18. The experiences of coaches with physical disability in South African sport

Abstract

Coaching is a developmental and athletic-enhancing process in which an experienced individual provides a protégé with training, guidance and support in achieving a sporting goal. However, research on the experiences of coaches with disabilities is scant. This study focused on the career experiences of coaches with a disability within the South African context. The study adopted a case- study design in which qualitative data were collected using semi- structured interviews and observations. The data were triangulated to corroborate the findings obtained from the interviews and observations. A purposive sampling technique was used to select the participants (n=3) and data were thematically analysed. The findings showed that coaches with disabilities had positive and negative experiences in their careers. The main positive experiences included having a sense of fulfilment in coaching practice, demonstrating that disability did not mean deviance or abnormality and the joy of seeing achievements by the athletes that they coached. Challenging experiences were associated with lackadaisical attitudes from parents and sometimes athletes, a lack of support and intrinsic limitations imposed by the disabilities resulting in communication problems. The results also revealed the adjustments that the coaches made to accommodate their disabilities, which consisted of rule changes to games and the use of assistive devices and equipment. The need to provide necessary support for coaches with disabilities as well as develop measures to raise awareness for society to have positive attitudes towards the capabilities of coaches with disabilities, are discussed and recommended.

19. Clinical update: Heparin-induced thrombocytopenia: An update for the COVID-19 era

Abstract

The increased use of heparin during the current COVID-19 pandemic has highlighted the risk of a rare but potentially serious complication of heparin therapy, viz. heparin-induced thrombocytopenia (HIT). This is a short review on the pharmacology of heparin and its derivatives, and the pathophysiology of HIT. Guidance on laboratory testing for and clinical management of HIT is presented in accordance with international guidelines. There are important similarities and differences between HIT and the new entity of vaccine-induced immune thrombotic thrombocytopenia, also known as thrombosis with thrombocytopenia syndrome, which clinicians need to be aware of.

20. Second Comparative Risk Assessment for South Africa (SACRA2) highlights need for health promotion and strengthened surveillance

Abstract

Background. South Africa (SA) faces multiple health challenges. Quantifying the contribution of modifiable risk factors can be used to identify and prioritise areas of concern for population health and opportunities for health promotion and disease prevention interventions. Objective. To estimate the attributable burden of 18 modifiable risk factors for 2000, 2006 and 2012. Methods. Comparative risk assessment (CRA), a standardised and systematic approach, was used to estimate the attributable burden of 18 risk factors. Risk exposure estimates were sourced from local data, and meta-regressions were used to model the parameters, depending on the availability of data. Risk-outcome pairs meeting the criteria for convincing or probable evidence were assessed using relative risks against a theoretical minimum risk exposure level to calculate either a potential impact fraction or population attributable fraction (PAF). Relative risks were sourced from the Global Burden of Disease, Injuries, and Risk Factors (GBD) study as well as published cohort and intervention studies. Attributable burden was calculated for each risk factor for 2000, 2006 and 2012 by applying the PAF to estimates of deaths and years of life lost from the Second South African National Burden of Disease Study (SANBD2). Uncertainty analyses were performed using Monte Carlo simulation, and age-standardised rates were calculated using the World Health Organization standard population. Results. Unsafe sex was the leading risk factor across all years, accounting for one in four DALYs (26.6%) of the estimated 20.6 million DALYs in 2012. The top five leading risk factors for males and females remained the same between 2000 and 2012. For males, the leading risks were (in order of descending rank): unsafe sex; alcohol consumption; interpersonal violence; tobacco smoking; and high systolic blood pressure; while for females the leading risks were unsafe sex; interpersonal violence; high systolic blood pressure; high body mass index; and high fasting plasma glucose. Since 2000, the attributable age-standardised death rates decreased for most risk factors. The largest decrease was for household air pollution (–41.8%). However, there was a notable increase in the age-standardised death rate for high fasting plasma glucose (44.1%), followed by ambient air pollution (7%). Conclusion. This study reflects the continued dominance of unsafe sex and interpersonal violence during the study period, as well as the combined effects of poverty and underdevelopment with the emergence of cardiometabolic-related risk factors and ambient air pollution as key modifiable risk factors in SA. Despite reductions in the attributable burden of many risk factors, the study reveals significant scope for health promotion and disease prevention initiatives and provides an important tool for policy makers to influence policy and programme interventions in the country.

22. Staff Responsiveness to Transformation Initiatives and Diversity at a South African University

Abstract

South African organisations and particularly institutions of higher learning have been confronted with workforces that increasingly reflect the diversity of the South African population. This changing workforce composition implies that the multitude of individual and cultural differences and similarities become increasingly apparent among employees. These differences and similarities can be disruptive and may lead to disharmony, but if managed appropriately, they can also result in increased vigour, stimulation and energy. The importance of workforce diversity in especially a university environment cannot be over-emphasised, as these positive consequences are conducive and sustain universities in their role to expand knowledge that benefits the communities within which they operate. Various transformative initiatives to address diversity, have been devised and implemented, but the impact needs to be measured and assessed to enable leaders to manage diverse workforces more effectively. This article therefore aims to firstly, statistically identify the underlying dimensions of diversity within the largest university in Africa, namely the University of South Africa (Unisa). The responsiveness of university staff members who attended transformative diversity-training workshops are secondly investigated by comparing their responses to that of a matched sample of non-participating staff members. These workshops aimed to sensitise academic, administrative and professional staff at middle-management level to the new realities of an increasing diverse workforce. It is evident that the workshops had a positive impact and that they could be described as successful. The importance of noting that diversity interventions not only entail creating awareness of diversity issues, but also applying the principles of diversity management on a continuous basis, is underscored in this article.

24. Prophylaxis is the new standard of care in patients with haemophilia

Abstract

Randomised controlled clinical trial evidence on prophylaxis as optimal care for patients with haemophilia was generated more than a decade ago. However, this knowledge has not translated into clinical practice in South Africa (SA) owing to many barriers to prophylaxis. These include the high treatment burden imposed by prophylaxis (frequent injections two to four times a week), the need for intravenous access to administer replacement clotting factor therapies, and the higher volume of clotting factor required compared with episodic treatment. The recently introduced non-factor therapies in haemophilia care have addressed many of these barriers. For example, emicizumab, which is currently the only globally approved non-factor therapy, can be administered subcutaneously less frequently (weekly, fortnightly or every 4 weeks) and has led to global adoption of prophylaxis as the standard of care in haemophilia by the bleeding disorders community. Haemophilia A is the most prevalent clotting factor deficiency in SA, with >2 000 people diagnosed to date. However, only a few of these patients are currently on prophylaxis. In this ‘In Practice’ article, we review the rationale for prophylaxis, outline its goals and benefits, and provide evidence-based guidance on which haemophilia patients should be prioritised for emicizumab prophylaxis. This consensus guidance facilitates the adoption of prophylaxis as a national policy and the new standard of care in haemophilia in SA.

Abstract

In South Africa, recent government plans to change the institutional landscape of higher education have resulted in mergers of colleges into universities or technikons. The research reported in this article focuses solely on the impact of a "college-into-university'' incorporation as manifested in the personal, emotional and career experiences of these college staff members. It traces the changes in their perceptions and emotions during and after the incorporation process. It also identifies recurring themes and issues evident in the personal lives of those affected by this incorporation. A unique research methodology was engaged: The College staff who had been appointed to the university after the merger, identified seven critical themes and then designed and conducted 30 semi-structured interviews among themselves. This article thus documents the impact of incorporation into a university on the individual and collective lives of there searchers themselves. The data suggest that the emotional impact of incorporation was intense and that the uncertainty, especially, led to considerable trauma. The most important concern emanating from this joint research project is that while a certain degree of distress is unavoidable in any institutional merger, inattention to the management of human resources, emotions and aspirations could linger on, possibly having a negative effect on the ambitions for the transformation of the new entity.
