Introduction: We analysed the neurological complications of patients with severe SARS-CoV-2 infection who required intensive care unit (ICU) admission. Patients and methods: We conducted a retrospective, observational, descriptive study of consecutive patients admitted to the ICU due to severe respiratory symptoms secondary to SARS-CoV-2 infection between 1 April and 1 June 2020. Results: We included 30 patients with neurological symptoms; 21 were men (72.40%), and mean age (standard deviation [SD]) was 57.41 years (11.61). The mean duration of ICU stay was 18.83 days (14.33). The neurological conditions recorded were acute confusional syndrome in 28 patients (93.33%), neuromuscular disease in 15 (50%), headache in 5 (16.66%), cerebrovascular disease in 4 (13.33%), and encephalopathies/encephalitis in 4 (13.33%). CSF analysis results were normal in 6 patients (20%). Brain MRI or head CT showed alterations in 20 patients (66.6%). EEG was performed in all patients (100%), with 8 (26.66%) showing abnormal findings. In 5 of the 15 patients with clinical myopathy, diagnosis was confirmed with electroneuromyography. We found a correlation between older age and duration of ICU stay (P = 0.002; 95% CI, 4.032–6.022; OR). Conclusions: Severe COVID-19 mainly affects men, as observed in other series. Half of our patients presented acute myopathy, and almost all patients left the ICU with acute confusional syndrome, which fully resolved; no correlation was found with EEG or neuroimaging findings. Older age is associated with longer ICU stay. Resumen: Introducción: Nos proponemos analizar las complicaciones neurológicas de los pacientes con infección grave por SARS-CoV2, que han requerido ingreso en Unidad de Cuidados Intensivos (UCI). Pacientes y métodos: Estudio descriptivo retrospectivo, observacional, de pacientes consecutivos ingresado en UCI por infección respiratoria grave por SARS-CoV-2, desde el 1 de Abril hasta el 1 de Junio de 2020. Resultados: Registramos 30 pacientes con síntomas neurológicos, 21 hombres (72,40%), edad media: 57,41 años ± 11,61 desviación estándar (DE). Estancia media en UCI: 18,83 ± 14,33 DE. A nivel sindrómico: 28 pacientes (93,33%) con síndrome confusional agudo, 15 pacientes (50%) con patología neuromuscular, 5 (16,66%) cefalea, 4 (13,33%) con patología cerebrovascular, y 4 (13,33%) con encefalopatías/encefalitis. Punción lumbar normal en 6 pacientes (20%). La RMN craneal o TAC craneal mostró alteraciones en 20 casos (66,6%). Se realizó EEG en todos los pacientes (100%), alterado en 8 pacientes (26,66%). En 5 de los 15 pacientes con miopatía clínica se ha podido confirmar con ENMG. Hemos encontrado relación entre la mayor edad y los días de ingreso en UCI (P = 0,002, IC 95% 4,032-6,022; OR:). Conclusiones: La infección grave por COVID-19 afecta mayoritariamente a hombres, similar a lo descrito en otras series. La mitad de nuestros pacientes presentan una miopatía aguda, y casi la totalidad de los pacientes salen de la UCI con sindromes confusionales agudos, que evolucionan a la resolución completa, sin correlacionarse con los resultados del EEG o de pruebas de neuroimagen. La mayor edad sí se asocia con un mayor número de días de estancia en UCI.

Hereditary ataxia (HA) and hereditary spastic paraplegia (HSP) are rare diseases; as such, they are rarely managed in general neurology consultations. We present a set of brief, practical recommendations for the diagnosis and management of these patients, as well as a standardised procedure for comprehensive evaluation of disability. We provide definitions for HA and “HA plus,” and “pure” and “complicated” HSP; describe the clinical assessment of these patients, indicating the main complementary tests and clinical scales for physical and psychological assessment of the patients; and summarise the available treatments. These recommendations are intended to facilitate daily neurological practice and to unify clinical criteria and disability assessment protocols for patients with HA and HSP. Resumen: Las ataxias hereditarias (AH) y paraparesias espásticas hereditarias (PEH) son enfermedades raras, poco frecuentes en las consultas del neurólogo general. Proponemos una guía práctica y breve de diagnóstico y manejo de estos pacientes, así como un procedimiento para la evaluación integrada del grado de su discapacidad. Se describen por apartados los conceptos y definiciones de AH y AH-plus y PEH pura y complicada, la valoración clínica de los pacientes con las principales pruebas complementarias a realizar, las escalas clínicas necesarias para poder graduar la condición física y psíquica de los pacientes, y se resumen los tratamientos disponibles. Esta guía pretende facilitar la asistencia clínica diaria por parte del neurólogo y unificar los criterios médicos y la metodología de evaluación de la discapacidad de los pacientes con AH y paraparesias espásticas hereditarias.

Introduction: Ataxia and hereditary spastic paraplegia are rare neurodegenerative syndromes. We aimed to determine the prevalence of these disorders in Spain in 2019. Patients and methods: We conducted a cross-sectional, multicentre, retrospective, descriptive study of patients with ataxia and hereditary spastic paraplegia in Spain between March 2018 and December 2019. Results: We gathered data from a total of 1933 patients from 11 autonomous communities, provided by 47 neurologists or geneticists. Mean (SD) age in our sample was 53.64 (20.51) years; 938 patients were men (48.5%) and 995 were women (51.5%). The genetic defect was unidentified in 920 patients (47.6%). A total of 1371 patients (70.9%) had ataxia and 562 (29.1%) had hereditary spastic paraplegia. Prevalence rates for ataxia and hereditary spastic paraplegia were estimated at 5.48 and 2.24 cases per 100 000 population, respectively. The most frequent type of dominant ataxia in our sample was SCA3, and the most frequent recessive ataxia was Friedreich ataxia. The most frequent type of dominant hereditary spastic paraplegia in our sample was SPG4, and the most frequent recessive type was SPG7. Conclusions: In our sample, the estimated prevalence of ataxia and hereditary spastic paraplegia was 7.73 cases per 100 000 population. This rate is similar to those reported for other countries. Genetic diagnosis was not available in 47.6% of cases. Despite these limitations, our study provides useful data for estimating the necessary healthcare resources for these patients, raising awareness of these diseases, determining the most frequent causal mutations for local screening programmes, and promoting the development of clinical trials. Resumen: Introducción: Las ataxias (AT) y paraparesias espásticas hereditarias (PEH) son síndromes neurodegenerativos raros. Nos proponemos conocer la prevalencia de las AT y PEH (APEH) en España en 2019. Pacientes y métodos: Estudio transversal, multicéntrico, descriptivo y retrospectivo de los pacientes con AT y PEH, desde Marzo de 2018 a Diciembre de 2019 en toda España. Resultados: Se obtuvo información de 1.933 pacientes procedentes de 11 Comunidades Autónomas, de 47 neurólogos o genetistas. Edad media: 53,64 años ± 20,51 desviación estándar (DE); 938 varones (48,5%), 995 mujeres (51,1%). En 920 pacientes (47,6%) no se conoce el defecto genético. Por patologías, 1.371 pacientes (70,9%) diagnosticados de AT, 562 diagnosticados de PEH (29,1%). La prevalencia estimada de AT es 5,48/100.000 habitantes, y la de PEH es 2,24 casos/100.000 habitantes. La AT dominante más frecuente es la SCA3. La AT recesiva más frecuente es la ataxia de Friedreich (FRDA). La PEH dominante más frecuente es la SPG4, y la PEH recesiva más frecuente es la SPG7. Conclusiones: La prevalencia estimada de APEH en nuestra serie es de 7,73 casos/100.000 habitantes. Estas frecuencias son similares a las del resto del mundo. En el 47,6% no se ha conseguido un diagnóstico genético. A pesar de las limitaciones, este estudio puede contribuir a estimar los recursos, visibilizar estas enfermedades, detectar las mutaciones más frecuentes para hacer los screenings por comunidades, y favorecer los ensayos clínicos.

Introduction: International criteria are used for the diagnosis of multiple sclerosis (MS) and Isolated Clinical Syndrome (ACS), and Neuromyelitis Optica (NMO). PEVs are not included in any of them. We intend to analyse the sensitivity and specificity of visual evoked potentials in the initial diagnosis of MS, ACS, and NMO. Patients and methods: Descriptive, observational, retrospective study of patients treated for a first episode of RR-EM, NMO, or ACS. VEPs are performed during the study of the first episode, with and without ocular symptoms, between October 2012 and March 2019. Results: 142 patients with suspected demyelinating disease, 100 with disease. Of them, 61 women (61%), mean age: 38.82 years ± SD 4.6. Of the total, 83 with RREM, 6 NMO, 11 ACS. Of the 6 patients with NMO, 1 (16.66%) had altered VEPs; of 11 with ACS, 2 (18.18%) with altered VEP; Of the 83 patients with RR-MS, 45 patients (54.21%) had altered VEP. The sensitivity and specificity of visual evoked potentials in MS was 92.86% and 84.74%, respectively. In NMO, the sensitivity was 16.66%, and the specificity was 93.34%. In ACS, the sensitivity was 18.18%, and the specificity was 96.94%. Conclusion: The sensitivity of VEP in our series is slightly higher than other series published in optic neuritis in RR-MS, being very low in patients with NMO and ACS. However, the very high specificity of the test in these variants can help its diagnosis. These data should be corroborated in larger series. Resumen: Introducción: Para el diagnóstico de esclerosis múltiple (EM) y síndrome Clínico Aislado (SCA) y Neuromielitis óptica (NMO) se utilizan los criterios internacionales. En ninguno de ellos no se incluyen los PEV. Nos proponemos analizar la sensibilidad y especificidad de los potenciales evocados visuales en el diagnóstico inicial de EM, SCA y NMO. Pacientes y métodos: Estudio descriptivo, observacional, retrospectivo, de pacientes atendidos por primer brote de EM-RR, NMO ó SCA. Se realizan los PEV durante el estudio del primer episodio, con y sin clínica ocular, entre Octubre 2012 y Marzo 2019. Resultados: 142 pacientes con sospecha de enfermedad desmielinizante, 100 con enfermedad. De ellos, 61 mujeres (61%), media de edad: 38,82 años +/-DE 4.6. Del total, 83 con EM-RR, 6 NMO, 11 SCA. De los 6 pacientes con NMO, 1 (16,66%) tenía los PEV alterados; de 11 con SCA, 2 (18,18%) con PEV alterados; de los 83 pacientes con EM-RR, 45 pacientes (54,21%) con PEV alterados. La sensibilidad y especificidad de los potenciales evocados visuales en EM fue de 92,86% y 84,74% respectivamente. En NMO la sensibilidad fue de 16,66%, y especificidad de 93,34%. En SCA, la sensibilidad fue de 18,18%, y especificidad de 96,94%. Conclusion: La sensibilidad de los PEV en nuestra serie es discretamente superior a otras series publicadas en neuritis óptica en EM-RR, siendo muy baja en pacientes con NMO y SCA. Sin embargo, la altísima especificidad del test en estas variantes puede ayudar a su diagnóstico. Estos datos deberían ser corroborados en series más amplias.

Resumen: Introducción: Analizamos la prevalencia, etiología y diagnóstico de los pacientes con plexopatía cervicobraquial o lumbosacra de nuestro centro, atendidos durante una década. Material y métodos: Análisis descriptivo, retrospectivo y observacional de adultos con diagnóstico de plexopatía, atendidos en la consulta monográfica de electromiografía, desde febrero de 2008 hasta diciembre de 2018 (ambos inclusive). Resultados: Setenta y siete pacientes (95,1%), edad media: 55,94 años ± 16,9 DE, 49 hombres (60,5%). Prevalencia: 2,37 por 100.000 habitantes/año. La plexopatía más frecuente es la cervical (84%). Las etiologías más frecuentes: traumática en el 35% (27 pacientes), Parsonage-Turner 20,9% (17 pacientes), tumoral en el 14,8% (12 pacientes) e idiopática en el 13,5% (11 pacientes). De los 12 pacientes con plexopatía de causa oncológica solo en 4 (33,3%) de los casos se confirmó la presencia de infiltración tumoral por prueba de neuroimagen. De los 77 pacientes con prueba de neuroimagen hecha (TAC o RM) el 85,2% (69 pacientes) no muestra enfermedad de plexo en la RMN o la TAC, p = 0,004, mostrando una imagen normal. Los servicios que más atienden a estos pacientes son: traumatología (36,36%), neurología (11,68%), rehabilitación (10,38%) y oncología (4%). Conclusiones: La prevalencia de las lesiones de plexo en nuestra área es de 2,37 por 100.000 habitantes/año. La etiología más frecuente en nuestra serie es la traumática en ambos sexos, asociándose a pacientes más jóvenes (menores de 55 años), aunque más de un tercio de la serie tiene etiología desconocida. La localización más frecuente es la cervicobraquial, donde además las pruebas de neuroimagen tienen más valor localizador. Abstract: Introduction: We analyse the prevalence, aetiology, and diagnosis of cervical-brachial and lumbo-sacral plexopathies at our hospital over a 10-year period. Material and methods: We conducted an observational, descriptive, retrospective analysis of adult patients diagnosed with plexopathy and attended at our centre's specialist electromyography clinic between February 2008 and December 2018, inclusive. Results: We included 77 patients (95.1% of the cases identified), with a mean (standard deviation) age of 55.94 (16.9) years; 49 patients (60.5%) were men. Prevalence amounted to 2.37 cases per 100 000 person-years. Cervical plexopathy was the most frequent type (84% of cases). The most frequent aetiologies were trauma (27 patients; 35%), Parsonage-Turner syndrome (17 patients; 20.9%), neoplasm (12 patients; 14.8%), and idiopathic (11 patients, 13.5%). Of the 12 patients with neoplastic aetiology, neuroimaging only confirmed tumour infiltration in 4 cases (33.3%). Of the 77 patients who underwent neuroimaging studies (CT or MRI), findings were normal in 69 (85.2%; P = .004). The hospital departments most frequently attending these patients were traumatology (36.36%), neurology (11.68%), rehabilitation (10.38%), and oncology (4%). Conclusions: The prevalence of plexopathies in our health district amounts to 2.37 cases per 100,000 person-years. In our series, the most frequent aetiology in both sexes was trauma, which was associated with younger age (under 55 years), although aetiology was unknown in more than one-third of patients. The cervical-brachial plexus was most frequently affected, and lesions to this region are more easily located with neuroimaging studies.

Introduction: REM sleep behaviour disorder (RBD) is characterised by violent behaviours (screaming, kicking, vivid dreams) during REM sleep. It has a prevalence of 1% to 2% of the general population and is especially frequent in men and the population older than 60. In the last decade, RBD has been suggested to be a prodrome of neurodegenerative disease. We analysed associated neurological diseases and responses to drug treatment in 33 patients with RBD treated in the multidisciplinary sleep disorders unit at Hospital Infanta Sofía. Patients and methods: We conducted an observational descriptive retrospective analysis of patients diagnosed with RBD and treated in our multidisciplinary sleep disorders unit between October 2012 and December 2015. We recorded age, sex, associated diseases, and treatments administered to these patients. Results: A total of 365 patients were attended at our unit, including 33 with RBD: 13 women (40%) and 20 men (60%). Mean age was 62.72 years. An associated disorder was identified in 48%, with the most common being mild cognitive impairment (69%). The percentage of patients with RBD and an associated disorder among patients older than 60 was 68%. Eighty-two percent of the patients required treatment. The most commonly used drug was clonazepam (76%), followed by melatonin (9%), gabapentin (6%), and trazodone (3%). Discussion: In our series, 48% of the patients had an associated disorder. The likelihood of detecting an associated disorder increases with patients’ age. The vast majority of patients required drug treatment due to symptom severity; the most frequently administered drug was clonazepam (76%). Resumen: Introducción: El trastorno de conducta de sueño REM (TCSR) se caracteriza por conductas violentas (gritos, patadas, sueños vívidos) durante la fase REM del sueño. Tiene una prevalencia del 1-2% de la población general, especialmente en varones y en mayores de 60 años. En la última década se ha asociado como pródromo a una enfermedad neurodegenerativa. Nos proponemos analizar las patologías asociadas a los 33 pacientes con TCSR atendidos en la Unidad Multidisciplinar de Trastornos del Sueño del Hospital Infanta Sofía, y su respuesta al tratamiento farmacológico. Pacientes y métodos: Análisis descriptivo, retrospectivo, observacional, de los pacientes con diagnóstico de TCSR, atendidos en la consulta monográfica de Neurología, desde octubre de 2012 hasta diciembre de 2015. Se valoran la edad, el sexo, las enfermedades asociadas, y los tratamientos empleados. Resultados: De los 365 pacientes valorados en la consulta, 33 presentan TCSR: 13 mujeres (40%) y 20 hombres (60%), con una edad media de 62,72 años. En el 48% se identifica una patología asociada: la más frecuente es el deterioro cognitivo leve (69%). El porcentaje de TCSR con patología asociada en mayores de 60 años se eleva al 68%. El 82% de los casos han requerido tratamiento. El fármaco más utilizado ha sido el clonazepam (76%), seguido de melatonina (9%), gabapentina (6%) y trazodona (3%). Discusión: En nuestra serie el 48% de los pacientes presentan una patología asociada. La mayor edad influye directamente en la posibilidad de encontrar una patología asociada. La gran mayoría han precisado tratamiento farmacológico por la severidad de los síntomas, siendo el clonazepam (76%) el fármaco más utilizado.
