Advances in neuromodulation technologies hold the promise of treating a patient’s unique brain network pathology using personalized stimulation patterns. In service of these goals, neuromodulation clinical trials using sensing-enabled devices are routinely generating large multi-modal datasets. However, with the expansion of data acquisition also comes an increasing difficulty to store, manage, and analyze the associated datasets, which integrate complex neural and wearable time-series data with dynamic assessments of patients’ symptomatic state. Here, we discuss a scalable cloud-based data platform that enables ingestion, aggregation, storage, query, and analysis of multi-modal neurotechnology datasets. This large-scale data infrastructure will accelerate translational neuromodulation research and enable the development and delivery of next-generation deep brain stimulation therapies.

The harmonic ratio (HR), derived from the Fourier analysis of trunk accelerations, has been described in various ways as a measure of walking smoothness, walking rhythmicity, or dynamic stability. There is an increasing interest in applying the HR technique to investigate the impact of various pathologies on locomotion; however, explanation of the method has been limited. The aim here is to present a clear description of the mathematical basis of HRs and an understanding of their interpretation. We present harmonic theory, the interpretation of the HR using sinusoidal signals, and an example using actual trunk accelerations and harmonic analyses during limb-loading conditions. We suggest that the HR method may be better defined, not as a measure of rhythmicity or stability, but as a measure of step-to-step symmetry within a stride. [ABSTRACT FROM AUTHOR]

Eight people with Parkinson's disease (PD), 8 age-matched older adults, and 8 young adults executed 3-dimensional rapid aiming movements to 1, 3, 5, and 7 targets. Reaction time, flight time, and time after peak velocity to the 1st target indicated that both neurologically healthy groups implemented a plan on the basis of anticipation of upcoming targets, whereas the PD group did not. One suggested reason for the PD group's deficiency in anticipatory control is the greater variability in their initial force impulse. Although the PD group scaled peak velocity and time to peak velocity similarly to the other groups, their coefficients of variation were greater, making consistent prediction of the movement outcome difficult and thus making it less advantageous to plan too far in advance. A 2nd finding was that the PD group exhibited increased slowing in time after peak velocity in the final segments of the longest sequence, whereas the other 2 groups did not. The increased slowing could be the result of a different movement strategy, increased difficulty modulating the agonist and antagonist muscle groups later in the sequence, or both. The authors conclude that people with PD use more segmented planning and control strategies than do neurologically healthy older and young adults when executing movement sequences and that the locus of increased bradykinesia in longer sequences is in the deceleration phase of movement. This article presents a life needs model of pediatric service delivery that is based on the values of the intrinsic worth, dignity, and strengths of individuals. This developmental, socio-ecological model outlines the major types of service delivery needs of children and youth with disabilities, their families, and their communities within three spheres of life (the personal, interpersonal, and external spheres). The model legitimizes the concept of need, emphasizes the values of family-centered services, and recognizes child and family strengths and capacities. We discuss the utility of this transdisciplinary model in guiding pediatric service delivery to meet the long-range goals of community participation and quality of life of children and youth with disabilities. Background: There is controversy surrounding the relative effectiveness of local irradiation and oral indomethacin for prophylaxis against heterotopic ossification following surgical treatment of acetabular fractures. The purpose of this study was to compare the efficacy of these two commonly used methods in a prospective, randomized trial.Methods: From July 1992 to June 1999, 166 patients in whom a fracture of the acetabulum was treated surgically through a posterior, extensile, or combination approach were randomized to receive either indomethacin or radiation therapy postoperatively. Seventy-eight patients received 800 cGy of local radiation therapy within seventy-two hours after surgery, and seventy-two patients received a six-week course of indomethacin (25 mg three times a day) beginning within twenty-four hours after surgery. Sixteen additional patients were randomized but did not receive treatment with either prophylactic regimen. At an average of fourteen months, the extent of heterotopic ossification was assessed on plain radiographs with use of the classification of Brooker et al. The grade of ossification was correlated with hip motion.Results: There was no significant difference between treatment groups with regard to patient age, gender, Glasgow Coma Scale, operative time, estimated operative blood loss, duration of follow-up, or presence of closed head injury. The Injury Severity Score appeared to be the only covariate that was significantly different between the groups (p = 0.019). Grade-III or IV ossification developed in eight (11%) of the patients in the indomethacin group and three (4%) in the radiation therapy group. The difference was not significant (p = 0.22; 95% confidence interval, -1.1%, +15.7%). No complications related to the prophylaxis were noted in either group. Heterotopic ossification developed in all sixteen patients who did not receive prophylaxis, with six demonstrating grade-III or IV changes. The overall prevalence of grade-III or IV heterotopic ossification was 7% (eleven of 150) in the treated groups and 38% (six of sixteen) in the untreated group. We did not find any association between the prevalence of heterotopic ossification and fracture type (p = 0.296) or posterior hip dislocation (p = 0.306). Grade-I, II, and III heterotopic ossification did not decrease the range of motion of the hip except in flexion.Conclusions: Both local radiation therapy and indomethacin were found to provide effective prophylaxis against heterotopic ossification following surgical treatment of acetabular fractures through a posterior or extensile approach. The 75-kDa low-affinity neurotrophin receptor (p75NTR) has been shown in previous reports to mediate neuronal cell death in vitro and in vivo under certain circumstances. Antisense oligonucleotides directed against p75NTR promote the survival of nerve growth factor-deprived dorsal root ganglia sensory neurons in vitro (Barrett, G.; Bartlett, P., Proc. Natl. Acad. Sci. USA 91:6501–6505; 1994) and axotomized dorsal root ganglia sensory neurons in vivo (Cheema, S. S.; Barrett, G. L.; Bartlett, P. F., J. Neurosci. Res. 46:239–245; 1996). In this study we compared the neuroprotective effects of antisense p75NTR oligonucleotides with two neurotrophic factors, namely nerve growth factor (NGF) and leukemia inhibitory factor, on cultured sensory neurons derived from postnatal day 7 and 14 rat dorsal root ganglia. After 3 d in culture, treatment with the neurotrophic factors had significant survival effects on sensory neuron cultures compared to treatment with basal medium (control). However, after 6 and 9 d in culture these rescue effects were not apparent. In contrast, antisense p75NTR oligonucleotides rescued significantly higher numbers of dorsal root ganglia sensory neurons after 6 and 9 d in culture than treatment with neurotrophic factors, sense oligonucleotides, and basal medium. Furthermore, antisense p75NTR oligonucleotides rescued trkA-, B-, and C-expressing neurons, while NGF and leukemia inhibitory factor targeted primarily the trkA-positive neurons. These findings suggest that antisense-based strategies that inhibit gene expression of cytotoxic molecules are more efficient at preventing postnatal sensory neuronal death in vitro than treatment with individual neurotrophic factors. Extension of the internode below the point of insertion of the flag leaf of Lolium temulentum L. takes place in the lower 10 to 15 mm. Extension of the long epidermal cells also takes place in this region. However, fibre cells show little extension in the lower 10 mm and maximum extension occurs at about 30 mm above the node. Thus, there is extension of fibre cells within that portion of the internode which has ceased extension and is bounded by an epidermis, the cells of which have also ceased elongation. This suggests the occurrence of intrusive growth of the fibres and evidence for this is shown from the measurement of macerated fibres and from electron micrographs. The walls of fibre cells measured in internodes of different lengths and ages thicken only very slightly in the lower 30 mm and most cell wall thickening occurs between 45 and 90 mm above the node. Thus most cell wall thickening occurs after the fibre cells have ceased to elongate. [ABSTRACT FROM AUTHOR]

Community-level coastal management programs are being introduced in some countries as a practical strategy to respond to conditions of poverty and unsustainable resource use practices. Two recently developed Special Area Management (SAM) programs developed in Sri Lanka are part of this international trend. These two SAM programs were assessedto identify planning and early management issues that may be relevant to future projects. This paper examines general issues in assessingcommunity-level projects. The particular focus is on a few issues ofgeneral relevance: community participation in the planning process; the adequacy of the boundary; quality of the technical analysis; adequacy of resource management activities; transparency of management decisions; community acceptance of the program; and sustainability of resource management activities.
