We present a measurement of the Cabibbo-Kobayashi-Maskawa unitarity triangle angle ϕ 3 (also known as γ) using a model-independent Dalitz plot analysis of B + → D ( K S 0 $$ {K}_S^0 $$ h + h − )h +, where D is either a D 0 or D ¯ $$ \overline{D} $$ 0 meson and h is either a π or K. This is the first measurement that simultaneously uses Belle and Belle II data, combining samples corresponding to integrated luminosities of 711 fb −1 and 128 fb −1, respectively. All data were accumulated from energy-asymmetric e + e − collisions at a centre-of-mass energy corresponding to the mass of the Υ(4S) resonance. We measure ϕ 3 = (78.4 ± 11.4 ± 0.5 ± 1.0)°, where the first uncertainty is statistical, the second is the experimental systematic uncertainty and the third is from the uncertainties on external measurements of the D-decay strong-phase parameters.

International audience; Epilepsy is a complex disease that requires various approaches for its study. In this short review, we discuss the contribution of theoretical and computational models. The review presents theoretical frameworks that underlie the understanding of certain seizure properties and their classification based on their dynamical properties at the onset and offset of seizures. Dynamical system tools are valuable resources in the study of seizures. By analyzing the complex, dynamic behavior of seizures, these tools can provide insights into seizure mechanisms and offer a framework for their classification. Additionally, computational models have high potential for clinical applications, as they can be used to develop more accurate diagnostic and personalized medicine tools. We discuss various modeling approaches that span different scales and levels, while also questioning the neurocentric view, and emphasize the importance of considering glial cells. Finally, we explore the epistemic value provided by this type of approach.

We report the discovery of a facile peptide macrocyclization and stapling strategy based on a fluorine thiol displacement reaction (FTDR), which renders a class of peptide analogues with enhanced stability, affinity, cellular uptake, and inhibition of cancer cells. This approach enabled selective modification of the orthogonal fluoroacetamide side chains in unprotected peptides in the presence of intrinsic cysteines. The identified benzenedimethanethiol linker greatly promoted the alpha helicity of a variety of peptide substrates, as corroborated by molecular dynamics simulations. The cellular uptake of benzenedimethanethiol stapled peptides appeared to be universally enhanced compared to the classic ring-closing metathesis (RCM) stapled peptides. Pilot mechanism studies suggested that the uptake of FTDR-stapled peptides may involve multiple endocytosis pathways in a distinct pattern in comparison to peptides stapled by RCM. Consistent with the improved cell permeability, the FTDR-stapled lead Axin and p53 peptide analogues demonstrated enhanced inhibition of cancer cells over the RCM-stapled analogues and the unstapled peptides., Strategies capable of stapling unprotected peptides in a straightforward, chemoselective, and clean manner, as well as promoting cellular uptake are of great interest. Here the authors report a peptide macrocyclization and stapling strategy which satisfies those criteria, based on a fluorine thiol displacement reaction.

International audience; Precise estimates of epileptogenic zone networks (EZNs) are crucial for planning intervention strategies to treat drug-resistant focal epilepsy. Here, we present the virtual epileptic patient (VEP), a workflow that uses personalized brain models and machine learning methods to estimate EZNs and to aid surgical strategies. The structural scaffold of the patient-specific whole-brain network model is constructed from anatomical T1 and diffusion-weighted magnetic resonance imaging. Each network node is equipped with a mathematical dynamical model to simulate seizure activity. Bayesian inference methods sample and optimize key parameters of the personalized model using functional stereoelectroencephalography recordings of patients’ seizures. These key parameters together with their personalized model determine a given patient’s EZN. Personalized models were further used to predict the outcome of surgical intervention using virtual surgeries. We evaluated the VEP workflow retrospectively using 53 patients with drug-resistant focal epilepsy. VEPs reproduced the clinically defined EZNs with a precision of 0.6, where the physical distance between epileptogenic regions identified by VEP and the clinically defined EZNs was small. Compared with the resected brain regions of 25 patients who underwent surgery, VEP showed lower false discovery rates in seizure-free patients (mean, 0.028) than in non–seizure-free patients (mean, 0.407). VEP is now being evaluated in an ongoing clinical trial (EPINOV) with an expected 356 prospective patients with epilepsy.

The influence of plasma beta effects on the edge plasma transport in the Wendelstein 7-X standard configuration is studied systematically by using EMC3-EIRENE combined with a 3D equilibrium code named HINT. The magnetic topology changes induced by plasma beta effects are significantly reflected in plasma transport behaviors and heat flux patterns on divertor targets. After validating the modeling strategy by comparisons with experimental data, the extended simulations for high performance plasmas show that the threshold separatrix density for accessing the power detachment is reduced in higher beta plasmas. Compared with the vacuum field case, the impurity radiation distributions with finite beta effects are modified in the magnetic island region. The divertor heat flux is distributed more evenly along the toroidal direction on the strike line at the vertical target. The strike line on the horizontal target moves towards the pumping gap with an increase in the plasma beta. In addition, the different pressure profiles with the same central beta also result in a modified heat flux pattern on the divertor targets.

An absolute measurement of the Λ_{c}^{+} lifetime is reported using Λ_{c}^{+}→pK^{-}π^{+} decays in events reconstructed from data collected by the Belle II experiment at the SuperKEKB asymmetric-energy electron-positron collider. The total integrated luminosity of the data sample, which was collected at center-of-mass energies at or near the ϒ(4S) resonance, is 207.2 fb^{-1}. The result, τ(Λ_{c}^{+})=203.20±0.89±0.77 fs, where the first uncertainty is statistical and the second systematic, is the most precise measurement to date and is consistent with previous determinations.

Mobile crowdsensing (MCS), as a novel large-scale data acquisition method, has attracted more and more attention. Since the participants’ quality directly affects the quality of perceptual task completion in MCS, participant selection has become a focus of researchers. However, due to the sparsity of participants’ information and data privacy, existing solutions have certain limitations in terms of security and accuracy in participant selection. To tackle these problems, this paper proposes a secure and accurate participant selection (SAPS) method. It employs blockchain-based cross-domain reputation sharing while labeling participants with personalized reputation tags as quality references to achieve security and accuracy in participant selection. In particular, SAPS utilizes a model of differential privacy to protect privacy during cross-domain sharing while guaranteeing the credibility of the data sources by leveraging the traceability and non-tamper nature of the blockchain. Comprehensive experiments on real datasets indicate that compared with CMABA, the tasks’ completion quality in SAPS is improved by 18%, and the execution cost of SAPS is reduced by 6%.

Background The COVID‐19 pandemic delayed diagnosis and care for some acute conditions and reduced monitoring for some chronic conditions. It is unclear whether new diagnoses of chronic conditions such as dementia were also affected. We compared the pattern of incident Alzheimer's disease and related dementia (ADRD) diagnosis codes from 2017 to 2019 through 2020, the first pandemic year. Methods Retrospective cohort design, leveraging 2015–2020 data on all members 65 years and older with no prior ADRD diagnosis, enrolled in a large integrated healthcare system for at least 2 years. Incident ADRD was defined as the first ICD‐10 code at any encounter, including outpatient (face‐to‐face, video, or phone), hospital (emergency department, observation, or inpatient), or continuing care (home, skilled nursing facility, and long‐term care). We also examined incident ADRD codes and use of telehealth by age, sex, race/ethnicity, and spoken language. Results Compared to overall annual incidence rates for ADRD codes in 2017–2019, 2020 incidence was slightly lower (1.30% vs. 1.40%), partially compensating later in the year for reduced rates during the early months of the pandemic. No racial or ethnic group differences were identified. Telehealth ADRD codes increased fourfold, making up for a 39% drop from face‐to‐face outpatient encounters. Older age (85+) was associated with higher odds of receiving telecare versus face‐to‐face care in 2020 (OR:1.50, 95%CI: 1.25–1.80) and a slightly lower incidence of new codes; no racial/ethnic, sex, or language differences were identified in the mode of care. Conclusions Rates of incident ADRD codes dropped early in the first pandemic year but rose again to near pre‐pandemic rates for the year as a whole, as clinicians rapidly pivoted to telehealth. With refinement of protocols for remote dementia detection and diagnosis, health systems could improve access to equitable detection and diagnosis of ADRD going forward.

The excitatory neurotransmitter glutamate shapes learning and memory, but the underlying epigenetic mechanism of glutamate regulation in neuron remains poorly understood. Here, we showed that lysine demethylase KDM6B was expressed in excitatory neurons and declined in hippocampus with age. Conditional knockout of KDM6B in excitatory neurons reduced spine density, synaptic vesicle number and synaptic activity, and impaired learning and memory without obvious effect on brain morphology in mice. Mechanistically, KDM6B upregulated vesicular glutamate transporter 1 and 2 (VGLUT1/2) in neurons through demethylating H3K27me3 at their promoters. Tau interacted and recruited KDM6B to the promoters of Slc17a7 and Slc17a6, leading to a decrease in local H3K27me3 levels and induction of VGLUT1/2 expression in neurons, which could be prevented by loss of Tau. Ectopic expression of KDM6B, VGLUT1, or VGLUT2 restored spine density and synaptic activity in KDM6B-deficient cortical neurons. Collectively, these findings unravel a fundamental mechanism underlying epigenetic regulation of synaptic plasticity and cognition.

Background Apoptosis-inducing factor (AIF), as a mitochondrial flavoprotein, plays a fundamental role in mitochondrial bioenergetics that is critical for cell survival and also mediates caspase-independent cell death once it is released from mitochondria and translocated to the nucleus under ischemic stroke or neurodegenerative diseases. Although alternative splicing regulation of AIF has been implicated, it remains unknown which AIF splicing isoform will be induced under pathological conditions and how it impacts mitochondrial functions and neurodegeneration in adult brain. Methods AIF splicing induction in brain was determined by multiple approaches including 5′ RACE, Sanger sequencing, splicing-specific PCR assay and bottom-up proteomic analysis. The role of AIF splicing in mitochondria and neurodegeneration was determined by its biochemical properties, cell death analysis, morphological and functional alterations and animal behavior. Three animal models, including loss-of-function harlequin model, gain-of-function AIF3 knockin model and conditional inducible AIF splicing model established using either Cre-loxp recombination or CRISPR/Cas9 techniques, were applied to explore underlying mechanisms of AIF splicing-induced neurodegeneration. Results We identified a nature splicing AIF isoform lacking exons 2 and 3 named as AIF3. AIF3 was undetectable under physiological conditions but its expression was increased in mouse and human postmortem brain after stroke. AIF3 splicing in mouse brain caused enlarged ventricles and severe neurodegeneration in the forebrain regions. These AIF3 splicing mice died 2–4 months after birth. AIF3 splicing-triggered neurodegeneration involves both mitochondrial dysfunction and AIF3 nuclear translocation. We showed that AIF3 inhibited NADH oxidase activity, ATP production, oxygen consumption, and mitochondrial biogenesis. In addition, expression of AIF3 significantly increased chromatin condensation and nuclear shrinkage leading to neuronal cell death. However, loss-of-AIF alone in harlequin or gain-of-AIF3 alone in AIF3 knockin mice did not cause robust neurodegeneration as that observed in AIF3 splicing mice. Conclusions We identified AIF3 as a disease-inducible isoform and established AIF3 splicing mouse model. The molecular mechanism underlying AIF3 splicing-induced neurodegeneration involves mitochondrial dysfunction and AIF3 nuclear translocation resulting from the synergistic effect of loss-of-AIF and gain-of-AIF3. Our study provides a valuable tool to understand the role of AIF3 splicing in brain and a potential therapeutic target to prevent/delay the progress of neurodegenerative diseases.

This paper investigates interval observer‐based controller design for switched linear systems involving additional disturbance and measurement noise, whose modes need not to be cooperative. First, by giving the upper bound and the lower bound of the disturbance and the measurement noise, we construct hybrid interval observer for the considered switched linear system by means of a switched coordinate transformation, which can transform the observer error systems into cooperative ones. The interval observer can supply certain state information at any instant. Compared with the interval observer design for switched systems with arbitrary switching sequences or dwell time switching based on common Lyapunov function, the difficulty consists in characterizing the jump of the multiple Lyapunov functions. Then, by using the multiple Lyapunov functions method and average dwell time scheme, some sufficient conditions are derived and applied to build the interval observer‐based state feedback controller. Finally, we provide an example to illustrate the validity of the derived results.

Pancreaticoduodenectomy, so‐called “Whipple operation,” is a time‐consuming and technically demanding complex operation. Traditionally, this procedure has been performed most usually by open approach, which results in a large and painful wound. With the introduction of laparoscopic and robotic surgery, minimally invasive surgery (MIS) has emerged as a worldwide trend to improve wound cosmesis and to minimize wound pain. Although MIS for pancreaticoduodenectomy has also been attempted at some centers, the role of MIS, either robotic or laparoscopic approach, has not been well‐established for complex pancreaticoduodenectomy. Given that laparoscopic pancreaticoduodenectomy has been limited by its technical complexity and the high level of advanced laparoscopic skills required for pancreatic reconstruction, a robotic surgical system is introduced to overcome several limitations related to the laparoscopic approach. Providing high‐quality three‐dimensional (3‐D) vision, high optical magnification, articulation of robotic instruments, greater precision with suture targeting, and elimination of surgeon tremor, robotic surgical systems innovatively perform more delicate and complex procedures involving extensive dissection and suturing techniques such as pancreaticoduodenectomy. Although associated with longer operative time, robotic pancreaticoduodenectomy (RPD) has been claimed to have the benefits of less delayed gastric emptying, less blood loss, shorter length of postoperative stay, and lower wound infection rate, as compared with the traditional open pancreaticoduodenectomy (OPD). Moreover, RPD seems to be not only technically feasible but also justified without compromising the survival outcomes for pancreatic head and ampullary adenocarcinomas. Therefore, RPD could be recommended not only to surgeons but also to patients in terms of surgical feasibility, surgical outcomes, and patient satisfaction., Robotic approach allows surgeons to perform more complicated and delicate procedures such as pancreaticoduodenectomy involving extensive dissection and restoration of digestive tract continuity for the pancreas, bile duct, and gastrointestinal tract. Robotic pancreaticoduodenectomy has been reported as associated with less blood loss, less delayed gastric emptying, shorter length of postoperative stay, and lower wound infection rate, but longer operative time, as compared with the traditional open pancreaticoduodenectomy.

High-salt diet (HSD) is associated with dysregulated metabolism and metabolic disorders. Although previous studies have indicated its effect on metabolic tissues, the involving molecular mechanisms are not quite understood. In the present study, we provided a comprehensive transcriptome analysis on multiple metabolic tissues of HSD-fed mouse model by RNA sequencing. We observed that several genes associated with de novo lipogenesis and cholesterol biosynthesis were significantly downregulated in white adipose tissue and liver tissue of HSD mice group, such as Fasn, Scd1, Acaca, and Thrsp. Furthermore, combined with secretome datasets, our results further demonstrated that HSD could alter expression levels of organokines in metabolic tissues, for example, Tsk and Manf, in liver tissue and, thus, possibly mediate cross-talk between different metabolic tissues. Our study provided new insight about molecular signatures of HSD on multiple metabolic tissues.

Background Shock from medical and traumatic conditions can result in organ injury and death. Limited data describe out‐of‐hospital treatment of shock. We sought to characterize adult out‐of‐hospital shock care in a national emergency medical services (EMS) cohort. Methods This cross‐sectional study used 2018 data from ESO, Inc. (Austin, TX), a national EMS electronic health record system, containing data from 1289 EMS agencies in the United States. We included adult (age ≥18 years) non‐cardiac arrest patients with shock, defined as initial systolic blood pressure ≤80 mm Hg. We compared patient demographics, clinical characteristics, and response (defined as systolic blood pressure increase) between medical and traumatic shock patients, looking at systolic blood pressure trends over the first 90 minutes of care. Results Among 6,156,895 adult 911 responses, shock was present in 62,867 (1.02%; 95% confidence interval [CI] = 1.01%–1.03%); 54,239 (86.3%) medical and 5978 (9.5%) traumatic, and 2650 unknown. Medical was more common than traumatic shock in women and older patients. The most common injuries associated with traumatic shock were falls (37.6%) and motor vehicle crashes (18.7%). Mean initial and final medical systolic blood pressure were 71 ± 10 mm Hg and 99 ± 24 mm Hg. Systolic blood pressure increased in 88.8% and decreased or did not change in 11.0%. Mean initial and final trauma systolic blood pressure were 71 ± 13 mm Hg and 105 ± 28 mm Hg; systolic blood pressure increased in 90.4% and decreased/did not change in 9.6%. On fractional polynomial modeling, systolic blood pressure changes were greater and faster for trauma than medical shock. Conclusions In this national series, 1 of every 100 EMS encounters involved shock. These findings highlight the current course and care of shock in the out‐of‐hospital setting.

BackgroundThere is a lack of well validated biomarkers in osteosarcoma, a rare, recalcitrant disease with variable outcome and poorly understood biologic behavior, for which treatment standards have stalled for decades. The only standard prognostic factor in osteosarcoma remains the amount of pathologic necrosis following pre-operative chemotherapy, which does not adequately capture the biologic complexity of the tumor and has not resulted in optimized patient therapeutic stratification. New, robust biomarkers are needed to understand prognosis and better reflect the underlying biologic and molecular complexity of this disease.MethodsWe performed microRNA sequencing in 74 frozen osteosarcoma biopsy samples, the largest single center translationally analyzed cohort to date, and separately analyzed a multi-omic dataset from a large (n = 95) NCI supported national cooperative group cohort. Molecular patterns were tested for association with outcome and used to identify novel therapeutics for further study by integrative pharmacogenomic analysis.ResultsMicroRNA profiles were found predict Recurrence Free Survival (5-microRNA profile, Median RFS 59 vs 202 months, log rank p=0.06, HR 1.87, 95% CI 0.96-3.66). The profiles were independently prognostic of RFS when controlled for metastatic disease at diagnosis and pathologic necrosis following chemotherapy in multivariate Cox proportional hazards regression (5-microRNA profile, HR 3.31, 95% CI 1.31–8.36, p=0.01). Strong trends for survival discrimination were observed in the independent NCI dataset, and transcriptomic analysis revealed the downstream microRNA regulatory targets are also predictive of survival (median RFS 17 vs 105 months, log rank p=0.007). Additionally, DNA methylation patterns held prognostic significance. Through machine learning based integrative pharmacogenomic analysis, the microRNA biomarkers identify novel therapeutics for further study and stratified application in osteosarcoma.ConclusionsOur results support the existence of molecularly defined phenotypes in osteosarcoma associated with distinct outcome independent of clinicopathologic features. We validated candidate microRNA profiles and their associated molecular networks for prognostic value in multiple independent datasets. These networks may define previously unrecognized osteosarcoma subtypes with distinct molecular context and clinical course potentially appropriate for future application of tailored treatment strategies in different patient subgroups.

Background Surgical views with high resolution and magnification have enabled us to recognize the precise anatomical structures that can be used as landmarks during minimally invasive distal pancreatectomy (MIDP). This study aimed to validate the usefulness of anatomy-based approaches for MIDP before and during the Expert Consensus Meeting: Precision Anatomy for Minimally Invasive HBP Surgery (February 24, 2021). Methods Twenty-five international MIDP experts developed clinical questions regarding surgical anatomy and approaches for MIDP. Studies identified via a comprehensive literature search were classified using Scottish Intercollegiate Guidelines Network (SIGN) methodology. Online Delphi voting was conducted after experts had drafted the recommendations, with the goal of obtaining >75% consensus. Experts discussed the revised recommendations in front of the validation committee and an international audience of 384 attendees. Finalized recommendations were made after a second round of online Delphi voting. Results Four clinical questions were addressed, resulting in 10 recommendations. All recommendations reached at least a 75% consensus among experts. Conclusions The expert consensus on precision anatomy for MIDP has been presented as a set of recommendations based on available evidence and expert opinions. These recommendations should guide experts and trainees in performing safe MIDP and foster its appropriate dissemination worldwide.

W7-X completed its plasma operation in hydrogen with island divertor and inertially cooled test divertor unit (TDU) made of graphite. A substantial set of plasma-facing components (PFCs), including in particular marker target elements, were extracted from the W7-X vessel and analysed post-mortem. The analysis provided key information about underlying plasma–surface interactions (PSI) processes, namely erosion, transport, and deposition as well as fuel retention in the graphite components. The net carbon (C) erosion and deposition distribution on the horizontal target (HT) and vertical target (VT) plates were quantified and related to the plasma time in standard divertor configuration with edge transform ι = 5/5, the dominant magnetic configuration of the two operational phases (OP) with TDU. The operation resulted in integrated high net C erosion rate of 2.8 mg s−1 in OP1.2B over 4809 plasma seconds. Boronisations reduced the net erosion on the HT by about a factor 5.4 with respect to OP1.2A owing to the suppression of oxygen (O). In the case of the VT, high peak net C erosion of 11 μm at the strike line was measured during OP1.2B which converts to 2.5 nm s−1 or 1.4 mg s−1 when related to the exposed area of the target plate and the operational time in standard divertor configuration. PSI modelling with ERO2.0 and WallDYN-3D is applied in an interpretative manner and reproduces the net C erosion and deposition pattern at the target plates determined by different post-mortem analysis techniques. This includes also the 13C tracer deposition from the last experiment of OP1.2B with local 13CH4 injection through a magnetic island in one half module. The experimental findings are used to predict the C erosion, transport, and deposition in the next campaigns aiming in long-pulse operation up to 1800 s and utilising the actively cooled carbon-fibre composite (CFC) divertor currently being installed. The CFC divertor has the same geometrical design as the TDU and extrapolation depends mainly on the applied plasma boundary. Extrapolation from campaign averaged information obtained in OP1.2B reveals a net erosion of 7.6 g per 1800 s for a typical W7-X attached divertor plasma in hydrogen.

Chronic obstructive pulmonary disease (COPD) is a common chronic pulmonary disease with multiple etiologies and pathological changes. PYK2 expression is significantly increased in lipopolysaccharide-induced lung injury, but it mediates chronic lung inflammation. The mechanism of its occurrence remains unclear. Quanzhenyiqitang is often used in clinical treatment of COPD, so this study explored the mechanism of its treatment of lipopolysaccharide-induced lung injury. In this study, transfection, flow cytometry, QRT-PCR, and Western blotting methods were used to study the mechanism of Quanzhenyiqitang lipopolysaccharide-induced lung injury. The results showed that the mechanism of occurrence remains unclear. Our novel observations imply that the PYK2/p38MAPK/HDAC2/CK2 pathway is one of the fundamental underlying mechanisms that mediate the pathogenic progression of COPD, and Quanzhenyiqitang may be the therapeutic drug to prevent chronic inflammation and delay the progression of COPD by inhibiting PYK2 signaling pathways.

We present recent highlights from the most recent operation phases of Wendelstein 7-X, the most advanced stellarator in the world. Stable detachment with good particle exhaust, low impurity content, and energy confinement times exceeding 100 ms, have been maintained for tens of seconds. Pellet fueling allows for plasma phases with reduced ion-temperature-gradient turbulence, and during such phases, the overall confinement is so good (energy confinement times often exceeding 200 ms) that the attained density and temperature profiles would not have been possible in less optimized devices, since they would have had neoclassical transport losses exceeding the heating applied in W7-X. This provides proof that the reduction of neoclassical transport through magnetic field optimization is successful. W7-X plasmas generally show good impurity screening and high plasma purity, but there is evidence of longer impurity confinement times during turbulence-suppressed phases. With the ceaseless expanding of big data and the Internet, it has more and more affected people’s life. The collection and construction of information has become the front-end technology of current industrial development. News reports and self-media channels are also more extensive. As one of the sections of news reports, sports has also encountered opportunities and challenges in its dissemination. As an innovative technology, blockchain has brought new impetus to the development of sports industry. It is also driving new technological upgrades and visual experiences. Blockchain is a chain data structure that combines data blocks in a sequential manner according to time sequence and is a cryptographically guaranteed untameably and unforgeable distributed ledger. Sports communication has more development opportunities in modern life. Therefore, the purpose of this paper is to explore a new path of physical integration and dissemination based on the theory of blockchain, analyze the development and integration path in the application of blockchain technology, and explore their respective advantages and disadvantages. This article will use the research method of specific analysis of specific problems to make data comparison and draw a conclusion, to carry out theoretical innovation and exploration, so as to find a suitable model to promote the development of sports industry. The so-called “sports big data” refers to the analysis and application of data in sports scenarios. Combined with competitive sports, the significance of data analysis lies in improving the level of competition and preventing sports injuries. The research results show that sports companies using blockchain technology have increased their communication efficiency by 37%, and audience popularity has increased by 15% year on year. The decentralized and anonymous features of blockchain technology ensure the originality and enthusiasm of sports data news teams. Therefore, through the analysis of the selection of application scenes and technologies of the sports industry by the blockchain technology, combined with the characteristics of the current era, the transformation is fully absorbed and combined with a new perspective. This paper analyzes different application dilemmas and explores the development prospects, thus putting forward the implementation path of physical industry blockchain technology application.

Background Histidine-tryptophan-ketoglutarate (HTK) and del Nido (DN) cardioplegia are intracellular-type and extracellular-type solution respectively, both can provide a long period of myocardial protection with single-dose infusion, but studies comparing the two are rare for adult cardiac surgery. This study aims to evaluate whether DN is suitable for cardioplegia in complex and high-risk valve surgery with long-term cardiac ischemia when compared with HTK. Methods The perioperative records of adult patients infused with DN/HTK as a cardioplegic solution who underwent complex valve surgery with an expected myocardial ischaemic duration longer than 90 min between Oct 2018 and Oct 2019 were analysed retrospectively. Results Of the 160 patients who received DN/HTK and underwent complex valve surgery, we propensity matched 73 pairs. Both groups achieved satisfactory cardiac arrest effects, and no significant difference was found in their cTnI and CK-MB levels within 12 to 72 h postoperatively. The DN group had a higher rate of return to spontaneous rhythm (0.88 v 0.52, P v 26%, P = 0.036), a higher postoperative stroke volume (65 v 59 ml, P = 0.011) and a higher cardiac output (6.0 v 4.9 L/min, P = 0.007) as evaluated by echocardiography, fewer transfusions and shorter ICU stays (both P Conclusions DN can be safely applied to complex valve surgery, and it has a similar myocardial protection effect as HTK. Further prospective studies are required to verify these retrospective findings. Trial registration retrospectively registered.

When PAH patients suffer from pulmonary infection or puerperal infection heart failure often rapidly develops. Low dose of lipopolysaccharide induces rapid right ventricular failure in rats with pulmonary arterial hypertension.Purpose: The objective of this study was to investigate whether the NLRP3 inflammasome mediates disturbance of the ventricular immune microenvironment of PAH rats and promotes right ventricular failure.Methods: Intraperitoneal injection of monocrotaline was used to induce PAH in rats. Right ventricular function was measured via echocardiography before and after the rats were treated with lipopolysaccharide and MCC950. The degree of immune microenvironment disturbance in right ventricular tissue was measured with a rat chemokine and cytokine antibody array, Western blot, flow cytometry and quantitative real-time PCR analysis.Results: After the rats were injected with LPS, they exhibited right ventricular dysfunction and a significant increase in right ventricular tissue inflammation with elevated M1 macrophage proportion. Administration of MCC950 suppressed inflammation and improved right ventricular function. The number of M1 macrophages was decreased after MCC950 treatment. NLRP3 inflammasome inhibition ameliorated LPS-induced changes in the immune microenvironment in the right heart and right ventricular dysfunction in rats with PAH.Conclusion: Selective inhibition of NLRP3 pathway interfered the interaction between hypertrophic cardiomyocytes and macrophages in the initial stage of inflammation and maintained the immune microenvironment balance, eventually contributing to attenuation of LPS-induced acute heart failure in PAH rats.Keywords: pulmonary arterial hypertension, right ventricular failure, NLRP3 inflammasome, inflammation, immune microenvironment

With almost 4 million deaths worldwide from the COVID-19 pandemic, the efficient and accurate diagnosis and identification of COVID-19-related complications are more important than ever. Scales such as the pneumonia severity index, or CURB-65, help doctors determine who should be admitted to the hospital or the intensive care unit. To properly treat and manage admitted patients, standardized sampling protocols and methods are required for COVID-19 patients. Using PubMed, relevant articles since March 2020 on COVID-19 diagnosis and its complications were analyzed. Patients with COVID-19 had elevated D-dimer, thrombomodulin, and initial factor V elevation followed by decreased factor V and factor VII and elevated IL-6, lactate dehydrogenase, and c-reactive protein, which indicated coagulopathy and possible cytokine storm. Patients with hypertension, newly diagnosed diabetes, obesity, or advanced age were at increased risk for mortality. Elevated BUN, AST, and ALT in severe COVID-19 patients was associated with acute kidney injury or other organ damage. The gold standard for screening COVID-19 is reverse transcriptase polymerase chain reaction (RT-PCR) using sputum, oropharyngeal, or nasopharyngeal routes. However, due to the low turnover rate and limited testing capacity of RT-PCR, alternative diagnostic tools such as CT-scan and serological testing (IgM and IgG) can be considered in conjunction with symptom monitoring. Advancements in CRISPR technology have also allowed the use of alternative COVID-19 testing, but unfortunately, these technologies are still under FDA review and cannot be used in patients. Nonetheless, increased turnover rates and testing capacity allow for a bright future in COVID-19 diagnosis.

Theoretically, pancreas transplant alone in uremic (PTAU) patients could also be one of the options for those waiting for both pancreas and kidney grafts, but it has never been reported. There were 160 cases of pancreas transplant in this study, including 16% PTAU. The 5-year patient survival was 66.2% after PTAU, 94.5% after SPK, 95.8% after PAK, and 95.4% after PTA. Rejection of pancreas graft was significantly lower in PTAU group (3.8%), followed by 16.7% in pancreas after kidney transplant (PAK), 29.8% in simultaneous pancreas and kidney transplant (SPK) and 37.0% in pancreas transplant alone (PTA). Fasting blood sugar and serum HbA1c levels after PTAU were not significantly different from those by other subgroups. The 5-year death-censored pancreas graft survival was 100% after PTAU and PAK, and 97.0% after SPK and 77.9% after PTA. However, the 5-year death-uncensored pancreas graft survival was 67.0% after PTAU, 100% after PAK, 91.3% after SPK, and 74.0% after PTA. The superior graft survival in the PTAU group was achieved only if deaths with a functioning graft were censored. In conclusion, given the inferior patient survival outcome, PTAU is still not recommended unless SPK and PAK is not available. Although PTAU could be a treatment option for patients with diabetes complicated by end-stage renal disease (ESRD) in terms of surgical risks, endocrine function, and immunological and graft survival outcomes, modification of the organ allocation policies to prioritize SPK transplant in eligible patients should be the prime goal.

To achieve the tamper-proof, reliability and traceability of transactions in a trustless environment, the blockchain requires each peer node to store the whole global ledger. However, as transactions keep increasing over time, the storage cost of each node increases. In addition, many schemes have been proposed to boost rapid transactions which will even lead transactions to grow explosively. The problem of storage is becoming one challenge of blockchain since the storage overhead of each node increase rapidly. Reducing the storage overhead of each participant is very urgent and worthy. In this paper, we present GCBlock: a grouping overlay network storage scheme for the blockchain which can reduce the storage overhead of nodes and cut the whole storage cost of the blockchain greatly while keeping the underlying protocols. In our scheme, we try to group the nodes according to their physical fuzzy distance to reduce the overall delay when tracing. We set rules of autonomous check to deal with evil behavior within the group. To further enhance the stability of our scheme, we propose the transcript fractional repetition code which is newly constructed based on the fractional repetition code to encode data. Finally, we make a comprehensive evaluation of GCBlock and the results show that it is workable and reasonable.

Engineering the band gap chemically by organic molecules is a powerful tool with which to optimize the properties of inorganic 2D materials. The obtained materials are however still limited by inhomogeneous compositions and properties at nanoscale and small adjustable band gap ranges. To overcome these problems in the traditional exfoliation and then organic modification strategy, an organic modification and then exfoliation strategy was explored in this work for preparing 2D organic metal chalcogenides (OMCs). Unlike the reported organically modified 2D materials, the inorganic layers of OMCs are fully covered by long-range ordered organic functional groups. By changing the electron-donating ability of the organic functional groups and the electronegativity of the metals, the band gaps of OMCs were varied by 0.83 eV and their conductivities were modulated by 9 orders of magnitude, which are 2 and 107 times higher than the highest values observed in the reported chemical methods, respectively., Here, the authors report a new class of 2D materials with inorganic metal chalcogenide layers covered by long-range ordered organic groups. The range of modulation on their band gaps and electrical conductivities are two times and 7 orders of magnitude better than the highest values reported so far.

In this paper, a numerical investigation was conducted on a subsonic compressor rotor with blade angle slot casing treatments. The purpose of the investigation is to reveal the influence of axial overlap of blade angle slot on the compressor stability. Six kinds of blade angle slot casing treatments with different axial overlap rates were investigated in this paper. The results show that with the increasing of axial overlap rates, the stall margin improvement firstly increased and then decreased. And the optimal blade angle slot can obtain 62.51% improvement of stall margin. The flow field analyses show that the bleeding flows formed inside slots can restrain the adverse tip leakage flow, which is the critical factor making the onset of the rotor stall. With the increasing of axial overlap rate of the slots, the relative position between bleeding flows in the slots and tip leakage flow plays an important role in the stall margin improvement.

SPARC is a cysteine-rich acidic secreted protein. It is a non-collagen component of bone, which is widely distributed in humans and animals and plays an important role. SPARC has been found in a variety of human cancers (breast cancer, stomach cancer, ovarian cancer, etc.) and diabetes-related research. Especially the muscle and fat metabolism are closely related. In this study, we used a DNA pool to detect a new SNP site (g.12454T > C). A total of 616 samples of four breeds of Qinchuan cattle (QC, n=176), Xianan cattle (XN, n=160), Pinan cattle (PN, n=136) and Jiaxian cattle (JX, n=144) were analyzed and identified with ARMS-PCR. In addition, we correlated SNP with growth traits and showed significant correlation with growth traits such as rump length, hip width, and body length (p). Moreover, we tested the SPARC gene expression level in different tissues belonging to XN adult cattle (n=3) and found its high expression in muscle tissues (relative to the kidney). Further, we found the SNP is able to increase the SPARC expression level in skeletal muscle (n=12). According to statistical data, this SNP site may be applied to a molecular marker of an early marker-assisted selection for early growth of beef cattle.
