6. A group of home educating mothers in Turkey : their reasons, pedagogic approaches and use of digital resources

Introduction and background. Home education is not regulated in Turkey; it is a relatively new concept, gaining popularity in recent years among parents who are not satisfied with the national education system. I came across the term home education on Turkish parents' blogs and I found a discussion group in an online social network site. For this study, a sample of home educating mothers were recruited from this discussion group, and interviewed to examine the current situation of home education in Turkey. Aims and research questions. This research investigates home educating mothers' values and beliefs, pedagogic approaches and environments, and their use of digital resources for home education. The research questions that emerged from preliminary observations and the literature review were: (i) to establish whether/how a sample of home-educating mothers in Turkey differ in crucial respects in their values and beliefs. (ii) To explore and examine which pedagogic approaches and environments a sample of home-educating mothers in Turkey adopt for their children's education. (iii) To explore and examine the pedagogic use of digital resources to meet the educational needs of a sample of Turkish home-educating mothers' children. Design, methodology and methods. A cross sectional, descriptive and explanatory design was used. The data were gathered through semi-structured interviews with 44 home educating mothers in Turkey. Symbolic interactionism was the theoretical framework adopted and thematic analysis was used to analyse the interview data with the help of a computer-assisted qualitative data analysis software package- Nvivo. Results. In this sample, home educating families in Turkey were found to be small families with children mainly at preschool and primary school level. These home educating mothers were found to have strong views about an ideal education and how the current education system was failing to meet their expectations. They indicated that they were aware of alternatives for educating their children, including home education, but they tended to send their children to school when they reached secondary school age. This may have been due to the unregulated situation of home education in Turkey. The decision to home educate was taken mostly by mothers and most teaching at home was done by mothers. Their approaches to home education were varied but fell into four groups which they described as flexi-schooling, homeschooling, deschooling and unschooling. However, not only did these groups overlap, but as the parents became more experienced in home education, they could move in each direction along a continuum from less structured to more structured teaching approaches. While all mothers in this study used digital resources for their own personal development as parents, only about one quarter employed digital resources as a tool in their children's home education. Discussion. The discussion considers how the Results contribute to the literature reviewed in earlier chapters and identifies priorities for future research.

Objective: Our research goal was to explore the interplay between attachment styles and defense mechanisms in individuals seeking psychotherapy including the investigation of the roles of potential mediating variables. Methods: Participants who were admitted to Psychotherapy Unit of İstanbul University, İstanbul Faculty of Medicine, Department of Psychiatry were included in the study. A detailed psychosocial development history of the patients is recorded through interviews, consisting of at least 3 sessions. After these interviews psychodynamic formulations were made, and the Adult Attachment Scale and the Defense Style Questionnaire 40 were applied. Results: Totally, 102 people participated in this study. While 55 of the participants had borderline personality organization (BPO), 47 of them have neurotic personality organization (NPO). In the group with BPO, the anxious attachment scores (P < .05) and the immature defense scores (P = .01) were significantly higher than participants with NPO. Significant relationships were found between anxious attachment and neurotic (P < .05) and immature defenses (P < .001) and between avoidant attachment and immature defenses (P < .001). Multiple regression analyses of the 3 attachment styles as dependent variables were performed. While age, gender, and personality organization level did not predict any of the attachment styles, immature defenses predicted anxious (P = .010) and avoidant attachment (P = .010). Conclusion: Defense mechanisms appear to be more effective in determining attachment style than sociodemographic and clinical variables like personality organization level, age, and gender. The chemical composition, in vitro total gas and CH
4 production and performance of cattle fed on factory black tea waste (Camellia sinensis) (BTW), alfalfa (Medicago Sativa), sainfoin (Onobrychis sativa) and white clover (Trifolium repens) was investigated. The gas production was quantified at the 24th hour of the incubation process. BTW was found to vary from roughages in chemical composition (p < 0.05). In addition, the roughages differed in terms of nutrient composition and gas production (p < 0.05). In legume roughages, acetic acid (AA), propionic acid (PA), butyric acid (BA), and total volatile fatty acids (TVFA) values ranged from 52.36–57.00 mmol/L, 13.46–17.20 mmol/L, 9.79–12.43 mmol/L, and 79.71–89.05 mmol/L, respectively. In comparison with black tea waste, legume roughages had higher values of AA, PA, BA, and TVFA. Black tea waste contained a higher acetic acid ratio than legume roughages when compared as a percentage. There was a similar ratio of propionic acid to the rate calculated for sainfoin (Onobrychis sativa) and clover (Trifolium repens), and a similar ratio of butyric acid to the ratio determined for alfalfa (Medicago Sativa). The current study shows that the 5.7–6.3% tannin content of black tea waste can be used in ruminant rations with high-quality roughages. Due to the fact that BTW reduces methane emissions from ruminants and eliminates energy waste from them, the environment can be improved. 28. Psychotherapy training in Turkey: Experience of early career psychiatrists.

- Abstract
Objective: Theoretical and practical psychotherapy training improves psychiatrists' case management and treatment skills. However, little is known about the extent that in Turkey early career psychiatrists have access to these opportunities and their experiences, so in this study, we aimed to investigate various aspects of the psychotherapy training of early career adult, child and adolescent psychiatrists in Turkey. Method: A 22 items questionnaire was disseminated in Turkey to psychiatry trainees and early career psychiatrists (psychiatry and child and adolescent psychiatry trainees and specialists who are in the first 5 years of their career and younger than 40). The questionnaire inquired about participants': i) sociodemographics; ii) the quality of the psychotherapy training; iii) organizational aspects of psychotherapy training; iv) satisfaction with psychotherapy training. Results: A total of 103 individuals (n=41 psychiatrists, n=37 psychiatry trainees, n=12 child and adolescent psychiatrists, and n=13 child and adolescent psychiatry trainees) responded to this questionnaire. While 68% of the participants stated that psychotherapy training is included in psychiatry training, 89.3% stated they have conducted psychotherapy training themselves and 76.7% reported that they received supervision. Only 41.7% stated that they were going to their own personal psychotherapy or had gone before, and 59.2% were not a qualified psychotherapist. Discussion: One third of psychiatry and child and adolescent psychiatry trainees in Turkey cannot access psychotherapy training in their institutes, and one fourth cannot access supervision opportunities, and more than half are deprived of their personal therapy processes. Objective: This systematic review was planned in order to explore the sexual activity which happen after myocardial infarction by reviewing studies were conducted on this subject, provide clues which can guide treatment. Method: In the collection of data, CINAHL Plus, Cochrane, Science Direct, Pubmed, Ovid and Google Scholar data bass were used. PICOS question was used to elicit insight into current body of evidence. In the study total of article, which was published in last 11 years in English language and examine sexual functions after myocardial infarction, were chosen. The full text versions of the articles are available in English. In the study, limited articles related to affection of sexual activity after myocardial infarction were obtained. Results: It was determined that case studies which were conducted experimentally randomized and nonrandomized controlled and definitive-relationship seeker-analytic research designs were used. In the studies, it was reported that individuals who were observed 3-24. months after myocardial infarction, experiences issues such as; relapse of myocardial infarction, experiencing pain, fear of death, lack of sexual interest, drug used, decrease in sexual satisfaction and frequency of sexual activity. Usage in Practice: Positive effects of provided guidance/consulting and rehabilitation services before and after discharge on coping with sexual problems were determined. In this period, it is required that health professionals should be able to evaluate the sexual status of patient with cardiovascular disease in order to provide care for them so that they can take necessary actions for their well-being, caring and consulting services must be planned., Amaç: Bu sistematik derleme, miyokard enfaktüs sonrası cinsel aktivite konusu ile ilgili yapılan çalışmaları inceleyerek, MI sonrası dönemde yaşanan cinsel sorunları ortaya çıkarmak, bakım ve tedaviye rehberlik edecek ipuçlarını sunmak amacıyla planlanmıştır. 30. Investigation of the Effects of the Covid-19 Pandemic on Caregiver Burden in Disabled Nursing Homes

It is very well documented that the number of patients residing in nursing homes has increased in recent years due to disabilities related with mental diseases. There is mounting evidence of the enormous burden placed on caregivers who care for people suffering from mental illness. Throughout the Covid-19 pandemic, the working conditions of personnel caring for mentally ill patients in nursing homes have deteriorated. Shifts with 14-day intervals and quarantine have been implemented in order to minimize the risk of transmission. We designed this study to ascertain the impact of the pandemic and the associated regulations, as well as the issues encountered in the routine work system, on the mental health of personnel. We used sociodemographic data form to determine the personnel's sociodemographic and clinical characteristics, the Coronavirus Anxiety Scale (CAS) to determine anxiety levels associated with Covid-19, the Zarit Burden Interview (ZBI) to determine caregiver burden, and the Montgomery–Asberg Depression Rating Scale (MADRS) to determine depressive symptoms. The participants' mean CAS score was found to be 2.72±2.91, their mean ZBI score was 41.6±10.6, and their mean MADRS score was 11.55±7.70. Nurses had significantly higher MADRS and ZBI scores than caregivers. There was a positive and significant correlation between ZBI scores, and CAS and MADRS scores (p, Ruhsal hastalıklara bağlı engellilik nedeniyle son yıllarda bakımevlerinde yaşayan hasta sayısında artış olduğu bilinmektedir. Ruhsal hastalıklara bakım veren kişilerde bakım veren yükünün yüksek olduğuna dair veriler artmaktadır. Covid-19 pandemi sürecinde bakımevlerinde ruhsal hastalara bakım veren personellerin çalışma şartlarında değişiklikler olmuştur. Bulaş riskini azaltmak amacıyla 14 gün aralık mesai ve karantina uygulaması başlatılmıştır. Rutin çalışma düzenindeki zorlukların yanında pandemi ve pandemiye bağlı düzenlemelerin personelin ruhsal sağlığı üzerindeki etkilerini belirlemek amacıyla bu çalışmayı planladık. Personelin sosyodemografik ve klinik özellikleri belirlemek amacıyla sosyodemografik veri formunu, Covid-19 ile ilişkili anksiyete düzeylerini belirlemek amacıyla Koronavirus Anksiyete Ölçeği’ni, (KAÖ) bakım veren yükünü belirlemek amacıyla Zarit Bakım veren Yükü Ölçeği’ni (ZBYÖ), depresif belirtileri tespit etmek amacıyla Montgomery-Asberg Depresyon Ölçeği’ni (MADRS) kullandık. Introduction: Deterioration in metabolic parameters is one of the major reasons of cardiovascular system-related morbidity and mortality in psychiatric diseases and is particularly common in chronic diseases such as schizophrenia and bipolar disorder (BD). We aimed to investigate the metabolic characteristics of patients with BD, a group that has received relatively less attention, by comparing them with patients with schizophrenia. Methods: 26 patients with BD and 27 patients with schizophrenia were involved in this study. Clinical and sociodemographic data, BMI, GAF, and CGI scores of the patients were recorded, and fasting blood glucose, HBA1C, total cholesterol, triglyceride, LDL, VLDL and HDL values performed during routine examinations in the last month were compared. Results: There was no significant difference between the two groups in terms of age, gender, disease duration, mean number of psychotropic drugs, and mean number of antipsychotic drugs (p>0.05). While BMI (p=0.02), Triglyceride (p=0.008), Total cholesterol (p=0.018), and VLDL (p=0.008) values of bipolar disorder patients were significantly higher than schizophrenia patients. In the regression analysis, it was determined that the status of staying in a nursing home was a positive determinant in terms of triglyceride levels. Conclusion: The fact that schizophrenia patients with lower functionality and needing support and care stay in a nursing home can be protective in terms of metabolic risks due to factors such as diet control and regular doctor visits. Bipolar disorder patients followed up in community mental health centers should also be carefully evaluated in terms of metabolic risks, and if necessary, support should be provided to the patients in terms of healthy nutrition and exercise. Aim: Hypoxemia due to obstructive sleep apnea (OSA) is thought to cause peripheral nerve damage. A case-control study was conducted to test the accuracy of this phenomenon. Materials and Methods: The patient group consisted of 45 patients with OSA who had an apnea-hypopnea index (AHI) score of >10 and were newly diagnosed with OSA and control group consisted of 37 individuals who did not have OSA. Sensory and motor nerve conduction investigations were performed on both the upper and right lower extremity Mann-Whitney U, chi square and independent sample T tests were carried out. Results: Both groups had the same average age of 46. The mean body mass index (BMI) in patients with OSA was 31.81; the average AHI was 55.17; the periodic leg movements in sleep (PLMS) was 30.71; the T90 value was 74.23; the lowest saturation was 37; the average saturation value was 72.97. The other nerves' sensory and motor distal latency lengths were more prolonged than the control group, except the ulnar nerve. The averages of the left median, ulnar, and right peroneal motor nerves' compound motor action potential (CMAP) amplitude were lower in individuals with OSA than in the control group. Compared to the control group, patients with OSA had slower right tibial nerve motor conduction velocity. The mean snap amplitude of bilateral median and left ulnar sensory nerve were higher in patients with OSA. Conclusion: OSA-induced hypoxia may have an impact on peripheral nerve damage. Purpose: To determine the relationship between the individualized care perceptions and spiritual care perceptions of nurses. Design and Methods: This descriptive and correlational study was conducted with 263 nurses. Data were collected using a Structured Questionnaire Form, the Individualized Care Scale‐A‐Nurse Version and the Spirituality and Spiritual Care Rating Scale. Findings: There was a positive and significant relationship between the individualized care perceptions and spiritual care perceptions of the nurses. Practice Implications: The results of this study revealed the reflections of the philosophy of nursing on the field of practice, with findings on the individualized care perceptions and spiritual care perceptions of nurses and the relationship between these variables. Purpose: To determine the knowledge and practices of nurses working in psychiatry clinics on the ventrogluteal (VG) injection. Design and Methods: This study was conducted as a descriptive and cross‐sectional study. A total of 91 nurses constituted the sample of the study. Findings: It was determined that 35.7% of the nurses preferred the VG site. It was determined that the nurses who preferred the VG site knew and applied the preparation for injection, and the stages during and after the injection. Activated charcoal is a safe and effective material widely used for decontamination in poisoning cases. The case is here presented of a 3-year-old male child who developed severe respiratory failure due to activated charcoal aspiration and recovered with supportive treatment and repeated bronchoscopic irrigation. When using activated charcoal it must be kept in mind that there could be complications resulting in severe morbidities or even be life-threatening. This case is presented to draw attention to the potentially serious complications of activated charcoal, which is relatively often used in pediatric emergency practice and to emphasize that serial bronchoscopic irrigation can contribute to the recovery of ventilation parameters. Purpose: The aim of this study is to examine the relationship between thyroid hormone levels and positive, negative, general, and cognitive symptoms in euthyroid patients with psychosis. Materials and Methods: Thirty-three schizophrenia patients were included in this cross-sectional study. Sociodemographic and clinical data of the participants, positive and negative symptoms assessment scale (PANSS), general assessment of functionality scale (GAF), and clinical global impression scale (CGI) scores were recorded. The relationships between TSH, fT3, and fT4 levels and cognitive performances of patients which is measured via a neurocognitive battery consisting of Trail Making Test (TMT) A, TMT B, verbal fluency test (VFT), digit span (DS), forward and backward, and Wisconsin Card Sorting Test (WCST) were investigated. Results: Significant correlations were found between fT3 levels and WCST performances. fT3 levels was significantly correlated with the number of categories achieved in WCST (r=.508; p=0.003). A significant correlation was found between fT4 levels and TMT B (r=- .376; p=0.031) and VFT (p=.355; p=0.043) performances. In the linear regression model, it was found that fT3 levels significantly predicted the number of categories achieved in WCST (UB=1.680; p=0.029). Conclusion: The results of our study suggested that fT3 has more significant effects, primarily on executive functions, compared to T4. [ABSTRACT FROM AUTHOR]

The aim of this study was to examine the association between the quality of life and the fall behavior in elderly individuals. The study was conducted as a descriptive and correlational study design. Data were collected with the Patient Information Form, The Fall Behavior Scale for the Elderly and the World Health Organization Quality of Life module for the elderly. There were 110 older people included in study. When the relationship between the Fall Behavior Scale for the Elderly and the World Health Organization Quality of Life Module for the Elderly sub-scale and total scores were examined, there was a statistically significant negative correlation between the Fall Behavior Scale for the Elderly safe movement sub-scale and the World Health Organization Quality of Life Module social participation sub-scale scores (p <.01). Background and Objective: Although high-flow nasal cannula (HFNC) is widely used in children, there is no consensus on the methods for starting, maintenance, and weaning. The aim of this study was to compare weaning methods in children. Methods: The study included all patients in pediatric intensive care unit (PICU) who were started on HFNC treatment. The respiratory assessment score was used in the decisions for starting, continuing, and weaning from HFNC. The patients who responded and for whom weaning was planned were randomized by month into 2 groups as directly weaned from HFNC and weaned by reducing the flow. Success rates, treatment, and length of stay (LOS) in weaning methods were compared. Results: Of the 145 patients initially included in the study, 32 (22%) were excluded, and analysis was made of 113 patients. Successful weaning from HFNC was obtained in 76.9% of the patients, in 82.1% of flow weaning, and 73.6% of direct weaning, with no statistically significant difference determined between the groups (P = 0.286). The median duration of HFNC and the median LOS in PICU were determined to be statistically significantly shorter in direct weaning than in flow weaning [36 h interquartile range (IQR) 24–48 h] versus 60 h (IQR 60–72 h), P < 0.001 and 6 days (4–14 days) versus 9.5 days (5.25–20.75 days, P = 0.043, respectively). Conclusion: In patients who responded to HFNC in PICU, the responses to direct weaning and flow reduction were seen to be similar. In patients directly weaned off, both the HFNC duration and LOS in PICU were significantly shorter. [ABSTRACT FROM AUTHOR]

Introduction: The Coronavirus Disease 2019 (COVID-19) outbreak has resulted in huge psychological distress, especially for people working under risky conditions, such as healthcare workers. We aimed to investigate the psychological challenges of Turkish healthcare workers during the outbreak and discuss the factors that increase their burden, including stigmatization. Methods: A cross-sectional online survey composed of sociodemographic data, questions about perceived stigma, and concerns about working conditions was completed. Depression Anxiety and Stress Scale-21 (DASS-21) was also used. Results: 634 participants completed the survey and the prevalence of moderate to severe depression, anxiety, and stress-related symptoms were 36%, 35%, and 22%, respectively. Being female, working with close contact with patients with COVID-19 and having a psychiatric disorder history were risk factors for psychological distress. The healthcare workers with significantly higher depression, anxiety, and stress scores described having less amount of personal protective equipment, lower support from their supervisors, and more unsafe working conditions. Additionally, 7% of the participants stated that they perceived stigmatization by their families and close friends, 14% perceived stigmatization by their neighbours, relatives, and less intimate friends; and 7% perceived stigmatization by unfamiliar people. Healthcare workers who perceived other people as more dismissive had significantly higher depression and anxiety scores. Conclusion: Our study shows that healthcare workers are at risk of developing psychological disturbances during infectious outbreaks. To minimize the psychological impact, additional risk factors such as having a previous psychiatric disorder, working under unsafe conditions, and stigmatization should be taken into account and a more supportive and safer environment should be provided.
