7. Compressive Non-Toxic Intrathoracic Goiter in Patient with Contraindication to Surgical Treatment: Case Report

Abstract

Introduction:Thyroidectomy is the treatment of choice for non-toxic compressive Multinodular Goiter (MNG). However, when surgery is contraindicated, other therapeutic options should be evaluated. In this case report, non-surgical therapeutic possibilities are reviewed, and the results obtained with Radioiodine Therapy (RAI) after stimulating with Recombinant Thyroid-Stimulating Hormone (rhTSH) are described. Case report:A 92-year-old patient with multiple comorbidities, non-toxic MNG, and symptomatic compression of the trachea and esophagus. Accordingly, RAI was chosen due to the high surgical risk. Initially, pre-treatment with methimazole was performed to increase TSH, which was interrupted due to drug hepatitis. Then, RAI was chosen in the post-stimulus with a low dose of rhTSH, with significant and early reduction of goiter and symptoms. Discussion:Although total thyroidectomy is the treatment of choice for non-toxic and symptomatic intrathoracic MNG; it does present risks, especially in patients with multiple comorbidities. Thus, interest in less invasive techniques is increasing. Thermal radio-ablations exhibit satisfactory results, however it is still an inaccessible technique. Radioiodine is an effective treatment option subsequent to the use of rhTSH or hypothyroidism methimazole-induced. RhTSH and methimazole can increase the level of radiation absorbed by the gland, which can lead to a reduction in the required I-131 dose. Conclusion:It is possible to mitigate the symptoms and improve the quality of life of patients with non-toxic and symptomatic MNG and multiple comorbidities, without surgical intervention. In this case report we describe alternatives to surgical intervention and show the effectiveness and safety of RAI along with rhTSH in the management of MNG.

Abstract

An analysis is performed on the dynamical coupling between the variability of the extratropical stratospheric and tropospheric circulations during the Northern Hemisphere winter. Obtained results provide evidence that in addition to the well-known Charney and Drazin mechanism by which vertical propagation of baroclinic Rossby waves is nonlinearly influenced by the zonal mean zonal wind, topographic forcing constitutes another important mechanism by which nonlinearity is introduced in the troposphere--stratosphere wave-driven coupled variability. On the one hand, vortex variability is forced by baroclinic Rossby wave bursts, with positive (negative) peaks of baroclinic Rossby wave energy occurring during rapid vortex decelerations (accelerations). On the other hand, barotropic Rossby waves of zonal wavenumbers s = 1 and 3 respond to the vortex state, and strong evidence is presented that such a response is mediated by changes of the topographic forcing due to zonal mean zonal wind anomalies progressing downward from the stratosphere. It is shown that wavenumbers s = I and 3 are the dominant Fourier components of the topography in the high-latitude belt where the zonal mean zonal wind anomalies are stronger; moreover, obtained results are in qualitative agreement with the analytical solution provided by the simple topographic wave model of Charney and Eliassen. Finally, evidence is provided that changes of barotropic long (s [less than or equal to] 3) Rossby waves associated with vortex variability reproduce a NAO-like dipole over the Atlantic Ocean but no dipole is formed over the Pacific Ocean. Moreover, results suggest that the nonlinear wave response to topographic forcing may explain the spatial changes of the NAO correlation patterns that have been found in previous studies.

Abstract

Genes encoding regulatory RNAs known as short RNAs (sRNAs) or non-coding sRNAs (ncRNAs), modulate physiological responses through different mechanisms, through RNA-RNA interaction or RNA-protein interaction. These molecules transcribed in trans and in cis relative to the target RNA. They are located between the coding regions of proteins, i.e., in the intergenic regions of the genome and show signals of promoters and termini sequences generally Rho-independent. The size of the ncRNAs genes ranges from ~ 50 to ~ 500 nucleotides and several transcripts are processed by RNase with smaller end products, which modulate physiological responses through different mechanisms, by RNA-RNA interaction or RNA-protein interactions and some interactions may be stabilized by the Hfq chaperone. The Riboswitches constitute another class of ncRNAs, located in the 5'UTR region of an mRNA that promote transcriptional regulation through their interaction with a linker molecule. Recently, in prokaryotes, CRISPR (Clustered Regularly Interspaced Short Palindromic Repeats) regions have described, which repeats of sequences of palindromic bases are. Each replicate consists of short segments of "spacer DNA" from exposures prior to a bacteriophage virus or exogenous plasmid. The CRISPR system consists of an immune system of resistance to exogenous molecules.

Abstract

Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . Abstract

Recombinant human thyrotropin (rhTSH) reduces the activity of radioiodine required to treat multinodular goiter (MNG), but acute airway compression can be a life-threatening complication. In this prospective, randomized, double-blind, placebo-controlled study, we assessed the efficacy and safety (including airway compression) of different doses of rhTSH associated with a fixed activity of 131I for treating MNG. Euthyroid patients with MNG (69.3 ± 62.0 mL, 20 females, 2 males, 64 ± 7 years) received 0.1 mg (group I, N = 8) or 0.01 mg (group II, N = 6) rhTSH or placebo (group III, N = 8), 24 h before 1.11 GBq 131I. Radioactive iodine uptake was determined at baseline and 24 h after rhTSH and thyroid volume (TV, baseline and 6 and 12 months after treatment) and tracheal cross-sectional area (TCA, baseline and 2, 7, 180, and 360 days after rhTSH) were determined by magnetic resonance; antithyroid antibodies and thyroid hormones were determined at frequent intervals. After 6 months, TV decreased significantly in groups I (28.5 ± 17.6%) and II (21.6 ± 17.8%), but not in group III (2.7 ± 15.3%). After 12 months, TV decreased significantly in groups I (36.7 ± 18.1%) and II (37.4 ± 27.1%), but not in group III (19.0 ± 24.3%). No significant changes in TCA were observed. T3 and free T4 increased transiently during the first month. After 12 months, 7 patients were hypothyroid (N = 3 in group I and N = 2 in groups II and III). rhTSH plus a 1.11-GBq fixed 131I activity did not cause acute or chronic changes in TCA. After 6 and 12 months, TV reduction was more pronounced among patients treated with rhTSH plus 131I.

Abstract

The sensitivity of the climate impact of Mt. Pinatubo eruption in the tropics and extratropics to different QBO phases is investigated. Mt. Pinatubo erupted in June 1991 during the easterly phase of the QBO at 30 hPa and the phase change to westerly took place in August 1992. Here, the consequences are analyzed if the QBO phase had been in the opposite phase during the eruption of Mt. Pinatubo. Hence, in this study, simulations are carried out using the middle atmosphere configuration of ECHAM5 general circulation model for two cases – one with the observed QBO phase and the other with the opposite QBO phase. The response of temperature and geopotential height in the lower stratosphere is evaluated for the following cases – (1) when only the effects of the QBO are included and (2) when the effects of aerosols, QBO and SSTs (combined response) are included. The tropical QBO signature in the lower stratospheric temperature is well captured in the pure QBO responses and in the combined (aerosol + ocean + QBO) responses. The response of the extratropical atmosphere to the QBO during the second winter after the eruption is captured realistically in the case of the combined forcing showing a strengthening of the polar vortex when the QBO is in its westerly phase and a warm, weak polar vortex in the easterly QBO phase. The vortex is disturbed during the first winter irrespective of the QBO phases in the combined responses and this may be due to the strong influences of El Niño during the first winters after eruption. However, the pure QBO experiments do not realistically reproduce a strengthening of the polar vortex in the westerly QBO phase, even though below normal temperatures in the high latitudes are seen in October-November-December months when the opposite QBO phase is prescribed instead of the December-January-February winter months used here for averaging.
