Background: Elevated lipoprotein (a) [Lp(a)] concentrations are linked mainly to genetic factors. The relationship between Lp(a) and other lipid disorders or cardiovascular (CV) risk factors has been less investigated. The aim of this study was to assess the occurrence of lipid disorders and other CV risk factors according to Lp(a) concentrations. Methods: A cross-sectional analysis of 200 primary-care patients who had not been diagnosed with CV disease was conducted. The following risk factors were assessed: older age, history of hypertension, diabetes mellitus or dyslipidemia, smoking, lack of physical activity, body mass index (BMI), and waist circumference. The following lipid parameters were measured: total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), non-high-density lipoprotein cholesterol (non-HDL-C), high-density lipoprotein cholesterol (HDL-C), triglycerides (TG), and small, dense LDL (sdLDL-C). Patients were divided into two groups based on their Lp(a) concentrations: <30 mg/dL and ≥30 mg/dL. Results: In 70% of patients, the Lp(a) concentration was <30 mg/dL. The concentrations of lipid parameters did not differ between the groups. The rate of patients with sdLDL-C >1.0 mmol/L was higher in the low-Lp(a) group (10.0 vs. 1.7%, p = 0.04), with no significant differences regarding the other analyzed lipid disorders (p > 0.05). Both in the low- and high-Lp(a) group, most patients had two other abnormal lipid factors (45.0% and 60.0%, respectively). The distribution of impaired lipid parameters (p = 0.41) and other CV risk factors (p = 0.16) was similar in both groups. There was a lower rate of patients >60 years old (15.0% vs. 32.9%, p = 0.01) and with a BMI ≥ 25 kg/m
2 (46.7% vs. 63.6%, p = 0.026) in the high-Lp(a) group, and previously diagnosed hyperlipidemia was more prevalent in this group (65.0% vs. 47.1%, p = 0.02). The occurrence of other cardiovascular risk factors did not differ significantly between the Lp(a) groups (p > 0.05). In the high-Lp(a) group, the highest proportion (25.0%) had two CV risk factors, and in the low-Lp(a) group, 31.4% had four CV risk factors. 8. A Systematic Review and Meta-Analysis of the Diagnostic Value of Galectin-3 in Acute Coronary Syndrome.

Background/Objectives: We investigated the potential diagnostic role of galectin-3 (Gal-3) in patients presenting with suspected acute coronary syndromes (ACS). Methods: We searched PubMed Central, Scopus, EMBASE, and the Cochrane Library from inception until 20 June 2024. We measured effect sizes using odds ratios (OR) with 95% CIs for dichotomous data and mean differences (MD) with CIs for continuous data. Random synthesis analysis was performed if I2 was less than 50% or Q test p values were less than 0.05. Otherwise, a fixed pooled meta-analysis was performed. Results: The meta-analysis includes 15 eligible studies. Gal-3 levels were substantially higher in the ACS group (12.84 ± 8.48 ng/mL) compared to the control group (7.23 ± 6.05 ng/mL; MD = 3.89; 95% CI: 2.83 to 4.95; p < 0.001). Gal-3 levels in acute myocardial infarction (AMI) and control groups differed (10.09 ± 8.16 vs. 4.64 ± 3.07 ng/mL, MD = 4.30; 95% CI: 0.41 to 8.18; p < 0.001). Statistical analysis revealed significant differences in Gal-3 levels between ST-elevated myocardial infarction (STEMI) and control groups (10.62 ± 7.34 vs. 5.54 ± 2.96 ng/mL; MD = 5.54; 95% CI: 3.12 to 7.97; p < 0.001). No significant differences were found between the non-ST-elevated myocardial infarction (NSTEMI) vs. control groups or patients with STEMI vs. patients with NSTEMI. Conclusions: Gal-3 may be beneficial for detecting acute coronary syndromes but not NSTEMI or differentiating between ACS types. This meta-analysis is promising, but further research is needed to prove Gal-3's potential diagnostic value, exact cut-offs, and advantages over cardiospecific troponins. Gal-3 may be a useful diagnostic biomarker; however, more clinical trials are needed to prove its utility. [ABSTRACT FROM AUTHOR]

Background: The rate of in-stent restenosis (ISR) is decreasing; however, it is still a challenge for contemporary invasive cardiologists. Therapeutic methods, including drug-eluting balloons (DEBs), intravascular lithotripsy, excimer laser coronary atherectomy, and imaging-guided percutaneous coronary intervention (PCI) with drug-eluting stents (DES), have been implemented. Patients with diabetes mellitus (DM) are burdened with a higher risk of ISR than the general population. Aims: DM-Dragon is aimed at evaluating the clinical outcomes of ISR treatment with DEBs vs. DES, focusing on patients with co-existing diabetes mellitus. Methods: The DM-Dragon registry is a retrospective study comprising data from nine high-volume PCI centers in Poland. A total of 1117 patients, of whom 473 individuals had DM and were treated with PCI due to ISR, were included. After propensity-score matching (PSM), 198 pairs were created for further analysis. The primary outcome of the study was target lesion revascularization (TLR). Results: In DM patients after PSM, TLR occurred in 21 (10.61%) vs. 20 (10.1%) in non-diabetic patients, p = 0.8690. Rates of target vessel revascularization (TVR), target vessel myocardial infarction, device-oriented composite endpoint (DOCE), and cardiac death did not differ significantly. Among diabetic patients, the risk of all-cause mortality was significantly lower in the DEB group (2.78% vs. 11.11%, HR 3.67 (95% confidence interval, CI) [1.01–13.3), p = 0.0483). Conclusions: PCI with DEBs is almost as effective as DES implantation in DM patients treated for ISR. In DM-Dragon, the rate of all-cause death was significantly lower in patients treated with DEBs. Background: The literature review shows that female patients are more frequently underdiagnosed or suffer from delayed diagnosis. Recognition of sex-related differences is crucial for implementing strategies to improve cardiovascular outcomes. We aimed to assess sex-related disparities in the frequency of fractional flow reserve (FFR)-guided procedures in patients who underwent angiography and/or percutaneous coronary intervention (PCI). Methods: We have derived the data from the national registry of percutaneous coronary interventions and retrospectively analyzed the data of more than 1.4 million angiography and/or PCI procedures [1,454,121 patients (62.54% men and 37.46% women)] between 2014 and 2022. The logistic regression analysis was conducted to explore whether female sex was associated with FFR utilization. Results: The FFR was performed in 61,305 (4.22%) patients and more frequently in men than women (4.15% vs. 3.45%, p < 0.001). FFR was more frequently assessed in females with acute coronary syndrome than males (27.75% vs. 26.08%, p < 0.001); however, women with chronic coronary syndrome had FFR performed less often than men (72.25% vs. 73.92%, p < 0.001). Females with FFR-guided procedures were older than men (69.07 (±8.87) vs. 65.45 (±9.38) p < 0.001); however. less often had a history of myocardial infarction (MI) (24.79% vs. 36.73%, p < 0.001), CABG (1.62% vs. 2.55%, p < 0.005) or PCI (36.6% vs. 24.79%, p < 0.001) compared to men. Crude comparison has shown that male sex was associated with a higher frequency of FFR assessment (OR = 1.2152–1.2361, p < 0.005). Conclusions: Despite a substantial rise in FFR utilization, adoption in women remains lower than in men. Female sex was found to be an independent negative predictor of FFR use. Objectives. It is prescribed to determine blue-light hazard (BLH) weighted radiances, $ {L_\textrm{B}} $ L B , for an assessment of spotlights with an angular subtense $ \alpha \ge 11{\mkern 1mu} \,\textrm{mrad} $ α ≥ 11 mrad . The BLH weighted irradiance, $ {E_\textrm{B}} $ E B , can be used alternatively for smaller sources. Appropriate instruments are not common among persons commissioned with risk assessment (RA), and especially $ {L_\textrm{B}} $ L B measurements may be challenging. Therefore, a practical BLH RA approach is proposed that is based on illuminance, $ {E_\textrm{v}} $ E v , pre-calculated blackbody BLH efficacies of luminous radiation, $ K_{\textrm{B,v}}^{\textrm{Planck}} $ K B,v Planck , and solid angle considerations. Methods. The practicality of this method was examined and compared against other RA approaches. Results. To ensure comparability of the applied instruments, measurements were performed close to a radiance standard, showing deviations within the lamp's expanded uncertainties ( $ \lt 4{\mkern 1mu} \% $ < 4 %), whereas the deviations were $ \pm 15\% $ ± 15 % for longer distances. Focusing on a complex light-emitting diode (LED) spotlight, all detected values could be converted to $ {L_\textrm{B}} $ L B by means of the RA methods within $ \pm 20\% $ ± 20 %. Two field tests with several spotlights yielded maximum permissible exposure durations (MPED) obtained from the different RA approaches that agreed among each other within uncertainties largely below $ \pm 30\% $ ± 30 %. Conclusion. The general practicality of the proposed $ {E_\textrm{v}} $ E v method can be concluded for a workplace BLH RA of white-light sources. [ABSTRACT FROM AUTHOR]

Background: Post-COVID-19 syndrome (PCS) may affect a substantial proportion of patients who have had COVID-19. The rehabilitation program might improve the physical capacity, functioning of the cardiopulmonary system, and mental conditions of these patients. This study aimed to investigate the effectiveness of personalized rehabilitation in patients with PCS according to gender. Methods: Adults who underwent a 6-week personalized PCS rehabilitation program were enrolled in a prospective post-COVID-19 Rehabilitation (PCR-SIRIO 8) study. The initial visit and the final visit included the hand-grip strength test, the bioimpedance analysis of body composition, and the following scales: modified Borg's scale, Modified Fatigue Impact Scale (MFIS), Functioning in Chronic Illness Scale (FCIS), modified Medical Research Council (mMRC) dyspnea scale, and tests: 30 s chair stand test (30 CST), Six-Minute Walk Test (6MWT), Short Physical Performance Battery test (SPPB)e. Results: A total of 90 patients (54% female) underwent the rehabilitation program. Rehabilitation was associated with an increase in skeletal muscle mass (24.11 kg vs. 24.37 kg, p = 0.001) and phase angle (4.89° vs. 5.01°, p = 0.001) and with a reduction in abdominal fat tissue volume (3.03 L vs. 2.85 L, p = 0.01), waist circumference (0.96 m vs. 0.95 m, p = 0.001), and hydration level (83.54% vs. 82.72%, p = 0.001). A decrease in fat tissue volume and an increase in skeletal muscle mass were observed only in females, while an increase in grip strength was noticed selectively in males. Patients' fatigue (modified Borg's scale, MFIS), physical capacity (30 CST, 6MWT), balance (SPPB), dyspnea (mMRC), and functioning (FICS) were significantly improved after the rehabilitation regardless of gender. Conclusions: Personalized rehabilitation improved the body composition, muscle strength, and functioning of patients diagnosed with PCS. The beneficial effect of rehabilitation on body composition, hydration, and phase angle was observed regardless of gender. [ABSTRACT FROM AUTHOR]

Lipoprotein(a) [Lp(a)] is made up of a low-density lipoprotein (LDL) particle and a specific apolipoprotein(a). The blood concentration of Lp(a) is approximately 90% genetically determined, and the main genetic factor determining Lp(a) levels is the size of the apo(a) isoform, which is determined by the number of KIV2 domain repeats. The size of the apo(a) isoform is inversely proportional to the blood concentration of Lp(a). Lp(a) is a strong and independent cardiovascular risk factor. Elevated Lp(a) levels ≥ 50 mg/dl (≥ 125 nmol/l) are estimated to occur in more than 1.5 billion people worldwide. However, determination of Lp(a) levels is performed far too rarely, including Poland, where, in fact, it is only since the 2021 guidelines of the Polish Lipid Association (PoLA) and five other scientific societies that Lp(a) measurements have begun to be performed. Determination of Lp(a) concentrations is not easy due to, among other things, the different sizes of the apo(a) isoforms; however, the currently available certified tests make it possible to distinguish between people with low and high cardiovascular risk with a high degree of precision. In 2022, the first guidelines for the management of patients with elevated lipoprotein(a) levels were published by the European Atherosclerosis Society (EAS) and the American Heart Association (AHA). The first Polish guidelines are the result of the work of experts from the two scientific societies and their aim is to provide clear, practical recommendations for the determination and management of elevated Lp(a) levels. A linear reduction in cardiovascular adverse events has been shown even when LDL-C reduction surpassed recommended treatment goals, thus postulating 'the lower, the better for longer' as a therapeutic strategy in patients with acute coronary syndrome (ACS). It was linked with more and more data on dual lipid lowering therapy (LLT) with statin and ezetimibe that showed to be associated with additional reduction of inflammatory markers as compared with statin alone. Thus, we recommend dual LLT consisted of high dose of potent statin and ezetimibe to be applied from the very beginning of hospitalization in all ACS patients. We recommended further increase the education of patients and improve the standards of care by physicians/cardiologists with the discharge letter recently suggested by the Polish Cardiac Society and Polish Lipid Association added to the standardized discharge letter. At the first follow-up study visit, achievement of the therapeutic LDL-C target should be assessed. Patients who did not achieve and are not expected to achieve this target, and those who have been diagnosed with statin intolerance should be referred to a lipidological consultation. Lipoprotein (a) [Lp(a)] has pro-inflammatory and pro-atherosclerotic properties. Concentration of Lp(a) is predominantly determined by genetics (> 90%), more than any other lipoprotein. Elevated concentration of Lp(a) is associated with increased risk of atherosclerotic cardiovascular disease, aortic stenosis, cardiovascular and all-cause mortality. Therefore, Lp(a) should be assessed once during hospitalization in patients with ASC. Patients with Lp(a) > 50 mg/dL (> 125 nmol/L) should be referred to a lipidological consultation. These recommendations regarding LLT apply to patients enrolled in to the ELECTRA-SIRIO 2 trial, however, we encourage to consider them for treatment of other patients with ACS. [ABSTRACT FROM AUTHOR]

Introduction: The influence of the COVID-19 pandemic on the incidence and effectiveness of dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) in the context of out-of-hospital cardiac arrests (OHCA) remains an area of ambiguity and requires further elucidation. The study aims to conduct a systematic review and meta-analysis of the DA-CPR among pre- vs. during COVID-19 periods. Material and methods: This search was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. A systematic review of the current literature was performed to identify articles that evaluated the impact of the pandemic on DA-CPR. English-language literature was searched up to December 11, 2023, in three databases, including MEDLINE (via PubMed), Embase, and the Cochrane Library. Results: Six studies with 605,354 patients (431,308 patients with OHCA from before the pandemic period and 174,046 OHCA patients from the COVID-19 pandemic period) were included. Pooled analysis showed that DA-CPR before and during the COVID-19 period varied and amounted to 61.4% vs. 62.4%, respectively (OR = 0.94; 95% CI: 0.90 to 0.99; p = 0.02). Moreover, dispatchers recognized cardiac arrest by phone in 61.8% of cases in the pre-pandemic period as well as in the pandemic period (OR = 0.98; 95%CI: 0.95 to 1.01; p = 0.17). Conclusions: This systematic review and meta-analysis suggest the resilience and adaptability of emergency response systems in maintaining DA-CPR effectiveness despite the challenges posed by the pandemic. Oxidative stress plays a key role in the development of heart failure. The study is a phase III, single-centre, randomized, double-blind, clinical trial that assesses the influence of early intravenous levosimendan administration in patients with decompensated CHF on oxidative stress parameters and the function of enzymatic antioxidative mechanisms. A total of 50 patients with diagnosed CHF with reduced ejection fraction, LVEF = 35% are randomized in a 1:1 ratio to receive either levosimendan or dobutamine. The hypothesis is that levosimendan in patients with decompensated heart failure may reduce ROS production and/or enhance antioxidant protection. INTRODUCTION: This study aims to evaluate the prognostic value of the platelet-to-lymphocyte ratio in determining the severity and mortality of adults hospitalized for COVID-19 using retrospective data and a meta-analysis of previous studies on the platelet-to-lymphocyte ratio worldwide. MATERIAL AND METHODS: A retrospective study was conducted at the Kırdar City Hospital (Istanbul, Turkey) and included 521 COVID-19 patients. A systematic literature search of EMBASE, MEDLINE, the Cochrane Central Register of Controlled Trials (CENTRAL), and Google Scholar databases was performed for relevant trials relating to the PLR ratio in COVID-19 published before April 12, 2023. RESULTS: In the retrospective part of the study, PLR values were found to predict COVID-19 severity at admission with an AUC of 0.61 (SE = 0.03; 95% CI: 0.56 to 0.65; p = 0.0003) as well as survival status in a statistically significant fashion with an AUC of 0.59 (SE = 0.03; 95% CI: 0.55 to 0.64; p = 0.0004). Results of our meta-analysis showed a significant relationship between PLR and COVID-19 severity, with a pooled standardized mean difference (SMD) of 1.34 (95% CI: 1.13 to 1.55; p < 0 .001), and that PLR was significantly lower among patients who survived compared to deceased patients (SMD = -1.32; 95% CI: 1.57 to -1.07; p < 0.001). Introduction: Widespread immunization is critical to bringing the COVID-19 pandemic to an end. The objective of this cross-sectional data analysis was to investigate the impact of vaccination on the frequency of COVID-19 occurrence and the presence of its symptoms and clinical presentations. Material and methods: This was a retrospective, cross-sectional study, analysing medical records of patients hospitalized at the Dr Antoni Jurasz University Hospital No. 1 in Bydgoszcz in the years 2019-2021. The analysis considered age, sex, vaccination status, the result of the reverse transcription polymerase chain reaction (RT-PCR) test for the detection of SARS-CoV-2 RNA, and disease symptoms and clinical presentations in accordance with ICD-10 codes. Results: The study group consisted of 27,209 patients. There were 1,393 persons (5.12%) who tested positive for COVID-19. The vaccinated patients accounted for 17.3% (n = 4,704). Those vaccinated were significantly less likely to test positive for COVID-19 (3.93% vs. 5.38%; p < 0.0001). Among unvaccinated patients with a positive test result for COVID-19, symptoms and/or clinical presentations occurred in 359 study participants (29.72%), while in vaccinated COVID-19-positive patients only in 49 persons (26.49%). The symptoms that occurred in both vaccinated and unvaccinated patients were mainly respiratory and circulatory. The most common clinical presentation, both in the group of vaccinated and unvaccinated patients, was viral pneumonia, not elsewhere classified (J12), which occurred in 17.30% and 9.19% of patients, respectively (p = 0.005). Other symptoms and clinical presentations showing a statistically significant difference in the frequency of occurrence between the groups were pneumonia in diseases classified elsewhere (J17; p = 0.019) and abnormalities of breathing (R06; p = 0.001). Complex metabolic disorders associated with obesity and diabetes pose a serious therapeutic challenge. The DEMETER-SIRIO 11 study is a phase III, multicenter, randomized, open-label, investigator-initiated clinical trial with a 6-month follow-up aimed at performing a comparative evaluation of the effect of two empagliflozin doses (10 mg vs. 20 mg) on selected metabolic parameters in patients with metabolic syndrome. The primary hypothesis of the study is that a higher dose of empagliflozin will result in a significant reduction of BMI and HbA1c in patients with obesity and MS receiving empagliflozin 20 mg as compared to 10 mg. Sample size and power calculation were based on a superiority assumption for the primary efficacy endpoint (the difference in decrease of body weight by > 1.5 kg and HbA1c by > 0.4%) for the higher vs. standard dose arm at 6-months of follow-up. Therefore, a sample size of 79 patients per arm is required to provide 80% power to detect a higher decrease in BMI, and 85 patients per arm is required to provide 80% power to detect a higher decrease in HbA1c in the 20 mg versus 10 mg arm with a type I error rate of 0.05. Summing up, enrollment of a total of 200 patients (100 in each arm) is planned to compensate for the potential drop-out rate from the study of up to 15%. Prespecified subanalyses will be performed according to: 1) diabetes mellitus; 2) chronic kidney disease (GFR < 60 mL/min/1.73 m2); 3) gender; and 4) age. A greater comprehensive improvement in biochemical, functional, and anthropometric parameters reflecting favorable metabolic changes is expected at the higher dose of empagliflozin compared to the standard dose.
