As hérnias umbilicais são muito comuns em cães, uma complicação frequente desta anomalia quando a hérnia se torna muito grande é o encarceramento ou estrangulamento de alças intestinais. Foi atendido na Clínica Escola Veterinária Prof. Marcos Vinicíus Tranquilim (CEVET), da Universidade Estadual do Centro-Oeste (UNICENTRO) um cão, macho com aproximadamente oito meses de idade apresentando fístula intestinal congênita em região de hérnia umbilical. Logo após o atendimento o animal foi encaminhado para o procedimento cirúrgico, o qual foi realizado com sucesso e o animal recuperou-se completamente. A realização do procedimento cirúrgico imediato foi de grande importância para a recuperação do paciente, uma vez que ainda não havia derrame de conteúdo fecal na cavidade abdominal.

RESUMEN Introducción: el ombligo es la única cicatriz considerada normal, es un componente esencial de la belleza. La paciente presenta una forma atípica del mismo acompañado de una hernia umbilical por lo que se decide realizar una umbilicoplastia basada en una técnica que dé una imagen estéticamente agradable. Caso clínico: paciente femenino, 17 años, llega a Consulta de Cirugía Plástica por presentar un ombligo que le provoca sentimientos de inferioridad y “complejo”, su forma se asemeja a una “cerradura de puerta” con una depresión a nivel de la piel por debajo de la cicatriz umbilical. Discusión: es necesario evaluar de forma integral el estado de salud. En el caso del ombligo acuden para cambiar el tamaño, forma, profundidad y posición para hacerlo ver estéticamente más agradable. Se realizó una incisión y cierre en “V-Y”, quedó de esta manera un ombligo con las características anatómicas normales, además de una forma y tamaño adecuados. Conclusiones: se logró un resultado estético adecuado. ABSTRACT Introduction: The navel is the only scar considered normal, being an essential component of beauty. RESUMEN Las deformidades de la pared abdominal son frecuentes en Burkina Faso por lo que constituyen motivo de consulta de los pacientes que la padecen en los servicios Cirugía General. Estas causan serios trastornos estéticos, funcionales y psicológicos en los mismos. La asociación de procederes quirúrgicos para el tratamiento de las hernias abdominales asociadas con el vientre péndulo data de la época de la década de los años 1970 la que ha tenido seguidores y ha demostrado resultados satisfactorios, al comprobar que se evitan complicaciones y al facilitar el campo operatorio al cirujano a la hora de hacer la hernioplastia. Se presenta un caso de un paciente que padece una hernia umbilical gigante asociada con vientre péndulo. ABSTRACT Abdominal wall deformities are frequent in Burkina Faso and are a reason for consultation of patients who suffer from it in the General Surgery services. These cause serious aesthetic, functional and psychological disorders in them. The association of surgical procedures for the treatment of abdominal hernias associated with the pendulum belly dates back to the 1970s, which has had followers and has shown satisfactory results, when checking that complications are avoided and when facilitating the operative field to the surgeon when doing a hernioplasty. We present a case of a patient suffering from a giant umbilical hernia associated with pendulous belly.

RACIONAL: Os defeitos da linha média podem ser congênitos ou adquiridos. Os procedimentos convencionais para correção desse defeito geralmente envolvem grandes incisões, com grandes descolamentos de pele e tecido celular subcutâneo. O uso da videocirurgia para a correção desses defeitos, ainda é controverso. OBJETIVOS: Realizar descrição inédita na literatura, mostrando a experiência inicial do uso da robótica nas reconstruções de linha média, associando a cirurgia minimamente invasiva à técnicas consagradas como Rives/Stoppa e separação de componentes. MÉTODOS: Foram operados cinco pacientes no mesmo hospital, pela mesma equipe, usando o sistema robótico da Vinci S. RESULTADOS: Foram três mulheres e dois homens, sem mortalidade na amostra. Duas pacientes foram reoperadas com hérnia pelo tunel entre os músculos retos do abdomen e aponeurose posterior, com fechamento dos mesmos na reoperação. CONCLUSÕES: O procedimento robótico para reconstrução da linha média mostrou-se factível e esteticamente aceitável. Tem a vantagem de seguir os princípios tradicionais aventados para a parede abdominal através de via minimamente invasiva.BACKGROUND:The weakness of the linea alba can be caused by congenital and aquired factors. The conventional procedure to correct these imperfections generally involve large incisions with big detachments of the skin and subcutaneous tissue. The use of videosurgery for the repair of these weaknesses is still controversy. AIM: To describe a new procedure using robotics in the repair of the linea alba, associating minimally invasive tecniques by Rives/Stoppa and component separation tecniques. METHODS: Five patients undergone surgery in the same hospital, the same operating team and using the Da Vinci S. robotics equipment. RESULTS: Three women and two men undergone surgery, with no mortality. Two of these patients were re-operated due a recurrent hernia between muscle and posterior sheath that was closed in the re-access. CONCLUSIONS: The robotic procedure in the reconstruction of the linea alba showed itself feasible and aesthetically acceptable. Also, in advantage, the procedure follows the traditional principals reputable by experts of the abdominal wall trough minimally invasive surgery.

RACIONAL: As hérnias spigelianas são raros defeitos da parede abdominal. Apresentam quadro clínico pouco esclarecedor e com risco real de estrangulamento. RELATO DO CASO: Mulher de 55 anos, apresentava-se com massa endurecida e irredutível em fossa ilíaca esquerda além de aumento localizado da parede abdominal à direita e uma volumosa hérnia umbilical. A tomografia computadorizada de abdome mostrou duas hérnias spigelianas direita e esquerda, com conteúdo encarcerado. O tratamento foi herniorrafia seguida de colocação de tela. CONCLUSÃO: As hérnias de Spiegel caracterizam-se por serem raras e com diagnóstico muitas vezes de exclusão. Necessitam de tratamento cirúrgico pelo importante risco de estrangulamento do seu conteúdo.BACKGROUND: Spiegelian hernias are rare defects of the abdominal wall, which present a not much clarifying medical profile as well as the risk of strangulation. CASE REPORT: A fifty-five year old women presented herself with a hardened and irreducible mass in the left iliac foss, as well as local increase of the right abdominal wall and a voluminous umbilical hernia. Computerized tomography of the abdomen showed two spiegelian hernias, a right and a left, with incarcerated content. The treatment was herniorrhaphy followed by mesh placement. CONCLUSION: Spiegelian hernias are rare conditions, which are many times diagnosed using exclusion. These patients need surgical treatment due to the risk of content strangulation.

gastroschisis is a congenital structural defect of the abdominal wall, most often to the right of the umbilicus, through which the abdominal viscera protrude. Its developmental, etiological and epidemiological aspects have been a hot topic of controversy for a long time. However, recent findings suggest the involving of genetic and chromosomal alterations and the existence of a stress-inducing pathogenetic pathway, in which risk factors such as demographic and environmental ones can converge. To expand the frontier of knowledge about a malformation that has showed a growing global prevalence, we have conducted a review of the medical literature that gathers information on the embryonic development of the ventral body wall, the primitive intestine, and the ring-umbilical cord complex, as well as on the theories about its origin, pathogenesis and recent epidemiological evidence, for which we consulted bibliographic databases and standard search engines., Competing Interests: Conflict of interest: The author do not declare any conflict of interest, (Copyright © 2021 Colombia Medica.)

The use of robot-assisted minimally invasive surgery in ventral/incisional hernia repair has increased exponentially in recent years. This increase is probably related to the advantages of robotic surgery, among which are better visualization, the implementation of articulated instruments and better ergonomics for the surgeon. The TARUP (Robotic Transabdominal Retromuscular Umbilical Prosthetic Hernia Repair) technique combines the benefits of minimally invasive surgery, in terms of less wound-related morbidity, also allowing the placement of a mesh in a retromuscular position facilitated by the use of the robotic platform., (Copyright © 2020 AEC. The authors report a case of large postoperative umbilical hernia following laparoscopic cholecystectomy, which occurred in the early postoperative period despite primary suture of the trocar insertion site. Forced dilation of the fascial layer is proposed as the etiological mechanism. This procedure should be avoided and an accurate and aseptic removal of the gallbladder should be performed in order to prevent risk of abdominal wall weakness and visceral herniation.
