为改善我国粮油物料机械化烘干过程中存在的热效率不高、热风温度波动大及污染环境等问题，研发了新型粮油烘干机，对新型粮油烘干机的工艺参数、结构、高效供热装置、多重环保装置、过程监测控制等方面开展的集成化设计和创新型应用工作进行了阐述。新型粮油烘干机采用负压吸入式进风形式，工艺参数上设置不同的烘干缓苏时间比，热风风量较以往设计提高20%，控制表观风速在0.20~0.25 m/s，实时监测料温以控制受热温度和时间；结构上采用全镀锌装配式结构，多段烘干和缓苏相结合工艺，变截面角状上下交错布置形式，分层进风梯度冷却方式，变频电机驱动可调节排料机构，多点进料、多点出料方式；同时采用天然气线性燃烧技术和燃烧装置供热，重力沉降和离心除尘相结合对烘干废气进行处理及多个温度传感器进行干燥过程的监测控制。实际应用表明，新型粮油烘干机具有高效、节能、低碳、清洁的特点，符合我国现代农业高质量发展的实际需求。In order to improve problems such as the low thermal efficiency, hot air temperature fluctuations and environmental pollution existed in drying process of grain and oilseed in China, a new type of grain and oilseed dryer was researched and developed.The integrated design and innovative application of the process parameters, structure, efficient heating device, multiple environmental protection device, process monitoring and control of the new grain and oilseed dryer were expounded. The new grain and oilseed dryer adopted negative pressure suction air intake form, different drying retardation time ratios were set on the process parameters, the hot air volume increased by 20% compared with the previous design, the apparent wind speed was controlled at 0.20-0.25 m/s, and the material temperature was monitored in real time to control the heating temperature and time. The structure adopted the fully galvanized assembled structure, the combination of multi-stage drying and retardation process, the variable cross-section angular upper and lower staggered arrangement form, the gradient cooling mode of layered air intake, the frequency conversion motor-driven adjustable discharging mechanism, the multi-point feeding and discharging mode, while using natural gas linear combustion technology and combustion devices for heating, gravity settling combined with centrifugal dust removal for drying exhaust gas treatment and multiple temperature sensors for drying process monitoring and control. The actual application shows that the new model has the characteristics of high efficiency, energy saving, low carbon, clean, in line with the actual needs of China′s modern agricultural development of high quality.

Introduction: The popular traditional Chinese medicine (TCM) compound FYTF-919 (Zhong Feng Xing Nao prescription) may improve outcome from acute intracerebral hemorrhage (ICH) through effects on brain edema, hematoma absorption, and the immune system. This study is to assess whether FYTF-919 is safe and effective as compared to matching placebo treatment in patients with acute ICH. Methods: The ongoing Chinese Herbal medicine in patients with Acute INtracerebral hemorrhage (CHAIN) is a multicenter, prospective, randomized, double-blind placebo-controlled trial of FYTF-919 in patients with acute ICH at 20–30 hospital sites in China. Eligible ICH patients presenting within 48 h after symptom onset are randomly allocated to receive either FYTF-919 (100 mL per day × 28 d, oral) or matching placebo. A sample size of 1,504 patients is estimated to provide 90% power (α 0.05) to detect a ≥20% improvement in average utility-weight scores on the modified Rankin scale (UW-mRS) assessed at 90 days, with 6% non-adherence and 10% lost to follow-up. The primary efficacy outcome is UW-mRS at 90 days. Secondary outcomes include binary measures of the mRS, neurological impairment on the National Institute of Health Stroke Scale, and health-related quality of life on the EuroQol EQ-5D-5L scale at different time points over 6 months of follow-up. The key safety measure is serious adverse events. Conclusion: CHAIN is on schedule to provide reliable evidence over the benefits of a popular herbal TCM for the treatment of acute ICH. Background: Uncertainty exists over the optimal level of blood pressure (BP) after mechanical thrombectomy (MT) for acute ischemic stroke (AIS). Objectives: We aim to determine the effectiveness and safety of intensive BP-lowering following MT reperfusion of large-vessel occlusion (LVO)-related AIS. Design: The second ENhanced Control of Hypertension ANd Thrombolysis strokE stuDy (ENCHANTED2) is an investigator-initiated, multicenter, prospective, randomized, open, blinded-endpoint (PROBE) trial of intensive systolic BP (SBP) control in reperfused (extended treatment in cerebral infarction (eTICI) classification 2b/2c/3) LVO-AIS patients with persistent hypertension (SBP ⩾ 140 mmHg) at 60+ sites in China, and Australia and the United Kingdom. Eligible patients are centrally randomly allocated to more- (target SBP ⩽ 120 mmHg within 1 h) or less-intensive (target SBP 140–180 mmHg) BP management, to be maintained for 72 h. Primary outcome is an ordinal shift analysis of scores on the modified Rankin scale (mRS) at 90 days. Sample size of 2257 patients provides 90% power to detect a 6.5% absolute reduction in poor outcome from more-intensive BP-lowering using ordinal logistic regression. Progress: Recruitment started in China in July 2020. At a meeting of the independent Data and Safety Monitoring Board in March 2022 to review primary outcome data available for 347 patients, they recommended suspension of recruitment due to safety concerns in the more-intensive group; which was implemented by the Trial Steering Committee (TSC) with 817 randomized patients only in China. The TSC then stopped recruitment after the safety concerns persisted on further review of the data in June 2022. The TSC will make a decision on restarting the trial with modification of the protocol when the results are made public. Discussion: ENCHANTED2 will provide further randomized evidence on the role of intensive BP-lowering after reperfusion in MT-treated AIS patients. Background: The epidemiology of stroke is evolving in China as the population undergoes demographic, lifestyle, and economic transitions. An updated review is pertinent to providing feedback on current, and in planning future, prevention and management strategies. Aims: To identify high-quality epidemiological studies for quantifying the prevalence, incidence, mortality, and case fatality for stroke in China. Methods: A search was undertaken across a range of bibliographic databases on 30 November 2021 without time limitation. Assessments were made of the risk of bias of the included studies. The outcomes were synthesized using a random-effects model. Subgroup analysis and meta-regression models were used to define the source of heterogeneity. Results: Of 9407 identified records, 26 population-based studies were included. Due to significant heterogeneity across the studies, the original range for crude rates of indices was wide. The pooled annual prevalence was 1329.5/100,000 (95% confidence interval (CI) 713.6–2131.9, p < 0.001), incidence 442.1/100,000 (327.6–573.8, p < 0.001), mortality 154.1/100,000 (52.6–308.8, I2 = 100%, p < 0.001), and case fatality 35.8% (26.1% to 46.1%, I2 = 97%, p < 0.001). The prevalence and incidence of stroke have increased, but stroke-related case fatality has declined in China over recent decades. There are significant regional and rural-urban differences in incidence rates. Conclusion: Despite improved public health policies and healthcare delivery, the burden of stroke remains high in China. Further coordinated efforts are required in prevention and community care to offset the likelihood of further expansion in the absolute number of stroke cases in this large population. A commentary on the effects of scar massage on burn scars was published in the Journal of Clinical Nursing. The study conducted a systematic review and meta-analysis of randomized controlled trials and quasi-experimental trials to investigate the physical and psychological effects of scar massage on burn victims. The findings showed that scar massage greatly improves scar development and lessens anxiety and itching in burn patients. The study suggests that non-pharmacological techniques, such as massage therapy, can help reduce discomfort and anxiety in burn sufferers. However, further research is needed to address limitations in the included studies and to compare the effects of different types of massage therapy. [Extracted from the article]

The intestinal mucosal barrier is critical for the absorption of nutrients and the health of both humans and animals. Recent publications from clinical and experimental studies have shown the importance of the nutrients‐bacteria‐host interaction for the intestinal homeostasis. Dysfunction of these interactions has been reported to be associated with metabolic disorders and development of intestinal diseases such as the irritable bowel syndrome and inflammatory bowel diseases. Tryptophan and its metabolites, including kynurenine, kynurenic acid, and 5‐hydroxytrptamine, can influence the proliferation of enterocytes, intestinal integrity, and immune response, as well as intestinal microbiota, therefore, regulating and contributing to the intestinal health. In this review, we highlight recent findings on the effect of tryptophan and its metabolites on the mucosal barrier and intestinal homeostasis and its regulation of innate immune response. Moreover, we present the signaling pathways related to Trp metabolism, such as mammalian target of rapamycin, aryl hydrocarbon receptor, and pregnane X receptor, which contribute to the intestinal homeostasis and discuss future perspectives on spontaneous interference in host tryptophan metabolism as potential clinical strategies of intestinal diseases. Introduction: We investigated the effect of hematoma volume reduction with minimally invasive surgery (MIS) on intracranial pressure (ICP) and cerebral perfusion pressure (CPP) in patients with large spontaneous intracerebral hemorrhage (ICH). Methods: Post-hoc analysis of the Minimally Invasive Surgery Plus Alteplase for Intracerebral Hemorrhage Evacuation (MISTIE III) study, a clinical trial with blinded outcome assessments. The primary outcome was the proportion of ICP readings ≥20 and 30 mmHg, and CPP readings <70 and 60 mm Hg. Secondary outcomes included major disability (modified Rankin scale >3) and mortality at 30 and 365 days. We assessed the relationship between proportion of high ICP and low CPP events and MIS using binomial generalized linear models, and outcomes using multiple logistic regression. Results: Of 499 patients enrolled in MISTIE III, 72 patients had guideline based ICP monitors placed, 34 in the MIS group and 38 in control (no surgery) group. Threshold ICP and CPP events ≥20/ <70 mmHg occurred in 31 (43.1%) and 52 (72.2%) patients respectively. On adjusted analyses, proportion of ICP readings ≥20 and 30 mmHg were significantly lower in the MIS group vs. control group [Odds Ratio (OR) 0.27, 95% Confidence Interval [CI] 0.11–0.63 (p = 0.002); OR = 0.18, 0.04–0.75, p = 0.02], respectively. Proportion of CPP readings <70 and 60 mm Hg were also significantly lower in MIS patients [OR 0.31, 95% CI 0.15–0.63 (p = 0.001); OR 0.30, 95% CI 0.11–0.83 (p = 0.02)], respectively. Higher proportions of CPP readings <70 and 60 mm were significantly associated with short term mortality (p = 0.04), and (p = 0.006), respectively. Long term mortality was significantly associated with higher proportion of time with ICP ≥ 20 (p = 0.04), ICP ≥ 30 (p = 0.04), and CPP <70 mmHg (p = 0.01). Conclusion: Our results are consistent with the hypothesis that surgical reduction of ICH volume decreases proportion of high ICP and low CPP events and that these variables are associated with short- and long-term mortality. Background and Purpose: The systemic immune–inflammation index (SII) is a novel prognostic index in various diseases. We evaluated the predictive value of SII in patients with intracerebral hemorrhage (ICH). Methods: Patients with primary spontaneous ICH were enrolled. SII was constructed based on peripheral platelet (P), neutrophil (N), and lymphocyte (L) and defined as P*N/L. In addition to admission testing, acute phase SII was collected to analyze the potential dynamic change. Poor outcome was defined as modified Rankin Scale of more than 3 at 90 days. Results: We included 291 patients; 98 (34%) achieved favorable functional outcomes. Day-1 SII was higher and was more related to poor outcome than was admission SII. Median time of day-1 SII was 29 h from onset. Day-1 SII had an OR in outcome (mRS >3) 1.74 (95% CI = 1.03–3.00, p = 0.04). The binary cutoff point of SII calculated using the area under the curve (AUC) method was 1,700 × 109/L, AUC 0.699 (95% CI = 0.627–0.774) (sensitivity 53.3%, specificity 77.3%) (OR = 2.36, 95% CI = 1.09–5.26, p = 0.03). We study a single‐period inventory planning problem for a category of substitutable products. This is an important practical problem facing category managers who have to maintain high service levels for constantly expanding product catalogs. We formulate the problem as a stochastic optimization model that minimizes the total stocking cost subject to service level requirements, which consist of product‐specific and category‐wide targets for inventory availability (ready rates) through the selling season. Our model accounts for stochastic customer arrivals, captures stockout‐based substitutions, and determines initial stocking quantities jointly for all products. Recognizing the challenges that these aspects pose in solving the problem, we propose an optimization‐based method that estimates the ready rates using a deterministic approximation and discretizes the selling season into a finite number of time intervals. This novel modeling approach permits us to recast the stochastic optimization model as a deterministic mixed integer linear program that can accommodate several common stockout‐based substitution schemes. We characterize the worst‐case behavior of this approach to develop performance guarantees. We also implemented and applied this model to randomly generated numerical instances featuring different types of product differentiation and varying in parameter values. We observe that the approach is robust to changes in problem parameter values and yields solutions very quickly, outperforming an enumeration‐based alternative, a practical heuristic, and an approach based on extant literature. Finally, we applied our approach to data from a re‐seller of Information Technology products. Proper molecular packing is key to improving the performance of non-fullerene organic solar cells (OSCs) based on perylene diimides (PDI). Herein, we implement a novel concept of introducing intramolecular π–π stacking for the design of PDI-based acceptors. We synthesized and systematically studied two isomeric PDI-tetramers with different functionalization positions (α- and β-positions) of naphthalene, and thus different extents of intramolecular packing. Interestingly, both PDI molecules exhibit two π–π stacking peaks with intramolecular and intermolecular stacking distances of 3.7–3.8 Å and 4.6–4.7 Å, respectively. Most importantly, the α-positioned PDI shows stronger intramolecular π-stacked features as evidenced by UV-Vis and photoluminescence, and morphological measurements. Consequently, the α-positioned PDI achieved a higher device efficiency of 8.6% with an open-circuit voltage of 1.15 V and a reasonably high fill factor of 60%. Abstract: Compared to the rapid development of nonfullerene organic solar cells (OSCs) based on the state‐of‐the‐art indacenodithiophene (IDT)‐based small molecule acceptors (SMAs), the progress for perylene diimide (PDI)‐based electron acceptors has lagged behind owing to the lack of understanding on the structure–morphology–performance relationship of PDI SMAs. Given the ease of synthesis for PDIs and their high intrinsic electron mobility, it is crucial to identify key material parameters that influence the polymer:PDI blend morphology and to develop rational approaches for molecular design toward high‐performance PDI‐based SMAs. In this study, three pairs of PDI‐based SMAs with and without ring‐fusion are investigated and it is found that ring‐fusion and domain purity are the key structural and morphological factors determining the fill factors (FFs) and efficiencies of PDI‐based nonfullerene OSCs. This data shows that nonfullerene OSCs based on the ring‐fused PDI‐based SMAs exhibit much higher average domain purity and thus increased charge mobilities, which lead to enhanced FFs compared to those solar cells based on nonfused PDIs. This is explained by higher Florry Huggins interaction parameters as observed by melting point depression measurements. This study suggests that increasing repulsive molecular interactions to lower the miscibility between the polymer donor and PDI acceptor is the key to improve the FF and performance of PDI‐based devices. Abstract: The device performance of polymer solar cells (PSCs) is strongly dependent on the blend morphology. One of the strategies for improving PSC performance is side‐chain engineering, which plays an important role in controlling the aggregation properties of the polymers and thus the domain crystallinity/purity of the donor–acceptor blends. In particular, for a family of high‐performance donor polymers with strong temperature‐dependent aggregation properties, the device performances are very sensitive to the size of alkyl chains, and the best device performance can only be achieved with an optimized odd‐numbered alkyl chain. However, the synthetic route of odd‐numbered alkyl chains is costly and complicated, which makes it difficult for large‐scale synthesis. Here, this study presents a facile method to optimize the aggregation properties and blend morphology by employing donor polymers with a mixture of two even‐numbered, randomly distributed alkyl chains. In a model polymer system, this study suggests that the structural and electronic properties of the random polymers comprising a mixture of 2‐octyldodecyl and 2‐decyltetradecyl alkyl chains can be systematically tuned by varying the mixing ratio, and a high power conversion efficiency (11.1%) can be achieved. The challenge of continuous printing in high-efficiency large-area organic solar cells is a key limiting factor for their widespread adoption. A materials design concept for achieving large-area, solution-coated all-polymer bulk heterojunction solar cells with stable phase separation morphology between the donor and acceptor is presented. The key concept lies in inhibiting strong crystallization of donor and acceptor polymers, thus forming intermixed, low crystallinity, and mostly amorphous blends. Based on experiments using donors and acceptors with different degree of crystallinity, the results show that microphase separated donor and acceptor domain sizes are inversely proportional to the crystallinity of the conjugated polymers. This methodology of using low crystallinity donors and acceptors has the added benefit of forming a consistent and robust morphology that is insensitive to different processing conditions, allowing one to easily scale up the printing process from a small-scale solution shearing coater to a large-scale continuous roll-to-roll (R2R) printer. Large-area all-polymer solar cells are continuously roll-to-roll slot die printed with power conversion efficiencies of 5%, with combined cell area up to 10 cm2. Here we report a series of tetraphenyl carbon-group (tetraphenylmethane (TPC), tetraphenylsilane (TPSi) and tetraphenylgermane (TPGe)) core based 3D-structure non-fullerene electron acceptors, enabling efficient polymer solar cells with a power conversion efficiency (PCE) of up to ∼4.3%. The results show that TPC and TPSi core-based polymer solar cells (PSCs) perform significantly better than that based on TPGe. A family of three isobenzofulvene mono-adduct fullerene derivatives are synthesized and applied as the acceptor for organic photovoltaic (OPV) cells. PTB7-based OPV cells incorporating these new fullerene derivatives show increased Voc compared to the corresponding PC60BM-based cells. The best performing derivative in this fullerene family is IC60MA-4C that contains the longest alkyl chain and that yields a power conversion efficiency of 6.5% when combined with PTB7. The authors conclude that endoscopic hematoma evacuation was safe and effective treatment for mass effect for those patients. The conventional method to prepare bulk‐heterojunction organic photovoltaics (OPVs) is a one‐step method from the blend solution of donor and acceptor materials, known as blend‐casting (BC). Recently, an alternative method was demonstrated to achieve high efficiencies (13%) comparable to state‐of‐the‐art BC devices. This two‐step‐coating method, known as "sequential processing," (SqP) involves sequential deposition of the donor and then the acceptor from two orthogonal solvents. However, the requirement of orthogonal solvents to process the donor and acceptor constrains the choice of materials and processing solvents. In this paper, an improved version of SqP method without the need for using orthogonal solvents is reported. The success is based on donor polymers with strong temperature‐dependent aggregation properties whose solution can be processed at a high temperature, but the resulting film becomes completely insoluble at room temperature, which allows for the processing of overlying acceptors from a wide range of nonorthogonal solvents. With this approach, efficient SqP OPVs is demonstrated based on a range of donor/acceptor materials and processing solvents, and, in every single case, SqP OPVs can outperform their BC counterparts. Background: Minimally invasive surgery (MIS) for spontaneous supratentorial intracerebral haemorrhage (ICH) is controversial but may be beneficial if end-of-treatment (EOT) haematoma volume is reduced to ≤15 mL. We explored whether MRI findings of cerebral small vessel disease (CSVD) modify the effect of MIS on long-term outcomes., Methods: Prespecified blinded subgroup analysis of 288 subjects with qualified imaging sequences from the phase 3 Minimally Invasive Surgery Plus Alteplase for Intracerebral Haemorrhage Evacuation (MISTIE) trial. We tested for heterogeneity in the effects of MIS and MIS+EOT volume ≤15 mL on the trial's primary outcome of good versus poor function at 1 year by the presence of single CSVD features and CSVD scores using multivariable models., Results: Of 499 patients enrolled in MISTIE III, 288 patients had MRI, 149 (51.7%) randomised to MIS and 139 (48.3%) to standard medical care (SMC). Median (IQR) ICH volume was 42 (30-53) mL. In the full MRI cohort, there was no statistically significant heterogeneity in the effects of MIS versus SMC on 1-year outcomes by any specific CSVD feature or by CSVD scores (all P interaction >0.05). In 94 MIS patients with EOT ICH volume ≤15 mL, significant reduction in odds of poor outcome was found with cerebral amyloid angiopathy score <2 (OR, 0.14 (0.05-0.42); P interaction =0.006), absence of lacunes (OR, 0.37 (0.18-0.80); P interaction =0.02) and absence of severe white matter hyperintensities (WMHs) (OR, 0.22 (0.08-0.58); P interaction =0.03)., Conclusions: Following successful haematoma reduction by MIS, we found significantly lower odds of poor functional outcome with lower total burden of CSVD in addition to absence of lacunes and severe WMHs. Exploring the molecular mechanisms of PD-1/PDL-1 blockade for non-small cell lung cancer (NSCLC) would facilitate understanding for tumor microenvironment (TME) and development of individualized medicine. To date, biomarkers of response to PD-1 blockade therapy were still limited. In this study, we hypothesize that cell type in the tumor microenvironment can influence the effect of PD-1 blockade immunotherapy through specific genes. Therefore, we re-analyze the single-cell RNA sequencing data and validation in tissue from lung adenocarcinoma patients. Dynamic changes of cellular subpopulation were observed after anti-PD-1 immunotherapy among TMEs between primary/metastasis or good/poor response patients. Non-exhausted CD8 T cells and dysregulated genes were observed in responsing patients from PD-1 blockade therapy. Among all changed genes, JUN, involved in PD-1 blockade immunotherapy pathway, and could be considered as a PD-1 responsing biomarker.

Background: Renal failure is a major safety concern of intensive systolic blood pressure (SBP) lowering. We aimed to determine the effect of this treatment on early change in renal function in participants of the international Enhanced Control of Hypertension and Thrombolysis Stroke Study (ENCHANTED)., Methods: Post-hoc analysis of the ENCHANTED BP-arm in which thrombolyzed patients with acute ischemic stroke (AIS) were randomized to intensive (target 130-140 mm Hg within 1 h) or guideline-recommended (target <180 mm Hg) management within 6 h of symptom onset. Primary outcome is early change in renal function, defined by a difference in estimated glomerular filtration rate (∆eGFR = 24 h - baseline eGFR), analyzed using linear regression with adjustment for clinical variables. Key SBP parameters were attained (mean), variability (standard deviation [SD]) and magnitude of reduction within 24 h., Results: Of 2151 participants (mean age 66.9 years; 38% female) included with available baseline eGFR, there were significant differences in attained 144.3±10.2 vs 149.8±12.0 [5.5 mm Hg]; P<0.0001), variation (15.1±5.4 vs 14.0±5.6 mm Hg; P<0.0001) and magnitude of reduction (44.6±16.2 vs 38.7±17.6 mm Hg; P<0.0001) in SBP within 24 hours. 1718 (79.9%) participants with complete follow-up eGFR were included in the primary analysis, and there was no significant difference in ∆eGFR (adjusted mean difference -1.10, 95% confidence interval [CI] -3.14 to -0.94; P=0.29) between the intensive and guideline groups, respectively. The neutral effect on ∆eGFR was consistent in patients with different baseline eGFR stages and in sensitivity analysis after multiple imputation for missing follow-up eGFR. SBP variability was significantly associated with decreasing ∆eGFR (per 5 mm Hg increase by category: adjusted mean difference -1.35, 95%CI -2.43 to -0.28; P for trend=0.01)., Conclusions: Intensive SBP lowering with a target of 130-140 mm Hg had no impact on early renal function in thrombolyzed AIS patients.
