2. Could the Influence of Monitor Farm Programmes on Practice Change Be BETTER? Lessons from Sheep Farmers and Advisors in Ireland

Abstract

Purpose: To examine the influence of monitor farm programmes on practice change using the Irish BETTER farm sheep programme as a case study. Methodology: A mixed methods approach combining 16 semi-structured and 69 structured interviews with benchmarking data analysis. Findings: Provision of intensive, tailored advice helped support practice change on the participating monitor farms. They increased productivity through changes in management practices, which positively impacted on financial performance. Monitor farmers also positively influenced members of their associated discussion groups to make practice changes. Practical Implications: Practical recommendations for similar programmes are discussed such as the need for different and additional extension approaches to influence and support the adoption of more complex practices and the potential for greater input from farmers into the design and implementation of extension programmes. Theoretical Implications: From a practice change perspective evaluating the influence of a more structured participatory extension programme (PEP) on a participating farmer is easier than the evaluation of the influence of the PEP on the wider farming community, particularly when the evaluation is not incorporated into the original programme design. Originality: Adding to the relatively small number of PEP evaluations in developed countries, the influence of monitor farm programmes on both the monitor farmer and their associated discussion group peers is examined for a wide range of practices using a mixed methods approach.

5. Clinical psychologists' accounts of personal distress experienced within the profession : a discourse analysis

Abstract

This study is concerned with ways that clinical psychologists construct the clinical psychologist's experience of distress, in relation to their professional identity, and the implications these constructions have for their social practices in relation to personal distress. Nine qualified clinical psychologists in practice in the NHS were interviewed using a semi-structured interview approach. Data from these interviews was analysed using a discourse-informed approach to Thematic Analysis. Three overarching themes were formed: psychologist's distress is constructed as part of the human condition, work with distress as a difficult and skilled practice, negotiating dilemmas of professional identity and role. These themes are discussed with reference to the socio-cultural and historical context of the profession of Clinical Psychology. In dialogue with literature and research pertaining to clinical psychologist's distress and help-seeking, and broader aspects of the institutional and professional context. Findings supported the view that the ways contemporary clinical psychologists in the NHS are positioned by language, social practices, and institutions can function to constrain clinical psychologists from talking about personal distress and accessing support. However, findings also indicated that there is scope in the contemporary clinical psychologist professional identity for resistance to discourses and practices that limit space for clinical psychologists to acknowledge their own vulnerability. Implications for Clinical Psychology practice and further research are considered. In conclusion, it is suggested that the acknowledgement of a human vulnerability by clinical psychologists can create the conditions for individual and collective action to respond to distress experienced by clinical psychologists, and their colleagues, in the NHS.

Abstract

In February 2023, a report of morbidity and mortality in waterbirds triggered a collaborative regional wildlife disease outbreak investigation and response, led by Parks Victoria. Triage, rehabilitation and diagnosis of sick and dead birds were undertaken by Zoos Victoria (ZV), Agriculture Victoria, Vets for Compassion, Wildlife Victoria and Melbourne Veterinary School (MVS). The field response focused on collection of sick and dead birds for wildlife welfare, for diagnosis, and to reduce environmental contamination. Botulism was suspected, based on clinical signs and lack of significant gross pathology, and this diagnosis was confirmed by PCR testing. Low pathogenicity avian influenza (LPAI) viruses non H5 or H7 were detected in two birds and ruled out in all in others tested. These incidental, non‐clinical LPAI detections are considered part of the natural wild bird virus community in Australia. A number of elements contributed to the collaborative effort. Regional individuals had the necessary connections for reporting, collecting and transporting birds. There was rapid determination by the Victorian Department of Energy, Environment and Climate Action (DEECA) that Parks Victoria, as the land managers, should lead the response. Zoos Victoria provided capacity and expertise in wildlife triage and rehabilitation, and Agriculture Victoria, ZV and MVS were responsible for veterinary management of the response and diagnosis. Field investigation and response were conducted by Parks Victoria, Agriculture Victoria, MVS and veterinary teams from Vets for Compassion and Wildlife Victoria. Wildlife Health Australia (WHA) provided guidance and information, approved National Significant Disease Investigation Program funding and captured the event in the national wildlife health information database. Abstract

There have been calls for more research to investigate the sociocultural context of retail food environments. This paper examines how a segment of Ottawa, Ontario, Canada, residents described convenience stores (CS) in their local retail food environments. 84 social media discussions from Ottawa residents pertaining to their local retail food outlets were qualitatively analyzed, and three themes were formulated: 1) CS are interchangeable, 2) CS are not 'real stores,' and 3) CS are dangerous retail food outlets. We argue that these social constructions of CS have implications for healthy food environments and offer suggestions for future research. [ABSTRACT FROM AUTHOR]

Abstract

Milk production declines as dairy cows enter late lactation, resulting in reduced milk quality and negatively impacting milk processability, such as rennet coagulation time (RCT), milk pH and ethanol stability (ES), leading to seasonality issues for milk processors. Multispecies forages, containing grass, legume and herb species, require lower N inputs and are of interest to dairy farmers. However, little is known about the effect of grazing multispecies forages on milk processability characteristics in late lactation dairy cows. Forty-five autumn-calving dairy cows in late lactation were allocated to 1 of 3 grazing forages; perennial ryegrass (PRG; Lolium perenne), perennial ryegrass and white clover (Trifolium pratense) (PRGWC), and a 6 – species multispecies forage (MULTI) containing perennial ryegrass, timothy (Phleum pratense), white clover, red clover (Trifolium repens), chicory (Cichorium intybus) and plantain (Plantago lanceolata). Cows were allocated 12 kg DM grazed forage and supplemented with a grass – silage TMR and concentrate. Forage DMI was significantly lower for cows grazing PRG. Milk yield increased when cows grazed PRGWC (18.07 kg/d) and MULTI (17.84 kg/d) compared to PRG (16.08 kg/d). Milk RCT (mins) and ES (%) were unaffected by treatment. However, offering cows PRGWC and MULTI increased the concentration of C18:2 cis – 9, 12 and C18:3 cis – 9, 12, 15 in milk compared to PRG. Compared to PRG, grazing forages containing clover and herb species improved milk yield and beneficially altered milk fatty acid profile in late lactation dairy cows without negatively impacting milk processability. Abstract

Globally, there were an estimated 9.8 million measles cases and 207 500 measles deaths in 2019. As the effort to eliminate measles around the world continues, modeling remains a valuable tool for public health decision‐makers and program implementers. This study presents a novel approach to the use of a yearly transition function that formulates mathematically the vaccine schedules for different age groups while accounting for the effects of the age of vaccination, the timing of vaccination, and disease seasonality on the yearly number of measles cases in a country. The methodology presented adds to an existing modeling framework and expands its analysis, making its utilization more adjustable for the user and contributing to its conceptual clarity. This article also adjusts for the temporal interaction between vaccination and exposure to disease, applying adjustments to estimated yearly counts of cases and the number of vaccines administered that increase population immunity. These new model features provide the ability to forecast and compare the effects of different vaccination timing scenarios and seasonality of transmission on the expected disease incidence. Although the work presented is applied to the example of measles, it has potential relevance to modeling other vaccine‐preventable diseases. [ABSTRACT FROM AUTHOR]

20. Guys and Dolls: A Qualitative Study of Teachers' Views of Gendered Play in Kindergarten

Abstract

Drawing on data collected for a larger study investigating kindergarten teachers' online discussions of play, the present qualitative study examines teachers' discussions of gender. Findings suggest that teachers' project onto their kindergarten students many of their own gender prejudices about play. These teachers reinforced gendered attitudes by encouraging the children, especially the boys, to play only with toys and in activities traditionally associated with their gender. Email interviews were then conducted with seven kindergarten teachers to further explore the main theme from the netnographic research. Findings support the contention that further research is needed to examine gendered play in kindergartens, and that kindergarten teachers could benefit from becoming aware of their unintentional teachings and in learning how better to encourage gender equity in play-based class activities.

It is therefore important to assess empathy levels among law students using psychometrically sound tools that are suitable for this cohort.Participants and methods: The 20-item Jefferson Scale of Empathy – Health Profession Students Version was adapted for a law context (eg, the word “health care” became “legal”), and the new Jefferson Scale of Empathy – Law Students (JSE-L-S) version was completed by 275 students at Monash University, Melbourne, Australia. Data were subjected to principal component analysis.Results: Four factors emerged from the principal component analysis (“understanding the client’s perspective”, “responding to clients’ experiences and emotions”, “responding to clients’ cues and behaviors”, and “standing in clients’ shoes”), which accounted for 46.7% of the total variance. The reliability of the factors varied, but the overall 18-item JSE-L-S yielded a Cronbach’s alpha coefficient of 0.80. Several patterns among the item loadings were similar to those reported in studies using other versions of the Jefferson Scale of Empathy.Conclusion: The JSE-L-S appears to be a reliable measure of empathy among undergraduate law students, which could help provide insights into law student welfare and future performance as legal practitioners. Additional evaluation of the JSE-L-S is required to disambiguate some of the minor findings explored. Adjustments may improve the psychometric properties. In this note, it is shown that in a symmetric topological space, the pairs of sets separated by the topology determine the topology itself. It is then shown that when the codomain is symmetric, functions which separate only those pairs of sets that are already separated are continuous, generalizing a result found by M. Lynch.

Abstract

Since the year 2000, a concerted campaign against malaria has led to unprecedented levels of intervention coverage across sub-Saharan Africa. Understanding the effect of this control effort is vital to inform future control planning. However, the effect of malaria interventions across the varied epidemiological settings of Africa remains poorly understood owing to the absence of reliable surveillance data and the simplistic approaches underlying current disease estimates. Here we link a large database of malaria field surveys with detailed reconstructions of changing intervention coverage to directly evaluate trends from 2000 to 2015, and quantify the attributable effect of malaria disease control efforts. We found that Plasmodium falciparum infection prevalence in endemic Africa halved and the incidence of clinical disease fell by 40% between 2000 and 2015. We estimate that interventions have averted 663 (542-753 credible interval) million clinical cases since 2000. Insecticide-treated nets, the most widespread intervention, were by far the largest contributor (68% of cases averted). Although still below target levels, current malaria interventions have substantially reduced malaria disease incidence across the continent. Abstract

BackgroundMany women with hyperglycaemia in pregnancy do not receive care during and after pregnancy according to standards recommended in international guidelines. The burden of hyperglycaemia in pregnancy falls disproportionately upon Indigenous peoples worldwide, including Aboriginal and Torres Strait Islander women in Australia. The remote and regional Australian context poses additional barriers to delivering healthcare, including high staff turnover and a socially disadvantaged population with a high prevalence of diabetes.MethodsA complex health systems intervention to improve care for women during and after a pregnancy complicated by hyperglycaemia will be implemented in remote and regional Australia (the Northern Territory and Far North Queensland). The Theoretical Domains Framework was used during formative work with stakeholders to identify intervention components: (1) increasing workforce capacity, skills and knowledge and improving health literacy of health professionals and women; (2) improving access to healthcare through culturally and clinically appropriate pathways; (3) improving information management and communication; (4) enhancing policies and guidelines; (5) embedding use of a clinical register as a quality improvement tool. The intervention will be evaluated utilising the RE-AIM framework at two timepoints: firstly, a qualitative interim evaluation involving interviews with stakeholders (health professionals, champions and project implementers); and subsequently a mixed-methods final evaluation of outcomes and processes: interviews with stakeholders; survey of health professionals; an audit of electronic health records and clinical register; and a review of operational documents. Outcome measures include changes between pre- and post-intervention in: proportion of high risk women receiving recommended glucose screening in early pregnancy; diabetes-related birth outcomes; proportion of women receiving recommended postpartum care including glucose testing; health practitioner confidence in providing care, knowledge and use of relevant guidelines and referral pathways, and perception of care coordination and communication systems; changes to health systems including referral pathways and clinical guidelines.DiscussionThis study will provide insights into the impact of health systems changes in improving care for women with hyperglycaemia during and after pregnancy in a challenging setting. It will also provide detailed information on process measures in the implementation of such health system changes.
