Abstract Ion-beam radiotherapy is an advanced cancer treatment modality offering steep dose gradients and a high biological effectiveness. These gradients make the therapy vulnerable to patient-setup and anatomical changes between treatment fractions, which may go unnoticed. Charged fragments from nuclear interactions of the ion beam with the patient tissue may carry information about the treatment quality. Currently, the fragments escape the patient undetected. Inter-fractional in-vivo treatment monitoring based on these charged nuclear fragments could make ion-beam therapy safer and more efficient. We developed an ion-beam monitoring system based on 28 hybrid silicon pixel detectors (Timepix3) to measure the distribution of fragment origins in three dimensions. The system design choices as well as the ion-beam monitoring performance measurements are presented in this manuscript. A spatial resolution of $${{4}\,\hbox {mm}}$$ 4 mm along the beam axis was achieved for the measurement of individual fragment origins. Beam-range shifts of $${1.5}\,\hbox {mm}$$ 1.5 mm were identified in a clinically realistic treatment scenario with an anthropomorphic head phantom. The monitoring system is currently being used in a prospective clinical trial at the Heidelberg Ion Beam Therapy Centre for head-and-neck as well as central nervous system cancer patients.

Background Living with a family member who has a long-term health condition (HC) is associated with poorer health and well-being outcomes, but the number and socio-demographics of people and families impacted by a family member who has an HC is unknown. Methods Using the Integrated Data Infrastructure (IDI), a collection of linked administrative datasets for the full New Zealand (NZ) population, we identified n= 1,043,172 families using 2013 NZ Census data, and used health data over the previous 5-years to ascertain whether people in these families (n=3,137,517) had cancer, chronic obstructive pulmonary disease, coronary health disease, diabetes, dementia, gout, stroke, traumatic brain injury (TBI) or a mental health/behavioural condition (MHBC). Results Over 60% of families included at least one person with a HC. The most common HCs were MHBCs (39.4% of families), diabetes (16.0%), and TBI (13.9%). A high proportion of multi-generation families (73.9%) included a member with a HC. Two-thirds (67.7%) of Pacific Peoples either had a HC themselves or were living with a family member who had a HC, compared with 62.3% of Europeans and 62.5% of Māori (the indigenous peoples of NZ). At the highest level of socioeconomic deprivation, 57.6% of children lived with a family member who had a HC. Conclusions Three in five NZ families were living with a HC, with differences in the proportion affected according to family composition, socio-economic status and an individuals’ ethnicity. This suggests that there are a substantial number of people at risk of the associated poor outcomes.

Abstract Background Accounting for the co-occurrence of multiple environmental influences is a more accurate reflection of population exposure than considering isolated influences, aiding in understanding the complex interactions between environments, behaviour and health. This study examines how environmental ‘goods’ such as green spaces and environmental ‘bads’ such as alcohol outlets co-occur to develop a nationwide area-level healthy location index (HLI) for New Zealand. Methods Nationwide data were collected, processed, and geocoded on a comprehensive range of environmental exposures. Health-constraining ‘bads’ were represented by: (i) fast-food outlets, (ii) takeaway outlets, (iii) dairy outlets and convenience stores, (iv) alcohol outlets, (v) and gaming venues. Health-promoting ‘goods’ were represented by: (i) green spaces, (ii) blue spaces, (iii) physical activity facilities, (iv) fruit and vegetable outlets, and (v) supermarkets. The HLI was developed based on ranked access to environmental domains. The HLI was then used to investigate socio-spatial patterning by area-level deprivation and rural/urban classification. Results Results showed environmental ‘goods’ and ‘bads’ co-occurred together and were patterned by area-level deprivation. The novel HLI shows that the most deprived areas of New Zealand often have the most environmental ‘bads’ and less access to environmental ‘goods’. Conclusions The index, that is now publicly available, is able to capture both inter-regional and local variations in accessibility to health-promoting and health-constraining environments and their combination. Results in this study further reinforce the need to embrace the multidimensional nature of neighbourhood and place not only when designing health-promoting places, but also when studying the effect of existing built environments on population health.

The Timepix3 is a hybrid pixellated radiation detector consisting of a 256 px × 256 px radiation-sensitive matrix. Research has shown that it is susceptible to energy spectrum distortion due to temperature variations. This can lead to a relative measurement error of up to 35% in the tested temperature range of 10 °C to 70 °C. To overcome this issue, this study proposes a complex compensation method to reduce the error to less than 1%. The compensation method was tested with different radiation sources, focusing on energy peaks up to 100 keV. The results of the study showed that a general model for temperature distortion compensation could be established, where the error in the X-ray fluorescence spectrum of Lead (74.97 keV) was reduced from 22% to less than 2% for 60 °C after the correction was applied. The validity of the model was also verified at temperatures below 0 °C, where the relative measurement error for the Tin peak (25.27 keV) was reduced from 11.4% to 2.1% at −40 °C. The results of this study demonstrate the effectiveness of the proposed compensation method and models in significantly improving the accuracy of energy measurements. This has implications for various fields of research and industry that require accurate radiation energy measurements and cannot afford to use power for cooling or temperature stabilisation of the detector.

The Timepix3 readout ASIC chip is a hybrid pixelated radiation detector, designed at CERN, which contains a 256 px × 256 px matrix. Each of the 65,536 radiation-sensitive pixels can record an incoming particle, its energy deposition or time of arrival and measure them simultaneously. Since the detector is suitable for a wide range of applications from particle physics, national security and medicine to space science, it can be used in a wide range of temperatures. Until now, it has to be calibrated every time to the operating point of the application. This paper studies the possibility of energy measurement with Timepix3 equipped with a 500 m thick silicon sensor and MiniPIX readout interface in the temperatures between 10 ∘C and 70 ∘C with only one calibration. The detector has been irradiated by X-ray fluorescence photons in the energy range from 8 keV to 57 keV, and 31 keV to 81 keV photons from the 133Ba radioactive source. A deviation of 5% in apparent energy value may occur for a 10 ∘C change in temperature from the reference point, but, with the next temperature change, it can reach up to −30%. Moreover, Barium photons with an energy of 81 keV appear as deposited energy of only 55 keV at a detector temperature of 70 ∘C. An original compensation method that reduces the relative measurement error from −30% to less than 1% is presented in this paper.

Chemotherapy has led to many undesirable side effects, as these are toxic drugs that are unable to differentiate between cancer and normal cells. Polyphenols (tea catechins) are an ideal option as alternative chemotherapeutics owing to their inherent anticancer properties, antioxidant properties and being naturally occurring compounds, are deemed safe for consumption. However, without proper administration, the bioavailability of these compounds is low and inefficient. Therefore, proper delivery of these phenolic compounds is vital for cancer therapy. Herein, we analyzed three potential solutions to creating nanoparticle drugs using naturally occurring phenolic compounds (piceatannol (PIC), epigallocatechin gallate hydrophilic (EGCG) and l-epicatechin (EPI)). By using a simple pi-pi stacking mechanism, we utilized boronated PEG (PEG-Br) as an anchor to efficiently load EPI, PIC and EGCG, respectively, to produce three effective phenolic compound-based nanoparticles, which could be delivered safely in systemic circulation, yet detach from its cargo intracellularly to exert its anticancer effect for effective cancer therapy.

Chronic Obstructive Pulmonary Disease is a progressive lung disease affecting the respiratory function of every sixth New Zealander and over 300 million people worldwide. In this paper, we explored how the combination of social, demographical and environmental conditions (represented by increased winter air pollution) affected hospital admissions due to COPD in an urban area of Christchurch (NZ). We juxtaposed the hospitalisation data with dynamic air pollution data and census data to investigate the spatiotemporal patterns of hospital admissions. Spatial analysis identified high-risk health hot spots both overall and season specific, exhibiting higher rates in winter months not solely due to air pollution, but rather as a result of its combination with other factors that initiate deterioration of breathing, increasing impairments and lead to the hospitalisation of COPD patients. From this we found that socioeconomic deprivation and air pollution, followed by the age and ethnicity structure contribute the most to the increased winter hospital admissions. This research shows the continued importance of including both individual (composition) and area level (composition) factors when examining and analysing disease patterns.

Reactions of thiobenzamide or thioacetamide with 4-bromo-1,1-dimethyl-1,4-dihydroisoquinoline-3(2H)-one, 4-bromoisoquinoline-1,3(2H,4H)-dione and two α-bromo(phenyl)acetamides were examined under various conditions (base, solvent, thiophile, temperature) and structure/medium features that influence product distribution (Eschenmoser coupling reaction, Hantzsch thiazole synthesis and elimination to nitriles) were identified. The key factor that enables the successful Eschenmoser coupling reaction involves the optimum balance in acidity of nitrogen and carbon atoms of the intermediary α-thioiminium salts.

A highly modular method for the synthesis of (Z)-3-[amino(phenyl/methyl)methylidene]-1,3-dihydro-2H-indol-2-ones starting from easily available 3-bromooxindoles or (2-oxoindolin-3-yl)triflate and thioacetamides or thiobenzamides is described. A series of 49 compounds, several of which have previously been shown to possess significant tyrosin kinase inhibiting activity, was prepared in yields varying mostly from 70 to 97% and always surpassing those obtained by other published methods. The method includes an Eschenmoser coupling reaction, which is very feasible (even without using a thiophile except tertiary amides) and scalable. The (Z)-configuration of all products was confirmed by NMR techniques.

A family of oxazaborines, diazaborinones, triazaborines, and triazaborinones was prepared by reaction of polarized ethylenes, such as β-enaminoamides, with 4-methylbenzenediazonium tetraphenylborates. The reaction conditions (stirring in CH2Cl2 at room temperature (Method A) or stirring with CH3COONa in CH2Cl2 at room temperature (Method B) or refluxing in the CH2Cl2/toluene mixture (Method C)) controlled the formation and relative content of these compounds in the reaction mixtures from one to three products. Substituted oxazaborines gradually rearranged into diazaborinones at 250 °C. The prepared compounds were characterized by 1H NMR, 13C NMR, IR, and UV–Vis spectroscopy, HRMS, or microanalysis. The structure of individual compounds was confirmed by 11B NMR, 15N NMR, 1D NOESY, and X-ray analysis. The mechanism of reaction of enaminoamides with 4-methylbenzenediazonium tetraphenylborate was proposed.
