INTRODUÇÃO: O efeito da radioterapia adjuvante após a reconstrução mamária com retalhos autólogos é controverso. O objetivo deste estudo é analisar se a radioterapia pós-operatória causa alterações volumétricas e cosméticas após a reconstrução mamária imediata com retalho do músculo reto abdominal (TRAM, do inglês transverse rectus abdominis myocutaneous). MÉTODO: No total, foram avaliadas 25 pacientes submetidas a reconstrução autóloga com retalho TRAM pediculado pós-mastectomia por câncer de mama e radioterapia adjuvante. Os resultados estéticos tardios foram coletados após o intervalo mínimo de 6 meses posteriormente ao esquema completo da radioterapia. RESULTADOS: A média de idade das pacientes foi de 42,2 anos, variando de 30 anos a 53 anos. Duas (8%) perdas completas do retalho ocorreram por necrose gordurosa maciça, duas (8%) pacientes evoluíram com contratura do retalho e perda volumétrica, e 52% das pacientes apresentaram alterações cutâneas. Entretanto, a maioria das pacientes (84%) não evoluiu com perda significativa do volume inicial do retalho ou com distorção do contorno e contração do retalho. CONCLUSÕES: O resultado cosmético após a reconstrução mamária imediata com retalho TRAM irradiado foi aceitável, comparativamente aos dados descritos na literatura. Esses achados indicam que os retalhos podem sofrer distorções de contorno pela contração, além de perda volumétrica significativa em pacientes candidatas à reconstrução com TRAM e necessidade de radioterapia adjuvante pós-mastectomia. Assim, as complicações tardias da irradiação pós-operatória devem ser consideradas nesse contexto.BACKGROUND: The outcome of adjuvant radiotherapy performed after breast reconstruction using autologous flaps is controversial. In this study, we aimed to assess whether postoperative radiotherapy would induce volumetric and cosmetic changes after immediate breast reconstruction performed using a transverse rectus abdominis myocutaneous (TRAM) flap. METHODS: We evaluated 25 patients who were previously diagnosed with breast cancer and underwent post-mastectomy autologous reconstruction with a pedicled TRAM flap followed by adjuvant radiotherapy. Late aesthetic results were recorded starting 6 months after the completion of a full course of radiotherapy. RESULTS: The average patient age was 42.2 years (range, 30-53 years). Two (8%) complete flap losses occurred due to massive fat necrosis. Two (8%) patients exhibited flap contracture and volume loss, whereas skin changes were observed in 52% of patients. Most of the patients (84%), however, had no significant initial flap volume loss, contour distortion, or flap contraction . CONCLUSIONS: The cosmetic result obtained in the present study by immediate breast reconstruction performed using an irradiated TRAM flap was satisfactory compared to the data described in the literature. These findings suggest that the flap contours may be distorted by contraction and that significant volume losses can be observed in patients who may be candidates for TRAM flap reconstruction and require post-mastectomy adjuvant radiotherapy. Thus, in these circumstances, late complications due to postoperative irradiation should be considered.

INTRODUÇÃO: A reconstrução do complexo areolopapilar (CAP) constitui o passo final na criação da neomama, com o objetivo de aproximá-la da mama contralateral após a mastectomia. A restauração do CAP tem sido historicamente um procedimento feito em estágios, com a reconstrução da papila realizada por meio de enxertos ou retalhos locais, previamente, e da aréola, posteriormente, buscando a coloração adequada. Atualmente, a utilização da tatuagem areolar está alcançando popularidade cada vez maior, comparativamente às técnicas tradicionais. O objetivo deste trabalho é descrever a técnica e os equipamentos utilizados para a tatuagem intradérmica em pacientes mastectomizadas, realizada no Serviço de Cirurgia Plástica e Microcirurgia Reconstrutiva do Hospital Universitário Walter Cantídio (Fortaleza, CE, Brasil). MÉTODO: O presente trabalho descreve os passos para a realização do procedimento reparador utilizando a tatuagem areolar permanente com equipamento convencional de tatuagem artística profissional. O procedimento foi aplicado em 10 pacientes submetidas a reconstrução mamária pós-mastectomia. RESULTADOS: O método demonstrou as vantagens da simplicidade técnica do procedimento, não requerendo internamento nem resultando em morbidade de áreas doadoras. CONCLUSÕES: A tatuagem do CAP é um procedimento seguro, rápido, com baixa morbidade e bons resultados na finalização da reconstrução mamária.BACKGROUND: The final step in the creation of the neo-breast is reconstruction of the nipple-areolar complex (NAC), which is aimed toward bringing the appearance of the NAC closer to that of the contralateral breast following mastectomy. NAC restoration has historically been a stepwise procedure, in which reconstruction of the nipple by the use of grafts or local flaps was followed by reconstruction of the areola to achieve the correct color. Currently, the popularity of the areolar tattoo is increasing compared to traditional techniques. The aim of this study was to describe the technique and equipment used for intra-dermal tattooing in mastectomized patients at the Plastic Surgery and Reconstructive Microsurgery Service of the Walter Cantídio University Hospital (Fortaleza, CE, Brazil). METHODS: This study describes the steps of the repair procedure by using permanent areolar tattooing with conventional equipment from a professional tattoo artist. The procedure was used in 10 patients undergoing post-mastectomy breast reconstruction. RESULTS: The advantages of this method included the technical simplicity of the procedure, the ability to use this procedure in outpatients, and the lack of morbidity of the donor areas. CONCLUSIONS: Tattooing the NAC is a safe, fast procedure with low morbidity and good results of breast reconstruction.

INTRODUÇÃO: A reconstrução mamária com retalho do músculo reto abdominal (TRAM, do inglês transverse rectus abdominis myocutaneous) tem se tornado um procedimento comumente realizado desde a década de 1990. O TRAM ipsilateral vem demonstrando ser tão seguro quanto o retalho contralateral na reconstrução mamária. Entretanto, estudos avaliando o resultado estético com as duas técnicas são escassos na literatura. A proposta deste estudo é comparar os resultados cosméticos entre os retalhos pediculados ipsilateral e contralateral. MÉTODO: Foi realizada avaliação prospectiva de 29 pacientes submetidas a reconstrução imediata com TRAM ipsilateral e contralateral. Os grupos foram comparados entre si, analisando-se os resultados estéticos. RESULTADOS: O estudo incluiu 29 pacientes, com média de idade foi 43 + 7 anos. O grupo 1 (TRAM ipsilateral) apresentou o sulco inframamário bem definido em 91,7% dos casos, comparativamente a 52,9% no grupo 2 (TRAM contralateral). Observou-se abaulamento na região xifoide em 8,3% dos pacientes do grupo 1 e em 23,5% dos pacientes do grupo 2. A diferença na forma global da mama reconstruída não foi importante, com 66,7% e 70,6% de simetria nos grupos 1 e 2, respectivamente. CONCLUSÕES: O TRAM ipsilateral demonstrou melhor manutenção do sulco inframamário e menor abaulamento na região xifoide. Entretanto, a forma global da mama e a projeção do polo inferior da mama foram similares entre os dois grupos.BACKGROUND: Breast reconstruction by using the rectus abdominis muscle (transverse rectus abdominis myocutaneous, TRAM) flaps is a common procedure that has been performed since the 1990s. Ipsilateral TRAM flaps were proven to be as safe as contralateral flaps for breast reconstruction. However, studies comparing the aesthetic outcomes of the two procedures are poorly described in the literature. The aim of this study was to compare the cosmetic outcomes of ipsilateral and contralateral pedicled flaps. METHODS: We prospectively evaluated 29 cases of immediate reconstruction with ipsilateral (group 1) or contralateral (group 2) TRAM flaps. The aesthetic outcomes were analyzed and the two groups were compared. RESULTS: The average age of the patients was 43 ± 7 years. In group 1 (ipsilateral TRAM), 91.7% of the patients presented a well-defined inframammary fold, as compared to 52.9% of the patients in group 2 (contralateral TRAM). In group 1, 8.3% of patients showed xiphoid bulges, whereas they were observed in 23.5% of patients in group 2. The difference in the overall shape of the reconstructed breast was not significant; symmetry was observed in 66.7% and 70.6% of patients in groups 1 and 2, respectively. CONCLUSIONS: Ipsilateral TRAM flaps allow better preservation of the inframammary fold and cause less xiphoid bulge. However, the overall shape of the breast and the projection of its lower pole were similar between the two groups.

Neurofibromatose é uma doença de origem genética autossômica dominante composta por três tipos: neurofibromatose tipo 1 (NF1), neurofibromatose tipo 2 (NF2) e schwannomatose. As principais características da NF1 são mancha café com leite, neurofibromas dérmicos e plexiformes, falsas efélides axilares ou inguinais e nódulos de Lisch. Neste trabalho é apresentado o caso de um paciente do sexo masculino, 26 anos de idade, com relato de aparecimento de pequenos nódulos cutâneos desde os 3 anos de idade e manchas café com leite disseminadas e de diferentes dimensões. Após os 13 anos de idade, o paciente apresentou crescimento acelerado de volumosa massa em dorso e abdome, impossibilitando a deambulação e causando afastamento do convívio social. O tratamento cirúrgico foi constituído de exérese do tumor em duas etapas, com intervalo de 2 meses. Não existe cura para a neurofibromatose, porém a remoção cirúrgica está indicada nos casos em que há dor, déficit neurológico, desfiguramento, comprometimento de estruturas adjacentes e suspeita de malignidade. São aceitáveis ressecções parciais, quando não houver possibilidade de exérese total. O tratamento cirúrgico realizado constituiu ótima opção para o tratamento do presente caso, uma vez que permitiu a exérese da lesão, com cicatrização satisfatória de ótimo aspecto estético e melhora da qualidade de vida do paciente.Neurofibromatosis is a disease of genetic origin with autosomal dominant inheritance that is classified into 3 types: neurofibromatosis type I (NF1), neurofibromatosis type 2 (NF2), and schwannomatosis. The main characteristics of NF1 are café-au-lait spots, dermal and plexiform neurofibromas, false dermal and plexiform neurofibromas, false axillary or inguinal ephelides, and Lisch nodules. This study describes the case of a 26-year-old man who presented with small cutaneous nodules, present since he was 3 years old, and café-au-lait spots of different sizes distributed diffusely. At the age of 13, the patient developed a mass in the back and abdomen that subsequently developed into a rapidly growing voluminous mass, which hindered walking and caused social isolation. The surgical treatment consisted of the excision of the tumor in 2 phases, with a 2-month interval between procedures. Although a cure for neurofibromatosis has not been discovered, surgical removal is indicated in cases of neurological involvement, pain, disfigurement, possible involvement of adjacent structures, and suspicion of malignancy. Partial resection is acceptable if total removal is not possible. In the present case, surgical treatment was an excellent choice because it enabled the complete excision of the lesion, with satisfactory wound healing and aesthetic results, as well as the improvement of the quality of life of the patient.

A Síndrome de Parry-Romberg, também conhecida como atrofia hemifacial progressiva, é uma doença rara caracterizada por lenta e progressiva atrofia de hemiface. O tratamento ofertado para a síndrome, geralmente, visa melhorar o aspecto estético. Os enxertos gordurosos, as injeções de silicone ou as próteses de acrílico são alternativas sugeridas para correção da atrofia facial. Atualmente, a técnica recomendada para correção da atrofia facial é cirurgia dermatológica cosmética com lipoenxertia autóloga. O objetivo deste estudo é relatar um caso de SPR e demonstrar que a cirurgia dermatológica pode aliviar danos sérios à anatomia do paciente, a partir da discussão dos aspectos terapêuticos da síndrome, com ênfase na lipoenxertia autólogaParry-Romberg syndrome, also known as progressive hemifacial atrophy, is a rare disorder characterized by slow and progressive hemifacial atrophy. The treatment offered for the syndrome generally aims at improving aesthetics. Fat grafts, silicone injections or acrylic prosthesis are alternatives suggested for correction of facial atrophy. Currently, the recommended technique for correction of facial atrophy is cosmetic dermatologic surgery with autologous fat grafting. This study reports a case of Parry-Romberg syndrome and demonstrates that dermatologic surgery can relieve serious damage to the patient's anatomy, starting from the discussion of the therapeutic aspects of the syndrome with emphasis on autologous fat grafting

INTRODUÇÃO: O espasmo hemifacial (EH) é um distúrbio do movimento caracterizado por contrações involuntárias dos músculos inervados pelo nervo facial, a descompressão microvascular (DMV) é considerada tratamento definitivo, com baixas taxas de recidiva. Os casos refratários tornam-se um desafio para o cirurgião plástico. RELATO DO CASO: MSP, sexo feminino, 53 anos, portadora de EH, refratária à DMV, e com queixas de flacidez dos dois terços inferiores da face. Foi submetida à ritidoplastia, neurectomias e miectomias seletivas. Obteve-se ausência completa dos espasmos, manutenção da mímica facial e rejuvenescimento facial. A ritidoplastia cérvico-facial associada a neurectomias e miectomias mostrou-se eficaz no tratamento do EH.INTRODUCTION: The peripheral facial palsy is a disease that can leave significant aesthetical and functional sequelae in patients. According to medical literature, Bell's palsy (BP) is the most common cause, representing from 50 to 80% of the cases. However, it must be considered as an exclusion diagnosis. A slightly higher prevalence is estimated in women and its incidence is bimodal with peaks in the third and eighth life decades. Most patients recover within weeks, though a worsening degree may be common in the first 48 hours. Treatment must be performed to prevent complications and the medical procedures are not universally accepted. The following have been mentioned as BP risk factors: arterial hypertension, mellitus diabetes, pregnancy, puerperium and infection caused by type I herpes virus. Although there are several BP associated risk factors, regarding face-lifting surgery there is only one reported case in medical literature. CASE REPORT: In the present study, the authors describe the second case of Bell's palsy after face-lifting surgery.

A técnica microcirúrgica que até a década de 80 do século passado se constituiu um procedimento de exceção, somente utilizada em casos extremos, devido aos avanços facilmente observados em diversas publicações atuais, tornou-se em muitos procedimentos a primeira opção cirúrgica, fazendo com quase que, de maneira obrigatória, todos os médicos que exerçam a especialidade de Cirurgia Plástica tenham que realizar treinamento na citada técnica operatória. O presente artigo se propõe a apresentar um modelo de fácil execução como treinamento básico em microcirurgia.The microsurgical technique witch until the 1980's was an exclusion procedure, only chosen in extreme cases, due to easily observed advances seen in innumerous current publications, has become in various procedures the first surgical option, practically compelling all doctors who practice in the field of Plastic Surgery to perform training in the referred technique. The current article presents an easy executed model of microsurgery basic training.

Neste artigo é apresentado um instrumento cirúrgico alternativo para suspensão da camada fibroadiposa do terço médio da face. A agulha foi confeccionada com fio de Kirschner, de forma que ambas as pontas se tornaram rombas. A dupla ponta possibilita a passagem do fio pelo sistema musculoaponeurótico superficial da face (SMAS), mantendo a sutura com a profundidade necessária para evitar dermatografia. Os testes com o novo instrumento foram realizados em ritidoplastias com dissecção mínima. A agulha descrita demonstrou-se segura, com fácil migração pelos tecidos e ausência de lesões vasculonervosas, facilitando o posicionamento dos pontos de fixação nos locais planejados. A agulha desenvolvida possibilitou a realização da ritidoplastia sem descolamento resultante de forma rápida, simples e custo-efetiva, com mínimo dano às estruturas anatômicas e aos tecidos.This article describes an alternative surgical instrument that can be used for suspension of the malar fat pad. The needle was created using Kirschner wire to ensure that both tips were blunt. The double tip allows for passage of the wire through the superficial muscular aponeurotic system, ensuring that the suture is sufficiently deep to avoid dermatography. The utility of the new instrument was tested in rhytidoplasties involving minimal dissection. The needle described herein was proven safe, and could easily migrate through the tissues, without creating vascular and nerve lesions, and facilitate the positioning of the fixation points at the intended sites. The use of this needle for rhytidoplasty was quick, simple, and cost-effective, without any resulting dissection or damage to the anatomical structures or tissues.

We present a case of Plummer-Vinson syndrome, which is a rare condition nowadays. The diagnosis was made after years of the disease, many doctors having attended the patient. The treatment consisted of oral supplementation of iron and endoscopic dilatations. The patient is asymptomatic.
