ObjectivesWe aimed to explore the impact of the COVID-19 pandemic on cardiac diagnostic testing and practice and to assess its impact in different regions in Europe.MethodsThe online survey organised by the International Atomic Energy Agency Division of Human Health collected information on changes in cardiac imaging procedural volumes between March 2019 and March/April 2020. Data were collected from 909 centres in 108 countries.ResultsCentres in Northern and Southern Europe were more likely to cancel all outpatient activities compared with Western and Eastern Europe. There was a greater reduction in total procedure volumes in Europe compared with the rest of the world in March 2020 (45% vs 41%, p=0.003), with a more marked reduction in Southern Europe (58%), but by April 2020 this was similar in Europe and the rest of the world (69% vs 63%, p=0.261). Regional variations were apparent between imaging modalities, but the largest reductions were in Southern Europe for nearly all modalities. In March 2020, location in Southern Europe was the only independent predictor of the reduction in procedure volume. However, in April 2020, lower gross domestic product and higher COVID-19 deaths were the only independent predictors.ConclusionThe first wave of the COVID-19 pandemic had a significant impact on care of patients with cardiac disease, with substantial regional variations in Europe. This has potential long-term implications for patients and plans are required to enable the diagnosis of non-COVID-19 conditions during the ongoing pandemic.

BackgroundThe coronavirus disease-2019 (COVID-19) pandemic significantly affected management of cardiovascular disease around the world. The effect of the pandemic on volume of cardiovascular diagnostic procedures is not known.ObjectivesThis study sought to evaluate the effects of the early phase of the COVID-19 pandemic on cardiovascular diagnostic procedures and safety practices in Asia.MethodsThe International Atomic Energy Agency conducted a worldwide survey to assess changes in cardiovascular procedure volume and safety practices caused by COVID-19. Testing volumes were reported for March 2020 and April 2020 and were compared to those from March 2019. Data from 180 centers across 33 Asian countries were grouped into 4 subregions for comparison.ResultsProcedure volumes decreased by 47% from March 2019 to March 2020, showing recovery from March 2020 to April 2020 in Eastern Asia, particularly in China. The majority of centers cancelled outpatient activities and increased time per study. Practice changes included implementing physical distancing and restricting visitors. Although COVID testing was not commonly performed, it was conducted in one-third of facilities in Eastern Asia. The most severe reductions in procedure volumes were observed in lower-income countries, where volumes decreased 81% from March 2019 to April2020.ConclusionsThe COVID-19 pandemic in Asia caused significant reductions in cardiovascular diagnostic procedures, particularly in low-income countries. Further studies on effects of COVID-19 on cardiovascular outcomes and changes in care delivery are warranted.

Objectives: PET imaging of cardiac reporter gene expression holds promise for noninvasive monitoring of gene therapy. Wild-type HSV1-tk was imaged with 2´-fluoro-2´-deoxy-5[124I]-iodo-arabinofuranosyluracil([124I]FIAU), and mutant HSV1-sr39tk was imaged with 9-[4-[18F]-fluoro-3-(hydroxymethylbutyl)guanine([18F]FHBG). Methods: Adenovirus expressing wild-type HSV1-tk(Adtk), mutant HSV1-sr39tk (Adsr39tk), or control gene (AdLacZ) were directly injected into myocarcardium of 24 rats and 8 pigs. Two days later, dynamic PET imaging was performed for 120 min after injection of [124I]FIAU or [18F]FHBG with a clinical scanner. Imaging with [13N]-ammonia was performed to identify cardiac regions of interest. Pig hearts were sliced into 5 short-axis slices for ex vivo imaging, and regional tracer uptake was analyzed. Results: For [124I]FIAU, the largest difference of cardiac uptake between Adtk rats and controls was found at 10-30 min after injection (1.7±0.25 vs 0.87±0.22 %dose/ml, p=0.035). No difference was observed at later imaging times due to [124I]FIAU washout. For [18F]FHBG, difference between Adsr39tk rats and controls continuously increased over time and was largest at 105-120 min (1.45±0.45 vs 0.35±0.05 %dose/ml, P=0.0066). Global cardiac reporter probe kinetics in rats was confirmed by regional myocardial analysis in pigs. Transgene expression was specifically visualized by both approaches. Highest target/background ratio of [124I]FIAU in Adtk infected myocardium was 1.50±0.20 vs 2.64±0.49 for [18F]FHBG in Adsr39tk infected areas (P=0.01). In vivo results of rats were confirmed by ex vivo counting and autoradiography. In vivo segmental reporter probe uptake in pigs correlated well with those in ex vivo images. Conclusions: Both combinations were feasible for PET of cardiac transgene expression in different species. Specific probe kinetics suggests different myocardial handling of pyrimidine ([124I]FIAU) and acycloguanosine ([18F]FHBG) derivatives. Results are in favour of [18F]FHBG because of continuous accumulation over time and higher imaging contrast.

Background: PET imaging of reporter gene expression holds promise for noninvasive monitoring of gene therapy. Previously, two approaches based on the herpes simplex virus type 1 thymidine kinase gene(HSV1-tk) have been applied to the heart. Wild-type HSV1-tk was imaged with [124I]-fluoro-deoxy-iodo-arabinofuranosyluracil(FIAU) and mutant HSV1-sr39tk with [18F]-fluoro-hydroxymethylbutyl-guanine(FHBG), but a direct comparison has not yet been performed. Methods and Results: In H9c2 cardiac cells, in vitro FIAU accumulation was superior to FHBG following infection with adenovirus expressing wild-type HSV1-tk(Adtk), while FHBG uptake was superior to FIAU using adenovirus expressing mutant HSV1-sr39tk(Adtk). Subsequent in vivo studies employed dynamic PET two days after intramyocardial vector injection. FIAU was used after Adtk infection. Highest cardiac uptake compared to negative controls occurred at 10-30min after tracer injection. Specific uptake disappeared at later times due to FIAU washout. FHBG was used after Adsr39tk infection. Specific tracer uptake continuously increased over time and was highest vs controls at latest imaging times(105-120min). Global cardiac reporter probe kinetics in rats were confirmed by regional analyses in pigs. Transgene expression was specifically visualized by both approaches. The treatment of patients with diffuse large B cell lymphoma (DLBCL) would be greatly facilitated with a rapid method for determining prognosis that can be performed more easily and earlier than cytological or specific pathological examinations. It has been suggested that newly diagnosed patients with DLBCL who have low maximum standard uptake value (SUV(max)) on (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET) are more likely to be successfully treated and remain in remission compared with patients with high SUV(max), but this concept has been poorly studied. We retrospectively analyzed 50 patients with de novo DLBCL to evaluate the relationship between the SUV(max) and disease progression. For patients with low SUV(max) (n = 10) and high SUV(max) (n = 40) (P = 0.255), respectively, the 3-year overall survival rates were 90 and 72 %, and the progression-free survival (PFS) rates were 90 and 39 % (P = 0.012). By multivariate analysis, the revised International Prognostics Index (R-IPI) and SUV(max) at diagnosis were shown to predict longer PFS. The 3-year PFS for patients with low SUV(max) classified into the good prognosis group by R-IPI was 100 vs. 62 % for those with high SUV(max) (P = 0.161), and patients with low SUV(max) classified into the poor prognosis group by R-IPI was 80 vs. 18 % for those with high SUV(max) (P = 0.050). We conclude that the SUV(max) on FDG-PET for newly diagnosed patients with DLBCL is an important predictor of disease progression, especially for patients with poor prognosis by R-IPI. - Abstract
There is a certain number of patients with so-called ski-slope hearing loss, in which there is good hearing for lower frequencies in spite of little/no hearing in high frequencies. EAS (electric-acoustic stimulation) has recently been introduced for such patients with residual hearing at lower frequencies. Ski-slope hearing loss can have either a progressive nature or can be rather stable; therefore, decisions regarding timing of surgery are sometimes hampered. One advantage of genetic testing is that the possible prognosis for hearing, i.e. progressive or not, can be predicted for individual patients. The present study was performed to estimate the frequency of ski-slope hearing loss and investigate the genetic background of candidates for EAS. Using a 2587 subject DNA database of sensorineural hearing loss patients, 1) frequency of patients with ski-slope hearing loss, 2) their clinical features including inheritance mode, onset ages, and progression, and 3) involvement of four common genes with mutations in Japanese hearing loss patients, were evaluated. One hundred and fifty-one out of 2587 subjects fulfilled the audiological criteria for EAS. The frequency of patients possibly meeting the criteria for EAS was estimated to be 9.1% by restriction to probands only (139/1520). Various inheritance modes and onset ages were noted, with earlier onset in the patients with sporadic/recessive inheritance mode. Progressiveness was recognized in 56% of the patients. Genetic analysis identified mutations in 26.6% of the patients, including the mitochondrial 1555A>G mutation, and mutations in SLC26A4 , CDH23 , and GJB2 genes, suggesting that at the least, these four genes may be involved in a certain group of patients, but also leaving possible genetic causes in the majority of the patients undetermined. A 53-year-old male with fluctuating low frequency sensorineural hearing loss and tinnitus, but without vertigo, was evaluated by MRI obtained by intratympanic injection of a gadolinium-based contrast agent (GBCA) before and after the administration of isosorbide. The endolymphatic hydrops was semi-quantitatively evaluated by a 3.0-T MR scanner. For quantification, the affected side/contralateral side ratios were calculated. A gadodiamide (a kind of GBCA)-enhanced space surrounding the endolymph in the affected side with a 0.50 ratio (which may have represented endolymphatic hydrops) improved after isosorbide therapy to a 0.98 ratio. Thus, endolymphatic hydrops was demonstrated in a patient with 'atypical' Meniere's disease (MD), suggesting that at least some atypical MD may share similar etiology with, and therefore be a continuum of, MD. Also, therapeutic effects could be visualized by using MRI. Therefore, MRI-based diagnosis of MD-related disease will be a powerful tool not only because of its precision but also its usefulness for therapeutic evaluation. [ABSTRACT FROM AUTHOR]

Objective: The significant roles of brown adipose tissue (BAT) in the regulation of energy expenditure and adiposity are established in small rodents but have been controversial in humans. The objective is to examine the prevalence of metabolically active BAT in healthy adult humans and to clarify the effects of cold exposure and adiposity.Research Design and Methods: In vivo 2-[(18)F]fluoro-2-deoxyglucose (FDG) uptake into adipose tissue was measured in 56 healthy volunteers (31 male and 25 female subjects) aged 23-65 years by positron emission tomography (PET) combined with X-ray computed tomography (CT).Results: When exposed to cold (19 degrees C) for 2 h, 17 of 32 younger subjects (aged 23-35 years) and 2 of 24 elderly subjects (aged 38-65 years) showed a substantial FDG uptake into adipose tissue of the supraclavicular and paraspinal regions, whereas they showed no detectable uptake when kept warm (27 degrees C). Histological examinations confirmed the presence of brown adipocytes in these regions. The cold-activated FDG uptake was increased in winter compared with summer (P < 0.001) and was inversely related to BMI (P < 0.001) and total (P < 0.01) and visceral (P < 0.001) fat areas estimated from CT image at the umbilical level.Conclusions: Our findings, being against the conventional view, indicate the high incidence of metabolically active BAT in adult humans and suggest a role in the control of body temperature and adiposity. We present a case of sarcomatoid renal cell carcinoma (RCC). Although preoperative computed tomography (CT) scans demonstrated small lung and pleural nodules and mediastinal lymphadenopathy, these findings were not conclusive for metastases. Whole-body fluorine-18 fluorodeoxyglucose positron emission tomography and CT (FDG-PET/CT) performed 15 days after right nephrectomy showed intense FDG uptake in the aforementioned lesions (lung and pleural nodules, mediastinal lymph nodes). Unexpectedly, focal increased FDG uptake was found in the right infraspinatus muscle. FDG-PET/CT was considered useful for evaluating distant metastases and thus portending the aggressive nature of sarcomatoid RCC. [ABSTRACT FROM AUTHOR]

Background: Myocardial viability in area at risk of acute myocardial infarction (AMI) after reperfusion therapy may be underestimated by the 24-hour images due to reverse redistribution (r-RD).Methods: Subjects were 37 AMI patients in whom Tc-99m pyrophosphate (PYP)/Tl-201 dual-isotope SPECT was positive. The 24-hour delayed scan was performed with only a Tl window. One month later, follow up rest Tl SPECT was performed to evaluate myocardial viability. In early (at PYP/Tl-201 dual-isotope SPECT), 24-hour, and one month follow up Tl studies, Tl uptake in the area of AMI was scored into four grades: 3 as normal to 0 as severely reduced. The scores were evaluated.Results: Among the 37 AMI lesions, there were 16 r-RD, 3 RD, 16 fixed defect (FD) and 2 normal (positive PYP and normal Tl). Mean Tl scores were early; 1.4 +/- 1.1, 24-hr; 0.9 +/- 0.9 and one month; 1.3 +/- 1.1. The 24-hour Tl score was lower than the early and one month Tl scores (p < 0.01).Conclusion: Reverse redistribution is frequently observed in an area at risk where PYP SPECT was positive. Nuclear medicine physicians should be aware of the existence of frequent r-RD in Tl scan to avoid the underestimation of myocardial viability in the acute phase after PTCA.
