Prolonged air leak (PAL) represents the most commonly encountered complication following pulmonary resection. This review aims to show the role of autologous blood pleurodesis (ABPP) in the management of PAL. A search was conducted on the Web of Science, PubMed, MEDLINE, and Google Scholar for English-language articles until September 15, 2023, with titles containing the related phrase “autologous blood patch pleurodesis (ABPP), and prolonged air leaks.” Included studies comprised four randomised controlled trials, ten cohort studies, a case series, and nine case reports from various countries. Patients undergoing ABPP had a mean age of 52.7 years. Autologous blood volumes for pleurodesis varied (50 mL to 250 mL). Approximately 73.8% of cases responded to ABPP in 1 to 30 days (mean 3.75 days), and in-hospital complications were infrequent (3.9%). Follow-up durations post pleurodesis were up to 20 months. The evidence presented in this systematic review supports the role of ABPP as an effective and safe intervention for managing PAL, particularly after pulmonary resection.

Introduction Sarcoidosis involving the thyroid gland is an exceedingly rare condition characterized by non-necrotizing and non-caseating granuloma. This study represents a case of thyroid sarcoidosis in an elderly female.Case report A 62-year-old female presented with anterior neck swelling associated with difficulties in breathing and swallowing for six months. Physical examination revealed an enlarged, firm, and non-tender thyroid gland. Thyroid function tests were normal, with a high level of angiotensin-converting enzyme. A neck ultrasound revealed nodules in both thyroid lobes and bilateral cervical lymphadenopathies. The patient underwent a total thyroidectomy and lymph node excision. Histopathological examination of the specimen showed features suggestive of thyroid sarcoidosis.Conclusion Sarcoidosis involving the thyroid gland is a rare finding, and its diagnosis necessitates the exclusion of other potential explanations for similar symptoms. Surgery becomes necessary when there are compression symptoms or a risk of malignancy.

Spindle epithelial tumor with thymus-like differentiation (SETTLE) is a rare benign lesion characterized by the presence of spindle and epithelioid cells and features resembling thymic tissue. A 35-year-old asymptomatic female with a history of two cesarean sections and a positive family history of breast cancer underwent a thyroid checkup. Clinical examination revealed a normal thyroid gland, while diagnostic tests indicated a solid hypoechoic nodule in the right lower parathyroid gland, which was confirmed as SETTLE after surgical resection was performed, leading to an uneventful postoperative period. The patient, discharged on the first postoperative day, underwent comprehensive follow-up and oncology referral for risk stratification. In the literature review of nine cases of SETTLE (seven male, two female), age (8–70 years), past medical and surgical history of the majority is not available, most of them presented with painless and slowly enlarging mass in the neck, on laboratory examination most of the cases had test results within the normal range, ultrasound (U/S) and computed tomography (CT) Scan are preferred imaging modality, most of the patient had undergone surgical resection, histopathological examination showed SETTLE after management more than half were disease free. SETTLE is a rare tumor with ∼50 reported cases worldwide and a single case report that involves parathyroid glands. It can be asymptomatic or present as a mass. Besides the necessity of surgical resection, long-term follow-up is mandatory.

Introduction Hemangiomas are benign vascular lesions. The complicated hemangiomas that are causing significant functional or cosmetic problems need surgical excision. This paper presents a novel technique of reconstruction using surgical mesh after removal of a facial hemangioma.Case report A 50-year-old male patient presented with right cheek swelling for more than 40 years causing disproportion of the face. A flap was raised by a nasolabial linear incision, then a 4*2 cm surgical mesh was put in, and the wound closed cosmetically. The excised tissue was sent for histopathological examinations and the results confirmed hemangioma. After more than two years follow up, the patient’s face was symmetrical with normal facial contour.Conclusion The application of surgical mesh for patients who seek functional and cosmetic improvements after hemangioma removal is a constructive method that gives the desired outcome for the patient.

Abstract Introduction To date, only a limited number of case reports have documented the co-occurrence of PNS and melanocytic nevus in the medical literature. This study aims to report an exceptionally rare case of posterior chest wall PNS in conjunction with a melanocytic nevus. Case presentation A 46-year-old female presented with a long-standing black lesion on her left upper posterior chest wall, that had become painful in the two months prior to presentation. There was a painful, dark blue, non-erythematous, and non-tender nodule on the left upper posterior chest wall. Based on the patient’s desire for cosmetic purposes, the lesion was excised totally with primary closure under local anaesthesia. Histopathological examination revealed intradermal melanocytic nevus with inflamed pilonidal sinus. Discussion The rarity of posterior chest wall PNS associated with nevi poses unique diagnostic and therapeutic challenges for clinicians. The distinct anatomical location, different from the conventional region, and the rare association between the two conditions may delay accurate diagnosis and result in mismanagement or inappropriate interventions. Conclusion The posterior chest wall PNS is another type of atypical PNS that is extremely rare. The association between PNS and blue nevus is a fascinating medical finding that deserves further investigation.

Machine learning (ML) and deep learning (DL) techniques have demonstrated significant potential in the development of effective intrusion detection systems. This study presents a systematic review of the utilization of ML, DL, optimization algorithms, and datasets in intrusion detection research from 2018 to 2023. We devised a comprehensive search strategy to identify relevant studies from scientific databases. After screening 393 papers meeting the inclusion criteria, we extracted and analyzed key information using bibliometric analysis techniques. The findings reveal increasing publication trends in this research domain and identify frequently used algorithms, with convolutional neural networks, support vector machines, decision trees, and genetic algorithms emerging as the top methods. The review also discusses the challenges and limitations of current techniques, providing a structured synthesis of the state-of-the-art to guide future intrusion detection research.

Lingual osteoma, a rare, benign bone tumor that primarily affects the posterior tongue, can be difficult to diagnose. This study aims to report a case of osteoma affecting the tongue in a 17-year-old female. The patient had a foreign body sensation and a progressively growing lesion for 3 years and underwent clinical examination and diagnostic procedures. A well-defined, smooth-surfaced, white mass was discovered in the posterior third of the tongue. The 1.5 × 1 × 0.4 cm mass was completely excised under local anesthesia and histopathologically confirmed as a benign lingual osteoma. The 2-month post-operative outcome was uneventful. The rarity of lingual osteoma, as well as the fact that it is often asymptomatic, makes diagnosis difficult. The diagnosis entails a proper clinical examination, imaging studies, and histopathological analysis. Surgical intervention, primarily aimed at complete excision while preserving tongue function, remains the primary treatment option. Successful excision entails educating healthcare professionals about this rare benign bony tumor to ensure the best possible patient outcomes.

Objective The incidence of thyroglossal duct diseases in the general population is about 7%. We aimed to demonstrate the clinical presentations and management of thyroglossal duct diseases. Methods We conducted a retrospective review of all patients who underwent surgery for histopathologically confirmed thyroglossal duct cyst, sinus, or fistula at a single center. Results A total of 151 cases were included in this study. There were more female patients (87, 58%) than male patients (64, 42%). The patients’ ages ranged from 1 to 63 years old. The most prevalent complaint was painless upper midline neck swelling (93.3%). Most cases were diagnosed as thyroglossal duct cysts (137, 90.7%). Six cases (4%) were associated with carcinoma. All the cases were managed using the modified Sistrunk procedure. There were no procedure-related complications, and five cases of recurrence. Conclusions Although thyroglossal duct cyst is the most common neck anomaly in children, it may also present with various characteristics later in life. This condition can be managed successfully without complications and with a low recurrence rate.

Objective This study was performed to report and analyze the prevalence of permanent facial nerve paralysis following parotidectomy for various benign and malignant lesions in a single center. Methods This single-center retrospective study included all patients who underwent parotidectomy (total and superficial) for benign and malignant tumors and chronic inflammatory diseases during a 6-year period. Patients who had previously undergone an operation of the parotid gland and those with preoperative facial weakness were excluded. Results The study included 127 patients ranging in age from 14 to 83 years (median, 45.89 years). Most patients were female (n = 83, 65.4%). The most prevalent procedure was superficial parotidectomy (n = 117, 92.1%), followed by total parotidectomy (n = 6, 4.7%). The average operative duration was 138 minutes (range, 80–400 minutes). Histopathology revealed that 109 (85.8%) patients had benign tumors, 14 (11.0%) had malignant tumors, and 4 (3.1%) had chronic sialadenitis. Only two patients sustained an injury to the cervical branch of the facial nerve. Conclusion In this single-center experience of parotid surgery, the rates of transient and permanent facial paralysis were acceptably low at 9.0% and 1.6%, respectively, for all pathologies.

Follicular thyroid carcinoma (FTC) is a well-differentiated malignancy, while insular thyroid carcinoma (ITC) is a very rare and poorly differentiated tumor. The present study reports a case of ITC arising within the background of FTC. A 52-year-old housewife presented with an anterior neck swelling for a duration of 6 months. An ultrasound examination revealed a well-defined solid nodule measuring ~58x37x28 mm in the mid-lower third of the right thyroid lobe, and two small nodules of <3 mm in size in the left lobe. Fine needle aspiration cytology of the lesion led to the suspicion of follicular neoplasm. A right thyroid lobectomy was performed, and the histopathological examination revealed poorly differentiated insular carcinoma arising in the background of FTC. As a result of this diagnosis, a completion thyroidectomy was performed. ITC is a rare and highly aggressive thyroid malignancy that is morphologically and biologically regarded as an intermediate between fully differentiated and undifferentiated/anaplastic thyroid carcinomas. In comparison to patients solely with follicular carcinoma, patients with insular carcinoma are older, more often male, have larger tumor sizes, are at greater risk of metastasis, and have a lower survival rate. Poorly differentiated ITC with follicular carcinoma is a very rare and aggressive condition that could be managed by thyroidectomy followed by radioiodine therapy. [ABSTRACT FROM AUTHOR]

Intraparotid gland neurofibroma is a rare benign tumor that arises from Schwann cells of the facial nerve within the parotid gland. This case report discusses a 41-year-old woman who experienced a painless preauricular swelling on her right side for over 5 years. Clinical examination and ultrasound revealed a well-defined mass in the parotid gland. The patient underwent total mass excision, resulting in transient facial nerve dysfunction but complete recovery. These tumors often manifest as solitary masses in the parotid region and may compress nearby structures, causing facial paralysis or numbness. Their diagnosis can be challenging due to similarities with other parotid gland tumors and possible associations with neurofibromatosis. Smartphones have become an important Internet platform and play an essential role in individuals' daily lives, and their use is gaining popularity due to their various features. The study's main objective was to investigate the potential of smartphones as tools for improving English writing skills. The proliferation of Education has facilitated the broad adoption of mobile language learning, prompting this research to examine the growing inclination of undergraduate students to acquire English language skills via the use of Smartphones. The quantitative method was used to collect data by conducting a questionnaire to undergraduate students, and the survey included the recruitment of 52 individuals, with ages ranging from 18 to 23; among all those who took part, there were 41 females and 11 males. The results indicated that using smartphones positively impacted various aspects of English language proficiency, such as listening, reading, writing, grammar, vocabulary, pronunciation, and communication. The findings show that the respondents acknowledged the benefits of employing social media and smartphones to enhance their writing abilities by using smartphones, and this pattern is anticipated to persist in the forthcoming period. Copper -graphite composites are widely used in a great number of engineering applicationssuch as brushes, switches, sliding bearings, self-lubricating bearings, etc. due to their goodthermal and electrical conductivity and excellent tribological properties as compared with otherstructural materials. There are ongoing attempts in manufacturing copper composites with betterproperties to enhance their efficiency and increase their effective life. Present research aims to prepare hybrid 95wt.% copper –5wt.% graphite composites reinforced with yttria and tin particles by powder metallurgy technique and to study their effects on mechanical and physical properties of the prepared composites. Powder mixture was mixed by ball mill mixer at 100rpm for 120min with (5/1) balls to powder ratio. The powder mixture was cold pressed at 700MPa for 30sec, followed by sintering at 900 ?C for one hour. In the first stage, Yttria(Y2O3) was added with (2, 4, 6, 8, 10) wt% to pure copper (Cu) and to (95%Cu-5%Gr) matrices. Typical composite of this stage was ((95%Cu-5%Gr)-4%Y2O3. In the second stage, tin (Sn) was added with (2, 4, 6, 8, 10) wt% to pure copper and((95%Cu-5%Gr)-4%Y2O3 matrices. Typical composite of this stage was ((95%Cu-5%Gr)-4%Y2O3)-6%Sn. The results showed that hardness and true porosity of the composites increases with increasing yttria content. On the other hand both thermal and electrical conductivity of the composites decreases with increasing yttria content. It was also found that (95 wt.% Cu- 5 wt.% Gr) – Y2O3 composites have always lower wear rate than plain Cu-Y2O3 composites.

There are controversies regarding the management of umbilical pilonidal sinuses. The current study aims to report on the efficacy of a non‐operative, umbilical conserving procedure in the treatment of umbilical pilonidal sinus. This is a prospective, single cohort study. The cases were managed in a single private practice center. Patients were assessed and managed throughout the previous nine years (from January 2013 to June 2022). The required information was obtained from the center's medical database. The current study included 114 patients. There were 82 (71.9%) male patients. The patients' ages varied from 14 to 56 years (mean = 23.24). The umbilicus was retracted under local anesthesia. The sinuses were cleaned with povidone‐iodine. Following the drying of the cavity, the mixture was put in the umbilicus, and the area was dressed. The amount of mixture was determined by the size of the cavity. Following treatment, the patients were directly discharged home with instructions to remove all hair from the chest and abdomen and keep the dressing dry for three days. After three days, the patients were advised to use a clean cotton swab to remove the injected mixture. Recurrence was reported in 5 cases (4.4%). The current technique might be used effectively in the treatment of umbilical pilonidal sinus. It is an umbilical preserving technique with a minimal recurrence rate. [ABSTRACT FROM AUTHOR]

Introduction: There are multiple management modalities for idiopathic granulomatous mastitis, but the treatment of choice is still under debate. This study aims to evaluate the diagnosis and outcomes of different management modalities in patients with idiopathic granulomatous mastitis and to identify the risk factors associated with recurrence. Method: This is a single-group cohort study that included those patients who had idiopathic granulomatous mastitis. Ultrasonography was conducted for all of the cases using LOGIQ E9 with an ML6-15 transducer (5–15 MHz). A core needle biopsy was conducted to take samples from the cases for histopathological examination. The patients were put on steroid therapy. Whenever the cases did not respond to the steroid therapy, treatment with a combination of low-dose steroids and methotrexate was started. In the lack of response to conservative treatments, surgical interventions were started. Results: Sixty-three cases with a confirmed histopathological diagnosis of granulomatous mastitis were included. The mean age of patients was 35.7 years. The history of more than one childbirth was positive in a large portion of the cases (82.5%). The lesion side was unilateral in 58.7% of the cases. A large proportion of the lesions were classified as BIRADS category 2. The best treatment outcome was yielded by a combination of low-dose steroids and incision and drainage. The factors of age, lesion area (cm2), skin thickening, and white blood cell count enhanced the chance of recurrence. Conclusion: Incision and drainage in combination with a low dose of steroids can give an acceptable outcome with a low rate of recurrence. [ABSTRACT FROM AUTHOR]

The choice between nodulectomy and lobectomy for managing thyroid nodules is a subject of debate in the field of thyroid surgery. The present study aims to share the experience of a single center in managing solitary thyroid nodules through nodulectomy from January 2023 to October 2023. The inclusion criteria encompassed symptomatic or suspicious solitary nodules and medically necessitated cases. The extracted data included patient demographics, medical history, symptoms, diagnostic details, surgery indication, procedure outcome and histopathological findings. The follow-up included clinic visits and phone calls. The mean age of the patients was 36.64±11.63 years, with 85.0% females and 15.0% males. Predominantly, patients were housewives (58.5%). Neck swelling (62.3%) was the most common presentation. Ultrasound examination revealed mixed nodules in more than half of the cases (54.7%). Right nodulectomy was performed in 26 cases (49.1%) and left nodulectomy in 23 (43.4%), and four cases (7.5%) underwent isthmusectomy. The mean operation time was 36.04±9.37 min and no drainage tube was used in any of the cases. One case (1.9%) of seroma was the only observed complication during the observational period. Nodulectomy may be a suitable choice for managing benign, large, solitary thyroid nodules, small suspicious nodules or microcarcinomas. [ABSTRACT FROM AUTHOR]

In the absence of a definitive global therapy for coronavirus, COVID-19, disease, the Health Ministry of Kurdistan Region of Iraq decided to use convalescent plasma for coronavirus patients. The decision came into effect in the city of Sulaymaniyah in late March and continues to this date, and the health and administrative officials describe the procedure as a success. This paper studies convalescent plasma for people with COVID-19, and highlights its use in Kurdistan Region. Thyroglossal duct cysts (TGDCs) are unusual remnants of the thyroid gland that are rarely observed in association with carcinoma. The present study aimed to showcase the clinical characteristics, diagnosis and management of patients with TGDC carcinoma. It was a single-center study conducted from February 2016 to February 2020. The study involved the retrospective analysis of a series of cases with TGDC carcinoma. A total of 10 patients were included in the study, of whom eight (80%) were females. Their age ranged from 25 to 48 years with a mean age of 39.1 years. A total of five cases (50%) underwent only Sistrunk's procedure, four patients (40%) underwent total thyroidectomy along with Sistrunk's procedure and one patient (10%) was treated by Sistrunk operation and lobectomy. In all of the cases (100%), histopathological examination confirmed papillary thyroid carcinoma in TGDCs. In addition, in the thyroid tissue, 2 cases (20%) also had papillary thyroid carcinoma and 3 cases (30%) had papillary thyroid microcarcinoma. Radioiodine was administered in one patient (10%) with suppressive thyroxine. A three-year follow-up with ultrasound revealed no suspicious lesions in any of the cases. Although rare, carcinoma may develop in the thyroglossal cysts. In this situation, both the thyroid gland and different lymph node compartments should be evaluated for malignancy. Matrix metalloproteinase9, a member of the MMP family with activities in collagenase and gelatinase, is widely known to be highly expressed on OSCC’s invasive front and associated with OSCC’s progression and aggression. MMP-9 is involved in many biological processes due to its proteolytic cleavage activity in the extracellular environment. The aim of this study was to evaluate serum and saliva levels of this enzyme in patients with oral squamous cell carcinoma as well as to investigate whether it can be used as a biomarker for detection of oral squamous cell carcinoma. By using an MMP9 ELISA kit, the serum and saliva levels of Human MMP9 in 20 patients with oral squamous cell carcinoma (preoperatively) and 20 patients with OSCC (post-operatively) were measured. We also measured serum and saliva MMP9 levels of 20 healthy volunteers enlisted as normal controls. The results of present study showed Mean serum level of MMP-9 in OC patients preoperatively was higher than either that reporetd in OC patient postoperatively or in control group with a highly significant differences. In contrast, there was no significant difference in this level between OC patients postoperatively and control. Pre- and postoperative samples of saliva showed comparable mean of MMP-9 with no significant difference, while they both significantly higher than that of control. Serum and saliva levels are expressed at significantly high levels in oral squamous cell carcinoma. This report aims to present a case of thyroid hydatid disease with literature review. A 48-year-old female presented with painless anterior neck mass of about 2 year duration. There was an ill-defined, central anterior neck mass, with a smooth surface and mobile with deglutition. Ultrasound of the thyroid gland revealed an enlarged left lobe of thyroid gland due to well defined thick wall cystic nodule. The patient underwent left thyroid lobectomy under general anesthesia, the pathology report revealed hydatid cyst of thyroid gland. Liver and lungs are often the end destination for hydatid cysts, while other places like mediastinum, diaphragm, cardiac, smooth and skeletal muscles, abdominal and chest walls are rarely involved. In this case, the cyst affected even a rarer organ which was the left lobe of thyroid gland. Most of the time the disease is asymptomatic and is found accidentally, yet depending on the site and size of the cysts, symptoms can occur. Hydatid disease of thyroid gland is an extremely rare condition. The main presenting symptom is swelling. Operation under general anesthesia is the only modality of treatment. [ABSTRACT FROM AUTHOR]

Granulomatous mastitis (GM) is a rare inflammatory disorder that infrequently occurs with synchronous breast carcinoma. The present study reports the case of a patient who was initially diagnosed with recurrent GM, which eventually proved to be masking an underlying ductal carcinoma in situ (DCIS). A 30-year-old female presented with left breast pain. On clinical examination, there was a large, palpable and painful lump in the left breast, with axillary lymphadenopathy. Initially, the diagnosis was GM and conservative treatment was applied. Surgical resection was decided upon for the condition after it became recurrent, and the histopathological examination revealed extensive DCIS with GM. Later on, the patient underwent a mastectomy with an axillary sentinel lymph node biopsy. The postoperative follow-up was uneventful. In conclusion, tissue diagnosis has a key role in detecting DCIS masked by GM, especially in young females who are not undergoing regular mammogram screening. The present study shows the challenge that the specialists in this field may face when dealing with recurrent GM of the breast, and warns them to search for a second pathology such as the DCIS presented in the current case. Background: The purpose of this paper is to identify the effect of assisted training using branched-chain amino acids supplementation (BCAA) in developing the special strength and accuracy of offensive skills in volleyball for youth. Methods: The researchers used the experimental method in the style of the two groups (control and the experimental) due to its suitability to the research problem. The players of Al-Muqdadiya Sports Club represented the research sample, ages (18-19) years, who numbered (15) players and they were divided into two cases and control groups and for each group (6) players and (3) players, the reconnaissance experiment was conducted on them. The curriculum was applied to them for a period of two months, at the rate of (3) training units per week, where pre and post-tests were conducted, then data were collected and processed statistically with the (SPSS) program, and the results obtained Results: There are significant differences in favor of the experimental group and for all special strength tests at the expense of the control group. Conclusion: The supplement (BCAA) had a positive effect on improving the level of physical performance in volleyball, and the physical exercises had a positive role in the development of the accuracy of the performance of the players by using them as an aid in the training units. Despite its rarity, pilonidal sinus (PNS) in atypical locations poses significant diagnostic challenges, underscoring the need for early identification and appropriate treatment strategies. This case highlights a rare occurrence of a PNS in the axilla, emphasizing the importance of recognizing uncommon presentations of common ailments. A 27-year-old male presented with a 13-year history of painless axillary discharge, diagnosed with PNS based on clinical evaluation. Surgical excision under local anesthesia successfully treated the condition, showcasing the effectiveness of tailored management in addressing rare presentations of PNS. Acute pancreatitis is an inflammatory disease that can affect both the peripancreatic tissues and distant organs. There are few reports of acute pancreatitis caused by endourological procedures. However, an obstructing ureteral calculus causing acute pancreatitis is very rare., Case Presentation: A 36-year-old male patient presented with epigastric, and right loin pain with decreased urine output for 3 days. On physical examination, he had central abdominal and right flank tenderness. An abdominal ultrasound showed mild to moderate ascites, a hyperechoic pancreas, a small (26×77 mm) left kidney with increased echogenicity, right renal hypertrophy with moderate hydronephrosis, and a dilated upper ureter due to a 10 mm obstructing stone with a perirenal fluid collection. The diagnosis of acute pancreatitis with an obstructing right upper ureteric stone was established.
