Abstract Introduction Tuberculosis (TB) is an important cause of morbidity and mortality among people living with HIV (PLHIV). Current WHO-recommended strategies for diagnosing TB among hospitalized PLHIV rely on symptom screening and disease severity to assess eligibility for urine lipoarabinomannan lateral flow (LF-LAM) and molecular testing. Despite these recommendations, autopsy studies show a large burden of undiagnosed TB among admitted PLHIV. The EXULTANT trial aims to assess the impact of an expanded screening strategy using three specimens (sputum, stool, and urine) for TB diagnosis among PLHIV admitted to hospitals in two high HIV and TB burden African countries. Methods This is a multicenter, pragmatic, individually randomized controlled trial conducted across eleven hospitals in Tanzania and Mozambique. Participants in the intervention arm will be tested with Xpert MTB/RIF Ultra® from expectorated sputum, stool, and urine samples, with additional urine LF-LAM testing in the first 24 h after hospital admission, irrespective of the presence of the symptoms. The control arm will implement the WHO standard of care recommendations. Hospitalized adults (≥ 18 years) with a confirmed HIV-diagnosis, irrespective of antiretroviral (ART) therapy status or presence of TB symptoms will be assessed for eligibility at admission. Patients with a pre-existing TB diagnosis, those receiving anti-tuberculosis therapy or tuberculosis preventive treatment in the 6 months prior to enrolment, and those transferred from other hospitals will not be eligible. Also, participants admitted for traumatic reasons such as acute abdomen, maternal conditions, scheduled surgery, having a positive SARS-CoV2 test will be ineligible. The primary endpoint is the proportion of participants with microbiologically confirmed TB starting treatment within 3 days of enrolment. Discussion The EXULTANT trial investigates rapid implementation after admission of a new diagnostic algorithm using Xpert MTB/RIF Ultra® in several non-invasive specimens, in addition to LF-LAM, in hospitalized PLHIV regardless of TB symptoms. This enhanced strategy is anticipated to detect frequently missed TB cases in this population and is being evaluated as an implementable and scalable intervention. 8. A Novel Nine-Level Quadruple Boost Inverter for Electric Vehicle Applications

Lithium cells are commonly arranged in series and parallel configurations to fulfil the specific voltage and current necessities within electric vehicles (EVs). Voltage and charge equalization methods are implemented to safeguard these lithium cells. However, integrating these equalization circuits introduces some degree of loss into the overall circuit, and as a precautionary measure, the battery pack disconnects in case of any cell malfunction. Switched-capacitor (SC) inverters have gained prominence as an alternative approach to address voltage requirements. This article introduces a novel voltage boost multilevel inverter tailored for EV applications. The proposed nine-level quadruple boost (NLQB) inverter boasts an intrinsic capability for capacitor self-voltage balancing, streamlining its operational efficiency. Remarkably, capacitor voltage balancing algorithms or supplementary sensor circuits are rendered unnecessary due to this intrinsic self-balancing feature. Capacitors interfaced with a voltage source generate output levels with an appropriate charge/discharge pattern. The resultant output waveform spans nine levels, achieving a fourfold magnitude boost. A comprehensive comparison encompassing various parts, such as the number of power electronics, gate driver circuits, and capacitors, is presented. Additionally, system parameters, including voltage gain, blocking voltage, and total standing voltage (TSV) are evaluated to underscore the merits of the proposed NLQB inverter in contrast to recently proposed inverters.

Abstract Background In 2020, the WHO-approved Molbio Truenat platform and MTB assays to detect Mycobacterium tuberculosis complex (MTB) and resistance to rifampicin directly on sputum specimens. This primary health care center-based trial in Mozambique and Tanzania investigates the effect of Truenat platform/MTB assays (intervention arm) combined with rapid communication of results compared to standard of care on TB diagnosis and treatment initiation for microbiologically confirmed TB at 7 days from enrolment. Methods The Tuberculosis Close the Gap, Increase Access, and Provide Adequate Therapy (TB-CAPT) CORE trial employs a pragmatic cluster randomized controlled design to evaluate the impact of a streamlined strategy for delivery of Truenat platform/MTB assays testing at primary health centers. Twenty-nine centers equipped with TB microscopy units were selected to participate in the trial. Among them, fifteen health centers were randomized to the intervention arm (which involves onsite molecular testing using Truenat platform/MTB assays, process process optimization to enable same-day TB diagnosis and treatment initiation, and feedback on Molbio platform performance) or the control arm (which follows routine care, including on-site sputum smear microscopy and the referral of sputum samples to off-site Xpert testing sites). The primary outcome of the study is the absolute number and proportion of participants with TB microbiological confirmation starting TB treatment within 7 days of their first visit. Secondary outcomes include time to bacteriological confirmation, health outcomes up to 60 days from first visit, as well as user preferences, direct cost, and productivity analyses. Ethics and dissemination TB-CAPT CORE trial has been approved by regulatory and ethical committees in Mozambique and Tanzania, as well as by each partner organization. Consent is informed and voluntary, and confidentiality of participants is maintained throughout. Study findings will be presented at scientific conferences and published in peer-reviewed international journals. Trial Registration US National Institutes of Health’s ClinicalTrials.gov, NCT04568954. Registered 23 September 2020.

Bombali virus (BOMV) is a novel Orthoebolavirus that has been detected in free-tailed bats in Sierra Leone, Guinea, Kenya, and Mozambique. We screened our collection of 349 free-tailed bat lungs collected in Côte d’Ivoire and Tanzania for BOMV RNA and tested 228 bat blood samples for BOMV antibodies. We did not detect BOMV-specific antibodies but found BOMV RNA in a Mops condylurus bat from Tanzania, marking the first detection of an ebolavirus in this country. Our findings further expand the geographic range of BOMV and support M. condylurus’ role as a natural BOMV host.

CT angiography might be a suitable procedure to avoid arterial puncture in combined intracavitary and interstitial brachytherapy for cervical cancer curatively treated with combined chemoradiation and brachytherapy boost. Data in the literature about this technique are scarce. We introduced this method and collected brachytherapy data from patients treated in our department between May 2021 and April 2024. We analyzed the applicator subtype, needle insertion (planned versus implanted), implanted depth and the role of CT angiography in selecting needle trajectories and insertion depths. None of the patients managed through this protocol experienced atrial puncture and consequent hemorrhage. Needle positions were accurately selected with the aid of CT angiography with proper coverage of brachytherapy targets and avoidance of organs at risk. CT angiography is a promising method for guiding needle insertion during interstitial brachytherapy.

15. Determination of 11 kinds of pesticide residues in liquid milk by dispersive solid phase extraction coupled with ultra performance liquid chromatography-tandem mass spectrometry

Objective: This study aimed to evaluate the safety of pesticide residues in liquid milk. Methods: A method for simultaneous determination of 11 kinds of pesticide residues by ultra-high-performance liquid chromatography-tandem mass spectrometry was established. The samples were extracted by acetonitrile, purified by C18 and PSA solid phase extractants, separated on a column Xbridge BEH C18 (100 mm×2.1 mm, 1.7 μm), determined in multi-reaction monitoring mode, and optimized for pre-treatment parameters using response surface method. Results: The optimal conditions for pretreatment were 10 mL acetonitrile, 50 mg C18 and 30 mg PSA. The linear relationships of 11 kinds of pesticides were good in the concentration range of 1.0~100.0 μg/L, and the correlation coefficients (R2) were all > 0.999. With addition of 10, 20, 100 μg/kg, the average recoveries were 82.7%~102.0%, the relative standard deviations were 2.89% to 8.87%, and the limits of detection (LOD) and limits of quantitation (LOQ) were 2.0~3.0 μg/kg and 5.0~10.0 μg/kg, respectively. Conclusion: The method is simple, convenient, rapid, high recovery rate and high reproducibility, which could accurately determine 11 kinds of pesticide residues in liquid milk.

Cosmic microwave background (CMB) experiments have paved the way towards a greater understanding of early universe physics and building a standard model of cosmology. In this dissertation, we report on the development, integration, testing, and deployment of the ultra-high-frequency small aperture telescope for the Simons Observatory (SO). We also discuss development and optimization of detector arrays for CMB experiments, and describe the SO low-frequency wafers in detail. This work also directly impacts developments for CMB-S4 and LiteBIRD. We also discuss the ongoing calibration and scan strategy efforts in SO as the experiment moves towards initial science observations.

Summary: Background: While vaccines have established utility against COVID-19, phase 3 efficacy studies have generally not comprehensively evaluated protection provided by previous infection or hybrid immunity (previous infection plus vaccination). Individual patient data from US government-supported harmonized vaccine trials provide an unprecedented sample population to address this issue. We characterized the protective efficacy of previous SARS-CoV-2 infection and hybrid immunity against COVID-19 early in the pandemic over three-to six-month follow-up and compared with vaccine-associated protection. Methods: In this post-hoc cross-protocol analysis of the Moderna, AstraZeneca, Janssen, and Novavax COVID-19 vaccine clinical trials, we allocated participants into four groups based on previous-infection status at enrolment and treatment: no previous infection/placebo; previous infection/placebo; no previous infection/vaccine; and previous infection/vaccine. The main outcome was RT-PCR-confirmed COVID-19 >7–15 days (per original protocols) after final study injection. We calculated crude and adjusted efficacy measures. Findings: Previous infection/placebo participants had a 92% decreased risk of future COVID-19 compared to no previous infection/placebo participants (overall hazard ratio [HR] ratio: 0.08; 95% CI: 0.05–0.13). Among single-dose Janssen participants, hybrid immunity conferred greater protection than vaccine alone (HR: 0.03; 95% CI: 0.01–0.10). Too few infections were observed to draw statistical inferences comparing hybrid immunity to vaccine alone for other trials. Vaccination, previous infection, and hybrid immunity all provided near-complete protection against severe disease. Interpretation: Previous infection, any hybrid immunity, and two-dose vaccination all provided substantial protection against symptomatic and severe COVID-19 through the early Delta period. Thus, as a surrogate for natural infection, vaccination remains the safest approach to protection. Funding: National Institutes of Health.

Background: Mobile Information and Communication Technologies have led to the rise in dependence on smartphones worldwide and nomophobia (NMP) which is the fear of being out of mobile phone contact. Objectives: The objectives of the study were to assess smartphones usage among the medical students, to estimate the prevalence of NMP, it's gradation and it's association of different variables with grade of NMP. Methods: A community-based, analytical cross-sectional study was conducted among medical students selected by the systematic random sampling technique. A questionnaire comprising smartphone ownership/usage variables and questions on NMP Questionnaire was used. Results: Female students (70.9%) were more compared to males (29.1%). Majority (44.7%) were in the age group of 19–20 years. 52.4% of them had received ≤3 E-mails/day. 56.4% of them had installed ≤20 apps in their smart phones in their smartphones. Sixty-eight percent of them had installed ≤3 newer apps in last month. The prevalence of NMP was found to be 100% with the maximum (63.1%) having moderate grade of NMP. It was found that the associations of the community, possession of smartphone, average monthly mobile phone bill including internet usage, and number of E-mails sent per day with the difference in the grade of NMP were found to be statistically significant, (P = 0.04), (P = 0.04), (P = 0.01), and (P = 0.04), respectively. Conclusions: The high prevalence of NMP can be attributed to the use of smartphones for online classes due to the COVID-19 pandemic.

The Simons Observatory (SO) is a cosmic microwave background survey experiment that includes small-aperture telescopes (SATs) observing from an altitude of 5200 m in the Atacama Desert in Chile. The SO SATs will cover six spectral bands between 27 and 280 GHz to search for primordial B-modes to a sensitivity of σ ( r ) = 0.002, with quantified systematic errors well below this value. Each SAT is a self-contained cryogenic telescope with a 35° field of view, 42 cm diameter optical aperture, 40 K half-wave plate, 1 K refractive optics, and 12,000 transition edge sensor detectors. We describe the nominal design of the SATs and present details about the integration and testing for one operating at 93 and 145 GHz.

Weight Fair Queuing is an ideal scheduling algorithm to guarantee the bandwidth of different queues according to their configured Weights when the switching nodes of the network are congested. Many of the switching nodes based on FPGA in the current network support four physical ports or hundreds of virtual ports. Massive logic and storage resources would be consumed if each port implemented a WFQ scheduler. This paper proposes a Queue-Group-Based WFQ Scheduler (QGWFQS), which can support WFQ scheduling across multiple ports through the reuse of tag calculation and encoding circuits. We also propose a novel finish tag calculation algorithm to accommodate the variation in the link rate of each port. The remainder of integer division is also taken into account, which makes the bandwidth allocation fairer. Experimental results show that the proposed scheduler supports up to 512 ports, with 32 queues allocated on each individual port. The scheduler has the capability to operate at 200 MHz and the total scheduling capacity reaches 200 Mpps.

Abstract A systematic study on the biological effects of simulated microgravity (sµg) on human pluripotent stem cells (hPSC) is still lacking. Here, we used a fast-rotating 2-D clinostat to investigate the sµg effect on proliferation, self-renewal, and cell cycle regulation of hPSCs. We observed significant upregulation of protein translation of pluripotent transcription factors in hPSC cultured in sµg compared to cells cultured in 1g conditions. In addition to a significant increase in expression of telomere elongation genes. Differentiation experiments showed that hPSC cultured in sµg condition were less susceptible to differentiation compared to cells in 1g conditions. These results suggest that sµg enhances hPSC self-renewal. Our study revealed that sµg enhanced the cell proliferation of hPSCs by regulating the expression of cell cycle-associated kinases. RNA-seq analysis indicated that in sµg condition the expression of differentiation and development pathways are downregulated, while multiple components of the ubiquitin proteasome system are upregulated, contributing to an enhanced self-renewal of hPSCs. These effects of sµg were not replicated in human fibroblasts. Taken together, our results highlight pathways and mechanisms in hPSCs vulnerable to microgravity that imposes significant impacts on human health and performance, physiology, and cellular and molecular processes.

Summary: Background: Triage implementation in resource-limited emergency departments (EDs) has traditionally relied on intensive in-person training. This study sought to evaluate the impact of a novel digital-based learning strategy focused on the Interagency Integrated Triage Tool, a three-tier triage instrument recommended by the World Health Organization. Methods: A mixed methods study utilising pre-post intervention methods was conducted in two EDs in Papua New Guinea. The primary outcome was the mean change in knowledge before and after completion of a voluntary, multimodal training program, primarily delivered through a digital learning platform accessible via smartphone. Secondary outcomes included the change in confidence to perform selected clinical tasks, and acceptability of the learning methods. Findings: Among 136 eligible ED staff, 91 (66.9%) completed the digital learning program. Bombali virus (BOMV) is a novel Orthoebolavirus that has been detected in free-tailed bats in Sierra Leone, Guinea, Kenya, and Mozambique. We screened our collection of 349 free-tailed bat lungs collected in Côte d'Ivoire and Tanzania for BOMV RNA and tested 228 bat blood samples for BOMV antibodies. We did not detect BOMV-specific antibodies but found BOMV RNA in a Mops condylurus bat from Tanzania, marking the first detection of an ebolavirus in this country. Our findings further expand the geographic range of BOMV and support M. condylurus' role as a natural BOMV host. [ABSTRACT FROM AUTHOR]

Hydrocephalus is a disorder of abnormal accumulation of cerebrospinal fluid (CSF) in the ventricular system. Accumulation of CSF occurs due to an imbalance between CSF production and absorption. In the new-born and infants, hydrocephalus almost entirely presents as an enlarged head resulting from the separation of cranial bone plates, so any new-born or infant with an enlarged head should undergo evaluation. We report the case of a gross hydrocephalus in a 9-month-old infant (head circumference 55 cm) posted for ventriculoperitoneal shunt surgery with anaesthetic consideration of difficult intubation because of such a large head size, positioning, age-related pathophysiology and temperature regulation. The case was managed successfully with an uneventful hospital stay, and a significant decrease in head size was noted.

Electronic clinical decision support algorithms (CDSAs) have been developed to address high childhood mortality and inappropriate antibiotic prescription by helping clinicians adhere to guidelines. Previously identified challenges of CDSAs include their limited scope, usability, and outdated clinical content. To address these challenges we developed ePOCT+, a CDSA for the care of pediatric outpatients in low- and middle-income settings, and the medical algorithm suite (medAL-suite), a software for the creation and execution of CDSAs. Following the principles of digital development, we aim to describe the process and lessons learnt from the development of ePOCT+ and the medAL-suite. In particular, this work outlines the systematic integrative development process in the design and implementation of these tools required to meet the needs of clinicians to improve uptake and quality of care. We considered the feasibility, acceptability and reliability of clinical signs and symptoms, as well as the diagnostic and prognostic performance of predictors. To assure clinical validity, and appropriateness for the country of implementation the algorithm underwent numerous reviews by clinical experts and health authorities from the implementing countries. The digitalization process involved the creation of medAL-creator, a digital platform which allows clinicians without IT programming skills to easily create the algorithms, and medAL-reader the mobile health (mHealth) application used by clinicians during the consultation. Extensive feasibility tests were done with feedback from end-users of multiple countries to improve the clinical algorithm and medAL-reader software. We hope that the development framework used for developing ePOCT+ will help support the development of other CDSAs, and that the open-source medAL-suite will enable others to easily and independently implement them. Further clinical validation studies are underway in Tanzania, Rwanda, Kenya, Senegal, and India.

Abstract SM3 hash algorithm developed by the Chinese Government is used in various fields of information security, and it is being widely used in commercial security products. However, the performance of implementation on the software architecture is not sufficient for high‐speed applications. This study proposes a CPU‐FPGA co‐designed architecture which offloads the SM3 function on field‐programmable gate array so that high throughput can be achieved. The architecture can execute the SM3 hash algorithm with 16 concurrent streams or more, which means that multiple data streams can be processed in parallel. This design is implemented on the Xilinx XCKU115‐flva1517‐2‐e device and Dell commercial server, and the throughput of this design can reach up to 35.5 Gbps when 16 individual SM3 modules are processed in parallel. The proposed architecture results in an excellent performance in the CPU‐FPGA‐coupled environment.
