10. Hypertrophie bénigne de la prostate : une communauté digitale pour identifier les attentes des patients.

Abstract

L'objectif de cette étude qualitative était d'explorer l'impact de l'HBP dans la vie quotidienne des patients et des aidants et d'identifier leurs attentes et leurs besoins à partir du contenu des discussions d'une communauté digitale de patients et d'aidants. Une plateforme digitale conçue comme un forum de discussion sécurisé incluant trois espaces (patients, aidants, patients et aidants) a été créée. Un guide d'animation servait à animer la communauté, faciliter l'expression des membres autour de différentes thématiques, encadrer les sujets de discussion. Le contenu des discussions était analysé selon les deux objectifs de l'étude. Du 16 au 29 novembre 2020, 13 patients souffrant d'HBP âgés de 50 à 76 ans et 10 aidants (conjointes exclusivement) ont échangé leurs points de vue à travers plus de 50 discussions et 500 commentaires. L'HBP restait un sujet dont les patients ne parlaient pas en dehors du cercle familial, alors même que le bénéfice moral du partage avec des personnes rencontrant le même problème était mis en avant. Pour les patients, l'HBP impactait l'image qu'ils avaient d'eux-mêmes, freinait leur spontanéité au quotidien, et engendrait de la gêne, voire de la honte, dans certaines situations. Patients et conjointes soulignaient le manque d'information et de communication autour de l'HBP. Outre la démonstration de l'impact de l'HBP sur l'image que les patients ont d'eux-mêmes et sur leur qualité de vie, cette étude met en évidence leurs attentes en termes d'échanges, de communication et d'information autour de cette pathologie. The present qualitative study was to explore the impact of benign prostatic hyperplasia (BPH) in the daily life of patients and family caregivers and to identify their expectations and unmet needs based on comments on discussions of a digital community of patients and caregivers. A digital platform designed as a secure discussion forum including three spaces (patients, caregivers, and patients and caregivers) was created. A moderation guide was used to animate the community, facilitate the expression of its members around different themes, and frame the discussion topics. The content of the discussion was analyzed qualitatively according to the two study objectives. From 16 to 29 November 2020, 13 BPH patients aged 50-76 and 10 caregivers (only spouses) shared their views through more than 50 discussions and 500 comments. BPH remained a topic that patients did not talk about outside the family circle, although the moral benefit of sharing with other people suffering from the same condition was emphasized. For patients, BPH significant changed their self-image, inhibited their spontaneity in everyday life, and caused embarrassment (even shame) in certain situations. Patients and spouses emphasized the lack of information and communication about BPH. In addition to demonstrating the impact of BPH on patients' self-image and quality of life, this study highlights their expectations in terms of exchanges, communication, and information about this pathology. Abstract

Torsion of the spermatic cord is responsible for an ischemic necrosis of the testis in the absence of rapid resaturation of the perfusion. It is also responsible for contra-lateral parenchymal alterations resulting in subfertility. Torsion of the spermatic cord mostly occurs in newborns and adolescents. Its classical clinical pattern is that of an acute-onset ipsilateral scrotal pain. Its diagnosis is clinical and its suspicion must lead to an immediate surgical scrotal exploration. The surgical procedure may be preceded by an attempt of external detorsion, but never delayed by any para clinical exploration. If the diagnosis is confirmed at surgery, contra-lateral orchidopexy must be considered. Trophicity of the ischemic testicle and the fertility of the patient are uncertain. Improvement of the prognosis of patients with torsion of the spermatic cord needs an urgent surgical management of the patients. It also claims for the development of testicular parenchyma protective medical treatments. But the most important improvement should come from parental and teenagers educational information so that they urgently visit a practitioner in case of acute scrotal pain. [Copyright &y& Elsevier]

Abstract

STUDY DESIGN:&EMSP;:Prospective monocentric follow-up study. OBJECTIVES:&EMSP;:To assess long-term functional and urodynamic results of augmentation enterocystoplasty in spinal cord injury (SCI) patients with detrusor hyperreflexia and reflex incontinence who failed to respond to conservative treatment. SETTINGS:&EMSP;:Department of Urology (Pitié-Salpétrière Hospital, Paris, France), Department of Neurological Rehabilitation (Raymond Poincaré Hospital, Garches, France), Assistance Publique – Hopitaux de Paris, University Paris VI and V, France. METHODS:&EMSP;:Prospective study of 17 SCI patients (four above T6 level, nine between T6 and T12, and four below T12) with history of refractory urge incontinence to pharmacotherapy. Partial cystectomy (subtrigonal for 15) was performed with Hautmann enterocystoplasty (15) or detubularized clam cystoplasty (two). RESULTS:&EMSP;:Mean follow-up was 6.3±3.8 years (range 1.25–10.5 years). Fifteen of 17 (88.5%) patients were completely continent under self clean intermittent catheterization (CIC) (mean 4.6/day, range 4–7). The remaining two patients with pudendal nerve denervation had persistent stress urinary incontinence. No operative complications were noted. Long-term complications included recurrent pyelonephritis for one patient. Maximal cystometric capacity increased from 174.1±103.9 to 508.1±215.8 ml (P<0.05). Maximal end filling pressure decreased from 65.5±50.2 to 18.3±7.9 cm H2O (P<0.05). CONCLUSION:&EMSP;:Urodynamic evaluation and clinical assessment demonstrate long-term success of augmentation enterocystoplasty in an homogeneous population (SCI) without delayed complications in SCI patients.Spinal Cord (2000) 38, 490–494. Abstract Background To assess the efficacy of salvage radiotherapy (RT) for persistent or rising PSA after radical prostatectomy and to determine prognostic factors identifying patients who may benefit from salvage RT. Methods Between 1990 and 2003, 59 patients underwent RT for PSA recurrence after radical prostatectomy. Patients received a median of 66 Gy to the prostate bed with 3D or 2D RT. The main end point was biochemical failure after salvage RT, defined as an increase of the serum PSA value >0.2 ng/ml confirmed by a second elevation. Results Median follow-up was 38 months. The 3-year and 5-year bDFS rates were 56.1% and 41.2% respectively. According to multivariate analysis, only preRT PSA ≥1 ng/ml was associated with biochemical relapse. Conclusion When delivered early, RT is an effective treatment after radical prostatectomy. Only preRT PSA ≥1 ng/ml predicted relapse.
