Metatarsalgia occurring in individuals with pes cavus is typically associated with abnormal loading patterns in the forefoot resulting from structural alterations. Simultaneously, the frequent overstress of the plantar fascia (PF) caused by the persistence of this foot deformity may further exacerbate the chronic pain induced by metatarsal overload. We aimed to investigate and quantify the effects of PF stiffness on the internal biomechanics of pes cavus using a computational modelling approach. A patient-specific finite element model of the foot-ankle complex using the actual three-dimensional geometry of idiopathic pes cavus bones and soft tissues was reconstructed. A sensitivity study was conducted to evaluate the effects of varying elastic modulus (0–700 MPa) of the PF on the metatarsal stress distribution, and force transmission through the metatarsophalangeal (MTP) and tarsometatarsal (TMT) joints in the pes cavus. The results indicated that variations in PF stiffness led to stress redistribution in the metatarsal region. Peak stress gradually reduced with decreasing stiffness until the PF was released, eventually resulting in a reduction of 22.39% compared to the reference value of 350 MPa. Furthermore, adjusting the PF stiffness to twice the reference value (700 MPa) increased the contact forces through the TMT and MTP joints by up to 23% and 116%, respectively. The reduction of PF stiffness alleviated focal metatarsal loading, and therefore, surgical fascia release can be considered to alleviate metatarsalgia in patients with pes cavus. A causal link exists between structural differences in the foot and alterations in the lower limb biomechanics, which might predispose an individual to develop characteristic musculoskeletal disorders. This study aimed to determine how the foot structural characteristics, as represented by the medial longitudinal arch flexibility, affect lower limb joint coupling coordination and anterior-posterior ground reaction impulses (GRIs) during walking and running. Following the calculation of arch height flexibility, a total of fifty-four physically active males were grouped and completed gait experiments to collect kinematic and kinetic data synchronously. Inter-joint coordination and variability were calculated from the angle-angle plots of knee-hip, ankle-knee, and metatarsophalangeal (MTP)-ankle couplings based on an optimized vector coding technique. Our results indicate that coupling coordination of interest and its variability, as well as anterior-posterior GRIs, could potentially be influenced due to differences in arch height flexibility. Notably, the individuals with stiff arches exhibited significantly greater coordination variabilities during the early stance for both ankle-knee and MTP-ankle coordination yet significantly smaller for MTP-ankle coordination variabilities during the mid-stance phase. Furthermore, combining the statistical parametric mapping analysis results, the flexible arches experienced a greater proportion of GRIs in the anterior-posterior direction. In conclusion, these observations demonstrated that variations in arch flexibility led to differences in lower limb joint coordination variabilities and GRIs during gait. Background: Low back pain is one of the most common disorders of the musculoskeletal system, leading to impaired function and decreased quality of life in many patients. Foot arch alterations may lead to structural changes and/or affect its load distribution. It should be noted that even slight elevations of the foot’s medial longitudinal arch above the norm may lead to changes in load and pressure distribution. Hence, it becomes important to find the relationship between low back pain and altered foot arch indices leading to improper body balance. Aims and Objectives: The aims and objectives of the study are to find any correlation between lower back pain and an altered foot arch index in the Eastern Indian population. Materials and Methods: A total of 60 subjects aged 20–40 years with varying degrees of low back pain were selected. Low back pain scores of the subjects were found using modified oswestry low back pain disability questionnaire. Footprints in standing position using ink were taken on graph paper and the foot arch indices were calculated. Results: 54.16% of the male and 36.11% of female participants had low arched feet, 20.83% of male and 52.78% of female participants had normal arched feet, and 25% of male and 11.11% of female participants had high arched feet with varying degrees of low back pain. 77.42% of the participants with minimal disability due to low back pain were having normal arched feet, 71.43% of the participants with moderate disability due to low back pain were having low arched feet, and 100% of the participants with severe disability due to low back pain were having either high or low arched feet. Conclusion: This study showed that people having either low or high arched feet are at an increased risk of developing acute or chronic low back pain at some stage in their lives. Knowledge of this risk factor might even help people to choose appropriate occupations in the future.

Minimalist walking shoes have been shown to improve foot muscle size and strength in active adults, but not in our previous study involving children, which could relate to the more structured footwear used in our study. Hence, this study examined the effects of true minimalists on intrinsic foot muscle size and strength, foot arch integrity, and physical function among primary school children. After a baseline assessment, 30 primary school students aged between 9 and 12 were given a pair of minimalist shoes (minimalist index = 92%) as their regular school shoes for two school terms, followed by a re-assessment. Seventeen of the 30 participants in the minimalist group completed the study. Compared to the control group, the minimalist group showed significantly increased cross-sectional area of abductor hallucis (p = 0.047, Cohen's d = 0.57) and flexor digitorum brevis (p = 0.037, Cohen's d = 0.80), increased strength of the hallux (p = 0.015, Cohen's d = 0.76) and lesser toes (p = 0.014, Cohen's d = 0.66), greater arch height (p = 0.020, Cohen's d = 0.52) and standing long jump distance (p = 0.001, Cohen's d = 9.79). The control group exhibited improved standing long jump performance only (p = 0.020, Cohen's d = 10.70). Objective: To determine the distribution of foot arch type, associated symptoms, and factors associated with moderate to severe pain. Materials and Methods: The cross-sectional study was collected data from 5th year medical students, Faculty of Medicine, Thammasat University in academic year 2020. The distribution of foot arch type used the footprint and classified by Harris imprint index (HII), Chippaux-Smirak index (CSI), Staheli index (SI). The associated symptoms were collected into pain and tightness. Pain score was rated by the volunteer using numeric rating scale (NRS) at each foot/leg separately. Results: A total of ninety-eight medical students (196 feet) were recruited and analyzed. The distribution of foot arch type by HII, CSI and SI were 1) bilateral normal arched feet: 42.9%, 67.3%, 54.1% 2) bilateral pes planus: 8.2%, 12.2%, 21.4% 3) bilateral pes cavus: 25.5%, 6.1%, 5.1% 4) unilateral pes planus: 2%, 9.2%,15.3% 5) unilateral pes cavus: 21.4%, 5.1%, 4.1%, respectively. The most commonly associated symptom of pes planus was midfoot pain (17%) while pes cavus and normal arched foot were hindfoot pain (22.4% and 17.3%). The factor associated with moderate to severe pain was BMI ≥ 23 kg/m2 (OR = 3.23, 95%CI 1.63 - 6.41, p-value = 0.001). Conclusion: Bilateral normal arched feet were mostly found. Foot morphology and arch integrity do not remain constant during a running bout. Previous studies have reported inconsistent changes in foot sizes and arch parameters and this discrepancy may be related to the variation in their test duration, e.g., 15-min treadmill run vs. 30 KM trial. Hence, this study sought to evaluate the change in foot morphology, arch integrity and bilateral symmetry after a 10 KM run among 19 male recreational runners. Before and after the run, a portable foot scanner was used to capture the 3-dimensional foot images and measure foot dimensions in both weight-bearing and non-weight-bearing conditions. Foot arch integrity was quantified by arch height ratio, arch height index, and arch stiffness index (ASI). Bilateral symmetry was evaluated by calculating the symmetry index. Increased foot length (p = 0.007; ηp2 = 0.18) and decreased ball girth (p = 0.038; ηp2 = 0.11) were demonstrated following the run with absolute differences of less than 2 mm. Navicular height, dorsum height, arch height ratio and arch height index significantly decreased after the run (p < 0.001; ηp2 ≥0.30) whereas ASI increased (p < 0.001, ηp2 = 0.33) and navicular height drop reduced (p < 0.001, ηp2 = 0.37). Significances of symmetry index were only demonstrated for navicular height (p = 0.019, effect size = 0.37) and arch height ratio (p = 0.019, effect size = 0.42). A few changes in foot morphology were detected but a reduction in foot arch height was demonstrated, which may give shoe manufacturers insights into shoe design. Male recreational runners were recommended to choose shoes with arch support for maintaining foot arch function during a 10 KM run.

This work aimed to explore the acute effects of athletic taping techniques on foot arch deformity and plantar pressure in young female adults with flexible flatfoot (FFT). Twenty young female adults with FFT were recruited in the current study. Each participant was randomly divided into two taping groups, namely, augmented low-dye (ALD) and modified low-dye (MLD). The foot arch deformity and plantar pressure were measured at baseline, after taping and after 20 min of walking. The foot arch deformity was determined based on navicular drop distance (NDD) and resting calcaneal stance position (RCSP). Compared with baseline, the NDD values were significantly lower after taping. After 20 min of walking, ALD taping resulted in a lower NDD value than MLD (p < 0.001). ALD maintained a higher RCSP than baseline after 20 min of walking (p = 0.004). Furthermore, compared with baseline, medial midfoot force–time integration (p = 0.013) and contact area (p = 0.022) increased after taping with MLD, and peak pressure in the medial midfoot increased after walking for 20 min with MLD (p = 0.026). Peak pressure in the second to fifth toes significantly decreased after 20 min of walking with ALD compared with that after taping immediately (p = 0.002). ALD and MLD taping could improve FFT arch deformity and plantar pressure distribution, prospectively changing peak pressure of the second to fifth toe area and medial midfoot after 20 min of walking, integrated contact area and force–time integration medial midfoot during walking in young female adults. Furthermore, ALD taping could improve FFT deformity more than using MLD after 20 min of walking. Thus, when treating FFT in young female adults, ALD taping should be considered adaptively to guide arch support production and correct midfoot pronation. • AT could decrease midfoot deformity after taping and after 20 min of walking. • ALD led to better acute peak pressure release in second to fifth toes than MLD. • In clinical prescription for FFT, the midfoot deformity should also be considered. Background: Down syndrome is a genetic condition that has an impact on a person's health and development on many levels, including their feet. Children with Down syndrome frequently have flat feet and other foot deformities, which can make walking painful, uncomfortable, and challenging. Objective: The aim of the current study was to evaluate the foot alignment and its impact on quality of life in children with Down syndrome. Subjects and methods: This retrospective study included a total of 40children with Down syndrome from both genders with ages ranged from 8 to12 years old recruited from Alfolk Charity Institution, Outpatient Clinic, Intellectual Educational Schools, in Al Gharbia and Cairo Governorates. Foot arch angels were assessed by X-ray radiological assessment whereas; health status for children with foot and ankle problem, Quality of life of children with Down syndrome were assessed by the Oxford ankle foot questionnaire. Results: There was a significant increase in Talo-calcneal angle of the right foot of study group compared to the reference value (p = 0.001). There was a significant increase in Talo-calcneal angle of the left foot of study group compared to the reference value (p = 0.001). There was a significant increase in Bohler's angle of the right foot of study group compared to the reference value (p = 0.001). There was a significant increase in Bohler's angle of the left foot of study group compared to the reference value (p = 0.02). There was a significant decrease in school & play score of study group compared to the reference value (p = 0.001). Abstract Background Various clinical measures of static foot posture have been developed and used. However, consensus among clinical measures to classify foot posture remains to be established. Therefore, this study aimed to determine the level of agreement as a reliability component between two common clinical methods in asymptomatic adults: the normalised navicular height truncated (NNHt) and the Foot Posture Index-6 (FPI-6). Methods The NNHt and FPI-6 were conducted on 102 asymptomatic adults. The measurement sequence was randomly arranged for each participant. Weighted Kappa (K w ) was used to determine the agreement between the methods. Results Both the NNHt and FPI-6 achieved similar foot posture distributions: approximately 40–50% of the participants had a normal foot, approximately 40% had a pronated foot and approximately 10–20% had a supinated foot. The agreement between the methods to classify foot posture was excellent (K w = 0.84). Conclusions The present study found excellent agreement between two commonly used clinical measures. This finding highlights the NNHt and FPI-6 consensus for foot posture classification in asymptomatic adults.

Flatfoot (pes planus) is described as a reduction or absence of the medial longitudinal arch (MLA) of the foot, with or without additional deformities of the foot and ankle. It is a very common orthopedic manifestation in infants and children and usually resolves by adolescence. Flat foot is usually flexible and painless with no functional compromise so that described as physiologic. In some rare cases flatfoot can become painful or rigid and may be a sign of pathology such as vertical talus or tarsal coalition. Although it is very common, there is no standard definition and no universally accepted classification system for pediatric flatfoot. Furthermore there are no large, prospective studies comparing the natural history of flatfoot in response to various treatments during the developmental period. Current literature suggests that it is safe and appropriate to simply observe an asymptomatic child with flatfoot. INTRODUCTION: Endoscopic plantar fascia release (EPFR) is a minimally invasive surgical intervention for recalcitrant plantar fasciitis. Its efficacy has been convincing but the in vivo effect on medial longitudinal foot arch and footprint has not been studied. Our objective is to evaluate the changes of foot posture using radiographs and footprints following endoscopic plantar fascia release in recalcitrant plantar fasciitis. MATERIALS AND METHODS: This prospective cohort involved patients with recalcitrant plantar fasciitis who failed six months of conservative treatment. Two-portal endoscopic release of not more than 50% of plantar fascia width was performed. Footprint and standard weight-bearing anteroposterior and lateral radiographs of the foot were taken pre-operatively and at 12 months post-surgery. Arch index, normalised navicular height truncated, calcaneal inclination angle, calcano-1st metatarsal angle, talonavicular coverage angle and talus-2nd metatarsal angle were measured. RESULTS: Sixteen patients (18 feet) were reported. Patients’ follow-up ranged from 14 to 31 months after surgery (mean±SD: 23.44±5.76). Aim: The current study aimed to compare the arch angles of the foot, Achilles tendinopathy, subcutaneous fat tissue thickness of the heel, and epin calcanei (EC) in patients with osteochondral lesion of the talus (OLT). Materials and Methods: The magnetic resonance images (MRI) of the ankle performed by the radiology clinic between March 2020 and October 2021 were examined. Based on the MRI findings, the patients diagnosed with OLT were included in the study group (n = 55) and those not diagnosed with OLT formed the control group (n = 118). All the patients included in the study were investigated for their demographic characteristics and presence of EC. Weight-bearing lateral X-ray images were used to measure the lateral talusfirst metatarsal angle (Meary's angle), lateral talocalcaneal angle (LTCA), and calcaneal inclination angle (also known as the calcaneal pitch). Results: The study and control groups had no significant difference in terms of gender distribution (P = 0.575). The patients in the study group had a significantly higher mean age than the control group (P < 0.001). Evaluation of the arch angles of the foot showed the patients with OLT to have higher LTCA and calcaneal pitch angles and greater subcutaneous fat thicknesses (p < 0.001, p < 0.001, and p = 0.004, respectively). In addition, the study group showed higher rates of patients with EC and Achilles tendinopathy (p = 0.076 and p = 0.019, respectively). According to the binary logistic regression analysis model, each unit of increase in the patients' age increases the presence of osteochondritis dissecans (OCD) of the talus by 1.063 times (95% confidence interval (CI): 1.012-1.116; P = 0.015). Each unit of increase in LTCA increases the presence of OCD of the talus by 1.360 times (95% CI: 1.219-1.516; P < 0.001). Conclusion: Our study demonstrated that sex had no effect on OLT incidence, whereas changes in the LTCA and pitch angles elevated the risk of OLT. Furthermore, increased age was found to be associated with a higher risk of OLT formation. We also showed that Achilles tendinopathy and EC were significantly higher in patients with OLT. Human plantar intrinsic foot muscles consist of 10 muscles that originate and insert within the sole of the foot. It is known that the anatomical cross‐sectional area (ACSA) and muscle thickness of two plantar intrinsic foot muscles, the flexor hallucis brevis (FHB) and abductor hallucis (ABH), associate with morphological parameters of the foot, such as total and truncated foot length and navicular height. However, it is unclear how the size for each of the plantar intrinsic foot muscles associates with various morphological profiles of the foot. This study aimed to elucidate this subject. By using magnetic resonance imaging (MRI), serial images of the right foot were obtained in 13 young adult men without foot deformities. From the obtained MR images, ACSA for each of the individual plantar intrinsic foot muscles was analyzed along the foot length, and then its muscle volume (MV) was calculated. The analyzed muscles were the abductor digiti minimi (ABDM), ABH, adductor hallucis oblique head (ADDH‐OH), adductor hallucis transverse head (ADDH‐TH), flexor digitorum brevis (FDB), FHB, and quadratus plantae (QP). Furthermore, MV of the whole plantar intrinsic foot muscle (WHOLE) was defined as the total MVs of all the analyzed muscles. As morphological parameters, total foot length, truncated foot length, forefoot width, ball circumference, instep circumference, navicular height, great toe eversion angle, and little toe inversion angle were measured using a laser three‐dimensional foot scanner in standing and sitting conditions. In addition, navicular drop (ND) and normalized truncated navicular height (NTNH) were also calculated as medial longitudinal arch (MLA) height indices. The MV of WHOLE was significantly associated with the forefoot width, ball circumference, and instep circumference (r = 0.647–0.711, p = 0.006–0.013). Positive correlations were found between the forefoot width and MV of FHB, FDB, and QP (r = 0.564–0.653, p = 0.015–0.045), between the ball circumference and MV of QP (r = 0.559, p = 0.047), between the instep circumference and MV of FHB (r = 0.609, p = 0.027), and between the little toe inversion angle and MV of QP (r = 0.570, p = 0.042). The MVs of ABH, ABDM, and ADDH‐OH were not significantly correlated with any morphological parameters of the foot. Similarly, no significant correlations were found between MV of each muscle and either of the MLA height indices (ND and NTNH). Thus, the current results indicate that forefoot width and circumferential parameters (instep and ball circumference), not MLA height, associate with the size of the whole plantar intrinsic foot muscles, especially those specialized in toe flexion (FHB, FDB, and QP). Background: The spring ligament complex (SLC) supports the medial longitudinal arch of the foot, particularly in standing. We evaluated posture-related changes in the thickness and length of the three SLC bundles and their histology.Methods: The thickness and length of the supramedial calcaneonavicular ligament (smCNL), medioplantar oblique calcaneonavicular ligament (mpoCNL), and inferoplantar calcaneonavicular ligament (iplCNL) were measured in the supine and standing positions, using a multiposture magnetic resonance imaging system, in 72 healthy adult feet. Histological examination was performed for 10 feet from five cadavers.Results: The smCNL thickness decreased and its length increased from the supine to the standing position (P < 0.001); no other posture-related effects were noted. Histologically, smCNL fibers overlapped along multiple directions while mpoCNL and iplCNL, fibers were oriented horizontally along the longitudinal axis and vertically along the short axis, respectively.Conclusion: The complex, multidirectional, orientation of the smCNL allows an adaptive response to changes in loading. Research on the plantar segment has not been widely carried out in Indonesia’s population, even though the plantar segment data will be essential in further research and therapy of plantar-related problems. Therefore, this research intends to describe the plantar profile: the foot arch and the plantar pressure difference between the right and left foot. This research applied a cross-sectional study. Subjects were recruited from the Faculty of Medicine students, Universitas Indonesia, class 2012, with inclusion criteria aged 17-21 years and normal gait. Meanwhile, the exclusion criteria consisted of having postural abnormalities, a history of neuromusculoskeletal disorders in the lower limbs, a history of fractures in the spine and legs, a history of surgery on the spine and legs, and refusing to participate in the study. Research subjects stood on a plantar scanner, conducted at the Anatomy Laboratory, the Faculty of Medicine, Universitas Indonesia. The Mann-Whitney test was then used to analyze the difference in plantar pressure between the right and left foot. The results revealed that a hundred research subjects had a proportion of a low foot arch of 4%, a normal foot arch of 89%, and a high foot arch of 7%. The median right plantar pressure was 273.5 KPa, while the median left plantar pressure was 253.5 KPa. The Mann-Whitney test showed a p-value of 0.954 for the pressure difference between right and left foot. There was no plantar pressure difference between the right and left foot.

Improper foot biomechanics associated with uneven bodyweight distribution contribute to impaired balance and fall risks. There is a need to complete the panel of commercially available devices for the self-measurement of BMI, fat, muscle, bone, weight, and hydration with one that measures weight-shifting at home as a pre-specialist assessment system. This paper reports the development of the Early Notice Pointer (ENP), a user-friendly screening device based on weighing scale technology. The ENP is designed to be used at home to provide a graphic indication and customised and evidence-based foot and posture triage. The device electronically detects and maps the bodyweight and distinct load distributions on the main areas of the feet: forefoot and rearfoot. The developed platform also presents features that assess the user's balance, and the results are displayed as a simple numerical report and map. The technology supports data display on mobile phones and accommodates multiple measurements for monitoring. Therefore, the evaluation could be done at non-specialist and professional levels. The system has been tested to validate its accuracy, precision, and consistency. A parallel study to describe the frequency of arch types and metatarsal pressure in young adults (1034 healthy subjects) was conducted to explain the importance of self-monitoring at home for better prevention of foot arch- and posture-related conditions. The results showed the potential of the newly created platform as a screening device ready to be wirelessly connected with mobile phones and the internet for remote and personalised identification and monitoring of foot- and body balance-related conditions. The real-time interpretation of the reported physiological parameters opens new avenues toward IoT-like on-body monitoring of human physiological signals through easy-to-use devices on flexible substrates for specific versatility. Foot arch plays an important role in bearing body weight, absorbing ground reaction forces and maintaining balance, and its structure and function are bound to have a profound effect on physical activities and sports. Arch height and arch stiffness are 2 factors that represent the structure and function of the arch. Therefore, the purpose of this study was to explore the associations of arch height and arch stiffness with physical performance. A total of 56 men (aged 49.00 ± 7.95 years, mean body mass index [BMI] 26.80 ± 3.75 kg/m2) participated in this study. A 3-dimensional laser scanner was employed to obtain foot structure information of each participant, from which the arch height index (AHI) and arch stiffness index (ASI) were computed. Physical performance measures including agility, power, and proprioception were tested in a random order. The results indicated that the stepping forward and backward and vertical jump that represent agility and force respectively were negatively and significantly associated with AHI (r = -0.27, p =.045; r = -0.35, p =.009). When adjusted for age and BMI, only height of vertical jump was found to be correlated significantly with AHI (r = -0.29, p =.040); while no significant relationships were observed between physical performance measures and ASI. Multivariate linear regression analysis showed that AHI, age, and BMI can effectively predict the height of vertical jump. Background: The foot arch plays an important role in propulsion and shock absorption during walking and running; however, the relationship among the foot arch, metatarsal locking theory, and nature of the windlass mechanism (WM) remain unclear.Research Question: What are the differences in the kinematic relationship between the foot arch, hindfoot, and hallux during walking and running?Methods: Relative angles within the foot were measured in 18 healthy men using the Oxford foot model (OFM). Data for barefoot walking at a comfortable speed and rearfoot running at 2.0 m/s were collected. Angles of the forefoot relative to the hindfoot (OFM-arch), hallux relative to the forefoot (Hallux) on the sagittal plane, and hindfoot relative to the shank (Hindfoot) on three anatomical planes were obtained. The medial longitudinal arch (MLA) angle was calculated to verify that OFM-arch can substitute the MLA angle. Each parameter was subjected to cross-correlation analysis and Wilcoxon signed-rank tests to examine the relationship with OFM-arch and compare them during walking and running.Result: OFM-arch was similar to the conventional MLA projection angle in both trials (gait: 0.79, running: 0.96 p < 0.01). Synchronization of the OFM-arch and Hallux angles was higher in running than in walking (gait: -0.09, running: -0.75 p < 0.01). Hindfoot supination was unrelated to OFM-arch. Hindfoot angle on the transverse plane exhibited a moderate relationship with OFM-arch, indicating different correlations in walking and running (gait: 0.63, running: -0.68 p < 0.01).Significance: The elevation of the foot arch due to hallux dorsiflexion differed during walking and running; hence, other factors besides WM (such as intrinsic muscles) may affect the foot arch elevation during running. Background: The longitudinal arch is important for individuals with pes planus. The toe spread out exercise (TSO) has been widely used to continuously support the longitudinal arch by increasing the abductor hallucis (AbdH) muscle activation. However, the AbdH muscle is commonly lack of the sufficient activation during the TSO especially in individuals with pes planus. Objects: This study was performed to investigate the effect of arch support on the muscle activity and strength of the AbdH during TSO in standing position in individuals with pes planus. Methods: Twenty subjects with pes planus between 20 and 30 years of age participated in this study. The muscle activity and strength of the AbdH were measured using surface EMG system and the Smart KEMA tensiometer system. The AbdH muscle was evaluated during TSO between individuals with and without longitudinal arch support in standing position. The longitudinal arch was supported by using the insole. The paired t-test was used. The level of statistical significance was set at α = 0.05. Results: The muscle activity and strength of the AbdH during TSO with arch support in standing position was significantly greater than that without arch support. Conclusion: The muscle activity and strength of the AbdH during TSO in standing position can be influenced by the longitudinal arch support in individuals with pes planus. The AbdH strengthening during TSO in standing with arch support can be recommended especially in individuals with pes planus in the clinical settings.

Foot morphology and arch integrity do not remain constant during a running bout. Previous studies have reported inconsistent changes in foot sizes and arch parameters and this discrepancy may be related to the variation in their test duration, e.g., 15-min treadmill run vs. 30 KM trial. Hence, this study sought to evaluate the change in foot morphology, arch integrity and bilateral symmetry after a 10 KM run among 19 male recreational runners. Before and after the run, a portable foot scanner was used to capture the 3-dimensional foot images and measure foot dimensions in both weight-bearing and non-weight-bearing conditions. Foot arch integrity was quantified by arch height ratio, arch height index, and arch stiffness index (ASI). Bilateral symmetry was evaluated by calculating the symmetry index. Increased foot length ( p = 0.007; η p 2 = 0.18) and decreased ball girth ( p = 0.038; η p 2 = 0.11) were demonstrated following the run with absolute differences of less than 2 mm. Navicular height, dorsum height, arch height ratio and arch height index significantly decreased after the run ( p < 0.001; η p 2 ≥0.30) whereas ASI increased ( p < 0.001, η p 2 = 0.33) and navicular height drop reduced ( p < 0.001, η p 2 = 0.37). Significances of symmetry index were only demonstrated for navicular height ( p = 0.019, effect size = 0.37) and arch height ratio ( p = 0.019, effect size = 0.42). A few changes in foot morphology were detected but a reduction in foot arch height was demonstrated, which may give shoe manufacturers insights into shoe design. Foot arch structure contributes to lower-limb joint mechanics and gait in adults with obesity. However, it is not well-known if excessive weight and arch height together affect gait mechanics compared to the effects of excessive weight and arch height alone. The purpose of this study was to determine the influences of arch height and obesity on gait mechanics in adults. In this study, 1) dynamic plantar pressure, 2) spatiotemporal gait parameters, 3) foot progression angle, and 4) ankle and knee joint angles and moments were collected in adults with normal weight with normal arch heights (n = 11), normal weight with lower arch heights (n = 10), obesity with normal arch heights (n = 8), and obesity with lower arch heights (n = 18) as they walked at their preferred speed and at a pedestrian standard walking speed, 1.25 m/s. Digital foot pressure data were used to compute a measure of arch height, the Chippaux-Smirak Index (CSI). Our results revealed that BMI and arch height were each associated with particular measures of ankle and knee joint mechanics during walking in healthy young adults: (i) a higher BMI with greater peak internal ankle plantar-flexion moment and (ii) a lower arch height with greater peak internal ankle eversion and abduction moments and peak internal knee abduction moment (i.e., external knee adduction moment). Our results have implications for understanding the role of arch height in reducing musculoskeletal injury risks, improving gait, and increasing physical activity for people living with obesity. • Integrated non-invasive diagnostics to production method for low arch identification and automated customized foot orthotic design and manufacturing are developed and preliminary tests performed. • The set thresholds for foot types identification established as a part of the database development for the current study reduced the type 1 and type 2 errors to below 10.7% in comparison to previously reported above 55%. • Sedentary activity preceding to the measurements had greater effect on arch height. • Recommended test protocol — participant should not sit more than 100 min prior to the measurement, need to be applied to control this variability. • An automated algorithm is developed to translating scanned foot data into orthotic geometry, which decreases the required time of orthotic computer-aided design from over 3 h to less than 2 min. A non-invasive, no radiation, out-of-hospital automated system is proposed to identify low arch integrated in the design and manufacturing of personalized orthoses using parametric modelling. The aim of the design process is to integrate assistive technology with assessment and prevent low arch progressing to a more serious case - flatfoot. In the automated procedure, we developed an assessment method including reliable thresholds of foot type classification and test protocol to reduce interferences due to preceding activities, an automation to translate scanned data into parametric design for orthotic customization, finite element model evaluating effectiveness of the personalized design, and a personalized comparative test to evaluate the long-term improvement of foot arch shape. Our low arch threshold established by subject-specific 3D models reduced the misclassification rate from 55%, as previously reported to 6.9%. Individuals who engaged in sedentary activity (i.e. sitting) had the greater change in arch height compared to active activity (i.e. standing and walking), which is more likely to affect the obtained measure. Therefore, a test protocol now states that participants are not allowed to sit over 100 min prior the measurement to reduce such interference. We have proposed and tested an automated algorithm to translate scanned data including seven foot's parameters into customised parametric design of the insert. The method decreases the required time of orthotic computer-aided design from over 3 h to less than 2 min. A finite element analysis procedure was additionally developed to assess the performance of geometries and material of designed orthotic based on the distribution of plantar pressure and internal stress. The personalized comparative assessment based on midfoot contact area was carried out periodically for follow-up and the orthotic could be re-designed if necessary. The proposed automated procedure develops a pre-screening system to distinguish low arch and provide preventatives before it becomes symptomatic. Objective: To investigate effects of taping techniques on arch deformation in adults with pes planus. Methods: The following databases were searched up to March 2020, including Web of Science, Pubmed, EBSCO, CNKI and Cochrane Library. Heterogeneity and publication bias were assessed by I2 index and funnel plots, respectively. In addition, Cochrane scale was used to evaluate the quality of research. Results: Navicular height for three antipronation taping techniques significantly increased immediately post tape compared with baseline (mean difference = 4.86 mm, 95% CI = 2.86–6.87 mm, Z = 4.75, p < 0.001). The highest increase was observed in Augmented low-Dye (ALD). Modified low-Dye (MLD) was second only to ALD (p<0.001). Navicular height after walking for 10 min was much higher than baseline (p<0.001), with MLD decreased smaller than ALD. Conclusions: ALD was the most effective taping technique for controlling foot arch collapse immediately post tape compared with baseline, followed by MLD. By contrast, MLD could possibly performed better than ALD in maintaining immediate navicular height after walking for 10 min. Low-Dye could make resting calcaneal stance position closer to neutral position. Although positive effects of Navicular sling, low-Dye and Double X taping interventions were observed, they could not maintain this immediate navicular height effect after a period of higher intensity weight-bearing exercise. Introduction: The foot is an essential element of the kinetic system. Performing the supporting function, it maintains the correct body posture, but above all it is used while walking. Feet are vulnerable to all the changes that occur in human environment. The civilisation changes are particularly harmful to the locomotor system. Material and methods: The research was conducted in the academic year 2014/2015 among the second year Physiotherapy and Physical Education students in the University of Physical Education in Kraków. The total of 93 students took part in the research (48 females and 45 males). The average age of the subjects was 21 years. In order to assess the curvature of the foot longitudinal arch a podoscope was used, whereas the Tanita BC-418 scales were applied to define the BMI value and calculate the percentage of body fat tissue. Results: A vast majority of the subjects (70.9%) had a normal type of foot longitudinal arch. Higher longitudinal arch occurred more frequently among women than men, the normal type was less frequent. The BMI level was also within the norm among the majority of the students (79.6%). Overweight was more frequent among males than females. Both males and females with the correct BMI value in their majority had normal curvatures of both feet longitudinal arches. The highest percentage (75%) of the regular right foot longitudinal arch was recorded among females with low body fat deposition. In the case of the left foot the percentage was 68.8%. All the males with low fat deposition obtained normal values of the longitudinal arch curvature for both feet. Conclusions: The subjects most frequently had a regular curvature of foot longitudinal arch. No significant influence of the percentage of fat tissue and BMI value on the Clarke’s angle was recorded.

The original windlass mechanism describes a one-to-one coupling between metatarsal joint dorsiflexion and medial longitudinal arch rise. The description assumes a sufficiently stiff plantar aponeurosis and absence of foot muscle activity. However, recent research calls for a broader interpretation of the windlass mechanism that accounts for an extensible plantar aponeurosis and active foot muscles. In this study, we investigate the rise of the arch in response to toe dorsiflexion when sitting, standing, and walking to discuss the windlass mechanism's contributions in static and dynamic load scenarios. 3D motion analysis allowed a kinematic investigation of the rise and drop of the arch relative to the extent of toe dorsiflexion. The results suggest that static windlass effects poorly predict the relationship between arch dynamics and metatarsophalangeal joint motion during dynamic load scenarios, such as walking. We were able to show that toe dorsiflexion resulted in an immediate rise of the longitudinal arch during sitting and standing. In contrast, a decrease in arch height was observed during walking, despite toe dorsiflexion at the beginning of the push-off phase. Further, the longitudinal arch rose almost linearly with toe dorsiflexion in the static loading scenarios, while the dynamic load scenario revealed an exponential rise of the arch. In addition to that, the rate of change in arch height relative to toe motion was significantly lower when sitting and standing compared to walking. Finally, and most surprisingly, arch rise was found to correlate with toe dorsiflexion only in the dynamic loading scenario. These results challenge the traditional perspective of the windlass mechanism as the dominating source of foot rigidity for push-off against the ground during bipedal walking. Improper foot biomechanics associated with uneven bodyweight distribution contribute to impaired balance and fall risks. There is a need to complete the panel of commercially available devices for the self-measurement of BMI, fat, muscle, bone, weight, and hydration with one that measures weight-shifting at home as a pre-specialist assessment system. This paper reports the development of the Early Notice Pointer (ENP), a user-friendly screening device based on weighing scale technology. The ENP is designed to be used at home to provide a graphic indication and customised and evidence-based foot and posture triage. The device electronically detects and maps the bodyweight and distinct load distributions on the main areas of the feet: forefoot and rearfoot. The developed platform also presents features that assess the user’s balance, and the results are displayed as a simple numerical report and map. The technology supports data display on mobile phones and accommodates multiple measurements for monitoring. Therefore, the evaluation could be done at non-specialist and professional levels. The system has been tested to validate its accuracy, precision, and consistency. A parallel study to describe the frequency of arch types and metatarsal pressure in young adults (1034 healthy subjects) was conducted to explain the importance of self-monitoring at home for better prevention of foot arch- and posture-related conditions. The results showed the potential of the newly created platform as a screening device ready to be wirelessly connected with mobile phones and the internet for remote and personalised identification and monitoring of foot- and body balance-related conditions. The real-time interpretation of the reported physiological parameters opens new avenues toward IoT-like on-body monitoring of human physiological signals through easy-to-use devices on flexible substrates for specific versatility.

The arch of the human foot is unique among hominins as it is compliant at ground contact but sufficiently stiff to enable push-off. These behaviours are partly facilitated by the ligamentous plantar fascia whose role is central to two mechanisms. The ideal windlass mechanism assumes that the plantar fascia has a nearly constant length to directly couple toe dorsiflexion with a change in arch shape. However, the plantar fascia also stretches and then shortens throughout gait as the arch-spring stores and releases elastic energy. We aimed to understand how the extensible plantar fascia could behave as an ideal windlass when it has been shown to strain throughout gait, potentially compromising the one-to-one coupling between toe arc length and arch length. We measured foot bone motion and plantar fascia elongation using high-speed X-ray during running. We discovered that toe plantarflexion delays plantar fascia stretching at foot strike, which probably modifies the distribution of the load through other arch tissues. Through a pure windlass effect in propulsion, a quasi-isometric plantar fascia's shortening is delayed to later in stance. Context: Flat foot is one of the lower extremity deformities that might change kinetic variables of gait. Fatigue is one of the factors that can alter the vertical ground-reaction force (GRF). The effect of a fatiguing condition on vertical GRF has not been documented in individuals with flat feet. Objective: To examine the fatigue effect on vertical GRF in individuals with flat feet compared with a normal group during barefoot walking. Design: Repeated-measure ANOVA for the effects of fatigue on individuals with flat feet and normal feet. Setting: Biomechanics laboratory. Participants: 17 subjects with flat feet and 17 normal subjects (recruited according to their arch-height ratio). Main Outcome Measures: Three vertical GRF measures (F1, the first peak force; F2, minimum force; and F3, the second peak force) were extracted before and after a functional fatigue protocol. Results: No significant interaction between fatigue and group was observed for the 3 vertical GRF measures. For F2, fatigue and group effects were significant (P = .001 and P = .02, respectively). Furthermore, F2 was higher in the flat-feet group than in the normal group; F2 also increased after fatigue. For F3, only a significant fatigue effect was observed (P = .004). F3 decreased after fatigue in both groups. Conclusions: In the flat-feet group, a decrease in the variation of vertical GRF might be due to more flexible foot joints. Background: Intramedullary fusion bolts (FB) were introduced to stabilize the medial column of the instable Charcot foot (ICF), but complications as bolt loosening or breakage are frequent. We compared the use of a standard FB and a high-profile threaded FB with a grit-blasted surface. We hypothesized that implant related complications occurred less often and osseous consolidation of fusion sites was more distinctive using the latter type of FB.Methods: Consecutive patients suffering from an ICF were stabilized with a high-profile threaded and surface-modified FB (HTFB) (n=20) or with a standard FB (n=20) which was placed into the first ray. Additional bolts and dorsal low-profile plates were applied in every patient. In a retrospective assessment osseous consolidation of the fusion sites was analyzed at 3 month and quantified by CT scan. At 3 and 12 month longitudinal foot arch collapse and rate of bolt loosening were assessed.Results: Compared to the control group, the HTFB group reached significant higher consolidation after 3 month. No dislocation and a single bolt breakage was observed in the HTFB group after the fourth month, while the control group included 3 patients with bolt dislocation at 3±1 month and 5 patients with bolt breakage at 6±1.8 month. Compared to preoperative values, the improvement of Meary's angle after one-year was significant higher in the HTFB group (23.4°±14) than in controls (11.7°±13).Conclusions: Modification of bolt design improves the stability of the medial column: A higher rate of osseous consolidation of the medial column leads to lower rate of bolt dislocation/breakage and finally to permanently erected longitudinal foot arch. Flat-foot involves total collapse of medial longitudinal arch, which becomes distressful when children begin to stand on their feet. Flat foot is usually diagnosed on clinical examination, however due to lack of a standardized protocol, there are conflicting views regarding its management. The objective of this study is to determine the accuracy of radiographic and podometric measurements in diagnosing paediatric flat foot. Methods: It was a cross sectional diagnostic accuracy study. Eighty-four children of preschool and school going age were recruited equally into control and flat foot groups. Each child was examined clinically and was further classified into having flexible or rigid flat foot. For radiographic assessment, lateral and anteroposterior foot radiographs were taken while footprints were captured using podoscope and analysed with FREESTEP software. The diagnostic accuracy and correlation among different parameters were also computed. Results: Clinically, of the 42 children in the flat foot group, 26 had flexible while 16 had rigid flat foot. Of these, 40 children were flat footed on one or more radiographic parameters, while 36 had podometric measurements within the flat foot range. Radiography had the sensitivity and specificity of 95% and 69%, respectively, while podometry was 86% sensitive and 47% specific. Clark's angle had highest sensitivity of 90%, with AUC of 0.952. A strong positive correlation was found between arch index and talocalcaneal angle (r=.805). Conclusion: The diagnostic accuracy of radiography was more than podometry. More specific and sensitive parameters in combination with clinical assessment may prove to be useful in the management of paediatric flat foot. [ABSTRACT FROM AUTHOR]

Prestressed concrete (PC) continuous girder-arch composite bridges, using concrete filled steel tubes (CFST) as the arch ribs to support the PC main girder, have gained their popularity in recently constructed large span bridges. The adoption of CFSTs brings many advantages, such as the concrete infill help to prevent the occurrence of local buckling in steel tubes, while the steel tubes can serve as framework for concrete core and strengthen the encased concrete. In order to achieve the above merits, the arch foots connecting the CFST arch ribs and PC girder are supposed to have sufficient strength and stiffness. However, the extensive use of CFST arch ribs showed that concrete cracking are commonly found at the arch foots. This paper studied the structural behavior of arch foot by employing a comprehensive numerical model. The influence of pumping pressure from concrete infill at construction stage on local stress of the arch foots was presented and discussed. The results indicated that the radial stress produced by pumping pressure at the joint zone squeezed the concrete around the arch foots, which resulted in significant hoop tensile stresses in the surrounding concrete. The outcomes of this study can provide technical support to improve the construction quality and structure stability, and optimize the construction procedures for this type of bridges. The objective of this research is to relate the performance in three vertical jump events with morphological variables of the foot and stature. A total of 177 practitioners of 12 sporting events aged 24.5 ±8.0 years, with 71.01 ±13.00 kg of body mass, 1.71±0.09 m height, and BMI of 24.29±3.24 kg·m-2 were evaluated with an anthropometer in terms of foot length (FL), forefoot width (FW), navicular height (NH), and hindfoot width (HW). These variables were normalized to the height of the subjects. From the footprint record the arch index (AI) was obtained, which indicates the morphology of the medial longitudinal foot arch (MLFA). Performance was evaluated in three vertical jump events: countermovement jump (CMJ), squat jump (SJ), and drop jump (DJ), recording the height reached. FL, FW and HW show a weak positive correlation (r<0.4; p<0.05) with the heights achieved in the three types of jump. The stature is strongly associated with FL, FW and HW (r=0.8; r=0.7 and r=0.6; respectively; p<0.05) and with the height in CMJ, SJ, and DJ (r=0.37; r=0.41 and r=0.32, respectively, p<0.05). The only normalized morphological foot variable that maintained consistency in the correlations analysis was the normalized foot length (NFL) with CMJ (r = 0.2, p<0,05). The subjects whose left foot length was equivalent to 14 % of the stature jumped 27.94 ±6.63 cm, those with 15 % jumped 30.96 ±7.4 cm, and those with 16 % jumped 31.03 ±7.8 cm. FL, FW, HW, and stature are moderately correlated with performance in vertical jump events. Pes planus ("flat feet") is one of the most common orthopaedic conditions observed in adult health practice. The purpose of this study was to evaluate the foot arch quality and the occurrence of toes deformities among physiotherapy students (N = 106; 33 men and 73 women). The mean age of students was 22.28 years. The CQ-ST Podoscope was used as the main research tool. We evaluated the Clark's angle and Weisflog index as foot parameters to assess the quality of the longitudinal foot arch and the transverse foot arch, respectively. We also evaluated the incidence of toes deformities of the big toe angle (ALFA) and little toe angle (BETA) according to Weisflog. Most students had a physiologically arched longitudinal foot arch. Despite the young age of subjects, we found an increased occurrence of the flat transverse foot arch and big toe deformities, especially among women. [ABSTRACT FROM AUTHOR]

Swing is consider across his simpliness as complex exercise, which brings a lot of benefits. How swing effects the arch of foot is according to available resources little explored area. With weakened muscles, the arch decreases (flattening) even during the standing, resting phase. According to Hoke, no mechanism can replace the function m. posterior tibialis, m. tibialis anterior a m. flexor hallucis longus while maintaining the arch. The researchs shows that the occurrence of flat and high foot can be considered serious even in the younger population, which formed a sample in our research. Improper arching of the foot arch can also result in incorrect posture. That is why we decided in our research to focus on this possible effect of the kettlebell swing exercise. The main aim of our research was to find out the effect of the swing exercise on the arch of the foot and how a possible change in the footprint taken by the podoscope will show. On research has participated 16 respondents, most of them active sportsmen. Arch of foot was evaluated befor beginning of six weeks cycle of doing swing exercises three times per week and after at the end of the cycle by podoscope. The pictures captured by podoscope were evaluated by Chippaux-Smirak index, which characterized level of arch. After the anlysis of results has been shown statisticaly important effect of swing on arch of foot. In flat foot has been shown effect of increasing its arch and at high arch has been shown the oposite effect decreasing its arch. Partial aim was to find out the subjective feelings of the respondents after the end of testing, where lots of respondents experienced improvements in different eras. Using a dedicated three-compartment forefoot subdivision, we studied asymptomatic subjects and quantified disorders related to the metatarsal arch. Foot motion and arch shape were measured in 30 healthy subjects with a motion-capture system and force plates. Kinematic results were expressed using a novel model, which anatomically divides the forefoot into three parts. This model integrated the medial longitudinal arch angle and the metatarsal arch height and width. During the first part of stance phase, the medial longitudinal arch flattens and all foot segments move toward dorsiflexion. During terminal stance and preswing phase, medial longitudinal and metatarsal arch restoration was noted with plantarflexion of all segments, an eversion and abduction of the medial forefoot, and an inversion and adduction of the lateral forefoot. Kinematics obtained with the proposed forefoot model corroborates metatarsal arch restoration in late stance. This observation supports the fact that foot architecture is supple until midstance and subsequently creates a rigid lever arm with restored arches to support propulsion. The intrinsic muscles of the foot are important to maintain the arch of the foot and to participate in sports activities. Using ultrasound shear wave elastography, we investigated the effect of different toe flexion methods on the activity of the intrinsic and extrinsic muscles of the foot. The study included 15 healthy adults who performed toe flexion under 2 conditions: with interphalangeal (IP) joint flexion and without IP joint flexion. The applied load during flexion was 500 g. Muscle stiffness was measured in the abductor hallucis, flexor hallucis brevis, flexor digitorum brevis, quadratus plantae, flexor hallucis longus, and flexor digitorum longus muscles using ultrasound shear wave elastography. Muscle stiffness was statistically compared with IP flexion and without IP flexion (P < 0.05). The stiffness of the abductor hallucis (P < 0.0005), flexor hallucis brevis (P = 0.022), and flexor digitorum brevis muscles (P < 0.0005) was significantly greater without IP flexion than with IP flexion. In contrast, the muscle stiffness of the flexor hallucis longus (P = 0.001) and the flexor digitorum longus (P = 0.004) was significantly greater during with IP flexion than without IP flexion. This study shows that the abductor hallucis, flexor hallucis brevis, flexor digitorum brevis muscles are more active during toe flexion without IP flexion. Introduction. The foot due to its complex structure provides three basic functions: supportive, locomotive and amortization. The human foot is an important static-dynamic part of the motor system. A properly shaped foot should be supported by three points: the first and the fifth metatarsal bone and the external part of the heel. Purpose of research. The aim of the study was to analyze the feet construction and find the relationships between the foot structure parameters and BMI among 8-year-old boys. Material and methods. The study involved 85 boys. The weight and the body height of the children were measured, body mass index was calculated, and the footprint using the was made. On the basis of the foot picture, the basic parameters for foot assessment as well as relations between these indicators and BMI were calculated. Subsequently, the Clarke angle, the hallux valgus angle and Wejsflog index were determined in manual way. Then the dependency between these parameters and BMI was investigated. Results. The number of lower foot arch or flat foot in the study group amounted to 53%, the transverse flat foot was observed among 17% of the studied boys feet and the hallux valgus angle beyond the norm occurred in 9% of the studied feet. There is a relationship between the Clark’s angle, Wejsflog index, hallux valgus angle α and BMI of boys. Conclusions. Boys with a higher BMI have lower values of the Clark angle and the Wejsflog index which are the evidence of a lower longitudinal and transverse arch. Medial longitudinal arch characteristics are thought to be a contributing factor to lower limb running injuries. Running biomechanics associated with different foot types have been proposed as one of the potential underlying mechanisms. However, no systematic review has investigated this relationship. The aim of this study was to conduct a systematic literature search and synthesize the evidence about the relationship between foot posture and running biomechanics. For this systematic review and meta-analysis different electronic databases (Pubmed, Web of Science, Cochrane, SportDiscus) were searched to identify studies investigating the relationship between medial longitudinal arch characteristics and running biomechanics. After identification of relevant articles, two independent researchers determined the risk of bias of included studies. For homogenous outcomes, data pooling and meta-analysis (random effects model) was performed, and levels of evidence determined. Of the 4088 studies initially identified, a total of 25 studies were included in the qualitative review and seven in the quantitative analysis. Most studies had moderate and three studies a low risk of bias. Moderate evidence was found for a relationship between foot posture and subtalar joint kinematics (small pooled effects: −0.59; 95%CI −1.14 to − 0.003) and leg stiffness (small pooled effect: 0.59; 95%CI 0.18 to 0.99). Limited or very limited evidence was found for a relationship with forefoot kinematics, tibial/leg rotation, tibial acceleration/shock, plantar pressure distribution, plantar fascia tension and ankle kinetics as well as an interaction of foot type and footwear regarding tibial rotation. While there is evidence for an association between foot posture and subtalar joint kinematics and leg stiffness, no clear relationship was found for other biomechanical outcomes. Since a comprehensive meta-analysis was limited by the heterogeneity of included studies future research would benefit from consensus in foot assessment and more homogenous study designs. Background: The demonstration of the relationship between midfoot passive mechanical resistance and foot pronation during gait may guide the development of assessment and intervention methods to modify foot motion during gait and to alter midfoot passive mechanical resistance.Research Question: Is foot pronation during the stance phase of gait related to the midfoot passive mechanical resistance to inversion?Methods: The resistance torque and stiffness provided by midfoot soft tissues of 33 participants (21 females and 12 males) with average of 26.21 years were measured. In addition, the participants' forefoot and rearfoot kinematic data during the stance phase of gait were collected with the Qualisys System (Oqus 7+). Correlation Coefficients were calculated to test the association between kinematic variables representing pronation (forefoot-rearfoot inversion, forefoot-rearfoot dorsiflexion and rearfoot-shank eversion) and maximum resistance torque and maximum stiffness of the midfoot with α = 0.05.Results: Reduced maximum midfoot resistance torque was moderately associated with increased forefoot-rearfoot inversion peak (p = 0.029; r = 0.38), with forefoot-rearfoot dorsiflexion peak (p = 0.048; r = -0.35) and with rearfoot-shank eversion peak (p = 0.008; r = -0.45). Maximum midfoot stiffness was not associated to foot pronation.Significance: The smaller the midfoot resistance torque, the greater the forefoot-rearfoot inversion and dorsiflexion peaks and the rearfoot-shank eversion peak during gait. The findings suggest the existence of a relationship between foot pronation and midfoot passive mechanical resistance. Abstract Forefoot strike becomes popular among runners because it facilitates better impact attenuation. However, forefoot strike may overload the plantar fascia and impose risk of plantar fasciitis. This study aimed to examine and compare the foot arch deformation and plantar fascia tension between different foot strike techniques in running using a computational modelling approach. A three-dimensional finite element foot model was reconstructed from the MRI of a healthy runner. The foot model included twenty bones, bulk soft tissue, ligaments, tendons, and plantar fascia. The time-series data of segmental kinematics, foot muscle force, and ankle joint reaction force were derived from a musculoskeletal model of the same participant based on the motion capture analysis and input as the boundary conditions for the finite element analysis. Rearfoot strike and forefoot strike running were simulated using a dynamic explicit solver. The results showed that, compared to rearfoot strike, forefoot strike reduced the foot arch height by 9.12% and increased the medial longitudinal arch angle by 2.06%. Forefoot strike also increased the plantar connective tissues stress by 18.28–200.11% and increased the plantar fascia tensile force by 18.71–109.10%. Although it is currently difficult to estimate the threshold value of stress or force that results in injury, forefoot strike runners appeared to be more vulnerable to plantar fasciitis. Introduction: Osteoarthritis knee is a common condition and is leading to long-term physical disability that affects a large proportion of the older population. Increased medial compartment loading has been observed in patients with osteoarthritis knee. The feet are important to proper biomechanics but often neglected. Simply stated a foot arch that overpronates can cause pain in the knee, hip, shins, foot, and lower back due to poor alignment of the leg. Materials and Methods: A total of 40 patients with overpronated foot along with medial compartment osteoarthritis knee were selected according to the inclusion and exclusion criteria. Detailed procedure was explained to the patients in colloquial terms, and informed consent was given to those were interested. The selected patients were randomly assigned into two groups 20 in each group using the lottery method. The Group A patients were treated with Interferential Therapy along and isometric exercises to knee and the Group B patients were treated with Interferential Therapy and isometric exercise to knee along with short foot exercise. Results: From statistical analysis made with the quantitative data revealed that there was statistically significant difference found between the two groups (Control-Group A and Experimental-Group B) in which Group B is found to be more effective when compared with Group A. The post-test mean values of navicular drop test for Group A and Group B are 13.35 and 10.60 with a standard deviation of 1.81 and 1.43, respectively. Conclusion: The study concluded that group with short foot exercise along with Interferential Therapy and isometric showed a higher level of positive outcome in terms of decreasing the pain and improving the functional activities of the lower limb. Abstract Introduction Anthropometric changes to the feet with pregnancy may increase risk for musculoskeletal disease. Customized arch-supportive orthoses to prevent anthropometric changes could potentially provide an inexpensive means to prevent musculoskeletal impairments and improve quality of life for women during their post-reproductive years. The objective of this prospective, randomized controlled trial was to determine whether customized foot orthoses can prevent arch collapse during pregnancy. Materials and methods Seventy-two first trimester women (age 18–40) were randomized to wear either their usual footwear (control) or their usual footwear with custom-molded arch-supportive orthoses. Each participant in the customized orthosis group had orthoses customized by a certified orthotist. Baseline assessments were completed during the first trimester and follow-up assessments were completed approximately 8 weeks postpartum. The primary outcome measurements were static arch height index (AHI), arch drop, arch rigidity index, and center of pressure excursion index (CPEI). Results There were no statistically significant differences in the change in static foot structure or dynamic arch function between baseline and follow-up within either group. Comparing the customized orthosis and control groups, no significant differences were detected in change in AHI sitting (p =.44), AHI standing (p =.48), arch drop (p =.67), arch rigidity (p =.68) or CPEI (p =.77). Conclusions There was no difference in arch change when comparing women who were randomized to wear customized orthoses or self-selected footwear. The finding of no arch drop in either group may indicate that both groups supported their arches during the study period or that neither group was predisposed to lose arch height with pregnancy. The objective of the present study was to comparatively analyze the clinical status of the lower limbs in healthy subjects and subjects with back pain. Forty three (43) male and female subjects with average age of 36.91± 3.97 were selected as the statistical sample of the study. This population was recognized intelligently by utilizing the Nordic Musculoskeletal Questionnaire (NMQ). Furthermore, the NMQ was used to designate 43 male and female subjects with average age of 37.88 ± 4.12 and similar anthropometric characteristics with the statistical population as the research control sample. In the next phase, tibial torsion and femoral torsion angles were measured using a goniometer. Foot typicality was designated using a foot arch index while intercondylar and intermalleolar distances were measured using a clinical caliper. Based on our own research and the literature overview we verified the concept of the transverse arch of the foot and the use of this concept in the aetiology of acquired female foot changes. The results of podological studies carried out for many years in the Orthopaedic Department in Lublin have not demonstrated the existence of a transversal arch, and thus questioned its impact on the formation of forefoot changes in women. In order to obtain a fully objective assessment of the issue, a study was carried out at the Vincent Pol University in Lublin in 2013, including, among others, a dynamic podographic evaluation of 25 women aged from 17 to 26 years. The analysis of the results obtained undermined the historical view of the existence of the transversal arch of the foot and its role in the formation of acquired pathological changes of the forefoot. Aim: In view of the notion of the transversal arch of the foot that can still be found in a number of publications attributing to it a significant function in the formation of forefoot overload in women, the aim of the present study was to explain the above issue explicitly.
