1. Psychiatrists’ Readiness for Digital Psychiatry in Pakistan: A Multicenter CrossSectional Study with Regression Analysis

Abstract

Introduction The concept of digital psychiatry, encompassing technologies such as mental health apps, Virtual Reality (VR), Artificial Intelligence (AI), and telepsychiatry, emerges as a potential solution to bridge the existing gaps in the mental health system of Pakistan. However, one of the major barriers to the implementation of these technologies is hesitancy to adopt digital tools by psychiatrists. Objectives This study aims to explore the current understanding of digital psychiatry, the barriers faced by psychiatrists in its’ widespread implementation, and their willingness to adopt these services in clinical practice. Methods This cross-sectional study surveyed psychiatrists’ knowledge, attitudes, and practices on digital psychiatry from 39 public hospitals across Pakistan using an online validated questionnaire from January to July 2023. Participants included psychiatry residents, fellows, and consultants practicing in Pakistan. Responses were analyzed with Raosoft software, Quirkos, and SPSS 26 using thematic analysis and correlation. Results A total of 200 participants responded to the questionnaire, primarily in the age range of 20-30 years (56%). The gender distribution was 55% male (N = 111) and 45% female (N = 89). Among the professional roles, 23% were consultants, 7% were registrars, 54% were psychiatry residents, and 17% were medical officers. Respondents came from both rural (N = 148, 74%) and urban (N = 52, 26%) practice settings. Regarding telepsychiatry, 46% strongly agreed that they are familiar with telepsychiatry, while 58% agreed that telepsychiatry can save time and money. Additionally, 22% strongly agreed that it’s a viable approach for patient care. Concerning perspectives on Artificial Intelligence (AI) in digital psychiatry readiness, only 40% of participants had received AI training. However, 55% expressed interest in collaborating with international centers on AI-related projects. In terms of mental health apps, 62% of respondents reported limited familiarity with them. Nevertheless, 65% believed that these apps could potentially save time and money for psychiatric health systems. Lastly, concerning Virtual Reality (VR) in psychiatric care, 57% of participants were familiar with VR technology, but only 43% were acquainted with its applications in psychiatry. Notably, 71% did not view VR as a viable replacement for in-person psychiatric management. Conclusions This is the first study conducted on understanding digital psychiatry in Pakistan’s healthcare system, which revealed multiple challenges to digital health competency among psychiatrists. 3. Assessment of Antibiotics Resistance pattern of Pseudomonas aeruginosa Isolated from Patients Admitted in selected Hospitals in Kebbi State, Nigeria

Abstract

Pseudomonas aeruginosa is one of the ESKAPE (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter spp) that induce a dignified healthcare aggravation among hospital admitted patients. This results in prolonged hospital stays, which increase costs for healthcare providers and patients' families. Therefore, this study aimed to isolate Pseudomonas aeruginosa and evaluate its resistance patterns from patients admitted to selected hospitals in Kebbi State. A total of 185 clinical samples, including nasal intubation, urine catheters, and wound swabs, were obtained. The bacteria were isolated and identified following standard microbiological methods. Modified Kirby Bauer techniques was used to determine the susceptibility status of the isolates. Out of 185 clinical samples collected, 43 (23.2%) bacterial isolates yield positive and 13 (30.2%) of which were P. aeruginosa from the studied hospitals. Prevalence of P. aeruginosa was found to be higher among females 08 (61.5%). The age groups 6-11 years had the highest prevalence P. aeruginosa 07 (53.8%). P. aeruginosa was isolated most from wound swab samples, 07 (24.1%). The Pseudomonas aeruginosa isolates exhibit high level resistance (100%) to Amoxicillin/Clavulanic acid, Cefpodoxime, Cefepime, Cepotaxime and Meropenem and showed least resistant to Imepenem 05 (38.4%). The increasing resistance of Pseudomonas aeruginosa isolates to multiple antimicrobial agents that are currently considered as first- line agents for the treatment of Pseudomonas aeruginosa infections, this highlights the need for careful use of these agents and also suggests the need for careful and up-to-date monitoring of multidrug- resistant strains diffusion in the various health care facilities of the country. Treatment options should be guided by medical laboratory scientist via microscopy culture and sensitivity testing, as well as local epidemiological surveillance data.

7. Entrepreneurial Propensity in Pakistan and UK: A comparative study of Pakistani and UK Prospective Teachers

Abstract

This research compares entrepreneurial inclination of Pakistani and UK primary level prospective teachers (B.Ed. students). Factor analysis revealed entrepreneurial intentions, instrumental readiness and self-efficacy as three common factors among both the data sets. Both the groups of respondents were also compared on five conceptual variables namely locus of control, self efficacy, entrepreneurial intentions, instrumental readiness and subjective norms. The prospective teachers from UK were found to be significantly lower on instrumental readiness and entrepreneurial intentions than their Pakistani counterparts. There were some partial impacts of demographic variables on entrepreneurial propensity of both the groups of respondents. Some implications for planning and policy were outlined.

Abstract

This article explores entrepreneurial attitudes among potential entrepreneurs in Pakistan. Multi-stage sampling maximized representation. Four hundred and eighty masters of business administration (MBA) students (potential entrepreneurs) from six public sector Pakistani universities returned completed questionnaires. Three factors emerged: entrepreneurial acceptability, entrepreneurial intentions and personal factors. Moreover, the perceptions of the potential entrepreneurs on locus of control, self efficacy, subjective norms and instrumental readiness were also analyzed. The majority of students showed generally positive attitudes towards entrepreneurship at all six universities. Overall there was a significant difference between negative and positive attitudes (negative mean 184, positive mean 284). There was also some impact of demographic variables, such as university, parental income and profession. Both genders exhibited similar attitudes at most of the sample institutions. The implications for practice and policy are discussed.

The asymmetric unit of the title compound, C13H11ClN2, contains two geometrically distinct molecules; one molecule is close to planar [dihedral angle between the aromatic rings = 2.44 (18)°] and the other is twisted about the linking hydrazide group [dihedral angle = 14.08 (19)°]. In the crystal, the N—H groups do not form hydrogen bonds and the molecules are linked by weak C—H...π interactions.

The two symmetry-independent molecules in the asymmetric unit of the title compound, C15H13NO2, differ in conformation, with the virtually planar 4-methylaniline (r.m.s. deviations of 0.0511 and 0.0082 Å) and piperonal groups (r.m.s. deviations of 0.0241 and 0.0486 Å) forming dihedral angles of 19.40 (5) and 42.90 (6)°. In the crystal, molecules are linked by C—H...O and C—H...π interactions. The H atoms of the two methyl groups are disordered over two sets of sites of equal occupancy.

In the crystal of the title compound, C16H19O4P, the molecules are dimerized with R22(10) ring motifs through the hydroxy and P=O O atoms. The dihedral angle between the aromatic rings is 66.89 (9)°. There are π–π interactions [centroid–centroid distance = 3.9669 (16) Å] between the benzene rings of adjacent benzyl groups. A C—H...π interaction between the aromatic rings where C—H is from a benzyl group is also present.

Abstract

The crystal of the title compound, 0.6C14H12N2O3·0.4C14H12N2O3, contains a mixture of its neutral (OH containing) and zwitterionic (NH containing) forms, in a 0.60 (4):0.40 (4) ratio. The former generates an S(6) loop via an intramolecular O—H...N hydrogen bond and the latter generates an S(6) loop via an N—H...O hydrogren bond. The aromatic rings are oriented at a dihedral angle of 42.52 (10)°. In the crystal, C—H...π interactions occur and aromatic π–π stacking interactions [centroid–centroid separations = 3.7106 (12) and 3.9177 (13) Å] consolidate the packing.

Abstract

We search for coincident gravitational wave signals from inspiralling neutron star binaries using LIGO and TAMA300 data taken during early 2003. Using a simple trigger exchange method, we perform an inter-collaboration coincidence search during times when TAMA300 and only one of the LIGO sites were operational. We find no evidence of any gravitational wave signals. We place an observational upper limit on the rate of binary neutron star coalescence with component masses between 1 and 3 M_sun of 49 per year per Milky Way equivalent galaxy at a 90% confidence level. Abstract

We use data from the second science run of the LIGO gravitational-wave detectors to search for the gravitational waves from primordial black hole (PBH) binary coalescence with component masses in the range 0.2--$1.0 M_\odot$. The analysis requires a signal to be found in the data from both LIGO observatories, according to a set of coincidence criteria. No inspiral signals were found. Assuming a spherical halo with core radius 5 kpc extending to 50 kpc containing non-spinning black holes with masses in the range 0.2--$1.0 M_\odot$, we place an observational upper limit on the rate of PBH coalescence of 63 per year per Milky Way halo (MWH) with 90% confidence., Comment: 7 pages, 4 figures, to be submitted to Phys. Abstract

We use 373 hours ($\approx$ 15 days) of data from the second science run of the LIGO gravitational-wave detectors to search for signals from binary neutron star coalescences within a maximum distance of about 1.5 Mpc, a volume of space which includes the Andromeda Galaxy and other galaxies of the Local Group of galaxies. This analysis requires a signal to be found in data from detectors at the two LIGO sites, according to a set of coincidence criteria. The background (accidental coincidence rate) is determined from the data and is used to judge the significance of event candidates. No inspiral gravitational wave events were identified in our search. Using a population model which includes the Local Group, we establish an upper limit of less than 47 inspiral events per year per Milky Way equivalent galaxy with 90% confidence for non-spinning binary neutron star systems with component masses between 1 and 3 $M_\odot$., 20 pages, 12 figures, to be submitted to Phys. Abstract

We perform a search for gravitational wave bursts using data from the second science run of the LIGO detectors, using a method based on a wavelet time-frequency decomposition. This search is sensitive to bursts of duration much less than a second and with frequency content in the 100-1100Hz range. It features significant improvements in the instrument sensitivity and in the analysis pipeline with respect to the burst search previously reported by LIGO. Improvements in the search method allow exploring weaker signals, relative to the detector noise floor, while maintaining a low false alarm rate, O(0.1) microHz. The sensitivity in terms of the root-sum-square (rss) strain amplitude lies in the range of hrss~10^{-20} - 10^{-19}/sqrt(Hz) No gravitational wave signals were detected in 9.98 days of analyzed data. We interpret the search result in terms of a frequentist upper limit on the rate of detectable gravitational wave bursts at the level of 0.26 events per day at 90% confidence level. We combine this limit with measurements of the detection efficiency for given waveform morphologies in order to yield rate versus strength exclusion curves as well as to establish order-of-magnitude distance sensitivity to certain modeled astrophysical sources. Abstract

We have performed a search for bursts of gravitational waves associated with the very bright Gamma Ray Burst GRB030329, using the two detectors at the LIGO Hanford Observatory. Our search covered the most sensitive frequency range of the LIGO detectors (approximately 80-2048 Hz), and we specifically targeted signals shorter than 150 ms. Our search algorithm looks for excess correlated power between the two interferometers and thus makes minimal assumptions about the gravitational waveform. We observed no candidates with gravitational wave signal strength larger than a pre-determined threshold. We report frequency dependent upper limits on the strength of the gravitational waves associated with GRB030329. Near the most sensitive frequency region, around 250 Hz, our root-sum-square (RSS) gravitational wave strain sensitivity for optimally polarized bursts was better than h_RSS = 6E-21 Hz^{-1/2}. Abstract

We report on the first joint search for gravitational waves by the TAMA and LIGO collaborations. We looked for millisecond-duration unmodelled gravitational-wave bursts in 473 hr of coincident data collected during early 2003. No candidate signals were found. We set an upper limit of 0.12 events per day on the rate of detectable gravitational-wave bursts, at 90% confidence level. From simulations, we estimate that our detector network was sensitive to bursts with root-sum-square strain amplitude above approximately 1-3x10^{-19} Hz^{-1/2} in the frequency band 700-2000 Hz. We describe the details of this collaborative search, with particular emphasis on its advantages and disadvantages compared to searches by LIGO and TAMA separately using the same data. Benefits include a lower background and longer observation time, at some cost in sensitivity and bandwidth. We also demonstrate techniques for performing coincidence searches with a heterogeneous network of detectors with different noise spectra and orientations. These techniques include using coordinated signal injections to estimate the network sensitivity, and tuning the analysis to maximize the sensitivity and the livetime, subject to constraints on the background., Comment: 15 pages, 11 figures, to be submitted to Phys Rev D. Abstract

We place direct upper limits on the amplitude of gravitational waves from 28 isolated radio pulsars by a coherent multi-detector analysis of the data collected during the second science run of the LIGO interferometric detectors. These are the first direct upper limits for 26 of the 28 pulsars. We use coordinated radio observations for the first time to build radio-guided phase templates for the expected gravitational wave signals. The unprecedented sensitivity of the detectors allow us to set strain upper limits as low as a few times $10^{-24}$. These strain limits translate into limits on the equatorial ellipticities of the pulsars, which are smaller than $10^{-5}$ for the four closest pulsars., Comment: 6 pages, 2 figures

Abstract

We report on a search for gravitational waves from coalescing compact binary systems in the Milky Way and the Magellanic Clouds. The analysis uses data taken by two of the three LIGO interferometers during the first LIGO science run and illustrates a method of setting upper limits on inspiral event rates using interferometer data. The analysis pipeline is described with particular attention to data selection and coincidence between the two interferometers. Abstract

The microbial community is dynamic in a mangrove ecosystem and thus controls various nutrient transformation processes in the ecosystem. In the current efforts to rehabilitate mangroves, this study focused on detecting significant variation within the bacterial community distribution in sediments, at two different depths, of an old regenerated mangrove forest stand of Avicennia marina. The regenerated mangrove forest site was artificially induced by placing wave breakers. Sediment samples were collected at 0-15 cm and 15-30 cm depth and analysed for soil properties. The bacterial community distribution was investigated using high throughput deoxyribonucleic acid (DNA) sequencing of the 16S rRNA gene. Bioinformatics analysis of the DNA sequence data showed that sediments were dominated by Proteobacteria (mostly Gammaproteobacteria and Deltaproteobacteria), followed by Firmicutes (Clostridia), Chloroflexi (Anaerolinae and Dehalococcoidetes), Actinobacteria (Acidimicrobiia, Coriobacteria), Acidobacteria (RB25 and BPC 102), Bacteroidetes (Saprospirae, Flavobacteriia, Bacteriocedes), Gemmatimonadetes and others. However, the intensity of the bacterial groups differed with respect to sampling depths and soil properties. The DNA sequencing revealed the sustenance of observed phylum in the soils of regenerated mangrove forest as compared to old mangrove stand. 33. Platelet indices in preeclampsia: comparative analysis with normotensive pregnant women.

Background: To compare plateletcount (PC), mean platelet volume (MPV), and platelet distribution width (PDW)between women with preeclampsia (PE) and normotensive pregnant women, andevaluate their effectiveness as predictors of PE., Research Design Andmethods: This cross-sectionalstudy at Nishtar Hospital, Multan, included 141 women: 74 normotensive and 67preeclamptic. Data was collected using an automated hematology analyzer andanalyzed with SPSS version 26 and ROC curves., Results: Mean age was 27.45 ± 5.18 years for cases and 28.41 ± 5.28 years for controls ( p  = 0.280).Gestational age was lower in the preeclamptic group (31.97 ± 4.07 weeks)compared to controls (33.92 ± 3.30 weeks) ( p  = 0.002). Blood pressures werehigher in preeclamptic women ( p  < 0.001). Platelet count was lower inpreeclamptic women (183.42 ± 95.69) vs. controls (256.42 ± 77.98) ( p  < 0.001). MPV (10.98 ± 1.55 vs. 9.79 ± 1.59, p  < 0.001) and PDW (16.82 ± 5.70vs. 14.20 ± 2.40, p  < 0.001) were higher in preeclamptic women. ROC analysisshowed PDW had an AUC of 0.73 and MPV an AUC of 0.71., Conclusions: PDW and MPV aresignificantly altered in preeclamptic women and can aid in early detection,potentially enhancing management.

34. Functional response of two Mesocomys species on factitious host Antheraea pernyi and their biocontrol potential against the Japanese giant silkworm Caligula japonica.

Abstract

Background: The Japanese giant silkworm (JGS) Caligula japonica is a significant defoliator pest in East Asia, causing severe economic losses in forest and fruit production. To establish a cost-effective biological control program against JGS, the age-stage, two-sex life table method was used to accurately assess the potential efficacy of two Mesocomys species (M. albitarsis and M. trabalae) in controlling this pest. Our study focused on the functional response of two Mesocomys species to Chinese oak silkworm (COS) Antheraea pernyi eggs (factitious host), as well as their reproductive attributes when exposed to JGS eggs (target host)., Results: Both Mesocomys species exhibited a type II functional response to increasing densities of COS eggs. Mutual interference of M. trabalae was greater than that of M. albitarsis at high parasitoid densities. Two Mesocomys parasitoids reared from COS eggs effectively parasitized JGS eggs, albeit with differing parasitism capacities. M. albitarsis had an average fecundity of 190.4 offspring, which was 3.8-fold higher than M. trabalae. Consequently, M. albitarsis demonstrated superior reproductive metrics when parasitizing JGS eggs, with more oviposition days (O d ), intrinsic rate of increase (r), finite rate of increase (λ) and net reproduction rate (R 0 ) compared to M. trabalae. Additionally, the net killing rate (C 0 ) of M. albitarsis was 145.2 eggs/female, significantly higher than M. trabalae (32.6 eggs/female)., Conclusion: Mesocomys albitarsis was identified as a more potent biocontrol agent against JGS than M. trabalae under indoor conditions. Abstract

Cardiovascular diseases (CVDs) are a leading cause of mortality worldwide. As a chronic inflammatory disease with a complicated pathophysiology marked by abnormal lipid metabolism and arterial plaque formation, atherosclerosis is a major contributor to CVDs and can induce abrupt cardiac events. The discovery of exosomes' role in intercellular communication has sparked a great deal of interest in them recently. Exosomes are involved in strategic phases of the onset and development of atherosclerosis because they have been identified to control pathophysiologic pathways including inflammation, angiogenesis, or senescence. This review investigates the potential role of stem cell-derived exosomes in atherosclerosis management. We briefly introduced atherosclerosis and stem cell therapy including stem cell-derived exosomes. The biogenesis of exosomes along with their secretion and isolation have been elaborated. The design engineering of exosomes has been summarized to present how drug loading and surface modification with targeting ligands can improve the therapeutic and targeting capacity of exosomes, demonstrating atheroprotective action. Moreover, the mechanism of action (endothelial dysfunction, reduction of dyslipidemia, macrophage polarization, vascular calcification, and angiogenesis) of drug-loaded exosomes to treat atherosclerosis has been discussed in detail. Abstract

Background: Contractures are a debilitating problem for individuals living in long-term care settings. However, there is a lack of education and training among the care staff regarding the identification of risk factors related to contractures and the preventive strategies that can decrease their development or progression. Addressing this knowledge gap has the potential to improve the quality of care provided to residents in care homes.The objective of this quality improvement (QI) project was to investigate the impact of a newly developed educational video on the awareness, knowledge and understanding of contractures among the care staff., Methods: This QI project involved two sequential Plan-Do-Study-Act cycles and employed a pre and post-test design to evaluate the impact of the contracture educational video. Primary outcomes were assessed using paper surveys to capture prevideo and postvideo levels of knowledge and understanding of contractures. Furthermore, both verbal and written feedback from participants were gathered to identify areas of strengths and improvement., Results: Baseline data revealed that about 56% of the care staff lacked knowledge and understanding of contractures with another 33% reporting possessing only basic knowledge. Following the video intervention, percentage of care staff who reported good knowledge and understanding increased to 67% while 22% reported basic knowledge and understanding of contractures. The care staff suggested changes to the video to improve accessibility of the information, this was incorporated in the refilming of the video., Conclusion: This QI project demonstrated that the introduction of a contracture educational video is a feasible and positively received method of enhancing awareness, knowledge and understanding of contractures among care staff. 37. Differentiating reactive and neoplastic gamma-delta (γδ) T-cell expansions in the peripheral blood and bone marrow.

Abstract

The clinical and immunophenotypic attributes of reactive γδ T-cell expansions are less well characterized than their malignant counterparts, which can pose diagnostic challenges. This study aims to investigate the characteristics and long-term clinical outcomes of reactive γδ T-cell expansions. A retrospective review was performed to identify patients with expanded γδ T-cell population (>15% of T-cells) by flow cytometry in peripheral blood and/or bone marrow specimens over a 17-year period. The cases were divided into reactive and malignant categories and their clinical and immunophenotypic findings were compared. Clinical follow-up was performed. 97 patients were identified including 19 malignant and 78 reactive cases with a variety of underlying conditions. The median absolute γδ T-cell count and median percentage of γδ T-cells per total T-cells were significantly lower in reactive vs. malignant cases (p = 0.0001 and p < 0.00001, respectively). Reactive cases showed more frequent brighter surface CD3 expression (87.1% vs. 42.1%; p < 0.0001), no discrete loss of CD7 (0% vs. 36.9%; p < 0.0001), less frequent lack of CD5 (25.7% vs. 42.4%; p < 0.0001), and no homogeneous CD56 expression (0% vs. 31.6%; p > 0.0001) as compared with malignant cases. Upon long-term follow-up, none of the reactive cases showed clinical evidence of malignant evolution. Reactive expansions of γδ T-cells can be seen in a variety of conditions including hematologic neoplasms, autoimmune and post-transplant states, and infections. Such cases have significantly lower γδ T-cell counts and percentages and no discrete loss of CD7. Lack of CD5 on its own is not an indication of immunophenotypic aberrancy in γδ T-cells. Abstract

The plant Cissus tuberosa Moc is abundant in phenolics, has been documented to have neuroprotective properties. The study seeks to determine the neuroprotective effects of C. tuberosa ethanolic extract (CTE) against Parkinson's disease by evaluating its impact on motor dysfunction, cognitive deficits, neuroinflammation, and neurodegeneration in paraquat-induced Parkinson's disease models. The research hypothesizes that CTE can modulate key biomarkers involved in Parkinson's pathology, including α-synuclein, interleukin-1β (IL-1β), and tumor necrosis factor-α (TNF-α), assessed through qRT-PCR, as well as interleukin-6 (IL-6) and TNF-α, evaluated through ELISA. Parkinson disease was induced by using paraquat intraperitoneally. The study was designed by considering various groups with their respective treatments, control group treated normally, disease control receiving paraquat (1 mg/kg, i.p.), standard treated grabbed with (levodopa+carbidopa), and three treatment groups received plant extract (150, 300, 600 mg/kg) respectively for 21 days study period. Both behavioral, and biochemical analysis were performed. HPLC analysis revealed the presence of several phenolic compounds. CTE significantly improved motor function and cognitive performance in rats, showing a dose-dependent reduction in paraquat-induced neurotoxicity (150 < 300 < 600 mg/kg, P<0.001). CTE significantly restored antioxidant enzyme levels (P<0.001), contributing to the alleviation of oxidative stress. Neurotransmitter levels were significantly improved in a dose-dependent manner (P<0.001), while acetylcholinesterase (AChE) levels were significantly reduced (P<0.001). CTE treatment showed significant restoration of brain tissue, reducing neuroinflammation and neurodegeneration, thereby preserving normal brain structure. ELISA testing demonstrated a significant (P<0.001) downregulation of IL-6 and TNF-α levels in CTE-treated groups. qRT-PCR results showed significant downregulation of α-synuclein, IL-1β, and TNF-α mRNA expression in CTE-treated groups compared to the diseased group, suggesting neuroprotective effects. Abstract

All-trans retinoic acid (ATRA) and arsenic trioxide (ATO) have revolutionized the treatment of acute promyelocytic leukemia (APL), offering a cure rate of > 80%. Along with improved survival, the long-term consequences of anti-APL therapy are becoming increasingly apparent, including potential therapy-related myeloid neoplasms (t-MNs). T-MNs are well known to arise after cytotoxic chemotherapy, but the leukemogenic potential of regimens utilizing only ATRA/ATO is not well established. The objective of this study is to examine the incidence, long-term risk, and clinicopathologic features of t-MNs arising after anti-APL therapy and how they correlates with the therapeutic regimen employed. We retrospectively collected treated APL patients between 01/2001 and 02/2021, categorized them into ATRA/ATO + chemo and ATRA/ATO groups based on the regimen used, and evaluated for the development of t-MN. A total of 110 APL patients were identified, including 67 (61%) treated with ATRA/ATO + chemo and 43 (39%) treated with ATRA/ATO only. Overall, 4/110 (3.6%) patients developed t-MNs, with all four emerging in the ATRA/ATO + chemo group. Ultimately, the incidence of t-MN in ATRA/ATO + chemo group was significantly higher compared with ATRA/ATO only group(5.97% vs. 0.0%, respectively; p = 0.0289). Our data spanning over two decades suggests that conventional chemotherapy for APL is associated with a small but significant risk of t-MN, whereas ATR/ATO does not carry this risk. This takes on new significance, considering several recent and ongoing trials have shown that a chemotherapy-free approach might become feasible for all risk APL types in the near future. Consequently, the omission of leukemogenic and arguably unnecessary chemotherapy from APL regimens may reduce the incidence of t-MNs in long-term survivors without sacrificing their cure rates., Competing Interests: Declarations. Abstract

To assess the relevance of co-occurring somatic mutations in TP53-mutated myeloid neoplasms with ≥10% blasts, we pooled 325 individuals from 10 centres. We focused on comparing three published somatic co-alteration signatures comprising (1) nine MDS-related genes ('ICC-MDSR'), (2) ICC-MDSR + additional secondary mutations-related genes ('Tazi signature') and (3) EPI6 (comprising six genes). Outcomes examined were 24-month overall survival (OS24) and front-line complete response (CR1). The median age was 69 years with 77% receiving front-line hypomethylating agents (HMA). All three signatures ICC-MDSR (p = 0.009), Tazi signature (p = 0.001) and EPI6 (p = 0.025) predicted inferior CR1. In the low-intensity (HMA) subgroup, only Tazi signature (p = 0.026) predicted inferior CR1. In OS24 analysis of the HMA-treated subgroup (N = 200), only Tazi signature was adverse (hazard ratio, HR = 1.6 [1.1-2.2]; p = 0.011). However, a forward stepwise multivariable age-adjusted Cox model including all three signatures picked EPI6 as the sole significant adverse predictor in the entire cohort (p = 0.0001) as well as within the HMA-treated subgroup (p = 0.0071). 41. Bleeding Phenotype of Glanzmann Thrombasthenia (GT) and Treatment Outcomes in Over One Hundred Patients: A Two-Center Experience in North Pakistan.

Abstract

Background: Glanzmann thrombasthenia (GT) is a rare disease with an autosomal recessive inheritance pattern. This disorder is not so uncommonly encountered in routine clinical practice and laboratory settings in Pakistan let alone in the rest of the world. To describe the bleeding phenotype of GT and treatment outcomes in over one hundred patients in north Pakistan., Materials and Methods: This descriptive, cross-sectional, retrospective study was conducted on patients from 2011 to 2023 using a convenience sampling technique. A total of 103 patients of all ages and both genders diagnosed as having inherited GT were included in the study., Results: The median age of the study population was 1.1 years, with an interquartile range (IQR) of 0.8-2. Out of the total, 55 (53%) patients were males and 48 (47%) patients were females. Ninety-eight percent of patients were diagnosed using light transmission aggregometry, and only two (2%) patients were diagnosed by immunophenotyping. Due to the high incidence of interfamily marriages, 86 (84%) patients were born to consanguineous marriages. Thirty-nine (38%) patients had an episode of major bleeding as defined by the International Society on Thrombosis and Haemostasis (ISTH) criteria. Epistaxis in 73 (71%) patients, skin bruising in 63 (61%), and gum bleeding in 57 (55%) were the most common bleeding symptoms. Thirty-two (31%) required the use of r-VIIa for major bleeding and five (5%) patients underwent fully matched allogeneic HSCT (hematopoietic stem cell transplant). 44. Correlation Between Transient Elastography and Non-invasive Biomarker Scores for the Detection of Liver Fibrosis.

Abstract

Background and aim The study aimed to address the need for reliable and non-invasive biomarkers (NIBM) for detecting fibrosis among patients with chronic liver disease (CLD). Material and methods This was a diagnostic validation study executed at the Department of Gastroenterology, Jinnah Hospital, Lahore. The study was carried out from July 2023 to June 2024, enrolling a total of 88 patients using non-probability consecutive sampling. Patients with chronic liver disease (CLD) due to chronic viral hepatitis B (HBV), hepatitis C (HCV), and non-alcoholic fatty liver disease (NAFLD) were included in the study. A cut-off value of 12.5 kPa was used to label fibrosis using transient elastography. Blood samples were collected for recording values of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and platelet count, and the appropriate formulas were applied to calculate the aspartate aminotransferase to platelet ratio index (APRI), Fibrosis-4 (FIB-4), AST/ALT-to-platelet ratio index (AARPRI), and BARD scores (a scoring system for NAFLD fibrosis that predicts the risk of advanced fibrosis in NAFLD patients; the components include body mass index (BMI), aspartate aminotransferase (AST)/alanine aminotransferase (ALT) ratio, and diabetes status). Spearman's rank correlation coefficient was used to assess the strength and direction of the association between these variables. Results Among the 88 patients, 61.4% were male and HCV was the most common cause of CLD (63.6%). Statistically significant correlations were found between transient elastography and NIBM: FIB-4 (p < 0.001, area under receiver operating characteristic curves (AUROC), 0.835; sensitivity, 47.7%; specificity, 90.9%), APRI (p = 0.020; AUROC, 0.769; sensitivity, 25.0%; specificity, 93.2%), AARPRI (p = 0.011, AUROC, 0.782), and BARD (p = 0.033; AUROC, 0.648). FIB-4 demonstrated the strongest correlation with liver stiffness measurements (LSM) (r = 0.617), indicating its reliability in detecting liver fibrosis, whereas the aspartate aminotransferase to alanine aminotransferase ratio (AAR) showed the weakest correlation (r = 0.163). Conclusion This study reported that FIB-4 displayed the highest correlation with liver stiffness measurements obtained through transient elastography in detecting liver fibrosis. APRI and AARPRI reported a moderate correlation, while AAR and BARD were less consistent in their performance. Abstract

Background: This study aimed to investigate the demographic, clinical, and diagnostic aspects of adult pancytopenia while exploring its etiological spectrum through hematological parameters and bone marrow studies., Research Design and Methods: This observational study involved 117 adult individuals ranging from 13 to 85 years who presented with pancytopenia. A comprehensive examination of demographic features, hematological parameters, clinical presentations, and physical findings, including liver and spleen characteristics, was conducted. Additionally, serological analyses for HBsAg and Anti HCV were performed. The diagnostic spectrum was determined through bone marrow studies., Results: Pancytopenia manifested with varied clinical symptoms, with generalized weakness (72.65%), fever (64.1%), dyspnea (54.70%), bleeding (34.2%), and weight loss (25.6%) being prominent. Physical examination revealed a range of liver and spleen characteristics, with hepatomegaly observed in 32.48% and splenomegaly in 44.4% of cases. Serological findings indicated HBsAg positivity in 8.5% and Anti HCV positivity in 21.37% of cases. The diagnostic distribution encompassed diverse conditions, with aplastic anemia (17.1%), megaloblastic anemia (12.8%), and myelodysplastic syndromes (12.8%) being prevalent., Conclusions: This study provides a comprehensive overview of the demographic, clinical, and diagnostic aspects of pancytopenia. The observed prevalence of different diagnoses underscores the necessity of a thorough evaluation, including bone marrow studies, for accurate diagnosis and appropriate management.

Abstract

Recent updates in the classification of myeloid neoplasms (MNs) recognize the poor prognostic impact of TP53 mutations, with particular emphasis on the TP53 allele status. Studies on the effect of TP53 allele status exclusively in therapy-related MNs (t-MNs) are lacking. We compared the clinicopathologic and survival characteristics of t-MNs with single-hit (SH) and multi-hit (MH) TP53 mutations. A total of 71 TP53 -mutated t-MNs were included, including 56 (78.9%) MH and 15 (21.1%) SH. Both groups showed comparable genetic profiles with an excess of high-risk karyotypes and a paucity of other co-mutated genes. TP53 was the sole detectable mutation in 73.3% of SH and 75.0% of MH cases. The overall survival (OS) of SH TP53 -mutated t-MNs was not significantly different from MH cases (median survival: 233 vs.273 days, p  = 0.70). Our findings suggest that t-MNs with SH TP53 mutations share the poor prognostic and biologic profile of their MH counterparts.

47. Nasal microbionts differentially colonize and elicit cytokines in human nasal epithelial organoids.

Abstract

Nasal colonization by Staphylococcus aureus or Streptococcus pneumoniae is associated with an increased risk of infection by these pathobionts, whereas nasal colonization by Dolosigranulum species is associated with health. Human nasal epithelial organoids (HNOs) physiologically recapitulate human nasal respiratory epithelium with a robust mucociliary blanket. We reproducibly monocolonized HNOs with these three bacteria for up to 48 hours with varying kinetics across species. HNOs tolerated bacterial monocolonization with localization of bacteria to the mucus layer and minimal cytotoxicity compared to uncolonized HNOs. Human nasal epithelium exhibited both species-specific and general cytokine responses, without induction of type I interferons, consistent with colonization rather than infection. Only live S. aureus colonization induced IL-1 family cytokines, suggestive of inflammasome signaling. D. pigrum and live S. aureus decreased CXCL10, whereas S. pneumoniae increased CXCL11, chemokines involved in antimicrobial responses. Background: Leadership is a critical competency for medical professionals, yet it is often neglected in medical training. For ObGyn residents, leadership training is particularly crucial as it significantly impacts both maternal and newborn outcomes, as well as the operational efficiency of healthcare teams. The main objective of this study was to assess the perceptions of obstetrics and gynecology residents who served as group leaders in the emergency team at the Department of Gynecology, Ward 3, Dr. Ruth K.M. Pfau Civil Hospital Karachi., Methods: A Cross-sectional survey was conducted with purposively sampled 28 year-4 residents who worked as group leaders during last 3 years (from 2018 to 2020) of their residency program at the emergency team in the department of Gynecology Ward 3 Dr Ruth KM Pfau Civil Hospital Karachi. The perceptions on leadership were assessed on 25 items scale sent through a questionnaire on email. Grading of responses was done using a 4-point ordinal scale where 1 meant little importance and 4 was regarded as having great importance. Data was summarized with relevant descriptive statistics and was analyzed on SPSS version 22., Results: The mean age of residents was 30.36. The mean leadership scores of the group of residents were calculated to be 77.50 (SD ± 9.57) while 14(50%) residents showed good and 14 (50%) showed excellent leadership skills based on cumulative scores. Of the 25 traits examined in this study, the highest reported trait was humility 3.82 (± 0.39) followed by empowerment 3.68 (± 0.77) and effective communication 3.68 (± 0.77). While responding about learning experiences, 89.3% of participants felt that the experience enhanced their decision-making skills and boosted their confidence in dealing with emergencies., Conclusion: Our study highlights the critical importance of leadership development in the training of ObGyn residents, particularly in high-pressure emergency settings. The findings reveal that residents value leadership traits such as humility, empowerment, and effective communication, which are essential for building teamwork and ensuring optimal patient outcomes and patient satisfaction., (© 2024. 49. "Accelerated" chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL): unraveling the biological gray zone of CLL/SLL in the era of novel therapies.

Abstract

Accelerated chronic lymphocytic leukemia/small lymphocytic lymphoma (A-CLL/SLL) is a histologically aggressive subtype of CLL/SLL that lies in between conventional CLL/SLL (C-CLL/SLL) and Richter transformation (RT) on the biological spectrum. Although the histologic criteria for A-CLL/SLL were defined 14 years ago, the clinical and genetic characteristics and survival outcomes of these patients have yet to be studied in the era of novel therapies. We retrospectively analyzed the clinicopathologic, genetic, and survival characteristics of 34 patients with confirmed tissue diagnosis of A-CLL/SLL and compared them with 120 patients with C-CLL/SLL. Patients with A-CLL/SLL had significantly higher frequencies of B-symptoms, anemia and thrombocytopenia, splenomegaly, higher LDH, and more advanced Rai stages. A-CLL/SLL showed a significantly higher frequency of TP53 mutations (55.0% vs. 11.5%;p < 0.0001) and deletions (38.2% vs. 8.3%;p < 0.0001), lower isolated del(13q) (5.8% vs. 27.5%;p < 0.0001), and increased incidence of RT (11.76% vs. 0.83%;p = 0.0025). The overall survival of patients with A-CLL/SLL was significantly lower than C-CLL/SLL (median survival: 6.17 years vs. not reached; 2 and 5-year survival rates: 75.5% vs. 94.7% and 53.3% vs. 93.7%, respectively; p < 0.0001); however, novel agents have improved the outcomes dramatically compared to the previously published data in the pre-BTKi era. Our results support the categorization of A-CLL/SLL as a distinct biologically aggressive subtype of CLL/SLL and highlight the need to revise the diagnostic criteria utilizing a multifaceted approach that integrates the overall pathobiological profile of the disease, in addition to the histology., (© 2024.
