Anaplastic thyroid cancer (ATC) is considered to be one of the most virulent, treatment-refractory malignancies. Recent molecular insights into the biology of thyroid cancer have transformed ATC management, and BRAF/MEK targeted therapy is now incorporated into guideline-based multidisciplinary care. We report visceral perforation in the setting of an extreme response to such therapy in a patient with ATC. Molecularly targeted therapy afforded a dramatic but life-threatening response to treatment. This report highlights the complexities of care for the patient and treating clinicians.

The 2018 discovery of the probable and possible human carcinogen N-nitrosodimethylamine (NDMA), in the generic API valsartan, used to treat high blood pressure, began a chain of recalls and a [...], In this paper, the authors introduce a method that combines ion exchange chromatography with a easier-to-perform, simpler one-step post-column derivatization that is selective to nitrite and visible spectrophotometric detection to allow high sample loading volumes without affecting resolution. The method introduced here can be broadly applied to multiple polymeric excipient types. Ion-exchange chromatography post-column derivatization is used to determine ultra-low levels of nitrite in three excipients.

Hamstring injuries (HSIs) are prevalent in sports that involve changes in direction, kicking, and sprinting. These injuries are a major cause of time lost from competition, practice, and training, as well as increased healthcare costs. In a Division I collegiate football program, the authors implemented a multifactorial approach that included repeated performance assessments, detailed data analysis, and a flexible strength and conditioning regimen. Over a three-year period, this resulted in no game time loss due to HSI. This model can be adapted and implemented across sports settings.

19. Bystander Intervention in Bullying and Sexual Harassment Training: Mixed-Method Evaluation of NAB IT!

- Abstract
The goal of the current pilot study was to examine the effects of bystander intervention training on a sample of 27 high school students who were selected by their peers as opinion leaders. Measures of bystander intervention knowledge, attitudes, and behaviors were included in the within-subjects design. Sixteen of the students also participated in focus groups to provide feedback about their experiences with the training. Results showed that participating students reported increases in knowledge about bullying and sexual harassment, confidence in intervening, acceptance of responsibility, knowing what to do, and acting to intervene (including direct intervention, providing support, and reporting to an adult) from pre-to posttest. Themes from focus groups indicated that students found the training relevant and helpful, particularly for focusing on both bullying and sexual harassment and for providing opportunities to learn and practice multiple intervention strategies. Barriers to intervening based on the type of violence (e.g., physical vs. relational or verbal) and relationship with the people involved were also themes. Directions for future research and implications for practice, such as focusing on dissemination to the larger student body and implementing bystander intervention training within the context of a positive school climate that emphasizes social and emotional competencies, are discussed. 20. Bystander Intervention in Bullying and Sexual Harassment: Role of Personal and Perceived Peer Norms

This study investigated the extent to which adolescents' personal normative attitudes (also referred to as personal norms) and perceived peer norms regarding bullying, sexual harassment, and bystander intervention predicted each step of the five-step bystander intervention model (i.e., Notice, Interpret, Accept Responsibility, Know how to Help, Act) for bullying and sexual harassment among two-hundred thirty-three high school students in the Northeastern United States. Interaction effects of gender, personal norms, and perceived peer norms were also assessed. As predicted, perceived peer norms moderated the relations between personal norms and all five bystander intervention steps. However, some effects differed by gender and some differed in direction from predictions. Students who were more anti-bullying/harassment scored higher on some bystander intervention steps when they also perceived their peers to be more anti-bullying and harassment, with some models showing gender differences between male and female students. Personal and perceived peer norms are related to adolescents' engagement in the bystander intervention model, suggesting that both norms should be targets of interventions encouraging youth to intervene in incidents of bullying and sexual harassment.

Purpose: To evaluate the relationship between preoperative whole-joint imaging evaluation of the knee with patient-reported outcome (PRO) measures after cartilage restoration surgery (mosaicplasty, osteochondral allograft transplantation, matrix autologous chondrocyte implantation). Methods: We retrospectively evaluated patients who underwent knee articular cartilage restoration at our institution from 2014 to 2020. The patients’ knee magnetic resonance imaging (MRI) was evaluated with the Whole-Organ Magnetic Resonance Imaging Score (WORMS) and semiquantitative synovial inflammation imaging biomarkers of the preoperative MRI. To assess PRO score, Lysholm score and Knee injury and Osteoarthritis Outcome Score were completed at a minimum 2-year follow-up. Statistical analysis was performed using the Spearman rank test to obtain correlation values for WORMS score and PRO score for each survey. Results: Forty patients were enrolled in this study. The average age at baseline was 34.5 years. The average body mass index was 28.2, and 26 of 40 were men (age range, 20-58 years). The maximum preoperative WORMS score was significantly correlated with the postoperative Lysholm score (r = –0.52, P = .0013). The WORMS Meniscus and Cartilage subscales were significantly correlated with the Lysholm score (r = –0.36, P = .024 and r = –0.37, P = .021, respectively). The maximum WORMS score was significantly correlated with the Knee injury and Osteoarthritis Outcome Score daily living and sports/recreation subscores (r = –0.47, P = .0023 and r = –0.42, P = .0077, respectively). Semiquantitative synovial inflammation imaging biomarkers were not significantly correlated with PRO scores. Conclusions: Increasing preoperative degenerative change in the knee, as evidenced by a higher WORMS on preoperative MRI, was associated with inferior patient-reported outcomes at a minimum of 2 years after cartilage restoration surgery (mosaicplasty, osteochondral allograft transplantation, matrix autologous chondrocyte implantation). Semiquantitative scoring of the whole joint on preoperative MRI may allow for improved counseling regarding expected benefit for patients after cartilage restoration surgery. Level of Evidence: Level IV, prognostic case series.

22. Combined Medial Patellofemoral Ligament Reconstruction and Tibial Tubercle Osteotomy Has a Lower Risk of Recurrent Instability Requiring Revision Stabilization at 2 Years Than Either Procedure Alone

Purpose: To use a large nationwide administrative database to directly compare usage, complications, and need for revision stabilization surgery after medial patellofemoral ligament reconstruction (MPLFR), tibial tubercle osteotomy (TTO), and combined MPFLR and TTO (MPFLRTTO). Methods: The PearlDiver Mariner database was queried for all reported cases of MPLFR, TTO, and combined MPFLRTTO performed between 2010 and 2020 using Current Procedural Terminology codes. Subsets from those cohorts with laterality-specific International Classification of Diseases, Tenth Revision, codes for patellar instability were used to evaluate 2-year incidence of infection, stiffness, fracture, and revision stabilization with MPFLR and/or TTO. Multiple linear regression and χ2 analysis were used to analyze incidence trends and to compare complication rates. Results: A total of 70,070 patients were identified. MPFLR was found to be the most common procedure (73.1%), followed by TTO (19.2%) and then MPFLRTTO (7.6%). MPLFR was observed to have the lowest overall complication rate (5.4%), whereas both TTO (7.5%) and MPFLRTTO (7.1%) had greater complication rates (P < .001). MPFLR had the greatest rate of revision stabilization surgery at 3.7% compared with TTO at 2.7% and MPFLRTTO, which carried the lowest risk for revision at 2.4% (P < .001). Conclusions: Isolated MPFLR is the most common modality used for patellar instability, with increasing prevalence and the lowest 2-year complication rate. Isolated TTO was unchanged in its use and had the greatest overall complication rate. Combined MPFLRTTO increased the overall complication rate but had a lower 2-year rate of recurrent instability requiring revision than MPFLR alone. 23. The Role of Personal and Perceived Peer Norms in Bullying and Sexual Harassment Perpetration

Bullying, cyberbullying, and sexual harassment can be impacted by both personal attitudes and perceived social norms, although few empirical studies on this topic have been conducted with high school students. In this cross-sectional study, 233 high school students completed measures about personal normative attitudes, perceptions of peer norms, and perpetration of bullying, cyberbullying, and sexual harassment. Consistent with social norms theory, students perceived themselves to hold more prosocial (i.e., anti-bullying/anti-sexual harassment) personal normative attitudes than they perceived the typical student in their school to hold (i.e., peer norms). Path analyses revealed that students' personal normative attitudes (e.g., anti-bullying/anti-harassment) were negatively related to their bullying, cyberbullying, and sexual harassment perpetration, although perceived peer norms were negatively related to sexual harassment perpetration only. Multiple-group path analysis revealed significant gender differences. Personal normative attitudes related to females' behavior for all forms of perpetration and only sexual harassment and cyberbullying for males (with more anti-bullying/anti-harassment attitudes relating to less perpetration), although associations for males were stronger. Perceived peer norms related to bullying perpetration for males only. Results are discussed with regard to social-cognitive and peer contextual factors and implications for social norms interventions. 25. Impact of surgeon volume, experience, and training on outcomes after arthroscopic rotator cuff repair: a nationwide analysis of 1489 surgeons

Background: Given the complexity of arthroscopic rotator cuff repair (ARCR) and increasing prevalence, there is a need for comprehensive, large-scale studies that investigate potential correlations between surgeon-specific factors and postoperative outcomes after ARCR. This study examines how surgeon-specific factors including case volume, career length, fellowship training, practice setting, and regional practice impact two-year reoperation rates, conversion to total shoulder arthroplasty (anatomic or reverse), and 90-day post-ARCR hospitalization. Methods: The PearlDiver Mariner database was used to collect surgeon-specific variables and query patients who underwent ARCR from 2015 to 2018. Patient outcomes were tracked for two years, including reoperations, hospitalizations, and International Classification of Diseases, Tenth Revision codes for revision rotator cuff repair (RCR) laterality. Hospitalizations were defined as any emergency department (ED) visit or hospital readmission within 90 days after primary ARCR. Surgeon-specific factors including surgeon case volume, career length, fellowship training, practice setting, and regional practice were analyzed in relation to postoperative outcomes using both univariate and multivariate logistic regression. Results: 94,150 patients underwent ARCR by 1489 surgeons. On multivariate analysis, high-volume surgeons demonstrated a higher risk for two-year total reoperation (odds ratio [OR] = 1.06, 95% confidence interval [CI]: 1.01-1.12, P = .03) and revision RCR (OR = 1.06, 95% CI: 1.01-1.12, P = .02) compared to low-volume surgeons. Mechanical stimuli, such as stretch, shear stress, or compression, activate a range of biomolecular responses through cellular mechanotransduction. In the nervous system, studies on mechanical stress have highlighted key pathophysiological mechanisms underlying traumatic injury and neurodegenerative diseases. However, the biomolecular pathways triggered by mechanical stimuli in the nervous system has not been fully explored, especially compared to other body systems. This gap in knowledge may be due to the wide variety of methods and definitions used in research. Additionally, as mechanical stimulation techniques such as ultrasound and electromagnetic stimulation are increasingly utilized in psychological and neurorehabilitation treatments, it is vital to understand the underlying biological mechanisms in order to develop accurate pathophysiological models and enhance therapeutic interventions. This review aims to summarize the cellular signaling pathways activated by various mechanical and electromagnetic stimuli with a particular focus on the mammalian nervous system. Furthermore, we briefly discuss potential cellular mechanosensors involved in these processes.

Meniscal injury is common, and despite modern techniques, the failure rate following repair remains high. While there are recent treatment advances in the form of biologics, there is limited evidence and agreement on these emerging therapies and their role in meniscal healing. Amniotic tissue (umbilical cord allograft) is a biologic augmentation therapy that has been utilized in other musculoskeletal applications but has not been reported for use in meniscal repair. We describe a technique to deliver an allograft amniotic membrane into a meniscus tear repair site, potentially optimizing healing.

ABSTRACT The natural forest ecosystems of South Florida, USA, support a high biodiversity of plant and animal species and provide valuable ecosystem services. However, these ecosystems remain poorly represented in global studies, primarily due to a paucity of standardized data. Here, we present previously unpublished data from 332 censuses of 54 permanent 1‐ha tree inventory plots in the Racoon Point area of Big Cypress National Preserve, Florida, USA, including a total of nearly 100,000 measurements (diameter or height) of > 17,000 individual living trees and palms (with additional measurements of nearly 6000 dead pine snags) collected sporadically over a 19‐year period (1993–2012). These data, which were originally collected as part of a project to investigate tree responses to different experimental burning regimes, provide unique insight into the diversity, composition, structure, and dynamics of South Florida's unique and endangered pine forest ecosystems. Data files include the species identity, size (dbh = diameter at breast height), and location of all trees ≥ 5 cm dbh in 54 individual tree plots. Additional data are provided about heights of palm trees, and the location and burn history of each plot. These data are freely available for noncommercial scientific use under a Creative Commons license; users are encouraged to cite this paper when using the data.

40. Using Animated Videos to Increase Patient Knowledge: A Meta-Analytic Review

- Abstract
This article meta-analyzed 21 studies that tested the effectiveness of animated videos in improving learning in clinical and nonclinical settings compared with standard education. Animation was defined as the use of moving objects that are typically drawn or simulated. Videos ranged from just over 2 min in duration to 16 min in duration in articles published from 2009 through 2020. Mayer's Cognitive Theory of Multimedia Learning provided the theoretical model to frame the current analyses. Findings indicated an overall positive effect (d = 0.35) for use of animation in improving viewers' learning across a variety of health and clinical contexts, including surgery and diabetes. Moderator analyses indicated learning effects were greater in patient samples and samples with a higher proportion of male participants. Study findings were discussed in terms of the theoretical and practical implications for health communication scholars and practitioners.

Foraging is essential for animal survival, as it provides the nutritional resources to sustain metabolism and all activities that animals undertake. Communal latrines are sites where multiple individuals of the same species defecate and can have multiple functions. Latrine behavior has been recorded in many animal species, including lowland tapirs (Tapirus terrestris), which consume a wide variety of plants and fruits and defecate large piles of dung, sometimes containing many seeds. Due to the concentration of seeds and other defecated material, latrines may thus serve as direct sources of food for other vertebrates, including insectivores feeding on invertebrates attracted to dung. Here, we studied which vertebrate animals were recorded foraging in tapir latrines, how long they spent foraging on latrines, and how their foraging behavior varied with the time of day and days since tapir defecation. Since tapirs are generalist foragers with a large diet breadth, we expected that their feces might attract vertebrate foragers with a variety of dietary habits (e.g., frugivores, granivores, omnivores, insectivores) in distinct foraging periods within a day. We also expected that the foraging events would be greatest in the days immediately following tapir defecation. We monitored 27 tapir latrines with camera traps for six months, recording foraging vs. non-foraging behaviors. We observed nine vertebrate species foraging in tapir latrines with five frequent foragers: the frugivores Guerlinguetus brasiliensis (50% of total foraging records) and Tinamus solitarius (16%, also a granivore) and the omnivores Turdus albicollis (18%), Chamaeza campanisona (10%) and Odontophorus capueira (6%). Foraging and non-foraging events throughout the day differed significantly for G. brasiliensis (p= 0.006), T. solitarius (p= 0.01), and C. campanisona (p= 0.001), and all species were diurnal, foraging between 05:00 h and 18:00 h. The foraging probability of C. campanisona and T. albicollis was affected by the time lapse since tapir defecation. Our study highlights that communal latrines such as those maintained by lowland tapirs can be an important resource supply for some vertebrate consumers with different diets and could contribute to maintaining diversity in tropical forests. We call for future studies to inspect the importance of latrines as foraging sites for other species and ecosystems.

Background:. Plastic and reconstructive surgery is one of the most competitive residency programs, and given the increased number of applicants for a relatively fixed number of positions, successfully matching is a challenge. Match rates have declined since 2018, with a match rate of ~55% in 2022. Two common options before reapplying are a preliminary year of residency (preliminary year) or a research fellowship. This study investigated which option is more beneficial for reapplicants seeking a successful match. Methods:. This retrospective study included all applicants to an integrated plastic and reconstructive surgery residency from 2015 to 2023. Two cohorts based on reapplication strategy (research fellowship or preliminary year) were created. Demographic, applicant, and match data were collected. Pearson chi-squared, Fisher exact, and Wilcoxon rank sum testing were performed. Results:. In total, 125 reapplicants were included. Seventy-one (56.8%) reapplicants pursued a preliminary year, and 29 (23.2%) completed a research fellowship. Research fellowship reapplicants had a greater mean number of first author publications (8.8 versus 3.2, P < 0.001), non-first author publications (11.3 versus 5.9, P = 0.021), poster presentations (9.7 versus 6.0, P = 0.028), and oral presentations (11.8 versus 6.4, P

Introduction Assisted vaginal birth, also known as instrumental or operative vaginal birth, refers to a vaginal birth conducted with the help of an instrument such as forceps or a vacuum [...], Access to emergency obstetric care, including assisted vaginal birth and caesarean birth, is crucial for improving maternal and childbirth outcomes. However, although the proportion of births by caesarean section has increased during the last few decades, the use of assisted vaginal birth has declined. This is particularly the case in low- and middle-income countries, despite an assisted vaginal birth often being less risky than caesarean birth. We therefore conducted a three-step process to identify a research agenda necessary to increase the use of, or reintroduce, assisted vaginal birth: after conducting an evidence synthesis, which informed a consultation with technical experts who proposed an initial research agenda, we sought and incorporated the views of women's representatives of this agenda. This process has allowed us to identify a comprehensive research agenda, with topics categorized as: (i) the need to understand women's perceptions of assisted vaginal birth, and provide appropriate and reliable information; (ii) the importance of training health-care providers in clinical skills but also in respectful care, effective communication, shared decision-making and informed consent; and (iii) the barriers to and facilitators of implementation and sustainability. From women's feedback, we learned of the urgent need to recognize labour, childbirth and postpartum experiences as inherently physiological and dignified human processes, in which interventions should only be implemented if necessary. The promotion and/or reintroduction of assisted vaginal birth in low-resource settings requires governments, policy-makers and hospital administrators to support skilled health-care providers who can, in turn, respectfully support women in labour and childbirth. L'acces aux soins obstetriques d'urgence, y compris l'accouchement vaginal assiste et la cesarienne, est essentiel pour ameliorer les effets de la maternite et de l'accouchement. Toutefois, bien que la proportion de cesariennes ait augmente ces dernieres decennies, le recours a l'accouchement vaginal assiste a diminue. C'est particulierement le cas dans les pays a revenu faible ou intermediaire, bien que l'accouchement vaginal assiste soit souvent moins risque qu'une cesarienne. Nous avons donc mene un processus en trois etapes afin d'imaginer un programme de recherche qui permettrait d'augmenter le recours a l'accouchement vaginal assiste ou de le reintroduire. Apres avoir realise une synthese des donnees probantes, qui a servi de base a une consultation avec des experts techniques qui ont propose un programme de recherche initial, nous avons sollicite et incorpore les avis des representantes des femmes pour ce programme. Ce processus nous a permis d'imaginer un programme de recherche complet, avec des sujets classes comme suit: (i) la necessite de comprendre la perception qu'ont les femmes de l'accouchement vaginal assiste et de fournir des informations appropriees et fiables; (ii) l'importance de la formation des prestataires de soins de sante en matiere de competences cliniques, mais aussi de respect dans les soins de sante, de communication efficace, de prise de decision partagee et de consentement eclaire; ou (iii) les obstacles a la mise en reuvre et a la durabilite et les facteurs qui les facilitent. Les reactions de femmes nous ont appris qu'il etait urgent de reconnaitre que l'accouchement, la naissance et le post-partum sont des processus humains intrinsequement physiologiques et dignes au cours desquels les interventions ne devraient etre mises en reuvre qu'en cas de necessite. La promotion et/ou la reintroduction de l'accouchement vaginal assiste dans les regions a faibles ressources necessitent que les pouvoirs publics, les decideurs politiques et les administrations d'hopitaux soutiennent les prestataires de soins de sante qualifies, qui pourront a leur tour soutenir respectueusement les femmes pendant l'accouchement. El acceso a la atencion obstetrica de emergencia, incluido el parto vaginal asistido y el parto por cesarea, es crucial para mejorar los resultados de la maternidad y el parto. 48. Bystander Intervention in Bullying and Sexual Harassment: Role of Personal and Perceived Peer Norms

This study investigated the extent to which adolescents' personal normative attitudes (also referred to as personal norms) and perceived peer norms regarding bullying, sexual harassment, and bystander intervention predicted each step of the five-step bystander intervention model (i.e., Notice, Interpret, Accept Responsibility, Know how to Help, Act) for bullying and sexual harassment among two-hundred thirty-three high school students in the Northeastern United States. Interaction effects of gender, personal norms, and perceived peer norms were also assessed. As predicted, perceived peer norms moderated the relations between personal norms and all five bystander intervention steps. However, some effects differed by gender and some differed in direction from predictions. Students who were more anti-bullying/harassment scored higher on some bystander intervention steps when they also perceived their peers to be more anti-bullying and harassment, with some models showing gender differences between male and female students. Personal and perceived peer norms are related to adolescents' engagement in the bystander intervention model, suggesting that both norms should be targets of interventions encouraging youth to intervene in incidents of bullying and sexual harassment.

Background: Anatomic total shoulder arthroplasty (ATSA) and reverse total shoulder arthroplasty (RTSA) reliably alleviate pain and restore shoulder function for a variety of indications. However, these procedures are not well-studied in patients with neurocognitive impairment. Therefore, the purpose of this study was to investigate whether patients with dementia or mild cognitive impairment (MCI) have increased odds of surgical or medical complications following arthroplasty. Methods: The PearlDiver database was queried from 2010 through October 2021 to identify a cohort of patients who underwent either ATSA or RTSA and had a minimum 2-year follow-up. Current Procedural Terminology and International Classification of Diseases codes were used to stratify this cohort into three groups: (1) patients with dementia, (2) patients with MCI, and (3) patients with neither condition. Surgical and medical complication rates were compared among these three groups. Results: The overall prevalence of neurocognitive impairment among patients undergoing total shoulder arthroplasty was 3.0% in a cohort of 92,022 patients.
