This article summarizes a real-world study on the use of dupilumab in treating prurigo nodularis (PN). The study examined 64 adult patients with PN and found that dupilumab effectively reduced disease severity and improved symptoms in most patients. Over the past decade, dupilumab, a monoclonal human antibody that inhibits interleukin (IL)-4 and IL-13 signalling, has revolutionized the therapeutic management of moderate-to-severe atopic dermatitis (AD), facilitating long-term control of its signs and symptoms. The aim of this study was to identify histological predictors of the efficacy of dupilumab after 16 weeks of treatment in a cohort of 40 adults with moderate-to-severe AD who had undergone a skin biopsy for diagnostic purposes before treatment initiation. We found that Eczema Area and Severity Index (EASI) 75 and EASI 90 responses at week 16 were significantly associated with perivascular localization [odds ratio (OR) 17.6; P = 0.04] and lichenoid distribution (OR 31.8; P = 0.03) of the immune infiltrate. Moreover, for each unit increase in the number (cells mm–2) of CD4+ cells, the likelihood of achieving an EASI 75 response decreased by 1% (OR 0.99; P = 0.04). This article discusses the use of tralokinumab, a targeted therapy, in patients with moderate-to-severe atopic dermatitis (AD) who are unresponsive, intolerant, or have contraindications to other available treatments such as dupilumab and JAK inhibitors. The study included 17 difficult-to-treat patients who had previously failed dupilumab or upadacitinib, and the results showed significant improvements in disease severity scores and physician-based assessments after treatment with tralokinumab. Adverse events were reported in 23.5% of patients, and 29.4% discontinued treatment due to ineffectiveness or adverse events. With the aim to recognise the morphological and stylistic variations in the context of the dictates of uniformity imposed by this typology, a study was conducted on the facades on porticoes that surround the square for approximately one kilometer. The study method took the survey of shape and size as a basic information nucleus integrated with the typologic-morphological analyses, while the graphic-geometric models were an operational modality and metalanguage for investigating the languages of project and construction. Hidradenitis suppurativa (HS) is a chronic-relapsing inflammatory skin disease. It usually appears in the second and third decades, but a smaller proportion of patients develop late-onset HS. Geriatric HS, defined as the persistence or the development of HS after the age of 65 years, has been poorly explored. This study aimed to investigate the clinical features, treatment management and response to therapies of HS elderly subjects (≥65 years old). We designed a multicentric observational study, gathering data from seven Italian university hospitals. Demographic and clinical data of HS patients aged over 65 years were collected at baseline, week 12 and week 24. Overall, 57 elderly subjects suffering from HS were enrolled. At baseline, disease severity was predominantly moderate-to-severe, with 45.6% of patients classified as Hurley III. The gluteal phenotype was the most frequently observed; it also appeared to affect patients' quality of life more than other phenotypes. Gluteal involvement was detected in about half (49.1%) of cases and associated with severe stages of the disease. In terms of therapeutic response, Hurley III patients showed the persistency of higher values of mean IHS4, DLQI, itch- and pain-NRS scores compared to Hurley I/II. Https://doi.org/10.1093/ced/llad040 Dear Editor, Atopic dermatitis (AD) is the most common inflammatory skin disease in both children and adults with more than 230 million people affected in the world.[1] Over one-third of patients are classified as having severe forms of disease often requiring a systemic treatment for it to be adequately controlled.[1] Although AD can affect any body area, the involvement of face, hands and genitalia are associated with social embarrassment, stigmatization and deterioration of patients' quality of life, this being considered as one of the criteria for defining AD as moderate-severe regardless of the Eczema Area and Severity Index (EASI) score.[2] The achievement of stable control of AD in exposed areas is often challenging because of constant exposure to irritants and allergens.[2],[3] Moreover, the long-term use of topical steroids is contraindicated on the face and genitalia because of the high risk of skin atrophy and telangiectasias.[2],[3] Dupilumab, a monoclonal antibody blocking the -interleukin (IL)-4alpha subunit shared by the receptor for IL-4 and IL-13, is the first biologic drug approved for the treatment of moderate/severe AD. Five out of eight patients achieved a complete clearance of AD lesions located on visible body areas after 4 weeks of treatment with a long-term response maintained through 52 weeks.
