Abstract

A large-scale genomic analysis of patients with ASXL1-mutated myeloid disease has not been performed to date. We reviewed comprehensive genomic profiling results from 6043 adults to characterize clinicopathologic features and co-mutation patterns by ASXL1 mutation status. ASXL1 mutations occurred in 1414 patients (23%). Mutation co-occurrence testing revealed strong co-occurrence (p < 0.01) between mutations in ASXL1 and nine genes (SRSF2, U2AF1, RUNX1, SETBP1, EZH2, STAG2, CUX1, CSF3R, CBL). Further analysis of patients with these co-mutations yielded several novel findings. Co-mutation patterns supported that ASXL1/SF3B1 co-mutation may be biologically distinct from ASXL1/non-SF3B1 spliceosome co-mutation. In AML, ASXL1/SRSF2 co-mutated patients frequently harbored STAG2 mutations (42%), which were dependent on the presence of both ASXL1 and SRSF2 mutation (p < 0.05). STAG2 and SETBP1 mutations were also exclusive in ASXL1/SRSF2 co-mutated patients and associated with divergent chronic myeloid phenotypes. Abstract

Background: Patients with acute myeloid leukemia (AML) who have tumor protein p53 (TP53) mutations or a complex karyotype have a poor prognosis, and hypomethylating agents are often used. The authors evaluated the efficacy of entospletinib, an oral inhibitor of spleen tyrosine kinase, combined with decitabine in this patient population. Methods: This was a multicenter, open‐label, phase 2 substudy of the Beat AML Master Trial (ClinicalTrials.gov identifier NCT03013998) using a Simon two‐stage design. Eligible patients aged 60 years or older who had newly diagnosed AML with mutations in TP53 with or without a complex karyotype (cohort A; n = 45) or had a complex karyotype without TP53 mutation (cohort B; n = 13) received entospletinib 400 mg twice daily with decitabine 20 mg/m2 on days 1–10 every 28 days for up to three induction cycles, followed by up to 11 consolidation cycles, in which decitabine was reduced to days 1–5. Entospletinib maintenance was given for up to 2 years. The primary end point was complete remission (CR) and CR with hematologic improvement by up to six cycles of therapy. Results: The composite CR rates for cohorts A and B were 13.3% (95% confidence interval, 5.1%–26.8%) and 30.8% (95% confidence interval, 9.1%–61.4%), respectively. The median duration of response was 7.6 and 8.2 months, respectively, and the median overall survival was 6.5 and 11.5 months, respectively. The study was stopped because the futility boundary was crossed in both cohorts. Conclusions: The combination of entospletinib and decitabine demonstrated activity and was acceptably tolerated in this patient population; however, the CR rates were low, and overall survival was short. Novel treatment strategies for older patients with TP53 mutations and complex karyotype remain an urgent need. Entospletinib with decitabine combination therapy was well tolerated in patients with acute myeloid leukemia who had TP53 mutations with or without a complex karyotype or had a complex karyotype without TP53 mutations. Keywords: acute myeloid leukemia; FLT3; Precision medicine EN acute myeloid leukemia FLT3 Precision medicine 6079 6082 4 12/19/22 20221215 NES 221215 FMS-like tyrosine kinase 3 (FLT3) is a receptor tyrosine kinase involved in regulation of haematopoietic stem cell and progenitor cell proliferation and differentiation.[1] Activating mutations in I FLT3 i are found in 25%-35% of adults with acute myeloid leukaemia (AML), most commonly in-frame internal tandem duplications (ITD) within the intracellular juxtamembrane domain (JMD).[2] This domain, consisting of residues 572-603, serves an autoinhibitory function, in part by preventing the activation loop from unfolding into its active conformation.[3] I FLT3 i -ITD mutations cause a gain-of-function phenotype with increased proliferation and protection from apoptosis[4] and are associated with worse clinical outcomes.[5] Most other activating I FLT3 i mutations are found in the tyrosine kinase domain (TKD),[2] though rare activating JMD deletions have also been described.[[6]] We identified three patients with rare I FLT3 i JMD missense mutations (Table 1). In-house NGS myeloid mutation panel revealed I FLT3- i ITD, a type A I NPM1 i frameshift mutation (VAF: 35.4%) and missense mutations in I DNMT3A i (c.2645G>A, p.R882H, VAF: 26.3%) and the I FLT3 i JMD (c.1775T>G, p.V592G, VAF: 10.6%). Additionally, Alotaibi I et al. i reported three cases of Ph chromosome emergence after treatment with FLT3 inhibitors in patients with I FLT3-ITD i mutations [[5]]. Despite advances in the acute myeloid leukemia (AML) treatment armamentarium over the last 4 years, patients with relapsed/refractory (R/R) AML have dismal outcomes [[1]]. The p190 (e1a2) transcript was identified in 12/14 patients where RT-PCR testing was performed, including 6/8 AML patients and 3/3 MDS patients [[4]]. Abstract

Purpose: To implement and optimize a pilot transitions of care model for scheduled chemotherapy admissions in patients with hematologic malignancies at our institution. Methodology: We utilized the plan-do-study-act (PDSA) quality improvement technique to prospectively measure success of interventions related to improving transitions of care processes that occurred in multiple stages including development of standardized operating procedures, electronic medical record documentation, and education to the malignant hematology multidisciplinary group. Chart review was performed retrospectively for at least nine patients per PDSA cycle. Areas of intervention addressed and measured regarding communication between the ambulatory care and acute care settings included: admission purpose, processes related to insurance benefits investigations for specialty medications required in the post-discharge setting, and plan for growth factors, prophylactic antimicrobials, and follow-up. Results and conclusions: We included 28 patients and performed a total of three PDSA cycles demonstrating specific improvements in: communication regarding status of benefits investigations performed for specialty medications prior to admission, resolution of these benefits investigations at various time points, improvement in efficient use of the electronic medical record for chemotherapy orders, and patient instructions for appropriate use of prophylactic antimicrobials. Although improvement was noted initially with prescribing of discharge antiemetics and antimicrobials, regression to baseline was noted with the third PDSA cycle. [ABSTRACT FROM AUTHOR]

Abstract

Eosinophilia in the peripheral blood and in bone marrow biopsies is encountered frequently by practicing hematologists and hematopathologists. The clinical presentation and underlying etiologies are quite varied. In some cases, [...], * Context.--The advent of molecular tools capable of subclassifying eosinophilia has changed the diagnostic and clinical approach to what was classically called hypereosinophilic syndrome. Objectives.--To review the etiologies of eosinophilia and to describe the current diagnostic approach to this abnormality. Data Sources.--Literature review. Conclusions.--Eosinophilia is a common, hematologic abnormality with diverse etiologies. The underlying causes can be broadly divided into reactive, clonal, and idiopathic. Classically, many cases of eosinophilia were grouped together into the umbrella category of hypereosinophilic syndrome, a clinical diagnosis of exclusion. In recent years, an improved mechanistic understanding of many eosinophilias has revolutionized the way these disorders are understood, diagnosed, and treated. As a result, specific diagnoses can now be assigned in many cases that were previously defined as hypereosinophilic syndrome. Most notably, chromosomal rearrangements, such as FIP1L1-PDGFRA fusions caused by internal deletions in chromosome 4, are now known to be associated with many chronic eosinophilic leukemias. When present, these specific molecular abnormalities predict response to directed therapies. Although an improved molecular understanding is revolutionizing the treatment of patients with rare causes of eosinophilia, it has also complicated the approach to evaluating and treating eosinophilia. Here, we review causes of eosinophilia and present a framework by which the practicing pathologist may approach this diagnostic dilemma. Finally, we consider recent cases as clinical examples of eosinophilia from a single institution, demonstrating the diversity of etiologies that must be considered.
