Sonlu elemanlar analizi, lomber omurganın biyomekanik tepkisini incelemek için sıklıklakullanılan güçlü bir araçtır. Bu analiz için kullanılan anatomik ağlar genellikle sabittir,böylece morfometrik parametrelerin ayarlanmasını önler. Bu çalışmanın temel amacı,önceden belirlenmiş L4 L5 omurga seviyesinin morfometrik parametrelerinin bağımsızolarak ayarlanmasına izin verecek şekilde segmentli bir sonlu eleman modeli ve parametrikbir sonlu eleman modeli geliştirmektir. Yakınsama çalışmaları ile kapsamlı bir doğrulamaprosedürü uygulandı. Doğrulama ölçütlerimiz olarak, moment-dönüş eğrileri, intradiskalnükleer basınçlar ve faset eklem temas kuvvetleri bu iki modelden ve literatürden elde edilenverilerden toplanmış ve modellerimizi doğrulamak için birbirleriyle karşılaştırılmıştır.Segmentli sonlu elemanlar modelinin literatürle iyi bir uyum içinde olduğu gözlendi, bunedenle, ölçüm karşılaştırmalarımıza göre sonuçları geçerli kabul edildi. Parametrik sonluelemanlar modelinin, ani dönme eğrisi tahminleri ve intradiskal nükleer basınç tahminleriiçin segmentli sonlu elemanlar modeli ve literatür ile iyi bir uyum içinde olduğugörülmüştür. Finite element analysis is a powerful tool that is often used in order to study thebiomechanical response of the lumbar spine. Anatomical meshes that are used for suchanalysis are often fixed, hence preventing the tuning of morphometrical parameters. Theprimary objective of this study was to develop a segmented finite element model and aparametric finite element model that would allow the independent tuning of previouslydetermined morphometrical parameters of the L4 L5 spinal level. An extensive verificationprocedure was applied via convergence studies. As our validation metrics, moment-rotationcurves, intradiscal nuclear pressures, and facet joint contact forces were collected from theseboth models, as well as the data from the literature, and compared with each other in orderto validate our models. Segmented finite element model was observed to be in a goodagreement with the literature, therefore, validated for all of our validation metric predictions.The parametric finite element model, on the other hand, was observed to be in a goodagreement with the segmented finite element model, and with the literature, for its momentrotation curve predictions and intradiscal nuclear pressure predictions. Facet joint contactforces were observed to be overpredicted by the parametric finite element model. Futurestudy on the facet joint geometry and inclusion of the non-symmetric nature of the vertebraegeometry was proposed as a future study. 0

Bu çalışma kapsamında endoskopik cerrahide kullanılan kamera görüntüsü üzerine konumlandırılan navigasyon işaretçisinin, cerrahi performans üzerindeki etkisini daha iyi anlamak için endo-beyin cerrahisi eğitim programlarının gerekliliklerine uygun bir bilgisayar tabanlı simülasyon yazılımı geliştirilmiştir. Bu çalışma İdiopatik Parkinson Hastalığında STN'a veya GPİ'ya uygulanan DBS'in motor semptomlar, non-motor semptomlar ve denge üzerine etkilerinin değerlendirmek amacıyla gerçekleştirilmiştir.Çalışmada parkinson dışında herhangi bir nörolojik hastalığı olmayan STN DBS cerrahisi uygulanan 6 hasta ve GPİ DBS cerrahisi uygulanan 6 hasta olmak üzere toplam 12 hasta cerrahi öncesi, cerrahi sonrası 6. ay, 12.ay ve 24.ayda değerlendirilmiştir. Mobilite ve denge yeteneğini değerlendirmek için Süreli Kalk Yürü Testi, 12 m Yürüme Testi, Otur-Kalk Testi, posturografi uygulanmıştır. Emosyonel statüyü değerlendirmek için Hastane Anksiyete ve Depresyon Ölçeği (HAD) kullanılmıştır. Parkinson hastalığının klinik şiddetini belirlemek için Birleşik Parkinson Hastalığı Değerlendirme Ölçeği (BPHDÖ) kullanılmıştır. Parkinson hastalığının evresini belirlemede ise Hoehn&Yahr Parkinson Hastalığı Düzeyi Ölçeği, günlük yaşam aktivitelerinin (GYA) değerlendirilmesi için Schwab ve England Günlük Yaşam Aktiviteleri Değerlendirme Ölçeği kullanılmıştır.Çalışmanın sonucunda bilateral DBS cerrahisi Parkinson hastalarının fiziksel fonksiyon, emosyonel durum, günlük yaşam aktivite düzeylerinde gelişmeler sağlamaktadır. Kognitif fonksiyonlarda gelişme elde edilmemiştir. DBS cerrahisi hedef seçiminde, Parkinson hastalığında hastanın yaşam kalitesini belirleyen motor ve non-motor semptomların beraber göz önünde bulundurulması gerektiğini düşünmekteyiz. Bu seçimin aynı zamanda DBS'in amaçları (örn. The aim of this study was to descibe evaluation of effects of motor symptoms, non-motor symptoms and balance of STN DBS and GPİ DBS in the idiopathic parkinson's disease.Twelve patients without any other neurological was assesed at before, after sixth months, twelfth months and twentyfourth months after surgery. Six of this patients underwent STN DBS surgery and the other six patients GPI DBS surgery. Time up and go test, a 12 m walking test, chair stand test, posturography were used to asses mobility and balance ability. Hospital anxiety and depression scale (HAD) was applied to detect depressive symptomy and to describe anxiety score. Unified Parkinson's Disease Rating Scale (UPDRS) was applied to define severity of Parkinson's disease. To desribe the stage of Parkinson's disease, the Hoehn&Yahr Scale was also used. The Schwab and England Test was used to evaluate the daily living activities.The result of this study showed that DBS surgery suplied improvement in the physical function, emotional status and the daily living activities of the parkinson's disease.we didn't find improvement in the cognitive functions. We think that while we choose DBS surgery in Parkinson disease, we must to evaluate motor and non-motor symptoms of patients together which establish the patients daily living activity. Background: Shaving before cranial surgery has been a common practice for many decades. Most of the studies were about the infection rates, but there is a few data about the effect of shaving on patient mood. The aim of this study was to determine the effects of not shaving when performing a craniotomy on the mood of the patients. Methods: Patients who underwent a craniotomy between September 2010 and September 2011 and agreed to complete a questionnaire after the surgery were included in the study. Results: 207 patients included in the study. The mean age of the patients was 44.51 +/- 18.00 years. 114 patients (55.07%) did not think about shaving when they first learned they would undergo a craniotomy; 133 (64.2%) did not ask their surgeons about being shaved during the craniotomy. 30 patients (14.4%) pointed out that not being shaved was an important factor in choosing the surgeon. Nineteen (9.1%) patients complained about the wide scar tissue and alopecia. Conclusion: Not shaving is a strong factor that affects the time necessary to get back to one's routine and orientation to social life after craniotomy without increasing infection rates. However, it is not a factor that affects the patient's choice of doctors. Kraniyal ameliyatlardan önce hastanın saçlarının kesilmesi çok uzun yıllardan beri uygulanan genel bir uygulamadır. Bu konuda yapılan çalışmaların çoğu enfeksiyon oranı üzerinedir; ancak hastaların duygu durumlarına yönelik çok az çalışma bulunmaktadır. Bu çalışmanın amacı saç kesilmeden yapılan kraniyotomilerin hasta duygu durumu üzerine etkilerini araştırmaktır. Yöntem: Eylül 2010 ve Eylül 2011 tarihleri arasında kraniyotomi uygulanan ve cerrahi sonrası anket formunu doldurmayı kabul eden hastalar çalışmaya dahil edilmiştir. Bulgular: 207 hasta çalışmaya dahil edilmiştir. Hastaların ortalama yaşı 44.51±18.00 yıl idi. 114 hasta (55.07%) ilk olarak kraniyotomi uygulanacağını öğrendiğinde saç kesilmesi hakkında düşünmemişti; 133 (64.2%) kişi cerrahlarına kraniyotomi sırasında saçlarının kesilip kesilmeyeceğini sormamışdı. 30 hasta (14.4%) saç kesilmemesinin cerrahlarını seçmede önemli bir etken olduğunu belirtti. On dokuz (9.1%) hasta geniş skar dokusu ve alopesiden şikayetçiydi. Sonuç: Saç kesilmemesi kraniyotomiden sonra enfeksiyon oranlarını arttırmadan kişinin sosyal hayata oryantasyonunu ve rutin hayatına dönme süresini kısaltan önemli bir faktördür. Bununla beraber hastaların doktor seçimini etkileyen bir faktör değildir.

Arteriovenöz malformasyonlar (AVM) atardamarlar ile toplardamarların arada kılcal damar yatağı olmadan düzensiz olarak birbirine bağlandığı damar yumaklarına denir. AVM'lerin içerisinde sağlıklı kan akışı yoktur, dolayısıyla yüksek kanama riskiyle yaşamsal tehdit oluştururlar. Gelişim döneminde oluşmaya başladığı düşünülen AVM'lerin, moleküler patogenezleri hakkındaki bilgimiz oldukça kısıtlıdır. AVM gelişimindeki moleküler mekanizmaları gün ışığına çıkarmak ve etkin tedavi yaklaşımları geliştirebilmek için, beynin damarsal gelişimini moleküler düzeyde anlamak oldukça önemlidir. Ayrıca, yeni birçok önemli bulgu göstermektedir ki, kan damarları ve nöronlar sinir sistemini oluşturmak ve korumak için ortak sinyal ve mekanizmaları kullanmaktadır. Buna ek olarak, anjiogenezin ve anjiogenezde rol alan önemli faktörlerin merkezi sinir sisteminin nörodejenerasyondan korunmasında etkin olduğu birçok çalışmada gösterilmiştir. Bu nedenle, anjiogenezin nörodejeneratif mekanizmalardaki rolünü iyi anlayabilmek için beyin damarsal gelişiminin normlarını ve bu süreçte rol alan genleri bilmek gereklidir. Ancak, anjiogenez hakkındaki moleküler düzeydeki bilgimizin büyük kısmı patolojik çalışmalardan elde edilmiştir. Literatürde, beynin damarsal gelişiminin moleküler normlarını ele alan bütünsel bir çalışma bulunmamaktadır. Bu çalışma çerçevesinde, yolak-odaklı, düşük-yoğunluklu mikroarray sistemleri kullanılarak, literatürde ilk kez, beynin damarsal gelişimi sistemik olarak incelenmiştir. 113 adet anjiogenez ile ilgili genin ekspresyonu, zamansal ve birbiriyle kıyaslamalı olarak incelenmiş ve serebrovasküler gelişimde rol alan genler belirlenmiştir. Bai1, Nudt6, Ctgf gibi genlerin beyin gelişiminde rol aldığı ilk defa bu çalışma ile gösterilmiştir. Bulguların, çok kısıtlı olan beyin damarsal gelişimi araştırmalarına önemli katkıda bulunması ve AVM, nörodejenerasyon konularında gerçekleştirilecek işlevsel çalışmalar için referans oluşturması beklenmektedir. Bunun dışında, çok bakir bir alanda yapılan çalışmada alınan sonuçların ilerideki insan araştırmaları için de yol gösterici olacağı düşünülmektedir. Arteriovenous malformations (AVMs) are direct abnormal connections of arteries and draining veins without a capillary bed. AVMs, in which normal blood flow is interrupted, carry high risk of bleeding throughout life. They are thought to arise during development and there is very limited knowledge on the molecular pathogenesis of the AVMs. To unravel the molecular mechanisms and develop therapy modalities, detailed understanding of the brain vascular development at molecular level is crucial. Besides, today compelling number of evidence suggests that blood vessels and neurons use common signals and mechanisms to shape the nervous systems. Moreover, angiogenesis and some of its key modulators were recently shown to protect the nervous system from neurodegeneration. Thus, the knowledge of the norms of brain vascular development and the genes involved is essential for detailed understanding of the role of angiogenesis in neurodegenerative mechanisms. However, most of what we know about angiogenesis at molecular level is derived from pathological studies. In literature, there are no multi-gene level studies covering the norms of brain vascular development at molecular level. In this study, for the first-time in literature, brain vascular development was investigated using a pathway-specific, low-density microarray system in a systemic manner. Temporal and comparative expression of 113 angiogenesis-related genes was analyzed and genes that play a role in cerebrovascular development were determined. Genes like Bai1, Nudt6, Ctgf, were shown to be expressed in brain development for the first time in this study. The findings are expected to make important contributions to the limited research on brain vascular development and are expected to be an important reference for functional studies in AVM and neurodegeneration; they will also form the basis for further studies in humans. 103

ÖZETDOKU INHOMOJENITE YAKINLARINDA ÜÇ BOYUTLU GAMMAKNIFE DOZ DAGILIMININ NORMOKSIK JEL DOZIMETRE ILEINCELENMESIBu çalismadaki ana hedef Gamma Biçagi tedavisinde doku inhomojeniteleri yakinlarindaolusan doz dagilimini normoksik polimer jel fantomlar ile 3 boyutlu incelemektir. Jel fantomisinlanmasinin ardindan MR ile görüntülenmekte ve bu görüntülerin yeniden islenmesi ile hernoktadaki gerçek T2 degeri ile o noktaya verilen doz hesaplanmaktadir. Insan kafasinda yer alanyumusak doku-hava inhomojeniteleri yakinlarinda, elektron dengesi olusamamasi nedeniyle, tümkafa içi dokulari su esdegeri kabul eden tedavi planlama algoritmalari hatali doz dagilimiöngörebilmektedir. Bu problem fiziksel fantomlar ile simüle edildi. Birinci deneyde kulak içihava boslugu, diger deneyde ise sinüs hava boslugu yakinlarindaki bölgelere verilen dozlarin buhava bosluklarindan etkilenmeleri ve dagilimlari arastirilmistir. Kulak boslugunun çalisildigideneyde, cam balon içine hava bosluklarini temsil eden iki sise mantari uçlara yerlestirilmistir.Sinüslerin çalisildigi deneyde ise maksimal sinüslere benzesim yapacak boyutlarda bir mantar ilehomojen olmayan fantom olusturulmustur. Her iki deneyde de içi MAGIC polimer jel iledoldurulmus 16 cm çapindaki cam balonlar, homojen fantom olarak kullanilmistir. Her biri 100ml. hacimli plastik siselere doldurulmus jeller belirli dozlar ile isinlanarak doz-R2 kalibrasyonegrisi elde edilmistir. Deneylerde homojen ve homojen olmayan fantomlarda ayni koordinattakinoktala ra tek bir atislik doz verilmis ve olusan doz dagilim haritalari hesaplanmistir. Doz kalitekontrolü açisindan, Gamma Biçagi Planlama Sisteminin hesapladigi doz dagilimi ile deneyselsonuçlar karsilastirilmistir. Homojen fantom için gel dozimetrenin hesapladigi doz dagilimi ileGammaPlan'in hesapladigi doz dagilimi birbirleri ile uyum içindedir. Ancak homojen olmayanfantomda doz dagilimi uzaysal olarak farklidir ve degisik doz seviyeleri gözlemlenmistir.Yumusak doku-hava arayüzüne yakin bölgelerdeki dozun hizli azalmasi, bizleri GammaPlan'inbölgesel olarak fazla doz hesapladigi sonucuna götürmektedir. Kulak içi hava boslugu deneyindehomojen ve homojen olmayan fantomlarda olusan doz dagilimlarinda, aksiyel eksen Z=105 mmdüzleminde %50'lik izodoz egrilerinin çaplari X ekseni boyunca %35, Y ekseni boyunca % 1;yine koronal eksen Y=105 düzleminde ise X eksen boyunca %39 ve Z ekseni boyunca %5'lik farkgöstermistir. Paranosal Sinüs boslugu deneyinde ise homojen ve homojen olmayan fantomlardaZ=71, 75 ve 79 mm'lerde yer alan aksiyel düzlemlerde %50'lik izodoz egrilerinin X ve Y eksenleriboyuncaki çaplari sirasiyla %42 ve %47; %42 ve %60 ve de %52 ve %70'lik farklar göstermistir.GammaPlan'in bazi lezyonlarin arkasinda veya yakinlarindaki hava bosluklari yakininda olusan anidoz düsümünü hesaplayamamasi, gerçekte verilen dozun daha fazla olarak hesaplanmasina nedenolmaktadir. ABSTRACT3-D GAMMA KNIFE DOSE DISTRIBUTION BY NORMOXIC GELDOSIMETRY NEAR TISSUE INHOMOGENETIESThe primary goal in this study was to investigate the three dimensional dose distributions,near the areas of tissue inhomogeneities, in Gamma Knife Radiosurgery with the normoxicgel dosimetry. Following irradiation, when scanned in MR and post processing the MRimages, dose imparted to any particular point in the gel phantom can be calculated via thetrue T2 relaxation time at that point. In the neighborhood of air-tissue inhomogeneity inthe head, electronic disequilibrium can lead to errors in dose calculated with the treatmentplanning algorithms that presume the head as a homogeneous media. Two experimentswere designed to investigate the inhomogeneity effects in the Gamma Knife radiosurgery:one experiment simulating the volume near the auditory canal cavity and, the othersimulating the volume near the paranosal sinuses cavity. In the auditory canal cavityexperiment, an identical balloon of a diameter of 16 cm with two corks placed on each sideto represent the air cavities constitutes the inhomogeneous phantom. In the paranosalsinuses cavity experiment, a cylindrical cork is placed to represent the maximal sinuses. Inboth experiments, the homo geneous phantom is a spherical glass balloon filled withnormoxic polymer gel. For dose calibrations, 100 ml vials filled with the same gel areirradiated at predefined doses, and the R2 -dose calibration curve is extracted. Dosedistributions are the results of a single shot, by using all 201 Cobalt sources, delivered to aknown point in the phantoms. In the aspect of dosimetrical quality control, the GammaKnife planning system predicted dose distribution is compared with the experimentalresults. In the homogeneous phantoms, the gel dosimetry calculated dose distribution is ingood agreement with the GammaPlan predicted dose distribution. However, in the case ofinhomogeneous phantoms, the dose distribution is spatially different and significantdifferences in dose levels are observed. The dose decrease near the air-tissue interfacecauses the overshooting of the dose by the GammaPlan. This underdosing effect can beessential for the lesions near tissue inhomogeneities. In the auditory canal cavityexperiment, the diameter of the 50% isodose curves differ by 35% in the X axis and 1% inthe Y axis for Z=105 mm axial plane; and by 39% in the X axis and 5% in the Z axis forY=105 mm coronal plane in the inhomogeneous phantom as compared to thehomogeneous phantom. In the paranosal sinuses cavity experiment, the diameter of the50% isodose curves differ by 42% in the X axis and 47 %in the Y axis for Z=71mm axialplane; 42% in the X axis and 60%in the Y axis for Z=75 mm axial plane; 52% in the Xaxis and 70% in the Y axis for Z=79 mm axial plane respectively in the inhomogeneousphantom when compared to the homogeneous phantom. The dose decrease near the air-tissue interface causes the Gamma Plan?s predicted dosage to be higher than that actuallydelivered. ÖZET Selective amygdalo-hippocampectomy operasyonundan önce ve sonra Türk epilepsi hastalarında hayat kalitesi Nuran Aydemir Bu araştırmanın temel amacı, operasyondan önce ve sonra Türk epilepsi hastalarında hayat kalitesini değerlendirmektir. Bu amaçla araştırmada, yirmi operasyona girmeyi bekleyen epilepsi hastası, 21 operasyona girmiş epilepsi hastası ve 20 eşleştirilmiş kontrol katılımcısı kullanıldı. Katılımcılara, hayat kalitesindeki değişiklikleri belirlemek amacıyla hayat kalitesi, depresyon, durumluluk-genel anksiyete, epilepsinin etkisi, stigma, sözel hafıza ve açık uçlu sorulardan oluşan bir hayat kalitesi bataryası verildi. Sonuçlara göre; SF-36'nın tüm alt ölçeklerinde operasyon sonrası ve kontrol grubunun skorları daha yüksek olduğu halde, operasyon öncesi, sonrası ve kontrol grubu katılımcıları arasında hayat kalitesi açısından anlamlı bir fark elde edilmedi. Operasyon öncesi ve sonrası grup arasında da, sadece RE (duygusal problemlerden kaynaklana rol kısıtlanmaları) alt ölçeğinde anlamlı bir fark elde edildi. Sonuçlar ayrıca, operasyondan sonra geçen zamanın, operasyondan önce ve sonra geçirilen nöbet sayısının, kullanılan ilaç sayısının ve komorbit bir durumun hayat kalitesini etkilediğini ortaya koydu. Ayrıca, operasyon öncesi grupta anlık anksiyete ve epilepsinin etkisi skorları operasyon sonrası grubunkilerden anlamlı düzeyde yüksek çıktı. Sözel hafıza performansı da gruba ve lateralizasyona bağlı olarak anlamlı farklılıklar gözlendi. Son olarak sonuçlar, Türk epilepsi hastalarının `bağımsızlık` kavramını, operasyondan önce en çok kaygı yaratan durum, operasyondan sonra ise en büyük kazanç olarak ifade ettiklerini belirledi. Anahtar Kelimeler: Temporal lob epilepsisi, hayat kalitesi, amygdalo- hippocampectomy ABSTRACT The Quality of Life of Turkish Epilepsy Patients with Mesial Temporal Sclerosis before and after Selective Amygdalo-Hippocampectomy by Nuran Aydemir The main aim of the present study was to assess the quality of life of patients with epilepsy before and after surgery. For this purpose, twenty patients who were waiting for surgery (pre-op group), 21 patients who already had surgery (post-op group) and 20 matched control subjects were used. To determine the changes in quality of life, a quality of life battery which consisted quality of life, depression, state-trait anxiety, impact of epilepsy, stigma, verbal memory and open-ended questionnaires were administered to both pre-op, post-op and control subjects. The results of the present investigation showed that there was no significant difference among the three groups in terms of quality of life, although in all of the subscales of SF-36 post-op and control groups had better scores than the pre-op group. A significant difference between pre-op and post-op groups in terms of quality of life was observed only in RE (role limitations because of emotional problems) scores. The results also revealed that time after surgery, number of postoperative seizures, number of preoperative seizures, amount of medication and comorbity affects the quality of life of the patients. Additionally, the results demonstrated state-anxiety and impact of epilepsy scores were significantly higher in the pre-op subjects, the results indicated that the type of group, and lateralization make a significant difference in the verbal memory performance. Finally, the results showed Turkish epilepsy patients reported `independence` as their most important concern before, and their most important gain after surgery.
