Abstract

Anetoderma or macular atrophy is a rare skin condition of unclear pathogenesis, often associated with autoimmune diseases and skin damage from various infections. Human immunodeficiency virus (HIV), syphilis, and poxviruses have been implicated in the development of anetoderma. A 37‐year‐old male patient with HIV and recent unprotected sexual encounters presented with more than 400 skin lesions, consistent with Mpox. Symptomatic treatment for Mpox resulted in acute symptom resolution. However, 8 months later he developed papular anetoderma lesions in areas previously affected by Mpox. Biopsy confirmed the loss of elastic fibers in the affected skin areas, leading to the diagnosis of Mpox‐induced anetoderma. Abstract

Objective: Interpersonal partner violence (IPV) is a pervasive social and public health problem worldwide. In the context of the response by the World Health Organization to this issue, proposals were developed to improve the usefulness of International Classification of Diseases (ICD-11) descriptions for helping clinicians to reliably identify relational problems and maltreatment (RPM), including IPV, as part of clinical practice. This study focuses on factors associated with the accuracy of diagnostic decisions made by mental health professionals (MHPs) around the world when using the proposed ICD descriptions for RPM with standardized clinical vignettes in an ICD-11 internet-based field trial study. Method: A total of 1,221 MHPs from ninety countries evaluated randomly assigned vignettes, which described patients presenting with or without RPM and with or without a co-occurring depressive or anxiety disorder. Results: Findings showed that increased diagnostic accuracy for RPM was associated with MHPs being female or having greater knowledge/experience of RPM, as well as with the absence of a mental disorder in the patient. Conclusions: Findings highlight the importance of providing IPV-related training programs for MHPs to (a) better identify and address IPV in mental health settings and (b) improve knowledge and understanding of the consequences of IPV for physical and mental health. Abstract

Background and aims: The ICD-11 chapter on mental, behavioral and neurodevelopmental disorders contains new controversial diagnoses including compulsive sexual behavior disorder (CSBD), intermittent explosive disorder (IED) and gaming disorder. Using a vignette-based methodology, this field study examined the ability of mental health professionals (MHPs) to apply the new ICD-11 diagnostic requirements for impulse control disorders, which include CSBD and IED, and disorders due to addictive behaviors, which include gaming disorder, compared to the previous ICD-10 guidelines. Methods: Across eleven comparisons, members of the WHO's Global Clinical Practice Network (N 5 1,090) evaluated standardized case descriptions that were designed to test key differences between the diagnostic guidelines of ICD-11 and ICD-10. Results: The ICD- 11 outperformed the ICD-10 in the accuracy of diagnosing impulse control disorders and behavioral addictions in most comparisons, while the ICD-10 was not superior in any. The superiority of the ICD-11 was particularly clear where new diagnoses had been added to the classification system or major revisions had been made. However, the ICD-11 outperformed the ICD-10 only in a minority of comparisons in which mental health professionals were asked to evaluate cases with non-pathological high involvement in rewarding behaviors. Discussion and Conclusions: Overall, the present study indicates that the ICD-11 diagnostic requirements represent an improvement over the ICD-10 guidelines. However, additional efforts, such as training programs for MHPs and possible refinements of diagnostic guidance, are needed to avoid over-diagnosis of people who are highly engaged in a repetitive and rewarding behavior but below the threshold for a disorder. Abstract

The aim of this study was to estimate the level of empathy among primary care (PC) health professionals and its relationship with their approach to patients at risk due to alcohol consumption. This is an observational, descriptive, and multicenter study that included 80 PHC professionals. The professionals completed a questionnaire comprising socio-occupational questions and inquiries regarding their actions when dealing with patients suspected of risky alcohol consumption. The Jefferson Scale of Empathy was used to measure their level of empathy and was completed by 80 professionals, of whom 57.5% were family physicians, 10% were nurses, and 32.5% were family- and community-medicine residents. The mean age was 39.5 ± 13.1 (SD) (range of 24–65 years) and 71.3% were females. The mean empathy level score was 112.9 ± 11.1 (95% CI: 110.4–115.4; range: 81–132 points). Actions that stood out for their frequency were providing health advice in the general population, offering advice to pregnant women, and recommending abstinence to users of hazardous machinery or motor vehicles. The level of empathy was associated with age (p = 0.029), the health center's scope (p = 0.044), systematic alcohol exploration (p = 0.034), and follow-ups for patients diagnosed with risky consumption (p = 0.037). The mean score obtained indicated a high level of empathy among professionals. Professionals with greater empathy more frequently conducted systematic screening for risky alcohol consumption. 24. Enhancement and Tunability of Plasmonic-Magnetic Hyperthermia through Shape and Size Control of Au:Fe3O4 Janus Nanoparticles.

Abstract

Nanohyperthermia therapies have appeared as a promising alternative for the treatment of diverse cancer tumors. Multifunctional nanosystems offer a step forward in these therapies. They harness more efficient and multistimuli hyperthermia for low-dose application, as well as supplementary functions for imaging, targeting, controlled release, and sensing. Among them, Janus Au:Fe3O4 nanoparticles (JNPs) are highly versatile and a prominent example for dual photo- and magneto-thermia capabilities. To achieve the highest efficiencies of these nanomaterials, which allow low-dose applications, optimization in terms of size and shape is imperative. Here, we have expanded the synthesis of Janus nanostructures and carried out a systematic study to understand the structure–performance relationship and improve their hyperthermia efficiency. JNPs were synthesized by seed-mediated growth processes to obtain Janus nanostars (JNSs) and Janus nanospheres (JNSphs) with controlled sizes, together with initial heterodimers and indented F3O4 NPs. The hyperthermia abilities were then evaluated using AC magnetometry and under near-infrared laser irradiation. The results showed a clear effect of size and shape on the tuning of both magnetothermia and photothermia. Iron oxide size showed the biggest effect on magnetothermia, which could be tuned by the size and shape of the gold component. Likewise, for photothermia, JNSs offered the best performance and a clear correlation between the proximity of the plasmonic band to the irradiation source and their photothermal performance, modulated by the presence of iron oxide. A SARmax of 3 kW g–1 for the strongest field and frequency tested, 0.48 kW g–1 for biological safety limits in magnetothermia, and 8.3 kW g–1 per W cm–2 of applied light for photothermia were obtained. The acquired results support the proper selection of the best JNPs for dual hyperthermia and allow one to set up the design rules for obtaining more efficient multifunctional nanosystems, opening new avenues toward advanced heating-based therapies. Abstract

[SPA] Esta tesis doctoral se presenta bajo la modalidad de compendio de publicaciones. La contribución de la agricultura al bienestar humano va más allá de la mera producción de alimentos y materias primas. También abarca la provisión de bienes y servicios de no mercado que pueden tener impacto, positivo o negativo, en el bienestar humano. Así, la agricultura es la principal actividad desarrollada al amparo de los agroecosistemas, donde la presión antrópica de las prácticas agrícolas afecta a su funcionamiento y a los niveles de provisión de servicios y contraservicios ecosistémicos. La valoración económica de estos servicios y contraservicios, y en consecuencia de las prácticas agrícolas que los fomentan, permite orientar las decisiones de política agrícola de acuerdo a la contribución de los agroecosistemas al bienestar humano. En este contexto, la presente tesis doctoral tiene como objetivo central la valoración económica de la demanda social de servicios y contraservicios de los agroecosistemas, así como de las prácticas agrícolas que los promueven. Para ello, y utilizando experimentos de elección como metodología principal, se formula un enfoque integral para la valoración de agroecosistemas en una región semiárida del Mediterráneo, la Región de Murcia (sudeste de España). La validación de este enfoque es llevada a cabo por parte de los agentes implicados en su gestión y permite seleccionar los servicios y contraservicios más importantes, cuya demanda social y valor de no mercado son posteriormente estimados. Más allá de las cifras concretas, los resultados ofrecen una visión amplia sobre el enfoque necesario en las políticas agrícolas para que logren aumentar el bienestar de la sociedad. Así, las acciones de política agrícola deberían centrarse en aumentar la provisión de alimentos, promover la biodiversidad de los agroecosistemas, reducir la temperatura local, generar oportunidades para el ocio y recreo en los paisajes agrícolas, mientras se busca regular el suministro de agua para riego y mitigar la contaminación por nutrientes agrícolas. En relación a este último aspecto, en esta tesis también se aborda la evaluación de la demanda social de medidas agrícolas con el fin de reducir la contaminación por nitratos de la agricultura. Todas las medidas propuestas, pese a la heterogeneidad detectada en su demanda, contaron con apoyo social, lo que ha de brindar orientación a los gestores públicos para el establecimiento de estrategias para la mitigación de la contaminación agrícola por nitratos que sean socialmente respaldadas. Así se espera que esta tesis proporcione una mejor comprensión de los vínculos entre agricultura y bienestar humano, orientando la formulación de políticas mejor informadas y eficaces en aras de su contribución al bienestar social. [ENG] This doctoral dissertation has been presented in the form of thesis by publication. The contribution of agriculture to human wellbeing goes beyond food production. It also encompasses the provision of non-commodity goods and services that may impact human wellbeing both positively and negatively. Agriculture is the main activity developed within agroecosystems, where human pressures, mainly through agricultural practices, affect their innate functioning. This leads to the provision of agroecosystem services and disservices. Economic valuation of agroecosystem services and disservices, and agricultural practices in accordance, allow us to guide policy decisions in line with the contribution of agroecosystems to human wellbeing. In such a context, this thesis aims to economically evaluate the integrated social demand of agroecosystem services and disservices, and the agricultural practices that promotes them, by adapting a comprehensive approach for agroecosystem valuation in a semiarid Mediterranean region – the Region of Murcia (SE Spain). Discrete choice experiment was the central methodology used. The non-market value of agroecosystem services and disservices, and consequently, of agricultural practices impacting on them, was disentangled aiming to reflect the contribution of agroecosystems to human wellbeing in monetary terms. The validation of a comprehensive approach for agroecosystem valuation by stakeholders settled the agroecosystem services and disservices whose social demand and non-market value was then estimated. Regardless of the specific economic values, the non-market results provide deep insight into the expected focus of agricultural policies for increasing their impact on human wellbeing. Agricultural measures should therefore be centred around increasing food provision, promoting agroecosystem biodiversity, reducing the local temperature, generating opportunities for recreation in agricultural landscapes, whilst seeking to regulate water supply for irrigation and mitigate agricultural nutrient pollution. In accordance with the latter, the social demand for agricultural measures aiming to reduce nitrate pollution from agriculture was addressed. Despite the great preference heterogeneity, social support was revealed to the measures, providing guidance for policy makers in the establishment of socially supported strategies for agricultural nitrate pollution mitigation. To conclude, this thesis expects to provide better insight into the links between agriculture and human wellbeing, in the hopes that better- informed policy actions will, therefore, be developed that aim to boost human wellbeing. Abstract

Background: Cardiac amyloidosis (CA) could be a common cause of heart failure (HF). The objective of the study was to estimate the prevalence of CA in patients with HF. Methods: Observational, prospective, and multicenter study involving 30 Spanish hospitals. A total of 453 patients ≥ 65 years with HF and an interventricular septum or posterior wall thickness > 12 mm were included. All patients underwent a 99mTc-DPD/PYP/HMDP scintigraphy and monoclonal bands were studied, following the current criteria for non-invasive diagnosis. In inconclusive cases, biopsies were performed. Results: The vast majority of CA were diagnosed non-invasively. The prevalence was 20.1%. Most of the CA were transthyretin (ATTR-CM, 84.6%), with a minority of cardiac light-chain amyloidosis (AL-CM, 2.2%). The remaining (13.2%) was untyped. The prevalence was significantly higher in men (60.1% vs 39.9%, p = 0.019). Of the patients with CA, 26.5% had a left ventricular ejection fraction less than 50%. Conclusions: CA was the cause of HF in one out of five patients and should be screened in the elderly with HF and myocardial thickening, regardless of sex and LVEF. Few transthyretin-gene-sequencing studies were performed in older patients. In many patients, it was not possible to determine the amyloid subtype. [ABSTRACT FROM AUTHOR]

37. Anakinra versus Baricitinib: Different Strategies for Patients Hospitalized with COVID-19

Abstract

Background: Immunomodulatory drugs have been used in patients with severe COVID-19. The objective of this study was to evaluate the effects of two different strategies, based either on an interleukin-1 inhibitor, anakinra, or on a JAK inhibitor, such as baricitinib, on the survival of patients hospitalized with COVID-19 pneumonia. Methods: Individuals admitted to two hospitals because of COVID-19 were included if they fulfilled the clinical, radiological, and laboratory criteria for moderate-to-severe disease. Patients were classified according to the first immunomodulatory drug prescribed: anakinra or baricitinib. All subjects were concomitantly treated with corticosteroids, in addition to standard care. The main outcomes were the need for invasive mechanical ventilation (IMV) and in-hospital death. Statistical analysis included propensity score matching and Cox regression model. Results: The study subjects included 125 and 217 individuals in the anakinra and baricitinib groups, respectively. IMV was required in 13 (10.4%) and 10 (4.6%) patients, respectively (p = 0.039). During this period, 22 (17.6%) and 36 (16.6%) individuals died in both groups (p = 0.811). Older age, low functional status, high comorbidity, need for IMV, elevated lactate dehydrogenase, and use of a high flow of oxygen at initially were found to be associated with worse clinical outcomes. No differences according to the immunomodulatory therapy used were observed. For most of the deceased individuals, early interruption of anakinra or baricitinib had occurred at the time of their admission to the intensive care unit. Abstract

Aging treatments in water and in air and controlled neutron irradiation were performed on commercial crosslinked low-density polyethylene (XLPE) for promoting different mesostructural arrangements of crystallites and crosslinking degree. Infrared spectroscopy, differential scanning calorimetry, differential thermal analysis, thermogravimetry, and dynamic mechanical analysis were used as characterization techniques. The relaxation peak related to the mobility of the grain boundaries from crystalline zones in XLPE was identified at around 260 K (at 7 Hz), involving an activation energy of 90 ± 4 kJ mol −1 . The usual equation for describing the grain boundary mobility in metals involving the movement of grain boundary dislocations was adapted for studying the mobility of the boundaries among the crystalline zones, successfully. In addition, a new mechanical relaxation peak that appears at around 300 K (at 7 Hz), which involves an activation energy of 94 ± 5 kJ mol −1 , was found. The driving force controlling this peak was determined as the dragging of the polymer chains at the amorphous zones adjacent to the crystals controlled by the mobility of the crystallites boundaries. Abstract

Triple negative breast cancer (TNBC) is a heterogeneous disease representing about 15% of all breast cancers. TNBC are usually high-grade histological tumors, and are generally more aggressive and difficult to treat due to the lack of targeted therapies available, and chemotherapy remains the standard treatment. There is a close relationship between pathological complete response after chemotherapy treatment and higher rates of disease-free survival and overall survival. In this review of systemic treatment in early triple negative breast cancer, our purpose is to analyze and compare different therapies, as well as to highlight the novelties of treatment in this breast cancer subtype.
