Abstract

The trend of renovate residential houses especially the interior of the house has become a common phenomenon for homeowners nowadays in Malaysia. This scenario is quiet concern because sometimes no modifications to comply with the law and the guidelines set by the government housing. Modifications with not done properly can cause injury and harm to families and the people around. To reduce this problem, the concept of sustainable inner walls with flexible floor plan should be incorporated in every house in Malaysia. This is because the wall is the basic structure of a building and usually serves as the border, supporting structures and dividing the space with another space. Wall also causes an increase of the price of a house. This is due to the increase in raw material costs and labor costs, land subsidence have to bear by the developer. The increasing in house prices is causing among Malaysians, especially young executives cannot afford to buy their first home. To reduce the price of the home, reduction in construction interior wall in wet construction should be done and replaced with the sustainable inner wall. This sustainable inner wall also can save the space and the owner simplify can added or reduced the room according their need without spending too much money for renovation in the future.

Abstract

In Lafia, Nasarawa State, a study was conducted to assess the growth response of Soybean (Glycine max) genotypes cultivated with Rice Husk Biochar. Two genotypes, T.G.S 1448 and T.G.S 1449, provided by the State's Nasarawa Agricultural Development Program (NADP), were used. The experiment was conducted using three replicates in a Complete Randomized Block Design (CRBD). Four levels of rice husk Biochar were used in the treatments: 0, 60, 120, and 180 g/pot. The TGS1448 genotype, when subjected to the 180g biochar treatment (B180g), had the highest number of leaves (70.33), significantly more than the control which had the fewest leaves (56.00), and the other treatments. The variety TGS1449 exposed to biochar treatment B180g had the maximum plant height (59.33cm), which was substantially higher than the control (50.60 cm) and the other treatments. The same pattern was seen in the TGS1448 variety when it was subjected to the biochar treatment B180g. After 9 weeks, plants with 180g biochar-amended soil produced 11 seeds per plant, whereas control plants did not produce pods or seeds. The study concluded that the addition of biochar to the soil significantly enhanced plant growth in the growth-restricted alfisol. [ABSTRACT FROM AUTHOR]

7. Endoplasmic reticulum stress in oral lichen planus-associated fibroblasts.

Abstract

The role of fibroblasts and their response to environmental stressors in the pathogenesis of oral lichen planus (OLP) is uncertain. Following endoplasmic reticulum stress (ERS), fibroblasts can differentiate into myofibroblasts and activate unfolded protein response (UPR), to maintain cell metabolism, regulate inflammatory responses and determine cell fate. This in vitro study sought to determine the effect of tunicamycin-induced ERS (T-ERS) on fibroblasts cultured from OLP lesions (OLP-F), in relation to the expression of the key markers of UPR. Normal oral fibroblasts (NOMF) and OLP-F cell lines (n = 3 each) were subjected to T-ERS for 12 hours and 24 hours. The expression of UPR markers (GRP78, IRE1, ATF6, and PERK) and SMA (myofibroblastic differentiation marker) were semi-quantitatively determined using immunocytochemistry. Data were analyzed using the Kruskal-Wallis test, with statistical significance set at P <.05. Without ERS, the expression of IRE1 was significantly lower in OLP-F compared with NOMF. The same trend was seen for GRP78 and ATF6, but it was not statistically significant. With T-ERS, UPR protein levels were generally lower in NOMF, except for PERK. In OLP-F, IRE1 and ATF6 levels were increased at 12 hours but reduced at 24 hours, although not significantly. SMA levels were significantly lower in OLP-F at 24 hours. The findings indicate that OLP-F and NOMF respond differently to ERS with a trend toward reduction of UPR protein expression. This suggests differential activation of UPR pathways in OLP with implications relating to its pathogenesis. Abstract

Introduction: Oral squamous cell carcinoma (OSCC) is an aggressive, highly immunosuppressive cancer with a high mortality rate. The interaction between ProgrammedDeath 1 (PD-1) (on T cells) and Programmed-Death Ligand 1 (PD-L1) (on tumour cells) within the tumour microenvironment facilitates T-lymphocytic exhaustion and down-regulation. The tumour thus creates an immunosuppressive network that promotes tumour survival and resists innate immunological defence. Elevated PD-L1 expression has been associated with poor prognosis in various other cancers. Blockage of PD-L1 enhances the immune response in a range of tumour-types. Objectives: To investigate and compare the expression of PD-L1 in non-dysplastic, dysplastic and OSCC samples using immunohistochemistry (IHC). Methods: IHC was performed on formalin-fixed, paraffinembedded, archival tissues. The tissues were divided into three groups; non-dysplastic (n = 20), dysplastic (n = 20) and OSCC (n = 20) tissues. Qualitative and quantitative analyses of positively stained cells were undertaken. One-way analysis of variance (ANOVA) was performed to detect statistically significant differences between PD-L1 expression in the dysplastic and OSCC groups when compared against the non-dysplastic group, which served as a control in this experiment. Results: A higher proportion, darker intensity and a higher immunoreactive score of PD-L1-positive cells was found in the OSCC and dysplastic groups, when compared to the non-dysplastic control group. This difference was statistically significant (p<0.05). There was no significant difference between the OSCC and dysplastic tissues. Conclusion: Greater expression of PD-L1 was correlated to the presence of dysplastic and malignant change. An increase in PD-L1 expression may indicate a disease progression from non-dysplastic tissue to dysplasia and malignancy. [ABSTRACT FROM AUTHOR]

Abstract

Caries results in the demineralization and destruction of enamel and dentine, and as the disease progresses, irreversible pulpitis can occur. Vital pulp therapy (VPT) is directed towards pulp preservation and the prevention of the progression of inflammation. The outcomes of VPT are not always predictable, and there is often a poor correlation between clinical signs and symptoms, and the events occurring at a molecular level. The inflamed pulp expresses increased levels of cytokines, including tumour necrosis factor (TNF)‐α, interleukin (IL)‐1α, IL‐1β, IL‐4, IL‐6, IL‐8, IL‐17 and IL‐23, which recruit and drive a complex cellular immune response. Chronic inflammation and sustained cytokine release can result in irreversible pulp damage and a decreased capacity for tissue healing. Other chronic inflammatory diseases, such as psoriasis, inflammatory bowel diseases and rheumatoid arthritis, are also characterized by an dysregulated immune response composed of relatively high cytokine levels and increased numbers of immune cells along with microbial and hard–soft tissue destructive pathologies. Whilst anti‐cytokine therapies have been successfully applied in the treatment of these diseases, this approach is yet to be attempted in cases of pulp inflammation. This review therefore focuses on the similarities in the aetiology between chronic inflammatory diseases and pulpitis, and explores how anti‐cytokine therapies could be applied to manage an inflamed pulp and facilitate healing. Further proof‐of‐concept studies and clinical trials are justified to determine the effectiveness of these treatments to enable more predictable outcomes in VPT. [ABSTRACT FROM AUTHOR]

Abstract

The biochemical response to ursodeoxycholic acid (UDCA)—so-called “treatment response”—strongly predicts long-term outcome in primary biliary cholangitis (PBC). Several long-term prognostic models based solely on the treatment response have been developed that are widely used to risk stratify PBC patients and guide their management. However, they do not take other prognostic variables into account, such as the stage of the liver disease. We sought to improve existing long-term prognostic models of PBC using data from the UK-PBC Research Cohort. We performed Cox's proportional hazards regression analysis of diverse explanatory variables in a derivation cohort of 1,916 UDCA-treated participants. We used nonautomatic backward selection to derive the best-fitting Cox model, from which we derived a multivariable fractional polynomial model. We combined linear predictors and baseline survivor functions in equations to score the risk of a liver transplant or liver-related death occurring within 5, 10, or 15 years. We validated these risk scores in an independent cohort of 1,249 UDCA-treated participants. The best-fitting model consisted of the baseline albumin and platelet count, as well as the bilirubin, transaminases, and alkaline phosphatase, after 12 months of UDCA. In the validation cohort, the 5-, 10-, and 15-year risk scores were highly accurate (areas under the curve: >0.90). Conclusions: The prognosis of PBC patients can be accurately evaluated using the UK-PBC risk scores. They may be used to identify high-risk patients for closer monitoring and second-line therapies, as well as low-risk patients who could potentially be followed up in primary care. Abstract

There is a global increase in the prevalence of human papillomavirus (HPV)-driven oropharyngeal squamous cell carcinoma (OPSCC) in Australia and New Zealand. Risk factors for HPV-positive OPSCC are male gender, white race, age older than 40 but younger than 59 years old, having multiple lifetime sex partners, having oro-genital and oro-anal sex. High-risk HPV subtypes play a major role in the pathogenesis of OPSCC, however, they play a much lesser role in oral squamous cell carcinoma (OSCC). Among the laboratory tests used to detect oncogenic HPV infection, polymerase chain reaction is a sensitive method but does not reflect the role of HPV in oncogenesis. While widely used, p16 immunohistochemistry is both a sensitive and a specific surrogate marker for oncogenic HPV infection in OPSCC, but not in OSCC. However, it is a useful prognostic marker in OPSCC. The current gold standard to accurately detect oncogenic HPV infection is E6/E7 mRNAin situ hybridization. Because both HPV-positive and p16-positive OPSCC have better short-term prognoses there is current debate and trials on treatment de-escalation in HPV-positive OPSCC. Abstract

Aim To compare the accuracy of film and digital periapical radiography ( PR) in detecting apical periodontitis ( AP) using histopathological findings as a reference standard. Methodology Jaw sections containing 67 teeth (86 roots) were collected from nine fresh, unclaimed bodies that were due for cremation. Imaging was carried out to detect AP lesions using film and digital PR with a centred view ( FP and DP groups); film and digital PR combining central with 10˚ mesially and distally angled (parallax) views ( FPS and DPS groups). All specimens underwent histopathological examination to confirm the diagnosis of AP. Sensitivity, specificity and predictive values of PR were analysed using rater mean ( n = 5). Receiver operating characteristics ( ROC) analysis was carried out. Results Sensitivity was 0.16, 0.37, 0.27 and 0.38 for FP, FPS, DP and DPS, respectively. Both FP and FPS had specificity and positive predictive values of 1.0, whilst DP and DPS had specificity and positive predictive values of 0.99. Negative predictive value was 0.36, 0.43, 0.39 and 0.44 for FP, FPS, DP and DPS, respectively. Area under the curve ( AUC) for the various imaging methods was 0.562 (FP), 0.629 (DP), 0.685 (FPS), 0.6880 (DPS). Conclusions The diagnostic accuracy of single digital periapical radiography was significantly better than single film periapical radiography. The inclusion of two additional horizontal (parallax) angulated periapical radiograph images (mesial and distal horizontal angulations) significantly improved detection of apical periodontitis. Abstract

Background Since 1970, there has been a 400% increase in liver-related deaths due to the increasing prevalence of chronic liver disease in the United Kingdom (UK). The 2013 UK National Confidential Enquiry into Patient Outcome and Death report found that only 47% of patients who died from alcohol-related liver disease received 'good care' during their hospital stay. Aim To develop a 'care bundle' for patients with decompensated cirrhosis, aiming to ensure that evidence-based treatments are delivered within the first 24 h of hospital admission. Methods This work gives practical advice about how to implement the bundle and examines its effects on patient care at three National Health Service Hospital Trusts in the UK by collecting data on patient care before and after introduction of the bundle. Results Data were collected on 228 patients across three centres (59% male, median age 53 years). Alcohol-related liver disease was the aetiology of chronic liver disease in 85% of patients. The overall mortality rate during hospital admission was 15%. The audits demonstrated improvements in patient care for patients with a completed care bundle who were significantly more likely to have a diagnostic ascitic performed within the first 24 h ( P = 0.020), have an accurate alcohol history documented ( P < 0.0001) and be given antibiotics as prophylaxis against infection following a variceal haemorrhage ( P = 0.0096). In Newcastle, the bundle completion rate increased from 25% to 90% during the review periods. Conclusions The introduction of a care bundle was associated with increased rates of diagnostic paracentesis and antibiotic prophylaxis with variceal haemorrhage in patients with decompensated cirrhosis. Background: The aim of this study was to investigate the expression of toll-like receptor 2 (TLR2) on cells associated with oral squamous cell carcinoma, epithelial dysplasia and irritative hyperplasia, using immunohistochemistry.Results: More immune cells expressed TLR2 in carcinoma and dysplasia than in hyperplasia (P<0.001). No hyperplastic samples showed positive TLR2 staining on keratinocytes, whereas keratinocytes in 64% of cases of carcinoma and 74% of cases of dysplasia were TLR2 positive.Conclusion: Positive TLR2 expression in the microenvironment suggests activation of immune surveillance against the altered epithelium, whereas TLR2 expression by malignant keratinocytes may be indicative of resistance to apoptosis as a pro-survival mechanism. Abstract

The aim of this study is to determine the incidence and frequency of oral maxillofacial tumour reported by Department of Oral Pathology and Oral Medicine, Faculty of Dentistry Universiti Kebangsaan Malaysia (UKM) during the five years period from 1999 to 2003. A total of 226 reports were reviewed. Majority of the specimens reported were sent by the oral surgeons and dental practitioners in the Oral Maxillofacial Unit Hospital Universiti Kebangsaan Malaysia (HUKM), Faculty of Dentistry UKM and Oral Health Division, Kementerian Pertahanan Malaysia. Two hundred and nine diagnosed tissues (87%) were of benign in nature and 17 (7.5%) were malignant. Pyogenic granuloma was the most common lesion with 23 cases (10.2%) reported, followed by squamous cell carcinoma (6.6%), fibroepithelial polyp / fibrous epulis (6.2%), mucous extravasation cyst (6.2%) and ameloblastoma (6.2%). Of the 17 malignancies retrieved, squamous cell carcinoma (n=15) was the most predominant. The relative frequencies of most oral maxillofacial tumour seen in UKM dental faculty are comparative to most similar surveys in the literature particularly with the ones in South East Asia region. Squamous cell carcinoma was ranked 2nd in the series of diagnosed lesion and was the commonest malignancy seen. [ABSTRACT FROM AUTHOR]

Abstract

Aim Methodology Results Conclusion To investigate the influence of type 2 diabetes (T2D) and hyperglycaemia on blood vessels and angiogenic markers in the dental pulp.Extracted non‐carious permanent molar teeth were collected from patients with well‐controlled T2D (n = 10) and non‐T2D (controls) (n = 10). The pulp was examined qualitatively using haematoxylin and eosin and Van Gieson stains. Immunohistochemistry (IHC) identified the primary receptor for VEGF, VEGFR2, and the endothelial cell marker CD34. Primary human dental pulp cell (hDPC) lines (n = 3) were established from tissue explants and cells were grown in media containing 5.5 mM D‐glucose (control), 12.5 mM (prediabetes) and 25 mM (T2D) D‐glucose under normoxic conditions for 24, 48 and 72 h. Assays for metabolic activity (PrestoBlue) and cell viability (Crystal Violet staining) assessed the hDPC response to hyperglycaemia. The expression of angiogenic genes VEGFA, KDR, FLT‐1, ANGPT1, ANGPT2, TIE1 and TEK were analysed using quantitative real‐time polymerase chain reaction. ELISAs were used to quantify the level of expressed protein for VEGFA, ANG1, ANG2, TIE1, and TIE2 in the media. Data analyses were performed using GraphPad Prism and anova tests at p < .05.Blood vessels in T2D samples had thicker walls and stained strongly for elastin and collagen compared with non‐T2D samples. VEGFR2 protein was not seen in any T2D samples but consistently detected in healthy specimens. Culturing healthy cells in high glucose (25 mM) significantly reduced cell viability at 24 h compared to the control (p = .005) and 12.5 mM glucose (p = .001) but the metabolic activity was not greatly affected by glucose and time. VEGFA mRNA and VEGFA protein expression were detected in the hDPCs in the presence of hyperglycaemia over time; however, the primary receptor, VEGFR2/KDR, was not detected. Genes for the ANG1 and ANG2 and their receptors were expressed at all glucose concentrations but hyperglycaemia upregulated ANG2 mRNA. Proteins for all growth factors were detected in the media however proteins for TIE1 and TIE2 receptors were not.T2D and hyperglycaemia may impair the angiogenic response in the pulp similar to other body site. The scarcity of VEGFR2 and increased expression of ANG2 in response to hyperglycaemia suggests that VEGF and ANG‐Tie1/Tie2 signalling may be compromised. [ABSTRACT FROM AUTHOR]

36. OP24 Chronic hepatitis B virus infection in the UK: a multicentre study of clinical and virological characteristics.

Abstract

Introduction Hepatitis B virus (HBV) infection is an important health problem in the UK where 180 000 to 326 000 people are estimated to be chronic carriers of the virus and at increased risk of cirrhosis and hepatocellular carcinoma. Aim To establish a national register of chronic HBV patients for long-term follow-up to provide comprehensive data on demographic, clinical, and virological characteristics of chronic HBV infection in the UK. Method Adult patients with chronic HBV infection (HBsAg positive >6 months) under active follow-up in 15 UK liver centres were eligible. Patient recruitment commenced in February 2007. Demographic and clinical data were recorded for all patients. Serum samples were collected on a subset of patients to determine HBV genotype and presence of mutations by sequencing. Results 1147 patients have been registered. Their mean age was 43±13 years; 57% were male. The most common ethnicities were Chinese (26%), white (22%) and Pakistani (18%). The majority (81%, 676/831) were born outside the UK primarily in Pakistan (15%), Hong Kong (12%) and China (10%). 22% were HBeAg positive. Cirrhosis was present in 17% of cases that had a liver biopsy (n=447). 63% were HBV antiviral treatment naïve. 33% were currently on treatment of which 53% were on monotherapy primarily with either lamivudine (53%) or entecavir (28%). HBV DNA was currently undetectable in 53% (193/362) of treated subjects. The prevalence of HBV genotypes in those sequenced (n=293) was as follows: A (14%), B (17%), C (18%), D (42.7%), E (8%) and G (0.3%). HBV genotypes differed according to ethnicity (p<0.001): whites were predominantly genotypes A (46%) and D (46%), Chinese genotypes B (44%) and C (46%), black Africans genotypes A (21%) and E (53%) and Pakistanis genotype D (97%). Precore and basal core promoter region mutations (BCP; T1762/A1764) occurred in 45% (127/280) and 15% (42/280) of those tested, respectively. 20% (56/280) of the samples carried both precore and BCP mutations. HBsAg mutations indicative of altered antigenicity and could potentially be involved in vaccine escape were detected in 19% (54/286). Polymerase mutations associated with antiviral resistance were identified in 23% (18/77) (only those currently/previously on treatment). Conclusion We have established the first nationwide chronic HBV register in the UK. In our study population, chronic HBV infection was predominant in immigrants born in countries with high HBV prevalence. A strong association was found between HBV genotypes and ethnicity. Precore mutations were more common than BCP mutations and one-fifth of those tested carried both mutations. The cross-sectional data have enabled exploration of epidemiological associations with chronic HBV infection, the use of antivirals, molecular typing and emergence of novel resistance strains in clinical practice which may differ from clinical trial results. Abstract

Background & aims: Genome-wide association studies in primary biliary cholangitis (PBC) have failed to find X chromosome (chrX) variants associated with the disease. Here, we specifically explore the chrX contribution to PBC, a sexually dimorphic complex autoimmune disease. Methods: We performed a chrX-wide association study, including genotype data from 5 genome-wide association studies (from Italy, United Kingdom, Canada, China, and Japan; 5244 case patients and 11,875 control individuals). Results: Single-marker association analyses found approximately 100 loci displaying P < 5 × 10-4, with the most significant being a signal within the OTUD5 gene (rs3027490; P = 4.80 × 10-6; odds ratio [OR], 1.39; 95% confidence interval [CI], 1.028-1.88; Japanese cohort). Although the transethnic meta-analysis evidenced only a suggestive signal (rs2239452, mapping within the PIM2 gene; OR, 1.17; 95% CI, 1.09-1.26; P = 9.93 × 10-8), the population-specific meta-analysis showed a genome-wide significant locus in East Asian individuals pointing to the same region (rs7059064, mapping within the GRIPAP1 gene; P = 6.2 × 10-9; OR, 1.33; 95% CI, 1.21-1.46). Indeed, rs7059064 tags a unique linkage disequilibrium block including 7 genes: TIMM17B, PQBP1, PIM2, SLC35A2, OTUD5, KCND1, and GRIPAP1, as well as a superenhancer (GH0XJ048933 within OTUD5) targeting all these genes. GH0XJ048933 is also predicted to target FOXP3, the main T-regulatory cell lineage specification factor. Consistently, OTUD5 and FOXP3 RNA levels were up-regulated in PBC case patients (1.75- and 1.64-fold, respectively). Conclusions: This work represents the first comprehensive study, to our knowledge, of the chrX contribution to the genetics of an autoimmune liver disease and shows a novel PBC-related genome-wide significant locus.

Abstract

[BACKGROUND & AIMS] Primary biliary cholangitis (PBC) is a chronic liver disease in which autoimmune destruction of the small intra-hepatic bile ducts eventually leads to cirrhosis. Many patients have inadequate response to licensed medications, motivating the search for novel therapies. Previous genome-wide association studies (GWAS) and meta-analyses (GWMA) of PBC have identified numerous risk loci for this condition, providing insight into its aetiology. We undertook the largest GWMA of PBC to date, aiming to identify additional risk loci and prioritise candidate genes for in silico drug efficacy screening. [METHODS] We combined new and existing genotype data for 10, 516 cases and 20, 772 controls from five European and two East Asian cohorts. [RESULTS] We identified 56 genome-wide significant loci (20 novel) including 46 in European, 13 in Asian, and 41 in combined cohorts; and a 57th genome-wide significant locus (also novel) in conditional analysis of the European cohorts. Candidate genes at newly identified loci include FCRL3, INAVA, PRDM1, IRF7, CCR6, CD226, and IL12RB1, each having key roles in immunity. Pathway analysis reiterated the likely importance of pattern recognition receptor and TNF signalling, Jak-STAT signalling, and differentiation of TH1 and TH17 cells in the pathogenesis of this disease. Drug efficacy screening identified several medications predicted to be therapeutic in PBC, some well-established in the treatment of other autoimmune disorders. [CONCLUSIONS] This study has identified additional risk loci for PBC, provided a hierarchy of agents that could be trialled in this condition, and emphasised the value of genetic and genomic approaches to drug discovery in complex disorders. In this study, we analysed genetic information from 10, 516 people with PBC and 20, 772 healthy individuals recruited in Canada, China, Italy, Japan, UK, or USA. We identified several genetic regions associated with PBC. Each of these regions contains several genes. For each region, we used diverse sources of evidence to help us choose the gene most likely to be involved in causing PBC. Abstract

Primary biliary cirrhosis (PBC) is a classical autoimmune liver disease for which effective immunomodulatory therapy is lacking. Here we perform meta-analyses of discovery data sets from genome-wide association studies of European subjects (n=2,764 cases and 10,475 controls) followed by validation genotyping in an independent cohort (n=3,716 cases and 4,261 controls). We discover and validate six previously unknown risk loci for PBC (Pcombined, Primary biliary cirrhosis is an autoimmune liver disease with poor therapeutic options. Here Cordell et al. a perform meta-analysis of European genome-wide association studies identifying six novel risk loci and a number of potential therapeutic pathways.

Abstract

Background: Treatment guidelines recommend a stepwise approach to primary biliary cholangitis: all patients begin treatment with ursodeoxycholic acid (UDCA) monotherapy and those with an inadequate biochemical response after 12 months are subsequently considered for second-line therapies. However, as a result, patients at the highest risk can wait the longest for effective treatment. We determined whether UDCA response can be accurately predicted using pretreatment clinical parameters. Methods: We did logistic regression analysis of pretreatment variables in a discovery cohort of patients in the UK with primary biliary cholangitis to derive the best-fitting model of UDCA response, defined as alkaline phosphatase less than 1·67 times the upper limit of normal (ULN), measured after 12 months of treatment with UDCA. We validated the model in an external cohort of patients with primary biliary cholangitis and treated with UDCA in Italy. Additionally, we assessed correlations between model predictions and key histological features, such as biliary injury and fibrosis, on liver biopsy samples. Findings: 2703 participants diagnosed with primary biliary cholangitis between Jan 1, 1998, and May 31, 2015, were included in the UK-PBC cohort for derivation of the model. Abstract

Introduction: This is a retrospective study that examines the risk of non-COVID-19 respiratory infection (RI)-related emergency department (ED) visits and hospitalizations among autistic adults. The study compares these findings to non-autistic adults using the 2018 Healthcare Cost and Utilization Project Nationwide Emergency Department Sample., Methods: The data were analyzed in 2022 using the ICD-10-CM codes to extract 46,996 case records that included an autism diagonosis matched by age and sex (140,997) records that did not include an autism diagnosis in a 1:3 case-control ratio. Respiratory infections were also identified using the ICD-10-CM codes and classified by type. Logistic regression models were conducted for the likelihood of presenting with respiratory infections to the emergency department and subsequent hospitalization. All models were adjusted for covariates., Results: Autistic adults were more likely to present with any type of respiratory infection at the emergency department (AOR=1.83, CI=1.69-2.42), lower respiratory infections (AOR=1.37, CI=1.09-1.50), and pneumonia (AOR=2.42, CI=1.98-2.47) compared to non-autistic adults.. They were also more likely to be hospitalized from respiratory infection during emergency department visits (AOR=3.87, CI=3.21-4.30), including upper respiratory infections and lower respiratory infections, pneumonia, and bronchitis., Conclusions: Individuals with autism were more likely to experience emergency department visits and hospitalizations because of respiratory infections than individuals without autism. Amid growing evidence of the disproportionate impact of COVID-19 on the autistic population, the study findings highlight a broader, pre-existing burden of respiratory infections among adults with autism in the U.S. that extend beyond the recent pandemic., (Copyright © 2024 Elsevier Inc. Abstract

Gastric cancer remains a major global health concern with high incidence and mortality rates, particularly in East Asia. Patients often have poor outcomes due to limited treatment efficacy. Zolbetuximab, a monoclonal antibody targeting claudin 18.2 (CLDN18.2)-overexpressed in 50% to 80% of gastric cancers-demonstrates promise by initiating antibody-dependent cellular cytotoxicity and complement-dependent cytotoxicity in CLDN18.2-positive cells. In clinical trials, zolbetuximab with chemotherapy improved progression-free survival (PFS) and overall survival (OS). The FAST trial showed a median OS increase from 8.4 months to 13.2 months (HR, 0.72; P  < .01). The SPOTLIGHT trial found PFS extended to 11.0 months vs. 8.9 months (HR, 0.73; P  = .0024) with OS reaching 18.2 months in the zolbetuximab arm. The GLOW trial also confirmed efficacy, with median OS improving from 12.16 months to 14.39 months (HR, 0.771; P  = .0118). Zolbetuximab's targeted action, combined with manageable adverse effects, positions it as a promising therapy for advanced gastric cancer.

Abstract

Purpose: This research aimed to investigate the relative frequency of odontogenic tumours (OT) and selected odontogenic cysts in a single oral pathology center in New Zealand from 2008 to 2023., Methods: Histopathological records from the Oral Pathology Centre, University of Otago (2008-2023) were examined to identify OT. Odontogenic keratocyst (OKC) and calcifying odontogenic cyst (COC), previously classified as OT were also included. Patient demographics, clinical details and histopathologic diagnoses were recorded. Data were analyzed using SPSS., Results: Of the 34,225 biopsies over the 15-year period, 1.8% were identified as OTs, COC and OKCs and accounted for 47%, 1.5% and 51.5% respectively. The most prevalent OT types were odontoma (43.7%), ameloblastoma (27%) and cemento-ossifying fibroma (7.5%). Malignant OT, ameloblastic carcinoma, constituted 1.4% of OT. The average age at diagnosis for OKC, COC and OT patients were 48.2 ± 20.9, 33.7 ± 23.3 and 28.9 ± 19.3 years. Overall, male and mandibular site predilections were observed. Recurrence of OKC and ameloblastoma occurred in 15.2% and 13.7% of patients. The time for recurrence for OKC and Ameloblastoma were 61.7 ± 56.5 months and 122 ± 152 months respectively., Conclusion: The demographic features and range of OT, COC and OKC in New Zealand align with those of other western countries. The study also confirms need for long term follow up for patient with OKC and ameloblastoma., (© 2024. Abstract

This study utilized faradaic and non-faradaic electrochemical impedance spectroscopy to detect alpha synuclein amyloid fibrils on gold interdigitated tetraelectrodes (AuIDTE), providing valuable insights into electrochemical reactions for clinical use. AuIDE was purchased, modified with zinc oxide for increased hydrophobicity. Functionalization was conducted with hexacyanidoferrate and carbonyldiimidazole. Faradaic electrochemical impedance spectroscopy has been extensively explored in clinical diagnostics and biomedical research, providing information on the performance and stability of electrochemical biosensors. This understanding can help develop more sensitive, selective, and reliable biosensing platforms for the detection of clinically relevant analytes like biomarkers, proteins, and nucleic acids. Non-faradaic electrochemical impedance spectroscopy measures the interfacial capacitance at the electrode-electrolyte interface, eliminating the need for redox-active species and simplifying experimental setups. It has practical implications in clinical settings, like real-time detection and monitoring of biomolecules and biomarkers by tracking changes in interfacial capacitance. The limit of detection (LOD) for normal alpha synuclein in faradaic mode is 2.39-fM, The LOD for aggregated alpha synuclein detection is 1.82-fM. The LOD for non-faradaic detection of normal alpha synuclein is 2.22-fM, and the LOD for nonfaradaic detection of aggregated alpha synuclein is 2.40-fM. Abstract

Objectives: Type 2 diabetes (T2D) is a metabolic disease of major public health concern. It impacts peripheral tissues and the central nervous system, leading to systemic dysmetabolism and neurocognitive impairments, including memory deficits, anxiety, and depression. The metabolic determinants of these neurocognitive impairments remain unidentified. Here, we sought to address this question by developing a proprietary (P-) high-fat diet (HFD), in which glucose intolerance precedes weight gain and insulin resistance., Methods: The P-HFD model was nutritionally characterized, and tested in vivo in mice that underwent behavioral and metabolic testing. The diet was benchmarked against reference models. ., Results: P-HFD has 42% kcal from fat, high monounsaturated/polyunsaturated fatty acid ratio, and 10% (w/v) sucrose in drinking water. When administered, from the early stages of glucose intolerance alone, animals exhibit anxiety-like behavior, without depression nor recognition memory deficits. Long-term P-HFD feeding leads to weight gain, brain glucose hypometabolism as well as impaired recognition memory. Using an established genetic model of T2D (db/db) and of diet-induced obesity (60% kcal from fat) we show that additional insulin resistance and obesity are associated with depressive-like behaviors and recognition memory deficits., Discussion: Our findings demonstrate that glucose intolerance alone can elicit anxiety-like behavior. Through this study, we also provide a novel nutritional model (P-HFD) to characterize the discrete effects of glucose intolerance on cognition, behavior, and the physiology of metabolic disease.
