Due to its effect on insulin resistance, ease of administration and favorable safety profile, metformin has been included in the recommendations of foreign medical communities for the management of pregnant women with hyperglycemia since 2008. However, in Russia, the use of any oral hypoglycemic agents during pregnancy is still contraindicated. However, recent studies demonstrate the safety and positive effects of metformin on pregnancy in patients with pregestational diabetes mellitus, polycystic ovary syndrome and gestational diabetes mellitus. In 2023, the Federal Service for Surveillance in Healthcare of the Ministry of Health of Russia updated the instructions for the medical use of Glucophage® and Glucophage®Long: pregnancy was excluded from the “Contraindications” section and moved to the “With caution” section. This resolution is intended to evaluate studies of the effectiveness and safety of metformin, as well as to study the experience of foreign colleagues and Russian legal aspects of prescribing metformin in the stages of preparation for pregnancy, during it and in the post-gravid period.

BACKGROUND: It is important to evaluate the effect of androgen replacement therapy on glycemic control.AIMS: Assessment of the effect of transdermal testosterone therapy on glycemic control in men with hypogonadism and T2DM.MATERIALS AND METHODS: The prospective, comparative study included 300 men aged 55[49;61] years: men receiving both glucose-lowering therapy (GLT) and transdermal testosterone; and patients receiving only GLT. Observation period 1 year. A medical history, a questionnaire for symptoms of androgen deficiency, and measurement of total testosterone and glycated hemoglobin were carried out. Groups were compared using nonparametric methods.RESULTS: Patients receiving androgen replacement therapy in combination with GLT at both 6 and 12 months from the moment of inclusion in the study, they had a statistically significantly higher level of total testosterone and less severity of symptoms of androgen deficiency. When assessing the magnitude of changes in the studied parameters, it was found that patients receiving testosterone replacement therapy (TRT) were characterized by a statistically significantly more pronounced decrease in the level of glycated hemoglobin (average difference 0,3%). In 29 (20,4% (95% ДИ 13,8–27,0)) men who received only GLT, hypogonadism was eliminated. In 3 patients from the TRT group, a pathological increase in the level of total blood PSA was observed, and therefore TRT was discontinued. An increase in hemoglobin above the reference value (>172 g/l) was detected in 8% and 1,3% of men on TRT and without correction of hypogonadism, respectively, p=0,011.CONCLUSIONS: The combination of transdermal TRT and GLT has a positive effect on glycemic control, which is manifested by a decrease in glycated hemoglobin to a greater extent than when using GLT alone. Normalization of testosterone levels leads to a decrease in the symptoms of androgen deficiency, which is accompanied by an improvement in quality of life. Elimination of hypogonadism only with GLT is possible in a small number of cases.

Hyperprolactinemia is a persistent excess of the blood serum prolactin. The syndrome contains various symptoms, the most characteristic is a violation of the reproductive system. There are multiple endogenous and exogenous causes of hyperprolactinemia. The main treatment method is dopamine agonist therapy, in case of prolactinoma existence, surgical and radiation methods can be applied. About 15% of patients are resistant to dopamine agonist therapy, which determines creation of individual management tactics. The article presents a draft of clinical guidelines for the diagnosis and treatment of hyperprolactinemia, which provides a modern examination algorithm, discusses the basic principles of diagnostics and treatment approaches.

Congenital adrenal hyperplasia (CAH) is a group of autosomal recessive diseases characterized by a defect in one of the enzymes or transport proteins involved in the cortisol synthesis in the adrenal cortex. The most common form of CAH, which occurs in more than 90% of cases, is a 21-hydroxylase enzyme deficiency. The latter is subdivided into nonclassical and classic (salt-losing and virilizing) forms. The prevalence of classic forms of 21-hydroxylase deficiency ranges from 1: 14,000 to 1:18,000 live births worldwide. According to the data of neonatal screening in the Russian Federation, the prevalence of the disease in some regions ranges from 1: 5000 to 1: 12000, in the country as a whole - 1: 9638 live newborns. The non-classical form of CAH occurs more often - from 1: 500 to 1: 1000 among the general population. In second place is the hypertensive form of CAH - a deficiency of 11β-hydroxylase, which, according to the literature, occurs in about 1 per 100,000 newborns. These clinical guidelines were compiled by a professional community of narrow specialists, approved by the expert council of the Ministry of Health of the Russian Federation, and updated the previous version published in 2016. The clinical guidelines are based on systematic reviews, meta-analyses and original articles, and scientific work on this issue in the Russian Federation and other countries. The purpose of this document is to provide clinicians with the most up-to-date, evidence-based guidelines for the CAH diagnosis and treatment

Clinical guidelines have long been one of the working tools of the modern doctor, helping him quickly navigate the most effective proven methods of treatment and prevention of various diseases, and also to adapt these methods to the specific tasks of their patients and to achieve maximum personalization of treatment. Clinical practice guidelines are drawn up by professional non-profit associations and are approved by the Scientific Council of the Ministry of Health of the Russian Federation, while often one recommendation is prepared by two or even three associations. The peculiarity of the recommendations offered to your attention is that not only endocrinologists, but also therapists, cardiologists, gynecologists, gastroenterologists, and experts of many other specialties are involved in the prevention and treatment of obesity. The Multidisciplinary Working Group presents this a project in a multidisciplinary journal to bring together the efforts of several professional associations that associated with the need to pay attention not only to obesity itself but also to comorbid conditions. We are looking forward to constructive criticism and a comprehensive discussion of the problem on the pages of our journal.

Overexpression of the Gadd45 gene in the nervous system of Drosophila melanogaster increases the life span. We have performed the Gompertz approximation of the survival curve and revealed a life span increase as a result of decrease of the R parameter, reflecting the death level at the beginning of adult life, whereas the other Gompertz parameter, α, responsible for final stage of the life, does not change essentially. In addition, we have analyzed the case of radiation hormesis (low doses of radiation increase life span) and found a similar effect. We conclude that it is promising to perform geroprotector screen by testing substances for the ability to decrease R. Its measurement is less labor-consuming than the construction of the complete survival curve.

Цель. Сравнительный анализ эффективности гестагенов с пероральным и парентеральным (интравагинальным) путем введения (дидрогестерон с пероральным путем введения, микронизированный натуральный прогестерон с парентеральным путем введения) в коррекции нарушений МЦ у девушек с СД 1 и гипертриглицеридемией. Материалы и методы. В исследование были включены: 70 девушек с СД 1 в состоянии компенсации и субкомпенсации основного заболевания. Пациентки были распределены на 3 группы. 1-ю группу составили 34 девушки с СД 1 в возрасте от 14 до 16 лет (средний возраст ? 15,9?0,7 лет). Средний уровень HbA1c составил 7,1?1,8%. Уровень триглицеридов в этой группе больных был в пределах нормальных значений. Препарат назначали перорально с 16-го по 25-й день МЦ по 1 таблетке 2 раза в сутки после еды при сохраненном ритме менструаций либо по 1 таблетке 2 раза в сутки в течение 10 дней начиная с 45-го дня цикла?. 2-ю группу составили 36 девушек в возрасте 14,1?0,5 лет с СД 1 в сочетании с гипертриглицеридемией. Средний уровень HbA1c составил 7,0?0,9%. У 16 (44,5%) из них выявлялась олигоменорея, у 4 (11%) ? вторичная аменорея, у 16 (44,5%) ? сохраненный ритм менструаций с выраженной недостаточностью лютеиновой фазы цикла. У больных с нерегулярными менструациями препарат назначали по 1 капсуле 2 раза в сутки интравагинально в течение 12 дней, начиная с 45-го дня цикла?. Резульататы. У 40 девочек физическое развитие соответствовало возрастной норме. Задержка физического развития выявлена у 20 больных, опережение показателей физического развития за счет избыточной массы тела ? у 10 девочек. Средний ИМТ составил 21,3?1,2 кг/м2. При оценке динамики менструальной функции на фоне подбора адекватной инсулинотерапии для достижения компенсации СД 1 к концу 3-го месяца у 10 (42%) девушек с олигоменореей отмечено восстановление регулярного МЦ. К концу 6-го месяца терапии у 21 (87,5%) девушки произошло восстановление самостоятельного ритма менструаций. Заключение. У девушек в возрасте до 16 лет, находящихся в фазе декомпенсации СД 1 и имеющих выраженную гипоэстрогенемию при выборе метода коррекции нарушений МЦ, в первую очередь необходимо достичь компенсации основного заболевания, коррегируя дозу инсулина, что позволяет нормализовать нарушения МЦ без использования гормональной терапии у 91,6% пациенток. Сравнительная оценка результатов гормонального лечения в коррекции нарушений МЦ у девушек с СД 1 свидетельствует о высокой эффективности применения гестагенов с пероральным и парентеральным путем введения.

Цель. Оценить влияние гормональной рилизинг-системы с интравагинальным путём введения нова-Ринг? на показатели углеводного и липидного обмена, а также систему гемостаза у женщин с СД 1 типа в позднем репродуктивном периоде в течение 6 месяцев. Материалы и методы. В исследование включены 30 женщин с СД 1 типа. Контрольную группу составили 30 женщин с СД 1 типа аналогичного возраста, не использующие какие-либо методы контрацепции, а также 30 здоровых женщин, использующих нова-Ринг?. Общее клиническое обследование включало сбор анамнестических данных, общий и гинекологический осмотр. Гинекологический осмотр включал осмотр и пальпацию молочных желез; бимануальное гинекологическое исследование; осмотр шейки матки в зеркалах; мазок по Папаниколау, мазок из влагалища на флору, а также степень чистоты и др. Для оценки степени избыточной массы тела или ожирения вычисляли индекс массы тела (ИМТ). Результаты. Оценка динамики ОТ (см) выявила, что до проведения контрацепции в группе женщин с нормальной массой тела данный показатель составил исходно 82,1?2,4 см; через 3 мес? 83,4?3,2 см; через 6 мес ? 80,1?2,8 см. В группе женщин с ожирением данный показатель составил исходно 102,6+3,5 см; через 3 мес ? 104,1+2,9 см; через 6 мес ? 103,4+3,2 см. Изменения показателей липидного спектра крови в контрольных группах достоверно не изменялись в процессе наблюдения. Характеристика показателей гемостаза у женщин с СД 1 типа позднего репродуктивного периода на фоне использования контрацептивной системы нова-Ринг? также не выявила достоверно значимых изменений. Выводы. Рилизинг-система нова-Ринг? является надежным и безопасным средством контрацепции для женщин с СД 1 типа позднего репродуктивного периода. Использование контрацептивной системы нова-Ринг? у женщин с СД 1 типа в позднем репродуктивном периоде не оказывает клинически значимого влияния на показатели углеводного и липидного обмена на фоне стойкой и удовлетворительной компенсации углеводного обмена и обладает нейтральным эффектом на систему гемостаза. Гормональная рилизинг-система нова-Ринг? может использоваться у женщин с ИМТ > 30 кг/м.
