Abstract

Abstract Background Type 1 Brugada syndrome (BrS) is a hereditary arrhythmogenic disease showing peculiar electrocardiographic (ECG) patterns, characterized by ST-segment elevation in the right precordial leads, and risk of Sudden Cardiac Death (SCD). Furthermore, although various ECG patterns are described in the literature, different individual ECG may show high-grade variability, making the diagnosis problematic. The study aims to develop an innovative system for an accurate diagnosis of Type 1 BrS based on ECG pattern recognition by Machine Learning (ML) models and blood markers analysis trough transcriptomic techniques. Methods The study is structured in 3 parts: (a) a retrospective study, with the first cohort of 300 anonymized ECG obtained in already diagnosed Type 1 BrS (75 spontaneous, 150 suspected) and 75 from control patients, which will be processed by ML analysis for pattern recognition; (b) a prospective study, with a cohort of 11 patients with spontaneous Type 1 BrS, 11 with drug-induced Type 1 BrS, 11 suspected BrS but negative to Na + channel blockers administration, and 11 controls, enrolled for ECG ML analysis and blood collection for transcriptomics and microvesicles analysis; (c) a validation study, with the third cohort of 100 patients (35 spontaneous and 35 drug-induced BrS, 30 controls) for ML algorithm and biomarkers testing. Discussion The BrAID system will help clinicians improve the diagnosis of Type 1 BrS by using multiple information, reducing the time between ECG recording and final diagnosis, integrating clinical, biochemical and ECG information thus favoring a more effective use of available resources. Abstract

Aim: To evaluate Attain Performa (Medtronic, Dublin, Ireland) quadripolar lead performance in clinical practice and, secondarily, to compare its long term clinical outcomes vs bipolar leads for left ventricular (LV) pacing. Methods and results: We retrospectively analyzed clinical, procedural and follow-up data of 215 patients implanted with a quadripolar lead. One hundred and twenty one patients implanted with bipolar lead were selected to compare long-term clinical outcomes. The quadripolar lead was implanted in the target vein in 196 patients (91%) without acute dislodgements. In 50% of patients the chosen final pacing configuration at implant would not have been available with bipolar leads. A dedicated quadripolar pacing vector was chosen more frequently when the LV tip location was apical than otherwise (65.6% vs 42.7%, p = 0.003). After a median follow-up of 14 months, the LV pacing threshold was less than 2.5 V at 0.4 ms in 98 patients (90%) with a safety margin between phrenic nerve and LV pacing threshold >3 V in 97 patients (89%). We observed a slight trend toward a lower risk of heart failure worsening and a lower incidence of ventricular arrhythmias and pulmonary congestion in patients implanted with quadripolar leads compared with the control group. Conclusion: Quadripolar leads improve the management of phrenic nerve stimulation at no trade-off with pacing threshold and lead stability. Quadripolar leads seems to be associated with a lower incidence of VT/VF and pulmonary congestion, when compared with bipolar leads, but further investigations are necessary to confirm that this positive effect is associated with better LV reverse remodeling. Main funding source(s): This research project is funded by Tuscany Region Background/Introduction Electrocardiograms (ECGs) are rapidly moving from analog to digital versions. Consequently, a series of automatic analyses of standard 12-lead ECGs are attracting interest for their ability to support clinicians in the automatic recognition of specific features associated with different cardiac diseases [2]. Artificial Intelligence applications and Machine Learning (ML) algorithms have gained much attention in the last years for their ability to figure out patterns from data independently, without being explicitly taught rules. Peculiar features define the ECGs of patients with Brugada Syndrome (BrS); however, ambiguities still exist for the correct diagnosis of BrS and discrimination with respect to other pathologies. Purpose The BrAID (Brugada syndrome and Artificial Intelligence applications to Diagnosis) project aims to develop an innovative system for diagnosing Type 1 BrS based on ECG pattern recognition through the application of ML algorithms. In this work, an application of Echo State Networks (ESN), a type of Recurrent Neural Network (RNN), for the diagnosis of BrS from ECG is presented. Methods After approval from the Local Ethical Committees, 12-lead ECGs were obtained in patients enrolled in 5 Centers diagnosed with typical spontaneous Type 1 pattern (coved) (group A, 81 patients). Baseline ECG was also collected in patients undergoing the ajmaline test, classified as positive (group B, 37 patients) or negative (group C, 14 patients) according to test results. 174 patients with no clinical and familial history of arrhythmias were considered controls (group D). Data were collected from 4 beats extracted from the ECGs as input to the ESN. The datasets obtained in the different groups were used for the ESN model’s training and assessment (testing) through a double cross-validation approach. Results As shown in Table 1, the performances using three leads (V1, V2, V3) or V2 only were compared. The algorithm performance was assessed in all the datasets (group A+B+C+D) and in spontaneous BrS (group A) and controls (group D). A good accuracy (79.21%) was seen when the three leads were considered for groups A and D only; the best test set accuracy (80.20%) was obtained in the case in which V2 only was used as input in all the datasets. Conclusion(s) In this work, a novel system for diagnosing Type 1 BrS using an ESN approach was developed. Our preliminary results show that this ML model is able to detect ECG patterns associated with Type 1 BrS with good and comparable accuracy both when three leads (79.21% ) or V2 only (80.20%) were analyzed. The future availability of larger datasets could improve the model performance, increasing the ESN potentialities as a clinical support system tool to be used in everyday clinical practice. Table 1. 5. Four mm-long versus longer implants in augmented bone in atrophic jaws: 1-year loading RCT results

Abstract

BACKGROUND: Dental implants are used to replace missing teeth for rehabilitating edentulous patients. In many patients it is not possible to place “standard length” dental implants because of the crestal bone vertical resorption. Clinicians, therefore, are faced with the dilemma of whether to attempt an augmentation procedure or to place shorter implants. Short implants could be a simpler alternative if they could provide similar clinical outcomes to longer implants placed in augmented bone. AIM/HYPOTHESIS: The aim of this RCT was to compare the outcome of 4 mm-long implants with implants at least 10 mm-long placed in posterior jaws augmented either with a mandibular interpositional equine block or with a porcine-derived granular bone placed through a lateral window below the lifted maxillary sinus. MATERIAL AND METHODS: Forty patients with atrophic posterior mandibles having 5–6 mm bone height above the mandibular canal and 40 pa- tients with atrophic maxillae having 4–5 mm below the maxillary sinus, were randomised according to a parallel group design to receive one to three 4.0 mm-long implants or one to three implants, which were at least 10 mm-long, in augmented bone. All implants had a diameter of 4.0 or 4.5 mm. Mandibles were vertically augmented with interpositional equine bone blocks and resorbable barriers. Implants were placed 4 months after interpositional grafting. Maxillary sinuses were augmented with particulated porcine bone via a lateral window and implants were placed simultaneously. Four months later, screw-retained reinforced acrylic restorations were delivered, and then replaced after 4 months by definitive screw-retained metal-composite prostheses. Outcome measures included prosthesis and implant failures, any complication and peri-implant mar- ginal bone level changes. RESULTS: One mandible fractured and the patient did not want to go ahead with the treatment. Six implant from the augmented group failed versus five 4.0 mm implants from the short implant group. There were no statistically significant differences in implant failures (P (chi-square test) = 1.000) or prostheses failures (P (chi-square test) = 0.399). At mandibular sites, nine augmented patients were affected by complications versus one patient treated with short implants (P (chi-square test) = 0.003), with the difference being statistically significant. No significant dif- ferences were found for the maxillae: eight sinus lift patients versus three patients rehabilitated with maxillary short implants were affected by complications (P (chi-square test) = 0.077). The difference in peri-implant marginal bone loss was statistically significant in the mandibles (mean difference: −0.12 mm, P (ANCOVA) = 0.006), but not in the maxillae (mean difference: −0.02 mm, P (ANCOVA) = 0.711). CONCLUSIONS AND CLINICAL IMPLICATIONS: Four months after loading 4.0 mm-long implants achieved similar results, if not better, than longer implants in augmented jaws, but were affected by fewer complications. Short implants might be a preferable choice to bone augmentation, especially in mandi- bles, since the treatment is less invasive, faster, cheaper, and associated with less morbidity, however, 5- to 10-year post-loading data is necessary before making reliable recommendations.

6. Immediate, immediate-delayed and delayed post-extractive single implants: 1-year loading RCT data

Abstract

BACKGROUND: Osseointegrated dental implants are traditionally placed in healed ridges. Immediate post-extractive implants and immediate- delayed are those implants placed immediately after tooth extraction and after 2–6 weeks, respectively. The main advantage is to shorten treatment and to presumably decrease the naturally occurring bone resorption after tooth extraction. To the best knowledge of the authors there are no RCTs comparing all the three procedures. AIM/HYPOTHESIS: The aim of this RCT was to compare the clinical outcome of single implants placed immediately after tooth extraction with im- plants placed 6 weeks after tooth extraction (immediate-delayed placement), and with implants placed after 4 months of socket healing (delayed placement). MATERIAL AND METHODS: Two hundred and ten patients requiring a single implant-supported crown to replace a tooth to be extracted were randomised to receive immediate post-extractive implants (70 patients), immediate-delayed implants at 6 weeks (70 patients), and delayed implants after 4-month healing (70 patients) according to a parallel group design. When needed, patients had the socket grafted with a bone substitute and covered with a resorbable membrane. Implants inserted with at least 25 Ncm torque were left to heal unloaded for 4 months, whereas those inserted with less than 25 Ncm were left to heal unloaded for 6 months. Temporary crowns were delivered and were to be replaced by definitive ones after 4 months. Outcome measures were crown and implant failures, complications, peri-implant marginal bone level changes, aesthetic assessed using the Pink Esthetic Score (PES), and patient satisfaction recorded by blinded assessors. Patients were followed up to 1 year post-loading. RESULTS: Four implants (6%) failed in the immediate, four (6.2%) in the immediate-delayed, and one (1.6%) in the delayed group (P (chi-square test) = 0.369). Six immediate, six immediate-delayed and four delayed implants were affected by one complication each (P (chi-square test) = 0.792). Mean peri-implant marginal bone loss after 1 year was −0.25 ± 0.17 mm (CI95% −0.29, −0.20) at immediate, −0.29 ± 0.14 mm (CI95% −0.32, −0.25) at immediate-delayed, and −0.31 ± 0.16 mm (CI95% −0.35, −0.27) at delayed-placed implants (P (Kruskal Wallis test) = 0.015). One year after loading, the mean total aesthetic score was 12.52, 12.49 and 11.78 at the immediate, immediate-delayed and delayed groups, respectively (P (Kruskal Wallis test) < 0.001). All patients were fully satisfied both for function and aesthetics, and would undergo the same procedure again, with four exceptions (partially satisfied with aesthetics) (P = 0.785). CONCLUSIONS AND CLINICAL IMPLICATIONS: No statistically significant differences for failures, complications and patient satisfaction were observed when placing single implants immediately, 6 weeks or four months after tooth extraction, nevertheless failures were more frequent at immediate and immediate-delayed placed implants. Bone level changes were similar between the different procedures, but aesthetics showed better results at immediate and immediate-delayed implants.

Abstract

This article reports a measurement of the production cross section of prompt isolated photon pairs in proton-antiproton collisions at √s=1.96TeV using the CDF II detector at the Fermilab Tevatron collider. The data correspond to an integrated luminosity of 5.36fb -1. The cross section is presented as a function of kinematic variables sensitive to the reaction mechanisms. The results are compared with three perturbative QCD calculations: (1) a leading-order parton shower Monte Carlo, (2) a fixed next-to-leading-order calculation and (3) a next-to-leading-order/next-to-next-to-leading-log resummed calculation. The comparisons show that, within their known limitations, all calculations predict the main features of the data, but no calculation adequately describes all aspects of the data. © 2011 American Physical Society.

Abstract

Background: Multiple recession defects can be successfully treated using envelope-type coronally advanced flaps. The aim of the present study was to evaluate the long-term (5 years) stability of clinical outcomes achieved with the surgery and the association between patient variables and long-term stability. Methods: Seventy-three Miller Class I and H gingival recessions affecting 22 young, systemically healthy subjects were treated with coronally advanced flaps with no releasing incisions. All patients were instructed to perform a coronally directed roll technique to minimize the toothbrushing trauma to the gingival margin. The clinical reevaluation was made I year after the surgery. At this point, 13 patients took part in a supportive periodontal care program consisting of oral hygiene instructions, control of toothbrushing technique, and professional tooth cleaning every 4 months. The remaining nine patients did not participate and received only sporadic care by general dentists. At 5 years post-surgery, all patients were reexamined. Results: At the 5-year examination, 94% of the root surfaces initially exposed due to gingival recession were still covered with soft tissue, and 85% of the treated recession defects showed complete coverage. Complete root coverage in all recessions was maintained in 15 out of 22 patients (68%). The long-term stability of the soft-tissue margin in the treated sites was significantly influenced by the patient's regular participation in the recall program and the susceptibility to gingival recession in other areas of the mouth. A statistically significant increase of keratinized tissue (0.80 +/- 0.64 mm) was observed between the 1- and 5-year observation visits, and the average increase of keratinized tissue between the baseline and the 5-year follow-up amounted to 1.38 +/- 0.90 mm. This increase was significantly affected by the baseline keratinized tissue (KT) and recession (REC) depth: in particular, the 5-year increase in the amount of keratinized tissue was greater in sites with a greater recession depth and lower amount of keratinized tissue at baseline. Conclusions: 1) The successful root coverage results obtained with the coronally advanced flap for multiple recession defects were well maintained over the 4-year observation period. 2) Negative patient characteristics such as a lack of compliance with a supportive care program and individual susceptibility to gingival recession were significantly associated with the recurrence in gingival recession. 3) The increase in keratinized tissue height that followed the coronally advanced flap procedure may be attributed to the tendency of the mucogingival line to regain its genetically determined position.

Abstract

BACKGROUND: Unfavorable conditions at the soft tissues adjacent to a recession defect may preclude performing pedicle flaps (advanced or rotational) both as a root coverage procedure, and as a covering flap for a connective tissue graft. Free gingival grafts may not be recommended because of the low root coverage predictability and the poor esthetic outcome. The goal of the present case report is to suggest modifications of the two-stage surgical technique aimed at improving root coverage and esthetic outcomes, and reducing patient morbidity. METHOD: In the first case report, a Miller class II gingival recession, associated with a deep buccal probing depth, affecting a lower central incisor was treated. In the first step of the surgery an epithelized graft with an apical-coronal dimension equal to the keratinized tissue height of the adjacent teeth was sutured apical to the bone dehiscence. Four months later, a coronally advanced flap was performed to cover the root exposure. In the second case report, a Miller class III gingival recession, complicated with a deep buccal probing depth affecting the mesial root of the first lower molar was treated. In the first step of the surgery, a free gingival graft was positioned mesially to the root exposure to create keratinized tissue lateral to the recession defect. This was adequate to perform the laterally moved, coronally advanced flap that was used as a second-step root coverage surgical procedure. RESULTS: In the first case report complete root coverage, an increase (4 mm) in keratinized tissue height and realignment of the mucogingival line were achieved 1 year after the surgery. The reduced dimension of the graft permitted to minimize patient's discomfort and to obtain good esthetics of mucogingival tissues. These successful outcomes were well maintained for 5 years. In the second case report successful root coverage, increase (3 mm) in keratinized tissue height and good harmony of mucogingival tissues were achieved 1 year after the surgery. These outcomes were well maintained 5 years after the surgery. CONCLUSIONS: The present study suggested that modifications of the two-stage procedure by minimizing the dimension of the graft and by standardizing the surgical techniques allowed successful results to be achieved in the treatment of gingival recessions characterized by local conditions that otherwise preclude the use of one-step root coverage surgical techniques.

Abstract

Aim To evaluate the clinical outcomes of the use of a xenogeneic collagen matrix (CM) in combination with the coronally advanced flap (CAF) in the treatment of localized recession defects. Material & Methods In a multicentre single-blinded, randomized, controlled, split-mouth trial, 90 recessions (Miller I, II) in 45 patients received either CAF + CM or CAF alone. Results At 6 months, root coverage (primary outcome) was 75.29% for test and 72.66% for control defects (p = 0.169), with 36% of test and 31% of control defects exhibiting complete coverage. The increase in mean width of keratinized tissue (KT) was higher in test (from 1.97 to 2.90 mm) than in control defects (from 2.00 to 2.57 mm) (p = 0.036). Likewise, test sites had more gain in gingival thickness (GT) (0.59 mm) than control sites (0.34 mm) (p = 0.003). Larger (≥3 mm) recessions (n = 35 patients) treated with CM showed higher root coverage (72.03% versus 66.16%, p = 0.043), as well as more gain in KT and GT. Conclusions CAF + CM was not superior with regard to root coverage, but enhanced gingival thickness and width of keratinized tissue when compared with CAF alone. For the coverage of larger defects, CAF + CM was more effective.

Abstract

Mucogingival esthetic surgery is dedicated to the treatment of mucogingival esthetic alterations. These may be caused by gingival recession, often in association with non caries cervical lesions and/or root caries or, on the contrary, by altered passive eruption. Objectives of mu- cogingival esthetic surgery are also to augment gingival thickness and height around prostheses or implants and soft tissue augmentation procedures in edentulous ridg- es. Techniques for edentulous ridge augmentation are addressed in the second volume. This volume describes and illustrates mucogingival surgical techniques as ap- plied to natural teeth or implants with the chief aim of satisfying the patient’s esthetic requirements.

Abstract

We present a search for the standard model Higgs boson produced in association with a W boson in root s = 1.96 TeV p (p) over bar collision data collected with the CDF II detector at the Tevatron corresponding to an integrated luminosity of 9.45 fb(-1). In events consistent with the decay of the Higgs boson to a bottom-quark pair and the W boson to an electron or muon and a neutrino, we set 95% credibility level upper limits on the WH production cross section times the H -> b (b) over bar branching ratio as a function of Higgs boson mass. At a Higgs boson mass of 125 GeV/c(2), we observe (expect) a limit of 4.9 (2.8) times the standard model value.

Abstract

Aim To compare the clinical outcomes of laterally moved, coronally advanced flap (LMCAF) versus Bilaminar technique (BT) in the treatment of single gingival recession on molar teeth. Material and Methods Fifty patients showing Miller I and II gingival recessions at first molar teeth were treated: 25 were randomly assigned to the BT group and 25 belonged to the LMCAF group. Patient's post-operative morbidity was assessed 1 week after the surgery, while aesthetic evaluation and the clinical evaluation were made 1 year later. Results No statistically significant difference was demonstrated in terms of recession and PPD reduction. Statistically greater probability of complete root coverage (CRC, Odds Ratio 22.1) and greater increase in gingival thickness were observed in the BT group. Greater increase in keratinized tissue was obtained in the LMCAF. Patient satisfaction with aesthetics was very high in both treatment groups. Better post-operative course was observed in the LMCAF, while better post-operative sensitivity and root coverage evaluation were demonstrated in patients treated with BT. Conclusions Gingival recession at first molar teeth can be successfully treated with LMCAF and BT. Abstract

The presence of a localized alveolar ridge defect, especially in the maxillary anterior dentition, may complicate an esthetic rehabilitation. The goal of this case report is to describe a novel subepithelial connective tissue graft technique for soft tissue augmentation in Class III ridge defects. Surgical intervention consisted of in situ maintenance of a connective tissue "platform" at the edentulous space, which facilitated the stabilization and suturing of the connective tissue grafts used for soft tissue augmentation. Adequate graft thickness to treat the deep horizontal soft tissue loss was obtained by doubling the width of a de-epithelialized free gingival graft that was subsequently folded on itself. The soft tissue conditioning at the level of the pontic began 9 months after surgery by shaping the soft tissue with a bur and filling the space with flowable composite resin applied above the pontic. The final prosthetic phase began 14 months after surgery. A reproduction of the anatomical cementoenamel junction in the provisional and definitive restorations was performed to improve the soft tissue emergence profile. Nine months after surgery, a soft tissue augmentation of 5 mm in the vertical and 4 mm in the horizontal dimension was accomplished. The suggested surgical technique was able to accomplish horizontal and vertical soft tissue augmentation in a single surgical step.

Abstract

We report a measurement of the bottom-strange meson mixing phase βs using the time evolution of Bs0→J/ψ(→μ+μ-)ϕ(→K+K-) decays in which the quark-flavor content of the bottom-strange meson is identified at production. This measurement uses the full data set of proton-antiproton collisions at s=1.96 TeV collected by the Collider Detector experiment at the Fermilab Tevatron, corresponding to 9.6 fb-1 of integrated luminosity. We report confidence regions in the two-dimensional space of βs and the Bs0 decay-width difference ΔΓs and measure βs∈[-π/2,-1.51]∪[-0.06,0.30]∪[1.26,π/2] at the 68% confidence level, in agreement with the standard model expectation. 27. Combined orthodontic-mucogingival treatment of a deep post-orthodontic gingival recession

Abstract

In this article, the interdisciplinary management of an isolated-type recession defect in a severely compromised mandibular incisor of a young post-orthodontic patient is described. The prognosis of root coverage surgery was very questionable and unpredictable due to the severe root malposition (III Miller class gingival recession). The treatment plan consisted of: (1) interproximal enamel reduction to gain space within the dental arch, (2) orthodontic repositioning of the root of the affected tooth within the alveolar bone and (3) root coverage mucogingival surgery. Clinical re-evaluation 7 months after fixed orthodontic treatment revealed major improvements in the root coverage prognosis due to the resolution of root malposition and de novo formation of keratinized tissue apical to the root exposure (I Miller class gingival recession). A subepithelial connective tissue graft was performed as a root coverage surgical procedure. Clinical examination 1 year after surgery revealed complete root coverage, good color blending with adjacent soft tissue and an increase in facial gingival thickness. Successful periodontal and esthetic outcomes can be accomplished after the combined orthodontic-periodontal treatment of a severely mucogingivally compromised tooth.

Abstract

The coronally advanced flap has been documented as an effective surgical technique for the treatment of gingival recessions in cases of multiple adjacent recession defects, obtaining stable long-term results after 5 years of follow-up. The aim of the present study was to test the coronally advanced flap, in association with a connective tissue graft, in a case series of 10 patients presenting at least two gingival recessions on adjacent teeth in the posterior mandibular area. Periodontal parameters were recorded on teeth involved in the surgeries at baseline and 1 year. A total of 26 recessions were treated. No significant complication affected the surgeries, and no patient abandoned the study. Recession depth was reduced from 3.40 ± 0.83 mm at baseline to 0.28 ± 0.32 mm at the 1-year control, while differences in pocket depth were not significant. Keratinized tissue increased from 0.57 ± 0.46 mm to 3.05 ± 0.71 mm. Greater reductions in recession depth were observed in cases where the initial conditions were worse. A mean 91.2% ± 4.1% recession coverage was obtained. The coronally advanced flap in association with a connective tissue graft can be proposed as a valid therapeutic approach for multiple recession defects in mandibular posterior areas.

Abstract

We present the first measurement of polarization and CP-violating asymmetries in a Bs0 decay into two light vector mesons, B s 0→φφ, and an improved determination of its branching ratio using 295 decays reconstructed in a data sample corresponding to 2.9fb -1 of integrated luminosity collected by the CDF experiment at the Fermilab Tevatron collider. The fraction of longitudinal polarization is determined to be f L=0.348±0.041(stat)±0.021(syst), and the branching ratio B(Bs0→φφ)=[2.32±0.18(stat)±0. 82(syst)]×10 -5. Asymmetries of decay angle distributions sensitive to CP violation are measured to be A u=-0.007±0. 064(stat)±0.018(syst) and A v=-0.120±0.064(stat) ±0.016(syst). © 2011 American Physical Society.

Abstract

The gingival complex plays a major role in the overall beauty of a smile. To predictably achieve a successful aesthetic and functional result, the dentist must be able to precisely pre- dict the treatment outcome based on biologic determinants. The diagnosis and treatment of the “gummy smile” help the periodontist to provide the patients with the most beauti- ful smile possible. The nature of a high smile-line can be: dento-gingival, muscular, dento-alveolar (skeletal) or of a mixed nature. Excessive gingival display, frequently seen in adults and resul- ting in short clinical crowns, has been described in the lite- rature as “altered passive eruption”. It exists in adults when the gingival unit remains positioned on the convex promi- nence of the enamel rather than at or in proximity of the cemento-enamel junction.

Abstract

New materials (restorative and adhesive) for the treatment of cervical abrasions have produced better results in terms of esthetics (choice and stability of colors) and duration (marginal fit and wear resistance). Nevertheless, conservative restoration of cervical abrasion cannot be considered the most suitable treatment in certain clinical situations: (1) when the abrasion defect involves the root surface, either exclusively or primarily; (2) when a site has difficult esthetic demands resulting from excessive tooth length with gingival recession; and (3) in the presence of root caries. The aim of this case report is to describe the application of the bilaminar technique to treat a deep cervical abrasion associated with a recession-type defect. The bilaminar surgical approach shown here consisted of a connective tissue graft covered by a coronally advanced pedicle flap. The connective tissue graft was placed inside the root concavity to compensate the abrasion space and to prevent soft tissue flap collapse internally. The graft, by acting as a “biologic filler” or space maintainer inside the concave abrasion area, stabilized the covering flap and helped restore a correct tooth emergence profile.

Abstract

Recognizing the treatment factors that will increase the predictability of regenerative surgical procedures may encourage periodontal treatment of apparently hopeless teeth. This case report describes the long-term clinical outcome of a periodontal (15 mm of clinical attachment loss) and bone defect associated with a periapical lesion affecting a maxillary lateral incisor in an esthetically demanding, systemically healthy young patient. Treatment procedures comprised endodontic treatment of the periapical lesion and cause-related therapy aimed to eliminate bleeding on probing and control the infection in the rest of the mouth. Four months later, a regenerative surgical technique combining the simplified papilla preservation approach, application of enamel matrix proteins and bovine porous bone mineral, and a coronally advanced buccal flap at the adjacent teeth in the surgical area was performed. Clinical examination at the 1-year recall revealed clinical attachment gain (8 mm) with shallow residual probing pocket depths (3 mm) and a slight (1 mm) increase in gingival recession. The esthetic appearance of the treated tooth was improved via the addition of odontoplastic and resin composite. Three years postsurgery, both the esthetics and the periodontal health were fully maintained. The present case report suggests that successful periodontal and esthetic results can be accomplished and maintained for at least 3 years after regenerative treatment of an apparently hopeless tooth with extremely severe baseline periodontal conditions.

Abstract

Aim The purpose of this study was to assess the ability of enamel matrix derivative (EMD) to improve root coverage with a coronally advanced flap (CAF) during a 2-year follow-up. Methods Fifteen patients each with two single and similar bilateral Miller Class I or II gingival recessions (30 recessions) were selected. Each recession was randomly assigned to the test group (CAF+EMD) or the control group (CAF only). Clinical parameters recorded at baseline and at 6, 12 and 24 months were recession depth (R), recession width (WR), probing depth (PD), clinical attachment level (CAL) and keratinized tissue (KT). Results Reduction of R resulted in a significant CAL gain in both groups, whereas PD was not altered. In the test group, R decreased from 4.07 mm (SD+/-0.59) at baseline to 0.47 mm (SD+/-0.74) at 24 months, corresponding to a mean root coverage (MRC) of 90.67%, whereas in the control group R shrank from 4.13 mm (SD+/-0.74) at baseline to 0.60 mm (SD+/-0.83) at 24 months (MRC=86.67%). Complete root coverage was achieved at 24 months in 73.33% and 60% of the two groups. A significant KT increase was observed in both groups. Conclusions Root coverage outcomes were similar in both groups and no statistically significant differences were found at all between them. Hence, the additional use of EMD to CAF is not justified for clinical benefits of root coverage, but as an attempt of achieving periodontal regeneration rather than repair.

Abstract

A modified surgical approach to interproximal guided tissue regeneration procedures (GTR) was used when anatomic conditions at the defect-associated interdental area rendered papilla preservation techniques very difficult. The main goal of this study was to evaluate the effectiveness of the papilla amplification flap (PAF) in obtaining and maintaining primary soft tissue closure of the interdental space above nonresorbable, titanium-reinforced, expanded polytetrafluoroethylene (e-PTFE) membranes and to quantify the regenerative outcomes obtained using this procedure. Seventeen patients with one deep intrabony defect associated with a narrow interproximal space were selected for this case-series clinical study. The application of the PAF in combination with e-PTFE membranes resulted in clinically and statistically highly significant gains in clinical attachment levels (4.7 +/- 1.4 mm) and reductions in probing pocket depth (6.3 +/- 1.3 mm) after 1 year. Primary soft tissue closure of the interdental space was obtained in 100% of cases after completion of the surgery and maintained in 65% of cases during the initial healing period (6 weeks). Results from the present study indicate that the PAF can be considered a suitable soft tissue surgical approach for GTR treatment of intrabony defects when papilla preservation techniques are not recommended because of unfavorable local anatomic conditions interproximally.

Abstract

The objective of this study was to evaluate the relationship between integrated connective tissue (ICT), that is, the presence of connective tissue into the membrane structure, and the clinical outcome of membrane-supported periodontal surgery. Twenty-six systemically healthy subjects affected by chronic adult periodontitis were enrolled in the study. One tooth site per patient, associated with an angular bony defect and an attachment loss of > 7 mm, was selected to be treated by means of a guided tissue regeneration procedure using a bioabsorbable membrane. Barrier material was surgically removed after 4 weeks for SEM analysis. For each treated site, the difference in clinical attachment loss, probing depth, and gingival recession between the baseline examination and follow-up 6 months after the second surgery was calculated. Gain of attachment was statistically (P < 0.001) greater in sites with no membrane exposure when compared to sites with exposed barrier material (5.5 +/- 1.0 vs. 4.0 +/- 0.6), while further gingival recession was greater (3.0 +/- 0.9 vs. 2.1 +/- 0.5) in sites with clinically exposed membranes. The results of SEM analysis revealed that when connective tissue structures were observed on membrane surfaces, no bacteria could be detected; conversely, areas heavily colonized by bacteria did not show the presence of connective tissue. Regression analysis indicated that integrated connective tissue on the external layer of the barrier material was positively correlated with the amount of attachment gain and negatively with the amount of gingival recession. Bacterial colonization of the membrane was negatively correlated with attachment gain and positively with gingival recession. It was concluded that connective tissue integration is an important biological phenomenon in preventing membrane exposure and bacterial plaque colonization and thus in enhancing the clinical outcome following guided tissue regeneration surgery.

Abstract

The objective of the study was to evaluate bacterial colonization of the tooth-facing surface of bioabsorbable membranes and to determine its effect on the clinical outcome of membrane supported reconstructive periodontal surgery. Twenty systemically healthy subjects affected by chronic adult periodontitis were enrolled in the study. One non-furcation tooth site per patient, associated with an angular bony defect and a probing attachment loss of > 5 mm, was selected to be treated by means of a guided tissue regeneration procedure using a polyglicolactic membrane. Antibiotics (amoxicillin/clavulanate potassium 1 g per day) for 2 weeks were prescribed, in addition to the use of chlorhexidine for post-surgical plaque control. All patients were recalled once a week for 5 weeks for professional tooth cleaning. At 5 weeks sites with clinically exposed membranes underwent a second surgery to harvest residual barrier material which was analyzed by scanning electronic microscopic (SEM) for bacterial colonization. Sites with no membrane exposure at 5 weeks were allowed to heal without any other surgical intervention. Professional tooth cleaning and reinforcement of self-performed oral hygiene measures were given at 1 month intervals for the duration of the study. For each treated site the difference in probing attachment loss between baseline examination and a follow-up examination made 6 months after the second surgery was calculated. Gain of probing attachment was statistically (P < 0.001) greater in sites with no membrane exposure when compared to sites with partially exposed barrier material (4.2 +/- 0.5 vs. 3.3 +/- 0.6). The results of SEM analysis revealed that bacterial colonization was evident in all the microscopic fields of the exposed areas of the membranes. In the mid-part of the membranes 16 out of 39 microscopic fields (41%) demonstrated microbial colonization, while no bacteria-positive field was observed in the most apical portion of the membranes. Regression analysis indicated that gain in probing attachment level was negatively correlated to microbial colonization of the mid-part of the membranes. It was suggested the midportion of the tooth-facing surface of polyglicolactic membrane is a critical area for the healing process since its bacterial colonization was detrimental to the outcome of the GTR surgery.

Abstract

In this paper, we present CardioMat, a Matlab toolbox for cardiac electrophysiology simulation based on patient-specific anatomies. The strength of CardioMat is the easy and fast construction of electrophysiology cardiac digital twins from segmented anatomical images in a general-purpose software such as Matlab. CardioMat implements a quasi-automatic pipeline that guides the user toward the construction of anatomically detailed cardiac electrophysiology models. Importantly, the CardioMat framework includes the generation of physiologically plausible fiber orientation and Purkinje networks. The main novelty of our framework is its ability to handle voxel-based geometries as produced by segmentation procedures directly, without the need for an unstructured mesh. Indeed, the CardioMat monodomain solver uses a smoothed boundary approach and runs completely on GPU for fast simulations. We employed CardioMat in different application scenarios to show its potentialities and provide preliminary assessment of the feasibility, diagnostic performance, and accuracy of the toolbox. 42. Leadless Pacemaker Implantation Following Lead Extraction in a Patient With Recent Percutaneous Tricuspid Valve Repair.

Abstract

This case details the successful implantation of a leadless pacemaker following the extraction of transvenous leads in a 72-year-old female patient with a complex cardiovascular history. The patient had undergone a series of cardiac interventions, including a recent percutaneous tricuspid valve repair with a metal clip implant due to severe regurgitation. After presenting with an infection at the pacemaker site, methicillin-resistant Staphylococcus hominis was identified, necessitating the removal of the entire pacing system. A leadless pacemaker was subsequently implanted without complications, despite the recent tricuspid valve repair. This case highlights the efficacy and safety of leadless pacemakers in complex clinical scenarios, especially when traditional transvenous systems are no longer viable due to infections or other complications. The report underscores the importance of personalized management strategies for cardiac device infections, illustrating that leadless pacemakers offer a feasible and safe alternative even in patients with previous valvular interventions. TLE scenario is constantly evolving and it is a key procedure in lead management strategies. Leadless pacemaker (LP) implant is an effective and safe procedure in patients still requiring a ventricular pacing after TLE, with similar electrical performance and outcome compared with naïve patients at long-term follow-up. 43. Excess cement and peri-implant disease: A cross-sectional clinical endoscopic study.

Abstract

Background: Crown cementation is a common technique for implant-supported prosthodontics. However, for possible slipping of the cement below the mucosal margin, its thorough removal poses some issues. The objective of this study was to evaluate the presence of submucosal cement residues in patients with peri-implant disease by endoscopic visualization and to investigate the potential correlation between the pathological scenario and the spatial position of cement residues., Methods: The study sample comprised 46 consecutive patients exhibiting clinical and radiographic signs of peri-implant disease around cement-retained crowns. When at first visit bleeding on probing was the only pathological sign, the area was debrided and then treated with antiseptic agents for 30 days. Only those patients for whom bleeding was still found at re-evaluation were included in the study. All implants were therefore endoscopically evaluated to identify possible submucosal cement residues. For all implants showing residual cement, the spatial position of the residual cement was recorded with respect to predefined points., Results: Cement residues were detected in 80.4% of the patients and were predominantly located in the vestibular and lingual areas (88%). Analysis of the distances between the apical portion of the residues and anatomical landmarks revealed significant differences between mucositis and peri-implantitis patients (p = 0.001). In cases of peri-implantitis, the cement residue was more apically located than in mucositis., Conclusion: The presence of cement residue was associated with peri-implant disorders in the majority of patients. A potential correlation between the position of residues and the peri-implant disease scenario is here suggested., Plain Language Summary: A common method for fixing the crown on the dental implant is the use of specific cements. However, these products can slip below the gum line resulting in difficulties in their removal, which could cause peri-implant disease. This study aimed to evaluate the presence and location of residual cement hidden under the gums in patients with peri-implant disease, characterized by symptoms such as bleeding gums and, in some cases, bone loss around the dental implant. Using a small fiber-optic camera (endoscope), 46 patients were examined for any cement residue under the gums. The study showed that more than 80% of patients had cement residues, especially in the areas facing the cheeks and tongue. It was also found that in patients with a more severe form of inflammation (peri-implantitis), the cement was placed deeper than in those with a milder form (mucositis). This confirms that residual cement is common in patients with peri-implant disease, but also suggests that its location could influence the severity of the pathological manifestation. Abstract

Background: Personalized radiofrequency (RF) ablation for paroxysmal atrial fibrillation (PAF), adapting the ablation index (AI) to local left atrial wall thickness (LAWT), proved to be highly efficient maintaining high arrhythmia-free survival rates. However, multicentre data are lacking. This multicentre, prospective, non-randomized study was conducted at 5 tertiary hospitals and sought to assess the safety, efficacy, and reproducibility of the LAWT-guided ablation for PAF., Methods: Consecutive patients referred for first-time PAF were prospectively enrolled. The LAWT maps were obtained from preprocedural multidetector computed tomography and integrated into the navigation system. AI was titrated according to the local LAWT, and the ablation line was personalized to avoid the thickest regions while encircling the pulmonary veins (PVs)., Results: A total 109 patients (60.1 ± 9.4 years, 64.2% male) were enrolled. Median procedure time was 61.7 min (48.4-83.8), fluoroscopy time was 1.0 min (0.4-3.3), and RF time was 13.9 min (12.3-16.8). Median AI tailored to the local LAWT was 393 (374-412) for the anterior wall and 340 (315-378) for the posterior wall. Right and left PVs first-pass isolation was achieved in 89% and 91.7% of the patients, respectively. At 12-month follow-up, freedom from any atrial arrhythmia was 93.4% (95% CI 88.7-98.1), without differences across centres (P = 0.169). One patient experienced femoral artery pseudoaneurysm, with no other serious procedural-related complication., Conclusion: The Ablate-by-LAWT study proved that LAWT-guided PV isolation for PAF is safe, effective, and efficient in a multicentre setting. Abstract

Background: The combination of highly localized impedance (LI) and contact force (CF) may improve tissue characterization and lesion prediction during radiofrequency (RF) pulmonary vein isolation (PVI) in patients with atrial fibrillation (AF)., Objective: We report the outcomes of our acute and long-term clinical evaluation of CF-LI-guided PVI in consecutive AF ablation cases from an international multicenter clinical setting., Methods: Three hundred twenty-four consecutive patients from 20 European centers undergoing RF catheter ablation with the Stablepoint™ catheter were enrolled in the CHARISMA registry. Of these, 275 had a minimum follow-up of 1 year and were included in the primary analysis., Results: The mean procedure duration was 115 ± 47 min, and the mean fluoroscopy time was 9.9 ± 6 min. At the end of the procedures, all PVs had been successfully isolated in all study patients. Minor complications were reported in 12 patients (4.4%). At 1 year, 36 (13.1%) patients had had an AF recurrence, and freedom from antiarrhythmic drugs and AF recurrence was achieved in 228 (82.9%) patients. The recurrence rate was higher in patients with persistent AF (21/116, 18.1%) than in those with paroxysmal AF (15/159, 9.4%; p = 0.0459). On multivariate logistic analysis adjusted for baseline confounders, only time > 6 months from first diagnosis of AF to ablation (HR = 2.93, 95%CI 1.03 to 8.36, p = 0.0459) was independently associated with recurrences., Conclusion: An ablation strategy for PVI guided by CF-LI technology proved safe and effective and resulted in a low recurrence rate of AF over 1-year follow-up, irrespective of the underlying AF type., Clinical Trial Registration: Catheter Ablation of Arrhythmias with a High-Density Mapping System in Real-World Practice. 47. The Papilla Base CTG: A Novel Approach for Interdental Soft Tissue Reconstruction.

Abstract

Impairment or loss of interdental papilla is a common issue in patients with periodontal disease, leading to phonetic, functional, and aesthetic concerns. Numerous techniques have been explored to reconstruct and regenerate interdental papillae, but consistent success remains challenging. This article presents a novel surgical approach that applies the principles of the Connective Tissue Graft (CTG) wall technique to enhance papilla volume when interdental clinical attachment loss is present in the aesthetic zone. The case of a 35-year-old woman with an RT3 recession defect associated with loss of interdental hard and soft tissues is discussed. The patient underwent a procedure involving palatal incisions, application of amelogenins, and a trapezoidal shape CTG fixed at the base of the papilla under a coronally advanced flap. This approach aimed to stabilize the blood clot and prevent soft tissue collapse into the defect area, enhancing the position and volume of the interdental papilla. Results at 6- and 12-months follow-up indicated significant improvement in papilla appearance and complete root coverage. This case suggests that the modified CTG wall technique can effectively treat buccal and interdental gingival recessions associated with horizontal or infrabony defects. Further clinical trials are necessary to confirm these findings and establish the most effective approach for interdental papilla reconstruction.

49. Implant-Supported Vertical Soft Tissue Augmentation for Severe Hard and Soft Tissue Deficiencies at Malpositioned Implants: A Case Report of a Mucogingival and Restorative Approach.

Abstract

The management of severe mucogingival deformities surrounding malpositioned implants represents a complex issue with a high risk of aesthetic failure. The present case report describes a mucogingival and restorative approach for the treatment of severe localized peri-implant tissue deficiencies with interproximal bone loss combined with an interproximal gingival recession on the adjacent natural tooth. This consists of maintaining a non-restorable malpositioned implant, submerging it through one or multiple vertical soft tissue augmentations, according to the defect severity, and delivering a tooth supported bridge involving the adjacent natural teeth. The step-by-step surgical technique adopted for implant submergence and vertical soft tissue grafting, as well as the subsequent surgical and prosthetic interventions, are described, presenting results at 5 years from implant submergence and 3 years from prosthetic finalization.

Abstract

Background and Objectives : The influence of the quantity and quality of peri-implant soft tissue on implant health and long-term maintenance is controversial. This consensus aimed to assess the importance of peri-implant soft tissue by analyzing four aspects: the role of keratinized mucosa (KM), the efficacy of specific collagen matrix, the influence of abutment material, and soft-tissue thickness. Materials and Methods : Active members of the Italian Academy of Osseointegration (IAO) participated in the consensus. Four systematic reviews were conducted, and their results were discussed to provide guidelines on the importance of soft tissue around implants. The first review evaluated the effect of KM on soft-tissue health, peri-implant bone loss, and patient-related variables. The second one analyzed if there was a specific type of matrix that provided better results in terms of peri-implant buccal soft-tissue thickness and keratinized mucosa width compared to autogenous soft-tissue graft. The third review evaluated the influence of different abutment materials on the soft tissues, and the fourth assessed the effect of soft-tissue thickness on peri-implant marginal bone loss (MBL). Results and Conclusions : The agreements reached by the assembly were as follows: the presence of supra-periosteal keratinized tissue is considered to favorably influence peri-implant health and aesthetics but had no relation to preventing bone crest resorption unrelated to infection. It facilitates patient cleaning around implants and reduces patient-reported pain. The free gingival graft (FGG) is considered the best in terms of supra-periosteal KM increase. Connective tissue grafts (CTG) perform better than volume-stable collagen matrices to increase soft-tissue thickness. Collagen matrices reduce surgical time and patient morbidity and can give better camouflaging. The influence of abutment material (titanium or zirconia) on MBL remains controversial, and no conclusion could be reached on this issue. Peri-implant soft-tissue health and recession seem not to be influenced by abutment material, but data are limited to zirconia and titanium. Although this systematic review highlighted the absence of a correlation between soft-tissue thickness and MBL, the assembly failed to find a consensus on this issue.
