Introduction: The aim of this study is to evaluate the demographic, clinical and laboratory characteristics and treatment results of pediatric patients followed up with the diagnosis of brucellosis in our clinic.Methods: Patients who were followed up with the diagnosis of brucellosis in Kayseri Training and Research Hospital, Pediatric Infectious Diseases Clinic between October 2016 and December 2018 were retrospectively analyzed.Results: A total of 37 patients, 26 (70.3%) male, were included in the study. The mean age of the patients whose ages ranged from 1 to 17 years was 9.3±4.3 years. Unpasteurized milk and/or dairy products were consumed in 86.5% of the patients and 48.6% had a family history of brucellosis. Joint pain (75.7%) was the most common presenting complaint. In the laboratory evaluation, 8.1% of the patients had leukopenia, 2.7% had thrombocytopenia and 21.4% had ALT elevation. Doxycycline plus rifampicin (43.2%) was the most commonly preferred treatment regimen. Addition of aminoglycoside to initial therapy in hospitalized patients was statistically higher than in outpatients (81.2% vs. 23.8%) (p=0.001). During the follow-up, relapse developed in a total of 4 (10.8%) patients, 3 of whom were outpatients. There was no statistical relationship between the initial treatment regimen and relapse (p=0.418).Conclusion: In conclusion, brucellosis should be kept in mind in terms of differential diagnosis in patients who present with joint pain in our country and who have cytopenia and/or isolated aminotransferase elevation in their investigations.Keywords: Child, Brucellosis, Relapse

Orthodontic correction of single tooth malpositions due to eruption problems, early loss of tooth and other reasons, is an important issue especially in patients with ideal occlusion without bonding the whole arch. In this case report treatment of a 9-year-old female patient with an Angle class I malocclusion having her right lower permanent first molar tilted mesially towards the space occurred by the extraction of right second deciduous molar tooth was presented. Correction of right upper lateral tooth which was located palatally is also included. A mini-plate which is used currently in recent years was used for direct anchorage to make unilateral lower molar distalization and uprighting in this case. After 7 months, enough distalization and uprighting was maintained. Afterwards the eruption of right lower second premolar was followed and the treatment was finished.

Amaç: Sistemik lupus eritematozus (SLE), otoimmün bir bağ dokusu hastalığıdır ve sıklıkla 20-40 yaşlarında, doğurgan çağdaki kadınları etkilemektedir. Bu çalışmanın amacı gebelik öyküsü olan SLE hastalarında gebeliğin seyri ve sonuçlarının değerlendirilmesidir. Yöntem: 2010-2020 yılları arasında romatoloji polikliniğine başvurmuş, SLE tanısı ve gebelik öyküsü olan hastaların dosyaları retrospektif olarak tarandı. Gebelikten önce tedavisi düzenlenmiş, gerekli tetkikler yapılmış, 6 ay remisyon sağlanmış hastalar planlı gebe olarak kabul edildi. Bulgular: Gebelik öyküsü olan 31 SLE hastasının toplam 55 gebeliği çalışmaya dahil edildi. Hastaların ortalama yaşı 29,1±4,34 (26,5-32,0) idi. Hastaların %43,3'ünde (n=13) preterm doğum, %40'ında (n=12) IUGR ve %9,09'unda (n=5) preeklampsi öyküsü vardı. Spontan abortus 15 hastada (%27,3), fetal kayıp öyküsü 10 hastada (%18,2) görüldü. Neonatal kayıp ve anne ölümü görülmedi. Lupuslu gebelerin %20'sinde (n=11) anti SS-A, %3,64'ünde (n=2) anti SS-B pozitifti. Neonatal kalp bloğu hiçbir gebede görülmedi. Hastaların %34,5'inde (n=19) böbrek tutulumu vardı ve gebelikte lupus alevlenmesi sadece böbrek tutulumu olanlarda görüldü [%26 vs. %0 (p=0,006)]. On sekiz hastada (%32,7) antifosfolipid antikor sendromu (AFAS) vardı ve bu hastaların %50'sinde gebelik kaybı mevcuttu. Fetal kayıp sadece AFAS'ı olan gebelerde görülmüştü [%55,6 (n=10) vs. %0 (n=0) (p<0,001)]. Çalışmaya dahil edilen 55 gebeliğin 6'sı (%10,9) plansızdı ve bunların 3'ünde alevlenme saptandı, 5'i abortus ile sonuçlandı. Sonuç: Mevcut literatür SLE hastalarında obstetrik komplikasyonların, alevlenmelerin ve fetal morbiditenin artmış olduğu yönünde veriler içermektedir. Çalışmamızda gebelik öncesi remisyonda olan hastalarda bu risklerin anlamlı derecede az olduğunu gösterdik. Alevlenme sadece böbrek tutulumu olan hastalarda görülürken fetal kayıp sadece AFAS'lı gebelerde görüldü. SLE hastaları tanı aldığı dönemden itibaren bilgilendirildiğinde, gebelik sırasında romatoloji ve kadın doğum uzmanlarının iş birliği ile olumlu sonuçlar elde edilebilir. SLE hastaları günümüzde yakın ve uygun obstetrik, romatolojik ve neonatal izlem ile başarılı bir şekilde gebelik geçirip sağlıklı çocuklar doğurabilmektedir. [ABSTRACT FROM AUTHOR]

Objective/aim: Systemic Lupus Erythematosus (SLE), is common in women of childbearing age and is associated with obstetric complications. The aim of this study was to evaluate the course of pregnancy and its results in SLE patients with a history of pregnancy., Methods: Pregnant patients with SLE who applied to the Rheumatology outpatient clinic of between 2010 and 2020 were retrospectively screened., Results: Fifty-five pregnancies of 31 SLE patients were included in the study. Spontaneous abortion was observed in 27.3% (n:15) and foetal loss in 18.2% (n:10). Neonatal loss or maternal death was not observed in any of the patients. The rate of patients with renal involvement was 34.5% (n:19), and the rate of exacerbation was higher in pregnant women with kidney involvement (26% vs 0% (p:0.006)). Antiphospholipid antibody syndrome (APS) was present in 32.7% (n:18) of the cases, and there was a history of foetal loss before diagnosis in 50% (n:9) of the cases with APS. The foetal loss was seen only in pregnant women with APS (55.6% (n:10) vs. 0% (n:0) (p<0.001)). Six (10.9%) of 55 pregnancies in our study were unplanned pregnancies. Of these, five resulted in abortion. Exacerbation was observed in 3 of the unplanned pregnancies., Conclusion: Pregnancy complications were significantly lower in patients who were in remission since contraception. Exacerbations were more common in unplanned pregnancies and in patients with renal involvement.
