Background: Current management practices and outcomes in weaning from invasive mechanical ventilation are poorly understood. We aimed to describe the epidemiology, management, timings, risk for failure, and outcomes of weaning in patients requiring at least 2 days of invasive mechanical ventilation. Methods: WEAN SAFE was an international, multicentre, prospective, observational cohort study done in 481 intensive care units in 50 countries. Eligible participants were older than 16 years, admitted to a participating intensive care unit, and receiving mechanical ventilation for 2 calendar days or longer. We defined weaning initiation as the first attempt to separate a patient from the ventilator, successful weaning as no reintubation or death within 7 days of extubation, and weaning eligibility criteria based on positive end-expiratory pressure, fractional concentration of oxygen in inspired air, and vasopressors. The primary outcome was the proportion of patients successfully weaned at 90 days. Key secondary outcomes included weaning duration, timing of weaning events, factors associated with weaning delay and weaning failure, and hospital outcomes. This study is registered with ClinicalTrials.gov, NCT03255109. Findings: Between Oct 4, 2017, and June 25, 2018, 10 232 patients were screened for eligibility, of whom 5869 were enrolled. 4523 (77·1%) patients underwent at least one separation attempt and 3817 (65·0%) patients were successfully weaned from ventilation at day 90. 237 (4·0%) patients were transferred before any separation attempt, 153 (2·6%) were transferred after at least one separation attempt and not successfully weaned, and 1662 (28·3%) died while invasively ventilated. The median time from fulfilling weaning eligibility criteria to first separation attempt was 1 day (IQR 0–4), and 1013 (22·4%) patients had a delay in initiating first separation of 5 or more days. Of the 4523 (77·1%) patients with separation attempts, 2927 (64·7%) had a short wean (≤1 day), 457 (10·1%) had intermediate weaning (2–6 days), 433 (9·6%) required prolonged weaning (≥7 days), and 706 (15·6%) had weaning failure. Higher sedation scores were independently associated with delayed initiation of weaning. Delayed initiation of weaning and higher sedation scores were independently associated with weaning failure. 1742 (31·8%) of 5479 patients died in the intensive care unit and 2095 (38·3%) of 5465 patients died in hospital. Interpretation: In critically ill patients receiving at least 2 days of invasive mechanical ventilation, only 65% were weaned at 90 days. A better understanding of factors that delay the weaning process, such as delays in weaning initiation or excessive sedation levels, might improve weaning success rates. Soybean biodiesel (B100) has been playing an important role in Brazilian energy matrix towards the national bio-based economy. Greenhouse gas (GHG) emissions is the most widely used indicator for assessing the environmental sustainability of biodiesels and received particular attention among decision makers in business and politics, as well as consumers. Former studies have been mainly focused on the GHG emissions from the soybean cultivation, excluding other stages of the biodiesel production. Here, we present a holistic view of the total GHG emissions in four life cycle stages for soybean biodiesel. The aim of this study was to assess the GHG emissions of Brazilian soybean biodiesel production system with an integrated life cycle approach of four stages: agriculture, extraction, production and distribution. Allocation of mass and energy was applied and special attention was paid to the integrated and non-integrated industrial production chain. The results indicated that the largest source of GHG emissions, among four life cycle stages, is the agricultural stage (42-51%) for B100 produced in integrated systems and the production stage (46-52%) for B100 produced in non-integrated systems. Integration of industrial units resulted in significant reduction in life cycle GHG emissions. Without the consideration of LUC and assuming biogenic CO2 emissions is carbon neutral in our study, the calculated life cycle GHG emissions for domestic soybean biodiesel varied from 23.1 to 25.8 gCO2eq. MJ-1 B100 and those for soybean biodiesel exported to EU ranged from 26.5 to 29.2 gCO2eq. MJ-1 B100, which represent reductions by 65% up to 72% (depending on the delivery route) of GHG emissions compared with the EU benchmark for diesel fuel. Our findings from a life cycle perspective contributed to identify the major GHG sources in Brazilian soybean biodiesel production system and they can be used to guide mitigation priority for policy and decision-making. Projected scenarios in this study would be taken as references for accounting the environmental sustainability of soybean biodiesel within a domestic and global level.

The objective of this work was to evaluate the correlation between sugarcane yield and some physical and chemical attributes of soil. For this, a 42‑ha test area in Araras, SP, Brazil, was used. Soil properties were determined from samples collected at the beginning of the 2003/2004 harvest season, using a regular 100x100 m grid. Yield assessment was done with a yield monitor (Simprocana). Correlation analyses were performed between sugarcane yield and the following soil properties: pH, pH CaCl2, N, C, cone index, clay content, soil organic matter, P, K, Ca, Mg, H+AL, cation exchange capacity, and base saturation. Correlation coefficients were respectively ‑0.05, ‑0.29, 0.33, 0.41, ‑0.27, 0.22, 0.44, ‑0.24, trace, ‑0.06, 0.01, 0.32, 0.14, and 0.04. Correlations of chemical and physical attributes of soil with sugarcane yield are weak, and, per se, they are not able to explain sugarcane yield variation, which suggests that other variables, besides soil attributes, should be analysed.

Um dos desafios da agricultura de precisão é oferecer subsídios para a definição de unidades de manejo para posteriores intervenções. Portanto, o objetivo deste trabalho foi avaliar os atributos químicos do solo e a produtividade da cultura de cana-de-açúcar por meio da geoestatística e mineração de dados pela indução da árvore de decisão. A produtividade da cana-de-açúcar foi mapeada em uma área de aproximadamente 23ha, utilizando-se o critério de célula, por meio de um monitor de produtividade que permitiu a elaboração de um mapa digital que representa a superfície de produção para a área em estudo. Para determinar os atributos de um Argissolo Vermelho-Amarelo, foram coletadas as amostras no início da safra 2006/2007, utilizando-se uma grade regular de 50 x 50m, nas profundidades de 0,0-0,2m e 0,2-0,4m. Os dados dos atributos do solo e da produtividade foram analisados por meio da técnica de goestatística e classificados em três níveis de produção para indução de árvore de decisão. A árvore de decisão foi induzida no programa SAS Enterprise Miner, sendo utilizado algoritmo baseado na redução de entropia. As variáveis altitude e potássio apresentaram os maiores valores de correlação com a produtividade de cana-de-açúcar. A indução de árvores de decisão permitiu verificar que a altitude é a variável com maior potencial para interpretar os mapas de produtividade de cana-de-açúcar, auxiliando na agricultura de precisão e mostrando-se uma ferramenta adequada para o estudo de definição de zonas de manejo em área cultivada com essa cultura.One of the challenges of precision agriculture is to offer subsidies for the definition of management units for posterior interventions. Therefore, the objective of this work was to evaluate soil chemical attributes and sugarcane yield with the use of geostatistics and data mining by decision tree induction. Sugarcane yield was mapped in a 23ha field, applying the cell criterion, by using a yield monitor that allowed the elaboration of a digital map representing the surface of production of the studied area. To determine the soil attributes, soil samples were collected at the beginning of the harvest in 2006/2007 using a regular grid of 50 x 50m, in the depths of 0.0-0.2m and 0.2-0.4m. Soil attributes and sugarcane yield data were analyzed by using geostatistics techniques and were classified into three yield levels for the elaboration of the decision tree. The decision tree was induced in the software SAS Enterprise Miner, using an algorithm based on entropy reduction. Altitude and potassium presented the highest values of correlation with sugarcane yield. The induction of decision trees showed that the altitude is the variable with the greatest potential to interpret the sugarcane yield maps, then assisting in precision agriculture and, revealing an adjusted tool for the study of management definition zones in area cropped with sugarcane.

Severe intra-abdominal infection commonly requires intensive care. Mortality is high and is mainly determined by disease-specific characteristics, i.e. setting of infection onset, anatomical barrier disruption, and severity of disease expression. Recent observations revealed that antimicrobial resistance appears equally common in community-acquired and late-onset hospital-acquired infection. This challenges basic principles in anti-infective therapy guidelines, including the paradigm that pathogens involved in community-acquired infection are covered by standard empiric antimicrobial regimens, and second, the concept of nosocomial acquisition as the main driver for resistance involvement. In this study, we report on resistance profiles of Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa, Enterococcus faecalis and Enterococcus faecium in distinct European geographic regions based on an observational cohort study on intra-abdominal infections in intensive care unit (ICU) patients. Resistance against aminopenicillins, fluoroquinolones, and third-generation cephalosporins in E. coli, K. pneumoniae and P. aeruginosa is problematic, as is carbapenem-resistance in the latter pathogen. For E. coli and K. pneumoniae, resistance is mainly an issue in Central Europe, Eastern and South-East Europe, and Southern Europe, while resistance in P. aeruginosa is additionally problematic in Western Europe. Vancomycin-resistance in E. faecalis is of lesser concern but requires vigilance in E. faecium in Central and Eastern and South-East Europe. In the subcohort of patients with secondary peritonitis presenting with either sepsis or septic shock, the appropriateness of empiric antimicrobial therapy was not associated with mortality. In contrast, failure of source control was strongly associated with mortality. The relevance of these new insights for future recommendations regarding empiric antimicrobial therapy in intra-abdominal infections is discussed.

Soybean biodiesel (B100) has been playing an important role in Brazilian energy matrix towards the national bio-based economy. Greenhouse gas (GHG) emissions is the most widely used indicator for assessing the environmental sustainability of biodiesels and received particular attention among decision makers in business and politics, as well as consumers. Former studies have been mainly focused on the GHG emissions from the soybean cultivation, excluding other stages of the biodiesel production. Here, we present a holistic view of the total GHG emissions in four life cycle stages for soybean biodiesel. The aim of this study was to assess the GHG emissions of Brazilian soybean biodiesel production system with an integrated life cycle approach of four stages: agriculture, extraction, production and distribution. Allocation of mass and energy was applied and special attention was paid to the integrated and non-integrated industrial production chain. The results indicated that the largest source of GHG emissions, among four life cycle stages, is the agricultural stage (42-51%) for B100 produced in integrated systems and the production stage (46-52%) for B100 produced in non-integrated systems. Integration of industrial units resulted in significant reduction in life cycle GHG emissions. Without the consideration of LUC and assuming biogenic CO2 emissions is carbon neutral in our study, the calculated life cycle GHG emissions for domestic soybean biodiesel varied from 23.1 to 25.8 gCO2eq. MJ-1 B100 and those for soybean biodiesel exported to EU ranged from 26.5 to 29.2 gCO2eq. MJ-1 B100, which represent reductions by 65% up to 72% (depending on the delivery route) of GHG emissions compared with the EU benchmark for diesel fuel. Our findings from a life cycle perspective contributed to identify the major GHG sources in Brazilian soybean biodiesel production system and they can be used to guide mitigation priority for policy and decision-making. Projected scenarios in this study would be taken as references for accounting the environmental sustainability of soybean biodiesel within a domestic and global level.

This work aimed to analyze the pattern of variation in wave lengths in presence of wood holes and to develop a model capable of describing the process. To attain that end, wood pieces were used from species pequiá (Aspidosperma desmanthum), on which circular and linear artificial holes were made and gradually enlarged. Ultrasonic tests were performed using USLab equipment and 45 kHz transducers. Measurements were taken first on the intact piece and then after each stage of artificial hole enlargement. Results demonstrated that propagation velocities of ultrasonic waves are affected by presence of holes and also that reduction in velocity is caused by changes in wave path, since waves tend to deviate from empty space and travel through matter. The circular hole type had a slightly stronger influence on velocity reduction than the linear hole type. Variation in velocity as a function of increasing percentage of hollow space relative to the intact piece can be represented by a linear model. Neste trabalho, objetivou-se estudar o comportamento da variação do percurso da onda em presença de oco na madeira bem como elaborar um modelo que o descrevesse. Para atingir esse objetivo, foram utilizadas peças de madeira da espécie pequiá (Aspidosperma desmanthum) nos quais foram confeccionados orifícios artificiais (circular e linear). Esses orifícios foram sendo aumentados gradualmente. Os ensaios de ultrassom foram realizados utilizando equipamento da marca USLab e transdutores de 45 kHz. As medições foram realizadas na peça íntegra e após cada fase de confecção do oco artificial. Os resultados permitiram confirmar que as velocidades de propagação de ondas de ultrassom são afetadas pela presença de ocos e que a redução da velocidade é provocada pela mudança de percurso da onda, que tende a se desviar do vazio e percorrer o material. O oco do tipo circular teve influência levemente superior na redução da velocidade do que o oco do tipo linear. A variação da velocidade com o aumento da porcentagem de oco em relação à peça integra pode ser representada por um modelo linear.

The objective of this work was to evaluate the correlation between sugarcane yield and some physical and chemical attributes of soil. For this, a 42‑ha test area in Araras, SP, Brazil, was used. Soil properties were determined from samples collected at the beginning of the 2003/2004 harvest season, using a regular 100x100 m grid. Yield assessment was done with a yield monitor (Simprocana). Correlation analyses were performed between sugarcane yield and the following soil properties: pH, pH CaCl2, N, C, cone index, clay content, soil organic matter, P, K, Ca, Mg, H+AL, cation exchange capacity, and base saturation. Correlation coefficients were respectively ‑0.05, ‑0.29, 0.33, 0.41, ‑0.27, 0.22, 0.44, ‑0.24, trace, ‑0.06, 0.01, 0.32, 0.14, and 0.04. Correlations of chemical and physical attributes of soil with sugarcane yield are weak, and, per se, they are not able to explain sugarcane yield variation, which suggests that other variables, besides soil attributes, should be analysed. O objetivo deste trabalho foi avaliar as correlações entre a produtividade da cana‑de‑açúcar e alguns atributos físicos e químicos do solo. Para isso, uma área de 42 ha, em Araras, SP, foi selecionada. As propriedades do solo foram determinadas a partir de amostras coletadas no início da safra 2003/2004, por meio de uma grade regular de 100x100 m. A produtividade foi avaliada por meio de um monitor de produtividade (Simprocana). Análises de correlação foram realizadas entre a produtividade da cana‑de‑açúcar e as seguintes propriedades do solo: pH, pH CaCl2, N, C, índice de cone, teor de argila, matéria orgânica, P, K, Ca, Mg, H + Al, capacidade de troca catiônica e saturação por bases. Os coeficientes de correlação foram respectivamente de ‑0,05, ‑0,29, 0,33, 0,41, ‑0,27, 0,22, 0,44, ‑0,24, traço, ‑0,06, 0,01, 0,32, 0,14 e 0,04. As correlações de atributos químicos e físicos do solo com a produtividade da cana‑de‑açúcar são baixas, por si mesmas, e não são capazes de explicar a variação na produtividade da cana‑de‑açúcar, o que indica que, além das propriedades do solo, outras variáveis devem ser analisadas.

The aim of this work was to investigate the correlation between spatial variability of the soil attributes and sugarcane yield as a result of soil topography. To achieve this objective, a test area of 42 ha located at the Sao Joao Sugar Mill, in Araras, in the State of São Paulo, Brazil, was selected. Sugarcane yield was measured with a yield monitor fitted in a sugarcane harvester and GPS signal. A total of 170 soil samples were taken at regular 50 m grid, at a depth of 0 - 0.2 m. The area under study was divided into two sites based on topography. The following soil attributes were analysed: organic matter (OM) content, exchangeable potassium (K), calcium (Ca) and magnesium (Mg), their base saturation percentage (%BS), cation exchange capacity (CEC), pH, clay, silt, total sand and density. The use of landscape and geostatistics enable defining areas with different spatial variability in soil attributes and crop yield, providing the visualization and definition of homogeneous management zones. The largest spatial variability of soil attributes and sugarcane yield was in the lowest part of the field. 17. Correlation Between Chemical Soil Attributes and Sugarcane Quality Parameters According to Soil Texture Zones.

Significant changes in the Brazilian sugarcane agricultural sector have been observed in recent years, with the rapid expansion of planted areas, mainly in the western part of the country. In addition to the enlargement of boundaries and the installation of new industrial units, sugarcane expansion demands the development of new technologies to facilitate better use of the production areas, increase the yield and quality of the sugarcane, and reduce production costs. For that, the adoption of precision agriculture may be an important tool in the management of sugarcane production, which is based on the understanding of the relationship between soil attributes and sugarcane quality. Therein, the objective of this work was to study the Pearson correlation between chemical soil attributes and sugarcane quality parameters based on soil physical zones and to verify the spatial-temporal variability of the quality attributes over time. The work was carried out in an area of 10 ha located in Araras, Sao Paulo, Brazil, from November 2008 to July 2011. Sugarcane quality (Brix, pol, and fiber) was measured in samples collected within a grid of 30 × 30 m (total of 117 sample points) just before harvesting. The soil samples were collected immediately after harvesting in the same grid points. The chemical soil attributes analyzed were macronutrients and micronutrients, and the soil physical attribute was soil texture. The correlations obtained between the sugarcane quality parameters and soil chemical attributes varied from weak (Zn
0-0.2 : -0.36; Ca0.2-0.5 : -0.30 with fiber and soil organic matter0-0.2 : -0.36 with pol) in sandy loam zone at the first year to strong correlations (P0-0.2 : 0-77; Ca0-0.2 : 0.67 with fiber) in sand clay loam zones at the second year. They also revealed major negative soil property correlations (P, Ca, Mg, sum of base, cation exchange capacity, base saturation) with sugar concentration and positive correlations with fiber. Distraction osteogenesis is a surgical technique widely used in orthopedic surgery for treatment of various pathological skeletal conditions, namely correction of limb-length discrepancies, angular deformity and treatment of distal and severely comminuted fractures, or bone defects through bone transport. The basic principle consists on the gradual distraction of two bone segments, previously submitted to a corticotomy and promptly fixated generally using of circular external skeletal fixation. New bone tissue is generated in the bone gap between the two segments. This review aims to describe the biological fundaments and principles of this technique, the surgical steps performed to attempt distraction osteogenesis, and its possible complications with main focus on its application in companion animals.
