International audience; FFP2 respiratory protective devices for health professionals in the context of the fight against the spread of the SARS-CoV-2 virus in relation to the emergence of the Omicron variant (supplement)The French High Council for Public Health or HCSP is examining strategies for indicating the use of FFP2-type respirators in health care settings as part of the fight against the spread of SARS-CoV-2 in the context of the emergence of the Omicron variant.The HCSP takes into account that :The effectiveness and protective performance (filtration) of masks, whatever they may be, are closely dependent on the consistency of the wear, the fit to the face and the quality of the wear covering the nose, mouth and chin;The mask alone cannot stop completely the risk of transmission; it is one of the protective measures to be respected (vaccination, hand hygiene, ventilation of premises, etc.).The HCSP stresses the strong uncertainties and lack of scientific data on several important aspects of the analysis of the benefit/risk ratio in favour of extending the use of FFP2-type respirators to healthcare professionals in comparison with medical masks (so-called "surgical" masks) outside specific situations.FFP2 type respirators are indicated when performing invasive procedures or manoeuvres in the respiratory or ENT sphere that generate aerosols.In addition to the standard precautions and additional hygiene precautions specific to the management of suspected or confirmed Covid-19 patients, and subject to compliance with the conditions for wearing these respirators, and following the advice of the occupational health service, the HCSP allows the following possibilitiesWearing an FFP2-type respirator to healthcare professionals at risk of severe forms of Covid-19 or who have failed to be vaccinated and who take charge of a patient suspected or confirmed of SARS-CoV-2 infection;A change to routine and transitional use of FFP2-type respirators for all healthcare workers in contact with patients in an area with a complex, progressive and uncontrolled cluster of Covid-19 cases among healthcare workers or patients/residents.All these recommendations must be accompanied by specific training on the characteristics, performance and correct wearing of FFP2-type respirators, in particular the adaptation to the size and morphology of the face for the persons concerned.Lire en français :Appareils de protection respiratoire de type FFP2 par les professionnels de santé dans le cadre de la lutte contre la propagation du virus SARS-CoV-2 en rapport avec l'émergence du variant Omicron (complément) du 7 janvier 2022

Cette recommandation doit s’accompagner de formations spécifiques sur les caractéristiques, les performances et le port correct du masque FFP2 notamment l’adaptation à la taille et à la morphologie du visage pour les personnes concernées.En milieu scolaire et universitaire, le HCSP recommande le port de masque grand public en tissu UNS1 réutilisable (masque validé et certifié de performance de filtration équivalente au masque à usage médical, par la Direction générale de l’Armement) ou à usage médical (chirurgical) à usage unique pour les enseignants et les équipes pédagogiques au contact des enfants et étudiants. Il est fondamental de rappeler l’importance du port correct d’un masque quel qu’il soit, en particulier dans cette période épidémique.Le HCSP souligne qu’il parait difficilement envisageable de proposer le port d’un APR de type FFP2 aux enseignants, du fait de son inconfort sur la durée avec gêne respiratoire et du risque de perte de ses performances de filtration attendues lors de la parole et des mouvements. De plus, il paraît illusoire d’organiser le contrôle et le respect des conditions d’utilisation optimale d’un tel masque, notamment la vérification de l’ajustement au visage lié au modèle et à la taille d’APR de type FFP2 mis à disposition. According to the European Centre for Disease Control (ECDC) and recent literature, wearing masks is not only efficient in reducing the spread of the virus through respiratory secretions, but also protects people who wear them correctly from Covid-19. Although mask wearing is normalizing in enclosed environments and outdoor with high densities of people, the HCSP recalls that a physical distance of a least 1 meter between people remains paramount and cannot be abandoned.The HCSP encourages reusable masks to reduce waste and pollution. It recommends to promote the use of reusable masks (in accordance with Afnor recommendations), by facilitating the access to masks that can be reused up to 50 times, with constant and validated breathability and filtration performance, and by simplifying the maintenance conditions. Simple educational information through positive and updated messages should be diffused within public establishments when education and professional activities will resume after summer. The HCSP recommends the use of a reusable fabric or clothe mask for the general public, according to the Afnor S76-001 specifications and preferably category 1 (Non sanitary use, UNS1), in public and private indoor spaces and particularly for professionals. The HCSP recommends to read the composition label before using a reusable mask, and to follow the manufacturer’s recommendations, in particular to wash it before the first use and after each use, to wearing it for 4 hours maximum and to replace the masks as soon as they are wet.Clothe masks remain more protective than no mask, without reaching the level of protection of standardized ones. Thus, it is recommended to wear category 2 clothe masks if category 1 clothe masks or equivalent foreign standards are not available, for the general population in public establishments.In this period of very active circulation of the SARS-CoV-2 virus and as a precaution, the HCSP recommends children to wear a suitable mask by children from the age of 6 years in primary schools (from 1st grade to firth grade) is recommended, while respecting specific conditions, such as behavior disabilities.The HCSP specifies that there are no currently documented dermatological, pneumological, ENT, phoniatric or psychiatric contraindications to wear a mask, whatever its type. In the event of skin irritation, the HCSP recommends to seek medical attention to ensure appropriate dermatological treatment.The HCSP issues other recommendations on mask maintenance for the general public, information, communication and promotion of mask. Transmission occurs mainly from adult to adult and from adult to child but rarely from child to child or from child to adult. Contaminations occur mainly within families or during social gatherings with a high density of people, outside schools. The wearing of masks by adults in primary school with children under 11 is mainly intended to protect children from contamination by asymptomatic adults who carry the virus. Air renewal should be ensured within the rooms on a regular basis, through the supply of fresh air which should, be increased if possible.Besides, the continuous measurement of carbon dioxide (CO2) concentration by the means of sensors allows to assess the air quality.The HCSP recommends the population occupancy gauge to be strictly limited to what the actual flow of fresh air input allows, while respecting physical distancing between peoples. It suggests to maintain forced-air heaters and mechanical ventilation systems in continuous operation. The HCSP recommends to check that no obstacle impedes the proper functioning of air diffusion within the rooms, and to avoid air recycling through the centralized air handling system in order to prevent the possible transfer of viral aerosols between different rooms.The HCSP emphasizes the need to open windows for a few minutes several times a day to further increase the level of air exchange in the rooms. The present document completes these recommendations. It does not concern the healthcare settings.The HCSP reviewed the current state of knowledge on respiratory excretion of the virus as well as available modelling studies and experiments on airborne transmission. The review of the scientific literature that report contamination cases in enclosed public spaces (restaurants, public transports, cruise ships, choir rehearsals, etc.) and professional environments (slaughterhouses, etc.) stresses three conditions favourable to the airborne transmission of the SARS-CoV-2 virus: ventilation and air flow conditions, atmospheric conditions (low temperature, humidity) and the activities and physical efforts performed in these spaces.The HCSP recommends that the general population systematically wear preferably a clothe mask, made of reusable fabric (standard: AFNOR S76-001), in all enclosed public and private spaces and during outdoor gatherings with high densities of people in order to limit the emission of respiratory particles. The current pandemic caused by COVID-19 has underlined the impor-tance of a joint effort and approach to ensure patient and health care worker safety in medical care throughout Europe. In addition, the recent flood disasters in Germany and other countries called for immediate joint action, in this case with regard to the preven-tion of water-borne infections. Environmental disasters will increase with consequences for hospitals and nursing homes. Cooperative efforts are needed for preventing and controlling associated infection outbreaks, new pathogens will appear and a geographic shift of infectious diseases previously not detected in certain areas has already been observed. This approach to infection prevention and control must entail structural as well as regulatory aspects. The principle of equal protection against infections in all European countries must be implemented. Prevention and control of infections, including nosocomial infections, infections caused by antibiotic-resistant bacteria as well as pandemics, need to be based on equal standards in all of Europe. Protection against infections and other public health risks in all European countries is the best guarantor for building trust and identi-fication of citizens in our common Europe. Experts in the fields of hygiene, microbiology, infectiology and epidemi-ology have to pool the expertise on the prevention and control of infec-tions from different European countries and define key targets for achieving a high standard of hygiene measures throughout Europe. The participants of the Rudolf Scholke Foundation International Symposium call for immediate action and priority to be given to the realization of the proposed 16-point plan.

This control of ventilation in the PAB must be a strong point on which a health protocol can be based, all the more so when certain barrier measures, in particular masks or inter-individual distance, are not adapted to the activity of a re-opened PAB (e.g. restaurants, nursery schools, etc.).The HCSP recommends ventilating closed spaces in public buildings when people are present and opening windows for at least 5 minutes every hour. In schools and universities in particular, it is proposed to leave the doors and windows open between classes and lessons (cross ventilation). Only the windows should be left open during lessons or lectures (ideally two windows should be opened, if possible, to promote air circulation).In situations where ventilation is currently not possible or insufficient (e.g. blocked windows), technical solutions should be implemented (e.g. flaps or ventilators in the upper part of the windows, air supply ventilation, etc.). Reducing the occupancy rate of the premises can then contribute to achieving satisfactory air quality.The HCSP indicates that the air change rate can be easily approximated by measuring the carbon dioxide concentration in the air at representative points and periods during occupancy. A CO2 concentration above a threshold of 800 ppm should in all cases lead to the room not being occupied and to action being taken in terms of ventilation/air renewal and/or reducing the number of people admitted to the premises of an ERP.Finally, the HCSP recommends that measurement campaigns be carried out in the various PAB (e.g. schools and universities, cultural places, sports facilities, etc.) to identify areas at risk (e.g. certain rooms, corridors and toilets)

Wearing a mask is the only effective measure when a physical distance of at least 1 meter is not guaranteed. Protective visor alone is not sufficient to control the risk.The HCSP stresses that the distance between the workstations, their separation by screens as well as air renewal are essential elements to be taken into account for better risk control. It recommends employees to systematically wear a mask when moving across the company’s rooms and to wash their hands whenever they change work areas. It stresses that a symptomatic employee should not go to work. If employees become symptomatic at the workplace, they should self-isolate then undergo a diagnostic PCR test. Aim. The aim of this narrative review was to explore the potential contributions of CAM to reduce antibiotic use. Methods. We searched PubMed, Embase, and Cochrane Database of Systematic Reviews with a specific, limited set of search terms and collected input from a group of expert CAM researchers to answer the question: What is known about the contribution of CAM health and health promotion concepts, infection prevention, and infection treatment strategies to reduce antibiotic use? Results. The worldview-related CAM health concepts enable health promotion oriented infection prevention and treatment aimed at strengthening or supporting the self-regulating ability of the human organism to cope with diseases. There is some evidence that the CAM concepts of health (promotion) are in agreement with current conceptualization of health and that doctors who practice both CAM and conventional medicine prescribe less antibiotics, although selection bias of the presented studies cannot be ruled out. There is some evidence that prevention and some treatment strategies are effective and safe. Many CAM treatment strategies are promising but overall lack high quality evidence. Conclusions. CAM prevention and treatment strategies may contribute to reducing antibiotic use, but more rigorous research is necessary to provide high quality evidence of (cost-)effectiveness.
