Abstract

Background. The need to contain the spread of the SARS-CoV-2 pandemic has forced national and local organizations to define and implement targeted emergency response and management measures. As the knowledge about the infection grew, a wider range of organizational measures were deployed. Methods. This study involves the SARS-CoV-2 infected people managed by the Local Health Authority of Rieti (Italy). Diagnostic test waiting times and hospital admission rates in the Province of Rieti are investigated as the pandemic evolved. Trends were analyzed in relation to the tempora spreading of SARS-CoV-2, to the organizational actions taken by the Local Health Authority of Rieti, and to the deployment of actions across the territory. A municipalities classification of the province of Rieti was conducted after a cluster analysis based on the diagnostic test waiting times and the hospital admission rates. Results. Our findings show a declining trend, thus indicating a possible positive effect of the measures taken to contain the pandemic. The cluster analysis of the municipalities of the Province of Rieti makes evident an inhomogeneous geographical distribution of examined parameters (diagnostic test waiting times and the hospital admission rates), demonstrating the capability of Local Health Authority of Rieti to reach even the most disadvantaged areas and implying that the differences are due to the demographical variabilities. Conclusion. Despite some limitations, this study outlines the importance of management measures in response of the pandemic. These measures should adapt to social, cultural and geographical nature of the territory involved. Abstract

Background: Italy has one of the world’s oldest populations, and suffered one the highest death tolls from Coronavirus disease 2019 (COVID-19) worldwide. Older people with cardiovascular diseases (CVDs), and in particular hypertension, are at higher risk of hospitalization and death for COVID-19. Whether hypertensionmedicationsmay increase the risk for death in older COVID 19 inpatients at the highest risk for the disease is currently unknown. Methods: Data from 5,625 COVID-19 inpatients were manually extracted from medical charts from 61 hospitals across Italy. From the initial 5,625 patients, 3,179 were included in the study as they were either discharged or deceased at the time of the data analysis. Primary outcome was inpatient death or recovery. Mixed effects logistic regression models were adjusted for sex, age, and number of comorbidities, with a random effect for site. Results: A large proportion of participating inpatients were ≥65 years old (58%), male (68%), non-smokers (93%) with comorbidities (66%). Each additional comorbidity increased the risk of death by 35% [adjOR = 1.35 (1.2, 1.5) p < 0.001]. Use of ACE inhibitors, ARBs, beta-blockers or Ca-antagonists was not associated with significantly increased risk of death. There was a marginal negative association between ARB use and death, and a marginal positive association between diuretic use and death. Conclusions: This Italian nationwide observational study of COVID-19 inpatients, the majority of which ≥65 years old, indicates that there is a linear direct relationship between the number of comorbidities and the risk of death. Among CVDs, hypertension and pre-existing cardiomyopathy were significantly associated with risk of death. The use of hypertension medications reported to be safe in younger cohorts, do not contribute significantly to increased COVID-19 related deaths in an older population that suffered one of the highest death tolls worldwide.

Abstract

The aim of this contribution is to outline current re-search at CNR-IEIIT in the area of AI and health. Our research is focused on the development of methods to extract actionable knowledge from health-related data towards the development of novel digital systems to monitor, predict, and im-prove health. Examples of our recent research in-clude: eXplainable AI for patient monitoring using mobile and wearable devices; chronic disease pre-diction from biomarkers extracted from electronic medical records; unsupervised machine learning for patient segmentation and tailored intervention; and AI-based screening of hearing impairment and cognitive decline.

Abstract

Background: Clustered cases of Pseudomonas aeruginosa infection in immunocompromised patients' wards require rapid characterization of a potential epidemic to guide investigations and identify the potential source of contamination.Aim: To design and evaluate a rapid and simple typing method for P. aeruginosa in comparison to whole genome sequencing (WGS).Methods: A simplified polymerase chain reaction based on multiple-locus variable-number of tandem repeats analysis (MLVA) was designed and used to investigate cases of P. aeruginosa infection and colonization in a paediatric haematology department. The method was compared to WGS by using the Illumina method.Findings: On the 17 isolates recovered from 15 children (eight from blood cultures, three from urinary tract infections, one from sputum and five stool isolates), MLVA distinguished 10 different profiles, and seven isolates from six children shared the same profile. Analysis by WGS revealed that these seven isolates belonged to sequence type ST111 and serotype O12, allowing at least three different genotypes to be distinguished among them. Five environmental strains had three MLVA profiles; one was shared with a clinical isolate but WGS excluded any relationship.Conclusion: The simplified and inexpensive MLVA method enabled the exclusion, in less than 5 h, of most of the unrelated isolates and thus to focus investigations on a small number of cases, whereas WGS, taking several days of work, drew definitive conclusions concerning the outbreak and the genetic relationships of the ST111 isolates circulating in the department. We conclude that sequential use of both methods is the optimal strategy to investigate clustered cases of P. aeruginosa infections. [ABSTRACT FROM AUTHOR]

Abstract

Background: Beyond unambiguous advantages of mobile phones (MP), growing evidences document its problematic use and association with mental health issues. Unintentional injuries, such as road traffic or pedestrian accidents, result from dangerous use of the MP (DUMP). The aim of our study was to estimate the prevalence of DUMP and the addiction to MP in young while driving, cycling or walking (defined as DUMP) relating to some demographic and psychological factors. Methods: A cross-sectional prevalence study was carried out on a sample of 1130 Italian university students. The use and addiction to MP were studied with Italian version of the Mobile Phone Use Questionnaire (MPUQ) and Dependence test of MP (DTMP). The Cognitive Behavioral Assessment 2.0 (CBA 2.0) was used to investigate anxiety and obsessive-compulsive disorder. Results: Result statistics showed a continuous use of MP. One third of subjects showed MP addiction, associated to female and younger students. Two thirds showed risk of DUMP, associated to male and older. The obsessive-compulsive disorder was estimated only in a small fraction and significantly correlated with DTMP. Abstract

INTRODUCTION: Clostridium difficile infection (CDI) is currently considered the most common cause of health care-associated infections. The aim is to describe the trend of CDI in an Italian hospital and to assess the efficacy of the measures adopted to manage the burden. METHODS: we looked at CDI from 2016 to 2018. The incidence rate of CDIs was calculated as the number of new infected persons per month by the overall length of stay (incidence per 10,000 patient-days). Changes in the CDI rate during the period considered were analysed using a joinpoint regression model. RESULTS: thanks to the monitoring activity it was possible to adopt a new protocol, in order to manage CDI: the CDI episodes decreased from 85 in 2017 to 31 in 2018 (63% decrease). The joinpoint regression model was a useful tool to identify an important decrement during 2017, statistically significant (slope=-15.84; p= 0.012). CONCLUSIONS: reports based on routine laboratory data can accurately measure population burden of CDI with limited surveillance resources. Abstract

Background: The COVID-19 outbreak heavily attacked Italy, putting a strain for an extended time on the National healthcare system. Hospitals fastly rearranged the activity to cope with the crisis. This retrospective comparative study intended to investigate the impact of the lockdown imposed in Italy, in two different periods, during the COVID-19 outbreak on acute orthopedic trauma, in order to identify significant issues for improvement and future preparation. Materials and methods: We obtained data on total trauma access to a single University hospital DEA (Department of Emergency and Acceptance) in Rome during two periods of the COVID-19 pandemic lockdown in Italy: from March 9th, 2020 to May 4th, 2020(Phase 1), from May 10th, 2020 to June 30th, 2020(Phase 2) and then comparing them with the analogous period in 2019. We recorded demographic data; the characteristics of the lesion, including the anatomical area, fracture, sprain, dislocation, contusion, laceration, whether the lesion site was exposed or closed, where the trauma occurred and polytrauma. We also reported the waiting time in the emergency room and the mode of transport. Result: The study sample was composed of 1655 patients, 894 (54%) males and 761 (46%) females. The overall number of admissions in 2019 (pre-COVID-19 period) was 995; then it was 204 during Phase 1 and increased again to 456 during Phase 2. The average age of the Phase 1 group was 51.9 ± 24.8 years, significantly higher than that of the 2019 group (41.4 ± 25.7) and Phase 2 group (42.2 ± 22.5 years) (p=65 years) were the most commonly involved in the Phase 1 group, while in the pre-COVID-19 period and in Phase 2 they were middle-aged adults (15-44 years) (p

Abstract

International audience; Interlaboratory evaluations of Mucorales qPCR assays were developed to assess the reproducibility and performance of methods currently used. The participants comprised 12 laboratories from French university hospitals (nine of them participating in the Modimucor study) and 11 laboratories participating in the Fungal PCR Initiative. For panel 1, three sera were each spiked with DNA from three different species (Rhizomucor pusillus, Lichtheimia corymbifera, Rhizopus oryzae). For panel 2, six sera with three concentrations of R. pusillus and L. corymbifera (1, 10, and 100 genomes/ml) were prepared. Each panel included a blind negative-control serum. A form was distributed with each panel to collect results and required technical information, including DNA extraction method, sample volume used, DNA elution volume, qPCR method, qPCR template input volume, qPCR total reaction volume, qPCR platform, and qPCR reagents used. For panel 1, assessing 18 different protocols, qualitative results (positive or negative) were correct in 97% of cases (70/72). A very low interlaboratory variability in Cq values (SD = 1.89 cycles) were observed. For panel 2 assessing 26 different protocols, the detection rates were high (77-100%) for 5/6 of spiked serum. There was a significant association between the qPCR platform and performance. However, certain technical steps and optimal combinations of factors may also impact performance. The good reproducibility and performance demonstrated in this study support the use of Mucorales qPCR as part of the diagnostic strategy for mucormycosis.

Abstract

Territorial compensations of variability are a lever for smoothing the global photovoltaic production. They may allow a higher penetration rate of photovoltaic production in an energy mix, especially for non-interconnected islands. The aim of this paper is to analyse and optimize the smoothing effects of solar resource on a territory. In that view a novel typological classification-based methodology is proposed: intervals from highly intermittent episodes to ones with few fluctuations are discriminated. Then, the potential for compensation is demonstrated through the transformation from classes of high to low variability when summing solar radiation over several sites instead of one alone. The methodology is applied to Corsica. Solar radiation measured at 1 Hz on 8 sites which criss-crosses the island, when summed, demonstrates a reduction in the set of most intense variations compared to individual basis: they represent 5.5%–15.5% of the studied period for the sites taken individually against only 1.7% for the cumulated radiation. Optimization of the territorial compensation strategy leads to a near-disappearance of intense variations, with this class representing only 0.13% of the period. 39. Place Of Trichophyton Soudanense In The Trichophyton Rubrum Complex : A Clinical Isolates Analysis

Abstract

Objectives:Development of molecular laboratory tools has led to a complete revision of Arthrodermataceae classification and a consequent reduction of species number within this family. However, the phylogenetic place of Trichophyton soudanense remains uncertain inside the Trichophyton rubrum complex. Trichophyton soudanense is synonym either to T rubrum or T. violaceum according to the authors, while, more recently, De Hoog et al. gave it the status of u00ab borderline species u00bb[1]. T. soudanense remains one of the major etiological agents of tinea capitis in our hospital[2] and its epidemiological, clinical and phenotypic features led us to consider T. soudanense, T. rubrum and T. violaceum as separate entities. All studies that have been interested in this issue have only used collection strains.The main aim of our study was to determine the phylogenetic place of T. soudanense using clinical samples obtained in dermatology consultation at Saint Louis Hospital, Paris, France. Furthermore, this study aimed at evaluating the quality of identifications by phenotypic methods in our laboratory and to determine the discriminating power of different loci used in molecular identification of dermatophytes.[1] De Hoog et al., u00ab Toward a Novel Multilocus Phylogenetic Taxonomy for the Dermatophytes u00bb, 2017[2] Gits-Muselli et al., u00ab Continuous Increase of Trichophyton Tonsurans as a Cause of Tinea Capitis in the Urban Area of Paris, France: A 5-Year-Long Study u00bb, 2017Methods:We sequenced all the isolates obtained between April and September 2016 using four loci: ITS1-ITS4, u03b22-tubulin gene, u03b11 elongation factor gene and chitin synthase gene. Molecular identification of isolates was obtained by comparing ITS sequences to the Mycobanku00ae database. The sequences of the 4 loci were concatenated and a phylogenetic analysis was performed. In parallel, we photographed each culture on Sabouraud agar medium. The colors of the photographs were extracted and compared according to the RGB color model.Results:We analysed 122 clinical isolates phenotypically identified as T. rubrum (n=48), Trichophyton mentagrophytes/interdigitale (n=34), T. soudanense (n=10), Trichophyton tonsurans (n=22), Trichophyton sp. (n=1), Microsporum audouinii (n=6), and Microsporum canis (n=1). The concordance of phenotypic and molecular identifications was 94% with no error regarding tinea capitis cases. One isolate was identified as Chrysosporium queenslandicum which has not been yet described in human pathology. The color coding of the isolates objectified the pleomorphism of the dermatophytes (Figure 1). The phylogenetic organization of the genus Trichophyton was identical to that obtained by De Hoog et al. and isolates of T. soudanense appear to be separated from those of T. rubrum. The isolates identified as T. soudanense took an intermediate place between T. rubrum and T. violaceum when comparison of ITS sequences alone (Figure 2) but appear closer to T. rubrum with the other loci. The majority of isolates identified as T. interdigitale or T. mentagrophytes present identical ITS sequences, probably corresponding to the species T. interdigitale.Conclusion:The sequence differences between T. rubrum and T. soudanense remain tenuous whereas they are easily differentiated on a clinical presentation. The comparison of their transcriptome could help to understand the origin of their differences.

Byline: P. Poggi, M. Muselli, C. Cristofari This paper investigates the potentiality of Wind Electric Conversion Systems (WECS) integrated into the islander electrical grid of Corsica. Probabilistic models for WECS and their use in the reliability studies of combined wind electric and conventional generation systems are discussed. The method, based on the evaluation of the Loss of Load Probability (LOLP), uses a Markov model applied to the output power of 'classical' units and WECS, and to the hourly electrical energy demand. Then the margin state (the difference between grid total capacity state and load state) and its associated probability are computed to allow determination of the LOLP. So, the limit of the WECS power that can be supported by the grid is estimated. This study has been realised for the total thermal capacity installed in the grid and various ranges of WECS. Furthermore, we study the energetic balance on the grid with this wind farm and the benefit regarding gas emissions induced by this system.

This platform consists in a PV array coupled to H2 Chain (electrolyser – gas tanks – fuel cell) and electricity converters to regulate the injection on the electrical grid of Corsica.The different indicators considered in the comparison of the different simulations are on the one hand linked to energy balance in the system and on the other hand statistical indicators (RMBE, rRMSE, and CC) of each subsystem.The main result shown in this paper is that none of the prediction models is clearly distinguishable from others on the basis of those indicators. Best results are obtained with the hybrid model, which has a lower rRMSE than persistence model of approximately 15%.

Abstract

27th European Photovoltaic Solar Energy Conference and Exhibition; 4249-4253, As many other governments of sub-Saharan Africa, the Republic of Djibouti has focused its energy policy onto renewable power supply utilities. With a 30% electrification rate, a lot of Djiboutian people don’t access energy easily. Thereby, PV systems could be used for supplying the dispatched remote populations across the country. If sizing these systems requires estimating the amount of solar energy reaching the surface, the authors think that ambient air temperature is also a very important critical environment parameter. Indeed, this parameter acts fundamentally into photovoltaic conversion as well as into battery chemical reactions and, so, interacts with the 2 most important parts of an off-grid PV system by altering its efficiency and lifetime. This study presents the method we developed to take into consideration this parameter for sizing future off-grid PV systems in Djibouti. Ambient temperature was retrieved from 15 minutes Land Surface Temperature (LST) free accessible data, deriving from Meteosat 9 satellite images and developed by the Land Surface Analysis (LSA), a Satellite Application Facility (SAF) of the European Organization for the Exploitation of Meteorological Satellites (EUMETSAT). Different methods have been developed for retrieving air temperature data from LST. However, the number of available in situ weather stations and the need of night-time air temperature data incited us to develop and train a 3- layer feed-forward pattern associator neural network. The input variables were the LST pixel value, the related error value, the Solar Zenith Angle (SZA) and the year fraction. The output target was the ambient temperature retrieved by the different local temporary weather stations we have deployed in the country during the year 2010. Furthermore, in order to overcome the significant lack of LST data and the restricted number of available ground weather stations, we chose to estimate, in the first instance, monthly averages of ambient air temperature daily profile from nominal LST data, and smooth the possible extreme values in post processing. In order to assess performance of the complete process, we used statistical indices Root Mean Squared Error (RMSE) and Correlation Coefficient (CC). Final results show good robustness of the procedure, which could be generalized to other arid African developing countries.

Abstract

International audience; Systems based on renewable energies coupled to an hydrogen storage are original solutions for the electrical supplying of sites not very or not at all electrified. The H2/O2 battery, association of an electrolyzer, a fuel cell and gas tanks, could be an interesting solution to satisfy long periods of autonomy (typically seasonal storage). But its implementation is not so easy and the operation particularities due to involved electrochemical processes forced us to associate a secondary storage (typically a lead-acid battery) to this H2/O2 battery. This paper presents the reflections about electrical architectures achieved in the framework of the PEPITE French project whose objective was to supply a stand-alone technical site (meteorological pylon) from a photovoltaic generator and a H2/O2 battery.
