Background: In high-risk breast cancer patients with skin infiltration, the administration of a uniform dose to superficial tissues is fundamental in order to reduce local skin relapse. A personalized bolus may prevent the potential inadequate dose distribution of a standard bolus due to air gaps between the bolus and the skin. In this pilot study, we introduced into clinical practice the use of a personalized 3D-printed bolus filled with ultrasound transmission gel. Methods: Seven patients undergoing radiotherapy after mastectomy were selected. A 3D-printed bolus dosimetric assessment was performed with MOSFET dosimeters on an anthropomorphic phantom and, subsequently, on three selected cases with increasing bolus shape irregularity. Acute/late toxicity and local control were assessed. Results: Overall, for the clinical cases, the percentage median difference between the measured and calculated doses was −2.7% (−7.0–4.9%). The median follow-up was 21 months. After two years, one patient showed G2 pain, one patient manifested G1 telangiectasia, one patient showed G1 hyperpigmentation, and two patients had no relevant toxicity. Conclusions: A personalized 3D-printed bolus filled with ultrasound gel may easily reproduce the standard bolus’ consistency and provide accurate coverage of the target area with tolerable acute/late toxicity grades. This is a pilot study, and further investigations are needed.

CADASIL ,Parkinsonism ,Stroke ,Variant of uncertain significance ,Neurology. Diseases of the nervous system ,RC346-429 - Abstract
CADASIL (Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy) is a hereditary small vessel disease caused by mutations in the NOTCH3 gene, characterized by recurrent strokes, cognitive decline, and psychiatric symptoms. This report presents a novel NOTCH3 c.1564 T > A (p.Cys522Ser) mutation associated with early-onset parkinsonism and significant white matter lesions. We describe a patient who presented with early-onset parkinsonism, characterized by bradykinesia and rigidity, alongside extensive white matter lesions observed through neuroimaging. Genetic testing revealed a novel c.1564 T > A (p.Cys522Ser) mutation in the NOTCH3 gene, contributing to the clinical diagnosis of CADASIL. This case underscores the phenotypic variability of CADASIL and the potential for atypical presentations, including parkinsonism. Early identification of genetic mutations can facilitate appropriate management and counseling for affected individuals and their families. Further research is warranted to explore the mechanisms underlying the association between NOTCH3 mutations and parkinsonism. Our findings contribute to the understanding of CADASIL, suggesting that clinicians should consider CADASIL in differential diagnoses of early-onset parkinsonism, especially in patients with concurrent white matter lesions.

Trichinella spiralis ,Wolf (Canis lupus) ,Trichinellosis ,Italy ,Infectious and parasitic diseases ,RC109-216 - Abstract
Trichinella spiralis is a zoonotic nematode parasite of worldwide distribution. It is present in Europe with important foci, particularly in Eastern countries and Spain. This species is generally associated with a domestic cycle that involves primarily pigs. It is best adapted for pigs but can also infect a wide range of other domestic, synanthropic, and wild mammals including carnivores, omnivores and scavengers. Before 2016, when T. spiralis larvae were detected in a red fox (Vulpes vulpes) in the Piacenza province (Emilia Romagna region, Northern Italy), this parasite had only been reported in Italy occasionally, being found in horses or pork products imported from Eastern Europe. We describe here the first isolation of T. spiralis in a wolf (Canis lupus) in the Lazio region, Central Italy. In the wolf specimen T. spiralis was identified in coinfection with Trichinella britovi, a species endemic in Italian wildlife. Among the Trichinella species, T. spiralis is the most frequently associated with human disease in Europe and is known to cause more severe symptoms than T. britovi. In light of wolf population expansion, the detection of T. spiralis in Central Italy implies new scenarios for the risk of human trichinellosis because of the high risk this species represents for domestic and wild pigs. Active monitoring of wildlife living in these areas is necessary to define the actual distribution of this species and to detect its possible presence in other areas of the Italian peninsula.

Binge eating disorder (BED) is the most common eating disorder categorized in the DSM-V, but it is often not diagnosed in patients with obesity because it can be difficult to detect in these patients who often have altered eating patterns. In this narrative review, we have highlighted the most recent findings in the screening, diagnosis, and treatment of patients with BED and obesity. The results of our search showed that many BED patients are not obese, and most people with obesity do not have binge behavior. In the diagnostic assessment of these patients, it is important to evaluate not only the clinical and nutritional status and the presence of medical comorbidities, but also the psychological signs and symptoms related to psychiatric comorbidities to define the appropriate diagnosis and the consequent level of treatment. Well-tolerated drugs with action on both body weight and binges can be useful as a second-line complement to cognitive behavioral therapy (CBT). Specific guidelines are needed to obtain consensus on appropriate recommendations in patients with obesity and BED approaching bariatric surgery, taking into account not only weight reduction and clinical data, but also eating behaviors. Identification of BED is important for targeting individuals at high risk of obesity, adverse metabolic patterns, and cardiovascular disease. The challenge is to also achieve lasting weight loss in patients with BED and concomitant obesity.

Summary: Humans are the most versatile tool users among animals. Accordingly, our manual skills evolved alongside the shape of the hand. In the future, further evolution may take place: humans may merge with their tools, and technology may integrate into our biology in a way that blurs the line between the two. So, the question is whether humans can embody a bionic tool (i.e., experience it as part of their body) and thus if this would affect behavior. We investigated in virtual reality how the substitution of the hand with a virtual grafting of an end-effector, either non-naturalistic (a bionic tool) or naturalistic (a hand), impacts embodiment and behavior. Across four experiments, we show that the virtual grafting of a bionic tool elicits a sense of embodiment similar to or even stronger than its natural counterpart. In conclusion, the natural usage of bionic tools can rewire the evolution of human behavior.

Abstract Football has one of the highest incidence rates of mild traumatic brain injury (mTBI) among contact sports; however, the effects of repeated sub-concussive head impacts on brain structure and function remain under-studied. We assessed the association between biomarkers of mTBI and structural and functional MRI scans over an entire season among non-concussed NCAA Division I linemen and non-linemen. Concentrations of S100B, GFAP, BDNF, NFL, and NSE were assessed in 48 collegiate football players (32 linemen; 16 non-linemen) before the start of pre-season training (pre-camp), at the end of pre-season training (pre-season), and at the end of the competitive season (post-season). Changes in brain structure and function were assessed in a sub-sample of 11 linemen and 6 non-linemen using structural and functional MRI during the execution of Stroop and attention network tasks. S100B, GFAP and BDNF concentrations were increased at post-season compared to pre-camp in linemen. White matter hyperintensities increased in linemen during pre-season camp training compared to pre-camp. This study showed that the effects of repeated head impacts are detectable in the blood of elite level non-concussed collegiate football players exposed to low-moderate impacts to the heads, which correlated with some neurological outcomes without translating to clinically-relevant changes in brain anatomy or function.

This paper examines how spatial dynamics have impacted natural capital and the provision of ecosystem services. The units outlined by the National Strategy for Inland Areas (SNAI) have been used as the territorial units for this study. The SNAI is a public policy focused on enhancing the quality of services to citizens (such as transportation, healthcare, and education). It proposes the economic revitalization of inland areas undergoing processes of marginalization. Our focus on inland areas stems from two primary reasons: first, no previous studies in Italy have analyzed the changes in ecosystem services in SNAI areas; and second, SNAI areas are well-suited to providing ecosystem services that are in demand by urban areas. Although this study does not cover all aspects inherent to the topic, it represents a starting point aimed at understanding the links between environmental and socio-economic dynamics and ecosystem service changes. This is essential for both current and future generations. By analyzing the processes of permanence and transformation, modifications in the supply–demand balance have hereby been studied, as well as the economic variations in ecosystem services. The period considered runs from 1990 to 2018. These findings could help governmental institutions in developing sustainable governance models, in line with spatial policies and strategies.

The Giardia lamblia virus (GLV) is a non-enveloped icosahedral dsRNA and endosymbiont virus that infects the zoonotic protozoan parasite Giardia duodenalis (syn. G. lamblia, G. intestinalis), which is a pathogen of mammals, including humans. Elucidating the transmission mechanism of GLV is crucial for gaining an in-depth understanding of the virulence of the virus in G. duodenalis. GLV belongs to the family Totiviridae, which infects yeast and protozoa intracellularly; however, it also transmits extracellularly, similar to the phylogenetically, distantly related toti-like viruses that infect multicellular hosts. The GLV capsid structure is extensively involved in the longstanding discussion concerning extracellular transmission in Totiviridae and toti-like viruses. Hence, this study constructed the first high-resolution comparative atomic models of two GLV strains, namely GLV-HP and GLV-CAT, which showed different intracellular localization and virulence phenotypes, using cryogenic electron microscopy single-particle analysis. The atomic models of the GLV capsids presented swapped C-terminal extensions, extra surface loops, and a lack of cap-snatching pockets, similar to those of toti-like viruses. However, their open pores and absence of the extra crown protein resemble those of other yeast and protozoan Totiviridae viruses, demonstrating the essential structures for extracellular cell-to-cell transmission. The structural comparison between GLV-HP and GLV-CAT indicates the first evidence of critical structural motifs for the transmission and virulence of GLV in G. duodenalis.

The fifth edition of the WHO Classification of Tumors of the Central Nervous System (CNS), published in 2021, established new approaches to both CNS tumor nomenclature and grading, emphasizing the importance of integrated diagnoses and layered reports. This edition increased the role of molecular diagnostics in CNS tumor classification while still relying on other established approaches such as histology and immunohistochemistry. Moreover, it introduced new tumor types and subtypes based on novel diagnostic technologies such as DNA methylome profiling. Over the past decade, molecular techniques identified numerous key genetic alterations in CSN tumors, with important implications regarding the understanding of pathogenesis but also for prognosis and the development and application of effective molecularly targeted therapies. This review summarizes the major changes in the 2021 fifth edition classification of pediatric CNS tumors, highlighting for each entity the molecular alterations and other information that are relevant for diagnostic, prognostic, or therapeutic purposes and that patients’ and oncologists’ need from a pathology report.

Abstract To investigate the seasonal changes in physiological and psychological parameters of stress in collegiate swimmers. Fifteen NCAA Division I swimmers (8 men) participated in a tethered anaerobic swim test to determine physiological responses in an ecologically-relevant, graded exercise test. Wisconsin Upper Respiratory Symptom Survey (WURSS-21), Activation-Deactivation Adjective Check List (AD-ACL), Daily Analysis of Life Demands of Athletes (DALDA), and Pittsburgh Sleep Quality Index were assessed at post-season in April (V1), the end of off-season in June (V2), and pre-season in October (V3). The percent change was determined from V2–V1 (off-season phase), V3–V2 (pre-season phase), V1–V3 (in-season phase). Spearman’s rho correlation was used to examine associations between change in physiological and psychological outcomes. All data results showed a better swim performance occurred at V2. Men tended to have faster speed (p = 0.07) in fewer strokes (p = 0.10) and greater work per stroke (p = 0.10) at V2 than V1. Women were faster during V2 compared to V1 (p = 0.02) and V3 (p = 0.05). Women had fewer strokes (p = 0.02) and greater work per stroke (p = 0.01) at V2 compared to V3. Women had the lowest HR and lactate concentration at V3 compared to other visits (p

Late antique and early medieval documents often feature graphic symbols – graphic entities that may be composed of alphabetic or non-alphabetic signs – that communicate something different or something more than the written text. The NOTAE research project is concerned with the identification and classification of such graphic symbols, while also attempting to investigate the meaning they convey or take on in a definite context. This report illustrates the structure and operational functions of the open-access database, the main product of the project, in which graphic symbols, at the present more than six thousand, result catalogued, described and classified by type.

Introduction: To date, Emergency Department overcrowding represents one of the most important problems regarding the organisation of emergency healthcare. See and Treat was devised in England around the 1980s to provide an effective solution to the overcrowding issue. The Emergency Nurse Practitioner indicates a faster pathway in selected patients with certain characteristics. This model translates into a reduction in waiting times while maintaining the same quality of care. Aim: To evaluate the potential benefits for patients of the implementation of the See and Treat model within the Emergency Department. Materials and Methods: Using the GIPSE system, all Emergency Department entries from 2019 have been selected. Minor codes in the triage phase have been considered, as well as those potentially falling within the See and Treat protocol. Each included case has been associated to a See and Treat diagnosis. The waiting time and the time spent in the Emergency Department were derived from the data collected. Results: 9.41% of the sample is eligible for the See and Treat method, with an average waiting time of one hour and peaks of almost 4 hours for white codes. The application of the See and Treat model to the population covered by our study would reduce the waiting times at the Emergency Department by 6.99%, reduce the simultaneous presence of users on the Emergency Department by 8.88% and reduce the stay for other minor codes by 4.38%. Discussion: The percentage of patients treatable in the See and Treat system (11%) is in line with the Tuscany trial but statistically lower than the international context (from 63% to 90%). The results obtained from this study showed the significant impact that the See and Treat model could have on reducing overcrowding, positively affecting both users and staff.

Purpose: In our institution, stereotactic radiosurgery of multiple brain metastases is performed with the CyberKnife® (CK) device, using fixed/Iris collimators. In this study, nineteen fixed/Iris plans were recalculated with the multileaf collimator (MLC), to assess if it is possible to produce plans with comparable dosimetric overall quality. Materials and Methods: For consistent comparisons, MLC plans were re-optimized and re-normalized in order to achieve the same minimum dose for the total planning target volume (PTVtot). Conformation number (CN), homogeneity index (HI) and dose gradient index (DGI) metrics were evaluated. The dose to the brain was evaluated as the volume receiving 12 Gy (V12) and as the integral dose (ID). The normal tissue complication probability (NTCP) for brain radionecrosis was calculated as a function of V12. Results: The reoptimized plans were reviewed by the radiation oncologist and were found clinically acceptable according to the The American Association of Physicists in Medicine (AAPM) Task Group-101 protocol. However, fixed/Iris plans provided significantly higher CN (+8.6%), HI (+2.2%), and DGI (+44.0%) values, and significantly lower ID values (−35.9%). For PTVtot less than the median value of 2.58cc, fixed/Iris plans provided significantly lower NTCP values. On the other side, MLC plans provided significantly lower treatment times (−18.4%), number of monitor units (−33.3%), beams (−46.0%) and nodes (−21.3%). Conclusions: CK-MLC plans for the stereotactic treatment of brain multi metastases could provide an important advantage in terms of treatment duration. However, to contain the increased risk for brain radionecrosis, it could be useful to calculate MLC plans only for patients with large PTVtot.

The aim of this research is to study the bactericidal effects of copper–polypyrrole (PPy) composites deposited onto 316L stainless steel (SS) modified with silver nanoparticles (Np). The antimicrobial properties were evaluated against twenty-four strains of Gram-positive and Gram-negative bacteria. Among the twenty-four strains studied, isolates included reference strains (Escherichia coli ATCC 25922, Escherichia coli 0157:H7 EDL 933, Staphylococcus aureus ATCC 25923 and Listeria monocytogenes ATCC 7644), as well as strains isolated from food and clinical samples. The antimicrobial activity of the composites demonstrated that all PPy-modified films had antibacterial properties. Notably, Cu-PPyAgNp500 exhibited the strongest inhibitory activity against both Gram-negative and Gram-positive bacteria. Surface modification of 316L SS with these films is a promising and viable alternative for the development of novel antibacterial composites that can inhibit the growth of a significant number of bacteria.

The nematode Trichinella pseudospiralis is a cosmopolitan parasite capable of infecting both birds and mammals including humans. T. pseudospiralis has a limited zoonotic importance in comparison to that of the other Trichinella species. However, it has been recognized as the etiological agent of two outbreaks of trichinellosis due to the consumption of wild boar meat. The role played by birds in the epidemiology of T. pseudospiralis is still unclear and needs to be deepened. The aim of our work was to show the results of an extensive wild bird surveillance carried out in the Emilia-Romagna (E-R) region, northern Italy, over the last 16 years. As part of the regional wildlife surveillance program, 14,933 raptors and carrion-eating birds’ carcasses were necropsied from 2006 to 2021 and tested for the presence of Trichinella spp. larvae with only one positive result, an adult female of western marsh harrier. The larvae load (LPG) was evaluated on breast (100 LPG), wings (3.6 LPG), shoulder (2 LPG), head (4.5 LPG), thighs (8 LPG), lower legs (2 LPG) and tongue (0.77 LPG). The results of the present study confirm that also in northern Italy T. pseudospiralis has a low prevalence comparing to that of other Trichinella species. However, this study demonstrates that T. pseudospiralis can reach a high parasitic load in infested birds. The large distribution range, probably facilitated by its ability to infest birds, suggests the need for a continuous monitoring program.

The increasing importance of protected cultures in the last decades has led to the definition of aims for greenhouse design and functioning, in order to conjugate productivity and environmental sustainability. In particular, the optimization of greenhouses is finalized to better solar radiation adsorption and ensure to the crops a homogeneous temperature field for the whole growth time. An Italian case study is presented in this paper: the studied farm needs to project greenhouse geometry and design a hot-water pipes heating system to cultivate basil during the winter season, improving the exploitation of natural resources. A simulative analysis has been carried out by mean of COMSOL Multiphysics software, investigating the effects of greenhouse height, number of pipes and water temperature of the heating system on the inside temperature field through a parametric sweep approach. Twenty-seven scenarios have been simulated and three of them have been considered suitable for the farm applications. The simulative approach allows to collect data about the minimum number of pipes, the sensitivity of the internal temperature field to the outside temperature fluctuation (referred also to the height of the greenhouse) and the more suitable heating water temperature needed to ensure the required habitat to the basil crops, minimizing also temperature fluctuations.

Organoids offer a powerful model to study cellular self-organisation, the growth of specific tissue morphologies in-vitro, and to assess potential medical therapies. However, the intrinsic mechanisms of these systems are not entirely understood yet, which can result in variability of organoids due to differences in culture conditions and basement membrane extracts used. Improving the standardisation of organoid cultures is essential for their implementation in clinical protocols. Developing tools to assess and predict the behaviour of these systems may produce a more robust and standardised biological model to perform accurate clinical studies. Here, we developed an algorithm to automate crypt-like structure counting on intestinal organoids in both in-vitro and in-silico images. In addition, we modified an existing two-dimensional agent-based mathematical model of intestinal organoids to better describe the system physiology, and evaluated its ability to replicate budding structures compared to new experimental data we generated. The crypt-counting algorithm proved useful in approximating the average number of budding structures found in our in-vitro intestinal organoid culture images on days 3 and 7 after seeding. Our changes to the in-silico model maintain the potential to produce simulations that replicate the number of budding structures found on days 5 and 7 of in-vitro data. The present study aims to aid in quantifying key morphological structures and provide a method to compare both in-vitro and in-silico experiments. Our results could be extended later to 3D in-silico models.

The species of the genus Trichinella are etiological agents distributed all over the world and are able to infect mammals, birds, and reptiles. Trichinella spiralis is the species most adapted to domestic and wild pigs and is also the most important etiological agent of trichinellosis. The wild boar (Sus scrofa) is a nocturnal omnivorous mammal belonging to the Suidae family. S. scrofa has a great appetite and its diet includes a variety of small prey such as mice, rats, and other rodents, as well as carcasses of larger animals. The aim of this study was the identification and the molecular characterization of Trichinella larvae isolated from the muscle tissue of S. scrofa specimens collected in different counties of Romania. The muscle samples were examined by artificial digestion and the larvae identified at the species level by multiplex PCR. T. spiralis, a species that is able to infect a considerable number of different host species including humans, was identified. In Romania, S. scrofa is an important reservoir species for T. spiralis and plays an important role in linking the domestic and the wild cycle of Trichinella, with serious repercussions for human health.

About one-third of stroke survivors present unilateral spatial neglect (USN) that negatively impacts the rehabilitation outcome. We reported the study protocol and usability results of an eye-tracking (ET) biofeedback immersive virtual reality (iVR) protocol. Healthy controls and stroke patients with and without USN underwent a single session of the three iVR tasks. The system usability scale (SUS), adverse events (AEs), and ET data were collected and analyzed via parametric analysis. Twelve healthy controls (six young adults and six older adults) and seven patients with a diagnosis of single ischemic stroke (four without USN and three with confirmed diagnosis of USN) completed the usability investigation. SUS results showed good acceptability of the system for healthy controls and stroke patients without USN. ET results showed a lower performance for patients with USN concerning healthy controls and stroke patients without USN, in particular in the exploration of the left visual field. The results showed that the proposed iVR-ET biofeedback protocol is a safe and well-tolerated technique in patients with USN. The real-time feedback can induce a performance response supporting its investigation such as a treatment approach.

A rank-size rule following Zipf’s law was tested along a complete urban–rural hierarchy in Greece using 2021 census data released at different administrative levels. Testing five econometric specifications (linear, quadratic, and cubic forms, together with refined logistic and Gompertz forms) on log-transformed population numbers, deviations from the rank-size rule were assumed as an indicator of economic concentration (considering settlements, population, and activities jointly) along the density gradient in Greece. This hypothesis was verified using progressively disaggregated population numbers at (i) regional units (n = 75), (ii) ‘Kallikratis’ municipalities (n = 333), (iii) ‘Kapodistrian’ municipalities (n = 1037), and (iv) local communities (n = 6126). Econometric results were stable across geographical levels and indicate a relatively poor fit of linear specifications, the classical formulation of Zipf’s law. Quadratic specifications displayed a good fit for all territorial levels outperforming cubic specifications. Gompertz specifications outperformed logistic specifications under aggregate partitions (e.g., regional units and ‘Kallikratis’ municipalities). Quadratic specifications outperformed both logistic and Gompertz specifications under disaggregated levels of investigation (‘Kapodistrian’ municipalities and local communities). Altogether, these findings indicate the persistence of non-linear rank-size relationships estimated over a cross-section of population data at progressively detailed observational units. Such evidence enriches the recent literature on Zipf’s law, demonstrating the inherent complexity of rank-size rules tested on real data along the whole density gradient in a given country.

To delineate new directions of urban development in a context of demographic shrinkage in Southern Europe, the present study illustrates a comparative analysis of the demographic balance in metropolitan Athens, Greece (1956–2021). The analysis delineates short-term and long-term dynamics of the natural population balance, considering the impact of the Great Recession and COVID-19 pandemic, and assuming a contemporary increase in gross mortality rates and a marked fertility reduction associated with birth postponement. To address such objectives, we have compared the natural growth of population (the ratio of the total number of births to deaths) at ten year intervals (1956, 1960, 1970, 1980, 1990, 2000, 2009, 2019, 2020, 2021) in 115 municipalities of metropolitan Athens, controlling for the local context. The empirical results of descriptive statistics, spatial analysis, correlation statistics, non-parametric inference, and exploratory multivariate techniques outline the indirect impact of COVID-19 on population dynamics, being in some ways additive to the already observed effects of the Great Recession, reinforcing demographic shrinkage in specific local contexts. The COVID-19 pandemic and the Great Crisis likely accelerated the typical outcomes (population aging and low fertility) of the second demographic transition in Greece. These dynamics are associated with more volatile (and possibly reduced) immigration flows and with enhanced emigration, fueling urban shrinkage and a progressive economic decline of metropolitan regions, as our evidence suggests for Athens. Additional research should ascertain the aggregate, indirect role of pandemics in population dynamics as a proxy of urban and regional decline in European regions exposed to long-term aging.
