Abstract

Background: Head and neck cancers (HNC) present diagnostic challenges due to multifocal disease manifestations, posing difficulties in distinguishing between metastatic disease and second primary malignancies (SPM). This complexity underscores the need for advanced diagnostic approaches. Emerging technologies, such as next‐generation sequencing (NGS) and molecular classifier assays, show promise in providing precise insights into the diverse etiologies of HNC. Method: In this article, we employed NGS and molecular classifier assays to delve into three distinct clinical cases. The objective was to showcase the instrumental role of these technologies in facilitating accurate diagnoses and differentiating between metastatic disease and SPM in HNC cases. Results: The results of this series highlight the effectiveness of NGS and molecular classifier assays in enhancing diagnostic accuracy for HNC and contributing to the precise differentiation of disease etiologies. The utilization of these advanced technologies proved instrumental in avoiding unnecessary interventions and paved the way for more targeted and effective treatment strategies. Conclusion: Our findings underscore the necessity of incorporating advanced molecular testing technologies into the diagnostic and therapeutic approaches for HNC, thereby championing a more nuanced and effective approach to managing these complex cases. 11. Intervention work : an ethnography of 'NEET to EET' transitions

Abstract

The thesis examines the practices through which professionals organise interventions with young people who are not in education, employment or training - so-called NEETs. The thesis aims to contribute to a literature on ethnomethodological studies of work by offering a praxiological re-specification of how professionals organise and account for these interventions with cohorts of NEETs. By drawing from ethnographic fieldwork with four groups of professionals and practitioners, the thesis develops the notion of 'intervention work' to describe the practical knowledge, skills and methods that are used to organise transitioning young people 'from NEET to EET'. The analytic chapters present ethnographic materials drawn from participant and nonparticipant observation in 'perspicuous settings' where professionals put interventions with NEETs into action. The first analytic chapter describes the documentary practices through which employment-related service practitioners deliver a wage-subsidy program and how they make sense of the identity of clients by discussing and documenting troubles. The second chapter focuses on how youth service staff talk about young peoples' participation on a traineeship program through remarking upon their observed conduct and talk, demographic background, demeanour and appearance. The third chapter examines how local authority staff meet a statutory requirement to regularly produce 'data returns' for the Department for Education about the 'participation status' of young people. The chapter describes how members display their knowledge and skill about how to reflexively orientate to this data as intelligible. The last analytic chapter explicates how lobbying work is conducted in a membership organisation concerned with the provision of young peoples' education and skills. The chapter presents materials from regular board and operational meetings about how members of an advocacy group coordinate their activities through making sense of how to produce 'institutional products' with which to conduct and display their lobbying and campaigning work. The empirical content of the thesis includes transcripts of talk-ininteraction, field notes and descriptions of the practical activities through which members made sense of their intervention work on the topic of NEETs. The thesis aims to contribute to a literature on ethnomethodological studies of work and contemporary debates about the implementation of state interventions.

13. Scrutinising Statistics, Questioning BME Unemployment: The Uses of Labour Market Statistics in Parliamentary Select Committee Evidence Sessions

Abstract

This article describes how statistics are scrutinised as evidence. It focuses on the uses of a labour market statistic during House of Commons select committee evidence sessions. The statistic in question was '55.5% of economically active black men, aged 16-24, are unemployed'. The article describes how this individual piece of evidence was scrutinised through 'ordinary' interactional practices. By drawing on ethnomethodology and conversation analysis, the article describes how questions and answers about this piece of evidence were organised by politicians, policymakers and witnesses. These members accounted for this statistic by expressing horror, questioning and interrupting witnesses, giving analyses of educational attainment, critiquing the validity of the statistic, citing factors such as 'unconscious bias', professing they were working hard, and sanctioning witnesses' answers. The article shows how statistical evidence is both a tool and an object of scrutiny, a basic component for policy deliberation, and how it is interactionally organised through 'ordinary' practices.

Abstract

To enable interrogation of tumor HLA LOH as a clinical diagnostic for precision oncology, we developed and validated an assay that detects HLA LOH within the context of an FDA-approved clinical diagnostic test, Tempus xT CDx. Validation was conducted via: (1) analytical evaluation of 17 archival patient samples and 42 cell line admixtures and (2) independent clinical evaluation of LOH prevalence in the HLA-A gene (HLA-A LOH) across 10,982 patients. To evaluate the prognostic relevance of HLA-A LOH we assessed 256 immunotherapy-treated non-small cell lung cancer (NSCLC) patients. To determine the feasibility of prospectively identifying and enrolling HLA-A LOH patients into a clinical trial, we established BASECAMP-1 (NCT04981119). We observed a positive predictive agreement of 97% and a negative predictive agreement of 100% in samples with ≥ 40% tumor purity. We observed HLA-A LOH in 16.1% of patients (1771/10,982), comparable to previous reports. HLA-A LOH was associated with longer survival among NSCLC adenocarcinoma patients (HR = 0.60, 95% CI [0.37, 0.96], p = 0.032) with a trend towards shorter survival among squamous cell patients (HR = 1.64, 95% CI [0.80, 3.41], p = 0.183). In 20 months, we prospectively screened 1720 subjects using the Tempus AWARE program, identifying 26 HLA-A*02 LOH patients at 8 sites, with 14 (54%) enrolled into BASECAMP-1. In conclusion, we developed and validated an investigational assay that detects tumor HLA LOH within an FDA-approved clinical diagnostic test, enabling HLA LOH utilization in diagnostic, prognostic, and therapeutic applications. Abstract

This article examines the history of Home Office research on racism between 1976 and 1997. By analysing archival materials, the article focusses on the methodological orientations and bureaucratic negotiations associated with this corpus of administrative criminological studies. The article describes how Home Office criminologists adopted a stance that privileged quantification, restudies and aligning its research with pre-existing policy paradigms of Situational Crime Prevention and multi-agency coordination. In response to campaigners' calls for racial justice, Home Office policy officials used this research to demonstrate governmental concern, manage media coverage and coordinate community stakeholders. The article argues that British criminology should move beyond dismissing administrative criminology as 'theoretical empiricism' to enable a more critical appraisal of the entangled histories of racism, criminology and crime policy. Abstract

Context.--Pulmonary carcinoids are classified as typical or atypical by assessing necrosis and mitoses, which usually cannot be adequately assessed on small biopsies. Ki-67 is not currently used to grade pulmonary carcinoids, but it may be helpful to determine preliminary grade in biopsies. However, the rate at which Ki-67 could underestimate or overestimate grade on small biopsies has not been well studied. Objective.--To compare Ki-67 labeling obtained on small biopsies to subsequent resection. Design.--Ki-67 was performed on paired biopsy and resection specimens from 55 patients. Slides were scanned using Aperio ScanScope. Labeling index was determined using automated hot spot and tumor tracing methods. Results.--The study included 41 typical and 14 atypical carcinoids. Atypical carcinoids were larger and had more distant metastases. Death from disease occurred in 3 patients (all had atypical carcinoids). Median hot spot Ki67 labeling index was greater in resection compared with biopsy by 0.7% (P = .02). Median tumor tracing Ki-67 was lower in resection compared with biopsy by 0.5% (P < .001). Receiver-operating characteristic analysis showed similar hot spot Ki-67 cutoffs to predict atypical histology (3.5% for biopsy, 3.6% for resection; area under the curve [AUC], 0.75 and 0.74, respectively). Different optimal cutoffs were needed for tracing method based on biopsy (2.1%; AUC, 0.75) compared with resection (1.0%; AUC, 0.67). Conclusions.--Hot spot Ki-67 tends to underestimate grade on small biopsies, whereas grade is overestimated by tumor tracing. Hot spot Ki-67 cutoff of 3.5% predicted atypical histology for both biopsy and resection. Abstract

Recent debates on evidence-based policymaking have demonstrated limited engagement with the history of the Government Social Research (GSR) profession and its role in facilitating the translation of evidence into policy. Though there was a concerted scholarly focus on social research functions within government during the 1980s and 1990s, the recent limited focus on these professions has led to a 'blind spot' in contemporary governance research. As a case in point, the United Kingdom's GSR profession offers a critical vantage point upon which to develop new insights into the relationship between evidence and policy. We argue that just as the GSR profession is currently undergoing significant reform programmes, there is a critical need for a critical research agenda on the composition of research professions within governments. Such a research agenda would reflect on crucial questions about the interface between research evidence and other government functions. In conclusion, we offer four starting points for a comparative, interdisciplinary, transnational research agenda, focusing on the effects of reform programmes for (1) researchers' professional identities and values, (2) organisational change processes, (3) accountability challenges, and (4) intra-professional relationships with evidence producers. 48. Modeling the Rainfall Exploitation of the Reservoirs in Malaga Province, Spain.

Abstract

In areas with scarce water resources, it is so important to analyze the connection between the different elements of a river basin and the water collected by the basin's reservoir, to determine and predict the spatial and temporal variability of water on it. In this paper, we use the basic principles of hydrological modelling to develop a model for the exploitation of rainfall in reservoir basins in the province of Malaga, Spain. The monthly water input data of the seven reservoirs in the province of Malaga, provided by the Hidrosur Network of the Automatic Hydrological Information System (SAIH), as well as the precipitation and daily temperature of the stations of the State Meteorological Agency (AEMET) associated with the basins of each of these reservoirs were used. We assume that the entrance to a reservoir in a given month must depend on the precipitation produced in its watershed (both the amount of rain and the intensity with which it fell), the precipitation collected from the previous months (and the way in which it was produced) and the evapotranspiration produced during that period. For each reservoir, we propose a model with nine parameters to simulate the arrival of rainfall to the reservoir, covering aspects from the amount and intensity of rain, past and present, to the level of evapotranspiration on a given area for a given date. These nine parameters are optimally adjusted through an artificial intelligence algorithm to maximize the correlation between real and simulated contributions. The results show how this model, adjusted for each reservoir, will let us predict how changes in the rainfall and temperature patterns, predicted, for example, by the IPCC models, will affect the future water levels at the studied reservoirs.
