Abstract

Objective: This paper describes the Western Sydney University School of Medicine (WSUSoM) diversity education program, Medicine in Context (MiC). MiC implements community-engaged learning and partnership pedagogy in teaching diverse social determinants of health to first clinical year medical students. Central to MiC content and delivery methods is the local region’s diversity which is also reflected in the student population and MiC staff.Methodology: This is a descriptive report about how the WSUSoM staff with community and General Practice (GP) partners have co-designed, co-delivered, co-assessed and co-evaluated the MiC program in 2009-2018. In keeping with the community-engaged learning and partnership pedagogy, the report is co-authored by a cross section of MiC stakeholders: the WSUSoM staff members, community partners and an alumna.Results: Ten weeks' immersion in community-based services, with debriefing and scaffolding in tutorials and workshops, exposes students to the complex interplay between social determinants of health and clinical practice. Sharing of experiences, insights and reflections in safe environments enables students to overcome the uneasiness of diversity education. Quality assurance reviews identified positive trends in students’ quality of learning and satisfaction in the program following evidence-based continuous improvements of the program design and delivery. Conclusion: Implementation of community-engaged learning and partnership pedagogy in the MiC program, supported by ongoing commitment from the WSUSoM and its community and GP partners, has been successful in engaging students in diversity education. The synthesis of diversity education and clinical learning throughout the MiC program is an important step toward building competency in patient-centred care.

Abstract

Introduction: Hypertensive disorders of pregnancy (HDP) affect up to 10% of all pregnancies annually and are associated with an increased risk of maternal and fetal morbidity and mortality. This guideline represents an update of the Society of Obstetric Medicine of Australia and New Zealand (SOMANZ) guidelines for the management of hypertensive disorders of pregnancy 2014 and has been approved by the National Health and Medical Research Council (NHMRC) under section 14A of the National Health and Medical Research Council Act 1992. In approving the guideline recommendations, NHMRC considers that the guideline meets NHMRC's standard for clinical practice guidelines. Main recommendations: A total of 39 recommendations on screening, preventing, diagnosing and managing HDP, especially preeclampsia, are presented in this guideline. Recommendations are presented as either evidence‐based recommendations or practice points. Evidence‐based recommendations are presented with the strength of recommendation and quality of evidence. Practice points were generated where there was inadequate evidence to develop specific recommendations and are based on the expertise of the working group. Changes in management resulting from the guideline: This version of the SOMANZ guideline was developed in an academically robust and rigorous manner and includes recommendations on the use of combined first trimester screening to identify women at risk of developing preeclampsia, 14 pharmacological and two non‐pharmacological preventive interventions, clinical use of angiogenic biomarkers and the long term care of women who experience HDP. The guideline also includes six multilingual patient infographics which can be accessed through the main website of the guideline. All measures were taken to ensure that this guideline is applicable and relevant to clinicians and multicultural women in regional and metropolitan settings in Australia and New Zealand. Abstract

Cardiovascular disease is a major cause of mortality in women and is the leading cause of pregnancy‐related mortality in Australia. This study aims to discover the extent of teaching regarding women's cardiovascular health in Australian and New Zealand medical schools. All 22 medical schools in Australia and New Zealand were approached for participation in completing this survey. Seven medical schools (32%) completed the survey and demonstrated that within our sample population, findings suggest that while women's cardiovascular health is present in medical curricula, there is a large variability in the rigour and depth as to which it is taught, as well as possible lack of explicit teaching and examination regarding this topic. Abstract

Protein‐nanoparticle conjugation, or protein‐corona, is important for nanomedicine, nano diagnostic, and nano therapy applications. α‐Bismuth oxide nanoparticles (α‐Bi2O3 NPs) were prepared from bismuth nitrate powder using the sol‐gel method and characterized using various techniques, including Energy Dispersive X‐Ray Spectroscopy (EDS), X‐ray Diffraction (XRD), Raman spectroscopy, Scanning Electron Microscopy (SEM), Transmission Electron Microscopy (TEM), UV‐VIS‐NIR Spectroscopy, and Brunauer‐Emmett‐Teller (BET). X‐ray diffraction studies revealed that NPs are in the alpha phase with a monoclinic structure. The energy band gap, average surface area and crystallite size are 2.81 eV, 2.42 m2, and 15–106 nm, respectively. Transmission electron microscopy revealed particle size is 32.21 nm, with a d‐spacing of 2.529 Å. The surface‐to‐volume ratio is calculated and found to be 2.553×106 m−1. For the first time, a direct protein interaction with NPs was examined with standard proteins such as bovine serum albumin (BSA), carbonic anhydrase, phosphorylase b, and total human serum proteins and analyzed using mass spectroscopy. Proteomic analysis showed that proteins such as BSA, phosphorylase b, and carbonic anhydrase exhibited a strong affinity with Bi2O3 NPs. However, it was found that there was less or no association between Annexin A6, human serum albumin, Zinc finger protein, and Bestrophin‐2 with Bi2O3 NPs. [ABSTRACT FROM AUTHOR]
