Women's health is the foundation of society's health and it can be achieved only by addressing all aspects of their health. The aim of this systematic review and meta-analysis is to investigate the prevalence of social health of Iranian women and related factors. The PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) was used for reporting; the terms "social health," "women," "Iran," and related keywords were searched in PubMed, Web of Science, Scopus, PsychoInfo, ProQuest and local databases, SID, Magiran, Irandoc, Elmnet, and Noormags up to August 2022. The published English or Persian quantitative primary studies which were conducted in Iran and reported social health or its dimensions among Iranian women were included. The studies were assessed by quality assessment tool for observational cohort and cross-sectional studies developed by National Heart, Lung, and Blood Institute. From the 786 studies retrieved, 22 studies were finally included surveying different groups of women. Age, marital status, education, employment, socioeconomic status, social support, social participation, social trust, social security, communication skills, and self-esteem were the factors affecting women's social health. Five studies entered meta-analysis and mean score of social health was 98.54 (95% CI: 87.56–109.51) and it was the lowest among women who were the head of households. Since women's social health has been less considered in the society and research studies, further policies, legislations, and capacity building in mentioned fields are necessary. As social health is an important aspect of health, it is necessary for governments to address the known determinants of women's social health in order to plan and promote the health of women, family, and finally society. Background: The present systematic review and meta-analysis were aimed to assess the current and lifetime prevalence of alcohol consumption among school and university students in Iran. Method: Online databases including Web of Science, PubMed, Scopus, Embase, and domestic scientific databases including SID, Magiran, IRANDOC, RICeST were searched systematically from inception to December 2019, using Persian and English languages. Following a random-effects meta-analysis, we estimated the pooled current and lifetime prevalence of alcohol consumption among school and university students in Iran. Heterogeneity was assessed with the Cochran Q statistic and quantified with the I 2 statistic. Results: Of the 319 identified records, 64 articles met the inclusion criteria. The students’ overall pooled prevalence of current and lifetime alcohol consumption was 11.7% (95% CI, 10.0–13.4) and 14.5% (95% CI, 12.1–16.9), respectively. Prevalence of current and lifetime alcohol consumption among school students were 15.2% (95% CI, 7.8–11.5), and 15.4% (95% CI, 12.5–18.4) respectively, prevalence of current and lifetime alcohol consumption in university students were 9.7% (95% CI, 7.8–11.5), 13.7% (95% CI, 10.3– 17.2) respectively. Conclusions: Based on findings, it seems necessary to implement and formulate primary and comprehensive prevention strategies and policies based on various cultures, regions, geographies, ages, and ethnicity. Introduction: The family physician programme (FPP) was implemented nearly two decades ago as a major health reform. Since the health system and FPP function in a rapidly changing social and economic environment, successful expansion of the programme requires a detailed analysis of its multiple major challenges, including the crucial aspect of its funding system. This systematic review aimed to assess the challenges in the FPP relative to its financing. Methods: All published articles related to the FPP in Iran were included in this study. In particular, original qualitative studies published in English or Persian from 2011 to 2021 were included. In January 2022, international credible scholarly databases and Persian databases were searched. All selected articles were carefully studied, and the data were extracted using the sample, phenomenon of interest, design, evaluation and research type technique. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses were used in preparing the study report. Results: Among 491 articles retrieved from the search strategy, 50 met the inclusion criteria after their titles and abstracts were screened. Twenty-nine studies were excluded after their full texts were reviewed. A total of 11 eligible empirical studies were finally included. Based on the results, six broad categories (budget and funding, insurance system, tariffs, payments, accountability and injustice) were identified as financial challenges. Conclusion: This study identified the challenges associated with financing among family physicians, and the results could provide guidance for policy-making in the expansion of the FPP. In July 2003, the Bellagio Study Group on Child Survival estimated that the lives of 6 million children could be saved each year if 23 proven interventions were universally available in the 42 countries re-sponsible for 90% of child deaths in 2000. The aim of this study was to determine frequency of important causes of mortality among under-5 year old children in hospitals, Tehran, Iran. Information about Mortality data of under-5 year old children from 16 hospitals in the West of Tehran was collected. The study period was conducted from 1 October 2005 to 30 March 2006. Educated health personnel in each hospital interviewed parents of children who died in hospital and filled out a checklist. 142 under-5 year old children died over the course of study, of whom, 118 (83%) were neonates (under 28 days-old), 53.5% had low birth weights (< 2500 Kg), 62 (43%) were girls and 80 (57%) boys. The most common cause of under-5 year death was due to certain conditions originating in the perinatal period (ICD-10: P00-P96) (68%). Congenital abnormalities (12%) and pneumonia (5%) were the second and third most common causes respectively. Among 28-day to one-year old children, the leading cause of death was pneumonia (27.3%), while for children being 1 to 5 years of age, this included pneumonia and chronic hepatitis (about 30%). Overall, the most common causes of death were disorders related to short gestation and low birth weight. Therefore, achievement of the millennium development goal of reducing child mortality by two-thirds from 1990 rate will depend on renewed efforts to prevent and control low birth weight, preterm delivery, pneumonia, and infectious diseases in our setting.

35. Bioactive Glass versus Autogenous Iliac Crest Bone Graft in Adolescent Idiopathic Scoliosis Surgery

Surgery on the skeleton frequently requires harvesting of autogenous bone graft from the pelvis, but this procedure often is complicated by problems. The purpose of this retrospective, comparative descriptive study was to compare the efficacy of metal-derived bioactive glass (Novabone) versus autogenous iliac crest bone graft in adolescent idiopathic scoliosis surgery. The study was carried out on forty cases (aged 14-20 years) with 55 total curves fused for AIS. Posterior spinal fusion was performed using local bone grafts combined with autogenous iliac crest bone graft in 20 patients (group 1), and combined with Novabone in another twenty ones (group 2). The patients were observed for a minimum of 24 months after surgery, with a mean postoperative observation time of 34.7 months. The results were assessed clinically and radiologically. In group 1, average preoperative curve was 66° with immediate correction to 24.2° (59.7%) and final follow-up of 27.4° (54.3%), but in group 2 the calculated numbers included 63.8°, 25.8° (59.6%) and 28.4° (55.5 %) respectively. There were 5 indeterminate fusions (3 cases in group 1 and 2 in the other group), 1 acute infection, and 1 hook dislodgement in the synthetic group. These results justify and favor the use of bone substitutes for instrumented posterior spinal fusion in AIS. Potentially hazardous harvesting of pelvic bone is no longer necessary for such operations.

Context: Nearly 20 years ago, the family physician program (FPP) was established in Iran in rural areas and was expanded to some cities about 10 years ago. Different studies reported different challenges associated with the efficacy of human resources and their training or educational program. Objectives: This systematic review of the literature was conducted to comprehensively identify and collate the challenges associated with the education of family physicians in Iran. Evidence Acquisition: All published articles related to FPP in Iran were the subject of this study. The eligibility criteria included original articles, case studies, and reports published in English or Persian during 2011 - 2021 related to the challenges in the training and educational system of FPP in Iran. Data were extracted based on the SPIDER (sample, phenomenon of interest, design, evaluation, research type) technique and were reported based on the structure of PRISMA (preferred reporting items for systematic reviews and meta-analyses). International credible scholarly databases were searched. The search strategy was defined based on the search syntax and keywords. Results: The search strategy yielded 5570 potentially relevant articles and reports, of which 70 studies met the inclusion criteria. Following the full-text review, 58 studies were excluded, and, finally, 11 empirical studies and 1 report were included in the review. The study attempted to identify challenges associated with the educational system; knowledge and awareness of the FPs of the nature, activities, policies, and protocols of the FPP; and also challenges associated with the competencies of the FPs in areas like social conditions, clinical competencies, public health, and managerial skills. Conclusions: The current study identified consistent themes associated with the educational system and challenges encountered in preparing physicians for FPP upon graduation. 39. Adherence of cardiologist physicians to the American heart association guideline in approach to risk factors of cardiovascular diseases: An experience from a teaching hospital

BACKGROUND: Cardiovascular disease (CVD) is the leading cause of death globally and has enormous costs for healthcare systems. This disease has a strong association with lifestyle behaviors. Therefore, applying reliable and effective strategies for prevention and treatment of CVD is important. In this study, we aimed to evaluate the adherence of cardiologist physicians to the American Heart Association (AHA) guideline for prevention of CVD.METHODS: Using a cross-sectional study, data were gathered for 208 patients using their medical records in the cardiology ward of a general teaching hospital. A physician systematically reviewed the medical records and completed the checklist in each domain. Adherence to the AHA guideline was evaluated in treating physician's choices and recommendations regarding these eight variables: hypertension (HTN), dietary intake, weight management, diabetes management, physical activity, blood lipid management, smoking, and aspirin prescription.RESULTS: Medical records of 208 patients (109 men and 99 women) with the mean age of 62 ± 14 years were reviewed. The frequency of CVDs was 5.3% for coronary heart failure (HF) and 67.8% for the acute coronary syndrome (ACS). Cardiovascular risk factors of patients were HTN (53.8%), diabetes (34.6%), hyperlipidemia (17.3%), smoking (17.8%), and obesity (31.7%). We found a proportion of 59%, 15%, and 26% for high, moderate, and low adherence to AHA guideline, respectively.CONCLUSION: Our study showed almost 60% high adherence to the AHA guideline by physicians in a teaching hospital. The most and the least adherence to the AHA guideline were for obesity and diabetes recommendations, respectively. More studies are needed to evaluate preventive guideline adherence in Iran. Establishing national preventive and therapeutic guidelines may increase the physicians' adherence to them.

The study aimed to investigate the prevalence of domestic violence (DV) in pregnant women and to explore relationships of self-esteem and social support (SS) relating to DV. Six hundred young mothers were investigated. 28.2% reported that they experienced some type of DV during pregnancy. The occurrence of physical abuse was not related to self-esteem or SS. When verbal violence was reported, satisfaction with SS was lower and self-esteem was lower by trend. Self-esteem was substantially lower when sexual violence was indicated. The number of people available for support did not provide a substantial buffering effect, but the perceived quality of SS did. Higher education in the mother and husband, and women's employment represented protective conditions against the occurrence of DV.
