4. Estimating abundance of a small population of Bryde's whales: a comparison between aerial surveys and boat‐based platforms of opportunity.

Abstract

Accurate abundance estimates are essential for the development of effective conservation management strategies, yet they are difficult to produce for small populations that are elusive and sparsely distributed throughout their range. For such populations it is challenging to collect a representative dataset sufficient for robust estimation of detectability and abundance. Over a one‐year study, we used two methods to estimate abundance of a Nationally Critical, widely dispersed Bryde's whale population in the Hauraki Gulf, Aotearoa/New Zealand; (i) distance sampling from systematic line‐transect aerial surveys (n = 22 surveys, 9,944 km, total sightings 21–24 whales), and (ii) mark‐recapture (MR) using photo‐identification images collected from a platform‐of‐opportunity and small‐boat surveys (218 sampling occasions, 27 whales). From the aerial surveys, we estimated an average of 15 whales (95% CI = 6, 30; CV = 37%) at the sea‐surface at any time. For the boat‐based surveys, we developed a custom MR model to address seasonal and individual heterogeneity in capture probabilities and obtained an estimate of 72 distinct whales (95% CI = 38, 106; CV = 24%) in the population. These two approaches provide different perspectives on the abundance and dynamics of Bryde's whales. The aerial surveys estimate the average number of individuals present at any one time, whereas the MR model estimates the total number of animals that used the Gulf during the study. Although neither sampling method is optimal for estimating the abundance of this small, dispersed population, the use of two complementary approaches informs conservation managers about patterns of abundance and distribution over different temporal and spatial scales. It is common to have limited resources for marine research where model assumptions cannot be met. Abstract

INTRODUCTION: Our aim was to gain insight into the effect of COVID-19 measures on SARS-CoV-2 incidence in secondary schools and the association with classroom CO 2 concentration and airborne contamination. METHODS: Between October 2020-June 2021, 18 schools weekly reported SARS-CoV-2 incidence and completed surveys on school-initiated COVID-19 measures (e.g. improving hygiene or minimizing contacts). CO 2 was measured in occupied classrooms twice, and SARS-CoV-2 air contamination longitudinally using electrostatic dust collectors (EDC) and analyzed using RT-qPCR. National COVID-19 policy measures varied during pre-lockdown, lockdown and post-lockdown periods. During the entire study, schools were recommended to improve ventilation. SARS-CoV-2 incidence rate ratios (IRR) were estimated by Generalized Estimating Equation (GEE) models. RESULTS: During 18 weeks follow-up (range: 10-22) SARS-CoV-2 school-incidence decreased during national lockdown (adjusted IRR: 0.41, 95%CI: 0.21-0.80) and post-lockdown (IRR: 0.60, 0.39-0.93) compared to pre-lockdown. School-initiated COVID-19 measures had no additional effect. Pre-lockdown, IRRs per 10% increase in time CO 2 exceeded 400, 550 and 800 ppm above outdoor level respectively, were 1.08 (1.00-1.16), 1.10 (1.02-1.19), and 1.08 (0.95-1.22). Post-lockdown, CO 2-concentrations were considerably lower and not associated with SARS-CoV-2 incidence. No SARS-CoV-2 RNA was detected in any of the EDC samples. CONCLUSION: During a period with low SARS-CoV-2 population immunity and increased attention to ventilation, with CO 2 levels most of the time below acceptable thresholds, only the national policy during and post-lockdown of reduced class-occupancy, stringent quarantine, and contact testing reduced SARS-CoV-2 incidence in Dutch secondary schools. Widespread SARS-CoV-2 air contamination could not be demonstrated in schools under the prevailing conditions during the study.

Abstract

Abstract Background In the pivotal phase III RECOURSE trial, trifluridine/tipiracil (FTD/TPI) improved progression-free and overall survival (PFS, OS) of patients with pre-treated metastatic colorectal cancer (mCRC). Subsequently, the TALLISUR trial provided post-authorisation efficacy and safety data and patient-reported outcomes on quality of life (QoL) in a German patient cohort. The present analysis reports the final data on efficacy, safety and QoL and investigates the impact of baseline characteristics and associated prognostic subgroups on outcome. Methods In this prospective, multi-centre, Germany-wide, phase IV study, patients with pre-treated mCRC were given the choice to receive either FTD/TPI or best supportive care (BSC). To assess the primary endpoint, QoL, EORTC QLQ-C30 questionnaires were employed. Secondary endpoints included QoL assessed through EQ-5D-5L questionnaires, OS, PFS and safety. Abstract

This article synthesizes research on auditors’ ‘bright’ and ‘dark’ personality traits. Our systematic literature review identified 34 studies that met our inclusion criteria. These studies provide insights into auditors’ personality traits, their impact on audit quality, and their association with various professional behaviors and outcomes. Our review emphasizes the importance of understanding personality traits and considering them in the recruitment phase and throughout an auditor’s career. Recognizing the variation in auditors’ personalities and understanding their interplay within audit practices are relevant for improving and refining the profession.

11. Importin 13-dependent axon diameter growth regulates conduction speeds along myelinated CNS axons

Abstract

Abstract Axon diameter influences the conduction properties of myelinated axons, both directly, and indirectly through effects on myelin. However, we have limited understanding of mechanisms controlling axon diameter growth in the central nervous system, preventing systematic dissection of how manipulating diameter affects myelination and conduction along individual axons. Here we establish zebrafish to study axon diameter. We find that importin 13b is required for axon diameter growth, but does not affect cell body size or axon length. Using neuron-specific ipo13b mutants, we assess how reduced axon diameter affects myelination and conduction, and find no changes to myelin thickness, precision of action potential propagation, or ability to sustain high frequency firing. However, increases in conduction speed that occur along single myelinated axons with development are tightly linked to their growth in diameter. This suggests that axon diameter growth is a major driver of increases in conduction speeds along myelinated axons over time.

Abstract

PhD (Occupaional Hygiene), North-West University, Potchefstroom Campus Background: Outdoor agricultural workers such as farmers and farmworkers have been identified as an occupational group that is at high risk of exposure to solar ultraviolet radiation (UVR) during their work activities. Outdoor workers’ exposure to solar UVR need to be managed to prevent negative health effects such as skin cancer and ocular diseases. Skin colour and melanin content of skin provides a measure of genetic protection against exposure to high levels of solar UVR. Photoprotective measures such as wearing long sleeve clothing, broad-brimmed hats and sunglasses have been identified as measures that can reduce an individual’s exposure to solar UVR if used correctly. The solar UVR exposure of outdoor farmworkers has not previously been investigated in South Africa. Aims and objectives: The general aim of this thesis was to quantify the differences in skin colour and melanin content of facultative and constitutive skin of farmworkers on a macadamia nut and avocado farm in the Limpopo Province of South Africa, as well assessing the existing control measures and practices of those farmworkers together with their occupational exposure to solar UVR to determine if the exposure poses a risk to their health. The specific objectives were: (i) to determine the differences in skin colour (individual typology angle, ITA°) and melanin content (melanin index, MI) between the facultative and constitutive skin of farmworkers using skin bioengineering measurements, (ii) to assess the solar UVR practices, in particular the use of ocular protective measures, of the farmworkers on the farm using questionnaires, (iii) to determine the photoprotection provided by the clothing worn by farmworkers using polysulphone (PSF) badges on the shoulder both under and on top of the clothing and (iv) to assess the personal solar UVR exposure of outdoor farmworkers on the farm during autumn, spring and summer while carrying out their different work activities through polysulphone badges placed on their arms, top of the shoulder and top of any head covering worn. Methods: Outdoor and indoor farmworkers on the farm participated in this study. The skin colour and melanin content of the skin of farmworkers were classified using both a subjective method, namely questionnaires and objective methods namely measurement of ITA°) and MI. Outdoor farmworkers’ use of ocular photoprotective measures was determined with the use of a questionnaire. The photoprotection provided by the clothing worn by farmworkers was measured by placing a PSF badge on the shoulder beneath the clothing and on the outside of clothing and classified according to the requirements of the AS/NZS 4399:2017 standard. The solar UVR exposure of outdoor and indoor farmworkers was measured by placing PSF badges on the shoulder, arm and top of the head during autumn, spring and summer during different work activities. Workers’ exposure was compared with the highest value of the International Commission of Illumination (CIE) occupational exposure limit, namely 1.3 SED (standard erythemal dose). Research ethics approval for this study was obtained from the North-West University Health Research Ethics Committee (NWU-00101-17-A1). Results: Significant differences were found between both the ITA° and MI measured on the constitutive (inner upper arm) skin and facultative (back of the hand) of farmworkers. The skin colour of the majority of farmworkers were objectively classified as falling in the Fitzpatrick Phototype V/Brown. A strong correlation was found between ITA° and MI which indicates that both objective methods are appropriate to determine the skin colour of darkly-pigmented skin. The majority of outdoor farmworkers (80%) never wore sunglasses while working outside. Although more than 70% of outdoor farmworkers wore a hat while working, they preferred to wear a cap rather than a broad-brimmed hat which provides better ocular photoprotection. Statistically significant differences were found between the solar UVR underneath and outside of clothing worn by farmworkers during autumn, spring and summer. Only 5% of clothing did not provide the minimum required photoprotection. More than 75% of farmworkers reported that they removed the long sleeve overall jacket they wear as part of their uniform at some point during the day which exposes their arms to solar UVR. Statistically significant differences were found between the solar UVR measured on the shoulder, arm and top of the head of outdoor farmworkers during spring and summer. The arm received the highest solar UVR exposure during autumn (7.8 SED) while the top of the head received the highest levels during spring (11.6 SED) and summer (13.9 SED). The solar UVR exposure of all body sites exceeded the daily CIE exposure limit for more than 80% of measurements during all three seasons. Conclusion: Skin colour and melanin content of farmworkers in this study was found to provide a high level of genetic photoprotection. It was also found that objective and subjective measurement of skin colour and can both be used to classify the skin colour of an occupational group in order to identify genetic photoprotection in workers. The use of ocular photoprotective measures by the outdoor farmworkers in this study was inadequate. The clothing worn by farmworkers in the study provided sufficient photoprotection, but removal of clothing during the day decreased its effectivity. The exposure of farmworkers to solar UVR exceeded the recommended CIE exposure limit during not only summer, but autumn and spring as well which indicates that photoprotective measures should not only be in use during summer but also during the other two seasons. The solar UVR exposure of the outdoor farmworkers included in our study was generally higher than the exposure of farmers and farmworkers in other countries. Abstract

BACKGROUND: Right hemicolectomy or ileocecal resection are used to treat benign conditions like Crohn's disease (CD) and malignant ones like colon cancer (CC).AIM: To investigate differences in pre- and peri-operative factors and their impact on post-operative outcome in patients with CC and CD.METHODS: This is a sub-group analysis of the European Society of Coloproctology's prospective, multi-centre snapshot audit. Adult patients with CC and CD undergoing right hemicolectomy or ileocecal resection were included. Primary outcome measure was 30-d post-operative complications. Secondary outcome measures were post-operative length of stay (LOS) at and readmission.RESULTS: Three hundred and seventy-five patients with CD and 2,515 patients with CC were included. Patients with CD were younger (median = 37 years for CD and 71 years for CC (P < 0.01), had lower American Society of Anesthesiology score (ASA) grade (P < 0.01) and less comorbidity (P < 0.01), but were more likely to be current smokers (P < 0.01). Patients with CD were more frequently operated on by colorectal surgeons (P < 0.01) and frequently underwent ileocecal resection (P < 0.01) with higher rate of de-functioning/primary stoma construction (P < 0.01). Thirty-day post-operative mortality occurred exclusively in the CC group (66/2515, 2.3%). In multivariate analyses, the risk of post-operative complications was similar in the two groups (OR 0.80, 95%CI: 0.54-1.17; P = 0.25). Patients with CD had a significantly longer LOS (Geometric mean 0.87, 95%CI: 0.79-0.95; P < 0.01). There was no difference in re-admission rates. The audit did not collect data on post-operative enhanced recovery protocols that are implemented in the different participating centers.CONCLUSION: Patients with CD were younger, with lower ASA grade, less comorbidity, operated on by experienced surgeons and underwent less radical resection but had a longer LOS than patients with CC although complications' rate was not different between the two groups.

Abstract

A right hemicolectomy is among the most commonly performed operations for colon cancer, but modern high-quality, multination data addressing the morbidity and mortality rates are lacking.This study reports the morbidity and mortality rates for right-sided colon cancer and identifies predictors for unfavorable short-term outcome after right hemicolectomy.This was a snapshot observational prospective study.The study was conducted as a multicenter international study.The 2015 European Society of Coloproctology snapshot study was a prospective multicenter international series that included all patients undergoing elective or emergency right hemicolectomy or ileocecal resection over a 2-month period in early 2015. This is a subanalysis of the colon cancer cohort of patients.Predictors for anastomotic leak and 30-day postoperative morbidity and mortality were assessed using multivariable mixed-effect logistic regression models after variables selection with the Lasso method.Of the 2515 included patients, an anastomosis was performed in 97.2% (n = 2444), handsewn in 38.5% (n = 940) and stapled in 61.5% (n = 1504) cases. The overall anastomotic leak rate was 7.4% (180/2444), 30-day morbidity was 38.0% (n = 956), and mortality was 2.6% (n = 66). Patients with anastomotic leak had a significantly increased mortality rate (10.6% vs 1.6% no-leak patients; p0.001). At multivariable analysis the following variables were associated with anastomotic leak: longer duration of surgery (OR = 1.007 per min; p = 0.0037), open approach (OR = 1.9; p = 0.0037), and stapled anastomosis (OR = 1.5; p = 0.041).This is an observational study, and therefore selection bias could be present. For this reason, a multivariable logistic regression model was performed, trying to correct possible confounding factors.Anastomotic leak after oncologic right hemicolectomy is a frequent complication, and it is associated with increased mortality. Abstract

The building and construction sector is responsible for a large share of carbon emissions resulting in the need to reduce them to mitigate climate change. Timber construction methods promise to lower emissions combined with biogenic carbon storage in the built environment. While there are several studies comparing the emissions of mineral-based and timber-based buildings, a consistent comparison of different timber-based building assemblies is still missing. This study compares carbon emissions from material production and carbon storage capabilities of four timber-based and two brick and reinforced concrete building assemblies. These assemblies were designed for a residential multi-storey building in Berlin, Germany. To compare and rank the carbon impacts of these assemblies we introduce a carbon storage-to-emission ratio. The calculations were performed using a Carbon Cycle Assessment Model implementation in Python. The results indicate an average reduction in carbon emissions of timber-based building assemblies by 32.6% to “Brick” and 40.4% to “Reinforced Concrete”, respectively. Across the timber-based building assemblies, the carbon emissions range between 85 t and 115 t, leading to an average of 105 t per building. Pronounced differences were observed in carbon storage, with the “Dowel Laminated Timber” building assembly storing more than three times the amount of carbon compared to “Light Weight Timber” assembly. To further reduce emissions from buildings and the construction sector and potentially enhance urban carbon storage, “Glue Laminated Timber” and “Dowel Laminated Timber” building assemblies were identified as the most promising.

Abstract

Aim: Patient- and disease-related factors, as well as operation technique, all have the potential to impact on postoperative outcome in Crohn's disease. The available evidence is based on small series and often displays conflicting results. The aim was to investigate the effect of preoperative and intra-operative risk factors on 30-day postoperative outcome in patients undergoing surgery for Crohn's disease. Method: This was an international prospective snapshot audit including consecutive patients undergoing right hemicolectomy or ileocaecal resection. The study analysed a subset of patients who underwent surgery for Crohn's disease. The primary outcome measure was the overall Clavien–Dindo postoperative complication rate. The key secondary outcomes were anastomotic leak, reoperation, surgical site infection and length of stay in hospital. Multivariable binary logistic regression analyses were used to produce odds ratios and 95% confidence intervals. Results: In all, 375 resections in 375 patients were included. The median age was 37 and 57.1% were women. In multivariate analyses, postoperative complications were associated with preoperative parenteral nutrition (OR 2.36, 95% CI 1.10–4.97), urgent/expedited surgical intervention (OR 2.00, 95% CI 1.13–3.55) and unplanned intra-operative adverse events (OR 2.30, 95% CI 1.20–4.45). The postoperative length of stay in hospital was prolonged in patients who received preoperative parenteral nutrition (OR 31, 95% CI 1.08–1.61) and those who had urgent/expedited operations (OR 1.21, 95% CI 1.07–1.37). Conclusion: Preoperative parenteral nutritional support, urgent/expedited operation and unplanned intra-operative adverse events were associated with unfavourable postoperative outcome. Enhanced preoperative optimization and improved planning of operation pathways and timings may improve outcomes for patients. Abstract

Conceptual approaches to thoroughly study governance of cruise tourism are lacking in the literature. Relying on Castells’ network society, we analyze how two interconnected flows of cruise ships and passengers are governed by a marine community of users and policy makers. Bonaire is used as a case study. Research shows that the transnational cruise ship flow increasingly determines the local passenger flow at Bonaire. Therefore, the marine community increasingly connects with and adapts to the requirements of the transnational cruise network. Moreover, unequal power relations between cruise networks and flows prioritize the economy over the environment at Bonaire.

Abstract

Abstract Background The prognosis of patients with advanced biliary tract cancer (BTC) who have progressed on gemcitabine plus cisplatin is dismal. Trifluridine/tipiracil (FTD/TPI) and irinotecan have proven efficacy in different gastrointestinal malignancies. We therefore hypothesized that this combination might improve the therapeutic outcome in patients with BTC after failure of first line treatment. Methods TRITICC is an interventional, prospective, open-label, non-randomised, exploratory, multicentre, single-arm phase IIA clinical trial done in 6 sites with expertise in managing biliary tract cancer across Germany. A total of 28 adult patients (aged ≥ 18 years) with histologically verified locally advanced or metastatic biliary tract cancer (including cholangiocarcinoma and gallbladder or ampullary carcinoma) with documented radiological disease progression to first-line gemcitabine based chemotherapy will be included to receive a combination of FTD/TPI plus irinotecan according to previously published protocols. Study treatment will be continued until disease progression according to RECIST 1.1 criteria or occurrence of unacceptable toxicity. The effect of FTD/TPI plus irinotecan on progression-free survival will be analyzed as primary endpoint. Safety (according to NCI-CTCAE), response rates and overall survival are secondary endpoints. In addition, a comprehensive translational research program is part of the study and might provide findings about predictive markers with regard to response, survival periods and resistance to treatment. Discussion The aim of TRITICC is to evaluate the safety and efficacy of FTD/TPI plus irinotecan in patients with biliary tract cancer refractory to previous Gemcitabine based treatment. Abstract

Aim The anastomosis technique used following right-sided colonic resection is widely variable and may affect patient outcome. This study aimed to assess the association between leak and anastomosis technique (stapled vs handsewn)., Method This was a prospective, multicentre, international audit including patients undergoing elective or emergency right hemicolectomy or ileo-caecal resection operations over a 2-month period in early 2015. The primary outcome measure was the presence of anastomotic leak within 30 days of surgery, determined using a prespecified definition. Mixed effects logistic regression models were used to assess the association between leak and anastomosis method, adjusting for patient, disease and operative cofactors, with centre included as a random-effect variable., Results This study included 3208 patients, of whom 78.4% (n = 2515) underwent surgery for malignancy and 11.7% (n = 375) underwent surgery for Crohn's disease. An anastomosis was performed in 94.8% (n = 3041) of patients, which was handsewn in 38.9% (n = 1183) and stapled in 61.1% (n = 1858). Patients undergoing hand-sewn anastomosis were more likely to be emergency admissions (20.5% handsewn vs 12.9% stapled) and to undergo open surgery (54.7% handsewn vs 36.6% stapled). The overall anastomotic leak rate was 8.1% (245/3041), which was similar following handsewn (7.4%) and stapled (8.5%) techniques (P = 0.3). After adjustment for cofactors, the odds of a leak were higher for stapled anastomosis (adjusted OR = 1.43; 95% CI: 1.04-1.95; P = 0.03)., Conclusion Despite being used in lower-risk patients, stapled anastomosis was associated with an increased anastomotic leak rate in this observational study. Further research is needed to define patient groups in whom a stapled anastomosis is safe.

Abstract

Climate change has been credited with making the Arctic Ocean more accessible to maritime transport, but those navigating its waters must contend with highly dynamic and unpredictable weather and sea ice conditions. Despite ongoing developments in weather and sea ice services to enable responsible and safe operation and navigation, little is known about what and how these information services are used, and whether the services meet the information needs of maritime sectors. Here we address this gap by providing insights from the expedition cruise tourism sector active in European Arctic ocean areas. By applying a social practice theory perspective and relying on semi-structured interviews we show how weather and sea ice services are currently used by expedition cruise operators in various operational decision-making contexts. We argue that practice theory contributes to understanding user needs in a more contextualized way, which is essential for enhancing and tailoring information services. Our analysis suggests that weather and sea ice are only two elements in a complex socio-material setting. Finally, we argue that weather and sea ice services contribute to the resilience and rationalization of the expedition cruise sector in the European Arctic and beyond.

Abstract

Tax legislation traditionally distinguishes between returns on investment paid on equity and debt instruments. In the main, returns on debt instruments (interest payments) are deductible for the paying company, while distributions on equity instruments (dividends) are not. This difference in taxation can be exploited using hybrid instruments and often leads to a debt bias in investment patterns. South Africa, Australia and Canada have specific rules designed to prevent the circumvention of tax liability when company distributions are made in respect of hybrid instruments. In principle, Australia and Canada apply a more robust approach to prevent tax avoidance and also tend to include a wider range of transactions, as well as an unlimited time period in their regulation of the taxation of distributions on hybrid instruments. In addition to the anti-avoidance function, a strong incentive is created for taxpayers in Australia and Canada to invest in equity instruments as opposed to debt. This article suggests that South Africa should align certain principles in its specific rules regulating hybrid instruments with those in Australia and Canada to ensure optimal functionality of the South African tax legislation. Abstract

I Editor i Hidradenitis suppurativa (HS) is a chronic, inflammatory disease affecting the intertriginous skin of axillary, inguinal and perianal sites.1 In these areas, patients develop painful and disabling skin lesions such as inflamed nodules, abscesses and fistula with foul-smelling secretions. Data were determined for following patient Groups: Group 1 (HS patients with drug therapy according to HS guidelines [HS drug therapy]), Group 2 (HS patients without HS drug therapy), Groups 3 (patients with suspected HS diagnosis and with HS drug therapy), Groups 4 (patients with suspected HS diagnosis and without HS drug therapy), Group 5 (potential HS patient with misdiagnosis and with HS drug therapy) and Group 6 (potential HS patient with misdiagnosis and without HS drug therapy). Importantly, the amount of mean sickness benefits was higher for HS patients than for other insured individuals (727€ vs. 311 €; average for insured patients with sickness benefit claims in SHI cohort was 311€ in 2012, calculation by the authors). Abstract

Aim. To collect data on the use of placebo interventions by GPs in Germany. Methods. A questionnaire was mailed to 400 randomly selected GPs in Bavaria. Non-responders were reminded by telephone after 4 weeks and were given a second copy of the questionnaire after a further 3 weeks. Results. In all, 208 completed questionnaires were returned. The majority of GPs (88%) have used a placebo at least once in their practice; 45% have used pure placebos, such as saline injections and sugar pills, at least once last year; the median frequency of use was 5 [interquartile range (IQR), 2-10]. The use of impure placebos during the past year was more common: 76% of GPs have used impure placebos, i.e. medical interventions that have pharmacological or physical activity but have no intrinsic effect (e.g. pharmacological or physical action) on the patient's disease or its symptoms, with a median frequency of 20 times per year (IQR, 10-50). The main reason for the use of placebo was a possible psychological effect, followed by the expectation of patients to receive a treatment. For the majority of GPs placebo interventions were ethically justified if they were used for a possible psychological effect. Conclusions. Placebo interventions are a widely accepted part of medical treatment in German general practices and are used primarily for their psychological effects. Abstract

Aim The anastomosis technique used following right-sided colonic resection is widely variable and may affect patient outcome. This study aimed to assess the association between leak and anastomosis technique (stapled vs handsewn). Method This was a prospective, multicentre, international audit including patients undergoing elective or emergency right hemicolectomy or ileo-caecal resection operations over a 2-month period in early 2015. The primary outcome measure was the presence of anastomotic leak within 30 days of surgery, determined using a prespecified definition. Mixed effects logistic regression models were used to assess the association between leak and anastomosis method, adjusting for patient, disease and operative cofactors, with centre included as a random-effect variable. Results This study included 3208 patients, of whom 78.4% (n = 2515) underwent surgery for malignancy and 11.7% (n = 375) underwent surgery for Crohn's disease. An anastomosis was performed in 94.8% (n = 3041) of patients, which was handsewn in 38.9% (n = 1183) and stapled in 61.1% (n = 1858). Patients undergoing hand-sewn anastomosis were more likely to be emergency admissions (20.5% handsewn vs 12.9% stapled) and to undergo open surgery (54.7% handsewn vs 36.6% stapled). The overall anastomotic leak rate was 8.1% (245/3041), which was similar following handsewn (7.4%) and stapled (8.5%) techniques (P = 0.3). After adjustment for cofactors, the odds of a leak were higher for stapled anastomosis (adjusted OR = 1.43; 95% CI: 1.04-1.95; P = 0.03). Conclusion Despite being used in lower-risk patients, stapled anastomosis was associated with an increased anastomotic leak rate in this observational study. Further research is needed to define patient groups in whom a stapled anastomosis is safe.

Abstract

The strategic location of small islands in the Caribbean, close to the United States of America (USA), and their historical trade roots as former colonies of Europe make them an interesting business environment. Small islands' eagerness for economic development and their limited governance capacity often result in an unequal relationship between multinational private parties and small islands' policy actors, especially in regard to environmental management. This is also observed at St. Eustatius, a small Caribbean island that hosts a crucial oil storage and transshipment terminal that compromises the environmental state of the small island. However, in 2010, St. Eustatius (Statia) became part of the Netherlands, which significantly changed the responsibilities related to environmental management. Bringing the environmental state back in reversed existing power relations. To analyze these changing power dynamics, we apply the new social scientific concept of marine community, which encompasses a user community and a policy community and shows the different interests and power dynamics within and between them. While governance of the oil terminal used to be determined by structural power in the user community on behalf of NuStar, it currently relies on the structural power of the Dutch Ministry of Infrastructure and Environment (I&E) in the policy community. In theory, structural power to bring the environmental state back in would be beneficial for governance. In practice, however, this is challenging because a small island environmental state is different from the environmental state in countries in Western societies. Although the Dutch Ministry has structural power, the way it relates to others (dispositional power) and uses resources (relational power) should be better adapted to the needs and characteristics of small island environmental states such as Statia.

Abstract

Abstract The hobo syndrome (i.e., the wanderlust someone posits to frequently change employers) has a behavioural (i.e., frequent job-quitting behaviour) and an attitudinal dimension (i.e., attitudes towards frequent job-quitting). Across two studies, we examine both dimensions across 348 career starters. By doing so, we expand our understanding of Ghiselli’s hobo syndrome in two ways: (a) we explore the effect of both ‘bright’- and ‘dark’-side personality traits on each dimension of the hobo syndrome, and (b) using longitudinal research, we shed light on the role of each dimension of the hobo syndrome in predicting actual job-quitting behaviour. Data for both studies were gathered through a survey and LinkedIn. Results of regression analyses show that psychopathy is associated with both dimensions of the hobo syndrome. Conversely, openness to experience is only associated with the attitudinal dimension, while agreeableness and extraversion are only associated with the behavioural dimension. Finally, we find that only the behavioural dimension is associated with the length of tenure with the first employer, suggesting that one’s intentions to frequent job-quitting are more important in predicting one’s actual job-quitting behaviour than one’s attitudes towards frequent job-quitting.
