Abstract This scoping review evaluates recent advancements in data-driven technologies for predicting non-neonatal pediatric sepsis, including artificial intelligence, machine learning, and other methodologies. Of the 27 included studies, 23 (85%) were single-center investigations, and 16 (59%) used logistic regression. Notably, 20 (74%) studies used datasets with a low prevalence of sepsis-related outcomes, with area under the receiver operating characteristic scores ranging from 0.56 to 0.99. Prediction time points varied widely, and development characteristics, performance metrics, implementation outcomes, and considerations for human factors—especially workflow integration and clinical judgment—were inconsistently reported. The variations in endpoint definitions highlight the potential significance of the 2024 consensus criteria in future development. Future research should strengthen the involvement of clinical users to enhance the understanding and integration of human factors in designing and evaluating these technologies, ultimately aiming for safe and effective integration in pediatric healthcare.

In 2022–2023, the first survey of clinicians regarding Encephalitozoon cuniculi infection of rabbits was undertaken. The purpose was to assess the current worldwide knowledge of clinicians regarding influence of breed and age, clinical presentations, use of various diagnostic tools, and treatment regimens and to explore how responses may differ by geographic location. Identifying mostly as exotic animal clinicians, 339 respondents participated, with nearly half from the United States and the remainder from 35 other countries. Neurological signs were most consistently reported as associated with Encephalitozoon cuniculi and the most recurrent clinical case presentation. Other responses showed differences by geographic location. For example, age predilection for disease varied, with 71.1% of international respondents indicating that infection was more common in young rabbits versus 28.9% who suggested it was more frequent in older rabbits. This was significantly different (p = 0.001) from the information from respondents from the United States, where 48.0% indicated that younger rabbits had an increased incidence versus 52.0% who indicated that older age increased occurrence. While some differences may be related to the practice composition and the training level of respondents, many responses were in alignment with the existing literature. Future inquiries should focus more on these discrepancies to aid in generating guidelines for the diagnosis and treatment of this infectious disease.

Introduction While there have been several literature reviews on the performance of digital sepsis prediction technologies and clinical decision-support algorithms for adults, there remains a knowledge gap in examining the development of automated technologies for sepsis prediction in children. This scoping review will critically analyse the current evidence on the design and performance of automated digital technologies to predict paediatric sepsis, to advance their development and integration within clinical settings.Methods and analysis This scoping review will follow Arksey and O’Malley’s framework, conducted between February and December 2022. We will further develop the protocol using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for scoping reviews. We plan to search the following databases: Association of Computing Machinery (ACM) Digital Library, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Embase, Google Scholar, Institute of Electric and Electronic Engineers (IEEE), PubMed, Scopus and Web of Science. Conceptualizing physical pain and negative affect as potentially interactive, we hypothesized that higher levels of peripheral inflammatory markers would be observed consistently only among individuals with both higher negative affect and pain symptomatology. Participants were generally healthy midlife adults from the Bronx, NY (N ​= ​212, Mage ​= ​46.77; 60.8% Black, 25.5% Hispanic/Latina/o) recruited as part of a larger study. Key measures were: reported pain intensity and pain interference at baseline, recent negative affect averaged from self-reports 5x/day for 7 days, and peripheral inflammatory markers (C-reactive protein [CRP] and a composite cytokine measure based on seven cytokines). Controlling for age, BMI, gender, and education, recent negative affect significantly interacted with both pain variables to explain variance in CRP, with higher CRP levels observed only in individuals with both higher negative affect and either higher pain intensity or pain interference. These findings contribute to an emerging literature suggesting that negative affect, pain, and inflammation are related in important and complex ways.

People living with CHD do less moderate-to-vigorous activity than their peers. This study sought to examine the impact of a community-based physical activity intervention for individuals with CHD. Individuals with CHD and family members participated in a 3 h, one-day Fearless event consisting of a variety of physical activity and education sessions. Consenting participants completed self-administered questionnaires pre-/post-event and completed a post-event feedback form. Descriptive statistics and paired t-tests were calculated across subgroups for each outcome/questionnaire. Written feedback was analyzed using a six-phase framework of reflexive thematic analysis. A total of 32 participants (six children, six adolescents, five youth, five all ages, and ten adults) with CHD completed this study. Following Fearless, youth with CHD reported spending less time being ‘inactive’ and more time being ‘somewhat active’. Adults with CHD reported spending more time walking and partaking in moderate activity and less time partaking in vigorous activity. Fearless successfully engaged individuals with CHD who were more sedentary, less active, and older. Fearless is a fun, family-friendly, physical activity intervention for individuals with CHD. Attending a Fearless event helped children, adolescents, and adults with CHD make incremental improvements to their physical activity levels and provided a framework for sport and recreation leaders who aim to promote physical activity amongst individuals with CHD.

The presence and diversity of marine non-native species, the number of new invasions, and the impact on native communities and habitats are important metrics used to assess the health of marine ecosystems. Monitoring for marine non-native species, using traditional approaches such as rapid assessment surveys (RASs), requires taxonomic expertise and may still fail to detect rare or inconspicuous species. This study reports a validation process for a quantitative PCR (qPCR) assay based on the cytochrome oxidase 1 gene, designed to detect highly invasive tunicate Didemnum vexillum by targeting environmental DNA (eDNA) present in water samples. The D. vexillum qPCR assay showed high sensitivity, with the threshold limit of detection (LOD) and modeled LOD3 (based on triplicate qPCR reactions) estimated as 9.187 and 1.117 copies reaction–1, respectively and the limit of quantification (LOQ) was calculated as 18 copies reaction–1. Analyses of water samples collected from selected Pacific oyster farms and recreational marinas in Scotland showed 100% concordance between the historical data on presence of D. vexillum from RASs and detection of D. vexillum eDNA. Consistency of detection of D. vexillum eDNA among different sampling points within each infected sampling site varied, ranging between 100% positive throughout the site to some sampling points testing “negative” or only as “suspected” for D. vexillum. Sites with lower within-site detection consistency included sites with a low density of D. vexillum as reported by RASs or were sites undergoing D. vexillum management. The present pilot monitoring program demonstrates the potential to generate important data on presence of D. vexillum. This program will be scaled up across large geographic regions and used in the first instance to focus and target the traditional RASs to D. vexillum eDNA-positive sites in a cost-effective way, with an aim to verify the species presence by visual observation and direct Sanger sequencing of positive qPCR products.

Identification of genomic mutations by molecular testing plays an important role in diagnosis, prognosis, and treatment of myeloid neoplasms. Next-generation sequencing (NGS) is an efficient method for simultaneous detection of clinically significant genomic mutations with high sensitivity. Various NGS based in-house developed and commercial myeloid neoplasm panels have been integrated into routine clinical practice. However, some genes frequently mutated in myeloid malignancies are particularly difficult to sequence with NGS panels (e.g., CEBPA, CARL, and FLT3). We report development and validation of a 48-gene NGS panel that includes genes that are technically challenging for molecular profiling of myeloid neoplasms including acute myeloid leukemia (AML), myelodysplastic syndrome (MDS), and myeloproliferative neoplasms (MPN). Target regions were captured by hybridization with complementary biotinylated DNA baits, and NGS was performed on an Illumina NextSeq500 instrument. A bioinformatics pipeline that was developed in-house was used to detect single nucleotide variations (SNVs), insertions/deletions (indels), and FLT3 internal tandem duplications (FLT3-ITD). An analytical validation study was performed on 184 unique specimens for variants with allele frequencies ≥5%. Variants identified by the 48-gene panel were compared to those identified by a 35-gene hematologic neoplasms panel using an additional 137 unique specimens. The developed assay was applied to a large cohort (n = 2,053) of patients with suspected myeloid neoplasms. Analytical validation yielded 99.6% sensitivity (95% CI: 98.9-99.9%) and 100% specificity (95% CI: 100%). Concordance of variants detected by the 2 tested panels was 100%. Among patients with suspected myeloid neoplasms (n = 2,053), 54.5% patients harbored at least one clinically significant mutation: 77% in AML patients, 48% in MDS, and 45% in MPN. Together, these findings demonstrate that the assay can identify mutations associated with diagnosis, prognosis, and treatment options of myeloid neoplasms even in technically challenging genes.

Abstract Background It is frequently assumed that pre-invasive lesions are simpler precursors of cancer and will contain a limited subset of the genomic changes seen in their associated invasive disease. Driver mutations are thought to occur early, but it is not known how many of these are present in pre-invasive lesions. These assumptions need to be tested with the increasing focus on both personalised cancer treatments and early detection methodologies. Methods We examined genomic copy number changes in 256 pre-invasive and invasive samples from 69 oral cancer patients. Forty-eight samples from 16 patients were further examined using exome sequencing. Results Evidence of a shared ancestor of both dysplasia and carcinoma was seen in all but one patient. One-third of dysplasias showed independent copy number events. The remainder had a copy number pattern that was similar to or simpler than that of the carcinoma. All dysplasias examined contained somatic mutations absent in the related carcinoma. Previously observed copy number changes and TP53 mutations were very frequently observed, and almost always shared between dysplasia and carcinoma. Other gene changes were more sporadic. Pathway analysis confirmed that each patient’s disease developed in a different way. Examining the numbers of shared mutations and the rate of accumulation of mutations showed evidence that all samples contain a population of sub-clones, with little evidence of selective advantage of a subset of these. Conclusions These findings suggest that most of the genomic changes driving oral cancer occur in the pre-cancerous state by way of gradual random accumulation rather than a dramatic single event.

There is growing recognition that with sympathetic management, plantation forests may contain more biodiversity than previously thought. However, the extent to which they may support bat populations is contentious. Many studies have demonstrated active avoidance of coniferous plantations and attributed this to the lack of available roost sites and low invertebrate density. In contrast, other work, carried out in plantation dominated landscapes have shown that certain bat species are able to exploit these areas. However, the extent to which bats use plantations for roosting and foraging, or simply move through the plantation matrix to access more favourable sites is unclear. We radio tracked female Pipistrellus pygmaeus over two summers to establish the extent to which individual bats use Sitka Spruce plantations in southern Scotland for foraging and roosting and assess the implications for felling operations on bats. Maternity roosts identified (n = 17) were in all in buildings and most were large (> 500 individuals). We found no evidence of bats roosting in mature Sitka Spruce crop trees, although several bats used roosts in old or dead beech and oak trees as an alternative to their main maternity roost. Home ranges were much larger (mean 9.6 ± 3.12 km2) than those reported from other studies (0.6-1.6 km2), and it is likely that roost availability rather than food abundance constrains P. pygmaeus use of Sitka Spruce plantations. At the landscape scale, most individuals selected coniferous habitats over other habitat types, covering large distances to access plantation areas, whilst at a local scale bats used forest tracks to access water, felled stands or patches of broadleaf cover within the plantation. Sitka Spruce plantations support a high abundance of Culicoides impuctatus, the Highland midge which may act as a reliable and plentiful food source for females during lactation, an energetically expensive period. The use of felled stands for foraging by bats has implications for forest management as wind turbines, following small-scale felling operations, are increasingly being installed in plantations; wind turbines have been associated with high bat mortality in some countries. Decisions about siting wind turbines in upland plantations should consider the likelihood of increased bat activity post felling.
