1. Defected fuel rods identification in TRIGA Reactors: The experience at the ENEA Casaccia TRIGA RC-1 reactor

Abstract

Experience in running TRIGA reactors all around the world has shown that fuel cladding failures can occur. Fission products, especially in gaseous physical form, can exit the defected fuel rods being dispersed within the water primary coolant. Suspects of a cladding failure event can be confirmed by detection of short-lived fission products, i.e., Krypton, Xenon, and even Iodine isotopes in primary water, or within the ionic-exchange resins tank installed for the purification of the primary coolant loop. The magnitude of the release of those ‘key-indicators’ from the defected fuel element(s) is the driving method by which the defected rod(s) can be identified. ‘Significant’ releases can be detected with a direct online sampling of the water from the top of the suspected fuel rod with reactor at power, leading this water to an online high-resolution gamma spectrometry analysis system. By considering delays due to the water velocity in tubes and decay time of radionuclides identified within the gamma spectrum, it is possible to calculate the concentration of those radionuclides just emerging from the inspected fuel rod. Releases lower than the minimum detection capabilities of the previous online experimental configuration push to modify the detection method with an indirect identification of the release. This is the case when the ‘normal’ radioactive background of the activated water, when reactor is on power, is the dominant component in the gamma spectrum of the sampled water, and fission gases (even produced) are not identified promptly, i.e. a relationship to a specific fuel rod by the sampling circuit before could not be identified. The paper describes the experience carried out at the Italian ENEA TRIGA RC-1 reactor, deepening the technical aspects and solutions applied to solve the issue. In particular, a ‘significant’ release has been found in the instrumented fuel rod within ring B, i.e. the inner fuel ring, exposed to the maximum neutron flux of the reactor. The leaking element was found within a week, in two days of operation, being the sampling system designed on detection of minutes-shortlived fission gases (no need to ‘cool’ down the primary loop by waiting decay of hours-lived radionuclides). After the removal of the instrumented fuel rod in ring B, further days in searching other ‘significant’ release with the sampling circuit before have reported nothing detected. But in samples of water taken after reactor shutdown, some iodine emerged after decaying of ‘normal’ radioactive background of the activated water. This was sufficient evidence of another defected fuel rod in the pool. Identification of the latter defected fuel element is still ongoing, being based on: 1) selective removal of rod(s) from the reactor core, 2) run power of the reactor, 3) take samples of water after shutdown and measure iodine after decaying of ‘normal’ radioactive-background of the activated water, 4) identify the rod responsible of the remaining leakage when no further iodine is detected.

Abstract

The shallow landslides constitute a consistent current hazard that could increase in the future, especially in the Mediterranean areas. Regional scale hazard maps involving runout estimation are not yet particularly widespread. The study was conducted with the aim of testing a debris flows risk assessment process starting from the runout analysis of a debris flows dataset triggered by the event of October 1, 2009.Preliminary results highlight how the procedure permit to produce a runout hazard assessment in the valley areas where the more important and vulnerable exposed elements are situated. Despite the obvious limitations, the process appears particularly useful to address the public administration demand for simple tools to manage and direct regional planning. [ABSTRACT FROM AUTHOR]

Abstract

We provide a database of the coseismic geological surface effects following the Mw 6.5 Norcia earthquake that hit central Italy on 30 October 2016. This was one of the strongest seismic events to occur in Europe in the past thirty years, causing complex surface ruptures over an area of >400 km2. The database originated from the collaboration of several European teams (Open EMERGEO Working Group; about 130 researchers) coordinated by the Istituto Nazionale di Geofisica e Vulcanologia. The observations were collected by performing detailed field surveys in the epicentral region in order to describe the geometry and kinematics of surface faulting, and subsequently of landslides and other secondary coseismic effects. The resulting database consists of homogeneous georeferenced records identifying 7323 observation points, each of which contains 18 numeric and string fields of relevant information. This database will impact future earthquake studies focused on modelling of the seismic processes in active extensional settings, updating probabilistic estimates of slip distribution, and assessing the hazard of surface faulting.

Abstract

We present a 1:25,000 scale map of the coseismic surface ruptures following the 30 October 2016 M-w 6.5 Norcia normal-faulting earthquake, central Italy. Detailed rupture mapping is based on almost 11,000 oblique photographs taken from helicopter flights, that has been verified and integrated with field data (>7000 measurements). Thanks to the common efforts of the Open EMERGEO Working Group (130 people, 25 research institutions and universities from Europe), we were able to document a complex surface faulting pattern with a dominant strike of N135 degrees-160 degrees (SW-dipping) and a subordinate strike of N320 degrees-345 degrees (NE-dipping) along about 28km of the active Mt. Vettore-Mt. Bove fault system. Geometric and kinematic characteristics of the rupture were observed and recorded along closely spaced, parallel or subparallel, overlapping or step-like synthetic and antithetic fault splays of the activated fault systems, comprising a total surface rupture length of approximately 46km when all ruptures were considered.

Abstract

OBJECTIVES: This study aims to evaluate the reliability and clinical utility of NS3 sequencing in hepatitis C virus (HCV) 1-infected patients who were candidates to start a PI-containing regimen. METHODS: NS3 protease sequencing was performed by in-house-developed HCV-1 subtype-specific protocols. Phylogenetic analysis was used to test sequencing reliability and concordance with previous genotype/subtype assignment by commercial genotyping assays. RESULTS: Five hundred and sixty-seven HCV plasma samples with quantifiable HCV-RNA from 326 HCV-infected patients were collected between 2011 and 2014. Overall, the success rate of NS3 sequencing was 88.9%. The success rate between the two subtype protocols (HCV-1a/HCV-1b) was similarly high for samples with HCV-RNA >3 log IU/mL (>92% success rate), while it was slightly lower for HCV-1a samples with HCV-RNA ≤3 log IU/mL compared with HCV-1b samples. Phylogenetic analysis confirmed the genotype/subtype given by commercial genotyping assays in 92.9% (303/326) of cases analysed. In the remaining 23 cases (7.1%), 1 was HCV-1g (previously defined as subtype 1a), 1 was HCV-4d (previously defined as genotype 1b) and 1 was HCV-1b (previously defined as genotype 2a/2c). In the other cases, NS3 sequencing precisely resolved the either previous undetermined/discordant subtype 1 or double genotype/subtype assignment by commercial genotyping assays. Resistance-associated variants (RAVs) to PI were detected in 31.0% of samples. This prevalence changed according to PI experience (17.1% in PI-naive patients versus 79.2% in boceprevir/telaprevir/simeprevir-failing patients). Among 96 patients with available virological outcome following boceprevir/telaprevir treatment, a trend of association between baseline NS3 RAVs and virological failure was observed (particularly for HCV-1a-infected patients: 3/21 failing patients versus 0/22 achieving sustained virological response; P = 0.11). CONCLUSIONS: HCV-NS3 sequencing provides reliable results and at the same time gives two clinically relevant pieces of information: a correct subtype/genotype assignment and the detection of variants that may interfere with the efficacy of PI.

Abstract

Background & Aims: Despite the excellent efficacy of direct-acting antivirals (DAA) reported in clinical trials, virological failures can occur, often associated with the development of resistance-associated substitutions (RASs). This study aimed to characterize the presence of clinically relevant RASs to all classes in real-life DAA failures. Methods: Of the 200 virological failures that were analyzed in 197 DAA-treated patients, 89 with pegylated-interferon+ribavirin (PegIFN+RBV) and 111 without (HCV-1a/1b/1g/2/3/4=58/83/1/6/24/25; 56.8% treatment experienced; 65.5% cirrhotic) were observed. Sanger sequencing of NS3/NS5A/NS5B was performed by home-made protocols, at failure (N= 200) and whenever possible at baseline (N= 70). Results: The majority of the virological failures were relapsers (57.0%), 22.5% breakthroughs, 20.5% non-responders. RAS prevalence varied according to IFN/RBV use, DAA class, failure type and HCV genotype/subtype. It was 73.0% in IFN group vs 49.5% in IFN free, with the highest prevalence of NS5A-RASs (96.1%), compared to NS3-RASs (75.9% with IFN, 70.5% without) and NS5B-RASs (66.6% with IFN, 20.4% without, in sofosbuvir failures). In the IFN-free group, RASs were higher in breakthrough/non-responders than in relapsers (90.5% vs 40.0%, P= 2 DAA classes showed multiclass resistance, including 11/11 NS3+NS5A failures. Furthermore, 20.0% of patients had baseline-RASs, which were always confirmed at failure. Conclusions: In our failure setting, RAS prevalence was remarkably high in all genes, with a partial exception for NS5B, whose limited resistance is still higher than previously reported. This multiclass resistance advocates for HCV resistance testing at failure, in all three genes for the best second-line therapeutic tailoring.

Abstract

A collaboration between different facilities in Casaccia and Frascati ENEA research centers has recently opened the possibility of performing irradiation experiments using different kinds of particles such as protons, neutrons, and electrons. The facilities involved in the project are the TAPIRO fast nuclear research reactor, the TOP-IMPLART proton accelerator (71 MeV), the REX electron accelerator (5 MeV) and the FNG neutron facility (2.5/14 MeV). This suite of plans represents a distributed irradiation facility using different particles and spectra, offering various irradiation capabilities for experiments in many fields, such as: nuclear physics, accelerator physics, aerospace, science material, medicine, detectors, radiation diagnostics, etc. Complete numerical models of all the facilities have been implemented to perform start-to-end simulations before experimental irradiation using beam dynamics and nuclear transport simulation codes. In addition, our distributed irradiation facility makes it possible to recreate an essential part of the Van Allen Belt radiation allowing us to check the FLUKA quantitative calibration regarding this specific simulation, facilitating the disentanglements of the uncertainties. Finally, a comparison of the estimated silicon 1 MeV neutron equivalent fluence (SI1MEVNE) of the radiation damage imparted by the different facilities is presented.

Abstract

Current radiological characterization protocols applied in nuclear installations provide a valuable set of information used to assess level and distribution of radioactivity, which is a crucial information in the plant management. Nevertheless, typical routines may present limitations that novel technologies can alleviate. In this work, a new-generation 3D pixelated CdZnTe gamma imaging and spectrometer detector has been used at TRIGA RC-1 Research Reactor in ENEA Casaccia Research Centre to assess its performances in several possible tasks and effectively understand this technology readiness. In particular, three possible tasks have been considered: survey of potentially contaminated area (with determination of known and unknown hot spots), source identification and localization (including neutron sources via their gamma emissions), and quantification of isotope-specific activities in generic components thanks to the point-kernel-based algorithm the system is equipped with. The experience has demonstrated the capability to achieve reliable results in these tasks, with consequences that could impact not only the current characterization protocols but also the way some Nuclear Safeguards and Non-Proliferation operations are performed.

Abstract

The increasing worldwide interest in Lead-cooled Fast Reactors (LFRs) substantiates the need to validate the analytical codes and methods used to support their design. For neutronic analyses, this is chiefly reflected in assessing the impact of nuclear data uncertainties on the integral and local parameters resulting from such analyses. The aim of refining nuclear data moves continuous efforts for more accurate measurements, be them differential or integral, for which adequate facilities are required. The availability at the ENEA's Casaccia research center of a fast source reactor – RSV TAPIRO – provides a unique opportunity to perform new integral experiments in support of fast reactors, including LFRs, owing to the well-characterized neutron spectrum of the thermal column. A series of experiments has been envisaged, dealing with the use of Lead in a reactor. The experiments concern the propagation of neutrons through blocks of materials representing relevant elements of a reactor core, and ranging from pure Lead to mixtures reproducing portions of the reflector and shield in LFRs. The paper is focused on the feasibility study of some of these experiments in which Lead and mixture blocks are inserted in the so-called thermal column of the RSV TAPIRO reactor and irradiated by the neutron flux emerging from the Copper reflector surrounding the reactor core.

Abstract

Based on the heuristically generalized perturbation theory (HGPT) adapted to subcritical systems [1], a procedure for online operational monitoring of the subcriticality level of a hybrid fusion-fission system in which an “external” neutron source is generated in a magnetically confined tokamak-type plasma is described. This procedure [2], commonly referred to as power control-based subcriticality monitoring (PCSM), consists of compensating of slow and small movements of a specialized control rod in the nuclear fission zone, previously calibrated using a standard procedure, with equally slow and small alterations of the fusion neutron source. PCSM is verified by solving the multi-group transport equation for the direct flux, as well as for the importance function, associated with normalized fission power [3], and a technique is proposed for modifying the fusion neutron source strength based on plasma compression/expansion. Plasma and confining magnetic field adjustments, needed to implement the PCSM, are estimated with due account of the 0-D-plasma power balance.

Abstract

Fusion Fission Hybrid Systems (FFHS) could have a potential role in the management of fission reactor wastes, with (in principle) some advantage over the comparable Accelerator Driven systems, devoted to the same objective. The validation of the concept poses major challenges in the area of fusion “source” development. However, the physics of the multiplying and transmutation blanket has also to be carefully understood and validated. Simple experiments can be envisaged for that purpose. Some of these experiments have already been performed in a different research framework (i.e. validation of the ADS concepts) and could constitute a very first database. Moreover, a new series of similar experiments could be planned.

Abstract

A system for capturing thermal energy is planned for installation at the ENEA-Casaccia centre (Italy). Costs, primary energy requirements, pollutant emissions and seasonal electricity consumption were estimated. The configuration based on four boreholes of different depths and the sizing of the vertical loop (double-U) were performed according to the ASHRAE method. This pilot plant, the first under construction within an Italian research centre, is part of a long-term research project funded by the Italian Ministry of Economic Development. The results of this research could be applied throughout the Italian Tyrrhenian Apennine margin, which is geologically characterised by highly conductive lithotypes.

Abstract

Accurate and timely assessment of displacements and/or structural damages in nuclear reactor vessels’ components is a key action in routine inspections for planning maintenance and repairs but also in emergency situations for mitigating consequences of nuclear incidents. Nevertheless, all these components are maintained underwater and reside in high-radiation fields thus imposing harsh operative conditions to inspection devices which must cope with effects such as Cerenkov radiation background, Total Ionizing Radiation (TID), and occlusions in the detectors’ field of view. To date, ultrasonic techniques and video cameras are in use for inspection of components’ integrity and with measurements of volumetric and surface crack opening displacements, respectively. The present work reports the realization of a radiation tolerant laser scanner and the results of tests in a nuclear research reactor vessel for acquisition of 3D models of critical components. The device, qualified for underwater operation and for withstanding up to 1 MGy of TID, is based on a 515 nm laser diode and a fast-scanning electro-optic unit. To evaluate performances in a significant but controlled environment, the device has been deployed in the vessel of a research reactor operated by ENEA in the Casaccia Research Centre in Rome (Italy). A 3D model of the fuel rods assembly through a cooling water column of 7 m has been acquired. The system includes proprietary postprocessing software that automatically recognizes components of interest and provides dimensional analysis. Possible application fields of the system stretch to dimensional analysis also in spent nuclear fuel storage pools.

Abstract

The analysis of the triggering thresholds is aimed at answering the question of when an event can take place. Based on the historical information on rainfall—the main triggering factor of this type of landslides—the triggering thresholds of debris flows have been investigated with the aim to provide additional elements to the knowledge of the critical rainfall. Nevertheless, the most directly factor related to the triggering of debris flows is the soil moisture level that depends not only on the intensity of rainfall in a given moment, but also by the thermoprecipitation regime of the previous period. The state of soil saturation was analysed in this study in laboratory with the pressure plate extractors techniques and monitored in field by the FDR probes. The responses of the FDR probes seem to confirm the data modelling, showing in some cases a marked sensitivity to the rainfall input. Coupling rainfall and soil water content data, the study would give a contribution to the development of more effective early warning systems. . © Springer International Publishing Switzerland 2015.

Abstract

Through the recording of historical and recent events data, three different landslides inventory of the Messina town territory have been created. The first collects information about localization and date of historical landslides occurred between the XVI century and 2011. The second collects ground effects induced by the 1908 earthquake registered in numerous technical reports compiled by expert researchers and technicians of that time. The third gathers recent landslides and was carried out through field activities, by reviewing aerial photographs for the years 2007 and 2009 and by analysis of Google satellite images. These three landslides inventories represent a source of specific information, essential for the knowledge and the analysis of the territory. They represent a valid tool not only for experts in the sector but also for everyone who faces the issues linked to the geomorphologic risk assessment. Nevertheless the information on the landslide characteristics constituted the basis for the definition of the landslide susceptibility map and landslides’ trigger rainfall thresholds of the area. . © Springer International Publishing Switzerland 2015.

Famous because of its deepness and beauty of the landscape, Colca Valley is one of the main places of tourist attraction in Peru. Maca and Lari are two of the valley villages, facing each other on opposite banks of the Colca River. High slope energy is only one of the factors causing a widespread landslide activity, triggered primarily by seismic activity. A study carried out into a capacity building project (FORGEO) was focused on mapping landslide source areas in the territory of both the villages. Starting from an inventory of several previous phenomena occurred in the area, an evaluation of the local predisposing parameters to these events was used as input for susceptibility and hazard GIS based analysis. An assessment of exposure of structures and infrastructures was also performed, detecting residual risk condition in the territories of both villages. In this paper a landslide risk assessment in the area of Aguas Calientes village (Machu Picchu, Cusco, Perù) is applied. A study carried out into a capacity building project (FORGEO) was focused on localising source areas and runout of debris flows triggered by heavy rainfall in the Alcamayo catchment and on assessing debris flow intensity. Starting from an inventory of several previous debris flows occurred in the area, an evaluation of the local predisposing parameters to these events was used as input for susceptibility and hazard GIS based analysis. Empirical formulas reported in the literature were adapted to the research area context to estimate the runout of potential events. An analysis of exposure of structures and infrastructures was also performed during the project and high residual risk conditions were found for the Aguas Calientes village. In the last 4 years Messina Municipality has been hit by rainfall events that triggered a great number of debris flows. Due to 2009 event ENEA started landslide hazard researches in the south of the Municipality. Messina territory (210 km2) is located at the NE culmination of Peloritani Range, formed by seven metamorphic Units covered by late orogenic terrigenous formations and by Plio-Pleistocene deposits. Steep slopes lead to high erosion rates, that increased in last years because of intense rainfall. The first step of the hazard assessment method is an inventory of landslides, followed by the detection of landslide parameters. Through GIS analysis of this parameters landslide susceptibility is assessed. Geomorphological analysis allows evaluating the runout and energy of the future events. Rainfalls data analysis allows definition of triggering threshold for the area. Mud and debris flows constitute a serious threat for several Italian regions as attested by recent several disasters attributable to such phenomena. The adoption of effective mitigation measures is complicated by the difficulty in predicting spatial and intensity features of future events. This paper applies a practical application of debris/mud flows hazard assessment in the area of Messina province. The study was based on an inventory of several debris flows triggered by heavy rainfall occurred in the Giampilieri and Briga catchments after the event of October 1, 2009. Data elaboration was focused on mapping source areas and on assessing the runout of landslides and their intensity. Abstract

Climate change poses significant emerging risks to biodiversity, ecosystem function and associated socioecological systems. Adaptation responses must be initiated in parallel with mitigation efforts, but resources are limited. As climate risks are not distributed equally across taxa, ecosystems and processes, strategic prioritization of research that addresses stakeholder-relevant knowledge gaps will accelerate effective uptake into adaptation policy and management action. After a decade of climate change adaptation research within the Australian National Climate Change Adaptation Research Facility, we synthesize the National Adaptation Research Plans for marine, terrestrial and freshwater ecosystems. We identify the key, globally relevant priorities for ongoing research relevant to informing adaptation policy and environmental management aimed at maximizing the resilience of natural ecosystems to climate change. Informed by both global literature and an extensive stakeholder consultation across all ecosystems, sectors and regions in Australia, involving thousands of participants, we suggest 18 priority research topics based on their significance, urgency, technical and economic feasibility, existing knowledge gaps and potential for cobenefits across multiple sectors. These research priorities provide a unified guide for policymakers, funding organizations and researchers to strategically direct resources, maximize stakeholder uptake of resulting knowledge and minimize the impacts of climate change on natural ecosystems. Abstract

The aim of this paper is to describe the usefulness of multiparametric US in the diagnosis and subsequent follow-up of a case of right-side ischemic colitis and to present a review of the data reported in the literature. Ischemic colitis is frequently diagnosed in the field of gastroenterology. Diagnosis is usually based on the outcome of endoscopy and histological examination, and in case of right-side ischemic colitis also on the results of contrast-enhanced CT. In the described case, multiparametric US indicated the diagnosis including a prognostic judgment, and during follow-up US-monitored patency of the stents positioned under angiographic guidance. One of the available US techniques, CEUS, seems to have a special role in the detection of residual vascularization of the gastrointestinal tract affected by ischemia. However, before introducing this method into daily clinical practice, further studies are required to confirm its diagnostic accuracy.

Abstract

Strain elastography is a recently developed ultrasound method that seems to contribute to the diagnosis and monitoring of inflammatory bowel diseases. This pictorial essay aims to present the feasibility of strain elastography in clinical practice by comparing visual observation and semiquantitative parameters with endoscopic or radiological images in some common cases of Crohn's disease. In our opinion, strain elastography with visual observation is easily performed in connection with B-mode ultrasound and color Doppler imaging and may provide relevant information. Strain elastography with the elaboration of semiquantitative parameters is not as immediate as the visual observation method, but it seems to provide more objective data and seems more appropriate for monitoring the evolution of the disease. However, for the elastographic method to extensively contribute in inflammatory bowel diseases, standardization and further studies are required to confirm the promising results already reported in the literature.

To augment mammal conservation in the Eastern Himalayan region, we assessed the resident 255 terrestrial mammal species and identified the 50 most threatened species based on conservation status, endemism, range size, and evolutionary distinctiveness. By using the spatial analysis package letsR and the complementarity core-area method in the conservation planning software Zonation, we assessed the current efficacy of their protection and identified priority conservation areas by comparing protected areas (PAs), land cover, and global ecoregion 2017 maps at a 100 × 100 m spatial scale. The 50 species that were most threatened, geographically restricted, and evolutionarily distinct faced a greater extinction risk than globally nonthreatened and wide-ranging species and species with several close relatives. Small, medium-sized, and data-deficient species faced extinction from inadequate protection in PAs relative to wide-ranging charismatic species. There was a mismatch between current PA distribution and priority areas for conservation of the 50 most endangered species. To protect these species, the skewed regional PA distribution would require expansion. Where possible, new PAs and transboundary reserves in the 35 priority areas we identified should be established. There are adequate remaining natural areas in which to expand current Eastern Himalayan PAs. Distributions of Earth's species are changing at accelerating rates, increasingly driven by human-mediated climate change. Such changes are already altering the composition of ecological communities, but beyond conservation of natural systems, how and why does this matter? We review evidence that climate-driven species redistribution at regional to global scales affects ecosystem functioning, human well-being, and the dynamics of climate change itself. Production of natural resources required for food security, patterns of disease transmission, and processes of carbon sequestration are all altered by changes in species distribution. Ultrasound (US) is the first-line investigation in patients with abdominal symptoms and it has a role in the diagnosis and monitoring of inflammatory bowel diseases. Strain elastography (SE) is a US method for estimating tissue elasticity. This method is still not routinely used in clinical practice, although it is suggested by the EFSUMB Guidelines for the characterization of intestinal stenosis in Crohn's disease. The purpose of this Pictorial Essay is to confirm the feasibility of elastography in Crohn's disease in clinical practice by comparing elastography images with corresponding endoscopic or radiological images. At present, diagnostic accuracy of SE is reported in the literature only in connection with detection of bowel wall fibrosis in advanced cases of Crohn's disease. However, in our opinion, SE can add useful diagnostic information to grayscale and color Doppler US by providing a more accurate characterization of the bowel wall in the various manifestations of Crohn's disease.

Abstract

In recent years, novel antiretroviral drugs have become available for multi-experienced HIV-infected patients with limited options. We enrolled seven advanced HIV-patients, failing multiple previous HAART regimens, in virological failure on their current HAART regimen and showing recent clinical and immunological progression. All patients were prescribed a double-boosted tipranavir plus enfuvirtide based regimen, in addition to zidovudine, tenofovir and lamivudine for salvage therapy. To assess susceptibility to tipranavir, the tipranavir genotypic resistance score was calculated and two years later this was re-evaluated on an updated tipranavir genotypic score algorithm. At baseline, CD4 were 139/mcL (more or less 145), HIV-1 RNA was 822,700 cp/mL. All patients achieved HIV-1 RNA levels less than 400 cp/mL between 12 weeks and 24 weeks of observation; two reached less than 50 cp/mL during this period. At 48 weeks three patients had reached less than 50 cp/mL; three other patients had HIV RNA less than 200 cp/mL. At 72 and 96 weeks HIV viraemia was less than 50 cp/mL in six patients; CD4 T-cell counts 285/mcL (more o less 198). No AIDS-defining events were recorded. Adverse events did not need to stop or change HAART. Strong 3 NRTI backbone could help efficacy and durability, and frequent evaluations in complex patients can help to manage toxicity.

Abstract

Ruptured ectopic pregnancy is a leading cause of maternal mortality in the United States. The purpose of this study was to identify predictors of tubal rupture. A retrospective chart review of 236 ectopic pregnancies in 215 patients treated at the Cleveland Clinic Foundation from 1983 through 1996 was performed. Patients were stratified by rupture status of the ectopic pregnancy and compared for the following parameters: age, gravida, parity, aborta, gestational age, preoperative and postoperative hemoglobin level, need for blood transfusion, serum human chorionic gonadotropin (hCG) levels, and findings on ultrasonogram and at surgery. We also compared the two groups in multiple risk factors for ectopic pregnancy. We used the generalized estimating equation (GEE) methodology and unbalanced repeated measures analysis of variance to compare the two groups. Tubal rupture was found in 26.3% of ectopic pregnancies. Tubal rupture occurred with any serum hCG level, even with those under 100 mIU/ml. There was no significant difference between the two groups in gestational age, serum hCG levels, or ultrasound findings except that the frequency of ruptured pregnancies increases as the fluid amount documented on ultrasonography increases (p < 0.001). There was no association between any of the risk factors or the number of risk factors and frequency of tubal rupture. There was no significant decrease in the rupture rate over time (p = 0.34). There was an increased morbidity associated with tubal rupture. Tubal rupture cannot be predicted on the basis of any known risk factor, ultrasonogram findings, or serum hCG levels. Early diagnosis and treatment of ectopic pregnancy are the only modality available to prevent tubal rupture and its associated morbidity.

Abstract

Sixteen human cases of Fasciola hepatica infection are described. The liver was involved in 13 cases, the gall bladder in 9 cases and the stomach in 2 cases. Lesions containing parasitic remnants or fluke eggs were rarely seen. Surface scarring of the liver, scar tracks and granulomas within organs were the most characteristic changes seen and were the most useful for the histopathological diagnosis of the disease. The associated liver, bile and gastric lesions are briefly discussed.
