2. EP-069 - TRAGÉDIA EVITÁVEL: DESAFIOS NO MANEJO DE UM CASO DE TÉTANO ACIDENTAL NO INTERIOR DE MINAS GERAIS

Introdução: O tétano acidental é uma doença infecciosa aguda prevenível que está associada à elevada morbi-mortalidade. Objetivo: Descrever as dificuldades enfrentadas no manejo de um paciente com tétano acidental em um hospital de pequeno porte do interior de Minas Gerais. Método: Relato de caso. Esse relato foi feito através da revisão de prontuário eletrônico, após assinatura do TCLE. Resultados: Paciente masculino, 63 anos, natural e procedente de Itajubá-MG, deu entrada em pronto atendimento com história de perfuração em região plantar do pé direito por prego enferrujado há 5 dias. Na admissão, apresentou-se com trismo e nucalgia. Referia nunca ter sido vacinado. Apesar de sinais vitais estáveis foi admitido na UTI, pelo diagnóstico de tétano acidental. Após 4 horas da admissão apresentou crises espasmódicas generalizadas, evoluindo com paralisia diafragmática e parada cardiorrespiratória em assistolia por hipóxia. Após RCP e o retorno à circulação espontânea foi submetido à intubação endotraqueal, recebendo sedoanalgesia com Propofol e Fentanil. A avaliação neurológica demonstrou hipertonia e abalos musculares generalizados, a tomografia de crânio não apresentava alterações. Após 12 horas da admissão, foi administrado soro antitetânico: 20.000 UI EV, vacina antitetânica e metronidazol EV. Decorridas 36 horas após admissão na UTI, foi realizada traqueostomia e desbridamento da ferida com drenagem de abscesso do pé direito. Evoluiu com sintomas de disautonomia, caracterizado por labilidade pressórica e após 15 dias da admissão, começou a apresentar melhora dos espasmos musculares em uso de Diazepam, Midazolam, Fentanil e Rocurônio. Durante a internação, o paciente evoluiu com lesão renal aguda e necessidade de hemodiálise. Além disso, o paciente apresentou diversas Infecções Relacionadas à Assistência à Saúde (IRAS), como pneumonia associada à ventilação mecânica por Acinetobacter baumannii resistente a carbapenêmico e infecção de corrente sanguínea. Após 60 dias de internação na UTI, paciente evoluiu para óbito como consequência de choque séptico refratário. Esse foi o segundo caso de tétano acidental notificado em Itajubá no último ano. Conclusão: Apesar de ser uma doença prevenível, devido à baixa cobertura vacinal, novos casos de tétano acidental têm sido notificados na cidade de Itajubá. A escassez de recursos e a falta de capacitação da equipe de saúde dificulta o manejo dessa condição nas áreas mais remotas do país.

Introdução: A monkeypox (MPOX) é uma doença causada por um Poxvírus zoonótico que pertence ao gênero Orthopoxvirus e é transmitida por via respiratória ou dérmica. Suas principais manifestações estão relacionadas a erupções, febre e lesões genitais, com o diagnóstico através da história clínica, exame físico e para a confirmação, um swab de secreção de vesícula ou crosta de erupção cutânea. Não há um tratamento específico para a doença, porém alguns antivirais usados na varíola foram empregados, como o Tecovirimat (TPOXX®), Cidofovir (Vistide®) e Brincidofovir (Tembexa®). Objetivo: Apresentar uma revisão descritiva da incidência de MPOX em âmbito nacional no período de maio de 2022 até agosto de 2023. Método: Estudo descritivo e de série temporal da incidência de monkeypox, realizado por meio da análise de 24 boletins epidemiológicos divulgados pelo Ministério da Saúde. Os dados foram obtidos no sítio eletrônico do referido órgão. As variáveis analisadas incluíram a região de residência, faixa etária, estados, municípios, sexo de nascimento, raça/cor, sintomas, imunossupressão, tipo de amostra para análise laboratorial, exames diagnósticos realizados, gestantes, óbitos e hospitalizações. Resultados: Os resultados demonstraram a predominância dos casos em homens, especialmente aqueles que se identificaram como homossexuais, em comparação com um número muito menor de mulheres com a mesma identificação. A faixa etária mais afetada foi entre os 30 a 39 anos. Os sintomas mais comuns incluíram erupções cutâneas, febre, lesões genitais e cefaleia. Houve maior concentração de casos na Região Sudeste do Brasil, com destaque para o estado de São Paulo. O uso de swab das secreções de vesículas e crostas de erupções cutâneas foi predominante para as análises laboratoriais. Foi observada uma alta prevalência de casos em pacientes que vivem com HIV, principalmente entre homens jovens. Um total de 16 óbitos por MPOX ocorreram no Brasil, com a maioria no Rio de Janeiro. Conclusão: A pesquisa fornece uma análise abrangente da situação da Monkeypox no Brasil, destacando os principais aspectos epidemiológicos e desafios relacionados ao controle da doença.

Introdução: A dengue é uma arbovirose transmitida pela picada do mosquito Aedes aegypti. As manifestações clínicas incluem febre de início abrupto, astenia, cefaléia, artralgia, dor retro-ocular, dor abdominal, êmese e hemorragias. O diagnóstico é realizado por meio de avaliação clínica e exames laboratoriais. Objetivo: Apresentar o perfil epidemiológico de pacientes com diagnóstico confirmado de dengue no estado de Minas Gerais nas 18 primeiras semanas epidemiológicas do ano de 2024. Método: Este trabalho foi realizado a partir do levantamento de dados das 18 semanas epidemiológicas do ano de 2024 por meio do painel on-line de vigilância das arboviroses. Este painel é uma iniciativa da Coordenação Estadual de Vigilância das Arboviroses, e foi elaborado em parceria com as diretorias de Informações Epidemiológicas e de Vigilância de Condições Crônicas da Secretaria de Saúde do Estado de Minas Gerais, tendo como objetivo a divulgação pública dos casos, óbitos e incidência de dengue, chikungunya e zika no estado de Minas Gerais. Resultados: Existem 1.241.639 casos prováveis de dengue no Estado. Do total de 566.590 casos confirmados, 70.329 (12,41%) apresentavam algum tipo de comorbidade, com destaque para hipertensão e diabetes, sendo as faixas etárias mais afetadas a de 50 a 59 anos (população feminina) e 60 a 69 anos (população masculina). A maioria dos casos confirmados, 496.261 (87,59%) não apresentava comorbidades, com a faixa etária predominante de 20 a 29 anos em ambos os sexos. Do total de 518.007 de resultados laboratoriais liberados, 171.916 tiveram exames positivos e 333.095 negativos (positividade de 33,2). Dos exames, 48.029 foram realizados por biologia molecular (positividade de 17,4%), 34.373 IgM (positividade de 45,5%) e 3.497 IgG (44,5% de positividade). Foram confirmados 333 óbitos e existem 775 em investigação. Destes óbitos, 13,30% eram mulheres e 11,82% de homens, ambos na faixa etária de 70 a 79 anos. A letalidade foi de 3,57%, com 9.322 casos graves ou com sinais de alarme. Conclusão: O estudo fornece uma análise ampla do cenário da dengue no estado de Minas Gerais nas primeiras 18 semanas epidemiológicas de 2024, confirmando uma alta incidência de casos confirmados e óbitos na população idosa. Medidas de controle e prevenção do agravo, associadas à incorporação da vacina contra a dengue, capacitação aos profissionais de saúde e atividades de educação em saúde são capazes de contribuir para a redução da hospitalização, incidência e óbitos pela doença.

Desde 2017, temos apresentado, por meio de dados quantitativos, as várias dimensões do trabalho editorial, como número de submissões recebidas, distribuição geográfica dos autores e das autoras e quantidade de pareceres emitidos. Mais recentemente, buscamos incorporar a dimensão de gênero nos dados. O objetivo geral é prestar informações à comunidade da revista, manter um histórico de seus principais marcos e assegurar ampla transparência dos nossos processos editoriais, sempre com vistas ao aperfeiçoamento do trabalho editorial como um todo e à elevação da qualidade dos artigos publicados.

Abstract This paper aims to explain the Brazilian Central Bank’s (BCB) decision to create and operate a new instant payment system: PIX. Understanding that “digital financialization” can take different forms, this study sheds light on the factors that led to the BCB’s choice to launch a “state-owned Fintech”. The analysis considers three complementary factors. Firstly, the BCB’s institutional trajectory has made it protective of its perimeter of power. Secondly, the ideas shared by transnational networks support the active role of central banks in managing instant payment issues. Thirdly, the interests of traditional banks were not significantly impacted to the extent that they would oppose the creation of PIX. Through examining BCB documents and secondary literature, this study provides a first-hand account of the motivations and circumstances that led to the creation of PIX in Brazil.

15. Implementation of the Novel Duo‐Grid in GFDL's FV3 Dynamical Core

Abstract The gnomonic cubed‐sphere grid has excellent accuracy and uniformity, but the “kink” in the coordinates at the cube edges in the halo region can leave an imprint of the cube in the solution, and requires special edge handling. To reduce grid imprinting, we implement the novel “Duo‐Grid” within the Geophysical Fluid Dynamics Laboratory's (GFDL) Finite‐Volume Cubed‐Sphere Dynamical Core (FV3). The Duo‐Grid remaps a cube face's data from neighboring face from kinked to natural locations along great circle lines using 1D piecewise linear interpolation. A 2D interpolation algorithm is used to fill correct data at the eight corners of the cubed‐sphere needed for FV3's 2D advection scheme. The Duo‐Grid was tested in idealized tests using the 2D shallow water solver and the 3D hydrostatic and non‐hydrostatic solvers. We found that error norms are greatly reduced and grid imprinting is practically eliminated when employing the Duo‐Grid. These results indicate that FV3's accuracy and robustness have improved.

16. Interpreting taboo : the evaluation of practices by public service interpreters in the transfer of taboo language and the implications for professional training

- Abstract
This research aims to evaluate public service interpreter practices in relation to interpreting taboo, and to examine how far these practices should be reflected in their professional training. Currently, when healthcare professionals or patients turn to family members or friends to interpret for them, dire consequences have been noted, indicating the necessity of having trained interpreters. The original contribution is that as far as the author's knowledge goes, no one has carried out an evaluation of PSI and taboo across all types of taboo and across many cultures. Thanks to globalisation, there are interpreters who belong to different cultures but live in the same country and train together, which means it would be useful to know how this training needs to be tailored to simultaneously influence all cultures. The methodology used is a mixed type. Questionnaires were combined with open-ended, semi-structured interviews that allow the respondents to answer freely from their own frame of reference rather than being confined by the structure of pre-arranged questions. The sample size was chosen purposively (NRPSI Register). Descriptive, thematic analysis was followed. The theoretical framework used in the study included Nida's (2012), Newmark's (1998) and Bassnett-McGuire's (1991) functional (dynamic) equivalence, because this theory looks at the impact of texts on the audience as well as the transference of the spirit of the text. Reiss & Vermeer's Skopos theory (2015) relates to reflecting the intentions of speakers when interpreting, which is relevant as it is the intentions behind taboo that must be conveyed. For example, malicious swearing is different in intent compared to habitual swearing that carries no malice. Additionally, Pragmatics, specifically conversation analysis such as Austin's Speech Acts (2011) and Grice's Maxims of Communication (1975) are used since understanding them will lead to a more effective oral communication and interpreting. Although many interpreters state they would interpret taboo, the reality does not always match this statement and the question is, how much does the reality match the literature regarding such matters? This thesis studied this aspect and found that there were varying levels of convergence between literature and practice. From the interpreting strategies' aspect, the questionnaire results matched the top choice (equivalence) but there was a poor match with the assumed lowest theory (omission) where the study results placed it in second place. Literal translation ranked third in the questionnaire contrary to the literature where only a few advocate its use. The responses relating to the role of interpreters overall matched the literature, where the figures showed that linguistic and cultural communicators ranked highest, followed by mediators, educators and linguistic communicators only. Finally, part-time interpreters found there was a need for training compared to their full-time counterparts. Those who underwent PSI training found it had helped them when faced with taboo. Training requirements listed by respondents included terminology, strategies for interpreting taboo and analytical skills. All those listed were supported and matched by the literature. The thesis benefits readers from not just the specific findings listed, but from finding a structured piece of research on taboo in the context of public service interpreting. It opens the door to knowledge transfer into translation, for example, or cultural knowledge regarding taboo, where interpreters belonging to a culture can learn how other cultures deal with taboo, and finally, it may pave the way into considerations when new forms of taboo that were not addressed in the thesis appear, such as hate speech.

Características centrais do atual sistema financeiro brasileiro foram forjadas ao longo da implementação de reformas bancárias, entre 1994 e 2002. Apesar de sua importância, não se tem ainda um balanço apropriado das reformas dos anos 1990. Estudos existentes se contradizem em suas descrições e conclusões sobre as motivações, o papel do Estado e as consequências das reformas. Este artigo apresenta uma revisão abrangente dessa literatura e indica suas principais lacunas. Além disso, argumenta que as reformas dos anos 1990 estabeleceram novas fundações institucionais para a atividade bancária, sua regulação e, em especial, para a relação entre Estado e bancos no país. A pesquisa apresenta a política da mudança institucional, descrevendo as motivações em disputa e o andamento da implementação das reformas, que culminaram em novas formas de organização da propriedade, de produção e compartilhamento de informações, de regulação e de mecanismos para casos de falhas de participantes. A pesquisa baseia-se em análise qualitativa de conteúdo de documentos do período.

18. Multiple same-level and telescoping nesting in GFDL's dynamical core

Two-way multiple same-level and telescoping grid nesting capabilities are implemented in the Geophysical Fluid Dynamics Laboratory (GFDL)'s Finite-Volume Cubed-Sphere Dynamical Core (FV3). Simulations are performed within GFDL's System for High-resolution modeling for Earth-to-Local Domains (SHiELD) using global and regional multiple nest configurations. Results show that multiple same-level and multi-level telescoping nests were able to capture various weather events in greater details by resolving smaller-scale flow structures. Two-way updates do not introduce numerical errors in their corresponding parent grids where the nests are located. The cases of Hurricane Laura's landfall and an atmospheric river in California were found to be more intense with increased levels of telescoping nesting. All nested grids run concurrently, and adding additional nests with computer cores to a setup does not degrade the computational performance nor increase the simulation run time if the cores are optimally distributed among the grids.

19. Evaluation of Retinal and Posterior Segment Vascular Changes Due to Systemic Hypoxia Using Optical Coherence Tomography Angiography.

- Abstract
Background/Objectives: Retinal vascular occlusions are a significant cause of visual impairment in older adults, resulting in ischemic retinal damage and sudden vision loss. This study evaluates the retinal, optic nerve head (ONH), and choroidal capillary networks in chronic and acute-on-chronic hypoxia compared to normal controls using optical coherence tomography angiography (OCT-A). Methods: We evaluated a prospective study including twenty patients in the hypoxic group (mean age 61.2 ± 10.2) in two phases, chronic hypoxia and acute-on-chronic hypoxia, and 21 control subjects (mean age 59 ± 9.4 years). All patients underwent a comprehensive eye examination, OCT, and OCT-A imaging. The data were analyzed using OCT-A analysis software (Zeiss OCT-A software 2.1.0.55513) and Fiji software (1.51a). Vascular density of the retina and ONH, choriocapillaries, and foveal avascular zone (FAZ) size were measured. Results: The superficial peripapillary vascular density was higher for the control group (0.387 ± 0.03) compared to the hypoxic patients with (0.383 ± 0.03) and without O2 supplementation (0.383 ± 0.03; p = 0.018). No retinal angiographic differences were identified between the two study groups. The ganglion cell layer (GCL) was thinner in the hypoxic group. Both hypoxic subgroups demonstrated denser choriocapillaries (mean 13,073 ± 1812 and 12,689 ± 1815, with and without O2 supplementation, respectively) compared to the control group (mean 9749 ± 2881, p < 0.001 for both groups). Hypoxic patients demonstrated increased area size of choriocapillaries (+O2 supplementation—mean 44,347 ± 10,563; −O2 supplementation—mean 46,984 ± 12,822) compared to the control group (mean 30,979 ± 9635; p < 0.01 and p < 0.001, respectively). Conclusions: Chronic and acute-on-chronic hypoxia did not affect the retinal vascular network, most probably due to the strong autoregulation of vascular function of the retina. However, compared to the control group, GCL, ONH vasculature density, and most choriocapillaries indices were significantly altered among hypoxic patients. [ABSTRACT FROM AUTHOR]

This research explores how Northumbrian Water Ltd. (NWL), a UK private sector organization providing water supply and sewerage services, selects nonprofit organization (NPO) partners who contribute to addressing its material sustainability issues. The aim of this research is to provide a framework of partner selection for NWL and other businesses in the water industry who seek to address material sustainability issues through cross-sector (NPO-business) partnerships. There is limited research on how businesses should select NPO partners from an impact or value creation perspective. Moreover, systematc models of materiality analysis are rarely found in the literature. THE GRI is providing guidance on materiality analysis through practice-based standards, but systematic research into organizational practices is limited. NWL engages in a variety of strategic key partnerships which make it a suitable choice of case study for investigating the partner selection process. The study particularly examines how partnerships can generate impact on material sustainability issues, and how this prospective impact can be evaluated. Template analysis is used to interpret transcripts of semi-structured interviews and a focus group involving key informants from NWL and its NPO partners. The contribution to knowledge is established around the need for having both flexible/reactive/project-based and systematic/proactive/strategic approaches to partner selection while ensuring shared value co-creation. The study further highlights how materiality with respect to sustainability issues can be assessed both reactively and proactively from NWL's perspective, and how this assessment can better inform the partner selection process. Future research avenues can involve investigating the applicability of the partner selection framework resulting from this study to other businesses in the water industry.

Within just a few months, the coronavirus disease 2019 (COVID-19) pandemic managed to bring to the foreground the conversation that infection prevention and control (IPC) experts have been pushing for decades regarding the control of the spread of infections. Implementing the basics of IPC has been a challenge for all affected countries battling with an exponential COVID-19 curve of infection. Preventing nosocomial transmission of the disease has been difficult in highly resourced and stable contexts, but even more so in the conflict context of the Middle East. COVID-19 has added further challenges to the long list of existing ones, hindering the implementation of the optimal IPC measures that are necessary to break the chain of infection of both respiratory and non-respiratory infections in those settings. This paper outlines and gives examples of the challenges faced across the Middle East conflict setting and serves as a call for action for IPC to be prioritized, given the resources needed, and fed with contextualized evidence.

Abstract Background One of the most effective public health interventions in the world is immunization. However, some parents doubt its usefulness and safety. Many factors influence their decision to vaccinate, including their sociodemographic characteristics, their trust in the public health system, the parent-physician relationship, their level of knowledge and their attitudes towards vaccination. Our objective was to determine the factors, especially the parent-physician communication, associated with parental knowledge, attitudes and practices of their children’s vaccination. Methods Three thousand five hundred parents (father, mother or both) of children aged between 1 month and 15 years were approached by a trained personnel who performed the data collection through personal interviews (February–April 2019). Results The response rate was 79.5%. The results of the multivariable analysis showed that a better patient-physician communication was significantly associated with higher knowledge, better attitude and practice. Better knowledge was significantly associated with better attitude, whereas better knowledge and attitude were significantly associated with better practice. Conclusion Our study shows the importance of good physician-patient communication in improving knowledge, attitude and practice of parents towards their children’s vaccination.

Appropriate partner selection is a crucial concern for businesses who want to address material sustainability issues through cross-sector partnerships. However, the current research on materiality analysis and partner selection is still in the nascent stage of development. This paper contributes to understanding partner selection by corporates through an in-depth exploration of the business's selection of nonprofit organisation (NPO) partners, based on case study research of Northumbrian Water Ltd. (NWL), a UK private sector organisation providing water supply and sewerage services. Using the evidence from this study, we propose a framework for the selection of partners based on the prospective impact of cross-sector partnerships on material sustainability issues. Our paper also provides a useful toolkit for businesses to evaluate these prospective impacts. The framework demonstrates how materiality with respect to sustainability issues can be assessed both reactively and proactively, and how this assessment can better inform the partner selection process by corporates. This understanding of how materiality is assessed and partners are selected in practice is supported by a context specific exemplar and contributes to knowledge and practice of materiality analysis as well as cross-sector partnerships.
