13. Sufficient Sleep Duration in Autistic Children and the Role of Physical Activity

- Abstract
Emerging evidence suggests that physical activity may be associated with improved sleep in autistic children. We aimed to determine whether physical activity associated with sufficient sleep duration in children and whether this association was modified by reported autism spectrum disorder (ASD) status. We analyzed existing data of children 6-17 years old whose caregivers completed the 2018 National Survey of Children's Health (nonautistic N = 20,845; ASD N = 681). Logistic regression determined whether physical activity (days active in the past week) associated with sufficient sleep duration. Physical activity-by-ASD interactions were included to determine whether the association differed for autistic children. Physical activity-by-ASD-by-sex and physical activity-by-ASD-by-age-group interactions were also modeled. Physical activity was associated with increased odds of sufficient sleep duration (e.g. 0 days vs 4-6 days: odds ratio (OR) = 1.85; 95% confidence interval (CI): 1.48-2.32). We did not observe an overall statistically significant interaction between physical activity and reported ASD status; however, the positive association between physical activity and sufficient sleep duration was weaker in autistic children, especially those with more severe ASD, female autistic children, and autistic children ages 6-12 years old. In conclusion, physical activity is a promising approach to improve sufficient sleep duration but with nuanced findings in autistic children.

24. Value chain analysis in pro-poor tourism : towards a critical understanding of the contribution of tourism to poverty reduction

Each year over half a billion tourists cross an international border and visit a developing country. Can this huge movement of some of the richest people visiting some of the most economically fragile countries have a positive impact on the lives of low-income people living there? Addressing this question has been an enduring aim of the 'tourism and development' field over the past thirty years. This PhD by publication is based upon the author's research applying value chain analysis in pro-poor tourism and the resonance of this work with academics, donors, industry and Southern governments. The author's contribution was the conceptual, methodological and empirical approaches to pro-poor tourism. Conceptually, this involved introducing market development approaches to the 'tourism and development' discourse together with a more nuanced understanding of pro-poor growth in the context of private sector development. Methodologically, the author advanced the value chain framework - an established approach in agricultural and industrial sectors - to be applied systematically in the tourist sector for the first time. The value chain framework itself was also developed to more explicitly take account of developmental concerns. Significant effort was made to use this framework to move beyond an analysis of the current tourism value chain to provide an analytical basis to propose pro-poor change - and train others in the application of this approach. The author then employed this new approach to collect and analyse empirical data from diverse destinations in ten developing countries. From this analysis, some preliminary answers to the question above begun to emerge, and these findings were used to advocate for change in tourism policy and practice in economically fragile countries. Making full use of open source platforms to disseminate findings to a wide range of stakeholders, the author used the empirical data generated to contribute to tourism and development debates. Evidence is presented that the authors work resonated amongst diverse stakeholders, from development agencies to Southern Governments, tourism corporates and also academic researchers. This thesis provides an overview of my conceptual contribution to pro-poor tourism and questions the theoretical basis of critics of this approach. The methodology for measuring the impact of tourism is explained and the rationale for using tourism value chains is outlined. The question of research uptake - did any of this work have any discernible impact over the past decade - is then critically analysed.

30. Religious melancholia and the York Retreat, 1730-1830

This thesis will examine the hitherto understudied area of Georgian Quaker spirituality, and bring it to bear on the historically significant York Retreat asylum. The York Retreat opened in 1796 to serve the Quaker community. It was managed by the Tukes, an influential family of Quaker religious leaders. Georgian Quakers had a rich and idiosyncratic introspective tradition and spiritual life. Their spirituality entailed a depressive piety which escalated to despair, restrictive eating or suicidality in several narratives from Georgian Quaker religious leaders. It was common for Georgian Quakers to interpret these episodes of affliction from the perspective on religious melancholia common to several other radical dissenting movements, in which such episodes were seen as a divinely ordained trial that would ultimately add to the gravity and authority of the afflicted, and prepare them for a religiously orientated life. Yet this concern with religious melancholia has escaped the notice of previous writers on the York Retreat. The thesis will first examine Quaker worship, the cornerstone of Georgian Quaker practical theology. It will then show how religious doubt, despair and affliction were intrinsic and causally efficacious parts of Georgian Quaker narratives of spiritual progress, before examining accounts of religious distress in Quaker biography and at the York Retreat. This thesis therefore provides an alternative narrative on the early years of the York Retreat. The York Retreat will not be approached as a site of innovation in secular humanistic psychiatry, but as a relic of dissenting modes of experiential religion and religious melancholia. In so doing, the thesis will show that assumptions around a link between Quaker spirituality and the ‘moral treatment’ regime are unfounded; the liberal humanism of contemporary Quakerism has been imputed onto the history of the York Retreat by supporters and critics alike. Instead, it will be shown that Quakers gradually incorporated narratives of nervous affliction into their accounts of religious affliction, reflecting the longrunning embodied aspect of religious distress, at a time when it was not unheard of for the devout to be supported in religious reconciliation and bodily healing from within a madhouse.

The black jewfish (Protonibea diacanthus) occurs in tropical coastal waters throughout the central Indo‐Pacific. It has long been valued as an important recreational and artisanal fishery species but has become increasingly targeted by commercial fisheries due to demand for its large swim bladder. To better understand how changes in fishing pressure may impact the sustainable exploitation of P. diacanthus populations throughout Eastern Australia, we evaluated the reproductive biology of the species across two management regions in Central Queensland. Reproductive characteristics studied included the size at maturity, fecundity, spawning mode, and season. Spawning periodicity was evaluated throughout the two major management regions and revealed an increase in the gonadosomatic index during the early austral spring, followed by evidence of spawning occurring from September through March with a peak from September to November. Females were found to produce ∼4.5 million ± 1.4 million oocytes (mean ± SE) per batch. Spawning periodicity did not vary latitudinally but was found to differ from other regions in northern Australia. The present study provides reliable maturity and fecundity information to improve future assessment and sustainable management of P. diacanthus. [ABSTRACT FROM AUTHOR]

Background: Transjugular intrahepatic portosystemic shunt (TIPS) is an important therapy for complications of portal hypertension but remains underutilised in regional settings. Aims: The aim of this study is to explore the demographics, indications, outcomes and complications in patients undergoing TIPS in two regional hepatology centres. Methods: Retrospective analysis was undertaken of all patients undergoing TIPS at two regional centres between January 2017 and March 2023. The primary outcome measures were efficacy and complications of TIPS. Patient demographics (such as age, baseline liver severity scores and aetiology of liver disease) and indications for TIPS are detailed. Results: Forty‐eight patients underwent TIPS. Median age was 56 years (interquartile range (IQR): 46–65). The most common indications for TIPS were refractory ascites (n = 17) and failure of secondary prophylaxis of variceal bleeding (n = 13). Cumulative survival at 3 months and 1 year was 93% and 77% respectively. There was no significant difference in outcomes based on TIPS indication. The median number of paracenteses in patients undergoing TIPS for refractory ascites 1 year pre‐ and post‐TIPS were 10 (IQR: 4.5–16) and 2 (IQR: 0–4) respectively (P < 0.001). There were no procedure‐related deaths. Inpatient management of liver disease complications had a mean cost of $32 874.67 (SEM: 7779) in 1 year pre‐TIPS compared with $12 304.70 (SEM: 3531.1) in 1 year post‐TIPS (P < 0.001). Conclusions: TIPS is a safe and effective treatment to reduce complications of portal hypertension and can be performed successfully in the regional setting. [ABSTRACT FROM AUTHOR]
