ratio group. 3-beta-hydroxybutyrate (3-OHBA) and acetoacetate (AcAc) levels in the high G/I molar ratio group were elevated, and 3-OHBA level in the low G/I molar ratio group was lowered compared to those in the moderate G/I molar ratio group. Among three moderate G/I molar ratio sub-groups, there was no difference in 3-OHBA and AcAc levels. These results demonstrate that plasma ketone body levels are controlled by the plasma G/I molar ratio.

To clearly understand the feedback mechanism of renin secretion by intrarenal angiotensin II synthesis, we studied the stimulatory effect of captopril, an angiotensin converting enzyme inhibitor, on renin release from the isolated perfused guinea pig kidneys. Captopril did not affect the basal renin secretion. Captopril increased on a dose dependent basis renin release induced by 0.1 mg/ml furosemide; 5.8 +/- 1.3 ng/ml/hr at 0 mg/ml of captopril vs. 8.8 +/- 1.6 ng/ml/hr at 0.01 mg/ml (p < 0.05) and 11.8 +/- 2.4 ng/ml/hr at 0.1 mg/ml (p < 0.01), 15-20 min after furosemide infusion. After lowering the flow rates from 10 to 5 ml/min, renin secretion was not altered during the first 12 minutes. However, after adding 0.1 mg/ml captopril, renin secretion was enhanced within the following 8 minutes (8.3 +/- 1.5 vs. 3.7 +/- 0.5 ng/ml/hr at the end of low flow rate period, p < 0.01). Lowering the perfusion flow of the guinea pig kidneys decreases the NaCl flux at the macula densa and also seems to have a similar effect to furosemide. Therefore, these effects of captopril suggest that intrarenal angiotensin II production has an important role in the regulation of renin secretion, probably via effects on the macula densa function.

The dynamics of glucose movement across perfused livers were assessed in carbon tetrachloride (CCl4)-injured rats. Rats were given CCl4 for 8 weeks and became glucose intolerant and hyperinsulinemic. The fasted rat liver was cyclically perfused with 4 mM lactate and various concentrations (0-20 mM) of glucose for 20 min. In the CCl4-injured liver, net glucose output was less suppressed at high glucose levels than in the normal liver (147 +/- 70 vs 18 +/- 10 mumol at 20 mM glucose, P < 0.05). Deposition of the carbon from [14C] glucose into glycogen was stimulated at high glucose levels and was markedly reduced in the CCl4-injured liver compared to the normal liver (0.58 +/- 0.33 mumol vs 1.44 +/- 0.20 mumol at 20 mM, P < 0.01). Conversion of [14C] lactate to [14C] glucose was not different between the CCl4-injured and the normal liver at each glucose level. Deposition of the carbon from [14C] acetate into glycogen in the CCl4-injured liver was larger than that in the normal liver at 0 mM glucose (0.81 +/- 0.15 mumol vs 0.32 +/- 0.06 mumol, P < 0.01), but was similar to the normal at 20 mM glucose. In the CCl4-injured liver, utilization of exogenous glucose was impaired at high glucose levels, and gluconeogenetic activity was not impaired at low glucose levels. These changes in the hepatic glucose metabolism seem to account for postprandial hyperglycemia without fasting hypoglycemia associated with liver diseases.

Correlation between blood glucose, glucagon and adrenaline levels and gluconeogenesis was studied during the recovery from insulin-induced hypoglycemia in rats. Rats, overnight fasted, were intravenously injected with 40 microCi/kg of [U-14C]-lactate and 1 U/kg of porcine insulin under an anesthesia with pentobarbital sodium. Blood samples were drawn via the peripheral vein at 0, 5, 10 and 20 min. Plasma insulin level was 617 +/- 115 microU/ml at 5 min. Plasma glucose level was significantly decreased at 5 min (2.7 +/- 0.3 mM at 5 min v.s. 4.3 +/- 0.2 mM at 0 min, P < 0.01). Plasma glucagon and adrenaline did not significantly respond at 5 min, and then rised. Specific radioactivity of plasma [1-14C]-glucose was significantly higher at 5 and 10 min in the insulin-injected rat than the saline-injected rat (204 +/- 34 v.s. 130 +/- 14 d.p.m./mumol at 5 min, P < 0.01; 275 +/- 32 v.s. 186 +/- 16 d.p.m./mumol at 10 minm P < 0.01). These results suggest that lowering of blood glucose level stimulates gluconeogenesis independently of the release of counter-regulatory hormones in insulin-induced hypoglycemia.

A gastric cancer with liver metastases was associated with low morning levels of plasma glucose (24 mg/dl), insulin (< 2.5 microU/ml) and growth hormone (0.23 ng/ml). Primary and metastatic tumour tissue stained positively with anti-insulin-like growth factor II (IGF-II) monoclonal antibody. Western immunoblot analysis revealed a high molecular weight IGF-II in the serum: 15 kDa (normal: 7.5 kDa). Postmortem reverse transcription polymerase chain reaction on mRNA from both sites revealed 471 base pairs size cDNA encoding prepro-IGF-II. These results suggest that the gastric carcinoma encoded, expressed, and secreted IGF-II, probably causing the extrapancreatic tumour hypoglycaemia.

The effects of arterial-portal glucose difference on the gluconeogenesis from lactate were studied using the bivascular perfused liver isolated from the fasted rat. The liver was cyclically perfused at flow rates of 14 ml/min from the portal vein and of 7 ml/min from the hepatic artery with the total volume of 35 ml of perfusion medium containing 2 mM glucose, 3 mM lactate and (U-14C)-lactate for 20 min. Glucose was infused at a rate of 27.75 mumol/min into the arterial cannula (A-experiment) or the portal cannula (P-experiment), making each arterial-portal glucose gradient of +3.96 mM (arterial glucose > portal glucose) and -1.98 mM (arterial glucose < portal glucose) throughout the experiment. Perfusate lactate concentration was lower in A-experiment than in P-experiment (1.40 +/- 0.25 vs 1.93 +/- 0.23 mM at 20 min, mean +/- SD, p < 0.05). Incorporation of radioactivity from (U-14C)-lactate into glucose carbon 1 in perfusate was 5.7 +/- 0.8% of total radioactivity per 20 min in A-experiment vs 2.8 +/- 0.6%/20 min in P-experiment (p < 0.01). These results suggest that the arterial-portal glucose difference is an important factor to regulate the hepatic gluconeogenesis.

To study effects of the portal-arterial glucose difference on the hepatic glycogenesis, the liver was isolated from fasted rats and was bivascularly perfused. Thirty-five milliliters of Krebs-Ringer buffer (pH 7.4) with 2 mM glucose, 3 mM lactate, 20 ng/ml insulin, and [1-14C]glucose or [U-14C]lactate was recirculated at flow rates of 14 ml/min via the portal vein and 7 ml/min via the hepatic artery. Glucose was continuously infused at a rate of 27.75 mumol/min into the portal (P experiment) and the arterial cannula (A experiment), and the portal-arterial glucose gradients were +1.98 and -3.96 mM. Perfusate glucose concentration was not different between the P and A experiments within 20 min. Perfusate lactate level was higher in the P experiment than in the A experiment at 20 min. Incorporation of radioactivity from [14C]glucose into glycogen was higher in the P experiment than in the A experiment (0.245 +/- 0.014%/20 min vs 0.175 +/- 0.022%/20 min, P < 0.01), and not influenced by the addition of insulin. Incorporation of 14C from [14C]lactate into glycogen was not different between the P and A experiments, and was significantly increased with the addition of insulin. This activity, in the presence of insulin, was higher in the P experiment than in the A experiment (0.490 +/- 0.028%/20 min vs 0.406 +/- 0.025%/20 min, P < 0.05). These results suggest that the portal-arterial glucose difference has an important role in the regulation of hepatic glycogenesis from exogenous glucose and gluconeogenesis.

To investigate the direct effect of corticotropin (ACTH) on the renin-angiotensin-aldosterone system, isolated guinea-pig kidneys with adrenal glands were perfused with various doses of ACTH (0.1-1000 micrograms/l) and 0.3 mmol/l of dibutyryl cyclic AMP (cAMP) through each cannula inserted into the abdominal aorta and the inferior caval vein. Perfusate renin activity was increased in a dose-dependent manner by the addition of ACTH in a range of 0.1-1000 micrograms/l, and reached a plateau at 20 min with each dose. The perfusate cAMP level was dose-dependently increased with 10-1000 micrograms/l of ACTH. Perfusate renin activity was also markedly increased by the addition of dibutyryl cAMP. The same effects of ACTH on renin and cAMP secretions were observed in the kidney perfusion model from which the adrenal glands were excluded. Aldosterone secretion failed to respond to 0.1 micrograms/l of ACTH, and was increased by higher concentrations (1-1000 micrograms/l) in the same experiments. These results demonstrate that ACTH has a direct effect on renal renin release in a physiological concentration (0.1 micrograms/l), and that the action of ACTH is probably mediated by cAMP. The sensitivity of renin release to ACTH stimulation is no less than that of aldosterone secretion during ACTH infusion, so it is possible that ACTH is an important stimulator of the renin-angiotensin system.

To determine direct effects of epinephrine on adrenal cortisol secretion, bilateral adrenal glands were isolated from guinea pigs, together with bilateral kidneys, aorta, and inferior caval vein for influent and effluent routes. The preparation was perfused with oxygenated Krebs-Ringer bicarbonate solution (pH 7.4) containing 10 mM glucose, 0.2% bovine serum albumin, and 4.6% dextran. The perfusate cortisol level was elevated by the addition of epinephrine in a dose-dependent manner at concentrations greater than 100 pg/ml and increased eightfold as high as the basal level at 1 micrograms/ml epinephrine. The stimulatory effect of epinephrine on cortisol secretion was completely abolished by phentolamine, an alpha-adrenergic antagonist but was not affected by propranolol, a beta-adrenergic antagonist. These results demonstrate that epinephrine has a direct stimulatory effect on adrenal cortisol secretion via an alpha-adrenergic mechanism and also suggest that not only adrenocorticotropin but also epinephrine is a most important factor for the regulation of cortisol secretion.

To elucidate insulin action on hepatic glucose output (glycogenolysis) in the state exposed to an excess glucocorticoid, the fed rat liver was isolated and cyclically perfused with a medium containing 5 mM glucose and various concentrations of insulin. The rat was subcutaneously injected with 1 mg/kg of dexamethasone (Dex) for 7 days. Dex-treated rats showed marked increases of serum insulin and plasma glucose level compared with those in control rats. Hepatic glycogen contents in Dex group were markedly increased compared with those in control (115 +/- 5 and 28 +/- 4 mg/g, respectively). Insulin extraction rate in the perfused liver was not different between control and Dex group. Perfusate glucose level after 60 min perfusion was much higher in the Dex-treated rat liver than that of the control at 0 microU/ml insulin (34.5 +/- 2.5 vs 23.0 +/- 2.0 mM, P less than 0.01), and reduced to the nadir level (19.0 +/- 3.0 and 13.0 +/- 1.5 mM, respectively) at 100 microU/ml insulin in both groups, i.e., the decreasing rate in perfusate glucose level was not different between Dex and control group (43% and 44%, respectively). These results suggest that Dex-treatment augments hepatic glucose output, but does not affect the sensitivity and responsiveness of that to insulin.

To investigate anaerobic glycolysis and glucose oxidation in the diabetic heart, we studied the effects of anoxia and low exogenous free fatty acid (FFA) on myocardial adenosine triphosphate (ATP) content in the isolated perfused heart from streptozotocin-diabetic rats. The perfusate consisted of Krebs-Ringer buffer (pH 7.4) containing 5 mM glucose and 0.30 mM or 0.05 mM FFA, equilibrated with 95% O2 + 5% CO2 or 95% N2 + 5% CO2 gas. Under aerobic conditions, glucose uptake, lactate production and glucose oxidation by the diabetic heart were reduced by 68, 57 and 53% of the control, respectively, and FFA oxidation by the diabetic heart was increased by 67% of the control. Under anoxic conditions, the increments in glucose uptake and lactate production by the diabetic heart were much smaller than those by the control (2.6 and 9.5 mumol/g/30 min vs. 11.4 and 40 mumol/g/30 min, respectively). The ATP content in the diabetic myocardium was decreased more rapidly in the anoxic perfusion than that in the control. In the aerobic perfusion, the ATP content in the control myocardium was maintained for 60 min at low FFA (0.05 mM) in the perfusate. The ATP content in the diabetic myocardium was reduced after 40 min at low FFA but was well maintained for 60 min by the addition of 0.30 mM FFA. Thus the ATP content in aerobic diabetic myocardium was not well maintained when exogenous FFA supply was depleted. These results suggest that diabetic hearts have a primary defect in the stimulation of glycolysis and glucose oxidation, which does not result from increased FFA utilization.

The effect of thyroid hormone on plasma somatomedin-C (SmC) level and on SmC release from perfused rat liver was investigated. Plasma SmC levels and liver tissue SmC were significantly increased in thyroxine-treated rats. Physiological doses of triiodothyronine increased SmC release and SmC concentration in the perfused rat liver. These results indicate that thyroid hormone directly enhances the synthesis and release of SmC in the rat.

Using the isolated and perfused rat pancreas preparations, we examined whether or not the direct transmission of signals from the gastro-duodenum to the pancreatic islets is existent. Pancreases were isolated en bloc with stomach and the proximal portion of duodenum and were perfused without recirculation. After perfusion with 5.5mM glucose in the perfusate for 40 minutes, 1 ml solution of the following nutrients (0.9% and 9% NaCl, 5% and 50% glucose, 5% and 50% fructose) was respectively infused into the stomach. And ten minutes after the infusion, the pancreas was perfused with 16.7 mM glucose for 20 minutes. Although the glucose-induced insulin release was similar to controls in the pancreas with intragastric infusion of 0.9% NaCl, 5% glucose, and 5% fructose, it was significantly lowered in the pancreas with intragastric infusion of 9% NaCl, 50% glucose, and 50% fructose. These results indicate that there is a direct transmission of some signals from the gastro-duodenum to the pancreatic islets through intragastric high osmolarity.

This study was undertaken to elucidate the effect of thyrotropin (TSH) on the conversion of thyroxine (T4) to 3,5,3'-triiodothyronine (T3) in the isolated perfused rat kidney. The kidney was perfused with a synthetic medium containing 20 micrograms/dL T4 and the effect of constant infusion of bovine TSH (125 or 250 microU/mL) on the conversion of T4 to T3 was investigated. T4 uptake in the perfused kidney was not changed by the addition of TSH. However, the release of T3 (89 +/- 11 ng/g/30 min, mean +/- SD), tissue T3 (190 +/- 23 ng/g/30 min), net T3 production (227 +/- 37 ng/g/30 min), and the conversion rate of T4 to T3 (9.5 +/- 1.6%) in the kidney perfused with 250 microU/mL TSH were significantly (P less than 0.005) greater than those in controls (63 +/- 9, 143 +/- 15, 152 +/- 36 ng/g/30 min, and 6.3 +/- 1.9%), respectively. Degradation rate of T3 in perfused rat kidney was not changed by the addition of 250 microU/mL TSH. These results suggest that TSH may directly affect renal iodothyronine-monodeiodinating activity in rats in vitro.

This study was undertaken to elucidate the effect of diabetes on the conversion of T4 to T3 and rT3 in the isolated, perfused rat liver and kidney. The livers and kidneys from streptozocin (STZ)-induced (50 mg/kg i.p. 2 wk before killing) diabetic rats with or without T4 (30 micrograms/kg s.c. daily) treatment were perfused for 30 min with a synthetic medium containing T4 (6 micrograms/dl), and production of T3 and rT3 in the tissues was measured by radioimmunoassay. The production of T3 (111 +/- 38 ng/g/30 min, mean +/- SD) and conversion rate of T4 to T3 (19.7 +/- 5.8%) in the liver of diabetic rats without T4 treatment and those (124 +/- 41 ng/g/30 min and 21.6 +/- 4.9%) in the liver of diabetic rats with T4 treatment were significantly lower than those of controls (196 +/- 48 ng/g/30 min and 30.6 +/- 5.2%), respectively. The production of rT3 and conversion rate of T4 to rT3 in the liver of diabetic rats with or without T4 treatment were similar to those of controls. The production of T3 and rT3, and conversion rate of T4 to T3 and T4 to rT3, in the kidney of diabetic rats with or without T4 treatment were not significantly different from those of controls. These results suggest that the liver is far more important than the kidney in the overall reduction in the T4 to T3 conversion that occurs in diabetic rats.

To evaluate the effect of hepatocellular damage on thyroid hormone metabolism, the conversion of T4 to T3 in the perfused liver of rats with carbontetrachloride (CCl4)-induced liver injury was investigated. The liver was perfused non-recyclically with a synthetic medium containing 10 micrograms/dl T4 for 30 min, and the T3 production and the conversion of T4 to T3 were examined. The serum T3 level was significantly (P less than 0.05) lower in liver-injury rats (72 +/- 9 ng/dl) compared with controls (88 +/- 10 ng/dl). The uptake of T4 (4.15 +/- 0.85 micrograms/g/30 min), net T3 production (180 +/- 48 ng/g/30 min), and conversion rate of T4 to T3 (4.4 +/- 0.9%) in the perfused liver of liver-injury rats were similar to the values (3.78 +/- 0.66 micrograms/g/30 min, 191 +/- 31 ng/g/30 min, and 5.0 +/- 1.0%) in control rats, whereas the release of T3 (12.7 +/- 3.2 ng/g/30 min) in the effluent in injured livers was significantly (P less than 0.05) lower than that (16.9 +/- 3.0 ng/g/30 min) in control livers. We concluded that in CCl4-induced liver-injury rats, the low serum T3 level may partly be due to the decreased release of T3 from liver cells after conversion of T4 to T3 rather than to a decreased conversion of T4 to T3.

To estimate the residual beta-cell function, plasma glucose and C-peptide response to an intravenous injection of tolbutamide were observed in seven Type I (insulin dependent) and nine Type II (non-insulin dependent) diabetic patients on insulin therapy. Fasting plasma glucose in the patient was controlled below 11 mmol/l by conventional insulin therapy, and 1 g of tolbutamide was intravenously injected. In the serum C-peptide response following tolbutamide injection, seven of nine Type II diabetics showed the peak values of serum C-peptide more than 0.3 nmol/l from 20 min to 60 min after the load, and Type I diabetics gained the peak values less than 0.3 nmol/l except one patient. The decrease of plasma glucose within 60 min after tolbutamide injection was more than 20% of the basal level in eight of nine Type II diabetics, and in all of seven Type I diabetics the decrease did not exceed 15%. Both serum C-peptide and plasma glucose after tolbutamide load responded differently between Type I and Type II diabetics on insulin therapy, and it was thought that tolbutamide load-glucose response test may be a useful method to estimate the residual beta-cell function in the diabetics.

To study the intestinal insulinotropic effects in the diabetic state, an investigation was made into the release of insulin from isolated rat pancreas perfused with portal venous effluent (PVE) obtained from the isolated perfused intestine of streptozotocin-induced diabetic rats. Rat intestine was perfused with Krebs-Ringer bicarbonate medium for 1 h, and the PVE was collected from both untreated and glucose-treated intestines of control and diabetic rats. The PVE, after adjusting its glucose concentration to the desired level, was used as the perfusing medium for the pancreas of a different rat. Glucose concentration in the perfusion medium was maintained at 5.5 mM for 20 min and at 16.7 mM for the next 30 min. Insulin output from pancreas perfused with PVE from untreated intestine of diabetic rats (252 +/- 45 ng/30 min, mean +/- SD) was similar to that of controls (269 +/- 72 ng/30 min). Pancreatic insulin output with PVE from glucose-treated intestine of diabetic rats (579 +/- 100 ng/30 min) was significantly greater compared with either that using PVE from untreated intestine of diabetic rats (P less than 0.01) or that from glucose-treated intestine of control rats (368 +/- 57 ng/30 min) (P less than 0.02). These results indicate that the insulinotropic effect is markedly enhanced in streptozotocin-induced diabetic rats.

To elucidate the possible role of insulin, glucagon, and epinephrine on glucose intolerance in thyrotoxicosis, the secretion of insulin and glucagon in vivo (glucose, arginine, and epinephrine infusion tests) and in perfused pancreas and the hepatic action of insulin, glucagon, and epinephrine in perfused liver were investigated in experimental thyrotoxic rats (induced by thyroxine injection, 20 micrograms/kg sc, for 7 days). In thyrotoxic rats, fasting blood glucose (87 +/- 5 mg/dl, mean +/- SD) and plasma insulin (16 +/- 3 microU/ml) were significantly (P less than 0.001) higher than those in controls (74 +/- 5 mg/dl and 8 +/- 1 microU/ml), respectively. In glucose infusion test (0.5 g/kg iv), blood glucose, plasma insulin, and glucagon responses in thyrotoxic rats were not significantly different from those in controls. In arginine infusion test (5 mg/min for 20 min iv), the increments in blood glucose and plasma insulin after arginine in thyrotoxic rats were not significantly different from those in controls. Plasma glucagon response was almost the same in both groups. In epinephrine infusion test (100 micrograms/kg iv), the increments in blood glucose and plasma insulin in thyrotoxic rats were significantly greater than those in controls. In perfused pancreas, insulin and glucagon secretions in response to 16.7 mM glucose or 6.4 mM arginine in the presence of 5.5 mM glucose in thyrotoxic rats were not different from those in controls.(ABSTRACT TRUNCATED AT 250 WORDS)

To elucidate the mechanism of hyperinsulinemia and hyperglucagonemia in liver diseases, secretion of insulin and glucagon from perfused pancreas and extraction of insulin and glucagon by perfused liver were investigated in carbon tetrachloride inhalated liver injury rats. Fasting plasma insulin and glucagon levels and insulin and glucagon levels after arginine injection (0.25 g/kg) in liver injury rats were significantly higher than those in control rats. Glucagon levels after glucose injection (0.5 g/kg) were slightly increased in the liver injury group. Insulin responses to 16.7 mM glucose and 10 mM arginine from perfused pancreas were similar in liver injury rats and control rats. Glucagon response to 10 mM arginine from perfused pancreas was also similar in liver injury rats and control rats. Insulin extraction by perfused liver in liver injury rats (45 +/- 9%) was significantly (P less than 0.05) lower than that (64 +/- 11%) in control rats, and glucagon extraction by perfused liver was also significantly (P less than 0.05) decreased in liver injury rats (57 +/- 8%) compared with controls (73 +/- 9%). It is concluded that in carbon tetrachloride-induced liver injury rats, hyperinsulinemia and hyperglucagonemia may be dependent on decreased insulin and glucagon extraction by the liver.

To evaluate the effect of aging on hepatic extraction of insulin, peripheral plasma insulin and C-peptide levels after an oral glucose ingestion were measured in 39 non-obese healthy subjects with normal glucose tolerance test. The subjects were divided into two groups. One age group was below 30 years (n = 17) and another was above 50 years (n = 22). Blood glucose level was significantly higher in elderly subjects than in young at 30 and 60 min, and insulin level was also significantly higher in elderly subjects than in young subjects at 60 min after glucose ingestion. C-peptide response after glucose ingestion was similar in both groups. The molar ratio of C-peptide to insulin after glucose ingestion in elderly subjects was slightly lower than that in young subjects, and the difference between the incremental areas of C-peptide and insulin divided by the incremental area of C-peptide were significantly lower in elderly subjects. These results suggest that hepatic insulin extraction after glucose loading is decreased in elderly subjects and this may partly be responsible for the slightly altered glucose metabolism in elderly subjects with normal glucose tolerance test.

Glucose output was investigated in bivascularly perfused rat liver. Flow rate was 1 ml/min/g from hepatic artery and 2 ml/min/g from portal vein. Basal glucose output (130 +/- 30 microM/min) from bivascularly perfused liver was significantly (p less than 0.02) higher than that (80 +/- 20 microM/min) from usual monovascularly perfused (3 ml/min/g from portal vein) liver. However, glucagon-induced (1 ng/ml) increment in glucose output from bivascularly perfused liver 589 +/- 127 microM/30 min) was not significantly different from that (630 +/- 127 microM/30 min) of monovascularly perfused liver. These results suggest that glucagon-dependent glucose output was similar in monovascularly and bivascularly perfused rat liver, and that arterial blood supply may have some role in glycogenolysis in the perfused rat liver.

To study whether glucose level in mesenteric artery has some direct effect on the intestinal glucose absorption, the rat small intestine was isolated and vascularly and luminally perfused. The vascular perfusate consisted of Krebs-Ringer bicarbonate buffer (KRB) containing 4% dextran, 0.5% albumin and 1-28 mM glucose, and the luminal perfusate consisted of KRB containing 18 mM glucose. Glucose absorption rate was decreased in a dose-dependent manner by the increasing concentration of glucose in the vascular perfusate within a range of 1-28 mM, but not changed by the addition of fructose to the vascular perfusate. It is suggested that glucose in the mesenteric artery has a direct inhibitory effect on the intestinal glucose absorption, not through a change of osmotic pressure.

In order to elucidate the effect of hyperglycemia on serum thyroid hormone levels, serum T4 and T3 concentrations were measured in rats with continuous intravenous glucose infusion. 30% glucose solution was infused into the left jugular vein at a rate of 1 ml/hr for 12 hr, and serum glucose level was maintained at 400 mg/dl. Serum T4 levels were slightly but not significantly decreased and serum T3 levels were significantly decreased by infusion of glucose for 12 hr. The ratio of serum T3 to T4 was not significantly changed in rats with glucose infusion. These results suggest an inhibitory effect of hyperglycemia on thyroid hormone release.

To determine the effect of maturation on glucose-induced insulin release after starvation, we investigated the insulin release from perfused pancreas of young adult rats after fasting for 18-20 hr. In the rats weighing 300-400 g (12-14 w) and 200-250 g (7-8 w), the glucose-induced insulin secretion from perfused pancreas of starved rats was similar to that of fed rats. While, in the rats weighing 100-150 g (4-5 w), both the first and the second phase of glucose-induced insulin secretion from perfused pancreas of starved rats was markedly decreased compared with that of fed rats. These results suggest that insulin response to glucose is not matured in young below 150 g (5 w).

To elucidate the role of renal maltase in the metabolism of circulating maltose, glucose production from maltose was investigated in perfused kidney of normal and streptozotocin (STZ) diabetic rats. The kidney was perfused with 150 mg/dl maltose for 30 min and perfusate maltose and glucose were measured. Perfusate glucose concentration in the kidney of control rats gradually increased during perfusion. That in the kidney of diabetic rats was significantly lower than that in controls. Perfusate maltose concentration in the kidney of diabetic rats was significantly higher than that in controls. These results suggest that circulating maltose may enter kidney cells and subsequently metabolized to glucose, and that maltose uptake and glucose production were decreased in the kidney of STZ diabetic rats.

This study was undertaken to elucidate the effect of glibenclamide, one of sulfonylurea drugs, on thyroid hormone metabolism in vivo and on the conversion of thyroxine (T4) to 3,5,3'-triiodothyronine (T3) in the isolated perfused rat liver and kidney. Glibenclamide (0.2 mg/kg body weight) was intraperitoneally administered to normal and streptozotocin-induced (50 mg/kg) diabetic rats for 14 days. The liver and kidney of normal rats were perfused for 30 minutes with a synthetic medium containing 20 micrograms/dl T4 and glibenclamide (200 or 400 ng/ml), and production of T3 in the tissues was measured by radioimmunoassay. Serum T4 and T3 levels in control and streptozotocin-induced diabetic rats were not changed by daily intraperitoneal glibenclamide administration. The production of T3 (111 +/- 40 and 95 +/- 16 ng/g/30 min, mean +/- SD) and the conversion rate of T4 to T3 (11.1 +/- 2.9 and 10.2 +/- 2.3%) in the liver perfused with glibenclamide (200 and 400 ng/ml) were not significantly different from those in controls (109 +/- 41 ng/g/30 min and 12.8 +/- 5.4%). And those (120 +/- 33 and 99 +/- 19 ng/g/30 min, and 3.5 +/- 0.6 and 2.5 +/- 0.4%) in the kidney perfused with glibenclamide (200 and 400 ng/ml) were similar to those in controls (98 +/- 33 ng/g/30 min and 3.0 +/- 1.5%).

The present study was undertaken to elucidate the direct effect of thyrotropin (TSH) on the conversion of thyroxine (T4) to 3,5,3'-triiodothyronine (T3) in the isolated perfused rat liver. The liver was perfused without recirculation with a synthetic medium containing 10 micrograms/dl T4 and the effect of constant infusion of bovine TSH (125 or 250 microU/ml) on the conversion of T4 to T3 was examined. T4 uptake in the perfused liver was not changed by the addition of TSH. The release of T3 (10.3 +/- 1.4 ng/g/30min, mean +/- SD), tissue T3 production (99.5 +/- 21.4 ng/g/30min), net T3 production (102.6 +/- 20.2 ng/g/30min), and the conversion rate of T4 to T3 (14.8 +/- 3.5%) in the liver perfused with 250 microU/ml TSH were significantly higher than those in controls (8.1 +/- 1.2 ng/g/30min, 69.0 +/- 6.8 ng/g/30min, 69.9 +/- 6.1 ng/g/30min, and 10.0 +/- 0.8%), respectively. These results suggest that TSH may directly enhance hepatic conversion of T4 to T3 in rats in vitro.
