In this paper the authors draw on a qualitative study of low-work-intensity households in a disadvantaged suburb of Dublin in 2016–17 to identify some of the gaps in Ireland’s reformed ‘one-stop shop’ Public Employment Service. Drawing on the issues recognised as being required in an integrated Public Employment Service, the paper draws attention to gaps in information; training; services that support employment, such as childcare and housing; and links to employers. The authors conclude by drawing lessons on the issues which need to be addressed for a more tailored Public Employment Service.

9. Somewhere over the Border: Grammar in a Class of Its Own

- Abstract
The Grammar Gang blog has now passed its fourth anniversary as a borderless, non-proprietary language and learning online classroom. It gives wing to the aspirations of academic staff from four universities to explore language and learning across hemispheres. The Blog's recent birthday provides a timely opportunity to explore how this collaboration takes place and some possible ways to extend language, learning and academic support services. The Grammar Gang is an online classroom where people around the world can learn and explore their interest in language in a fun, conversational way. It is also a borderless classroom which queries the notion of institutional "ownership" in its traditional sense. Further, it opens wide the debate around the rights and wrongs of English language expression and learning. Four members of the Grammar Gang examine the implications of this digital collaboration in this context by addressing the ways in which the blog is used and some of the thinking within the literature around global English and institutional ownership of knowledge. The Grammar Gang continues to be an "adventure across the hemispheres" providing a model for others to follow.

Background Because interventions are available to prevent further recurrence in patients with recurrent Clostridioides difficile infection (rCDI), we identified predictors of multiple rCDI (mrCDI) in adults at the time of presentation with initial CDI (iCDI). Methods iCDI was defined as a positive C difficile test in any clinical setting during January 2018–August 2019 in a person aged ≥18 years with no known prior positive test. rCDI was defined as a positive test ≥14 days from the previous positive test within 180 days after iCDI; mrCDI was defined as ≥2 rCDI. We performed multivariable logistic regression analysis. Results Of 18 829 patients with iCDI, 882 (4.7%) had mrCDI; 437 with mrCDI and 7484 without mrCDI had full chart reviews. A higher proportion of patients with mrCDI than without mrCDI were aged ≥65 years (57.2% vs 40.7%; P <.0001) and had healthcare (59.1% vs 46.9%; P <.0001) and antibiotic (77.3% vs 67.3%; P <.0001) exposures in the 12 weeks preceding iCDI. In multivariable analysis, age ≥65 years (adjusted odds ratio [aOR], 1.91; 95% confidence interval [CI], 1.55–2.35), chronic hemodialysis (aOR, 2.28; 95% CI, 1.48–3.51), hospitalization (aOR, 1.64; 95% CI, 1.33–2.01), and nitrofurantoin use (aOR, 1.95; 95% CI, 1.18–3.23) in the 12 weeks preceding iCDI were associated with mrCDI. Conclusions Patients with iCDI who are older, on hemodialysis, or had recent hospitalization or nitrofurantoin use had increased risk of mrCDI and may benefit from early use of adjunctive therapy to prevent mrCDI. 13. All vision but no change? : determinants of implementation : the case of Ireland and mental health policy

This research was prompted by a concern about implementation based on the premise that in Ireland the policy system has been good at designing policies but has been inconsistent in its ability to implement them, leading to frustrations on the part of policy makers and service users alike. Recourse to the implementation literature identifies a complex array of approaches and variables which are suggested as influencing implementation, causing one researcher to comment that there are too few cases and too many variables. In a search for the main determinants of implementation, this research classifies some of these variables according to the main ideological stable of the implementation researchers as being driven from the 'top down' or 'bottom up' or being 'transformative', in that they are associated with change. A number of variables are present in all these approaches and are classified as 'common' variables. Taking the mental health policy A Vision for Change, which is recognised as being a policy of international standing, developed by an intensive and inclusive process, approved by government and with cross party political support, the content of the reports of an Independent Monitoring Group were analysed and interviews undertaken with people associated with implementation of the policy The results demonstrate that all of the variables identified played a role in the implementation of A Vision for Change but some were more influential than others. Authoritative accountable leadership and identifying the main resistors to change and addressing these were found to be fundamental to successful implementation. Other variables were found to be necessary but not sufficient on their own while others played a more contributory role. This finding has relevance for the implementation of nationally designed policies which are intended to be implemented across the country in an even-handed way.

Background We described changes in 2016─2020 carbapenem-resistant Enterobacterales (CRE) incidence rates in 7 US sites that conduct population-based CRE surveillance. Methods An incident CRE case was defined as the first isolation of Escherichia coli , Klebsiella spp. or Enterobacter spp. resistant to ≥1 carbapenem from a sterile site or urine in a surveillance area resident in a 30-day period. We reviewed medical records and classified cases as hospital-onset (HO), healthcare-associated community-onset (HACO), or community-associated (CA) CRE based on healthcare exposures and location of disease onset. We calculated incidence rates using census data. We used Poisson mixed effects regression models to perform 2016─2020 trend analyses, adjusting for sex, race/ethnicity, and age. We compared adjusted incidence rates between 2016 and subsequent years using incidence rate ratios (RRs) and 95% confidence intervals (CIs). Results Of 4996 CRE cases, 62% were HACO, 21% CA, and 14% HO. The crude CRE incidence rate per 100 000 was 7.51 in 2016 and 6.08 in 2020 and was highest for HACO, followed by CA and HO. From 2016 to 2020, the adjusted overall CRE incidence rate decreased by 24% (RR, 0.76 [95% CI,.70–.83]). Significant decreases in incidence rates in 2020 were seen for HACO (RR, 0.75 [95% CI,.67–.84]) and CA (0.75 [.61–.92]) but not for HO CRE. Conclusions Adjusted CRE incidence rates declined from 2016 to 2020, but changes over time varied by epidemiologic class. Background Nontuberculous mycobacteria (NTM) cause pulmonary (PNTM) and extrapulmonary (ENTM) disease. Infections are difficult to diagnose and treat, and exposures occur in healthcare and community settings. In the United States, NTM epidemiology has been described largely through analyses of microbiology data from health departments, electronic health records, and administrative data. We describe findings from a multisite pilot of active, laboratory- and population-based NTM surveillance. Methods The Centers for Disease Control and Prevention's Emerging Infections Program conducted NTM surveillance at 4 sites (Colorado, 5 counties; Minnesota, 2 counties; New York, 2 counties; and Oregon, 3 counties [PNTM] and statewide [ENTM]) from 1 October 2019 through 31 March 2020. PNTM cases were defined using published microbiologic criteria. ENTM cases required NTM isolation from a nonpulmonary specimen, excluding stool and rectal swabs. Patient data were collected via medical record review. Results Overall, 299 NTM cases were reported (PNTM: 231, 77%); Mycobacterium avium complex was the most common species group. Annualized prevalence was 7.5/100 000 population (PNTM: 6.1/100 000; ENTM: 1.4/100 000). Most patients had signs or symptoms in the 14 days before positive specimen collection (ENTM: 62, 91.2%; PNTM: 201, 87.0%). Of PNTM cases, 145 (62.8%) were female and 168 (72.7%) had underlying chronic lung disease. Among ENTM cases, 29 (42.6%) were female, 21 (30.9%) did not have documented underlying conditions, and 26 (38.2%) had infection at the site of a medical device or procedure. Conclusions Active, population-based NTM surveillance will provide data for monitoring the burden of disease and characterize affected populations to inform interventions. [ABSTRACT FROM AUTHOR]

Background Most multicenter studies of US pediatric sepsis epidemiology use administrative data or focus on pediatric intensive care units. We conducted a detailed medical record review to describe sepsis epidemiology in children and young adults. Methods In a convenience sample of hospitals in 10 states, patients aged 30 days–21 years, discharged during 1 October 2014–30 September 2015, with explicit diagnosis codes for severe sepsis or septic shock, were included. Medical records were reviewed for patients with documentation of sepsis, septic shock, or similar terms. We analyzed overall and age group–specific patient characteristics. Results Of 736 patients in 26 hospitals, 442 (60.1%) had underlying conditions. Most patients (613 [83.3%]) had community-onset sepsis, although most community-onset sepsis was healthcare associated (344 [56.1%]). Two hundred forty-one patients (32.7%) had outpatient visits 1–7 days before sepsis hospitalization, of whom 125 (51.9%) received antimicrobials ≤30 days before sepsis hospitalization. Age group–related differences included common underlying conditions (<5 years: prematurity vs 5–12 years: chronic pulmonary disease vs 13–21 years: chronic immunocompromise); medical device presence ≤30 days before sepsis hospitalization (1–4 years: 46.9% vs 30 days–11 months: 23.3%); percentage with hospital-onset sepsis (<5 years: 19.6% vs ≥5 years: 12.0%); and percentage with sepsis-associated pathogens (30 days–11 months: 65.6% vs 13–21 years: 49.3%). Conclusions Our data suggest potential opportunities to raise sepsis awareness among outpatient providers to facilitate prevention, early recognition, and intervention in some patients. 32. Use of Interactive Lecture Demonstrations: A Ten Year Study

The widely held constructivist view of learning advocates student engagement via interactivity. Within the physics education research community, several specific interactive strategies have been developed to enhance conceptual understanding. One such strategy, the Interactive Lecture Demonstration (ILD) is designed for large lecture classes and, if measured using specific conceptual surveys, is purported to provide learning gains of up to 80%. This paper reports on learning gains for two different Projects over ten years. In Project 1, the ILDs were implemented from 1999 to 2001 with students who had successfully completed senior high school physics. The learning gains for students "not" exposed to the ILDs were in the range 13% to 16% while those for students exposed to the ILDs was 31% to 50%. In Project 2, the ILDs were implemented from 2007 to 2009 with students who had not studied senior high school physics. Since the use of ILDs in Project 1 had produced positive results, ethical considerations dictated that all students be exposed to ILDs. The learning gains were from 28% to 42%. On the one hand it is pleasing to note that there is an increase in learning gains, yet on the other, we note that the gains are nowhere near the claimed 80%. - Abstract
Using historical and archival methodological approaches, this article provides an interdisciplinary empirical study of health experiences of prisoners in the Victorian convict prison in England and Wales. It explores the challenges of doing historical criminological work on prisoner health and disability. The article examines the health of long-term prisoners, those serving sentences of penal servitude and their interactions and treatment by the Prison Medical Officers in the system. Reflecting society outside, the prison population was drawn from populations that had experienced childhood illness, workplace accidents and war and therefore had to accommodate those suffering from both short- and long-term health issues. Using historical documentation and case studies of individual prisoners, this article highlights not only the deep scepticism the system had for those they saw as ‘malingerers’, but also the recognition that some people could not endure the full force of the prison labour regime. We evaluated healthcare facility use of International Classification of Diseases, Tenth Revision (ICD-10) codes for culture-confirmed candidemia cases detected by active public health surveillance during 2019–2020. This paper provides an overview of Irish dalliances with basic income over the last 40 years in the context of social security reform. A government Green Paper on Basic Income was published in 2002, but the proposals were never progressed. Now, the current 2020 Programme for Government contains a commitment to pilot basic income within the lifetime of the Government. It has transpired that two basic income schemes are being developed – a universal basic income scheme by the Government's Low Pay Commission and a sectoral basic income scheme for artists. The arts proposal is being led by the Green Party Minister for the Arts, a long-time advocate of basic income. The work of the Low Pay Commission is overseen by the Fine Gael leader and Minister for Employment, who has not traditionally supported basic income. Public discourse claims that these are separate proposals with a lack of clarity on whether they will be progressed separately, one will inform the other, or they will become integrated. The work in Ireland has drawn upon other basic income experiments taking place in Europe, especially the Finnish experience. The work to date can make a unique contribution to understanding basic income experimentation in Europe, especially through a government-led, twin-track approach. [ABSTRACT FROM AUTHOR]

Smith, Ashley Paulick, Michelle Adamczyk, BACKGROUND Efforts to prevent Clostridioides difficile infection continue to expand across the health care spectrum in the United States. Whether these efforts are reducing the national burden of C. difficile infection is unclear. METHODS The Emerging Infections Program identified cases of C. difficile infection (stool specimens positive for C. difficile in a person ≥1 year of age with no positive test in the previous 8 weeks) in 10 U.S. sites. We used case and census sampling weights to estimate the national burden of C. difficile infection, first recurrences, hospitalizations, and in-hospital deaths from 2011 through 2017. Health care–associated infections were defined as those with onset in a health care facility or associated with recent admission to a health care facility; all others were classified as community-associated infections. For trend analyses, we used weighted random-intercept models with negative binomial distribution and logistic-regression models to adjust for the higher sensitivity of nucleic acid amplification tests (NAATs) as compared with other test types. RESULTS The number of cases of C. difficile infection in the 10 U.S. sites was 15,461 in 2011 (10,177 health care–associated and 5284 community-associated cases) and 15,512 in 2017 (7973 health care–associated and 7539 community-associated cases). The estimated national burden of C. difficile infection was 476,400 cases (95% confidence interval [CI], 419,900 to 532,900) in 2011 and 462,100 cases (95% CI, 428,600 to 495,600) in 2017. With accounting for NAAT use, the adjusted estimate of the total burden of C. difficile infection decreased by 24% (95% CI, 6 to 36) from 2011 through 2017; the adjusted estimate of the national burden of health care–associated C. difficile infection decreased by 36% (95% CI, 24 to 54), whereas the adjusted estimate of the national burden of community-associated C. difficile infection was unchanged. The adjusted estimate of the burden of hospitalizations for C. difficile infection decreased by 24% (95% CI, 0 to 48), whereas the adjusted estimates of the burden of first recurrences and in-hospital deaths did not change significantly. CONCLUSIONS The estimated national burden of C. difficile infection and associated hospitalizations decreased from 2011 through 2017, owing to a decline in health care–associated infections.
