Abstract

Superconducting (SC) tokamak JT-60SA plays an essential role in fusion research and development by supporting and complementing the ITER project, providing directions to the DEMO design activity and fostering next generation scientists and engineers. Since the short circuit incident at the terminal joints of equilibrium field coil #1 during the integrated commissioning (IC) in March 2021, both EU and JA implementing agencies (IAs) have examined how to ensure safe operation of JT-60SA by mitigating the risk of possible discharge occurrence inside the cryostat. Based on the experience of the global Paschen tests, the IAs have established a strategy of risk mitigation measures, which is a combination of (i) reinforcement of insulation, (ii) avoiding unnecessary voltage application to the coil systems and (iii) immediate de-energization of the coils when deteriorated vacuum conditions are detected. Thanks to the considerable efforts of the Integrated Project Team members, the IC restarted in May 2023. After confirmation of the SC state of the coil systems (TF, EF and CS), the coil energization test and the plasma operation phase 1 (OP-1) started. The first plasma was successfully achieved on 23 October 2023 with a limited value of voltage and current applied to the coils. The plasma configuration control was also confirmed with low plasma current and low auxiliary heating power conditions. Based on the IO–F4E–QST collaboration, activities of JT-60SA have been shared with the IO and provided an important lesson for ITER assembly and commissioning, and will provide an outstanding contribution to fusion research at large. After OP-1, maintenance & enhancement phase 1 (M/E-1) starts from January 2024, in which in-vessel components are installed, and heating and diagnostic systems are extensively upgraded to allow a high power heating experiment planned in OP-2. In order to make the best use of JT-60SA, a newly organized JT-60SA experiment team will refine the research plan for the future high heating power operation phase.

Abstract

Previous work has proposed that the anomalous and topological Hall effects, associated with Weyl nodes, should have a signature in optical conductivity. Here, using THz optical spectroscopy, the authors assign these two effects to optical conductivity resonances, arising near band anti-crossings, in thin films of MnGe.

Abstract

The triple alpha reaction is one of the most important reactions in the nuclear astrophysics. However, its reaction rate in high temperature environments at T9> 2 was still uncertain. One of the major origins of the uncertainty was that the radiative-decay probability of the 31− state in 12C was unknown. In the present work, we have determined the radiative-decay probability of the 31− state to be 1.3−1.1+1.2×10−6 by measuring the 1H(12C,12Cp) reaction for the first time, and derived the triple alpha reaction rate in high temperature environments from the measured radiative-decay probability. The present result suggests that the 31− state noticeably enhances the triple alpha reaction rate although the contribution from the 31− state had been assumed to be small.

Abstract

Panchromatic Remote-sensing Instrument for Stereo Mapping (PRISM), one of onboard sensors carried by Advanced Land Observing Satellite (ALOS), was designed to generate worldwide topographic data with its optical stereoscopic observation. It has an exclusive ability to perform a triplet stereo observation which views forward, nadir, and backward along the satellite track in 2.5 m ground resolution, and collected its derived images all over the world during the mission life of the satellite from 2006 through 2011. A new project, which generates global elevation datasets with the image archives, was started in 2014. The data is processed in unprecedented 5 m grid spacing utilizing the original triplet stereo images in 2.5 m resolution. As the number of processed data is growing steadily so that the global land areas are almost covered, a trend of global data qualities became apparent. This paper reports on up-to-date results of the validations for the accuracy of data products as well as the status of data coverage in global areas. The accuracies and error characteristics of datasets are analyzed by the comparison with existing global datasets such as Ice, Cloud, and land Elevation Satellite (ICESat) data, as well as ground control points (GCPs) and the reference Digital Elevation Model (DEM) derived from the airborne Light Detection and Ranging (LiDAR).

5. Luminescence and deep-level transient spectroscopy of grown dislocation-rich Si layers

Abstract

The charge deep-level transient spectroscopy (Q-DLTS) is applied to the study of the dislocation-rich Si layers grown on a surface composed of dense arrays of Ge islands prepared on the oxidized Si surface. This provides revealing three deep-level bands located at EV + 0.31 eV, EC – 0.35 eV and EC – 0.43 eV using the stripe-shaped p-i-n diodes fabricated on the basis of these layers. The most interesting observation is the local state recharging process which proceeds with low activation energy (∼50 meV) or without activation. The recharging may occur by carrier tunneling within deep-level bands owing to the high dislocation density ∼ 1011 - 1012 cm-2. This result is in favor of the suggestion on the presence of carrier transport between the deep states, which was previously derived from the excitation dependence of photoluminescence (PL) intensity. Electroluminescence (EL) spectra measured from the stripe edge of the same diodes contain two peaks centered near 1.32 and 1.55 μm. Comparison with PL spectra indicates that the EL peaks are generated from arsenic-contaminated and pure areas of the layers, respectively.

Abstract

The widely used Streptococcus pyogenes Cas9 (SpCas9) nuclease derives its DNA targeting specificity from protein-DNA contacts with protospacer adjacent motif (PAM) sequences, in addition to base-pairing interactions between its guide RNA and target DNA. Previous reports have established that the PAM specificity of SpCas9 can be altered via positive selection procedures for directed evolution or other protein engineering strategies. Here we exploit in vivo directed evolution systems that incorporate simultaneous positive and negative selection to evolve SpCas9 variants with commensurate or improved activity on NAG PAMs relative to wild type and reduced activity on NGG PAMs, particularly YGG PAMs. We also show that the PAM preferences of available evolutionary intermediates effectively determine whether similar counterselection PAMs elicit different selection stringencies, and demonstrate that negative selection can be specifically increased in a yeast selection system through the fusion of compensatory zinc fingers to SpCas9., The PAM specificity of SpCas9 can be altered with positive selection during directed evolution. Here the authors use simultaneous positive and negative selection to improve activity on NAG PAMs while reducing activity on NGG PAMs.

Abstract

A powerful approach to understand protein structure and evolution is to perform computer simulations that mimic aspects of evolution. In particular, structure-based computational protein design (CPD) can address the inverse folding problem, exploring a large space of amino acid sequences and selecting ones predicted to adopt a given fold. Previously, CPD has been used to entirely redesign several proteins: all or most of the protein sequence was allowed to mutate freely; among sampled sequences, those with low computed folding energy were selected, and a few percent of them did indeed adopt the correct fold. Those studies used an energy function that was partly or largely knowledge-based, with several empirical terms. Here, we show that a PDZ domain can be entirely redesigned using a "physics-based" energy function that combines standard molecular mechanics and a recent, continuum electrostatic solvent model. Many thousands of sequences were generated by Monte Carlo simulation. Among the lowest-energy sequences, three were chosen for experimental testing. All three could be overexpressed and had native-like circular dichroism and 1D NMR spectra. Two exhibited an upshift of their thermal denaturation curves when a peptide ligand was present, indicating they were able to bind and were most likely correctly folded. Evidently, the physical principles that govern molecular mechanics and continuum electrostatics are sufficient to perform whole-protein redesign. This is encouraging, since these methods provide physical insights, can be systematically improved, and are transferable to other biopolymers and ligands of medical or technological interest.

Abstract

The electrical insulation of the Megavolt ITER Injector and Concept Advancement (MITICA) beam source (BS) at 1 MV in vacuum is a challenging issue, which could not be fully addressed so far on the basis of experimental results and of theoretical models available in literature. A specific high-voltage (HV) test campaign is being prepared to validate and optimize the voltage holding capability of the BS insulation under realistic conditions, using full-size mockup electrodes reproducing in detail the geometry of the BS and accelerator. The proposed test strategy will address both the single-gap and the multistage insulation, so as to obtain a verification of voltage holding at 1 MV before the installation of the real components. This approach is intended to reduce the risk related to the HV insulation at 1 MV and, if necessary, to allow the development of effective corrections. In this article, the test motivations and requirements are defined, and the electrode implementation and diagnostic setup are described. Finally, the test configurations and the experimental procedure are discussed.

Contains fulltext : 249977.pdf (Publisher’s version ) (Closed access) BACKGROUND: It is important to evaluate whether a new treatment for heart failure with reduced ejection fraction (HFrEF) provides additive benefit to background foundational treatments. As such, we aimed to evaluate the efficacy and safety of empagliflozin in patients with HFrEF in addition to baseline treatment with specific doses and combinations of disease-modifying therapies. METHODS: We performed a post-hoc analysis of the EMPEROR-Reduced randomised, double-blind, parallel-group trial, which took place in 520 centres (hospitals and medical clinics) in 20 countries in Asia, Australia, Europe, North America, and South America. Patients with New York Heart Association (NYHA) classification II-IV with an ejection fraction of 40% or less were randomly assigned (1:1) to receive the addition of either oral empagliflozin 10 mg per day or placebo to background therapy. The primary composite outcome was cardiovascular death and heart failure hospitalisation; the secondary outcome was total heart failure hospital admissions. An extended composite outcome consisted of inpatient and outpatient HFrEF events was also evaluated. Outcomes were analysed according to background use of angiotensin-converting enzyme (ACE) inhibitors, angiotensin II receptor blockers (ARBs) or angiotensin receptor neprilysin inhibitors (ARNIs), as well as β blockers and mineralocorticoid receptor antagonists (MRAs) at less than 50% or 50% or more of target doses and in various combinations. This study is registered with ClinicalTrials.gov, NCT03057977. FINDINGS: In this post-hoc analysis of 3730 patients (mean age 66·8 years [SD 11·0], 893 [23·9%] women; 1863 [49·9%] in the empagliflozin group, 1867 [50·1%] in the placebo group) assessed between March 6, 2017, and May 28, 2020, empagliflozin reduced the risk of the primary outcome (361 in 1863 participants in the empagliflozin group and 462 of 1867 in the placebo group; HR 0·75 [95% CI 0·65-0·86]) regardless of background therapy or its target doses for ACE inhibitors or ARBs at doses of less than 50% of the target dose (HR 0·85 [0·69-1·06]) and for doses of 50% or more of the target dose (HR 0·67 [0·52-0·88]; p(interaction)=0·18). A similar result was seen for β blockers at doses of less than 50% of the target dose (HR 0·66 [0·54-0·80]) and for doses of 50% or more of the target dose (HR 0·81 [0·66-1·00]; p(interaction)=0·15). Empagliflozin also reduced the risk of the primary outcome irrespective of background use of triple therapy with an ACE inhibitor, ARB, or ARNI plus β blocker plus MRA (given combination HR 0·73 [0·61-0·88]; not given combination HR 0·76 [0·62-0·94]; p(interaction)=0·77). Similar patterns of benefit were observed for the secondary and extended composite outcomes. Empagliflozin was well tolerated and rates of hypotension, symptomatic hypotension, and hyperkalaemia were similar across all subgroups. INTERPRETATION: Empagliflozin reduced serious heart failure outcomes across doses and combinations of disease-modifying therapies for HFrEF. Clinically, these data suggest that empagliflozin might be considered as a foundational therapy in patients with HFrEF regardless of their existing background therapy. Abstract

International audience; In multi-beamlet negative ion accelerators for neutral beam injectors, the transverse magnetic field necessary for suppressing the co-extracted electrons induces a deflection of the negative ion beamlets that must be corrected. For the design, particle-tracing simulation codes are used to compute ion trajectories and optical properties of the beamlets in the acceleration stage. In these codes, uniform boundary conditions are normally assumed for the ion current density distribution at the surface (called meniscus), where negative ions are extracted from the plasma and form beamlets, which are accelerated across the apertures of the accelerator grids. Recently, experimental campaigns dedicated to the accurate measurement of the beamlet deflection in the acceleration phase revealed higher deflection than foreseen by simulations. In this work, we demonstrate that an agreement with the experimental data can be obtained by incorporating in the numerical simulations a non-radially symmetric distribution of the ion current density extracted across the meniscus surface. In the first part, the asymmetry of the ion current density at the meniscus is studied in an empirical way, by analysing and fitting the experimental results obtained with different operating parameters. In the second part, we show that the ion current asymmetry estimated by this procedure is well consistent with the flow pattern of H − ions calculated in the meniscus zone using a detailed particle in cell (PIC) model of the ion source in the presence of transverse magnetic field.

It means that our new measurement and analysis method are successfully working well. Analysis for deriving the differential cross section of the \(\Sigma p\) scattering is on going.

International audience; The \(\pi ^{ - }p \to K^{0}\Lambda \) reaction is an important elementary process to produce \(\Lambda \) from a proton target and is a key for a \(\Lambda p\) scattering experiment where the momentum of \(\Lambda \) should be tagged from the missing momentum of the \((\pi ^{ - },K^{0})\) reaction. However, the \((\pi ^{ - },K^{0})\) spectroscopy method has not been established yet due to the difficulty of the K^0 detection. Therefore, we have proposed a new K^0 detection method where \(\pi ^{ + }\) and \(i^{ - }\) from the K^0 decay are measured by a forward magnetic spectrometer and a detector cluster surrounding the target, respectively. The feasibility of the K^0 detection method was confirmed by analyzing the J-PARC E40 data taken with such a detector system. In the analysis, \(\Lambda \)’s were successfully identified in the missing mass spectrum of the \(\pi ^{ - }p \to K^{0}X\) reaction.

Abstract

The accurate determination of protein–protein interactions has been an important focus of molecular biology toward which much progress has been made due to the continuous development of existing and new technologies. However, current methodscan have limitations, including scale and restriction to high affinity interactions, limiting our understanding of a large subset of these interactions. Here, we describe a modified bacterial-hybrid assay that employs combined selectable and scalable reporters that enable the sensitive screening of large peptide libraries followed by the sorting of positive interactions by the level of reporter output. We have applied this tool to characterize a set of human and E.coli PDZ domains. Our results are consistent with prior characterization of these proteins, and the improved sensitivity increases our ability to predict known and novel in vivo binding partners. This approach allows for the recovery of a wide range of affinities with a high throughput method that does not sacrifice the scale of the screen.

Abstract

International audience; J-PARC E40 aims to measure the differential cross sections of the Σ^±p elastic scatterings and the Σ^−p → Λn conversion. A clear peak of Σ^− was observed in a missing mass spectrum of the π^−p → K^+X reaction and recoil protons from the πp elastic scattering were successfully observed in the data taken in the summer 2018. The rest of data taking is coming in the spring 2019.

The optics design of the multi-beamlet negative ion accelerator for MITICA has been verified by means of specific experiments in the NITS test facility at the QST lab, employing a scaled-down configuration having the same features as the accelerator of MITICA and ITER HNB. The experiments confirmed the validity of the accelerator design. However it was found that, in the presence of transverse magnetic field in the ion extraction stage, the ion beamlet deflection in numerical simulations was somehow underestimated with respect to the beamlet deflection inferred from thermal images of a CFC target exposed to the negative ion beam. Abstract

In order to understand the physics mechanism of a negative ion extraction in negative ion sources, an emission surface of the negative ions around an aperture at a plasma grid, so-called a meniscus, has been analyzed by an inverse calculation of the negative ion trajectory in a two dimensional beam analysis code. In this method, the meniscus is defined as the final position of the negative ion trajectories which are inversely calculated from the measured beam profile to the plasma grid. In a case of the volume-produced negative ions, the calculated meniscus by the inverse calculation was similar to that obtained in conventional beam simulation codes for positive ion extractions such as BEAMORBT and SLACCAD. The negative ion current density was uniform along the meniscus. This indicates that the negative ions produced in the plasma are transported to the plasma grid uniformly as considered in the transportation of the positive ions. However, in a surface production case of negative ions, where the negative ions are generated near the plasma grid with lower work function by seeding cesium, the current density in the peripheral region of the meniscus close to the plasma grid surface was estimated to be 2 times larger than the center region, which suggested that the extraction process of the surface-produced negative ions was much different with that for the positive ions. Because this non-uniform profile of the current density made the meniscus shape strongly concave, the beam extracted from the peripheral region could have a large divergence angle, which might be one of origins of so-called beam halo. This is the first results of the determination of the meniscus based on the experiment, which is useful to improve the prediction of the meniscus shape and heat loads based on the beam trajectories including beam halo.

Abstract

A common problem of the multi-beamlet, negative-ion based Heating Neutral Beam (HNB) Injectors presently used in fusion research, is the undesired deflection of negative ions due the permanent magnets embedded in the Extraction Grid (EG) for the suppression of the unwanted co-extracted electrons. A new solution has been recently developed at Consorzio RFX, which makes use of an additional set of permanent magnets, also embedded in the EG, with a series of advantages in terms of performances and versatility with respect to the traditional electrostatic compensation. This solution is now the new standard for ITER. After a thorough validation of the design by different numerical models, it was decided to experimentally test this solution for the first time on the Negative Ion Test Stand (NITS) at National Institute for Quantum and Radiological science and technology (QST), in Japan, within the framework of a scientific cooperation agreement between QST and Consorzio RFX. To this purpose, an EG having an ITER-like profile and compatible with NITS accelerator was designed and constructed at Consorzio RFX. This paper describes the design solutions adopted for this grid and its related permanent magnets, the construction phase, and a brief report on the first experimental results.

In order to evaluate deposition of cesium (Cs) in the plasma grid (PG) and investigate the origin of the gradual degradation of the negative ion current in a 100 s long pulsed operation in the JT-60SA negative ion source, Cs behavior in the source is model. To do this, Cs adsorption/desorption coefficient with a wide range of temperature is experimentally measured by a surface ionization detector and a quartz crystal microbalance. As a result, both of the adsorption and desorption are drastically varied at temperature range from room temperature to 100 °C. Based on these data, the model calculates the amounts of the Cs mass in the chamber wall and PG. The calculation result shows that, in the operation, an increase of temperature in the chamber wall (Twall) causes the evaporation of the huge amount of Cs in it. This leads to over pilling-up of Cs in the PG and the slight degradation of the negative ion production. Therefore, it is found that the constant temperature operation is needed to sustain the thickness of the Cs monolayer in the PG, and the Twall is desired to be less than 60 °C to suppress the evaporation of Cs in the chamber wall. Abstract

Background To clarify the influence of hypertension and blood pressure ( BP ) control on thromboembolism and major hemorrhage in patients with nonvalvular atrial fibrillation, a post hoc analysis of the J‐ RHYTHM Registry was performed. Methods and Results A consecutive series of outpatients with atrial fibrillation was enrolled from 158 institutions. Of 7937 patients, 7406 with nonvalvular atrial fibrillation (70.8% men, 69.8±10.0 years) were followed for 2 years or until an event occurred. Hypertension was defined as a systolic BP ≥140 mm Hg, a diastolic BP ≥90 mm Hg, a history of hypertension, and/or antihypertensive drug use. Hypertension was an independent risk factor for major hemorrhage (hazard ratio 1.52, 95% CI 1.05–2.21, P =0.027) but not for thromboembolism (hazard ratio 1.05, 95% CI 0.73–1.52, P =0.787). When patients were divided into quartiles according to their systolic BP at the time closest to the event or at the end of follow‐up (Q1, CI 1.75–4.74, P CI 1.02–2.53, P =0.041) after adjustment for components of CHA 2 DS 2 ‐ VAS c score, warfarin use, and antiplatelet use. A systolic BP of ≥136 mm Hg was an independent risk factor for thromboembolism and major hemorrhage. Conclusions BP control appears to be more important than a history of hypertension and baseline BP values at preventing thromboembolism and major hemorrhage in patients with nonvalvular atrial fibrillation. In the Japanese LUNAR-A mission, penetrators will be deployed to the Moon for global seismic measurement. The unique communication system between the subsurface probes under the lunar surface and the lunar orbiter is described. Radiowave propagation through a crater which is formed at the penetration is investigated by means of scaled measurements in a simulating environment. Acquisition and tracking sequence is optimized within limited power capacity of the probe to maximize contact time between the probe and the spacecraft.

Abstract

We obtained highly polarized very cold neutrons through a quadrupole magnet with a large aperture. When neutrons go through a quadrupole magnet, parallel-spin neutrons are attracted to the magnet axis and anti-parallel-spin neutrons are repelled off the axis. Therefore, the neutrons through a sufficiently long quadrupole magnet have completely parallel-spin. At the downstream of the quadrupole magnet, we placed another quadrupole magnet to divide neutrons into two regions depending on their spin directions. We obtained the neutron polarization from the integrated count of the two regions and found it was 99.88±0.09 (stat.)±0.05 (syst.)%.

Abstract

The T2K long-baseline neutrino oscillation experiment in Japan needs precise predictions of the initial neutrino flux. The highest precision can be reached based on detailed measurements of hadron emission from the same target as used by T2K exposed to a proton beam of the same kinetic energy of 30 GeV. The corresponding data were recorded in 2007–2010 by the NA61/SHINE experiment at the CERN SPS using a replica of the T2K graphite target. In this paper details of the experiment, data taking, data analysis method and results from the 2007 pilot run are presented. Furthermore, the application of the NA61/SHINE measurements to the predictions of the T2K initial neutrino flux is described and discussed. © 2012

Abstract

e have developed a compact permanent-magnet sextupole lens for neutrons that can focus a pulsed beam with a wide wavelength range—the maximum wavelength being more than double the minimum—while sufficiently suppressing the effect of chromatic aberration. The bore diameter is ø15 mm. Three units of a double-ring sextupole with a length of 66 mm are cascaded, resulting in a total length of 198 mm. The dynamic modulation range of the unit-averaged field gradient is 1.06×10 4 1.06×104 –5.86×10 4 5.86×104 Tm −2 −2 . Permanent magnets and newly developed torque-canceling elements make the device compact, its production costs low, and its operation simpler than that of other magnetic lenses. The efficacy of this lens was verified using very cold neutrons. The diameter of the focused beam spots over the wavelength range of 27–55 Å was the same as that of the source aperture (2 mm diameter) when the magnification of the optical arrangement was unity. The total beam flux over this wavelength range was enhanced by a factor of 43. The focusing distance from the source to the detector was 1.84 m. In addition, in a demonstration of neutron image magnification, the image of a sample mask magnified by a factor of 4.1 was observed when the magnification of the optical arrangement was 5.0.

Drowning, once by far the most important external cause of child deaths in Japan,1 has reduced more rapidly than other injuries. Drowning mortality of children aged 1–4 years decreased from 45.4 per 100 000 in 1955, 4.5 times higher than that of traffic injuries, to 1.6 per 100 000 (ranking next to traffic injuries) in 2000. We could have achieved this by two main approaches: (1) environmental modification to reduce exposure to open water where most outdoor drownings occur2 and (2) health education to reduce risk of bathtub drowning, which causes most of the domestic drownings.2,3 To know how these approaches contributed to the mortality reduction, we separately examined the trends of outdoor and domestic drowning mortality among children aged 1–4 years. Shakespearean Entrances offer a systematic study of entrances and exits on the Shakespearean stage. Elizabethan playwrights and players not only routinely handled these movements but they also used them to bring about various effects. Through analyzing the surviving play-texts, the author attempts to identify the unspoken but standard rules that lay behind the minimal and conventionalized stage directions'Enter'and'Exit'/'Exeunt'. The findings provide means by which to recover effects and meanings that the original audience would have appreciated.

Abstract

and transport to the tokamak plasma strongly depends on the divergence and the deflection angle of each single beamlet with respect to its ideal trajectory. In fact, a very narrow window is available for the particle beam to pass through the neutralizer panels and the duct reaching the tokamak plasma. For this reason, beam optics quality is one of the key requirements in multi-stage multi-beamlet negative ion accelerator, such as the high power Heating Neutral Beam injectors for ITER and JT-60SA. In the framework of the collaboration established between Consorzio RFX (Padova, Italy) and QST (Naka, Japan) experimental campaigns have been organized on the Negative Ion Test Stand (NITS) in Naka employing an ITER-like multi-beamlet configuration [1]. These campaigns were intended to: o Test and optimize new solutions for cancelling the undesired ion deflection caused by the transverse magnetic field required for suppressing the co-extracted electrons [2]. o Better understand the physics underlying the extraction of single beamlets in the initial part of their trajectories, close to the ,,meniscus" region. o Validate and improve the numerical codes used for the accelerator design Although it was not possible to explore all the intended parameter space due to power supply limitations, the experiments carried out in 2016 and 2017 allowed to successfully achieve the first most important (engineering) objective and to provide bases for the achievement of the other two (physics) objectives, which will be completed during new already planned activities and collaborations.

Abstract

For the realization of the ITER neutral beam (NB) system, a method for achieving the acceleration voltage of 1 MV for the vacuum insulated beam source (VIBS) has been developed. Additionally, the design basis for the 1 MV vacuum insulation has also been developed by integrating previous empirical scaling of the voltage holding capability for plane and coaxial electrodes with new scaling for the corner region where the locally-concentrated electric field is generated. The scaling clarified that a product of electric field and voltage obtained by electric field analysis in the beam source vessel was much higher than the allowable level. Therefore, in order to improve the voltage holding capability, electrostatic shields having intermediate potential have been proposed. Consequently, it was found that the VIBS needs to be surrounded by more than three intermediate shields instead of a single gap insulation of 1 MV. Reliability of the design basis of the developed intermediated shields has been experimentally verified by improving the 1.3 m single gap insulation in the voltage holding test. As a result, the sustainable voltage has been significantly improved from 0.7 MV with the single gap to 1 MV with shielding up to the power supply limit. Abstract

Cys-loop receptors are a large superfamily of pentameric ligand-gated ion channels with various physiological roles, especially in neurotransmission in the central nervous system. Among them, zinc-activated channel (ZAC) is a Zn 2+ -activated ion channel that is widely expressed in the human body and is conserved among eukaryotes. Due to its gating by extracellular Zn 2+ , ZAC has been considered a Zn 2+ sensor, but it has undergone minimal structural and functional characterization since its molecular cloning. Among the families in the Cys-loop receptor superfamily, only the structure of ZAC has yet to be determined. Here, we determined the cryo-EM structure of ZAC in the apo state and performed structure-based mutation analyses. We identified a few residues in the extracellular domain whose mutations had a mild impact on Zn 2+ sensitivity. The constriction site in the ion-conducting pore differs from the one in other Cys-loop receptor structures, and further mutational analysis identified a key residue that is important for ion selectivity. 41. News coverage of older drivers' fatal car crashes: is it over-represented?

Abstract

Background: Japan's stringent licensing policies for older drivers have not been questioned, possibly due to negative perceptions of older drivers potentially influenced by media coverage of their car crashes. We examined whether older drivers' fatal crashes are over-represented in the news articles., Methods: To examine the news coverage of fatal crashes that occurred between January 2016 and December 2020, we extracted driver- and crash-related data from articles reporting fatal crashes in the two best-selling newspapers, Yomiuri and Asahi. We obtained the corresponding data of police-reported fatal crashes during the same period. We calculated the proportion of newspaper-reported fatal crashes to police-reported fatal crashes by at-fault driver's age group and crash characteristics., Results: Of 12,987 police-reported fatal crashes, 5,888 (45%) and 2,909 (22%) crashes were reported in Yomiuri and Asahi newspapers, respectively. Excluding 2,098 crashes where at-fault drivers or their ages were not identifiable, Yomiuri reported 39%, 35%, and 31%, and Asahi reported 20%, 16%, and 14% of fatal crashes caused by drivers aged <30 years, 30-69 years, and 70 years or older, respectively. Crashes that caused more fatalities or killed children tended to be reported regardless of at-fault drivers' age groups. Compared with young and middle-aged drivers, older drivers' fatal crashes involving child fatalities were more reported, whereas their single fatal crashes ending in their own deaths were less reported., Conclusion: Older drivers' at-fault fatal crashes were not over-represented in the news coverage of overall fatal crashes, and their crashes killing themselves were under-reported.

42. Effect of 1-Week Weight Loss While Maintaining Total Body Water on Jump Performance.

Abstract

Jumping performance is influenced by body composition and excess fat mass impairs performance. Maintaining optimal fat mass and fat-free mass (FFM) is crucial for enhancing jump height. However, there is limited evidence on short-term weight loss programs that reduce fat mass without water restriction and their effects on muscle function and jumping performance. This study aimed to clarify the effects of a 1-week weight loss program on jumping height and muscle function of volleyball players. The weight loss group engaged in two 40 min slow-paced jogging sessions in addition to their daily training routine. Energy intake was restricted without limitations on water intake. Total body water and body composition using the deuterium dilution method, muscle strength, and jump height before and after 1 week were evaluated for those in the weight loss and control groups. Body mass was significantly reduced in the weight loss group (-2.7 ± 1.3%, p < 0.05) with a significant reduction in fat mass (-17.7 ± 10.7%, p < 0.05). Meanwhile, there were no significant changes in total body water or FFM. Muscle strength and power tests indicated no significant differences between the groups; no notable differences were observed in handgrip strength or knee extension torque. The height of a single vertical and continuous jump remained consistent pre- and postintervention in the control group. In the weight loss group, although the height of a single vertical jump exhibited a slight decline postintervention, the height of a continuous jump displayed no significant changes. The short-term weight loss program significantly reduced fat mass without compromising muscle function, which is crucial for sports performance. 43. Incidence of the postpartum diagnosis of thyroid eye disease in relation to thyroid function in Graves' disease.

Abstract

It has been reported that Graves' disease (GD) sometimes improves spontaneously during pregnancy, although exacerbation of GD during postpartum period or relapse of hyperthyroidism caused by GD might occur. This study aimed to investigate the incidence of postpartum diagnosis of thyroid eye disease (TED) in relation to thyroid dysfunction. This retrospective cross-sectional study enrolled 11,104 deliveries from the patients with GD between January 2004 and August 2022. Within the 12-month postpartum period, 72 patients (0.65%) were diagnosed with TED. The thyroid function of the 72 patients comprised 9 remission, 13 continued antithyroid medicine, and 50 thyroid dysfunction; 30 newly diagnosed GD, 1 hypothyroidism, and 19 relapse/recurrence of GD. In the 49 patients with thyroid dysfunction, no difference was observed in the median values of thyroid-stimulating hormone (TSH) receptor antibody (TRAb) and TSH receptor stimulating antibody between the TED diagnosis and the development of hyperthyroidism. However, when the patients were classified into the newly developed GD and relapse/recurrence of GD groups, the difference became significant and the TRAb level was high in the newly developed GD (16.1 vs. 5.0 IU/L, p < 0.0001, and 15.0 vs. 6.0 IU/L, p = 0.0003). Thyroid dysfunction preceded TED diagnosis in more than half of the patients and the median time for each event was 6.5 vs. 8.1 months. The active phase TED was observed in 8 of the 72 patients. Of the 72 patients newly diagnosed with TED in postpartum, two-thirds were accompanied by thyroid dysfunction and 8 of them were in active phase.

44. Evaluating Laparoscopic Skills: Report on the Origami Crane Folding Competition Using Laparoscopic Instruments With Objective Criteria.

Abstract

Purpose: To evaluate the importance of daily laparoscopic training using laparoscopic forceps to fold origami paper cranes (a traditional Japanese paper craft) and assess the performance of laparoscopic origami crane folding in an actual competition., Method: A competition, named the "Kaminote Challenge World Championship," was used to evaluate the effectiveness of training. The participants folded the paper cranes using laparoscopic forceps. The judges evaluated the speed at which the paper cranes were folded and the quality of the completed cranes, using objective criteria., Results: The competition was held twice, in 2022 and 2023, with 27 and 36 participants, respectively. The participants were surgeons, veterinarians, and students from Japan, Mexico, and Vietnam. The completion rate for folding a paper crane within seven minutes was 70.4% in 2022 (19/27) and 44.4% in 2023 (16/36). In the second competition, 75.0% (12/16) of the participants who completed the origami crane within seven minutes had practiced folding more than 1,000 cranes. Despite the competitive pressure, the top performers folded paper cranes with minimal deductions for quality and used the laparoscopic forceps precisely. The winners of the 1st prize in 2022 and 2023 completed the task in 2 min 46 s and 2 min 45 s, respectively, without any penalty., Conclusions: Training by folding paper cranes using laparoscopic forceps is highly likely to lead to improved laparoscopic surgery skills. Abstract

Objective: There are few reports of subacute thyroiditis (SAT) during pregnancy. This study aimed to clarify the clinical characteristics of SAT in pregnant patients., Methods and Results: Seven patients diagnosed with SAT during pregnancy at our institution from January 2004 to December 2021 were identified, and their clinical findings were retrospectively examined. At SAT diagnosis, the median age was 34 (range: 31-42) years, the median duration of pregnancy was 5 (4-24) weeks, and all patients had neck pain but no fever. On laboratory examination, median (range) free thyroxine, free triiodothyronine, and C-reactive protein levels were 2.66 (1.14-7.77) ng/dL, 7.1 (3.3-16.1) pg/mL, and 2.22 (0.42-5.79) mg/dL, respectively, and all patients had a hypoechoic lesion of the thyroid gland. Three patients (43%) were treated with steroids, and three patients (43%) received replacement therapy with levothyroxine for hypothyroidism following destructive thyroiditis. There were no pregnancy complications in any of the cases. These seven patients (pregnancy group) were compared with 217 non-pregnant female patients (non-pregnancy group) aged 31 to 42 years who were diagnosed with SAT at our institution from 2016 to 2019. The frequency of body temperatures above 37°C was lower in the pregnancy group than in the non-pregnancy group (0% vs 65%)., Conclusion: Patients who develop SAT during pregnancy may have less fever than non-pregnant patients with SAT. There were no pregnancy complications in the pregnancy group in this study. This suggests that adverse pregnancy outcomes may be avoided by the appropriate management of SAT, including hypothyroidism after destructive thyroiditis.

Abstract

The goal of surgery for patients with newly diagnosed glioblastoma (GBM) is maximum safe resection of the contrast-enhancing (CE) lesion on magnetic resonance imaging. However, there is no consensus on the efficacy of FLAIRectomy, which is defined as the possible resection of fluid-attenuated inversion recovery (FLAIR)-hyperintense lesions surrounding the CE lesion. Although retrospective analyses suggested the potential benefits of FLAIRectomy, such outcomes have not been confirmed by prospective studies. Therefore, we planned a multicenter, open-label, randomized controlled phase III trial to evaluate the efficacy of FLAIRectomy compared with gross total resection of CE lesions in patients with newly diagnosed GBM. The primary endpoint is overall survival. In total, 130 patients will be enrolled from 47 institutions over 5 years. 47. Exacerbation of Thyroid Eye Disease and Dysthyroid Optic Neuropathy During Pregnancy: A Case Report.

Abstract

A 37-year-old, never-smoker, pregnant woman diagnosed with Graves' disease who had stable thyroid eye disease (TED) before pregnancy presented with aggravated proptosis and eyelid swelling at 13 weeks of pregnancy. Despite the administration of local triamcinolone and 3 cycles of corticosteroid pulse therapy from 25 to 28 weeks, the patient's visual acuity decline necessitated postpartum orbital decompression surgery. Although TSH receptor antibody (TRAb) levels decreased during the mid- to late term of pregnancy, the TED worsened. This finding suggests that factors other than anti-TSH receptor antibodies may have a significant effect on disease severity.

Abstract

Purpose: This study aimed to develop a user-friendly prediction formula for dose rate adjustment after initial 177 Lu-Dotatate therapy from a prospective observational study of patients., Materials and Methods: This study included consenting patients in a prospective observational study who underwent their first treatment in four cycles of 177 Lu-Dotatate treatment at our hospital between January 2022 and February 2024. All patients received 7.4 GBq of 177 Lu-Dotatate. The prediction formula was derived from the regression analysis of tumor-related factors and renal function. Creatinine clearance was estimated using the Cockcroft-Gault equation in this study for renal function., Results: Among the 13 patients (seven males, six females, median age: 59 years), logarithmically transformed total tumor volume (cc) and maximum tumor diameter (mm) of primary tumors or metastases showed strong correlations (p < 0.001, R 2  = 0.897). As such, the maximum tumor diameter was used as the tumor parameter in the prediction formula. Additionally, maximum tumor diameter and creatinine clearance showed strong (p < 0.001, R 2  = 0.768) and moderate (p = 0.013, R 2  = 445) correlations, respectively, with the ratio of the dose rate 5.5-h post-administration to the dose rate immediately post-administration (%) at 1 m from the body surface. The resulting formula, 51.4 + 0.360 × maximum tumor diameter (mm) - 0.212 × creatinine clearance (ml/min), demonstrated an extremely strong correlation (p < 0.001, R 2  = 0.937)., Conclusion: The present study showed that the maximum tumor diameter and renal function affected the declining the dose rate of patients surface after 177 Lu-Dotatate, which can inform post-administration dose rate management and potentially facilitate outpatient treatment in Japan., (© 2024. Abstract

In Japan, influenza activity was low throughout the COVID-19 pandemic until the 2022-23 season, when the first influenza outbreak occurred since the 2020-21 season. In our influenza surveillance during the COVID-19 pandemic, co-infection with SARS-CoV-2 and influenza virus had not been detected; however, in January 2024, we identified three pediatric outpatients co-infected with these viruses: one with SARS-CoV-2 Omicron EG.5 sublineage HK.3 and influenza A(H3N2) and two with SARS-CoV-2 Omicron BA.2.86 sublineage JN.1.5 and influenza A(H1N1)pdm09. We evaluated the susceptibility of SARS-CoV-2 against RNA-dependent RNA polymerase inhibitors (remdesivir and molnupiravir) and 3C-like protease inhibitors (nirmatrelvir and ensitrelvir), and that of influenza viruses against neuraminidase inhibitors (oseltamivir, peramivir, zanamivir, and laninamivir) and the cap-dependent endonuclease inhibitor baloxavir. All viruses tested were susceptible to these antiviral drugs and did not possess amino acid substitutions associated with reduced antiviral susceptibility. The patients were treated with anti-influenza drugs and did not develop severe symptoms despite the co-infection.
