Abstract In the inspection and diagnosis of concrete construction, crack detection is highly recommended in the earliest phases to prevent any potential risks later. However, the flaws in concrete surfaces cannot be reliably and effectively identified using traditional crack detection techniques. The suggested algorithm is a supportive tool for agents or authorities to use in crack detection mechanisms to monitor and assess the current condition of buildings or bridges. The researchers aim to establish an intelligent model for automatic crack detection on different concrete surfaces based on image processing technology. Three different concrete surfaces—bridge decks, walls, and concrete cubes—are used to test the model. A subset of the public dataset of bridge decks and walls from SDNET (2018) and 150*150*150 mm of concrete cubes taken from the material laboratory of the faculty of engineering at Ain Shams University are applied to the model. The model F1-score measures are 98.87%, 97.43%, and 74.11% for detecting cracks in bridges, walls, and concrete cubes, respectively. The validation of the applicability of the suggested novel approach is based on a comparison with recent methods for crack recognition. The contribution of this study is that it could be applied to detect cracks efficiently on three different types of concrete surfaces, including uneven concrete surfaces, random noise, voids, dents, colour changes, and stain marks. The proposed method is transparent in its workflow and has a lower computational cost compared with deep learning frameworks. Thus, the outcomes of this model demonstrate its effectiveness in concrete defect field investigation.

Background: White matter (WM) fiber tracts in the brainstem communicate with various regions in the cerebrum, cerebellum, and spinal cord. Clinically, small lesions, malformations, or histopathological changes in the brainstem can cause severe neurological disorders. A direct and non-invasive assessment approach could bring valuable information about the intricate anatomical variations of the white matter fiber tracts and nuclei. Although tractography from diffusion tensor imaging has been commonly used to map the WM fiber tracts connectivity, it is difficult to differentiate the complex WM tracts anatomically. Both high field MRI methods and ultrahigh-field MRI methods at 7T and 11.7 T have been used to enhance the contrast of WM fiber tracts. Despite their promising results, it is still challenging to achieve wide clinical adoption at 3T. In this study, we explored a clinically feasible method using a proton density weighted (PDW) 3D gradient echo (GRE) sequence to directly image the WM fiber tracts in the brainstem at 3T in vivo. Methods: We optimized a 3D high resolution, double echo, short TR, PDW GRE sequence on 5 healthy volunteers using a clinical 3T scanner to visualize the complicated anatomy of WM fiber tracts in the brain stem. Tissue properties including T1, proton density and T2* from in vivo quantitative MRI data were used for simulations to determine the optimal flip angle for the sequence. The visualization of multiple WM fiber tracts in the brainstem was assessed qualitatively and quantitatively using relative contrast and contrast-to-noise ratio (CNR). To improve the CNR, the final images were created by averaging over all echoes from two consecutive scans at the optimal flip angle. The results were compared to anatomical atlases and histology sections to identify the major fiber tracts. All the identified major fiber tracts were labeled on axial, sagittal and coronal slices. Results: The WM fiber tracts were found to have distinct hypointense signal throughout the brainstem and most of the major WM fiber tracts, such as the corticospinal tract, medial lemniscus, medial longitudinal fasciculus, and central tegmental tract, in the brainstem up to and including the thalamus were identified in all subjects. Both qualitative and quantitative evaluations showed that the 3° scan offered the best contrast for WM fiber tracts for a TR of 20 ms. The average over the first two echo times and two consecutive 3° scans gave a CNR of 47.8 ± 6.2 for the pyramidal tracts in particular and CNRs values greater than 6.5 ± 2.4 for the rest of the fiber tracts. Conclusions: All the major fiber tracts in the brainstem could be visualized. Given the reasonably short scan time of 10 min at 3T, double echo PDW GRE sequence is a very practical approach for clinical adoption.

Introduction: Pediatric patients account for 6–10% of emergency medical services (EMS) activations in the United States. Approximately 30% of these children are not transported to an emergency department (ED). Adult data in the literature reports higher hospitalization and complications following non-transport. Few studies discuss epidemiology and characteristics of pediatric non-transport; however, data on outcome is limited. Our primary aim was to determine outcomes of non-transported children within our urban EMS system before and during the COVID-19 pandemic. Our secondary objective was to explore reasons for non-transport. Methods: This was a prospective, descriptive pilot study. We compared EMS data for September 2019 (pre-COVID-19) to September 2020 (pandemic). Included were children aged 0–17 years who activated EMS and did not receive transport to the primary hospital for the EMS capture area. We defined outcomes as repeat EMS activation, ED visits, and hospital admissions, all within 72 hours. Data was obtained via electronic capture. We used descriptive statistics to analyze our data, chi square for categorical data, stepwise logistic regression, and univariate logistic regression to test for association of covariates with non-transport. Results: There were 1,089 pediatric EMS activations in September 2019 and 780 in September 2020. Non-transport occurred in 633 (58%) in September 2019 and 412 (53%) in September 2020. Emergency medical services was reactivated within 72 hours in the following: 9/633 (1.4%) in 2019; and 5/412 (1.2%) in 2020 (P = 0.77). Visits to the ED occurred in 57/633 (9%) in 2019 and 42/412 (10%) in 2020 (P = 0.53). Hospital admissions occurred in 10/633 (1.5%) in 2019 and 4/412 (0.97%) in 2020 (P = 0.19). One non-transported patient was admitted to the intensive care unit in September 2020 (12 years, and fever were associated with repeat EMS activation. The most common reason for non-transport in both study periods was that the parent felt an ambulance was not necessary (47%). Conclusion: In our system, non-transport of pediatric patients occurred in >50% of EMS activations with no significant adverse outcome. Age >12 years, fever, and Hispanic ethnicity were more common in repeated EMS activations. The most common reason for non-transport was parents feeling it was not necessary. Future studies are needed to develop reliable EMS guidelines for pediatric non-transport.

Abstract Background Cellular and cytoskeletal events of overactive bladder (OAB) have not been sufficiently explored in human bladder due to different limitations. Bladder outlet obstruction (BOO) had been induced in different animal models with different methods to induce (OAB). Similarity of the animal models of BOO to the human OAB is postulated but has not been confirmed. The interstitial cells of Cajal (ICCs), and telocytes (TCs) are an important players in smooth muscles conductivity, they had not been well investigated in the previous BOO models. Objectives are to investigate the morphological pattern of cellular, cytoskeleton and telocytes distribution in BOO rat model and to match the events in two time periods and compare it to the findings in real-world human OAB. Methods Female Sprague-Dawley rats (Rattus norvegicus) were randomly divided into: sham (n = 10), BOO 6 W (n = 10), BOO 8 W (n = 10). Operative procedure to Induce BOO was done under anesthesia with intraperitoneal Ketamine administration. The Effect of induction of BOO was evaluated after 6 and 8 weeks. The rats were anesthetized, and the urinary bladder was removed, while the rat was unconscious under anaesthesia it was transferred to the inhalation anaesthesia cage for euthanasia, rats were sacrificed under light anesthesia using isoflurane. Care of animals, surgical procedure, and euthanasia adhered to Guide for the Care and Use of Laboratory Animals, and AVMA Guidelines for the Euthanasia of Animals. The retrieved bladder was processed for examination with histopathology, immunohistochemistry (IHC), and transmission electron microscopy (EM). Results Histological examination of the bladder shows thinner urothelium, condensation of collagen between muscle bundles. IHC with c-kit shows the excess distribution of ICCs between smooth muscle bundles. EM shows frequent distribution of TCs that were situated between collagen fibers. Finings in BOO 6 W group and BOO 8 W group were comparable. Conclusion The animal model study demonstrated increased collagen/ smooth muscle ratio, high intensity of ICCs and presence of TCs. Findings show that a minimally invasive procedure to induce BOO in rats had resulted in an OAB that has morphological changes that were stable in 6 & 8 weeks. We demonstrated the distribution of TCs and ICCs in the rat animal model and defined them. The population of TCs in the BOO rat model is described for the first time, suggests that the TCs and ICCs may contribute to the pathophysiology of OAB. Similarity of animal model to human events OAB was demonstrated. These findings warrant further study to define the role of TCs in OAB. Clinical trial registry The study does not require a clinical trial registration; it is an experimental animal study in basic science and does not include human subjects.

As security breaches are increasingly widely reported in today’s culture, cybersecurity is gaining attention on a global scale. Threat modeling methods (TMM) are a proactive security practice that is essential for pinpointing risks and limiting their impact. This paper proposes a hybrid threat modeling framework based on system-centric, attacker-centric, and risk-centric approaches to identify threats in Operational Technology (OT) applications. OT is made up of software and hardware used to manage, secure, and control industrial control systems (ICS), and its environments include factories, power plants, oil and gas refineries, and pipelines. To visualize the “big picture” of its infrastructure risk profile and improve understanding of the full attack surface, the proposed framework builds on several threat modeling methodologies: PASTA modeling, STRIDE, and attack tree components. Nevertheless, the continuity and stability of vital infrastructure will continue to depend heavily on legacy equipment. Thus, protecting the availability, security, and safety of industrial environments and vital infrastructure from cyberattacks requires operational technology (OT) cybersecurity. The feasibility of the proposed approach is illustrated with a case study from a real oil and gas production plant control system where numerous significant cyberattacks in recent years have targeted OT networks more frequently as hackers realized the possibility of disruption due to insufficient OT security, particularly for outdated systems. The proposed framework achieved better results in detecting threats and severity in the design of the case study system, helping to increase security and support cybersecurity assessment of legacy control systems.

Abstract Background Radiation shielding in radiology has historically been achieved with lead; however, there has been an increasing demand for radiation shielding to be more environmentally friendly. Barium has shown promise as a substitute in many radiology applications. This study aims to investigate a barium sulphate shield in protecting the thyroid and the eye lens during panoramic radiography. Methods During a simulated panoramic examination, an anthropomorphic phantom and a solid-state detector measured the radiation dose to the surface thyroid and the eye lens. The measurements were taken using no shield and a barium sulphate shield. A Welch's T-test was employed to compute the shield's effect on radiation. Two radiologists assessed the image quality with and without the thyroid shields. Results The dose reduction was between 66 and 75% for the barium shield at the thyroid. The dose reduction ranged between 15 and 61% in the eye region. Images using a barium shield were deemed adequate for diagnostic interpretation. Conclusions Barium shields effectively reduce the radiation dose in the thyroid region during panoramic radiography without degrading image quality. The dose reduction depends on the tube voltage and the area of interest.

Prostate cancer (PC) is a prevalent and potentially fatal form of cancer that affects men globally. However, the existing diagnostic methods, such as biopsies or digital rectal examination (DRE), have limitations in terms of invasiveness, cost, and accuracy. This study proposes a novel machine learning approach for the diagnosis of PC by leveraging clinical biomarkers and personalized questionnaires. In our research, we explore various machine learning methods, including traditional, tree-based, and advanced tabular deep learning methods, to analyze tabular data related to PC. Additionally, we introduce the novel utilization of convolutional neural networks (CNNs) and transfer learning, which have been predominantly applied in image-related tasks, for handling tabular data after being transformed to proper graphical representations via our proposed Tab2Visual modeling framework. Furthermore, we investigate leveraging the prediction accuracy further by constructing ensemble models. An experimental evaluation of our proposed approach demonstrates its effectiveness in achieving superior performance attaining an F1-score of 0.907 and an AUC of 0.911. This offers promising potential for the accurate detection of PC without the reliance on invasive and high-cost procedures.

Using aluminium for bridge deck was firstly in 1933 in U.S.A, in Egypt it is very interested for constructing aluminium bridge. The use of Aluminium alloys for constructing bridges and bridge decks has much to offer because of their light weight, excellent corrosion resistance. Some of the aluminum alloys that used for bridges are 5086-H116, 6061-T651, 6061-T6 and 6063-T6 as given in AAHSTO. In this research aluminum bridges will be studied. Structural analysis, design, and comparison between two bridges having the same geometric dimensions are introduced. The first bridge consists of steel girders and concrete deck slab. The second one consists of aluminum girders and aluminium deck. the Loads and the design of the two bridges are according to (AASHTO LRFD Bridge). It is concluded that Aluminium Bridge can compete the composite one because of its excellent corrosion resistance and light weight of super structure. The aluminuim saves about 82% from the weight of superstructures

Abstract Background To compare the efficacy and safety of standard percutaneous nephrolithotomy (PCNL) with mini- PCNL for kidney stones 2–4 cm. Methods Eighty patients were enrolled in a comparative study, they were randomly divided into mini-PCNL group (n = 40) and standard-PCNL (n = 40). Demographic characteristics, perioperative events, complications, stone free rate (SFR) were reported. Results Both groups showed no significant difference in clinical data about age, stone location, back pressure changes, and body mass index. The mean operative time was (95 ± 17.9 min) in mini-PCNL, and (72.1 ± 14.9 min). Stone free rate were 80% and 85% in mini-PCNL and standard-PCNL respectively. Intra-operative complications, post-operative need for analgesia, hospital stay were significantly higher in standard-PCNL compared to mini-PCNL (85% vs. 80%). The study followed CONSORT 2010 guidelines for reporting parallel group randomization. Conclusion Mini-PCNL is an effective and safe treatment of kidney stones 2–4 cm, it has the advantage over standard-PCNL being has less intra-operative events, less post-operative analgesia, shorter hospital stay, while operative time and stone free rate are comparable when considering multiplicity, hardness, and site of stones.

The global economy was affected by the COVID-19 pandemic. This paper estimates the impact of the COVID-19 pandemic on the overall risk of construction projects in Egypt and identifies risk factors affected by the pandemic. A structured questionnaire was designed to collect data. One hundred thirty-two cases before the COVID-19 pandemic and 68 cases after the pandemic were managed. The outliers were excluded using Mahalanobis distance. The distribution of the samples before and after the COVID-19 pandemic was studied using the Kolmogorov-Smirnov test and the difference in medians using the Mann-Whitney U test. The delay in payment and the fluctuation in material prices are the most critical risk factors after the epidemic spread in residential projects in Egypt. Suspension of projects has been the most significant consequence of the COVID-19 pandemic, and the pandemic increased the overall risk by 17%. This research helps to identify the main risk factors after the pandemic, and then the project manager can prepare an appropriate risk response plan.

Abstract Recurrence and progression rates vary widely among different histological subtypes of bladder cancer (BC). Normal-appearing mucosa in non-muscle invasive bladder cancer and muscle-invasive bladder cancer in cystoscopy and histopathology is a factor in staging and treatment. Telocytes (TCs) are spindle-shaped cells that connect with other cell types allowing communication though cytoskeletal signaling. They are involved in tissue regeneration and pathogenesis of diseases and cancer. In this study, 12 normal-appearing tissues from urinary bladder with BC, both invasive and non-invasive were evaluated in patients who had either trans-urethral resection of bladder tumor or cystectomy. In each case, cystoscopy, intraoperative inspection, and histopathology all confirmed the absence of cancerous elements. Five patients with neurogenic bladder were used as a control group. Immunohistochemistry revealed that c-Kit + cells were intensively distributed in bladder layers from BC samples, while they were seldom detected in the control group. Ultrastructural examination of reprocessed tissue showed an intense distribution of TCs and telopodes in the submucosa and between smooth muscle cells in BC. Telopodes were numerous, arborizing, and intercommunicating. Whereas TCs and telopodes were scarce in the neurogenic bladder. Also, cancerous tissue had the highest expression level of ezrin protein, and this level gradually decreased as we moved away from the tumor. Our finding of TCs number in normal-appearing tissues in conjunction with ezrin expression may compete invasiveness and possibly a trail to reduce recurrence rates.

Abstract Background High-fidelity simulations based on real-life clinical scenarios have frequently been used to improve patient care, knowledge and teamwork in the acute care setting. Still, they are seldom included in the allergy-immunology curriculum or continuous medical education. Our main goal was to assess if critical care simulations in allergy improved performance in the clinical setting. Methods Advanced anaphylaxis scenarios were designed by a panel of emergency, intensive care unit, anesthesiology and allergy-immunology specialists and then adapted for the adult allergy clinic setting. This simulation activity included a first part in the high-fidelity simulation-training laboratory and a second at the adult allergy clinic involving actors and a high-fidelity mannequin. Participants filled out a questionnaire, and qualitative interviews were performed with staff after they had managed cases of refractory anaphylaxis. Results Four nurses, seven allergy-immunology fellows and six allergy/immunologists underwent the simulation. Questionnaires showed a perceived improvement in aspects of crisis and anaphylaxis management. The in-situ simulation revealed gaps in the process, which were subsequently resolved. Qualitative interviews with participants revealed a more rapid and orderly response and improved confidence in their abilities and that of their colleagues to manage anaphylaxis. Conclusion High-fidelity simulations can improve the management of anaphylaxis in the allergy clinic and team confidence. This activity was instrumental in reducing staff reluctance to perform high-risk challenges in the ambulatory setting, thus lifting a critical barrier for implementing oral immunotherapy at our adult center.

Abstract Background Zinner's syndrome is a mesonephric duct anomaly characterized by unilateral renal agenesis, ipsilateral seminal vesicle cyst, and ipsilateral ejaculatory duct obstruction due to insult occurred at urogenital tract embryogenesis during the first trimester. In the third and fourth decades of life, it is frequently diagnosed when patients begin to be symptomatic, such as lower urinary tract symptoms, infertility and painful ejaculation. Case presentation Herein we illustrate case review including five patients diagnosed as Zinner’s syndrome, three of them complaining from infertility; however, the remaining two cases were fertile and incidentally diagnosed. Conclusions Radiological investigations play significant role in the diagnostic and management processes including US, CT and MRI for detecting ipsilateral renal agenesis and unilateral seminal vesicles dilatation, but evaluation of ejaculatory duct can be done only by MRI and transrectal ultrasound; however, the latter may be sometimes nonconclusive or intolerable by patients, so MRI is considered the golden modality with its high capability to assess the lower male genital tract which played a significant role in our case series starting with detection of the presence or absence of the ejaculatory duct obstruction as well as the high delineation of the origin and nature the seminal vesicle cyst including also its size and content and finally by detection of the communication between ureteral bud and the seminal vesicles cystic dilatation.

INTRODUCTION Radiology education is essential for nonradiologist specialists and practitioners as well as for radiologists. We conducted a survey to gather the opinions of Turkish medical students from first to sixth grade regarding their radiology curricula, radiology education content, and perceptions of various imaging modalities and to assess the amount, adequacy, and homogeneity of radiology education in various schools. METHODS Turkish medical students were reached by student ambassadors from 10 different schools of medicine via social media and email. They were provided with a 20-question survey—via the SurveyMonkey platform—related to their radiology curriculum and their perceptions of the radiology education at their schools and of different imaging modalities. Subjective parameters were scaled by a 4-point Likert scale and the results are reported by percentages of students. RESULTS A total of 988 medical students (F/M: 61%/39%) from 41 different medical schools participated in this survey. Of those, 57% were preclinical students (≤ third year of medical school), while 43% were clinical students (> third year). More than half of the students (51%) stated that the amount of radiology education included in their curriculum was too little , while 44% of them stated it was just right and only 5% stated it was too much . Only 31% of the participants stated that they were able to review radiology images on their own. When asked about their level of confidence in identifying the position of lines and tubes, pneumonia, pneumothorax, and pleural effusion on chest radiographs, 41%, 39%, 41%, and 41% of the participants, respectively, stated that they were not confident. Thirty-five percent of the participants had not received any training in comparing normal to abnormal imaging of bone fractures, pneumonia, pleural effusion, subdural hemorrhage, or pneumothorax. The majority of the Turkish medical students in this survey had never heard (57%) nor used (64%) the American College of Radiology Appropriateness Criteria. CONCLUSIONS The radiology curriculum in Turkey differs among various schools and most students stated that preclinical radiology course content was inadequate. Further studies and improvements must be conducted to provide high-quality, equitable radiology education that begins during preclinical training with respect to the students’ opinions.

Abstract Background Radiology serves in the diagnosis and management of many diseases. Despite its rising importance and use, radiology is not a core component of a lot of medical school curricula. This survey aims to clarify current gaps in the radiological education in Egyptian medical schools. In February–May 2021, 5318 students enrolled in Egyptian medical schools were recruited and given a 20-multiple-choice-question survey assessing their radiology knowledge, radiograph interpretation, and encountered imaging experiences. We measured the objective parameters as a percentage. We conducted descriptive analysis and used Likert scales where values were represented as numerical values. Percentages were graphed afterwards. Results A total of 5318 medical students in Egypt answered our survey. Gender distribution was 45% males and 54% females. The results represented all 7 class years of medical school (six academic years and a final training year). In assessing students’ knowledge of radiology, most students (75%) reported that they received ‘too little’ education, while 20% stated the amount was ‘just right’ and only 4% reported it was ‘too much.’ Sixty-two percent of students stated they were taught radiology through medical imaging lectures. Participants’ future career plans were almost equally distributed. Near half of participants (43%) have not heard about the American College of Radiology Appropriateness Criteria (ACR-AR), while 39% have heard about it but are not familiar with. Conclusions Radiology is a novel underestimated field. Therefore, medical students need more imaging exposure. To accomplish this, attention and efforts should be directed toward undergraduate radiology education to dissolve the gap between radiology and other specialties during clinical practice. A survey answered by medical students can bridge between presence of any current defect in undergraduate radiology teaching and future solutions for this topic.

Abstract Background There is a worldwide deficit in teaching and training in the field of radiology for undergraduate medical students. This educational gap is prominent in many medical schools as most radiology curricula are a part of other specialty trainings, usually provided by non-radiologists. After COVID-19 pandemic, there was an increased trend in online education. However, questions have been raised about the efficacy and acceptance of online education. We developed a course on the principles of radiology and medical imaging basics to target Egyptian medical students. We then assessed the impact of these educational videos through several online surveys. Our "The Principles of Radiology Online Course" was delivered to students at various Egyptian medical schools; it was a prerecorded series composed of nine sessions, and each session followed the sequence of a pre-test, video, and post-test. There was a final survey to assess the overall feedback. Finally, we analyzed the results to give insight onto how teaching radiology through online lectures can help build better physicians. Results Among various medical schools around Egypt, 1396 Egyptian medical students joined this cohort. Cohort population percentage was 56% female and 44% male. Ninety-eight percent of the students agreed that this program increased their understanding of radiology. Eighty-four percent of the students found the platform friendly and easy to use. Abstract Background In multidisciplinary education, different perspectives from more than one discipline are used to illustrate a certain topic. The aim of this study was to evaluate the effectiveness of an online, multidisciplinary radiology curriculum to teach radiology to medical students in Egypt. A multidisciplinary team of radiologists, surgeons, and internists taught a series of 5 case-based radiology sessions on a web conference platform. Topics included common clinical case scenarios for various body systems. Undergraduate medical students across Egypt were enrolled in the course. A pre-test–post-test design was used to evaluate the efficacy of each session. Upon course completion, students filled out a subjective survey to assess the radiology education series. Results On average, 1000 students attended each session. For each session, an average of 734 students completed both the pre-test and post-test. BackgroundTraumatic brain injuries (TBIs) and intra-abdominal injuries (IAIs) are 2 leading causes of traumatic death and disability in children. To avoid missed or delayed diagnoses leading to increased morbidity, computed tomography (CT) is used liberally. However, the overuse of CT leads to inefficient care and radiation-induced malignancies. Therefore, to maximize precision and minimize the overuse of CT, the Pediatric Emergency Care Applied Research Network (PECARN) previously derived clinical prediction rules for identifying children at high risk and very low risk for IAIs undergoing acute intervention and clinically important TBIs after blunt trauma in large cohorts of children who are injured. ObjectiveThis study aimed to validate the IAI and age-based TBI clinical prediction rules for identifying children at high risk and very low risk for IAIs undergoing acute intervention and clinically important TBIs after blunt trauma. MethodsThis was a prospective 6-center observational study of children aged 24 hours for TBI, or hospital admission of ≥2 nights due to a TBI on CT). Prediction rule accuracy was assessed by measuring rule classification performance, using standard point and 95% CI estimates of the operational characteristics of each prediction rule (sensitivity, specificity, positive and negative predictive values, and diagnostic likelihood ratios). ResultsThe project was funded in 2016, and enrollment was completed on September 1, 2021. Data analyses are expected to be completed by December 2022, and the primary study results are expected to be submitted for publication in 2023. ConclusionsThis study will attempt to validate previously derived clinical prediction rules to accurately identify children at high and very low risk for clinically important IAIs and TBIs. Changes in wake/sleep architecture have been observed in both aged human and animal models, presumably due to various functional decay throughout the aging body particularly in the brain. Microglia have emerged as a modulator for wake/sleep architecture in the adult brain, and displayed distinct morphology and activity in the aging brain. However, the link between microglia and age-related wake/sleep changes remains elusive. In this study, we systematically examined the brain vigilance and microglia morphology in aging mice (3, 6, 12, and 18 months old), and determined how microglia affect the aging-related wake/sleep alterations in mice. We found that from young adult to aged mice there was a clear decline in stable wakefulness at nighttime, and a decrease of microglial processes length in various brain regions involved in wake/sleep regulation. The decreased stable wakefulness can be restored following the time course of microglia depletion and repopulation in the adult brain. Microglia repopulation in the aging brain restored age-related decline in stable wakefulness. Taken together, our findings suggest a link between aged microglia and deteriorated stable wakefulness in aged brains.

Planners face many obstacles during the planning phase of any new project, especially project scheduling due to the numerous details and complexity of each uniquely designed project; previous experience in similar projects and traditional scheduling methods are not sufficient. The 4D-BIM schedule is the best solution, as it can be integrated with other modern technologies such as UAS, which improves project scheduling by allowing the project team to access project plans, details, and time-related information to simulate construction sequences. Despite the benefits of using 4D-BIM, planners prefer traditional planning and scheduling methods because of the barriers to using modern technologies. This research proposes a structural equation model focusing on estimating the weights of BIM barriers in offshore construction projects, including: the investigation of barriers keeping the construction industry standing at 3D-BIM; the relationships between them; and the evaluation of the probability and impact of each, using fuzzy theory. To validate the proposed model, a case study of an offshore project was conducted. The most important latent variables were awareness, risk, demand, and management, while the most prominent observed variables were the uncertainty of the return on investment and the lack of experienced users. This research guides project managers on actions that can be taken for each key variable to enhance and develop the use of 4D-BIM in project scheduling.

Introduction:Blood cultures are often obtained from healthy paediatric patients with fever on presentation to the emergency department (ED). Although published guidelines and previous research outlined indications for obtaining blood culture and the relatively low risk of bacteraemia in vaccinated children, there may be practice variability among institutions and physicians. Primary objective: To describe the demographic characteristics, diagnosis, disposition and outcome of children who are fully vaccinated, healthy and have fever from whom bacterial blood cultures were obtained. Secondary objective: To determine the rate of blood culture contamination and outcomes.Methods:Retrospective chart review of all blood cultures collected in the ED between January 1, 2015, and December 31, 2015. Patients aged 6 months to 17 years were eligible for enrolment. Children not fully vaccinated, immunocompromised or with chronic, debilitating disease were excluded. Patients were divided into febrile and afebrile cohorts based on the initial temperature at ED presentation. Data were analysed with two-sample t-test and chisquared analysis.Results:Blood cultures were obtained from 7.980 children at the ED, with an overall positivity rate of 5.51%. No significant difference was detected in the number of positive blood cultures between the two cohorts (p=0.85). No significant difference was found between pathogenic cultures between the two cohorts (p=0.35). All patients who were discharged with a positive blood culture were called back for a repeat culture. None grew a pathogenic organism on the repeat culture. The overall positive blood culture rate for true pathogens was 1.3%, and the overall contamination rate was 1.9%.Conclusion:The rates of positive blood culture between febrile and afebrile cohorts presenting to the ED were comparable. While the overall rate of positive culture remains low, consistent with previously reported rates of bacteraemia, an unacceptably high rate of contamination resulting in return visits was observed. Routine blood culture in children who were fully immunised and had a history of febrile illness is not indicated.

Abstract Background Real-time automated analysis of videos of the microvasculature is an essential step in the development of research protocols and clinical algorithms that incorporate point-of-care microvascular analysis. In response to the call for validation studies of available automated analysis software by the European Society of Intensive Care Medicine, and building on a previous validation study in sheep, we report the first human validation study of AVA 4. Methods Two retrospective perioperative datasets of human microcirculation videos (P1 and P2) and one prospective healthy volunteer dataset (V1) were used in this validation study. Video quality was assessed using the modified Microcirculation Image Quality Selection (MIQS) score. Videos were initially analyzed with (1) AVA software 3.2 by two experienced investigators using the gold standard semi-automated method, followed by an analysis with (2) AVA automated software 4.1. Microvascular variables measured were perfused vessel density (PVD), total vessel density (TVD), and proportion of perfused vessels (PPV). Bland–Altman analysis and intraclass correlation coefficients (ICC) were used to measure agreement between the two methods. Each method’s ability to discriminate between microcirculatory states before and after induction of general anesthesia was assessed using paired t-tests. Results Fifty-two videos from P1, 128 videos from P2 and 26 videos from V1 met inclusion criteria for analysis. Correlational analysis and Bland–Altman analysis revealed poor agreement and no correlation between AVA 4.1 and AVA 3.2. Many previous studies have developed models for estimating the total cost, whether in the planning stage or the early stage of the project. However, models for estimating the overall risk were proposed in the planning stage only. This paper identifies the factors affecting the overall risk in residential projects at the early stage. The 43 risk factors at the planning stage were identified using a Delphi technique. Experts summarize the 43 risk factors into four factors that can be used to predict the overall risk in the early stage of the project. A multilayer perceptron model with one hidden layer was proposed. The mean absolute error rate for the proposed model was 10%. Risk factors can be used to develop a model to predict the impact of overall risk on project cost at the early stage. The developed model helps stakeholders decide whether the project should continue or be terminated.

Abstract Background Preoperative staging of urinary bladder carcinomas using TNM system is crucial in the management of bladder cancer which is determined mainly by stage and grade of tumor at diagnosis. We aim to evaluate the diagnostic accuracy of multiparametric MRI (mp-MRI) for assessment of the bladder cancer T stage and histologic grade. Results The overall T2-WI diagnostic accuracy for the T stage was 72.3%, increased to be 87.1% for contrast-enhanced images, and 92.6% for DWI, reaching the maximum accuracy 94.5% using the combined multi-parametric MRI technique. Diagnostic accuracies of mp-MRI in differentiating superficial from muscle-invasive (91%) and organ-confined from non-organ confined tumors (92%) were superior to DW-MRI (89% and 87%), DCE-MRI (84% and 83%), and T2W-MRI (74% and 71%), respectively. The agreement between MRI findings and histopathological staging was greater in mp-MRI (k = 0.91; excellent agreement) than in DW-MRI (k = 0.77; moderate agreement), DCE-MRI (k = 0.76; substantial agreement), and T2W-MRI (k = 0.53; fair agreement). Conclusion Mp-MRI provides useful information for evaluating the local T stages of bladder cancer and can predict the histological grades of urinary bladder cancers with high diagnostic accuracy.

Abstract Background Adrenal tumors can be detected incidentally in 4 to 8% of patients radiologically. Adenomas, pheochromocytomas, and adrenocortical carcinomas represent the most common tumors of the adrenal glands. Rare histopathological findings are uncommon. We aimed to report two rare primary adrenal tumors diagnosed initially as incidentalomas to identify clinical characteristics, management, and clinical outcomes after treatment. Case presentation The first case was a 52-year-old man presented with an incidentally discovered locally advanced primary adrenal angiosarcoma. The patient was managed surgically with no adjuvant therapy. The patient was followed up for 3 years without evidence of local recurrence. The second case was a 63-year-old woman, presented with an incidentally discovered primary diffuse B-cell lymphoma of the left adrenal gland. She was treated by adrenalectomy. Later on, adjuvant six cycles of CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisolone) chemotherapy were given. After 6 months follow-up, the patient was alive and disease-free. Conclusion The diagnosis of adrenal tumors increased nowadays because of the widespread use of imaging studies, though rare pathologies should be taken into consideration.

Residential projects are described as complex, dynamic systems that are subject to uncertainty. Cost performance is a fundamental challenge. As a result, project managers must adequately identify risks that might lead to cost overruns in residential construction projects. Simulation is noticed to be a useful technique for dealing with these complications. Therefore, this study developed a hybrid dynamic approach to study the effect of different risks on the cost performance of construction projects. The proposed approach combines system dynamics (SD) and discrete event simulation (DES), which can take into consideration the dynamics of the project environment, which contains various continuous influencing factors as well as the construction operations. The developed hybrid model is validated through serial model structure tests and model behavior tests, with the aid of data collected from a real construction project used in the simulation process. Based on the simulation results, it is concluded that the proposed hybrid dynamic approach is helpful to enhance the process performance by permitting construction managers to identify possible process improvement areas that traditional methods may miss.

The ability to foresee hazards early plays a critical role in estimating the entire cost of a project. Although several studies have established models to predict the total cost of a project at a conceptual stage, there remains a research vacuum in measuring the overall risk at this stage. Using artificial neural networks, this research provides a strategy for estimating the overall risk in residential projects at the conceptual stage. There are eight important components in the suggested paradigm. The model was created using data from 149 projects. In the first hidden layer in the model, there are five neurons, and in the second hidden layer, there are three neurons. The suggested model’s mean absolute error rate was 11.7%. In the conceptual stage of residential projects, the number of floors, the type of interior finishes, and the implementation of risk management processes are the significant aspects that influence the overall risk. The proposed model assists project managers in precisely estimating the project’s overall risk, which leads to a more accurate estimation of the contract’s entire worth at the conceptual stage, allowing the stakeholders to decide whether or not to proceed with the project.

Individuals who have suffered a severe brain injury typically require extensive hospitalization in intensive care units (ICUs), where critical treatment decisions are made to maximize their likelihood of recovering consciousness and cognitive function. These treatment decisions can be difficult when the neurological assessment of the patient is limited by unreliable behavioral responses. Reliable objective and quantifiable markers are lacking and there is both (1) a poor understanding of the mechanisms underlying the brain’s ability to reconstitute consciousness and cognition after an injury and (2) the absence of a reliable and clinically feasible method of tracking cognitive recovery in ICU survivors. Our goal is to develop and validate a clinically relevant EEG paradigm that can inform the prognosis of unresponsive, brain-injured patients in the ICU. This protocol describes a study to develop a point-of-care system intended to accurately predict outcomes of unresponsive, brain-injured patients in the ICU. We will recruit 200 continuously-sedated brain-injured patients across five ICUs. Between 24 h and 7 days post-ICU admission, high-density EEG will be recorded from behaviorally unresponsive patients before, during and after a brief cessation of pharmacological sedation. Once patients have reached the waking stage, they will be asked to complete an abridged Cambridge Brain Sciences battery, a web-based series of neurocognitive tests. The test series will be repeated every day during acute admission (ICU, ward), or as often as possible given the constraints of ICU and ward care. Following discharge, patients will continue to complete the same test series on weekly, and then monthly basis, for up to 12 months following injury. Functional outcomes will also be assessed up to 12 months post-injury. We anticipate our findings will lead to an increased ability to identify patients, as soon as possible after their brain injury, who are most likely to survive, and to make accurate predictions about their long-term cognitive and functional outcome. In addition to providing critically needed support for clinical decision-making, this study has the potential to transform our understanding of key functional EEG networks associated with consciousness and cognition.

Background: For ambulatory cases with spastic diplegic cerebral palsy, single event multilevel surgery (SEMLS) has replaced previous methods of treatment for improving gait. This research aimed for investigation if single event multilevel surgery improved gait and function among cases with spastic cerebral palsy. Patients and methods: This was a prospective interventional study carried out on 24 children with spastic lower limbs attending to pediatric orthopedic unit in Zagazig University Hospital and managed with lower limb single event multilevel surgery. Clinical as well as radiological assessments were applied pre-and 6 months post-operative. Results: Single event multi-level surgeries performed in the children with CP generated positive outcomes in the GMFM which changed significantly from (45.67±3.70) in the preoperative assessment to (52.63±3.34) in follow up after 2 years postoperative with p value<0.001. A paired samples t-test revealed that spasticity significantly decreased from pre- (M=15.46, SD=2.187) to post- (M=10.71, SD=2.293) intervention, t(23)=17.53, p<.05, d=1.327, for Spasticity (after intervention) a statistically significant difference was found between groups (2, 21) = 17.502, p < 0.001. Also, a marked improvement occurred in the range of motion (ROM) as determined by ROM score which changed from (44.67±8.67) preoperatively to (33.00±9.09) after 6 months with P value <0.001. Conclusion: The results of SEMLS regarding deformity correction and quality of life improvement are considered satisfactory with minimal complications. [ABSTRACT FROM AUTHOR]

The neuron-to-neuron propagation of misfolded α-synuclein (αSyn) aggregates is thought to be key to the pathogenesis of synucleinopathies. Recent studies have shown that extracellular αSyn aggregates taken up by the endosomal-lysosomal system can rupture the lysosomal vesicular membrane; however, it remains unclear whether lysosomal rupture leads to the transmission of αSyn aggregation. Here, we applied cell-based αSyn propagation models to show that ruptured lysosomes are the pathway through which exogenous αSyn aggregates transmit aggregation, and furthermore, this process was prevented by lysophagy, i.e., selective autophagy of damaged lysosomes. αSyn aggregates accumulated predominantly in lysosomes, causing their rupture, and seeded the aggregation of endogenous αSyn, initially around damaged lysosomes. Exogenous αSyn aggregates induced the accumulation of LC3 on lysosomes. This LC3 accumulation was not observed in cells in which a key regulator of autophagy, RB1CC1/FIP200, was knocked out and was confirmed as lysophagy by transmission electron microscopy. Importantly, RB1CC1/FIP200-deficient cells treated with αSyn aggregates had increased numbers of ruptured lysosomes and enhanced propagation of αSyn aggregation. Furthermore, various types of lysosomal damage induced using lysosomotropic reagents, depletion of lysosomal enzymes, or more toxic species of αSyn fibrils also exacerbated the propagation of αSyn aggregation, and impaired lysophagy and lysosomal membrane damage synergistically enhanced propagation. These results indicate that lysophagy prevents exogenous αSyn aggregates from escaping the endosomal-lysosomal system and transmitting aggregation to endogenous cytosolic αSyn via ruptured lysosomal vesicles. A direct contact co‐culture of skin explants to SZ95 sebocytes (3D‐SeboSkin) has been shown to preserve the integrity of epidermal keratinocytes and dermis. In this study, the properties of epidermal melanocytes were evaluated in the same 3D SeboSkin ex vivo model. Skin explants (n = 6) were maintained in the 3D‐SeboSkin model, in direct contact to fibroblasts and alone in serum‐free medium (SFM). Histopathological, immunohistochemical, apoptosis and oil red staining evaluations were performed at Days 0 and 6 of incubation. Results revealed preservation and prominent proliferation of basal keratinocytes of the skin explants in addition to preservation of dermal collagen and vasculature at Day 6 in the 3D‐SeboSkin culture model and to a lesser extent in co‐culture with fibroblasts but not in SFM alone. Melan‐A+/Ki67‐ epidermal melanocytes remained attached to the dermis even at sites of epidermal detachment in the three skin explant models tested. However, the number of epidermal melanocytes was significantly conserved in 3D‐SeboSkin cultures in comparison with skin explants in SFM (p < 0.05), whereas no difference was found in comparison with the co‐culture with fibroblasts. Few DAPI/TUNEL+ apoptotic melanocytes could mostly be observed in SFM‐incubated skin explants. Furthermore, only SZ95 sebocytes in contact to skin explants in 3D‐SeboSkin exhibited increased lipogenesis with accumulation of abundant lipid droplets. These results denote that the 3D‐SeboSkin model yielded significant preservation of epidermal melanocytes and hence it is optimal for ex vivo studies of abnormalities of skin pigmentation, melanocyte neoplasms and effects of different hormones, cytokines, carcinogens and various therapeutics in a pattern that recapitulates the in vivo environment.
