Research on the relationship between obstructive sleep apnea and cognitive functioning has yielded conflicting results, particularly in the older population, and moderators of this association have rarely been studied. Here we investigated the cross-sectional association between obstructive sleep apnea and cognitive functioning as well as the moderating effect of age, sex, apolipoprotein E4, and obesity on this association among community-dwelling older people. We analysed data from 496 participants (71.4 ± 4.4 years; 45.6% men) of the HypnoLaus study who underwent polysomnography and a battery of neuropsychological tests. The sample was categorised as no-to-mild obstructive sleep apnea (apnea-hypopnea index 0-14.9/h; reference), moderate obstructive sleep apnea (apnea-hypopnea index 15.0-29.9/h), or severe obstructive sleep apnea (apnea-hypopnea index ≥30/h). Regression and moderation analyses were performed with adjustment for confounders. Apolipoprotein E4 and obesity moderated the association between severe obstructive sleep apnea and processing speed, whereas no moderating effects were found for age and sex. In apolipoprotein E4 carriers only, severe obstructive sleep apnea was associated with lower performance in Stroop condition 1 (B = 3.13, p = 0.024). In obese participants only, severe obstructive sleep apnea was associated with lower performance in Stroop condition 1 (B = 3.02, p = 0.025) and Stroop condition 2 (B = 3.30, p = 0.034). Severe obstructive sleep apnea was also associated with lower executive function in the whole sample according to Stroop condition 3 (B = 3.44, p = 0.020) and Stroop interference score (B = 0.24, p = 0.006). Our findings support associations of severe obstructive sleep apnea (but not moderate obstructive sleep apnea) with lower performance in processing speed and executive function in the older general population. Apolipoprotein E4 and obesity appear to be vulnerability factors that strengthen the association between severe obstructive sleep apnea and lower performance in processing speed.

International audience; The long-term history of small and fragmented old-growth forests can provide insights into the legacies of past environmental changes and land-use history in the surrounding landscapes, which can support nature protection and restoration ecology measures.

The blood-brain barrier (BBB) is important to maintain brain homeostasis, which is crucial for the functionality of neuronal networks. BBB dysfunction is observed in several neurological diseases, including epilepsy. Prolonged BBB dysfunction and subsequent perivascular inflammation as well as alterations in the extracellular matrix (ECM) play an important role in epileptogenesis. In this chapter we aim to give an overview of the processes involved in BBB dysfunction and epileptogenesis and provide evidence from animal as well as from human studies. We will focus on modulators of BBB function including specific cell types, inflammatory mediators as well as ECM molecules. Furthermore, we will discuss new therapeutic targets and the results of intervention studies that are important for the development of novel therapeutic approaches in epilepsy.

BACKGROUND The objective of this study was to describe the determinants of adequacy and positivity of the p16/Ki-67 assay in a human papillomavirus (HPV)-positive screening population enrolled within the New Technologies for Cervical Cancer 2 (NTCC2) study. METHODS ThinPrep slides were immunostained for p16/Ki-67; each slide had 3 reports from different laboratories. The authors included population-related, sampling-related/staining-related, and interpretation-related variables in the analyses. Adequacy and positivity proportions were stratified by variables of interest. Univariate and multivariate logistic models were used to identify determinants of adequacy and positivity. RESULTS In total, 3100 consecutive HPV-positive cases were analyzed. Because every slide was interpreted by 3 centers, 9300 reports were obtained, including 905 (9.7%) that were inadequate and 2632 (28.3%) that were positive. The percentage of cases in which all 3 reports were inadequate increased with increasing age of the women and with inadequate cytology. The highest percentage of adequacy in all 3 reports and of cases with all 3 reports positive was observed in specimens from women who had grade ≥2 cervical intraepithelial neoplasia (CIN2+), atypical squamous cells of undetermined significance or more severe (ASC-US+) cytology, or mRNA positivity. The number of inadequate reports was significantly associated with increasing age, inadequate cytology, mRNA negativity, and scant cellularity. A positive p16/Ki-67 report was associated with an ASC-US+ result and with a positive mRNA result in cases both with and without CIN2+ but was associated with an HPV type 16 and/or 18 infection only in CIN2+ cases. The presence of CIN2+ was strongly associated with dual staining positivity. CONCLUSIONS The interpretation of p16/Ki-67 results may be influenced by several different variables, all of which are part of the steps in the procedure, and by the characteristics of the screened population.

L'état de mal épileptique (EME) est la plus fréquente urgence neurologique après les accidents vasculaires cérébraux, avec des hauts taux de morbidité et mortalité (Coeytaux et al., 2000). Son traitement est basé sur une approche en trois étapes (Meiekord et al., 2010). Dans ce contexte, un EME ne répondant pas aux benzodiazépines (1er ligne de traitement) suivi par des médicaments antiépileptiques (2ème ligne de traitement) est appelé EME réfractaire. Pour cette condition, représentant entre le 23% et le 43% des EME (Novy et al., 201O; Holtkamp et al., 2005), les actuelles recommandations préconisent un traitement par coma pharmacologique (3ème ligne de traitement), malgré un faible niveau d'évidence (Rossetti et al., 2011). En effet, l'impact du coma pharmacologique sur l'issue clinique n'a pas encore été clairement établi. Récemment, deux études américaines (Kowalski et al., 2012; Hocker et al., 2013) et une étude suisse (Sutter et al., 2014), ont montré un effet potentiellement délétère de ce type de traitement. Cependant, ces études étaient limitées à des patients hospitalisés aux soins intensifs et les analyses n'étaient pas ajustées pour tous les facteurs pronostiques connus. Le but de notre travail, publié dans Critical Gare Medicine (Marchi et al., 2015), était d'évaluer l'impact spécifique du coma pharmacologique sur le pronostic des patients avec EME, sans limitations aux soins intensifs et avec un·ajustement plus attentif concernant les autres facteurs pronostiques. En utilisant notre registre prospectif des patients avec EME traités aux Centre Hospitalier Universitaire Vaudois, nous avons comparé l'issue clinique à la sortie de l'hôpital des patients traités avec ou sans coma pharmacologique (467 épisodes au total). Ensuite, nous avons utilisé une régression logistique multinomiale pour ajuster les résultats par les autres facteur pronostiques connus (âge, absence de crises épileptiques précédentes, étiologie potentiellement fatale, gravité clinique de l'EME, comorbidités). Nous · avons pu mettre ainsi en évidence que le traitement avec coma pharmacologique est associé avec une mauvaise issue clinique après un EME. De plus, nous avons pu po_ur la première fois montrer que cet effet est d'autant plus important chez les patients avec un EME de type partiel complexe au moment du traitement. Nos résultats suggèrent que l'utilisation du coma pharmacologique ne doit pas être indiscriminée dans l'EME réfractaire et qu'une évaluation de la situation clinique de base permet une optimisation son emploi.

The project considers the whole wood supply chain, from forests to wood yards, with a particular emphasis on new remote sensing technologies and geographical information systems. The report presents the major findings of the project and outlines some key issues for a better mobilisation of alpine timber ressoures.

According to the Italian Legislative Decree 152/06 of April 3rd, 2006, concerning the consolidated law governing environmental issues ("Testo Unico recante le Norme in Materia Ambientale"), the Contamination Threshold Value is defined as follows (art. 240, comma 1, letter b): "threshold values are the values of contamination of environmental matrix above which the characterization and specific risk analysis of the contaminated site, as described in the Appendix 5 at part four of the present law, becomes necessary. In the event the potentially polluted site is located in an area where geogenic or anthropogenic factors are responsible for exceeding the threshold values for some parameters, the background content of these parameters is assumed as threshold." In the last decades the increase of industrial activities and the massive use of pesticides in agriculture, enhanced the risk of soil contamination by pollutants. Among these, heavy metals, due to their persistence, accumulate in soils possibly reaching concentrations above regulatory contamination thresholds. An accurate knowledge of the "usual" background content of metals in soils is then necessary to assess their contamination state. Aim of this work is to map the concentration of topsoil heavy metals using a Scorpan Kriging approach. The approach combines the trend of metals concentrations, as derived from the 1:50,000 soil map, with geostatistical modeling of the stochastic, locally varying but spatially correlated component. The trend component is described in terms of varying local means, calculated accounting for soil pedo-geochemical affinity and dominant land use.

A link between seizure susceptibility, blood-brain barrier (BBB) failure, and the activation of peripheral white blood cells has been recently proposed. However, the molecular players involved in this cascade of events are unknown. We tested the hypothesis that immunosupression by splenectomy or lack of perforin, a downstream factor of natural killer (NK) and cytotoxic T cells, could reduce seizure onset.Pilocarpine was used to induce seizures in adult rats wild-type and perforin-deficient mice. Splenectomy was performed prior to pilocarpine injection. Seizure onset was evaluated by electroencephalography (EEG) and joint time-frequency analysis. Spleens from control and pilocarpine-treated groups were analyzed for anatomical changes and CD3+ cell content. BBB damage was assessed by measuring albumin parenchymal extravasation. Fluorescence-activated cell sorting (FACS) analysis was performed on spleen and brain tissue of wild-type and perforin-deficient mice treated, or not, with pilocarpine.Splenectomy significantly reduced seizure-associated mortality. Histologic analysis of the spleens exposed to pilocarpine revealed altered white and red pulp anatomy and an increase in CD3+ T cells. Onset of status epilepticus (SE) and mortality were significantly decreased in perforin-deficient mice. Pilocarpine significantly increased spleen NK 1.1 and CD8+ cell percentage; in contrast, the brain inflammatory cell profile remained unchanged at the time of pilocarpine SE. BBB damage was reduced in the perforin-deficient pilocarpine-treated mice.Immunosuppressant maneuvers such as splenectomy or lack of perforin decrease the onset or the severity of pilocarpine SE. Our results suggest that cytotoxic lymphocytes, and specifically the cytolytic factor perforin, may be key molecular players involved in the axis between peripheral intravascular inflammation and seizures.
