We demonstrate two new important properties of the 1-path-norm of shallow neural networks. First, despite its non-smoothness and non-convexity it allows a closed form proximal operator which can be efficiently computed, allowing the use of stochastic proximal-gradient-type methods for regularized empirical risk minimization. Second, when the activation functions is differentiable, it provides an upper bound on the Lipschitz constant of the network. Such bound is tighter than the trivial layer-wise product of Lipschitz constants, motivating its use for training networks robust to adversarial perturbations. In practical experiments we illustrate the advantages of using the proximal mapping and we compare the robustness-accuracy trade-off induced by the 1-path-norm, L1-norm and layer-wise constraints on the Lipschitz constant (Parseval networks)., ICML 2020. Milk and dairy products could contain a variety of microorganisms, deriving from the environment or from infected udders, becoming an important source of foodborne pathogens. The main microorganisms involved in mastitis are Staphylococcus aureus, Prototheca spp., Mycoplasma spp. and Streptococcus spp., while the principal pathogens responsible for foodborne disease are enterotoxigenic Staphylococcus aureus, Salmonella spp., Listeria monocytogenes, Escherichia coli VTEC, Campylobacter spp. and Bacillus cereus. This chapter presents information on characteristics of these pathogens, and illnesses caused by them. An overview in the field of foodborne pathogen identification and detection by using different molecular techniques (multiplex PCR, quantitative PCR, digital PCR and Whole Genome Sequencing) is also reported.

The clinical efficacy of SLIT has been established in a number of clinical trials and meta-analyses. However, because SLIT is self-administered by patients without medical supervision, the degree of patient adherence with treatment is still a concern. The objective of this study was to evaluate the perception by allergists of issues related to SLIT adherence.Methods: We performed a questionnaire-based survey of 296 Italian allergists, based on the adherence issues known from previous studies. The perception of importance of each item was assessed by a VAS scale ranging from 0 to 10.Results: Patient perception of clinical efficacy was considered the most important factor (ranked 1 by 54% of allergists), followed by the possibility of reimbursement (ranked 1 by 34%), and by the absence of side effects (ranked 1 by 21%). Patient education, regular follow-up, and ease of use of SLIT were ranked first by less than 20% of allergists.Conclusion: These findings indicate that clinical efficacy, cost, and side effects are perceived as the major issues influencing patient adherence to SLIT, and that further improvement of adherence is likely to be achieved by improving the patient information provided by prescribers.Keywords: adherence, sublingual immunotherapy, efficacy, cost, side effects

Objectives: Up to 20% of HIV‐related focal brain lesion (FBL) diagnoses cannot be determined without invasive procedures. In such cases, brain biopsy is an important step in the evaluation algorithm. The aims of this study were to describe the clinical outcomes of patients with FBL, the proportion of diagnoses confirmed by brain biopsies and their aetiologies, and to analyse the proportion of patients in whom the biopsy motivated a change in therapeutic management. Methods: A retrospective cohort study was performed. The data from clinical records of patients with HIV‐related FBL admitted between January 2005 and December 2015 were reviewed. Results: A total of 137 patients were included in the study. The median age was 39 years [interquartile range (IQR) 33–44.5 years]. The median CD4 count was 54 cells/μL (IQR 21–124 cells/μL). Cerebral brain biopsy was performed in 21.16% of patients (29 of 137); 68.9% of these individuals (20 of 29) were diagnosed by histology, with results of central nervous system (CNS) lymphoma in 20.6% (six of 29), progressive multifocal leucoencephalopathy in 6.8% (two of 29), toxoplasmosis in 6.8% (two of 29), tuberculoma in 6.8% (two of 29), and other diagnoses in 27.6% (eight of 29). In nine patients, the histology was nonspecific. In 75.8% of patients (22 of 29), the result of the biopsy led to a change in the therapeutic management. We did not observe higher rates of mortality related to the procedure. Overall mortality at 30 and 90 days was similar in patients who were and were not biopsied. Conclusions: In this retrospective cohort study, cerebral biopsy was associated with significant adjustments in therapeutic management for a high percentage of patients. [ABSTRACT FROM AUTHOR]

The microprocessor performance is highly dependent on cache size and structure. This work presents a new design of a reconfigurable cache implemented on FPGA and based on a previous work. Advantages of the new design and the important enhancements are described and analyzed. Experimental background and tests carried out are also discussed. - Abstract
Since 1974, when the first policies about ‘do not attempt resuscitation' orders were published [1], the decision not to resuscitate patients in cardiac arrest has been a controversial issue in medical practice. For this reason, the ‘do not attempt resuscitation' order is, perhaps, the directive and the decision to withhold medical treatment with the widest bibliography. In this review, in accordance with the 2000 Guidelines for Cardiopulmonary Resuscitation [2], I will use the term ‘do not attempt resuscitation (DNAR)' instead of the more popular ‘do not resuscitate (DNR)'. What is known and Objective Psoriatic arthritis is an autoimmune disease characterized by chronic inflammation of the skin and joints. Anti- TNF drugs reduce the severity of the disease in the long term. This study compares the efficacy and safety of adalimumab, etanercept, infliximab and golimumab in patients with psoriatic arthritis. Methods Direct comparison was based on a literature search of drug comparison studies, whereas indirect treatment comparison was based on phase III clinical trials with biological agents, involving similar populations and durations, and with the same outcome. ACR50 was taken as primary outcome for comparison, whereas ACR20 and ACR70 were used as secondary outcomes. Indirect comparisons were made using infliximab as the reference drug and the Bucher method. In calculating δ (the maximum acceptable difference as a clinical criterion of equivalence), use was made of half of the absolute risk reduction obtained in the meta-analysis of the clinical trials included in the indirect comparison ( ARR 32%; δ: 16%). The four anti- TNF drugs were also compared in relation to the secondary outcomes and adverse effects. Results and Discussion Reported direct and indirect comparisons of the four drugs did not include golimumab, and did not yield conclusive results. Four clinical trials - one for each drug studied - were identified. The estimated differences for the primary outcome, ACR50, between infliximab and the other drugs were adalimumab ( ARR 4%, 95% CI −9·5 to 17·5), etanercept ( ARR 4%, 95% CI −10·5 to 18·5) and golimumab ( ARR 9%, 95% CI −5·4 to 23·4). Likewise, there were no relevant differences between the drugs in relation to the secondary efficacy outcomes, except for etanercept, which was less effective in ACR70 response. For adverse reactions, there were also no significant differences except for injection site, reactions which were more frequent with etanercept, with a mean difference of 26% relative to infliximab. What is new and Conclusion No significant differences were found in ACR50 responses to the four drugs after 24 weeks. Injection-site reactions were more common with etanercept, but this was insufficient to invalidate the inference that clinically the four drugs can be regarded as clinically equivalent for the treatment of psoriatic arthritis. : The objective of the present study was to evaluate the prelaryngeal position of the laryngeal mask airway (LMA
TM ) in children, and to determine the influence of mask positioning on gastric insufflation and oropharyngeal air leakage. : A total of 100 children, 3–11 years old, scheduled for surgical procedures in the supine position under general anaesthesia were studied. After clinically satisfactory LMA placement, tidal volumes were increased stepwise until air entered the stomach, airway pressure exceeded 30 cmH2 O, or air leakage from the mask seal prevented further increases in tidal volume. LMA position in relation to the laryngeal entrance was verified using a flexible bronchoscope. : The insertion of the LMA with a clinically satisfactory position was achieved in all patients at the first attempt. Gastric air insufflation occurred in five of 49 patients with malpositioned LMA. No incident of gastric air insufflation was observed in 51 patients with correctly positioned LMA. The minimum inspiratory pressure leading to mask leakage was 17 cmH2 O for incorrectly positioned LMA, and 25 cmH2 O for correctly positioned LMA. Clinically unrecognized LMA malposition was associated with a significantly increased incidence of either oropharyngeal leakage ( r = 0.59; P = 0.0001) or gastric insufflation ( r = 0.25; P = 0.01). : Clinically undetected LMA malpositioning is a significant risk factor for gastric air insufflation in children between 3 and 11 years, undergoing positive pressure ventilation, especially at inspiratory airway pressures above 17 cmH2 O. The number and seriousness of medical problems on passenger-carrying aircraft in flight are increasing. Medical incidents occur at a rate of approximately 10-50 per million passengers carried. Medical equipment carried on commercial aircraft is limited to three items: a first-aid kit, an emergency medical kit and sometimes an automatic external defibrillator. Telephone medicine, a lower level of telemedicine support, is well established for commercial air operations. The availability of satellite telecommunications on passenger-carrying aircraft permits more sophisticated forms of telemedicine. Recent telemedicine experiments have involved the transmission of three-lead electrocardiograms (ECGs), heart rate, blood pressure, arterial oxygen saturation, end-tidal CO2, respiratory rate, body temperature and realtime video. The challenge is to demonstrate that such techniques are practicable, improve patient outcomes and are cost-effective. [ABSTRACT FROM AUTHOR]

Endonasal surgery is already very common and its importance is increasing. Successful anaesthesia in endonasal sinus surgery is of critical importance for the success of the procedure and many surgeons prefer to use cocaine for this purpose. However, there is a great body of evidence that suggests that use of cocaine, even in experienced hands, can cause rapid, unexpected and severe toxic reactions. Therefore, its use in endonasal surgery can no longer be recommended, especially since better tolerated alternatives are available for topical and infiltration anaesthesia. These alternatives include lidocaine (lignocaine) and tetracaine (pantocaine) in combination with epinephrine (adrenaline), naphazoline or oxymetazoline. [ABSTRACT FROM AUTHOR]

- Abstract
In order to assess if a mixture of phenylephrine/lignocaine is as effective as cocaine for local analgesia and vasoconstriction during nasal fibreoptic intubation, 99 patients receiving topical nasal analgesia either with cocaine 10% or a mixture of phenylephrine 1% and lignocaine 4% were studied in a randomized double-blind investigation. After topical analgesia a flexible fibreoptic endoscope was advanced through a nostril. Larynx, glottis and trachea were endoscopically sprayed with lignocaine. Following induction of anaesthesia a nasotracheal tube was inserted fibreoptically. Pain intensity and amount of epistaxis during endoscopy were assessed.Blood pressure, heart rate and ECG-ST segment were determined before and after topical nasal analgesia, after induction of anaesthesia and after nasotracheal intubation. There were no significant differences in painintensity or epistaxis between groups during endoscopy, nor significant alterations in haemodynamic parameters or ST-segment. It is concluded that the mixture of phenylephrine and lignocaine is a useful alternative to cocaine for local analgesia and vasoconstriction during nasal fibreoptic intubation. In order to define the structure of the time series of rainfall and discharge, and also to improve the knowledge of the Barranco Peñalba karstic system, a correlation and spectral analysis has been made of the Iturriotz spring and the Juncal pluviometer data (period: 6th August 1983 to 12th October 1985). Such analysis reveals a relatively important memory effect and a regulation time of 23,5 days. However, it has been made clear a very small modulation effect with a spectral band of 0.30-0.32. The cross correlation reveals a lag very small between the input and output signals, in spite of the existence of a some regulation power at the end of the impulsional answer. These results do not match exactly any of the type-model defined by Mangin (1982), but suggests a system with mixed characteristics. The apparently contradictory features inferred from this analysis can be atributed to the heterogeneity of the aquifer materials and to the complexity of the recharge mecanisms.Con objeto de definir la estructura de las series cronológicas de lluvias y caudales, así como para mejorar el conocimiento del sistema kárstico del barranco de Peñalba, se ha aplicado el análisis de correlación y espectral a los datos del manantial de Iturriotz y del pluviómetro del Juncal, del período 6 de Agosto de 1983 a 12 de Octubre de 1985. Dicho análisis revela un efecto memoria relativamente importante y un tiempo de regulación de 23,5 días; sin embargo, se pone de manifiesto un efecto de filtro muy reducido, con una banda espectral de 0.30-0.32; el correlograma cruzado evidencia un desfase muy corto entre las señales de entrada y salida, a pesar de que existe un cierto poder regulador al final de la respuesta impulsional. Estos resultados no corresponden exactamente a ninguno de los modelos-tipo definidos por Mangin (1982), sino que sugieren un sistema de características mixtas. Los rasgos aparentemente contradictorios que se deducen de este análisis pueden ser atribuidos a la heterogeneidad del material acuífero y a la complejidad de los mecanismos de recarga.
