1. Feasibility and Acceptability of a Parent-Toddler Programme to Support the Development of Executive Functions in Children at Elevated Likelihood of Autism or ADHD: Pilot Findings

Abstract

This study reports feasibility, fidelity and acceptability of a pilot of START; a 12-week parent-toddler, group-based, neurodiversity-affirming programme aiming to support executive function development in toddlers at elevated likelihood of autism or ADHD. After 4 days' training, community early years practitioner pairs delivered START to 13 UK families with a toddler showing elevated autistic traits, or with a parent or sibling with autism or ADHD, in groups of 6 and 7. Sessions were audio-recorded and rated by practitioners and researchers regarding the extent to which programme and session aims were met. Practitioners' reflections on strengths and challenges in session delivery, adaptations to the session plan and researchers' observations from the audio recordings were probed in weekly debrief calls, and one-to-one interviews at programme end-point. Recruitment and retention were monitored. Parent participants were asked to complete a feedback questionnaire after each session. Results show recruitment to the programme is feasible, but a large minority of parents experience barriers to regular attendance, which is a challenge for achieving exposure targets. Practitioners delivered the programme to a high quality and at least partially met programme and session-specific aims in every session. The most significant barrier to fully meeting session aims was families' late arrival. Parents reported regularly engaging with the suggested activities at home and found the sessions useful, although not all parents responded each week. Overall, the results of this small-scale pilot indicate START is feasible and acceptable as a parent-mediated programme to support toddlers at elevated likelihood of autism or ADHD to thrive.

2. Continuing Virtual Observations: A Situational Review of Student Perspectives

Abstract

During the COVID-19 pandemic, virtual observations were utilised in teacher education programmes internationally (Murtagh, 2022; Mynott et al., 2022; Ó Grádaigh et al., 2021). In Scotland, virtual observations have been continued as part of teacher education programmes and this article explores student perspectives on their continued use. A situative analysis (Clarke et al., 2018) was used to explore questionnaire and interview data from student teachers. The analytical process examined the data and utilised memoing to consider emergent themes against the situation of virtual observations in teacher education. There is a duality to the findings. On the one hand, students express preference for in-person observation when they consider the process to be assessment-focused. Conversely, when not considering virtual observations as assessment, the ability to reflect more deeply on their practice, increase their control and agency over the observation and reduce the stress surrounding observations are all themes that emerged from the data. Therefore, the data suggests that further innovation moving from summative to formative observations might increase the benefits of virtual observations. Limited research exists on virtual observations. The literature that is available often focuses on university staff. This article considers virtual observations from the student perspective and provides clear feedback on how a pandemic response has been developed for post-pandemic purposes. The findings of this article can be further explored and built upon, and this will enhance the use of virtual observations within teacher education.

3. Sustained Benefits of Early Childhood Education and Care (ECEC) for Young Children's Development during COVID-19

Abstract

Early childhood education and care (ECEC) settings faced significant disruption during the COVID-19 pandemic, compromising the continuity, stability and quality of provision. Three years on from the first UK lockdown as pandemic-era preschoolers enter formal schooling, stakeholders are concerned about the impact of the disruption on children's cognitive and socioemotional development, especially those from socioeconomically disadvantaged backgrounds. Using parent-report data from 171 children aged 5 to 23 months (M = 15 months) in March to June 2020 living in the UK, we investigate whether previously attested positive associations between ECEC attendance and the development of language and executive functions was maintained as early years settings navigated operational challenges over the first full year of the pandemic. In response to concerns about 'school readiness', we analyse the relationship between ECEC attendance and children's communication, problem-solving and personal-social development. ECEC was associated with greater growth in receptive vocabulary over the 12-month period. In children from less advantaged backgrounds, ECEC was also associated with greater growth in expressive vocabulary. Our data suggest a similarly positive association between ECEC attendance and the communication and problem-solving skills of children from less advantaged backgrounds and between ECEC and the personal-social development of all children. Overall, results suggest that ECEC had sustained learning benefits for children growing up during the pandemic despite ongoing disruption to settings, with specific benefits for children from less affluent home environments. As pandemic-era children progress to primary school, we discuss the importance of adapting their learning conditions and adjusting the expectations placed on them.

Abstract

Abstract Background Digital technology is ubiquitous in the lives of many children and parents. To better understand any influence of technology use by infants, and mothers, on child development, technology use should be considered within the broader family system context in which children develop. This study aimed to investigate associations between infant and maternal technology use and infant 12-month development, taking into consideration other family factors. Methods This cross-sectional study used data from ORIGINS participants, collected at 12-months of age: maternal and child technology use (TechU-Q), sociodemographic factors (e.g. child sex, household income), parental mental health (DASS-21), and child development (Ages and Stages Questionnaire). Linear regression was used for analyses. Results When family factors were considered, higher infant mobile touchscreen device (MTSD) use was associated with poorer infant development for gross motor, problem-solving, and total ASQ-3 scores. In contrast there were no associations between infant television (TV) watching or maternal technology use and total ASQ-3 scores. Higher maternal technology use was associated with higher infant technology use. Poorer maternal and paternal mental health were associated with poorer infant development. Poorer maternal mental health was also associated with higher infant TV watching and higher maternal MTSD use. Conclusion There is a complex relationship between technology use, parental mental health and other family factors that together influence infant development. To improve infant development outcomes, less focus should be on infant or maternal technology use, and more on supporting the family as a whole, and parental mental health in particular.

Abstract Aim To describe where clinical information is contemporarily and commonly found in UK primary care, what is favoured by clinicians, and whether this is (1) publicly funded (2) has commercial potential conflicts of interest. Design and setting A mixed methods study, consisting of (1) site visits to general practices in Scotland, (2) online questionnaire, focused on UK general practice (3) analysis of materials cited by professionals. Methods Data about sources of clinical information used was obtained verbally, visually and via search histories on computers from visits. This was used to inform a questionnaire in which primary care clinicians in the four nations of the UK were invited to participate. This obtained data about the information sources used and preferred by clinicians. This information was searched for data about funding and conflicts of interest. Results Over 2022, four practices were visited. 337 clinicians, 280 of whom were general practitioners completed an online questionnaire. 136 different resources were identified. These were mainly websites but sources of information included colleagues, either in practice or through online networks, apps, local guidelines, health charities, and learning resources aimed at GPs. Of these, 70 were not publicly funded, and were a mixture of membership organisations, charities, or sponsored venues. Conclusions Primary care clinicians obtain information for themselves and patients from a wide variety of sources. Funding is from a variety of sources and some contain advertising and/or sponsorship, risking commercial bias. Protocol Pre-published at https://osf.io/wrzqk .

Abstract

Abstract Background Cardiopulmonary resuscitation (CPR) is an emergency procedure performed to restore heart function to minimize anoxic injury to the brain following cardiac arrest. Despite the establishment of emergency department and training on Pediatric Advanced Life Support (PALS) at Muhimbili National Hospital (MNH) the outcomes of pediatric in-hospital cardiac arrest have not been documented. We ought to determine the outcomes and factors associated with 24-h survival after pediatric in-hospital cardiac arrests at MNH in Tanzania. Methods We conducted a retrospective study of all patients aged 1 month to 18 years who had in-hospital cardiac arrests (IHCA) prompting CPR in the Emergency Medicine Department (EMD) at MNH, Tanzania from January 2016 to December 2019. Data was collected from electronic medical record (Wellsoft) system using a standardized and pretested data collection form that recorded clinical baseline, pre-arrest, arrest, and post-arrest parameters. Bivariate and multivariable logistic regression analyses were performed to assess the influence of each factor on 24-h survival. Results A total of 11,951 critically ill patients were screened, and 257 (2.1%) had cardiac arrest at EMD. Among 136 patients enrolled, the median age was 1.5 years (interquartile range: 0.5–3 years) years, and the majority 108 (79.4%) aged ≤ 5 years, and 101 (74.3%) had been referred from peripheral hospitals. Overall stained return of spontaneous circulation was achieved in 70 (51.5%) patients, 24-h survival was attained in 43 (31.3%) of patients, and only 7 patients (5.2%) survived to hospital discharge. Factors independently associated with 24-h survival were CPR event during the day/evening (p = 0.033), duration of CPR ≤ 20 min (p = 0.000), reversible causes of cardiac arrest being identified (p = 0.001), and having assisted/mechanical ventilation after CPR (p = 0.002). Conclusion In our cohort of children with cardiac arrest, survival to hospital discharge was only 5%. Factors associated with 24-h survival were CPR events during the daytime, short duration of CPR, recognition of reversible causes of cardiac arrest, and receiving mechanical ventilation. Future studies should explore the detection of decompensation, the quality of CPR, and post-cardiac arrest care on the outcomes of IHCA.

Abstract

Abstract The absorption of terahertz (THz) radiation by water molecules facilitates its application to several biomedical applications such as cancer detection. Therefore, it is critical for the THz technologies to be characterised with water content in a sample. In this paper, we analyse gelatine phantoms in the THz frequency range, with continuously varying hydration levels as they dry over time. Water molecules in close proximity to the protein molecule, termed ‘bound water’, feature properties different from the ‘free water’ molecules at larger distances. We find that a common model for predicting electromagnetic properties of phantoms and tissue samples, which assumes that only the free water varies with hydration while the bound water remains constant, does not agree well with measured results. To gain insight into this behaviour, we simultaneously measured the phantom in Raman spectroscopy, which shows a continuously varying concentration of bound water with hydration level. It follows from this investigation, that the permittivity contributions of neither the biomolecules nor water are expected to be linear with water density. This means that the often used, simple effective medium model will not be accurate for many biological tissues or phantoms.

Abstract

The goal of the Multiperiod Degree Constrained Minimum Spanning Tree (MPDCMST) problem is to determine the smallest weight-spanning tree that satisfies the vertex installation criterion for each period and maintains the degree requirement in each vertex. This issue emerges as a network connection problem. The degree requirement indicates the reliability of each vertex, and the vertex connection/installation requirement denotes the priority vertices that must be inserted in the network within a specific time frame. The installation is split up into multiple phases/stages. This is because of various considerations such as severe weather, budgetary limitations, etc. In this research, two algorithms for solving the MPDCMST using probability hybridized with Prim’s modification, and edge analysis are proposed. The algorithms are implemented on the undirected complete graph of orders 10 to 100. The solutions are compared with some heuristics which are already in the literature. The results show that the proposed algorithms perform better.

Abstract

Abstract Persistence of malaria parasites in asymptomatic hosts is crucial in areas of seasonally-interrupted transmission, where P. falciparum bridges wet seasons months apart. During the dry season, infected erythrocytes exhibit extended circulation with reduced cytoadherence, increasing the risk of splenic clearance of infected cells and hindering parasitaemia increase. However, what determines parasite persistence for long periods of time remains unknown. Here, we investigated whether seasonality affects plasma composition so that P. falciparum can detect and adjust to changing serological cues; or if alternatively, parasite infection length dictates clinical presentation and persistency. Data from Malian children exposed to alternating ~6-month wet and dry seasons show that plasma composition is unrelated to time of year in non-infected children, and that carrying P. falciparum only minimally affects plasma constitution in asymptomatic hosts. Parasites persisting in the blood of asymptomatic children from the dry into the ensuing wet season rarely if ever appeared to cause malaria in their hosts as seasons changed. In vitro culture in the presence of plasma collected in the dry or the wet seasons did not affect parasite development, replication or host-cell remodelling. The absence of a parasite-encoded sensing mechanism was further supported by the observation of similar features in P. falciparum persisting asymptomatically in the dry season and parasites in age- and sex-matched asymptomatic children in the wet season. Conversely, we show that P. falciparum clones transmitted early in the wet season had lower chance of surviving until the end of the following dry season, contrasting with a higher likelihood of survival of clones transmitted towards the end of the wet season, allowing for the re-initiation of transmission. We propose that the decreased virulence observed in persisting parasites during the dry season is not due to the parasites sensing ability, nor is it linked to a decreased capacity for parasite replication but rather a consequence decreased cytoadhesion associated with infection length.

Abstract

Aim: The aim of this study was to assess the accuracy of fine needle aspiration biopsy (FNAB) as a diagnostic tool for thyroid cancer in Sanjiwani Hospital, Gianyar. Methods: This study used a diagnostic test method with a retrospective cross-sectional design. A total of 138 cases of thyroid nodules that received FNAB and anatomical pathology examinations were selected consecutively. The level of accuracy was determined from the number of sensitivity, specificity, positive predictive value and negative predictive value. Results: The results of this study showed a low sensitivity 11.53%, but high specificity 100%, positive predictive value 100%, and negative predictive value 82.96% of FNAB. The accuracy of FNAB as diagnostic tools of thyroid cancer was 83,3% compared with anatomical pathology results as gold standard. Conclusion: FNAB is less accurate to diagnose thyroid cancer, but FNAB examination is accurate in differentiate benign and cancer lesion in thyroid nodule.

22. Rapid terahertz beam profiling and antenna characterization with phase-shifting holography

Abstract

Abstract In this paper we investigate the application of phase-shifting digital holography for the real-time characterization of electromagnetic sources in the THz frequency range. We use an off-the-shelf terahertz detector array composed of $$64 \times 64$$ 64 × 64 power-sensitive pixels, over an area of $$96\,\hbox {mm} \times 96\,\hbox {mm}$$ 96 mm × 96 mm , to record intensity interferograms cast between the coherent radiation emitted from a reference source and an unknown antenna under test. This approach parallelizes the acquisition process with respect to conventional near-field point scanning methods, reducing the measurement time by orders of magnitude. In our system, the measurement time is limited only by the refresh rate of the detector array and the speed of a delay line stage that is used to phase-shift the reference wave. As a proof-of-principle demonstration, we map the 2D near-field distribution and estimate the far-field radiation pattern emitted by a plano-convex PTFE spherical lens antenna illuminated by a diagonal horn at 290 GHz frequency with $$\sim \,1\,\hbox {Hz}$$ ∼ 1 Hz refresh rate.

In contrast to the field's predominant focus on teacher-led talk in reading contexts, this study considers child-led relaxed conversations about texts. It examines what is afforded by the occurrence of such relaxed book talk and how this is enabled with young children in school. Data are drawn from a co-participative study with four teachers in England that captured naturalistic video observations. The paper offers novel insights about the social and personal affordances of informal book talk, the role it can play in developing positive dispositions towards reading and the salience of familiar texts and the classroom reading culture.

24. Reflecting on Experiences of Learning among Adult Learners with Multiple Responsibilities: A Case of Evening Programmes at a University in Tanzania

Abstract

The focus of this paper is to report the findings of a qualitative study whose purpose was to analyse the experiences of adult learners pursuing evening postgraduate degree programmes at the university level in Tanzania. Using ethnographic research combined with multiple case research design, together with in-depth interviews and documentary review, data were generated from 20 postgraduate students from one school within a university in Tanzania. The resulting qualitative data were subjected to thematic analysis. The study found that university adult learners in these evening programmes combined learning with their full-time employment, family, and social responsibilities as well as extra income-generating activities. Inevitably, these multiple responsibilities, as the study established, adversely affected their academic performance and duration for programme completion. In fact, completion on a part-time basis depended on dispositional factors such as individual learner's efforts and strategies adopted to cope with the situation, with many others either delaying completion or dropping out altogether. This scenario invites rethinking the modus operandi of programmes to facilitate postgraduate students' completion rates amid widening participation in higher education.

Abstract

Background: Chronic Kidney Disease (CKD) patients undergoing hemodialysis usually undergo anemia condition. Calculation of hemoglobin and other hematological parameters such as Ret-he (Reticulocyte Hemoglobin) and reticulocytes along with Glomerulus Filtration Rate (GFR) calculations determine the stage of CKD. Erythropoietin (EPO) administration is one of the important therapies for CKD patients. Initiation of EPO therapy, according to Kidney Disease Improving Global Outcome (KDIGO). Method: This study was an observational cross-sectional analytical study. This research was carried out from October 2023 to November 2023 at the Hemodialysis Installation of Dr. RSUD. R Sosodoro Djatikoesoemo Bojonegoro. Normality of the data using the Shapiro-Wilk test and differences in Ret-he levels, reticulocyte counts, and hemoglobin levels pre and post-administration of erythropoietin using paired t-test was used if the data were normally distributed and the Wilcoxon test if the data were not normally distributed. Results: This study results showed that there was an increase in rethe levels pre and post-administration of erythropoietin with pre mean value of 30.50 ± 4.043 and post mean value was 32.20 ± 2.936. Reticulocyte levels also increase pre and post-administration of erythropoietin with a mean value of 1.63 ± 0.869 for pre-administration and 2.04 ± 0.915 for post-administration. Hemoglobin levels before giving erythropoietin with a value of 8.21 ± 0.855 and after giving erythropoietin the value was slightly increased to 8.29 ± 0.853. Rethe and reticulocytes were significant differences between pre and post-EPO administration with p=0.000 and hemoglobin showed that there are no significant difference values pre and post-EPO with p=0.373. Conclusion: There is a significant difference (p-value 0.000) between Ret-He and Reticulocyte levels pre and post-erythropoietin treatment and there is no significant difference (p-value 0.373) Hemoglobin levels pre and post-erythropoietin treatment.

Abstract Various histopathological, clinical and imaging parameters have been evaluated to identify a subset of women diagnosed with lesions with uncertain malignant potential (B3 or BIRADS 3/4A lesions) who could safely be observed rather than being treated with surgical excision, with little impact on clinical practice. The primary reason for surgery is to rule out an upgrade to either ductal carcinoma in situ or invasive breast cancer, which occurs in up to 30% of patients. We hypothesised that the stromal immune microenvironment could indicate the presence of carcinoma associated with a ductal B3 lesion and that this could be detected in biopsies by counting lymphocytes as a predictive biomarker for upgrade. Abstract

Abstract Camera trapping has revolutionized wildlife ecology and conservation by providing automated data acquisition, leading to the accumulation of massive amounts of camera trap data worldwide. Although management and processing of camera trap‐derived Big Data are becoming increasingly solvable with the help of scalable cyber‐infrastructures, harmonization and exchange of the data remain limited, hindering its full potential. There is currently no widely accepted standard for exchanging camera trap data. The only existing proposal, “Camera Trap Metadata Standard” (CTMS), has several technical shortcomings and limited adoption. We present a new data exchange format, the Camera Trap Data Package (Camtrap DP), designed to allow users to easily exchange, harmonize and archive camera trap data at local to global scales. Camtrap DP structures camera trap data in a simple yet flexible data model consisting of three tables (Deployments, Media and Observations) that supports a wide range of camera deployment designs, classification techniques (e.g., human and AI, media‐based and event‐based) and analytical use cases, from compiling species occurrence data through distribution, occupancy and activity modeling to density estimation. The format further achieves interoperability by building upon existing standards, Frictionless Data Package in particular, which is supported by a suite of open software tools to read and validate data. Camtrap DP is the consensus of a long, in‐depth, consultation and outreach process with standard and software developers, the main existing camera trap data management platforms, major players in the field of camera trapping and the Global Biodiversity Information Facility (GBIF). Under the umbrella of the Biodiversity Information Standards (TDWG), Camtrap DP has been developed openly, collaboratively and with version control from the start. We encourage camera trapping users and developers to join the discussion and contribute to the further development and adoption of this standard.

38. 'If I Do Well I Feel on Top of the World': Investigating the Impact of Psychology Students' Academic Achievement on Self-Esteem

Abstract

Academic achievement can be thought of as the extent to which learning has been accomplished. Previous work suggests that students' self-esteem is impacted by academic achievement, though it is not entirely clear why this is. The current study therefore explored this through interviewing eight psychology students from a Scottish university, before using Braun and Clarke's (2006) six stage reflexive thematic analysis to analyse the resultant data. Two themes of Grades and feedback and Peer comparison were identified, highlighting two important conclusions. Firstly, self-esteem is positively impacted by strong performance, and negatively impacted by poor performance but that crucially, this is mediated by others' poor performance. That is to say, good grades and feedback make students feel good, and poor grades and feedback make students feel bad, but such 'badness' is appeased if peers perform equally poorly, which has potential consequences for peer relationships. Secondly, students value feedback in order to increase their chances of academic achievement, noting the negative impact on self-esteem when feedback is not constructive, just critical. This highlights the importance of 'mindful marking' in terms of taking care to provide useful feedback to students to improve their work; something that is reported to be hugely appreciated. Implications, limitations and future research suggestions are detailed.

39. More Capable Others? Education Studies Undergraduates as Near-Peer Mentors for Year 12 Students

Abstract

This research focused on the benefits and challenges for second year Education Studies undergraduate students acting as near-peer mentors for year 12 students (aged 16-17). Near-peer mentoring often involves postgraduate students working with new undergraduates and is more common during Science Technology Engineering and Maths (STEM) programmes. There is little research available, outside of STEM, about how near-peer mentors might influence the aspirations of students to access HE, or of the impact on undergraduate mentors themselves. The student near-peer mentors were part of the Research Higher project in an area of England with unexplained low participation rates in HE. The project involved circa 200 students from local schools in a programme of weekly events where they designed and conducted their own research. Second year Education Studies undergraduate students supported two cohorts of year 12 students during their on-campus seminar activities. The findings draw on thematic analysis of undergraduate student interviews at the end of the project. They indicate that near-peer mentoring has unexpected benefits for undergraduate Education Studies students including meta-cognition about their own learning and confidence in working with older students. Recommendations for future near-peer mentoring programmes are proposed based on student feedback.

Background: UK cancer deaths remain high; primary care is key for earlier cancer diagnosis as half of avoidable delays occur here. Improvement is possible through lower referral thresholds, better guideline adherence, and better safety-netting systems. Few interventions target whole practice teams. We developed a novel whole-practice team intervention to address this. Aim: To test the feasibility and acceptability of a novel, complex behavioural intervention, ‘ThinkCancer!’, for assessment in a subsequent Phase III trial. Design & setting: Pragmatic, superiority pilot randomised controlled trial (RCT) with an embedded process evaluation and feasibility economic analysis in Welsh general practices. Method: Clinical outcome data were collected from practices (the unit of randomisation). Practice characteristics and cancer safety-netting systems were assessed. Individual practice staff completed evaluation and feedback forms and qualitative interviews. The intervention was adapted and refined. Results: Trial recruitment and workshop deliveries took place between March 2020 and May 2021. Trial progression criteria for recruitment, intervention fidelity, and routine data collection were met. Staff-level fidelity, retention, and individual level data collection processes were reviewed and amended. Interviews highlighted positive participant views on all aspects of the intervention. All practices set out to liberalise referral thresholds appropriately, implement guidelines, and address safety-netting plans in detail. Conclusion: ‘ThinkCancer!’ appears feasible and acceptable. The new iteration of the workshops was completed and the Phase III trial has been funded to assess the effectiveness and cost-effectiveness of this novel professional behaviour change intervention. Delivery at scale to multiple practices will likely improve fidelity and reach.

Abstract

ABSTRACT Biological invasions have caused the loss of freshwater biodiversity worldwide. The interplay between adaptive responses and demographic characteristics of populations impacted by invasions is expected to be important for their resilience, but the interaction between these factors is poorly understood. The freshwater gastropod Amnicola limosus is native to the Upper St. Lawrence River and distributed along a water calcium concentration gradient within which high‐calcium habitats are impacted by an invasive predator fish (Neogobius melanostomus, round goby), whereas low‐calcium habitats provide refuges for the gastropods from the invasive predator. Our objectives were to (1) test for adaptation of A. limosus to the invasive predator and the low‐calcium habitats, and (2) investigate if migrant gastropods could move from refuge populations to declining invaded populations (i.e., demographic rescue), which could also help maintain genetic diversity through gene flow (i.e., genetic rescue). We conducted a laboratory reciprocal transplant of wild F0 A. limosus sourced from the two habitat types (high calcium/invaded and low calcium/refuge) to measure adult survival and fecundity in home and transplant treatments of water calcium concentration (low/high) and round goby cue (present/absent). We then applied pooled whole‐genome sequencing of 12 gastropod populations from across the calcium/invasion gradient. We identified patterns of life‐history traits and genetic differentiation across the habitats that are consistent with local adaptation to low‐calcium concentrations in refuge populations and to round goby predation in invaded populations. We also detected restricted gene flow from the low‐calcium refugia towards high‐calcium invaded populations, implying that the potential for demographic and genetic rescue is limited by natural dispersal. Our study highlights the importance of considering the potentially conflicting effects of local adaptation and gene flow for the resilience of populations coping with invasive predators.

Abstract

Objectives and importance of the study: Pharmacist-administered vaccination has expanded in Australia but has not been comprehensively assessed. We aimed to assess the pharmacists’ role in vaccination in Australia before and during the first year of the COVID-19 pandemic, as well as the completeness of data on pharmacist-administered immunisations. Study type: Cross-sectional study. Methods: We analysed data on pharmacist-administered vaccinations that were reported to the Australian Immunisation Register (AIR) between 2016 and 2019, categorised by gender, jurisdiction, age group and vaccine type. We conducted a national survey of community pharmacists providing vaccination services during June and July 2020 to understand how pharmacists record and report vaccinations to the AIR. We assessed data completeness by comparing the number of vaccinations reported by surveyed pharmacists to the number recorded on the AIR. Results: 576 780 pharmacist-administered vaccinations were recorded on the AIR between 2016 and 2019, of which 94.7% were influenza vaccines. Abstract

In an attempt to accelerate the energy transition, the government of Indonesia released a new regulation regarding renewable energy tariffs, including for biomass power plants (BPPs). However, a gap persists in the academic literature focusing on biomass power plants under these new tariffs. This study examines the role of biomass power plants (BPP) in meeting Indonesia's renewable energy targets, with a specific focus on recently introduced renewable energy tariffs. Three scenarios were developed to comprehensively understand Indonesia's state-owned electricity company (PLN) perspective and Independent Power Producers (IPPs) perspective, with a case study in Kundur Island, Riau Province. The financial analysis for IPPs shows in an NPV of $1.8 million under the initial tariff, while the expansion tariff results in an NPV of -$0.3 million. Sensitivity analysis reveals that a capacity factor exceeding 94.5 % is required to yield a positive NPV under the expansion tariff. From PLN's perspective, BPPs under the initial tariff correspond to an NPV of $8.48 million, and a B-C ratio of 2.31, resulting in a financially viable project, due to substantial cost savings from de-dieselization. Furthermore, the emission analysis highlights an 83.7 % decrease in emissions emitted with the utilization of BPPs compared to diesel power plants. These findings underscore the need to incorporate average generation cost in formulating renewable energy tariffs. Ensuring renewable energy projects are economically sustainable for both PLN and IPPs, stimulates widespread adoption and sustains cost-savings, all while advancing energy transition and making energy more accessible for everyone.

Abstract

Introduction: Patients in the Intensive Cardiovascular Care Unit (ICCU) often present with cardiovascular disease (CVD) issues accompanied by various non-cardiovascular conditions. However, a widely applicable scoring system to predict patient outcomes in the ICCU is lacking. Therefore, developing and validating scores for predicting ICCU patient outcomes are warranted. The aims of the IndONEsia ICCU (One ICCU) registry include developing an epidemiological registry of ICCU patients and establishing a multicentre research network to analyse patient outcomes. Methods and results: This nationwide multicenter cohort protocol will capture data from patients receiving cardiovascular critical care treatment in 10 Indonesian hospitals with ICCU facilities. Recorded data will encompass demographic characteristics, physical examination findings at hospital and ICCU admission, diagnoses at ICCU admission, therapy, intervention, complications on days 3 and 5 of in-ICCU care, in-hospital outcomes, and 30-day outcomes. Conclusion: The One ICCU is a large, prospective registry describing the care process and advancing clinical knowledge in ICCU patients. It will serve as an investigational platform for predicting the mortality of ICCU patients.

Abstract

The tests revealed the exceptional combustion properties of a blended fuel consisting of castor oil and rhodium liquid (Rhl) as a highly effective homogeneous combustion catalyst. Our findings indicate that castor oil's unique molecular structure makes it an ideal fuel component, and the catalyst interacts with the fuel's triglycerides to enhance fuel properties and facilitate ignition.These findings support the pivotal role of the synthetic catalyst Rhl, which effectively reduces the binding forces within the triglyceride chain through polarization interactions. As a result, molecular bonds become more flexible, providing electrons with greater freedom of movement. Synthetic catalysts induce significant modifications in the triglyceride structure, increasing electron energy levels and enhancing the reactivity of fuel molecules, ultimately leading to improved fuel combustion efficiency. Integrating the Rhl synthetic catalyst also enhances fuel performance by reducing ignition duration and increasing the combustion rate. The elevated combustion temperatures of the fuel droplets highlight the effectiveness of promoting environmentally sustainable combustion processes.

Abstract

Background: Stroke is one of the leading causes of disability worldwide. In the next few decades, post-stroke disability is expected to increase by up to 35% due to the current increase in cerebrovascular risk factors. Disabilities caused by stroke can include cognitive and motor impairment. The current research on post-stroke rehabilitation mostly focuses on the motor disability aspect. Recently, the International Stroke Recovery and Rehabilitation Alliance 2018 stated that post-stroke cognitive impairment (PSCI) is one of the research priorities. Following recent technological developments, virtual reality (VR)-based therapy is now emerging. This technology is attractive because it can simulate various activities in a safe environment. However, research on VR for post-stroke patients still does not have specific guidelines that must be followed, so the types of interventions carried out through VR still vary. Therefore, this study aims to assess how the benefits of using VR in improving cognitive function in post-stroke patients and what VR interventions show the greatest benefits. Method: This research is a systematic review and meta-analysis conducted based on Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) 2020 guidelines. The data used for this study were obtained from PubMed, Scopus, and Science Direct databases. The search conducted in this systematic review was done without any year restriction. The studies included were randomized controlled trials (RCTs) and pilot RCTs. Results: This systematic review included eight studies involving 313 participants in total. All studies were used for the meta-analysis, with five, four, and six studies used in the rote memory function, executive function, and general cognitive analyses, respectively. Assessment using the trail making test (TMT) on the TMT-A test related to rote memory function (MD = -26.43, 95% CI -43.18 – -9.68, P = 0.002) and TMT-B test related to executive function (MD = -59.23, 95% CI -103.21 – -15.25, P = 0.008) showed significantly better results with shorter processing time in the VR group. In contrast, in the general cognitive assessment using the Montreal cognitive assessment (MoCA), although it showed better results in the VR group (MD = 1.45, 95% CI -0.20 – 3.09, P = 0.08), the results were not significant. The meta-analysis showed homogeneous result in rote memory (I2 = 6%) and executive function (I2 = 0%) analyses. For general cognitive analysis the result was heterogeneous (I2 = 86%). Conclusion: Interventions using VR have shown promising results in improving rote memory and executive function, especially when VR intervention is combined with a motorized assistive devices. This combination therapy not only trains cognitive skills, but also trains the patient's motor skills simultaneously. For general cognitive interventions, adaptive VR with task generators that represent daily activities showed the best results, but studies using this technology only focused on cognitive interventions. A new trial that uses the combination of these two technologies would be interesting to investigate further, especially considering that the existing trials still use small populations.
